ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. We review here a new model of private primary care provision emerging in South Africa, in which commercial companies provide standardized primary care services at relatively low cost. The structure and operation of one such company is described, and features of service delivery are compared with the most probable alternatives: a private general practitioner or a public sector clinic. In a case study of cost and quality of services, the clinics were popular with service users and run at a cost per visit comparable to public sector primary care clinics. However, their current role in tackling important public health problems was limited. The implications for public health policy of the emergence of this new model of private provider are discussed. It is argued that encouraging the use of such clinics by those who can afford to pay for them might not help to improve care available for the poorest population groups, which are an important priority for the government. Encouraging such providers to compete for government funding could, however, be desirable if the range of services presently offered, and those able to access them, could be broadened. However, the constraints to implementing such a system successfully are notable, and these are acknowledged. Even without such contractual arrangements, these companies provide an important lesson to the public sector that acceptability of services to users and low-cost service delivery are not incompatible objectives.
En los países de ingresos bajos y medianos el recurso a proveedores de asistencia sanitaria privados constituye una práctica generalizada, que es objeto de amplio debate. Examinamos aquí un nuevo modelo de suministro de atención primaria privada surgido en Sudáfrica, en el que empresas comerciales prestan servicios normalizados de atención primaria a un costo relativamente bajo. Se describe la estructura y el funcionamiento de una de dichas empresas, comparando las características de la prestación de servicios con las alternativas más probables: un médico general privado o un dispensario del sector público. Un estudio práctico sobre el costo y la calidad de los servicios reveló que los dispensarios eran populares entre los usuarios de los servicios y tenían un costo por visita comparable al de los dispensarios de atención primaria del sector público. Sin embargo, su contribución al control de problemas importantes de salud pública era limitada. Se examinan las implicaciones para las políticas de salud pública de la aparición de este nuevo modelo de proveedor privado. Se alega que la promoción del uso de tales dispensarios por quienes pueden pagar por ellos quizá no ayude a mejorar la atención disponible para los grupos de población más pobres, que constituyen una prioridad importante para el gobierno. No obstante, si lograra ampliarse el abanico de servicios actualmente ofrecido y el número de personas con acceso a ellos, sería deseable quizá que se alentara a tales proveedores a competir para conseguir financiamiento del gobierno. Sin embargo, las dificultades existentes para implantar con éxito esos sistemas son considerables y reconocidas. Aun sin tales arreglos contractuales, estas empresas suponen una importante lección para el sector público, a saber, que la aceptación de los servicios por los usuarios y la prestación de servicios de bajo costo no son objetivos incompatibles.
Le recours à des dispensateurs de soins de santé privés dans les pays à revenu faible ou intermédiaire est largement répandu et fait l'objet de nombreux débats. Nous examinons ici un nouveau type de dispensation des soins de santé primaire par des prestataires privés, qui fait son apparition en Afrique du Sud, où des sociétés commerciales proposent des services standardisés de soins de santé primaires à un coût relativement bas. L'article décrit la structure et le fonctionnement d'une de ces sociétés et compare les divers aspects des services offerts avec ceux des alternatives les plus probables, à savoir un médecin généraliste du secteur privé ou un dispensaire public. Lors d'une étude de cas portant sur le coût et la qualité des services, les dispensaires privés jouissaient de la faveur des usagers et fonctionnaient avec un coût par visite comparable à celui des dispensaires de soins de santé primaires du secteur public. Le rôle actuel de ces dispensaires dans la prise en charge de problèmes de santé publique importants est cependant limité. Les conséquences de l'émergence de ce nouveau type de prestataires sur les politiques de santé publique sont examinées. Il est souligné qu'en encourageant les personnes qui en ont les moyens à utiliser de tels services, on ne contribue pas nécessairement à améliorer les soins accessibles aux groupes de population les plus pauvres, qui constituent une priorité majeure pour le gouvernement. Il pourrait cependant être souhaitable d'encourager les prestataires privés à solliciter une aide gouvernementale si l'éventail des services offerts, ainsi que la population qui y aurait accès, étaient élargis. On sait toutefois que la réussite de la mise en œuvre d'un tel système se heurte à d'importantes contraintes. Mais, même en l'absence d'accords contractuels, ces sociétés démontrent clairement au secteur public que l'acceptabilité des services par les usagers et la fourniture de services à faible coût ne sont pas des objectifs incompatibles.

Efeito imunomodulador de hormônios tímicos sobre o trauma térmico experimental

O trauma grave, em particular térmico, promove a imunodepressão. Hormônios tímicos podem modificar a resposta imune nesta comdição. O objetivo desta pesquisa foi investigar o efeito imunomodulador do hormônio tímico timoestimulina sobre a imunidade celular em trauma térmico, traduzida pela resposta dos testes cutâneos de hipersensibilidade tardia. Na primeira etapa do experimento, ratos Wistar foram sensibilizados com KHL e posteriormente submetidos ao teste cutâneo de hipersensibilidade tardia (TCHT). Dez animais com resposta positiva foram submetidos a queimadura cutânea padronizada. Testes cutâneos seqüenciais revelaram significativa anergia cutânea no 21º e 30º dias pós-queimadura e interrupção do ganho de peso. Na etapa seguinte, ratos previamente sensibilizados e com resposta positiva ao TCHT, foram divididos nos grupos TAQ (seis ratos) tratados com timoestimulina, por via intramuscular, na dose de 0,25 mg, antes e após queimadura, grupo TDQ (seis ratos) tratados com a mesma dose de timoestimulina apenas após a queimadura e grupo CSQ (cinco ratos) tratados com solução salina, im, após a queimadura. Ocorreu aumento significativo do peso corpóreo e da resposta do TCHT a partir de duas semanas pós queimadura nos grupos TAQ e TDQ. Nas condições da presente pesquisa, timoestimulina misnistrada antes e após a queimadura experimental associa-se a melhora da resposta ao teste cutâneo de hipersensibilidade tardia e ganho de peso corpóreo.
Severe trauma, mainly burn, may promote immunodepression. Thimic hormones may modify the immune response to this condition. The aim of this experimental research was to investigate, after burn, the immunomodulatory effect of timoestimulin, a thimic hormone, on celular immunity studied by the delayed hypersensitivity skin test (TCHT). On the first phase of the experiment, Wistar rats were sensitized with KHL and submitted to TCHT. Ten animals with positive tests were submitted to a standardized skin burn. Sequencial skin tests showed significant skin anergy at 21 and 30 days after burn and interruption of weight gain. In the next phase, positively sensitized rats were divided randomly in group TAQ (six) and treated with timoestimulin, 0,25 mg, im, before and after burn; group TDQ (six rats) treated with the same dosage of timoestimulin just after burn and group CSQ (five rats) treated with saline, im, after burn. There was a significant increase of body weight and skin test response at two weeks after burn in TAQ and TDQ groups. At the conditions of the present research, timoestimulin given before and after experimental burn was associated to improvement of delayed cellular hypersensitivity skin test and body weight gain.
