Abstract Background: This study aimed to investigate the analgesic impact of S(+)-ketamine on pain behavior and synovial inflammation in an osteoarthritis (OA) model. Methods: Animals were grouped as follows: OA-Saline (n = 24) and OA-Ketamine (n = 24), OA induced via intra-articular sodium monoiodoacetate (MIA); a Non-OA group (n = 24) served as the control. On the 7th day post OA induction, animals received either saline or S(+)-ketamine (0.5 mg.kg–1). Behavioral and histopathological assessments were conducted up to day 28. Results: S(+)-ketamine reduced allodynia from day 7 to 28 and hyperalgesia from day 10 to 28. It notably alleviated weight distribution deficits from day 10 until the end of the study. Significant walking improvement was observed on day 14 in S(+)-ketamine-treated rats. Starting on day 14, OA groups showed grip force decline, which was countered by S(+)-ketamine on day 21. However, S(+)-ketamine did not diminish synovial inflammation. Conclusion: Low Intra-articular (IA) doses of S(+)-ketamine reduced MIA-induced OA pain but did not reverse synovial histopathological changes. IRB approval number: 23115 012030/2009-05. Abstract Background: Renal replacement therapy (RRT) is the object of constant analysis in the search for efficiency and sustainability. Objective: To calculate the direct cost of healthcare for the prevalent RRT population in the province of Toledo (2012/2013). Method: Population: All prevalent patients at some point in RRT in 2012 (669) and in 2013 (682). Costs included (€): 1) dialysis procedure; 2) inpatient, outpatient and emergency care, dialysis and non-dialysis related; 3) drug consumption; 4) medical transport. Calculation and analysis: The aggregate localized or reconstructed cost of each item was calculated from the individual cost of each patient. Annual cost and cost per patient/year was calculated for the whole RRT and for its subprograms (€). Results: Aggregate costs: The total cost of RRT amounted to 15.84 and 15.77 million euros (2012/2013). Dialysis procedures account for 40.2% of the total while the sum of hospital care and drug consumption represents 41.5%. Healthcare for patients on hospital haemodialysis (HHD) and combined haemodialysis (CHD), peritoneal dialysis (PD) and transplant (Tx) accounts for 70.0, 5.0 and 25.0% of the total respectively. Patient/year cost: From the number of patients/year provided by each subprogramme, the following values were obtained in 2012/2013: All RRT 26,130/25,379; HHD 49,167/53,289; CHD 44,657/44,971; PD 45,538/51,869 and Tx 10,909/10,984. Conclusions: Our results are consistent with others published, although our patient/year values are slightly higher, probably because they include elements such as outpatient pharmacy, hospital and medical transport cargo. The growing contribution of Tx to the survival of the whole RRT population contains the overall costs and reduces the patient/year cost, making RRT sustainable.

Resumen Introducción: El coste ocasionado por los programas de tratamiento renal sustitutivo (TRS) es objeto de análisis permanente en busca de su eficiencia y sostenibilidad. Objetivo: Calcular el coste directo de la atención a la población prevalente en TRS en Toledo en los años 2012 y 2013. Método: Población: Todos los pacientes prevalentes en algún momento en TRS en 2012 (669) y en 2013 (682). Componentes del coste (€): 1) procedimiento de diálisis; 2) atención hospitalaria: ingresos, consultas, procedimientos ambulatorios y urgencias; 3) consumo de fármacos; 4) transporte. Cálculo y análisis: para cada uno de esos componentes se calculó el coste agregado localizado o reconstruido a partir del coste individual de cada paciente. Se calculó el coste anual y el coste paciente/año del TRS y de cada uno de sus subprogramas (€). Resultados: Costes agregados: el coste anual fue de 15,84 (2012) y de 15,77 millones de euros (2013). Los procedimientos de diálisis representan el 40,2% y la atención hospitalaria más el consumo de fármacos, el 41,5%. La atención a los pacientes en hemodiálisis hospitalaria (HDH) y concertada (HDC), diálisis peritoneal (DP) y trasplantados (Tx) representan, respectivamente, el 70,0; el 5,0 y el 25,0% del total. Coste paciente/año: considerando el número de pacientes/año proporcionado por cada subprograma, se obtuvieron los siguientes valores en 2012/2013: para todo TRS 26.130/25.379; HDH 49.167/53.289; HDC 44.657/44.971; DP 45.538/51.869 y Tx 10.909/10.984. Conclusiones: Nuestros resultados son consistentes con otros publicados, aunque arrojan valores paciente/año ligeramente superiores, debido a que incluyen elementos como farmacia extrahospitalaria, carga hospitalaria y transporte sanitario. La contribución creciente del Tx a la sobrevida del conjunto de la población en TRS contiene los costes globales y reduce el coste paciente/año, lo que hace sostenible el TRS.

anesthesia and rotating instruments. In order to solve this problem, the chemical-mechanical method for the elimination of the carious tissue, is a conservative and effective alternative since it avoids the use of anesthetics and rotary instruments. According to studies, this method is more accepted by patients compared to the traditional method. This paper aims to present a clinical case of the use of papain gel (Papacárie ®) for the chemical-mechanical removal of carious tissue in permanent teeth.

En odontopediatría, una de las principales dificultades que se presenta en la remoción del tejido dental cariado, es el manejo del paciente niño debido al miedo y a la ansiedad por el uso de anestesia e instrumentos rotatorios. A fin de resolver este problema, el método químico-mecánico para la eliminación del tejido cariado, es una alternativa conservadora y eficaz ya que evita el uso de anestésicos e instrumental rotatorio. Según estudios, este método es más aceptado por los pacientes comparado con el método tradicional. Este artículo tiene como objetivo presentar un caso clínico del uso del gel a base de papaína (Papacárie ®) para la eliminación químico-mecánica del tejido cariado en dientes permanentes.

ABSTRACT Abstract: Overactive bladder syndrome is one of the lower urinary tract dysfunctions with the highest number of scientific publications over the past two decades. This shows the growing interest in better understanding this syndrome, which gathers symptoms of urinary urgency and increased daytime and nighttime voiding frequency, with or without urinary incontinence and results in a negative impact on the quality of life of approximately one out of six individuals – including both genders and almost all age groups. The possibility of establishing the diagnosis just from clinical data made patients' access to specialized care easier. Physiotherapy resources have been incorporated into the urological daily practice. A number of more selective antimuscarinic drugs with consequent lower adverse event rates were released. Recently, a new class of oral drugs, beta-adrenergic agonists has become part of the armamentarium for Overactive Bladder. Botulinum toxin injections in the bladder and sacral neuromodulation are routine modalities of treatment for refractory cases. During the 1st Latin-American Consultation on Overactive Bladder, a comprehensive review of the literature related to the evolution of the concept, epidemiology, diagnosis, and management was conducted. This text corresponds to the first part of the review Overactive Bladder 18-years.

ABSTRACT Traditionally, the treatment of overactive bladder syndrome has been based on the use of oral medications with the purpose of reestablishing the detrusor stability. The recent better understanding of the urothelial physiology fostered conceptual changes, and the oral anticholinergics – pillars of the overactive bladder pharmacotherapy – started to be not only recognized for their properties of inhibiting the detrusor contractile activity, but also their action on the bladder afference, and therefore, on the reduction of the symptoms that constitute the syndrome. Beta-adrenergic agonists, which were recently added to the list of drugs for the treatment of overactive bladder, still wait for a definitive positioning – as either a second-line therapy or an adjuvant to oral anticholinergics. Conservative treatment failure, whether due to unsatisfactory results or the presence of adverse side effects, define it as refractory overactive bladder. In this context, the intravesical injection of botulinum toxin type A emerged as an effective option for the existing gap between the primary measures and more complex procedures such as bladder augmentation. Sacral neuromodulation, described three decades ago, had its indication reinforced in this overactive bladder era. Likewise, the electric stimulation of the tibial nerve is now a minimally invasive alternative to treat those with refractory overactive bladder. The results of the systematic literature review on the oral pharmacological treatment and the treatment of refractory overactive bladder gave rise to this second part of the review article Overactive Bladder – 18 years, prepared during the 1st Latin-American Consultation on Overactive Bladder.

Da prescrição até a administração de medicamentos em pacientes podem ocorrer erros que causam dor, sofrimento e, em casos extremos, a morte. Assim, o objetivo geral deste artigo foi identificar os problemas relacionados aos utensílios da sala de preparo de medicação de um hospital da Grande Florianópolis no que tange à ergonomia física, cognitiva e organizacional, a fim de propor possíveis soluções no preparo de medicação para a eliminação ou minimização de erros no fluxo de medicação. Como método, fez-se: pesquisa bibliográfica, visita a campo e avaliação dos utensílios quanto à ergonomia física, cognitiva e organizacional. Como resultados, elaborou-se um quadro de análise de alguns utensílios da sala de medicação do hospital escola estudado, os quais foram relacionados com possíveis soluções para a otimização do processo de medicação. Assim, espera-se contribuir para a melhoria do trabalho dos profissionais da saúde e, consequentemente, promover uma melhor prestação de serviços à população.

From prescription to medication administration, errors that cause pain, suffering and, in extreme cases, death may occur in patients. Thus, the aim of this paper was to identify the problems related to the utensils used in the medication room of a hospital in Florianópolis, in respect to physical, cognitive and organizational ergonomics, in order to propose possible solutions to prepare medications for disposal or minimization of errors in the stream of medication. As a method, they were used: literature review, field visit and assessment tools as physical, cognitive and organizational ergonomics. As a results, it is presented a framework analysing some utensils of medication room, which has been elaborated and related with possible solutions to optimization of the medication process. Thus, it is expected to contribute to improving the work of health professionals and promote better service to the population.

Este estudo in vitro avaliou a influência de dois dispositivos para aplicação de força de cisalhamento em testes de resistência de união ao microcisalhamento e no padrão de fratura em dentina e esmalte de dentes decíduos. Oitenta molares decíduos foram selecionados e superficies planas em esmalte (40 dentes seccionados no sentido mésio-distal) e dentina (40 dentes seccionados transversalmente) foram obtidas. Ambas as superficies foram abrasionadas para padronização da lama dentinária. Dois sistemas adesivos de condicionamento ácido prévio (Adper Single Bond e XP Bond) foram utilizados. Tubos de polietileno foram colocados sobre as superficies, nas quais já haviam sido aplicados os sistemas adesivos, e preenchidos com resina composta. O teste de resistência de união ao microcisalhamento foi realizado após armazenamento dos espécimes em água (24 h/37 °C) utilizando dois dispositivos para a aplicação das forças de cisalhamento: cisalha côncava (Bisco Shear Bond Tester) e cisalha plana (Kratos Industrial Equipment). O padrão de fratura foi avaliado usando estereomicroscópio. Os dados de resistência de união foram submetidos à ANOVA e o teste quiquadrado foi utilizado para comparar a distribuição dos padrões de fratura (α=0,05). Não foram observadas diferenças estatisticamente significantes entre os grupos, tanto para a resistência de união a dentina e esmalte quanto para o padrão de fratura (p>0,05). O modo de falha predominante foi adesiva/mista. Em conclusão, os dispositivos para aplicação de força de cisalhamento não influenciam os valores de resistência de união, independente do sistema adesivo e substrato.

This in vitro study evaluated the influence of two devices for application of shear load in microshear tests on bond strength and fracture pattern of primary enamel and dentin. Eighty primary molars were selected and flat enamel (40 teeth sectioned mesio-distally) and dentin (40 teeth sectioned transversally) surfaces were obtained. Both surfaces were polished to standardize the smear layer. Two-step etch-and-rinse adhesive systems (Adper Single Bond and XP Bond) were used. Polyethylene tubes was placed over the bonded surfaces and filled with composite resin. The microshear testing was performed after storage in water (24 h/37 °C) using two devices for application of microshear loads: a notched rod (Bisco Shear Bond Tester) or a knife edge (Kratos Industrial Equipment). Failure modes were evaluated using a stereomicroscope. Bond strength data were subjected to ANOVA and chi-square test to compare the failure mode distributions (α=0.05). No significant differences were observed between the groups for dentin and enamel bond strength or fracture patterns (p>0.05). The predominant failure mode was adhesive/mixed. In conclusion, the devices for application of shear loads did not influence the bond strength values, regardless of adhesive system and substrate.

Objective: To evaluate the effect of application time on the resin-dentin bond strength (µTBS) and etching pattern of adhesive systems applied on sclerotic dentine. Material and Methods: A total of forty-two bovine incisors had their roots removed. The 1-step self-etch GO (SDI), the 2-step self-etch Adper SE Bond (3MESPE) and the 35% phosphoric acid (3MESPE) from the 2-step etch-and-rinse Adper Single Bond 2 (3MESPE) were applied on the bovine incisal surfaces according to the manufacturer's instructions or duplicating the recommended conditioning time. After adhesive application, thirty teeth were restored with composite resin, stored for 24 h in distilled water at 37°C, and sectioned into resin-dentin bonded sticks (0.8 mm 2 ) and tested according to the µTBS at 0.5 mm/min. The etching pattern of the remaining twelve teeth (n=4 for each material) was examined under scanning electron microscopy. Each tooth was divided into a buccal-to-lingual direction into three thirds, and each third randomly assigned to the groups: control (no treatment), according to the manufacturers' instructions and duplicating the recommended application time. The µTBS and the relative percentage of the tubule area opening were evaluated by two-way repeated measures ANOVA and Tukey's tests (α=0.05). Results: The duplication of the conditioning time favored only the GO adhesive (p<0.05). Both application methods significantly increased the tubule area opening (p<0.05) compared to the controls. Conclusions: The efficacy of duplicating the conditioning time was only effective for the 1-step self-etch adhesive system tested.

Objectives This study aimed to compare the micro-tensile bond strength of methacrylate resin systems to a silorane-based restorative system on dentin after 24 hours and six months water storage. Material and Methods The restorative systems Adper Single Bond 2/Filtek Z350 (ASB), Clearfil SE Bond/Z350 (CF), Adper SE Plus/Z350 (ASEP) and P90 Adhesive System/Filtek P90 (P90) were applied on flat dentin surfaces of 20 third molars (n=5). The restored teeth were sectioned perpendicularly to the bonding interface to obtain sticks (0.8 mm2) to be tested after 24 hours (24 h) and 6 months (6 m) of water storage, in a universal testing machine at 0.5 mm/min. The data was analyzed via two-way Analysis of Variance/Bonferroni post hoc tests at 5% global significance. Results Overall outcomes did not indicate a statistical difference for the resin systems (p=0.26) nor time (p=0.62). No interaction between material × time was detected (p=0.28). Mean standard-deviation in MPa at 24 h and 6 m were: ASB 31.38 (4.53) and 30.06 (1.95), CF 34.26 (3.47) and 32.75 (4.18), ASEP 29.54 (4.14) and 33.47 (2.47), P90 30.27 (2.03) and 31.34 (2.19). Conclusions The silorane-based system showed a similar performance to methacrylate-based materials on dentin. All systems were stable in terms of bond strength up to 6 month of water storage.

O objetivo deste estudo foi avaliar, por meio do DPPH, a atividade antioxidante de substâncias que poderiam ser propostas para reverter de imediato os problemas causados pelos procedimentos de clareamento. A porcentagem de atividade antioxidante (AA%) da solução de acido ascórbico 10% (AAcidS), gel de ácido ascórbico a 10% (AAcidG), solução de ascorbato de sódio 10% (SodAsS), gel de ascorbato de sódio 10% (SodAsG), bicarbonato de sódio 10% (Bicarb), NeutralizeÂ® (NE), DesensibilizeÂ® (DES), catalase C-40 10 mg/mL (CAT), solução alcoólica 10% de alfa-tocoferol (VitE), ListerineÂ® (LIS), clorexidina 0,12% (CHX), CrotonLechleri (CL), solução aquosa 10% de Uncaria Tomentosa (UT), saliva artificial (ArtS) e fluoreto de sódio 0,05% (NaF) foi avaliada em triplicata pelo teste de radicais livres 2,2-diphenyl-1-picryl-hydrazyl-hydrate (DPPH). Todas as substâncias apresentaram atividade antioxidante, exceto a CL. AAcidS, AAcidG e VitE mostraram os maiores valores de AA% (p<0,05). Por outro lado, CHX, NE, LIS e NaF mostraram os valores mais baixos de AA% (p<0,05). Em conclusão, AAcidS, AAcidG, SodAsS, SodAsG e VitE apresentaram os mais altos valores de atividade antioxidante entre as substâncias testadas. O teste DPPH é um método rápido e fácil para avaliar o potencial antioxidante.

The aim of this study was to assess, using the DPPH assay, the antioxidant activity of several substances that could be proposed to immediately revert the problems caused by bleaching procedures. The percentage of antioxidant activity (AA%) of 10% ascorbic acid solution (AAcidS), 10% ascorbic acid gel (AAcidG), 10% sodium ascorbate solution (SodAsS), 10% sodium ascorbate gel (SodAsG), 10% sodium bicarbonate (Bicarb), NeutralizeÂ® (NE), DesensibilizeÂ® (DES), catalase C-40 at 10 mg/mL (CAT), 10% alcohol solution of alpha-tocopherol (VitE), ListerineÂ® (LIS), 0.12% chlorhexidine (CHX), Croton Lechleri (CL), 10 % aqueous solution of Uncaria Tomentosa (UT), artificial saliva (ArtS) and 0.05% sodium fluoride (NaF) was assessed in triplicate by 2,2-diphenyl-1-picryl-hydrazyl-hydrate (DPPH) free radical assay. All substances exhibited antioxidant activity, except for CL. AAcidS, AAcidG and VitE exhibited the highest AA% (p<0.05). On the contrary, CHX, NE, LIS and NaF showed the lowest AA% (p<0.05). In conclusion, AAcidS, AAcidG, SodAsS, SodAsG and VitE presented the highest antioxidant activity among substances tested in this study. The DPPH assay provides an easy and rapid way to evaluate potential antioxidants.

OBJECTIVES: The aims of this study were to evaluate the effect of resin composite (Filtek Z250 and Filtek Flow Z350) and adhesive system [(Solobond Plus, Futurabond NR (VOCO) and Adper Single Bond (3M ESPE)] on the microtensile (μTBS) and microshear bond strength (μSBS) tests on enamel, and to correlate the bond strength means between them. MATERIAL AND METHODS: Thirty-six extracted human molars were sectioned to obtain two tooth halves: one for μTBS and the other one for μSBS. Adhesive systems and resin composites were applied to the enamel ground surfaces and light-cured. After storage (37(0)C/24 h) specimens were stressed (0.5 mm/min). Fracture modes were analyzed under scanning electron microscopy. The data were analyzed using two-way ANOVA and Tukey's test (α=0.05). RESULTS: The correlation between tests was estimated with Pearson's product-moment correlation statistics (α =0.05). For both tests only the main factor resin composite was statistically significant (p<0.05). The correlation test detected a positive (r=0.91) and significant (p=0.01) correlation between the tests. CONCLUSIONS: The results were more influenced by the resin type than by the adhesives. Both microbond tests seem to be positive and linearly correlated and can therefore lead to similar conclusions.

Host blood source was found to affect both the development and the reproductive performance of Rhodnius prolixus. The insects were reared on citrated human, rabbit, chicken, sheep and horse blood sources, through a membrane feeder, during an entire life cycle, from eggs to adults. Development and reproduction in terms of the number of unfed insects, number of moulting, mortality intermoulting period, number of egg/female, conversion of blood into egg (mg meal/egg) and percentage of hatch as effective physiological parameters were investigated. Our results showed that human or rabbit blood meals were more nutritionally efficient than the other blood samples used because (i) the insects developed faster, presented low mortality and about 80% of them reached the adult stage; and (ii) females oviposited an average of at least 100% more eggs. The inefficiency of chicken and horse blood sources as diets for R. prolixus was manifested in (i) a decrease of the amount of ingested blood and (ii) only a reasonable nutritional quality. The inadequacy of sheep blood was observed by a mortality extremely high, poor moulting response and drastic reduction in egg production.

Azadirachtin A was given through a blood meal to 4th-instar larvae and to adult females of Rhodnius prolixus. Development (ecdysis) and egg production were inhibited in a dose-dependent manner. Long-term experiments with subsequent four feedings on azadirachtin-free blood were performed with 4th-instar larvae and with adult females. Only in the low-dose azadirachtin larval groups (0.01 and 0.1 microng/ml of blood), development was partially restored; after a single 1.0 microng/ml treatment about 50% of the treated larvae were still alive 120 days later without any adult emergence. Similarly fed females had a dose-dependent lower survival and egg deposition rate. The results are discussed in relation to the mode of azadirachtin A action.

The results presented in this paper clearly indicate that precocene and azadirachtin are effective inhibitors of moulting and reproduction in the hemipteran Rhodnius prolixus. The time of application is important and only applications of these substances early in the intermoulting period cause their effects in nymphs. The inhibition of moulting is fully reversed by ecdysone therapy. Precocene and azadirachtin also affected drastically the oogenesis and egg deposition in this insect. Precocene-induced sterilization is reversed by application of juvenile hormone III. However, this hormone is unable to reverse the effect of azadirachtin on reproduction. Ecdysteroid titers in nymphs and adult females are decreased by these treatments. In vitro analysis suggest that precocene and azadirachtin may act directly on the prothoracic glands and ovaries producing ecdysteroids. Based on these and other findings the possible mode of action of these compounds on the development and reproduction of Rhodnius prolixus is discussed.
