Twenty years of real-life experience has demonstrated that hydroxyurea reduces pain attacks, vaso-occlusive events, including acute chest syndrome, the number and duration of hospitalizations and the need for transfusion. The therapeutic success of hydroxyurea is directly linked to access to the drug, the dose used and adherence to treatment which, in part, is correlated to the availability of hydroxyurea. Abstract Background: Real-world, primary data on the treatment of psoriasis are scarce, especially concerning the role of soluble biomarkers as outcome predictors. Objective: The authors evaluated the utility of Th1/Th17 serum cytokines along with clinical characteristics as predictors of drug survival in the treatment of psoriasis. Methods: The authors consecutively included participants with moderate to severe psoriasis who were followed up for 6 years. Baseline interferon-α, tumor necrosis factor-α, and inter-leukin (IL)-2, IL-4, IL-6, IL-10, and IL-17A were measured using a cytometric bead array; clinical data were assessed. The authors calculated hazard ratios (HRs) for drug survival using a Cox proportional hazards model. Results: The authors included 262 patients, most of whom used systemic immunosuppressants or biologics. In the multivariate model, poor quality of life measured by the Dermatology Life Quality Index (HR = 1.04; 95% CI 1.01–1.07; p = 0.012) and elevated baseline IL-6 (HR = 1.99; 95% CI 1.29–3.08; p = 0.002) were associated with treatment interruption. Study limitations: The main limitation of any cohort study is the presence of confounders that could not be detected in clinical evaluation. Conclusions: Poor quality of life and elevated baseline serum IL-6 level predicted treatment interruption in patients with moderate to severe psoriasis. ABSTRACT Introduction: Heart transplantation is the gold standard for advanced heart failure treatment. This study examines the survival rates and risk factors for early mortality in adult heart transplant recipients at a Brazilian center. Methods: This retrospective cohort study involved 255 adult heart transplant patients from a single center in Brazil. Data were collected from medical records and databases including three defined periods (2012-2015, 2016-2019, and 2020-2022). Statistical analysis employed Kaplan-Meier survival curves, Cox proportional hazards analysis for 30-day mortality risk factors, and Log-rank tests. Results: The recipients were mostly male (74.9%), and the mean age was 46.6 years. Main causes of heart failure were idiopathic dilated cardiomyopathy (33.9%), Chagas cardiomyopathy (18%), and ischemic cardiomyopathy (14.3%). The study revealed an overall survival of 68.1% at one year, 58% at five years, and 40.8% at 10 years after heart transplantation. Survivalimproved significantly over time, combining the most recent periods (2016 to 2022) it was 73.2% in the first year and 63% in five years. The main risk factors for 30-day mortality were longer time on cardiopulmonary bypass, the initial period of transplants (2012 to 2015), older age of the donor, and nutritional status of the donor (overweight or obese). The main causes of death within 30 days post-transplant were infection and primary graft dysfunction. Conclusion: The survival analysis by period demonstrated that the increased surgical volume, coupled with the team’s experience and modifications to the immunosuppression protocol, contributed to the improved early and mid-term outcomes. RESUMO: Originária da América Tropical, a goiabeira (Psidium guajava L.) tem grande relevância para o Brasil. Contudo, o surgimento do patossistema designado como declínio da goiabeira, problema fitossanitário provocado pelo parasitismo das raízes pelo nematoide-das-galhas (Meloidogyne enterolobii Yang & Eisenback) em associação com fungos oportunistas, dizimou muitos pomares em todas as regiões do Brasil e em outros países. No presente estudo, propagou-se vegetativamente, por miniestaquia, acessos de goiabeiras seminíferas mantendo seus genótipos preservados e reavaliando a resistência por meio dos clones de forma a comprovar ou não as reações das plantas hospedeiras. ABSTRACT: Native to the tropical Americas, guava (Psidium guajava L.) is an important crop in Brazil. However, the emergence of so-called guava decline, a complex disease resulting from root parasitism by the root-knot nematode (Meloidogyne enterolobii Yang & Eisenback) in association with opportunistic fungi, has decimated guava orchards across Brazil and in other countries. In the present study, seminiferous guava accessions were vegetatively propagated by minigrafting and their genotypes preserved for resistance reassessment in clones to confirm or not host plant reactions. The results indicated a highly virulent parasite, high host suitability of the P. guajava species, and widely varying reactions among plants of the same genotype and between different genotypes, demonstrating that the strategy of preserving the germplasm and reassessing reactions in clones may be important in identifying and selecting germplasms with a degree of resistance to M. enterolobii. The progeny of cv. RESUMEN Objetivos: identificar factores asociados al conocimiento, actitud y práctica de los ancianos en relación a la prevención del cáncer de piel. Métodos: estudio transversal, realizado con 120 ancianos de una Unidad Básica de Salud de Quixadá, Ceará, de septiembre a noviembre de 2018. Resultados: los individuos de 60 a 69 años y que trabajaban se asociaron estadísticamente con conocimientos adecuados (p=0,038). Tener piel, ojos y cabello claros se asoció con una actitud adecuada (p=0,030). Tener problemas en la piel, como heridas sangrantes, se asoció con una práctica adecuada (p=0,016). RESUMO Objetivos: identificar fatores associados a conhecimento, atitude e prática de idosos sobre a prevenção do câncer de pele. Métodos: estudo transversal, realizado com 120 idosos de uma Unidade Básica de Saúde em Quixadá, Ceará, de setembro a novembro de 2018. Resultados: indivíduos na faixa etária de 60 a 69 anos e trabalhando estiveram associados estatisticamente ao conhecimento adequado (p=0,038). O fato de ter pele, olhos e cabelos claros estava associado à atitude adequada (p=0,030). Ter problemas de pele, do tipo ferida sangrante, estava associado à prática adequada (p=0,016). Resumen La distancia social causada por la pandemia de COVID-19 condujo a cambios profundos en la rutina de las familias con niños pequeños, aumentando el estrés en el entorno del hogar. Este estudio analizó la experiencia de planificar e implementar un proyecto de extensión universitaria que ofreció orientación a los padres con niños de cero a 11 años a través de llamadas de audio durante la pandemia COVID-19. El protocolo de atención se desarrolló para satisfacer las necesidades de las familias de bajos ingresos y enumeró problemas específicos relacionados con el confinamiento en el hogar y el cierre de la escuela, seguido de una variedad de estrategias de afrontamiento. El análisis de 223 quejas informadas por los usuarios en 130 llamadas reveló que el 94% de los problemas informados por los padres fueron abordados por el protocolo de atención y estaban relacionados con los problemas de externalización (39%) o internalización (26%) de los niños o la disminución del bienestar subjetivo de los padres (29%). Los servicios de apoyo deberían aconsejar a los padres sobre el uso de prácticas receptivas y asertivas que promuevan el bienestar emocional del niño y establezcan expectativas de comportamiento en contextos estresantes. Resumo O distanciamento social ocasionado pela pandemia de Covid-19 levou a profundas mudanças na rotina das famílias com crianças pequenas, aumentando o estresse no ambiente doméstico. Este estudo analisou a experiência de planejamento e implementação de um projeto de extensão universitária que ofereceu orientação a pais com filhos de 0 a 11 anos por meio de chamadas de áudio durante a pandemia. O protocolo de atendimento foi desenvolvido para atender às necessidades de famílias de baixa renda e listava problemas específicos relacionados ao confinamento em casa e ao fechamento das escolas seguidos por uma variedade de estratégias de enfrentamento. A análise de 223 queixas relatadas pelos usuários em 130 ligações revelou que 94% dos problemas referidos pelos pais foram contemplados pelo protocolo de atendimento e estavam relacionados aos problemas externalizantes (39%) ou internalizantes (26%) das crianças ou ao declínio do bem-estar subjetivo dos pais (29%). Abstract The social distancing the COVID-19 pandemic entailed has led to profound changes in the routine of families with young children, increasing stress in the home environment. This study analyzed the experience of planning and implementing a university extension program that offered support to parents with children from 0 to 11 years old via audio calls during the COVID-19 pandemic. The service protocol was developed to meet the needs of low-income families and listed specific problems related to home confinement and school closure followed by a variety of coping strategies. The analysis of 223 complaints reported by users in 130 calls revealed that 94% of the problems reported by parents were addressed by the protocol and were related to children’s externalizing (39%) or internalizing (26%) problems or to the decline in parents’ subjective well-being (29%). Support services should guide parents on the use of responsive and assertive practices that promote the child’s emotional well-being and set behavioral expectations in stressful contexts. The reduction in conflicts between parents and children resulting from the use of these strategies tends to reduce parents’ suffering, increasing their sense of subjective well-being. ABSTRACT Objective: To assess the efficacy and safety of the use of midazolam as monotherapy, compared to the associated use of midazolam and hydroxyzine for minimum and moderate sedation of children in dental offices, using data obtained from clinical trials. Material and Methods: A systematic review protocol was developed and registered on PROSPERO (CR42020208633). An electronic search was carried out in Pubmed, Lilacs, Science Direct, Open Gray, Web of Science, and central Cochrane Library. No language restrictions were included. Clinical trials were carried out with children aged 0-12 years, using midazolam as monotherapy compared to the use of midazolam associated with hydroxyzine to verify the effectiveness and safety of oral sedation. The quality of the studies was individually assessed and grouped using the RoB 2 (Revised Cochrane risk-of-bias tool for randomized trials) and GRADE (Grading of Recommendations Assessment, Development and Evaluation) systems, respectively. Results: A total of 749 studies were found. After analyzing the inclusion and removal of duplicates, two studies were analyzed for the quality of evidence. Through this analysis, it was possible to verify the very low level of scientific evidence on the superiority of the efficacy and safety of the combined use of midazolam and hydroxyzine for oral sedation in children in dental offices. Conclusion: The conflicting results and limitations of the studies enabled to establish that there is insufficient evidence to support the use of these drugs combined. Abstract Limited experiential contact to natural environment can affect the knowledge of the local flora and, consequently, the intentions to conserve it. Therefore, this study aims to analyze whether high school students from Teresina (PI-Brazil) can identify more species of native than exotic plants and the factors that can affect this ability. 333 students from state public schools were interviewed through semi-structured questionnaires and with printed images of 20 species of plants found in the region. The data were analyzed using univariate non-parametric statistics. The results show that living in the countryside positively influenced the identification of local flora, but there was no significant difference in the number of identified plants between students who attended botany classes and those who did not. Two native plants (cashew-96.7% and pitomba-91.9%) and three exotic plants (mango-90.7%, guava-94.9% and acerola-82.0%) stood out among the most identified plant species. It was also found that, for two species of native plants very important for the State of Piauí, the caneleiro (0.6%) and the carnauba (26.7%), the identification rates were scarce. Platycyamus regnellii is a Brazilian tree used in landscaping, degraded area recovery, and its wood for furniture. It is evaluated the ecological indices and interactions between the groups of arthropods on P. regnellii leaf surface. Highest numbers of phytophagous arthropods Orthoptera Tettigoniidae and Tropidacris collaris (Romaleidae), and Hemiptera Pentatomidae; natural enemies Hymenoptera Camponotus sp. and Pseudomyrmex termitarius (Formicidae), and Diptera Dolichopodidae; and abundance and species richness of chewing insects and protocooperating ants were observed on the adaxial leaf face of P. regnellii plants. Abundance of spiders correlated, positively, with that of chewing insects and number of spiders with that of T. collaris. The knowledge of the leaf face, preferred by herbivorous insects, helps in the correct application of pesticides in integrated pest management programs.

Se evalúan los índices e interacciones ecológicos entre los grupos de artrópodos en la superficie foliar de P. regnellii. Mayor número de artrópodos fitófagos Orthoptera Tettigoniidae y Tropidacris collaris (Romaleidae), y Hemiptera Pentatomidae; enemigos naturales Hymenoptera Camponotus sp. y Pseudomyrmex termitarius (Formicidae) y Diptera Dolichopodidae; y se observó abundancia y riqueza de especies de insectos masticadores y hormigas protocooperadoras en la cara adaxial de la hoja de plantas de P. regnellii. La abundancia de arañas se correlacionó positivamente con la de insectos masticadores y el número de arañas con la de T. collaris. El conocimiento de la cara foliar, preferida por los insectos herbívoros, ayuda en la correcta aplicación de pesticidas en programas de manejo integrado de plagas.

Key points Pregnancy places a metabolic overload on the maternal thyroid, especially in the first trimester, mainly because of the demand imposed by the conceptus. The fetal thyroid becomes functionally mature only around pregnancy week 20. Until then, the fetus depends on the transfer of maternal thyroid hormones (THs). Thyroid hormones are essential for the adequate fetal neurofunctional and cognitive development. Hypothyroidism brings higher risks of obstetric and fetal complications, namely, first-trimester miscarriage, preeclampsia and gestational hypertension, placental abruption, prematurity, low birth weight, and higher perinatal morbidity and mortality. Primary hypothyroidism (involvement of the gland with difficulty in producing and/or releasing TH) is the most common form of disease presentation, with the main etiology of Hashimoto's thyroiditis of autoimmune origin. In about 85%-90% of cases of Hashimoto's thyroiditis, antithyroid antibodies are present; the antithyroperoxidase (ATPO) is the most frequent. Positivity for ATPO is determined when circulating values exceed the upper limit of the laboratory reference. It implies greater risks of adverse maternal-fetal outcomes. Such a correlation occurs even in ranges of maternal euthyroidism. The critical point for the diagnosis of hypothyroidism during pregnancy is an elevation of thyroid-stimulating hormone (TSH). The measurement of free thyroxine (FT4) differentiates between subclinical and overt hypothyroidism. In subclinical hypothyroidism, FT4 is within the normal range, whereas in overt hypothyroidism, FT4 values are below the lower limit of the laboratory reference. Treatment of hypothyroidism is performed with levothyroxine (LT4) replacement with the aim of achieving adequate TSH levels for pregnancy. Some women have a previous diagnosis of hypothyroidism, and may or may not be compensated at the beginning of pregnancy. Even in compensated cases, the increase in LT4 dose is necessary as soon as possible. In the postpartum period, adjustment of the LT4 dose depends on the condition of previous disease, on the positivity for ATPO, and also on the value of LT4 in use at the end of pregnancy. Recommendations In places with full technical and financial conditions, TSH testing should be performed for all pregnant women (universal screening) as early as possible, ideally at the beginning of the first trimester or even in preconception planning. In places with less access to laboratory tests, screening is reserved for cases with greater risk factors for decompensation, namely: previous thyroidectomy or radioiodine therapy, type 1 diabetes mellitus or other autoimmune diseases, presence of goiter, previous history of hypo or hyperthyroidism or previous ATPO positivity. The TSH dosage should be repeated throughout pregnancy only in these cases. The diagnosis of hypothyroidism is made from the TSH value > 4.0 mIU/L. Pregnant women with previous hypothyroidism, overt hypothyroidism diagnosed during pregnancy or those with the above-mentioned higher risk factors for decompensation should be referred for risk antenatal care, preferably in conjunction with the endocrinologist. Overt hypothyroidism in pregnancy is identified when TSH > 10 mIU/L, and treatment with LT4 is readily recommended at an initial dose of 2 mcg/kg/day. TSH values > 4.0 mUI/L and ≤ 10.0 mUI/L require FT4 measurement with two diagnostic possibilities: overt hypothyroidism when FT4 levels are below the lower limit of the laboratory reference, or subclinical hypothyroidism when FT4 levels are normal. The treatment for subclinical hypothyroidism is LT4 at an initial dose of 1 mcg/kg/day, and the dose should be doubled upon diagnosis of overt hypothyroidism. In cases of TSH > 2.5 and ≤ 4.0 mIU/L, if there are complete conditions, ATPO should be measured. If positive (above the upper limit of normal), treatment with LT4 at a dose of 50 mcg/day is indicated. If conditions are not complete, the repetition of the TSH dosage should be done only for cases at higher risk. In these cases, treatment with LT4 will be established when TSH > 4.0 mIU/L at a dose of 1 mcg/kg/day; if needed, the dose can be adjusted after FT4 evaluation. Women with previous hypothyroidism should have their LT4 dose adjusted to achieve TSH < 2.5 mIU/L at preconception. As soon as they become pregnant, they need a 30% increase in LT4 as early as possible. In practice, they should double the usual dose on two days a week. Levothyroxine should be given 30-60 minutes before breakfast or three hours or more after the last meal. Concomitant intake with ferrous sulfate, calcium carbonate, aluminum hydroxide and sucralfate should be avoided. The target of LT4 therapy during pregnancy is to achieve a TSH value < 2.5 mIU/L. Once the therapy is started, monthly control must be performed until the mentioned goal is reached. In the postpartum period, women with previous disease should resume the preconception dose. Resumo Objetivos: avaliar a mortalidade materna de janeiro de 2019 a janeiro de 2020 no estado do Ceará. Métodos: trata-se de um estudo ecológico descritivo e retrospectivo. A coleta de dados foi realizada no período outubro a novembro de 2020, considerando o intervalo temporal de janeiro de 2019 a janeiro de 2020, através das informações registradas em banco de dados de domínio público do estado do Ceará. As variáveis consideradas para delinear o perfil dos óbitos maternos foram: faixa etária, as causas, estado civil, cor/raça e escolaridade. A análise dos dados ocorreu pela estatística descritiva por distribuição de frequências e foi apresentada por meio de tabelas. Resultados: foram registrados 135 óbitos maternos, considerando causas externas, diretas (mais prevalentes), indiretas e tardias. A razão de mortalidade materna no Ceará foi de 60.43%, cujo perfil dos óbitos foi caracterizado por mulheres com nível superior incompleto (41.5%), pardas (80%), na faixa etária de 20-29 anos (43%) e solteiras (66.7%). Conclusão: as informações apresentadas neste estudo são de extrema relevância, pois possibilitam a compreensão de uma população-chave, as mulheres, além de auxiliar no delineamento de estratégias para o enfrentamento da mortalidade materna.

Abstract Objectives: evaluate maternal mortality from January 2019 to January 2020 in the state of Ceará. Methods: this is a descriptive and retrospective ecological study. Data collection was carried out from October to November 2020, considering the time interval from January 2019 to January 2020, through information registered in a public domain database in the State of Ceará, Brazil. The variables considered to outline the profile of maternal deaths were: age group, causes, marital status, color/race and education. Data analysis was performed using descriptive statistics by frequency distribution and presented through tables. Results: 135 maternal deaths were registered, considering external, direct (most prevalent), indirect and late causes. The maternal mortality ratio in Ceará was 60.43%, whose profile of deaths was characterized by women with incomplete higher education (41.5%), brown (80%), in their 20-29 years (43%) and single (66.7%). Conclusion: the information in this study is extremely importance, as it allows the understanding of a key population, women, in addition to assisting in the design of strategies for coping with maternal mortality.

Abstract INTRODUCTION: Monitoring trends in risk factors (RFs) and the burden of diseases attributable to exposure to RFs is an important measure to identify public health advances and current inadequate efforts. Objective: Analyze the global burden of disease attributable to exposure RFs in Brazil, and its changes from 1990 to 2019, according to the sex and age group. METHODS: This study used data from the Global Burden of Disease study. The Summary Exposure Value, which represents weighted prevalence by risk, was used to estimate exposure to RFs. The mortality and DALYs (Disability Adjusted Life Years) measurements were used to estimate the burden of diseases. For comparisons by year and between Brazilian states, age-standardized rates were used. RESULTS: Arterial hypertension was the factor responsible for most deaths in both sexes. For DALYs, the most important RF was the high body mass index (BMI) for women and alcohol consumption for men. Smoking had a substantial reduction in the attributable burden of deaths in the period. An important reduction was identified in the exposure to RFs related to socioeconomic development, such as unsafe water, lack of sanitation, and child malnutrition. Metabolic RFs, such as high BMI, hypertension, and alcohol consumption showed an increase in the attributable burden. CONCLUSIONS: Our findings point to an increase in metabolic RFs, which are the main RFs for mortality and DALYs. These results can help to consolidate and strengthen public policies that promote healthy lifestyles, thus reducing disease and death.
