ABSTRACT Respiratory syncytial virus (RSV) infections constitute a significant cause of morbidity, hospitalization, school and work absenteeism, as well as mortality worldwide, including in Paraguay. Currently, tools for RSV prevention are available. In 2012, Paraguay approved the use of palivizumab (MedImmune, USA), a monoclonal antibody produced through recombinant DNA technology. This antibody is administered in 5 doses, every 30 days and is indicated in infants born prematurely and those with cardiopulmonary disorders. Furthermore, a novel tool for RSV prevention has recently become available. Nirsevimab, a long-acting monoclonal antibody specific to the prefusion conformation of the F protein, has been approved by both the European Medicines Agency (EMA) and the Food and Drug Administration (FDA). This monoclonal antibody is indicated for the prevention of acute lower respiratory tract infections caused by RSV in newborns and infants born during or entering their first RSV season, as well as in premature infants and infants up to 24 months of age who remain vulnerable to severe RSV disease during their second RSV season. After analyzing the epidemiological situation of RSV in our country and evaluating the evidence of efficacy, efficiency, and effectiveness of this monoclonal antibody, academic institutions, scientific societies, and non-governmental and governmental organizations developed consensus guidelines on a new prevention alternative for the prevention of RSV infections, suggesting the incorporation of Nirsevimab in children under 12 months of age before their first RSV season and replacing Palivizumab. The new monoclonal has the potential to change the panorama of RSV infections in infants and produce an impact on the reduction of infant mortality and morbidity reducing the burden on the health system by decreasing hospital bed occupancy both in wards and in intensive care units, as well as the decrease in the burden on caregivers, physicians and health care providers.

RESUMEN Las infecciones por el VSR en constituyen una causa importante de morbilidad, hospitalización, ausentismo escolar y laboral, así como de mortalidad, en el mundo, así como en el Paraguay.En la actualidad, existen herramientas para la prevención del VSR. En el año 2012, el Paraguay, ha incorporado el palivizumab (MedImmune, EE. UU.), anticuerpo monoclonal, producido por tecnología de DNA recombinante. Este anticuerpo se administra en 5 dosis, cada 30 días y está indicado en lactantes nacidos prematuros y aquellos con trastornos cardiopulmonares. Por otro lado, actualmente se cuenta con una nueva herramienta para la prevención del VSR. El anticuerpo monoclonal de vida media prolongada Nirsevimab, específico para la conformación de prefusión de la proteína F, aprobado por EMA y FDA. Este monoclonal, está indicado para la prevención de las Infecciones respiratorias agudas bajas, causada por VSR en recién nacidos y lactantes nacidos durante o al ingresar a su primera temporada de VSR, en prematuros y lactantes hasta los 24 meses de edad que siguen siendo vulnerables a la enfermedad grave por VSR durante su segunda temporada de VSR. Luego de analizar la situación epidemiológica del VSR en el país así como la evidencia de eficacia, eficiencia y efectividad de este monoclonal; instituciones académicas, sociedades científicas, organizaciones no gubernamentales y gubernamentales se reunieron y elaboraron un consenso interinstitucional para la prevención de las infecciones por VSR, sugiriendo la incorporación del Nirsevimab, en menores de 12 meses de edad antes de su primera temporada de VSR y en reemplazo del Palivizumab, debido a que el nuevo monoclonal tiene el potencial de cambiar el panorama de las infecciones por VSR en el lactante y producir un impacto en la reducción la mortalidad y morbilidad infantil; reduciendo la carga al sistema de salud, en lo que se refiere a la disminución de la ocupación de camas hospitalarias tanto en sala como en unidades de cuidados intensivos, así como la disminución de la carga para los cuidadores, médicos y proveedores de atención médica y la mortalidad infantil.

Abstract Introduction: cystic fibrosis newborn screening must enable its earlier diagnosis, which may enhance outcomes. This study was a series case of delayed-diagnosis children submitted to cystic fibrosis newborn screening. Description: fourteen children were included; eight (57.1%) were due to false-negative screening, while six (42.9%) were due to processing errors. Two samples collected after 30 days of life were incorrectly classified as negative, and four infants with a positive test could not be located due to screening processing errors. Cystic fibrosis diagnosis was confirmed at a median (IQR) age of 5.3 (4.2-7.4) months. Poor nutritional status was the most prevalent clinical sign at diagnosis, being present in 78.6% of infants. The mean (SD) weight-for-length and length-for-age Z-scores were -3.46 (0.84) and -3.99 (1.16), respectively. Half of the children had Pseudo-Bartter syndrome, and 42.9% had breathing difficulties. Twelve children (85.7%) required hospitalization, with a median (IQR) length of stay of 17.0 (11.5-26.5) days. Discussion: newborn screening had some faults, from incorrect collections to inefficient active search. Early identification of these children in which screening was unsatisfactory is essential, emphasizing the importance and efforts to not miss them. Resumo Introdução: a triagem neonatal para fibrose cística deve contribuir para diagnóstico precoce e melhor prognóstico da doença. O estudo é uma série de casos com lactentes submetidos à triagem, porém com diagnóstico tardio da doença. Descrição: quatorze crianças foram incluídas; oito (57,1%) com triagem falso-negativo e seis (42,9%) com erros processuais na triagem neonatal. Duas amostras foram coletadas tardiamente, sendo incorretamente classificadas como negativas e quatro lactentes com triagem positiva não foram localizados, por erros na busca ativa. Confirmou-se o diagnóstico da fibrose cística com idade mediana (IIQ) de 5,3 (4,2-7,4) meses. O Comprometimento nutricional precoce foi o sinal clínico mais prevalente ao diagnóstico, presente em 78,6% das crianças. Os Z escores médios (SD) do peso para altura e altura para idade foram -3,46 (0,84) e -3,99 (1,16), respectivamente. Metade das crianças teve síndrome de Pseudo-Bartter e 42,9% dificuldade respiratória. Doze crianças (85,7%) precisaram hospitalização com tempo mediano de permanência de 17 dias. Abstract: The Brazilian Unified National Health System (SUS) has incorporated newborn screening for cystic fibrosis since 2001. The protocol involves two samples of immunoreactive trypsinogen (IRT1/IRT2). This study aims to analyze fixed and floating values at the first and second IRT (IRT1/IRT2) cutoff points and assess the accuracy of the IRT/IRT methodology in a population from Northeastern Brazil. Descriptive, individual-level data from the newborn screening reference service data system (2013-2017) were used in this observational population study. The sensitivity, specificity, and positive predictive values (PPV) for the protocol were calculated. The best cutoff point was determined using the Youden’s index. The previous year’s cut-off values for the IRT1 and IRT2 99.4-, 99.5-, 99.6-, and 99.7-percentiles were utilized for the floating cutoff. During the studied period, 840,832 newborns underwent screening for cystic fibrosis, obtaining 49 cystic fibrosis diagnoses: 39 by newborn screening (79.6%) and 10 (20.4%) by clinical suspicion (false negative). The sensitivity, specificity, and PPV of the protocol totaled 79.6%, 99.9%, and 6.1%, respectively. No proposed cutoff for IRT1 performed better than the current one. IRT2 performed similarly to the current protocol at a cutoff point of 90ng/mL, showing the appropriate sensitivity and specificity while reducing the frequency of false positives. The protocol to screen newborns for cystic fibrosis had low sensitivity, a predictive positive value, and a high number of false positives and negatives. A floating cut point for IRT1 or IRT2 seems to constitute no viable option. However, changing the IRT2 cut point from 70ng/mL to 90ng/mL seems to have advantages and should undergo consideration. Resumo: A triagem neonatal para fibrose cística oi incorporada ao Sistema Único de Saúde (SUS) em 2001. O protocolo envolve duas amostras de tripsinogênio imunorreativo (TIR/TIR). O objetivo foi analisar os valores fixos e flutuantes no primeiro e segundo pontos de corte da TIR (TIR1/TIR2) e avaliar a acurácia da metodologia TIR/TIR em uma população do nordeste brasileiro. Trata-se de um estudo observacional de base populacional que inclui dados descritivos em nível individual obtidos retrospectivamente do Serviço de Referência em Triagem Neonatal (2013-2017). Foram calculados a sensibilidade, a especificidade e o valor preditivo positivo (VPP) do protocolo. O melhor ponto de corte foi determinado pelo índice de Youden. Os pontos de corte do ano anterior para os percentis TIR1 e TIR2 de 99,4, 99,5, 99,6 e 99,7 foram utilizados para o ponto de corte flutuante. No período do estudo, 840.832 recém-nascidos foram submetidos à triagem neonatal para fibrose cística, com 49 diagnósticos de fibrose cística, sendo 39 pela triagem neonatal (79,6%) e 10 (20,4%) por suspeita clínica (falso-negativos). A sensibilidade, a especificidade e o VPP do protocolo de triagem neonatal para fibrose cística foram de 79,6%, 99,9% e 6,1%, respectivamente. Nenhum dos pontos de corte propostos para a TIR1 mostrou-se melhor do que o atual. A TIR2 teve desempenho semelhante ao atual no ponto de corte de 90ng/mL, demonstrando sensibilidade e especificidade adequadas, ao mesmo tempo que reduziu a frequência de falsos positivos. A triagem neonatal para fibrose cística apresentou valores baixos de sensibilidade e VPP, e número elevado de falso-positivos e negativos. Um ponto de corte flutuante para TIR1 ou TIR2 não parece ser uma opção viável. No entanto, a mudança do ponto de corte da TIR2 de 70ng/mL para 90ng/mL parece ter vantagens e deve ser considerada. Resumen: El tamizaje neonatal de fibrosis quística fue incorporado al Sistema Único de Salud (SUS) en el 2001. El protocolo implica dos muestras de tripsinógeno inmunorreactivo (TIR/TIR). El objetivo fue analizar los valores fijos y flotantes en el primer y segundo puntos de corte de la TIR (TIR1/TIR2) y evaluar la precisión de la metodología TIR/TIR en una población del Nordeste brasileño. Se trata de un estudio observacional de base poblacional que incluye datos descriptivos a nivel individual obtenidos retrospectivamente del Servicio de Referencia en Tamizaje Neonatal (2013-2017). Se calcularon la sensibilidad, la especificidad y el valor predictivo positivo (VPP) del protocolo. El mejor punto de corte lo determinó el índice de Youden. La sensibilidad, la especificidad y el VPP del protocolo tamizaje neonatal para fibrosis quística fueron del 79,6%, 99,9% y 6,1%, respectivamente. Ninguno de los puntos de corte propuestos para la TIR1 resultó ser mejor que el actual. La TIR2 tuvo un desempeño similar al actual en el punto de corte de 90ng/mL, lo que demuestra sensibilidad y especificidad adecuadas, a la vez que redujo la frecuencia de falsos positivos. El tamizaje neonatal para fibrosis quística presentó valores bajos de sensibilidad y VPP, y un elevado número de falsos positivos y negativos. Un punto de corte flotante para TIR1 o TIR2 no parece ser una opción viable. Sin embargo, cambiar el punto de corte de la TIR2 de 70ng/mL a 90ng/mL parece tener ventajas y debe tenerse en cuenta. ABSTRACT Objective: To verify the association between bullying and awake and sleep bruxism in school teenagers. Material and Methods: A cross-sectional study was conducted from March to June 2018 with adolescents aged 14 to 19 of both sexes enrolled in the state school system of a city in Pernambuco. Students who participated in the stages of data collection and filled out the questions about bullying and bruxism were considered eligible. Those with disabilities or dysfunctions that made it impossible to fill out the questionnaire themselves were excluded. For data collection, an instrument with questions from the National Survey of Adolescent Health (PeNSE) and the "Research Diagnostic Criteria for Temporomandibular Dysfunction" (RDC/TMD) questionnaire was used. Results: A total of 2,500 adolescents participated in this study. The prevalence of possible awake bruxism among the adolescents was 20.6%, the prevalence of possible sleep bruxism was 21.9%, and 23% of the adolescents reported having experienced bullying. About 2 of 10 male students had possible sleep bruxism (SB), while a fourth of the female students had the condition (p=0.004; OR=1.1). As seen in the awake bruxism, feelings of sadness, school bullying, and cyberbullying had significant correlations with possible SB (p<0.001, OR=2.4; p<0.001, OR=1.9 and p=0.01, OR=1.7, respectively). Conclusion: The association between possible awake and sleep bruxism and bullying in school adolescents was verified, regardless of sex, age, income, and mother's education. ABSTRACT Objective: To analyze vaccination coverage (VC) for polio in the municipalities of Vale do Paraíba in the State of São Paulo. Methods: This is an ecological and exploratory study of VC in 35 municipalities using a spatial approach; VC data were obtained from the IT Department of the Unified Health System (DATASUS), for the years 2015 and 2019, and categorized into Low (VC<95%) and ideal (≥95%). Information was obtained on gross domestic product (GDP), professional rates and number of basic health units (UBS) and maternal data such as age, marital status (MS) and education. Univariate and bivariate Moran indices were estimated for the years 2015 and 2019, and thematic maps were created for CV values. Results: The average VC values were 107.7%±27.2 in 2015, and 94.2%±27.8 in 2019 (p<0.05). In 2015 vs. 2019, there were 10 vs. 25 municipalities in the Low category. In 2015, the variables VC, number of UBS, age, education, and MS were spatially correlated, but in 2019 only maternal age and education were spatially correlated. The bivariate Moran was significant and negative for VC in 2019 with maternal education. There was an increase in municipalities with worsening VC values. Conclusions: The spatial approach identified a decrease in polio vaccination coverage in the studied region. RESUMO Objetivo: Analisar as coberturas vacinais (CV) da poliomielite nos municípios do Vale do Paraíba paulista. Métodos: Trata-se de um estudo ecológico e exploratório de CV em 35 municípios com abordagem espacial. Os dados de CV foram obtidos do Departamento de Informática do Sistema Único de Saúde (DATASUS), referentes aos anos de 2015 e 2019 e categorizados em baixa (CV<95%) e ideal (≥95%). Foram obtidas informações sobre Produto Interno Bruto (PIB), taxas de profissionais, número de unidades básicas de saúde (UBS) e dados maternos como idade, estado civil (EC) e escolaridade. Foram estimados índices de Moran univariados e bivariados para os anos de 2015 e 2019 e criados mapas temáticos para valores de CV. Resultados: Os valores médios de CV foram de 107,7%±27,2 em 2015 e 94,2%±27,8 em 2019 (p<0,05). Em 2015, eram dez municípios na categoria baixa e, em 2019, eram 25 municípios. Em 2015, as variáveis CV, número de UBS, idade, escolaridade e situação marital foram correlacionadas espacialmente, mas em 2019 apenas idade materna e escolaridade foram correlacionadas espacialmente. O Moran bivariado foi significativo e negativo para CV em 2019 com escolaridade materna. Houve aumento de municípios com piora nos valores de CV. Conclusões: A abordagem espacial identificou diminuição da cobertura vacinal contra a poliomielite na região estudada. ABSTRACT Clinical data: A nine-month-old female infant diagnosed with transposition of the great arteries with symptoms of heart failure associated with cyanosis and difficulty in gaining weight was referred to our center with late diagnosis (at nine months of age). Chest radiography: Cardiomegaly; attenuated peripheral vascular markings. Electrocardiography: Sinus rhythm with biventricular overload and aberrantly conducted supraventricular extra systoles. Echocardiography: Wide atrial septal defect, ventricular axis torsion with concordant atrioventricular connection and discordant ventriculoarterial connection. Computed tomography angiography: Concordant atrioventricular connection, right ventricle positioned superiorly and left ventricle positioned inferiorly; discordant ventriculoarterial connection with right ventricle connected to the aorta and left ventricle connected to pulmonary artery. Diagnosis: Crisscross heart is a rare congenital heart defect, accounting for 0.1% of congenital heart diseases. It consists of the 90º rotation of ventricles’ axis in relation to their normal position; therefore, ventricles are positioned in the superior-inferior direction rather than anterior-posterior. Most cases have associated cardiac anomalies, and in this case, it is associated with transposition of the great arteries. The complexity and rarity of its occurrence make diagnosis and surgical treatment challenging. Abstract Increasing habitat modification and species loss demand consistent efforts to describe and understand biodiversity patterns. The BIOTA/FAPESP Program was created in this context and it has been a successful initiative to promote studies on biodiversity and conservation in Brazil. The BIOTA/Araçá is an interdisciplinary project that provided a detailed evaluation of the biodiversity of Araçá Bay, a coastal seascape located on the North coast of the state of São Paulo, Southeast Brazil. The bay encompasses multiple habitats, such as beaches, mangroves, rocky shores, and a tidal flat, and provides important ecosystem services. Unfortunately, the bay is the subject of complex social-environmental conflicts that oppose economic, social, and environmental demands (i.e., the expansion of neighboring harbor activities vs. small-scale artisanal fisheries and protection of biodiversity). The present study presents a survey of the benthic species occurring in the different habitats of Araçá Bay, including data obtained during the BIOTA/Araçá project and previous assessments of the area. The benthic species play an important role in marine environments and studying the diversity of these organisms that live associated with the bottom is indispensable for comprehending the environment’s functioning. The macrofauna, meiofauna, and microorganisms associated with soft and hard bottom were listed, and additional information, such as the habitat and geographical distribution, were provided for each species. The checklist includes 826 species, almost 70% recorded during the BIOTA/Araçá project. The most speciose taxa were the annelids (225 spp.), mollusks (194 spp.), and crustaceans (177 spp.). Seven benthic species are endemic to Araçá Bay, 14 are considered threatened, and seven are economically exploited. Furthermore, the bay is the type locality of many taxa, and 11 new benthic species were described based on specimens sampled during the project. Resumo O aumento da modificação dos habitats e da perda de espécies demanda esforços consistentes para descrever e compreender os padrões de biodiversidade. O programa BIOTA/FAPESP foi criado nesse contexto e é uma iniciativa de sucesso para promover estudos em biodiversidade e conservação no Brasil. O BIOTA/Araçá é um projeto interdisciplinar que promoveu uma avaliação detalhada da biodiversidade da Baía do Araçá, um ecossistema costeiro localizado ao Norte do estado de São Paulo, Sudeste do Brasil. A baía engloba múltiplos habitats, tais como praias, manguezais, costões rochosos, e uma planície de maré, e também fornece importantes serviços ecossistêmicos. Infelizmente, a baía está sujeita à conflitos sócio-ambientais complexos que contrastam demandas econômicas, sociais e ambientais (i.e. a expansão das atividades do porto vizinho vs. a pesca artesanal de pequena escala e a proteção da biodiversidade). O presente estudo apresenta um levantamento das espécies bentônicas que ocorrem nos diferentes habitats da Baía do Araçá, incluindo dados obtidos durante o projeto BIOTA/Araçá e de investigações realizadas anteriormente na área. As espécies bentônicas desempenham um papel importante no ambiente marinho, e estudar a diversidade desses organismos que vivem associados ao fundo é indispensável para compreender o funcionamento do meio ambiente. A macrofauna, meiofauna, e microorganismos associados aos fundos consolidado e inconsolidado foram listados, e informações adicionais foram fornecidas para cada espécie, tais como a distribuição geográfica e nos habitats. Abstract The objective was to investigate the association between the school context and the occurrence of multiple partners among adolescents, considering individual variables (age, gender, Bolsa Família, LGB, early sexual initiation and use of alcohol or drugs in the last sex). Cross-sectional study with multilevel analysis carried out in 2018 with adolescent students from Olinda, Brazil. The variable (multiple partners) was collected based on the ‘Youth Risk Behavior Survey’ questionnaire. School context variables were time in school (regular school vs. full/semi-full school) and the Social Vulnerability Index of the school district. Of 2,500 participants, 1,044 were analyzed for being sexually active and most had two or more partners (63.89%). Regular school students were more likely (OR 1.47, CI 1.10-1.97) to have multiple sexual partners compared to those in full-day schools/half-day schools. However, no association was found in relation to the SVI of the schools’ neighborhoods (OR 1.18, IC 0.82-1.70). Resumo O estudo investiga a associação entre o contexto escolar e a ocorrência de múltiplos parceiros entre adolescentes, considerando as variáveis individuais. Estudo transversal com análise multinível, realizado no período de fevereiro a junho de 2018 com estudantes de 14 a 19 anos. A variável desfecho (múltiplos parceiros sexuais) foi coletada com base no questionário Youth Risk Behavior Survey (YRBS). As variáveis do contexto escolar foram tempo na escola (escola regular vs escola integral/semi-integral) e índice de vulnerabilidade social (IVS) do bairro onde a escola está localizada. De 2.500 participantes, 1.044 foram analisados por serem sexualmente ativos. A maioria dos adolescentes (63,89%) teve dois ou mais parceiros. Estudantes de escola regular (mínimo de 4h diárias) tiveram mais chances (OR 1.47, IC 1.10-1.97) de terem múltiplos parceiros sexuais quando comparados àqueles de escola integral/semi-integral (mínimo de 7h diárias). Cancer will remain one of the most significant challenges for public health, locally and globally. Currently, cancer is the leading cause of death in our country. Thanks to the enormous knowledge accumulated in recent decades on the cellular and molecular bases of cancer, precision oncology has been developed, an approach that allows for increasingly precise pharmacological treatment based on diagnostic tests. Advanced technologies such as next-generation sequencing are used for this purpose. It is essential to implement these technologies in current and future health systems to optimize the arsenal of strategies for cancer control. This review discusses some of the achievements of precision oncology, particularly applied to solid tumors. It addresses the state-of-the-art minimum biomarkers required for the diagnosis of this important group of neoplasms, the local situation regarding technological capabilities installed in the national territory, either for research or diagnosis, and the potential health impact of applying all this practical knowledge to serve people with cancer, both in the public and private sectors.

RESUMEN El cáncer seguirá siendo uno de los mayores desafíos para la salud pública a nivel local y mundial. Actualmente, en nuestro país, el cáncer es la principal causa de muerte. Gracias al enorme conocimiento acumulado en las últimas décadas sobre las bases celulares y moleculares del cáncer, se ha desarrollado la oncología de precisión, un enfoque que permite dirigir de manera cada vez más precisa el tratamiento farmacológico en función de los exámenes de diagnóstico. Para ello se utilizan tecnologías avanzadas, como la secuenciación de próxima generación. Es imprescindible implementar estas tecnologías en los sistemas sanitarios actuales y futuros para optimizar el arsenal de estrategias para el control del cáncer. En esta revisión, se discuten algunos alcances de la oncología de precisión, especialmente aplicada a tumores sólidos. Se aborda el estado del arte de los biomarcadores mínimos necesarios para el diagnóstico de este importante grupo de neoplasias, la situación local en cuanto a las capacidades tecnológicas instaladas en el territorio nacional ya sea con fines de investigación o diagnóstico, y el potencial impacto sanitario que tendría la aplicación de todo este conocimiento práctico al servicio de las personas con cáncer, tanto en el sector público como privado.

Abstract: The development of citizen participation processes for planning and urban design through new technological tools has been a growing phenomenon for decades, which has accelerated in recent years due to the global COVID-19 health emergency . The objective of this work is to analyze opportunities and challenges in a diverse group of urban participation processes that have been advanced in an exploratory manner in new methodologies and digital tools for participation both in Chile and Colombia, countries with different urban governances within Latin America. The research is carried out through interviews and focus groups with urban planners and designers belonging to the public and private spheres of both countries, apart from participant observation in some digital instances. Some of the results are the potential of ICT´s to strengthen processes, although it isa more complex methodological and procedural scenario that needs to be previously legitimized before the citizenry. This work concludes that there are significant obstacles to achieving open and collaborative digital environments that allow bottom-up urbanism, explained partly by the lack of a critical perspective to approach the digitalization of participation.

Este trabajo tiene como objetivo analizar las oportunidades y retos en un grupo diverso de procesos de participación urbana que han avanzado de forma exploratoria en nuevas metodologías y herramientas digitales, en Chile y Colombia, países con gobernanzas urbanas diferentes dentro de Latinoamérica. La investigación se desarrolla mediante entrevistas y grupos focales a planificadores y diseñadores urbanos pertenecientes al ámbito público y privado de ambos países, además de la observación participante en algunas instancias digitales. Algunos de los resultados son la potencialidad de las tic para fortalecer procesos, aun cuando son un escenario metodológico y procedimental más complejo que necesita legitimarse antes en la ciudadanía. Se concluye la existencia de obstáculos importantes para lograr entornos digitales abiertos y colaborativos que permitan un urbanismo bottom-up, explicado en parte por la falta de una perspectiva crítica para abordar la digitalización de la participación.

Abstract Objective: To analyze the performance of the cystic fibrosis (CF) newborn screening (NBS) program over its first five years in a Brazilian northeastern state. Methods: A population-based study using a screening algorithm based on immunoreactive trypsinogen (IRT)/IRT. Data were retrieved from the state referral screening center registry. The program performance was evaluated using descriptive indicators such as the results of an active search, coverage, newborn’s age at the time of blood sampling, the time between sample collection and its arrival at the laboratory, and the child’s age at diagnosis of disease. Results: The public CF screening program covered 82.6% of the 1,017,576 births that occurred, with an accumulated five-year incidence of 1:20,767 live births. The median (25th-75th) age at diagnosis was 3.5 (2.3–7.3) months. The sampling before 7 days of life for the first IRT (IRT1) increased between 2013 and 2017 from 42.2 to 48.3%. Around 5% of IRT1 samples and 30% of the second samples were collected after 30 days of life. In the first and second stages of screening, 23.6% and 19.9% of the infants, respectively, were lost to follow-up. In both stages of screening, the samples were retained at the health units for a median (25th–75th) of 9.0 (7.0–13.0) days. Conclusions: The coverage by the CF-NBS program was satisfactory as compared to other Brazilian state rates and the percentage of IRT1 samples collected within the first week of life increased progressively. However, time of samples retention at the health units, inappropriate sampling, inherent methodological problems, and loss of follow-up need to improve. Abstract Objective To evaluate clinical characteristics and outcomes of COVID-19 patients infected with HIV, and to compare with a paired sample without HIV infection. Methods This is a substudy of a Brazilian multicentric cohort that comprised two periods (2020 and 2021). Data was obtained through the retrospective review of medical records. Primary outcomes were admission to the intensive care unit, invasive mechanical ventilation, and death. Patients with HIV and controls were matched for age, sex, number of comorbidities, and hospital of origin using the technique of propensity score matching (up to 4:1). They were compared using the Chi-Square or Fisher's Exact tests for categorical variables and the Wilcoxon for numerical variables. Results Throughout the study, 17,101 COVID-19 patients were hospitalized, and 130 (0.76%) of those were infected with HIV. The median age was 54 (IQR: 43.0;64.0) years in 2020 and 53 (IQR: 46.0;63.5) years in 2021, with a predominance of females in both periods. People Living with HIV (PLHIV) and their controls showed similar prevalence for admission to the ICU and invasive mechanical ventilation requirement in the two periods, with no significant differences. In 2020, in-hospital mortality was higher in the PLHIV compared to the controls (27.9% vs. 17.7%; p = 0.049), but there was no difference in mortality between groups in 2021 (25.0% vs. 25.1%; p > 0.999). Conclusions Our results reiterate that PLHIV were at higher risk of COVID-19 mortality in the early stages of the pandemic, however, this finding did not sustain in 2021, when the mortality rate is similar to the control group.
