Resumen Introducción : La región selar es un sitio infrecuente de metástasis, encontrándose en el 1% de las cirugías hipofisarias. Los tumores primarios más habituales son mama y pulmón. En general son diagnosticadas en pa cientes con enfermedad avanzada, aunque pueden ser el debut de la enfermedad oncológica. Métodos : Análisis retrospectivo de las características clínicas, bioquímicas, radiológicas de pacientes con me tástasis selares o hipofisarias (MS) durante el periodo 2009-2020. Resultados : Se reportaron 18 casos de pacientes, 11 de ellos con confirmación histológica. La mediana de edad fue 53 años (rango: 35-75), 53% hombres. La loca lización del tumor primario fue: 8 pulmón, 6 mama, 1 carcinoma folicular de tiroides, 1 linfoma Hodgkin y 2 carcinomas renales de células claras. La media de tiem po entre el diagnóstico del tumor primario y la aparición de la MS -en los casos de presentación metacrónica- fue 108 meses (rango: 11-180). En 8 pacientes (44.4%), el diagnóstico de la neoplasia primaria se hizo a partir del hallazgo de la masa selar. Diabetes insípida, hipopitui tarismo, trastornos visuales, oftalmoplejía y cefalea se presentaron en el 78, 77, 61, 39 y 39%, respectivamente. Quince pacientes presentaron masas con extensión su pra/paraselar; y 3 lesión limitada a la hipófisis y tallo. Fueron operados 11/18 por vía transesfenoidal, para diagnóstico y/o descompresión. Fallecieron 17, con una mediana de sobrevida de 6 meses (1- 36). Discusión : La sospecha de MS debe estar presente ante una masa selar y supraselar con captación difusa del gadolinio, diabetes insípida, hipopituitarismo y/o disfunción visual, aun en pacientes sin antecedentes oncológicos.

Abstract Introduction : Sellar metastases (SM) are rare mani festations of malignancy. Breast and lung cancer are the most common primary tumors. Most cases are di agnosed in patients with advanced malignant disease; however, symptoms of pituitary involvement can pre cede the diagnosis of the primary tumor. Methods : Retrospective analysis of symptoms at presentation, hormonal, radiological and histological findings, management, and outcome of patients with SM from 2009 to 2020. Results : Eighteen patients´cases were included, 11 with histological confirmation. Median (m) age was 53 years (range 35-75), 53% male. Primary malignant tu mors: 8 lungs, 6 breast, 1 follicular thyroid carcinoma, 1 Hodgkin lymphoma, and 2 clear cell renal carcinomas. The m time between the diagnosis of the primary neo plasm and the occurrence of the SM was 108 months (range: 11-180). In 8 patients the diagnosis of the primary neoplasm was made after the finding of the symptom atic sellar mass. Insipidus diabetes, adenohypophysis deficit, visual disorders, headache, and cranial nerve deficits were evident in 78, 77, 61, 39 and 39% of the cases, respectively. Fifteen patients harbored supra / parasellar masses, in three a lesion was limited to the pituitary gland, and stalk. Eleven out of 18 (61.1%) of the patients were operated on by the trans-sphenoidal approach, for diagnostic and / or decompressive pur poses. Eighteen died, with a median survival time of 6 months (1-36). Discussion : In the presence of a pituitary lesion with diffuse gadolinium uptake, associated with insipidus diabetes and / or visual disorder SM should be suspected even in patients without a history of oncological disease.

Resumen Se analizó la dieta de gatos (Felis silvestris catus) y perros (Canis lupus familiaris) errantes de la Reserva Ecológica del Pedregal de San Ángel (REPSA), Ciudad Universitaria, UNAM, Ciudad de México. Se examinaron 12 muestras de gatos y 55 de perros obtenidas mediante el Programa de Remediación de Fauna Feral, Secretaría Ejecutiva de la REPSA. La determinación de las especies consumidas se realizó comparando el material con ejemplares depositados en colecciones científicas; adicionalmente para mamíferos, se utilizaron patrones morfológicos de la cutícula y médula del pelo. El análisis mostró que la dieta de gatos y perros está compuesta, principalmente, de mamíferos medianos (Bassariscus astutus, Didelphis virginiana, Otospermophilus variegatus, Spilogale angustifrons y Sylvilagus floridanus) y pequeños (Mus musculus, Neotoma mexicana, Peromyscus gratus, Rattus norvegicus, Reithrodontomys fulvescens y Sorex saussurei), en menor proporción de reptiles e insectos, y material alimenticio de consumo humano. Los mamíferos nativos son más consumidos que los introducidos y que la dieta de gatos y perros incluye especies endémicas de México (S. saussurei, P. gratus y Peromyscus melanophrys) o en alguna categoría de riesgo (S. saussurei y la lagartija Sceloporus grammicus). Estos resultados proveen información para justificar y desarrollar alternativas de conservación en reservas ecológicas urbanas.

Abstract Diet analysis was performed on free ranging cats (Felis silvestris catus) and dogs (Canis lupus familiaris) from Reserva Ecológica del Pedregal de San Ángel (REPSA), Ciudad Universitaria, UNAM, Mexico City. Twelve samples of cats and 55 of dogs were analyzed as part of the Wildlife Remediation Program of the REPSA’s Executive Secretariat. The determination of consumed species was made by comparing the obtained remains with specimens deposited in scientific collections; additionally, for mammals, morphological scales patterns and hair marrow were examined. Our results showed that the diet of cats and dogs is mainly composed of medium-size mammals (Bassariscus astutus, Didelphis virginiana, Otospermophilus variegatus, Spilogale angustifrons, and Sylvilagus floridanus) and small mammals (Mus musculus, Neotoma mexicana, Peromyscus gratus, Rattus norvegicus, Reithrodontomys fulvescens, and Sorex saussurei), with a smaller proportion of reptiles and insects and food material for human consumption. The native mammals are consumed more than introduced mammals, and that diet of cats and dogs includes species endemic to Mexico (S. saussurei, P. gratus, and Peromyscus melanophrys) or in any risk category (S. saussurei and the lizard Sceloporus grammicus). These results provide information for justify and developing conservation alternatives in urban ecological reserves.

Blending finer powders of Ti, Zr, Ta, Sc, and Nb compounds as inoculants have been a promising strategy to modify precipitation-hardened aluminum alloys for laser-based powder bed fusion, promoting a crack-free equiaxed microstructure. However, there is still a lack of comprehensive understanding of the influence of these fine Al-Nb-B inoculant particles on the AA 2017 powder's physical characteristics and flowability during the process. The results indicate that blended powder has a similar PSD to AA 2017 powder. Furthermore, the circularity, smoothness, and morphology of the particles indicate that both inoculant and AA2017 powders do not exhibit high sphericity, but the blended powder showed slightly more agglomerated particles. Regarding rheological properties, it was observed that a higher flow energy was required to move the blended powder in unconfined conditions compared to the AA 2017 powder. Additionally, the blended powder exhibited higher compressibility and tendency to retain air in packed conditions during the deposition and spreading process. In conclusion, the physical characterization techniques combined with rheological tests have proven to be a rapid and reliable approach for assessing the impact of the finer inoculant particles' characteristics on the laser-based powder bed fusion. RESUMO Objetivo Correlacionar os resultados da avaliação comportamental do processamento auditivo central e do questionário de autopercepção após o treinamento auditivo acusticamente controlado. Método Foram avaliados dez indivíduos com média de idade de 44,5 anos, que sofreram traumatismo cranioencefálico de grau leve. Os indivíduos foram submetidos a avaliação comportamental do processamento auditivo central e também responderam ao questionário de autopercepção “Treinamento Auditivo Formal” após a intervenção terapêutica. O questionário foi composto por questões referentes a percepção auditiva, compreensão de ordens, solicitação de repetição de enunciados, ocorrência mal-entendidos, tempo de atenção, desempenho auditivo em ambiente ruidoso, comunicação ao telefone e autoestima e os pacientes foram solicitados a assinalar a frequência de ocorrência dos comportamentos listados. ABSTRACT Purpose To correlate behavioral assessment results of central auditory processing and the self-perception questionnaire after acoustically controlled auditory training. Methods The study assessed 10 individuals with a mean age of 44.5 years who had suffered mild traumatic brain injury. They underwent behavioral assessment of central auditory processing and answered the Formal Auditory Training self-perception questionnaire after the therapeutic intervention - whose questions address auditory perception, understanding orders, request to repeat statements, occurrence of misunderstandings, attention span, auditory performance in noisy environments, telephone communication, and self-esteem. Patients were asked to indicate the frequency with which the listed behaviors occurred. Results Figure-ground, sequential memory for sounds, and temporal processing correlated with improvement in following instructions, fewer requests to repeat statements, increased attention span, improved communication, and understanding on the phone and when watching TV. Abstract Introduction : Primary hypophysitis (PH) is a rare disease that represents a challenge among differential diagnosis and management. Our aim was to describe clinical characteristics, diagnostic criteria and different treatment outcomes in patients with PH. Multicentric, retrospective study. Clinical presentation, endocrine function, magnetic resonance imaging findings, visual field defects at diagnosis and treatment outcomes were recorded. Methods : Twenty-eight patients (23 women), with PH were included. Median age: 37. Results : The most frequent symptoms: headache: 68%, polyuria-polydipsia: 50% and visual disturbances: 48%. At diagnosis, anterior pituitary deficiency was present in 71%, being hypogonadotrophic hypogonad ism the most frequent manifestation. The radiological findings: symmetric lesion: 78.5%, homogeneous en hancement: 78.5% and pituitary stalk thickening: 70%. Association with pregnancy or puerperium was found in 4/23 women (17%). Fourteen patients did not receive any treatment (“wait and see” group), 8 underwent surgery for mass reduction or resection and 6 were treated with immunosuppression therapy. Among 15 patients with histopathological diagnosis, 9 were lymphocytic hypophysitis, 5 IgG4 related hypophysitis and 1 xanthomatous hypophysitis. Thirteen were di agnosed by established clinical criteria. Mass reduction was observed in 43% of “wait and see group” patients, 62.5% of operated patients and 50% with immuno suppression therapy. Compressive symptoms showed improvement in the 3 groups, with modest effect on anterior pituitary function, diabetes insipidus did not resolve in any patients. Discussion : In patients without severe compressive symptoms, we adopted a “wait and see” approach. In patients with uncertain diagnosis of PH or severe com pressive symptoms, transsphenoidal surgery was the best option.

Resumen Introducción : La hipofisitis es una enfermedad in frecuente que plantea un desafío en el diagnóstico y tratamiento. El objetivo de este estudio multicéntrico y retrospectivo fue describir: a) características de pa cientes con hipofisitis primaria (HP), b) métodos diag nósticos, y c) tratamientos realizados. Además, evaluar: a) presentación clínica, b) bioquímica, c) radiológica, d) oftalmológica al diagnóstico y evolución según el tratamiento recibido. Métodos : Estudio retrospectivo donde se estudiaron 28 pacientes (23 mujeres/5 varones) con HP, edad pro medio de 38±11.1 años. Resultados . Los síntomas fueron: cefalea: 68%, po liuria-polidipsia: 50% y alteraciones visuales: 48%. El examen de laboratorio inicial informó disfunción ade nohipofisaria en 71% de los pacientes, siendo el eje gonadal el más afectado. Los hallazgos radiológicos más frecuentes fueron: lesión simétrica:78.5%, aumento homogéneo post contraste: 78.5% y engrosamiento de tallo:70%. En 4/23 mujeres (17%) se relacionó con em barazos o puerperio La conducta inicial fue expectante (CE) en 14 pacientes, cirugía de resección/descompresión en 8 y tratamiento con inmunosupresores en 6. Quince pacientes tuvieron confirmación histológica, 9 resulta ron hipofisitis linfocitaria, 5 hipofisitis por IgG4 y una xantomatosa. Trece fueron diagnosticados por criterios clínicos establecidos. El tamaño de la lesión disminuyó en 43% de pacientes con CE, 62.5% con cirugía y 50% con inmunosupresores. Los síntomas compresivos mejora ron en los 3 grupos, con escaso efecto sobre la función adenohipofisaria, sin resolución de la diabetes insípida. Discusión : En pacientes sin síntomas compresivos adoptamos CE. En aquellos pacientes sin certeza diag nóstica o con síntomas compresivos graves, se optó por cirugía transesfenoidal.

RESUMO Uma nova espécie de Rhynchospora (Cyperaceae), conhecida apenas para o Planalto dos Parecis, no sudoeste da Amazônia brasileira é descrita. Rhynchospora parecisensis foi encontrada apenas em ambientes abertos de floresta sazonal decídua e enclaves de savana, em meio a áreas de floresta estacional sempre verde, estado do Mato Grosso, centro-oeste do Brasil. Rhynchospora parecisensis difere das outras espécies de Rhynchospora sect. Pluriflorae subsect. Subebracteatae pelas suas glumas puberulentas a pubescentes distalmente. A nova espécie é semelhante a Rhynchospora acanthoma, porém difere pelos escapos subtriangulares a quadrangulares em secção transversal, e pelas glumas puberulentas a pubescentes distalmente, com margens ciliadas a cilioladas. ABSTRACT A new species of Rhynchospora (Cyperaceae) only known from the Parecis plateau, on the southwestern edge of the Brazilian Amazon, is described here. Rhynchospora parecisensis was found growing only in open environments of deciduous seasonal forest and savanna enclaves in the midst of seasonal evergreen forest, in the state of Mato Grosso, western Brazil. Rhynchospora parecisensis differs from the other species of Rhynchospora sect. Pluriflorae subsect. Subebracteatae by the distally puberulent to pubescent glumes. The new species resembles Rhynchospora acanthoma, but differs from it by the subtriangular to quadrangular scapes in cross-section and the distally puberulent to pubescent glumes, with ciliate to ciliolate margins. What impact has the COVID-19 pandemic brought to the musculoskeletal health of formal home office workers? To answer this question, a prospective observational study was carried out through data collection via an online questionnaire, from September 2020 to October 2021. Questions were created about the profile of formal workers, frequency of physical activities, back pain and others joint and other factors associated with remote work and social isolation. A sample of 98 participants was obtained. In conclusion, there was a change in the pattern of physical activities and, the hours worked in the period, caused worsening and/or emergence of pain.

Qual o impacto que a pandemia da COVID-19 trouxe para a saúde osteomuscular dos trabalhadores formais em home office? Para responder esta pergunta foi realizado estudo prospectivo observacional por meio da coleta de dados via questionário online, no período de setembro de 2020 a outubro de 2021. Foram criadas questões sobre o perfil dos trabalhadores formais, frequência de atividades físicas, dores na coluna e outras articulações e demais fatores associados ao trabalho remoto e ao isolamento social. Foi obtida amostra de 98 participantes. Como conclusão, verificou-se alteração do padrão de atividades físicas e, a quantidade de horas trabalhadas no período, acarretou piora e/ou surgimento de dores.

RESUMEN El presente artículo resume el proceso de elaboración de la Guía de Práctica Clínica (GPC) para el manejo de dolor en pacientes oncológicos. Este proceso se llevó a cabo con la participación de un equipo multidisciplinario de médicos asistenciales, metodólogos y diversos revisores externos (especialistas con dominio en la metodología y el tema). La priorización de preguntas PICO fue realizada por el Grupo Elaborador de la GPC (GEG), acordando trabajar cinco preguntas PICO. Para dar respuesta a las preguntas se realizó una búsqueda sistemática de GPC, revisiones sistemáticas y estudios primarios. Se utilizó la metodología GRADE y los lineamientos de la normativa nacional para la formulación de recomendaciones. Se formularon 12 recomendaciones (10 fuertes y 2 débiles), 5 puntos de buena práctica clínica y 4 cuadros consensuados sobre el manejo de dolor oncológico. Los temas que abarcaron las recomendaciones para el manejo de dolor en pacientes oncológicos fueron: intervención temprana de tratamiento, terapia analgésica en dolor leve a moderado y en dolor moderado a severo, dolor neuropático e intervenciones no farmacológicas.

ABSTRACT The article summarizes the process of elaboration of the Clinical Practice Guide (CPG) for the management of cancer patients. The elaboration process was carried out with the participation of a multidisciplinary team of assisting physicians, methodologists and various external reviewers (specialists with mastery in the methodology and the subject). The prioritization of PICO questions was carried out by the GPC Elaboration Group (GEG), after which 05 PICO questions were concluded. To answer the questions, a systematic search of CPGs, systematic reviews and primary studies was carried out. The “GRADE” methodology and the guidelines of national regulations were used to formulate recommendations. Twelve recommendations were made (ten strong and two weak), 5 points of good clinical practice, 04 consensus tables on the management of cancer pain. The topics that covered the recommendations for pain management in cancer patients were: early treatment intervention, analgesic therapy in mild to moderate pain and moderate to severe pain, neuropathic pain and non-pharmacological interventions.

Resumo Este estudo trata das Cyperaceae ocorrentes na Serra dos Martírios-Andorinhas (Parque Estadual Serra dos Martírios Andorinhas e Área de Proteção Ambiental de São Geraldo do Araguaia), no município de São Geraldo do Araguaia, Pará, Brasil. A Serra está no “Arco do Desmatamento”, uma área de elevada vulnerabilidade ambiental com os maiores índices de desmatamento da Amazônia. A fitofisionomia predominante é a de Savana Amazônica, caracterizada pelos ambientes abertos com o predomínio de herbáceas, como as Cyperaceae, representadas por apenas quatro espécies em inventário florístico prévio. Estudar a diversidade de Cyperaceae nos permite compreender melhor a dinâmica do estrato herbáceo, contribuindo para a conservação, monitoramento e manejo destes peculiares e fascinantes ambientes. Para tanto, foram analisadas 276 amostras de Cyperaceae da Serra das Andorinhas provenientes de coletas de campo e de acervos de herbários. Atualmente, Cyperaceae é representada por 12 gêneros e 72 espécies na área de estudos, das quais, sete são novos registros para o estado do Pará. São apresentadas chave de identificação, descrições sucintas com caracteres diagnósticos, distribuição geográfica, hábitat, ilustrações de espécies e gêneros.

Abstract This study focuses on the Cyperaceae occurring in the Serra dos Martírios-Andorinhas (Serra dos Martírios- Andorinhas State Park, and São Geraldo do Araguaia Environmental Protection Area), in the São Geraldo do Araguaia municipality, state of Pará, Brazil. The Serra is in the “Deforestation Arc”, an environmentally vulnerable area with the highest deforestation rates in the Amazon Forest. The predominant phytophysiognomy is the Amazonian savanna, characterized by open environments dominated by herbaceous plants, such as Cyperaceae, represented by only four species in a previous floristic inventory. Studying the diversity of Cyperaceae allows us to a better understanding of the herbaceous layer’s dynamics and assisting in the conservation, monitoring, and management of these fascinating and peculiar environments. Therefore, 276 field and herbarium samples of Cyperaceae from Serra dos Martírios-Andorinhas were examined. Now, Cyperaceae are represented by 12 genera and 72 species in the study area, of which seven represent new records for the State of Pará. A taxonomic identification key, short descriptions with main diagnostic characters, geographic distribution, habitat data, and illustrations for all species and genera are provided.

Abstract AlCoCrFeNi alloys are widely studied due to the combination of high strength in a wide temperature range, good corrosion and wear resistance, but they typically present low ductility. Due to this limited ductility, traditional bulk fabrication processes that involve forming are difficult, and casting processes are also non-ideal due to pronounced segregation and coarse microstructure. Therefore, both the selective laser melting and the powder metallurgy are more promising routes for this alloy, both of these use powder as starting material. This work aimed to produce powder of Al20Co20Cr20Fe20Ni20 alloy by gas atomization and to perform its detailed characterization.The results show that the powder is mainly spherical, which resulted in good flowability. Most of the powder (62%) has a size smaller than 75µm, and microscopy analyses confirmed that a B2 dendritic microstructure was formed, with A2 cuboidal precipitates.

Resumen: Antecedentes y Objetivos: Phaseolus coccineus es una planta alógama, con variación en color de la flor, que depende de polinizadores para su reproducción. La especie se cultiva bajo dos modalidades, una en la cual no se emplean agroquímicos para controlar arvenses y plagas (manejo tradicional) y otra en la que sí se usan (manejo tecnificado). No existen estudios que hayan evaluado la importancia del manejo agrícola y color de flor sobre los visitantes florales de esta especie, por lo que nuestro objetivo fue analizar cómo influyen estos factores en la diversidad y frecuencia de visitantes florales de P. coccineus. Métodos: Se estudiaron dos variedades de P. coccineus (de flor blanca y roja), cultivadas bajo dos sistemas de manejo diferentes. En el pico de floración, durante ocho días, se realizaron observaciones simultáneas, contabilizando el número de visitantes florales y de visitas. Se calculó la riqueza específica y los números de especies efectivas como medida de diversidad, se aplicaron pruebas de chi-cuadrada, se practicó un análisis de correspondencias y se construyeron gráficos de rango-abundancia. Resultados clave: Se registraron 42 morfoespecies, agrupadas en seis órdenes; predominaron Apis mellifera e Hylocharis leucotis. El número de visitas registradas fue de 1613 y 1105 en manejo tradicional de flores blancas y rojas, respectivamente, y de 1427 y 815 en el manejo tecnificado, existiendo diferencias estadísticas consistentes entre manejos, no así para color de flor. En flores blancas, el manejo tradicional fue dos veces más diverso que el tecnificado; en flores rojas, la diferencia fue de 1.26 veces. Conclusiones: La diversidad y frecuencia de visitantes florales de P. coccineus se ven favorecidas por el manejo agrícola tradicional, ello fundamenta la conveniencia de fomentar esta modalidad de cultivo. Bajo las condiciones estudiadas, el color de flor no influyó en la riqueza de especies, ni mostró tendencias definidas en la frecuencia de visitas.

Abstract: Background and Aims: Phaseolus coccineus is an allogamous plant, with variation in flower color, and reproduction that depends on pollinators. The species is cultivated under two approaches: one in which agrochemicals are not used to control weeds and pests (traditional management) and another in which they are (technified management). No studies have evaluated the influence of agricultural management system and flower color on the floral visitors of this species; our objective was therefore to analyze how these factors affect the diversity and frequency of the floral visitors of P. coccineus. Methods: A study was conducted with two landraces of P. coccineus (white-flowered and red-flowered), cultivated under two different management systems. Simultaneous observations were conducted for a period of eight days during the peak of flowering, quantifying the number of floral visitors and visits. Species richness and the effective number of species were calculated, the latter was used as a measure of diversity. Chi-square tests were applied, a correspondence analysis performed, and rank abundance curves constructed. Key results: Forty-two morphospecies were recorded, grouped into six orders. Apis mellifera and Hylocharis leucotis were the predominant species. The numbers of visits recorded under traditional management were 1613 and 1105 in white and red flowers, respectively. Under the technified management, these values were 1427 and 815, respectively. There were consistent statistical differences between management systems, but not for flower color. In the white flowers, the traditional management was twice more diverse than the technified management. In the red flowers, this difference was 1.26 times. Conclusions: The diversity and frequency of floral visitors of P. coccineus are favored by the traditional agricultural management, demonstrating the desirability of promoting this type of cultivation. Under the conditions studied, flower color did not influence the species richness or demonstrate any defined tendencies on the frequency of visits.

ABSTRACT Background: To provide recommendations based on the best available scientific evidence for the multidisciplinary management of febrile neutropenia. The development included the stages: approval of the creation of the Working Group (WG); CPG search; analysis and synthesis of the evidence that led to establishing the recommendation of the selected CPGs, design of the consensus document, internal review. All phases were carried out through panel meetings using virtual platforms during 03 months. The "GRADE" system was used to establish the strength and direction of the recommendations. The recommendations cover the prevention, diagnosis and treatment of febrile neutropenia in oncohematological patients. Twenty-eight recommendations were developed. The topics approached were: correct evaluation; timely diagnosis, adequate antibiotic treatment, vaccination against influenza and pneumococcus, administration of colony-stimulating factor and special management of pediatric patients for diagnosis, treatment.

RESUMEN Introducción: Brindar recomendaciones basadas en la mejor evidencia científica disponible para el manejo multidisciplinario de la neutropenia febril. El desarrollo abarco las etapas: aprobación de la conformación del Grupo Elaborador (GE); búsqueda de GPC; análisis y síntesis de la evidencia que llevó a la formulación de las recomendaciones, diseño del documento consenso, revisión interna. Todas las fases fueron llevadas a cabo mediante reuniones de panel a través del uso de plataformas virtuales durante 03 meses. Se utilizó el sistema "GRADE" para establecer la fuerza y dirección de las recomendaciones. Se formularon 28 recomendaciones sobre prevención, diagnóstico y tratamiento de la neutropenia febril en pacientes oncohematológicos. Los temas abordados han sido relacionados a: correcta evaluación; diagnóstico oportuno, tratamiento antibiótico adecuado, vacunación contra influenza y neumococo, administración de factor estimulante de colonias y manejo especial en pacientes pediátricos para diagnóstico, tratamiento.

ABSTRACT Lung cancer is a public health problem due to its high morbidity and mortality worldwide and in Peru. In non-small cell lung cancer, the detection of mutations of the epidermal growth factor receptor (EGFR) has been useful for the choice of therapeutics for this disease. In the present article we aim to discuss current and relevant information on the best diagnostic technique for EGFR in patients with non-small cell lung cancer. The main cancer guidelines and two systematic reviews showed evidence in favor of the diagnosis of EGFR gene mutations on liquid biopsy as an alternative to tissue biopsy at the beginning of diagnosis and with a greater acceptance use, in the clinical setting of NSCLC patients with sensitive EGFR mutations. This healthcare technology may be useful in our country, and we propose its use in two clinical scenarios.

RESUMEN El cáncer pulmonar es un problema de salud pública debido a su alta morbimortalidad mundial y en el Perú. En el cáncer pulmonar de células no pequeñas (CPCNP) la detección de mutaciones del receptor del factor de crecimiento epidérmico (EGFR) ha sido útil para elección de la terapéutica de esta enfermedad. El presente artículo tiene como objetivo discutir la información actual y relevante sobre la biopsia liquida como técnica diagnóstica en detección de mutaciones del gen EGFR en pacientes con cáncer pulmonar de células no pequeñas. Las principales guías de cáncer y dos revisiones sistemáticas muestran evidencia a favor de la biopsia líquida en busca de mutaciones del gen EGFR, esto como una alternativa a la biopsia de tejido al inicio de diagnóstico y con una mayor aceptación de uso en el escenario clínico de pacientes con CPCNP con mutaciones sensibles de EGFR. Esta tecnología sanitaria puede ser útil en nuestro país, y proponemos su uso en dos escenarios clínicos.

ABSTRACT Introduction: The manuscript summarizes the process of elaboration of the Clinical Practice Guide (CPG) for the management of glycemic crises in adult patients with diabetes mellitus of the AUNA Clinic Network. A multidisciplinary team of medical assistants and methodologists carried out the development of the CPG and then there was an external review by a specialist in the field. Methods: The Elaboration Group of the CPG (GEG) concluded on 10 PICO questions. A systematic search for CPG, systematic reviews and primary studies was carried out to answer these PICO questions. To make recommendations we used the "GRADE-Adolopment" methodology and the guidelines of the national regulations. Results: Ten recommendations were made (nine strong and one weak), 18 points of good clinical practice and two flowcharts for management (one for diagnosis and the other for the treatment of glycemic crises), 04 consensus tables on management and 01 table for surveillance and monitoring. The topics covered by the recommendations for the management of glycemic crises were hyperglycemic crises (glycosylated hemoglobin evaluation; b-hydroxybutyrate evaluation; insulin, potassium, 0.9% sodium chloride, phosphorus, sodium bicarbonate treatments) and hypoglycemic crises (carbohydrate administration, monitoring, educational program to avoid reentry). Conclusion: This article summarizes the methodology and evidence-based recommendations of the CPG for the management of glycemic crisis in patients with diabetes mellitus in AUNA.

RESUMEN Introducción: El artículo resume el proceso de elaboración de la Guía de Práctica Clínica (GPC) para el manejo de las crisis glucémicas en pacientes adultos con diabetes mellitus de la Red de Clínicas AUNA. Métodos: Las preguntas PICO fueron priorizadas por el Grupo Elaborador de la GPC (GEG) luego de lo cual se concluyó en trabajar 10 preguntas PICO. Para dar respuesta a las preguntas se realizó una búsqueda sistemática de GPC, revisiones sistemáticas y estudios primarios. Se utilizó la metodología “GRADE-Adolopment” y los lineamientos de la normativa nacional para la formulación de recomendaciones. Resultados: Se formularon 10 recomendaciones (nueve fuertes y una débil), 18 puntos de buena práctica clínica, dos flujogramas para el manejo (uno para el diagnóstico y el otro para el tratamiento de crisis glucémicas), 5 tablas resumen sobre el manejo y 1 tabla para la vigilancia y seguimiento. Los temas que abarcaron las recomendaciones para el manejo de las crisis glucémicas fueron: crisis hiperglucémicas (evaluación de hemoglobina glucosilada; evaluación de b-hidroxibutirato; tratamiento con insulina, potasio, cloruro de sodio 0.9%, fósforo y bicarbonato de sodio) y crisis hipoglucémicas (administración de carbohidratos, monitoreo y programa educativo para evitar el reingreso). Conclusión: El presente artículo resume la metodología y las recomendaciones basadas en evidencia de la GPC para el manejo de la crisis glucémica en pacientes con diabetes mellitus de la Red de Clínicas AUNA.

Abstract: The motivation of sentences constitutes a guarantee of due process, which must comply with minimum standards and parameters with sufficient grounds so that there is no violation of rights. The objective of this study is to demonstrate the violation of the guarantee of motivation in the sentences of abbreviated procedure in the crimes of robbery in the Judicial Unit of Criminal Guarantees based in the canton of Santo Domingo. The research was carried out from the qualitative-quantitative approach allowing to obtain results about the violation of the motivation in more than 50% of abbreviated procedure sentences. It was concluded that it is necessary to establish continuous training workshops for judges regarding the motivation of sentences in abbreviated procedures.

Resumen: La motivación de las sentencias constituye una garantía del debido proceso, la cual debe cumplir con estándares y parámetros mínimos con fundamentos suficientes para que no exista vulneración de derechos. El presente estudio tiene como objetivo demostrar la vulneración de la garantía de motivación en las sentencias de procedimiento abreviado en los delitos de robo en la Unidad Judicial de Garantías Penales con sede en el cantón Santo Domingo. La investigación se realizó a partir del enfoque cuali-cuantitativo permitiendo obtener resultados en torno a la vulneración de la motivación en más del 50% de sentencias de procedimiento abreviado. Concluyendo que es menester establecer talleres de capacitación continua a los jueces referente a la motivación de sentencias en los procedimientos abreviados.
