Resumo: O objetivo foi descrever a gravidade das complicações relacionadas ao aborto, os fatores relacionados às complicações, os tipos de tratamento e a experiência de atendimento no Brasil. Foi realizado um estudo transversal em vinte hospitais (dez no Distrito Federal, três em Rondônia e sete no Maranhão). Durante três meses, todos os dados de todas as mulheres tratadas por aborto/aborto espontâneo foram coletados. A gravidade das complicações foi definida de acordo com os critérios da Organização Mundial da Saúde. As mulheres com hemorragia, infecção ou lesão de órgãos foram convidadas a responder a uma entrevista sobre a experiência do atendimento. A análise estatística foi realizada usando o teste de qui-quadrado e modelos de regressão de Poisson. Entre as 1.683 mulheres incluídas, 82,5% tiveram complicações leves, 13,6% tiveram complicações moderadas, 3,2% tiveram condições potencialmente ameaçadoras à vida (PLTC, acrônimo em inglês) e 0,7% tiveram resultados maternos graves (SMO, acrônimo em inglês). A maioria das mulheres (94,2%) precisou de esvaziamento uterino. Entre elas, 91,5% precisaram de esvaziamento cirúrgico (com ou sem uso de uterotônicos) e 8,5% usaram apenas uterotônicos. O método de esvaziamento cirúrgico mais frequente foi a curetagem (66,9%), seguido pela aspiração manual a vácuo (32,3%). Os fatores associados à PLTC/SMO vs. complicações leves foram ter idade gestacional ≥ 13 semanas (razão de prevalência - RP = 3,09; intevalo de 95% de confiança - IC95%: 1,42-6,72), ter sido tratado no Maranhão (RP = 0,27; IC95%: 0,12-0,63) e em Rondônia (RP = 0,64; IC95%: 0,20-0,99). Os fatores associados às complicações moderadas vs. leves foram expulsão dos produtos da concepção antes da chegada ao serviço de saúde (RP = 2,55; IC95%: 1,64-3,96) e ter sido tratado no Maranhão (RP = 0,58; IC95%: 0,38-0,87). A maioria das mulheres que responderam à entrevista foi tratada com gentileza (95,6%), no entanto, 66,7% se sentiram estressadas e 10,1% relataram que suas preferências não foram respeitadas durante a internação. Nove em cada dez mulheres atendidas em hospitais públicos brasileiros devido a complicações relacionadas ao aborto são submetidas a algum procedimento cirúrgico, sendo o mais comum a curetagem uterina. Aproximadamente quatro em cada 100 mulheres apresentam complicações graves. É fundamental garantir o fornecimento de equipamentos para aspiração manual a vácuo e incentivar programas de educação médica continuada para aumentar a conscientização dos profissionais de saúde sobre tratamentos mais seguros para evacuação uterina.

Abstract: This study aimed to describe the severity of abortion-related complications, factors associated with complications, the types of management and the experience of care in Brazil. A cross-sectional study in twenty hospitals (10 in Federal District, 3 in Rondônia and 7 in Maranhão). For 3 months, all women treated for abortion/miscarriage had their data collected. The severity of complications was defined according to World Health Organization criteria. Women with hemorrhage, infection or organs injury were invited to answer an interview about experience of care. Statistical analysis was performed using chi-square test and Poisson regression models. Among 1,683 women included, 82.5% had mild complications, 13.6% had moderate complications, 3.2% had potentially life-threatening conditions (PLTC) and 0.7% had severe maternal outcomes (SMO). Most women (94.2%) required uterine evacuation. Among these, 91.5% required surgical evacuation (with or without the use of uterotonics) and 8.5% used only uterotonics. The most frequent surgical evacuation method was curettage (66.9%), followed by manual vacuum aspiration (MVA) (32.3%). Factors associated with PLTC/SMO vs mild complications were having a gestational age ≥ 13 weeks (pravlence ratio - PR = 3.09; 95% confidence interval - 95%CI: 1.42-6.72), having been treated in Maranhão (PR = 0.27; 95%CI: 0.12-0.63) and in Rondônia (PR = 0.64; 95%CI: 0.20-0.99). Factors associated with moderate vs. mild complications were expulsion of products of conception before arrival to health facility (PR = 2.55; 95%CI: 1.64-3.96) and having been treated in Maranhão (PR = 0.58; 95%CI: 0.38-0.87). Most women who responded to the interview were treated kindly (95.6%), however, 66.7% felt stressed and 10.1% reported that their preferences were not respected during hospitalization. Nine out of ten women treated in Brazilian public hospitals due to abortion-related complications undergo some surgical procedure, the most common of which is uterine curettage. Approximately four in every hundred women experience severe complications. It is essential to ensure the supply of equipment for MVA and to encourage continuing medical education programs to increase the awareness of healthcare professionals about safer treatments for uterine evacuation.

ABSTRACT. The aim of this study was to examine the effect of longer cutting intervals on morphogenetic and structural traits, herbage production, nutritional value, and in vitro digestibility of Guinea grass cv. Mombaça (Mombaça grass). Four cutting intervals (49, 63, 77, and 91 days) were evaluated in two crop years (2015-2016 and 2016-2017) during the rainy season, in two replicates. Cutting intervals influenced structural and morphogenetic traits, except for number of live leaves (4.35 leaves tiller-1) and final leaf length (72.94 cm) in the 2015-2016 crop year. As the cutting intervals increased, dry matter yield and stem percentage increased, whereas leaf percentage and leaf-to-stem ratio declined. Regardless of the evaluated crop year, the dry matter, acid detergent fiber, and lignin contents increased linearly; however, the neutral detergent fiber content was unaffected. Cutting intervals affected the crude protein content and in vitro digestibility. Considering leaf appearance rate, stem appearance rate, and leaf-to-stem ratio, the recommended harvest age for Mombaça grass for optimum yield and nutritional value is 77 days. ABSTRACT We discuss the importance of fern and lycophyte species in the Atlantic Forest ecosystem as well as the need for floristic inventories to understand their distribution and conservation status. We focus on the Serra do Papagaio region in southeastern Brazil, which is known for its high plant diversity but lacks comprehensive inventories of ferns and lycophytes. In this study, we conducted a floristic inventory of Serra do Papagaio and identified 172 fern and 29 lycophyte species. Our findings reveal 21 threatened species and two new records for Minas Gerais State. Additionally, we compared the floristic composition of Serra do Papagaio with that of other areas within the Atlantic Forest using non-metric multidimensional scaling (NMDS) and generalized dissimilarity modeling (GDM) analyses. The results highlight distinct clusters of areas based on elevation and geographic location. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Background There is a remarkable variability in the frequency of HLA-B27 positivity in patients with spondyloarthritis (SpA), which may be associated with different clinical presentations worldwide. However, there is a lack of data considering ethnicity and sex on the evaluation of the main clinical and prognostic outcomes in mixed-race populations. The aim of this study was to evaluate the frequency of HLA-B27 and its correlation with disease parameters in a large population of patients from the Brazilian Registry of Spondyloarthritis (RBE). Methods The RBE is a multicenter, observational, prospective cohort that enrolled patients with SpA from 46 centers representing all five geographic regions of Brazil. The inclusion criteria were as follow: (1) diagnosis of axSpA by an expert rheumatologist; (2) age ≥18 years; (3) classification according to ASAS axial. The following data were collected via a standardized protocol: demographic data, disease parameters and treatment historical. Results A total of 1096 patients were included, with 73.4% HLA-B27 positivity and a mean age of 44.4 (±13.2) years. Positive HLA-B27 was significantly associated with male sex, earlier age at disease onset and diagnosis, uveitis, and family history of SpA. Conversely, negative HLA-B27 was associated with psoriasis, higher peripheral involvement and disease activity, worse quality of life and mobility. Conclusions Our data showed that HLA-B27 positivity was associated with a classic axSpA pattern quite similar to that of Caucasian axSpA patients around the world. Furthermore, its absence was associated with peripheral manifestations and worse outcomes, suggesting a relevant phenotypic difference in a highly miscegenated population. Abstract We predicted miRNAs with regulatory impact on NFKB1 and TRAF6 gene expression and selected the miR-194-5p, miR-124-3p, miR-9-5p, and miR-340-5p and their target genes for expression analyses on CD14+ monocytes from rheumatoid arthritis (RA) patients and healthy controls. Additionally, we evaluated the influence of genes and miRNA expression on RA patients’ cytokine levels. No difference was observed in genes or miRNAs expression when compared to healthy controls and RA patients or clinical parameters. However, we found a significant difference between miR-194-5p and miR-9-5p levels (FC=-2.31; p=0.031; FC=-3.05;p=0.031, respectively) and non-prednisone users as compared to prednisone using patients. We conducted correlation analyses to identify the strength of the relationship between expression data and cytokine plasma levels. We observed a moderate positive correlation between miR-124-3p expression and IL-6 plasma levels (r=0.46; p=0.033). In addition, overexpression of miRNAs was concomitant to TRAF6 and NFKB1 genes as indicated by correlation analyses: TRAF6 and miR-194-5p (r=0.60;p<0.001) and miR-9-5p (r=0.63;p<0.001) and NFKB1 and miR-194-5p (r=0.72;p<0.001), miR-9-5p (r=0.72;p<0.001) and miR-340-5p (r=0.61;p<0.001). NFKB1 and TRAF6 genes and miRNAs monocyte expression do not appear to be related to RA but showed a significant difference in different groups of RA therapy. Abstract Background Patients with immune-mediated rheumatic diseases (IMRDs) have been prioritized for COVID-19 vaccination to mitigate the infection severity risks. Patients with rheumatoid arthritis (RA) are at a high risk of severe COVID-19 outcomes, especially those under immunosuppression or with associated comorbidities. However, few studies have assessed the safety of the COVID-19 vaccine in patients with RA. Objective To evaluate the safety of vaccines against SARS-CoV-2 in patients with RA. Methods This data are from the study “Safety and Efficacy on COVID-19 Vaccine in Rheumatic Diseases,” a Brazilian multicentric prospective phase IV study to evaluate COVID-19 vaccine in IMRDs in Brazil. Adverse events (AEs) in patients with RA of all centers were assessed after two doses of ChAdOx1 (Oxford/AstraZeneca) or CoronaVac (Sinovac/Butantan). Stratification of postvaccination AEs was performed using a diary, filled out daily and returned at the end of 28 days for each dose. Results A total of 188 patients with RA were include, 90% female. CoronaVac was used in 109 patients and ChAdOx1 in 79. Only mild AEs were observed, mainly after the first dose. The most common AEs after the first dose were pain at the injection (46,7%), headache (39,4%), arthralgia (39,4%), myalgia (30,5%) and fatigue (26,6%), and ChAdOx1 had a higher frequency of pain at the injection (66% vs 32 %, p < 0.001) arthralgia (62% vs 22%, p < 0.001) and myalgia (45% vs 20%, p < 0.001) compared to CoronaVac. The more common AEs after the second dose were pain at the injection (37%), arthralgia (31%), myalgia (23%), headache (21%) and fatigue (18%). Arthralgia (41,4% vs 25%, p = 0.02) and pain at injection (51,4% vs 27%, p = 0.001) were more common with ChAdOx1. No serious AEs were related. With Regard to RA activity level, no significant difference was observed between the three time periods for both COVID-19 vaccines. Conclusion In the comparison between the two immunizers in patients with RA, local reactions and musculoskeletal symptoms were more frequent with ChAdOx1 than with CoronaVac, especially after the first dose. ABSTRACT Objective: Evaluate high school students’ knowledge about basic life support and analyze the associations between the level of knowledge and sociodemographic and academic variables. Method: Quantitative, descriptive, cross-sectional, and correlational study. The sample consisted of 59 high school students from a Secondary School in the metropolitan area of Porto - Portugal. Descriptive analyses, Pearson’s correlation coefficient, and t test (or non-parametric tests when assumptions were not met) were used. Results: 35 (59.3%) had basic life support training, and 52 (88.1%) were interested in undergoing the training. Only seven (11.9%) were aware of an automatic external defibrillator and 14 (23.7%) identified when to seek specialized help. Conclusion: Knowledge about basic life support can contribute to achieving better outcomes in situations of cardiorespiratory arrest, and students should increase their skills in this area by using training in the school context. Abstract Background Obesity is associated with the development of cardiovascular diseases and is a serious public health problem. In animal models, high-fat diet (HFD) feeding impairs cardiac structure and function and promotes oxidative stress and apoptosis. Resistance exercise training (RT), however, has been recommended as coadjutant in the treatment of cardiometabolic diseases, including obesity, because it increases energy expenditure and stimulates lipolysis. Objective In this systematic review, we aimed to assess the benefits of RT on the heart of rats and mice fed HFD. Methods Original studies were identified by searching PubMed, Scopus, and Embase databases from December 2007 to December 2022. This study was conducted in accordance with the criteria established by PRISMA and registered in PROSPERO (CRD42022369217). The risk of bias and methodological quality was evaluated by SYRCLE and CAMARADES, respectively. Eligible studies included original articles published in English that evaluated cardiac outcomes in rodents submitted to over 4 weeks of RT and controlled by a sedentary, HFD-fed control group (n = 5). Results The results showed that RT mitigates cardiac oxidative stress, inflammation, and endoplasmic reticulum stress. It also modifies the activity of structural remodeling markers, although it does not alter biometric parameters, histomorphometric parameters, or the contractile function of cardiomyocytes. Resumo Fundamento A obesidade está associada ao desenvolvimento de doenças cardiovasculares e constitui um grave problema de saúde pública. Em modelos animais, a alimentação com uma dieta hiperlipídica (DH) compromete a estrutura e a função cardíaca e promove estresse oxidativo e apoptose. O treinamento resistido (TR), entretanto, tem sido recomendado como coadjuvante no tratamento de doenças cardiometabólicas, incluindo a obesidade, porque aumenta o gasto energético e estimula a lipólise. Objetivo Na presente revisão sistemática, nosso objetivo foi avaliar os benefícios do TR no coração de ratos e camundongos alimentados com DH. Métodos Foram identificados estudos originais por meio de busca nas bases de dados PubMed, Scopus e Embase de dezembro de 2007 a dezembro de 2022. O presente estudo foi conduzido de acordo com os critérios estabelecidos pelo PRISMA e registrado no PROSPERO (CRD42022369217). O risco de viés e a qualidade metodológica foram avaliados pelo SYRCLE e CAMARADES, respectivamente. Abstract Brycon orbignyanus exhibits seasonal reproduction where males can be distinguished from females by the roughness present in the anal fin of the specimens that represents a secondary sexual characteristic known as hooks. This study aimed to describe the appearance and morphology of hooks on the anal fin in B. orbignyanus and relate them to the phases of the reproductive cycle of these animals to determine a parameter that can be used for the selection of suitable males for use in induced reproduction. Monthly male specimens of B. orbignyanus of different ages were collected (n = 50 total; n = 10/month) and the specimens were euthanised, biometrically measured, and the testes and anal fins were collected. As sample were processed according to the typical techniques for light microscopy, stereomicroscope, diaphanization and scanning electron microscopy. Except for specimens in the testicular stage of Regenerating, hooks were observed in all other stages of the reproductive cycle in B. orbignyanus males; however, specimens that possess six or more rays with hooks are considered Spawning Capable and can be selected for breeding. Resumo Brycon orbignyanus apresenta reprodução sazonal onde os machos podem ser diferenciados das fêmeas pela aspereza presente na nadadeira anal dos exemplares, que representa uma característica sexual secundária conhecida como espículas. Este estudo teve como objetivo descrever a morfologia das espículas na nadadeira anal de B. orbignyanus e relacioná-los com as fases do ciclo reprodutivo desses animais para determinar um parâmetro que possa ser utilizado na seleção de machos adequados para reprodução induzida. Foram coletados mensalmente machos de B. orbignyanus de diferentes idades (n = 50 no total; n = 10/mês), no qual foram eutanasiados, biometricamente mensurados e tiveram os testículos e nadadeiras anais coletados. As amostras foram processadas segundo as técnicas típicas de microscopia óptica, estereomicroscópio, diafanização e microscopia eletrônica de varredura. Com exceção dos espécimes na fase testicular de Regeneração, foram observadas espículas em todas as outras fases do ciclo reprodutivo em machos de B. orbignyanus; entretanto, espécimes que possuem seis ou mais raios com espículas são considerados Aptos a Espermiar e devem ser selecionados como reprodutores. Narrow Band Imaging versus White Light for the Detection of Sessile Serrated Colorectal Lesions: A Randomized Clinical Trial

Resumo Introdução: O cancro do cólon e reto é a neoplasia mais frequente considerando os dois géneros. . A deteção de lesões pré-malignas por colonoscopia está associada a uma redução da incidência e da mortalidade. Estudos sobre a utilização da luz de banda estreita (NBI) na deteção de lesões serreadas tiveram resultados promissores, mas heterogéneos. Métodos: Realizámos um ensaio clínico randomizado para comparar o número médio de lesões serreadas e lesões hiperplásicas ≥10 mm com NBI ou luz branca de alta-definição (HD-WL). Como resultados secundários comparámos a prevalência e as taxas de deteção de lesões serreadas sésseis, adenomas e todas as lesões. Resultados: Foram randomizados 782 doentes (392 no grupo HD-WL e 390 no grupo NBI). O número médio de lesões serreadas e hiperplásicas ≥10 mm não apresentou diferença estatisticamente significativa entre dois grupos (0.118 vs. 0.156, p = 0.44). A taxa de deteção de adenomas (55.2% vs. 53.2%, p = 0.58) e a taxa de deteção de lesões serreadas sésseis (6.8% vs. 7.5%, p = 0.502) também não foram diferentes. O tempo de retirada foi maior no grupo NBI (10.88 vs. 9.47 min, p = 0.004) e o tempo total de procedimento teve um ligeiro aumento não atingindo significância estatística (20.97 vs. 19.30 min, p = 0.052). Conclusão: A utilização da luz NBI por rotina não aumenta a deteção de lesões serreadas nem de qualquer lesão pré-maligna e aumenta o tempo de retirada na colo-noscopia.
Abstract Background: Colorectal cancer (CRC) is a leading cause of cancer. The detection of premalignant lesions by colonoscopy is associated with reduced CRC incidence and mortality. Narrow band imaging has shown promising but conflicting results for the detection of serrated lesions. Methods: We performed a randomized clinical trial to compare the mean detection of serrated lesions and hyperplastic polyps ≥10 mm with NBI or high-definition white light (HD-WL) withdrawal. We also compared all sessile serrated lesions (SSLs), adenoma, and polyp prevalence and rates. Results: Overall, 782 patients were randomized (WL group 392 patients; NBI group 390 patients). The average number of serrated lesions and hyperplastic polyps ≥10 mm detected per colonoscopy (primary endpoint) was similar between the HD-WL and NBI group (0.118 vs. 0.156, p = 0.44). Likewise, the adenoma detection rate (55.2% vs. 53.2%, p = 0.58) and SSL detection rate (6.8% vs. 7.5%, p = 0.502) were not different between the two study groups. Withdrawal time was higher in the NBI group (10.88 vs. 9.47 min, p = 0.004), with a statistically nonsignificant higher total procedure time (20.97 vs. 19.30 min, p = 0.052). Conclusions: The routine utilization of narrow band imaging does not improve the detection of serrated class lesions or any pre-malignant lesion and increases the with-drawal time.
