RESUMO Objetivo: Investigar a intenção materna de amamentar exclusivamente (IMA) e variáveis associadas entre as mulheres no terceiro trimestre de gravidez. Métodos: A coleta de dados foi realizada por meio de entrevista e questionário semiestruturado e autoadministrado, entre dezembro/2018 e novembro/2019. As gestantes (n=653) responderam à escala Infant Feeding Intentions (IFI) testada, traduzida e adaptada para o português do Brasil e ao questionário contendo perguntas sobre variáveis sociodemográficas, biológicas e familiares, bem como relacionadas à gestação, ao aleitamento materno, à assistência à saúde e a hábitos. Foram realizadas análises estatísticas descritivas e de regressão logística múltipla hierarquizada, com nível de significância de 5%, para estimar a associação entre as variáveis independentes e o desfecho. A IMA, medida pela escala IFI, foi dicotomizada pela mediana (<16 ou =16). Resultados: A pontuação média±desvio padrão para a escala IFI foi de 14,4±2,6. Os resultados da análise de regressão mostraram que as gestantes que não tinham intenção de oferecer mamadeira (OR=4,33; IC95% 2,79-6,72) ou não sabiam (OR=1,85; IC95% 1,21–2,82), que planejaram a gestação (OR=1,52; IC95% 1,09–2,12), aquelas que acreditavam que teriam ajuda nos cuidados com o bebê (OR=3,60; IC95% 1,51–8,56) ou que não sabiam (OR=3,97; IC95% 1,26–12,51), bem como aquelas que relataram conhecer as recomendações da Organização Mundial de Saúde sobre amamentação (OR=1,73; IC95% 1.13–2.64) tinham mais chances de mostrar uma IMA muito forte. Conclusões: As gestantes no terceiro trimestre de gestação apresentaram forte IMA. A maior IMA esteve significativamente associada aos determinantes estruturais, contextuais e individuais. ABSTRACT Objective: To investigate exclusive breastfeeding (EBF) intention and associated variables among women in the third trimester of pregnancy. Methods: The data were collected with a questionnaire for the pregnant women (n=653), from December/2018 to November/2019. They answered the Infant Feeding Intentions (IFI) scale, translated and adapted to Brazilian Portuguese, and a questionnaire on sociodemographic, biological, family, pregnancy, breastfeeding, health care, and habits variables. Descriptive statistics and multiple logistic regression analyses were performed with a 5% significance level, following a multilevel hierarchical model that estimated the association between the dependent and independent variables. The outcome EBF intention measured by the IFI score was dichotomized by the median (<16 or =16). Results: Mean±standard deviation score for the IFI scale was 14.4±2.6 (score 0: very strong intention to not breastfeed at all; score 16: very strong EBF intention up to six months). The results from the regression analysis showed that pregnant women who had no intention to bottle feed (OR=4.33; 95%CI 2.79-6.72) or did not know (OR=1.85; 95%CI 1.21–2.82), those who planned the pregnancy (OR=1.52; 95%CI 1.09–2.12), those who believed they would have help to care for the baby (OR=3.60; 95%CI 1.51–8.56) or did not know (OR=3.97; 95%CI 1.26–12.51), and those who reported knowing the World Health Organization recommendations on breastfeeding (OR=1.73; 95%CI 1.13–2.64) were more likely to show a very strong EBF intention. RESUMO OBJETIVO Analisar o impacto do projeto estadual de pesquisa-ação nos indicadores de imunização (coberturas vacinais – CV, homogeneidade de cobertura vacinal – HCV, taxa de abandono – TA e classificação de risco) antes e após a intervenção em municípios e Gerências Regionais de Saúde/Superintendências Regionais de Saúde (GRS/SRS) prioritários. MÉTODOS O projeto estadual de pesquisa-ação foi um ensaio clínico comunitário, do tipo antes-depois, realizado em 212 municípios pertencentes a oito GRS/SRS do estado de Minas Gerais, Brasil. Compuseram a amostra do estudo as SRS/GRS com tendência decrescente para a cobertura vacinal de rotina em crianças menores de 1 ano, no período de 2015 a 2020. Neste estudo, foram utilizados dados secundários de CV e TA de 10 imunobiológicos recomendados para crianças menores de 2 anos, no período de janeiro a dezembro de 2021 (período pré-intervenção, anterior ao projeto estadual de pesquisa-ação) e de janeiro a dezembro de 2022 (período pós-intervenção). As variáveis categóricas foram apresentadas em proporções e, inicialmente, realizou-se a comparação entre as de TA, HCV e a classificação de risco para a transmissão de doenças imunopreveníveis, segundo os dois períodos (2021 e 2022), utilizando-se o teste McNemar. RESULTADOS Observou-se um aumento de todos os indicadores de imunização após a realização do projeto de pesquisa-ação. No ano de 2021, 80,66% dos municípios do estado tiveram a classificação de risco para transmissão de doenças imunopreveníveis como “alto e muito alto risco”. Em 2022, o valor foi reduzido para 68,40%. CONCLUSÕES A classificação de risco para transmissão de doenças imunopreveníveis é um mecanismo importante para auxiliar os gestores na definição de prioridades. O projeto estadual de pesquisa-ação empregou um método que possibilitou a construção e a execução de planos de ação singulares a cada município, direcionando a melhoria dos indicadores de imunização no estado.

ABSTRACT OBJECTIVE Analyze the impact of the state research-action project on immunization indicators (vaccination coverage – VC, homogeneity of vaccination coverage – HVC, dropout rate – DR, and risk rating) before and after the intervention in municipalities and priority Regional Health Administrations/Regional Health Superintendencies (RHA/RHS). METHODS The state research-action project was a before-after community clinical trial conducted in 212 municipalities belonging to eight RHA/RHS in the state of Minas Gerais, Brazil. The study sample comprised RHA/RHS with a decreasing trend for routine vaccination coverage in children under one year from 2015 to 2020. This study used secondary VC and DR data from 10 immunobiologicals recommended for children younger than two years from January to December 2021 (pre-intervention period, prior to the state research-action project) and from January to December 2022 (post-intervention period). The categorical variables were presented in proportions, and initially, a comparison was made between those of DR, HVC, and the risk rating for the transmission of vaccine-preventable diseases, according to the two periods (2021 and 2022), using the McNemar test. RESULTS All immunization indicators increased after conducting the research-action project. In 2021, 80.66% of the state’s municipalities had a risk rating for the transmission of vaccine-preventable diseases as “high and very high.” In 2022, the value reduced to 68.40%. CONCLUSIONS Risk rating for the transmission of vaccine-preventable diseases is an important mechanism to assist managers in defining priorities. The state research-action project used a method that enabled the construction and execution of unique action plans for each municipality, directing the improvement of immunization indicators in the state.

ABSTRACT This study aimed to identify factors associated with colonization by community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) in adult patients admitted to a Brazilian hospital. This is a cross-sectional study, in which patients underwent a nasal swab and were asked about hygiene behavior, habits, and clinical history. Among the 702 patients, 180 (25.6%) had S. aureus and 21 (2.9%) MRSA. The factors associated with MRSA colonization were attending a gym (OR 4.71; 95% CI; 1.42 – 15.06), smoking habit in the last year (OR 2.37; 95% CI; 0.88 – 6.38), previous hospitalization (OR 2.18; CI 95%; 0.89 – 5.25), and shared personal hygiene items (OR 1.99; 95% CI; 0.71 – 5.55). At the time of admission, colonization by CA-MRSA isolates was higher than that found in the general population. This can be an important public health problem, already endemic in hospitals, whose factors such as those associated with habits (smoking cigarettes) and behaviors (team sports practice and activities in gyms) have been strongly highlighted. Abstract Background Congenital heart diseases (CHD) are one of the most prevalent malformations, and the screening tests to identify critical congenital heart disease (CCHD) is the pulse oximetry test, with subsequent investigation and treatment. Objective To quantify positive pulse oximetry tests and verify the prevalence of CCHD detected by it in asymptomatic newborns, ≥35 gestational weeks, in a Brazilian maternity hospital. Methodology This is an observational, retrospective, quantitative, analytical and cross-sectional study, conducted from October 2020 to May 2022, in a maternity hospital in southern Brazil, through the collection of records of positive oximetry pulse tests, following the norms of screening CCHD test of the Brazilian Society of Pediatrics (SBP), after they were evaluated with echocardiography for confirmation or exclusion of CHD. Results A total of 5,667 newborns were evaluated in this study, according to the inclusion criteria; 0.17% (n = 10) had a positive pulse oximetry test. Regarding the results of the echocardiography of the neonates with a positive test, two were normal, seven were cases of patent foramen ovale (PFO), and one was a case (0,017%) with interatrial communication (IAC) diagnosis. In the same period, five neonates with CCHD were born, symptomatic, diagnosed by physical examination, and referred to neonatal intensive care units (NICU) before taking the neonatal cardiac screening (< 24 hours of life). Conclusion The prevalence of positive pulse oximetry tests was 0.17% and none CCHD was detected. Five cases of CCHD were born in this period, but they were diagnosed before the recommend time to perform the screening test. Resumo O acesso aos serviços de saúde é bandeira de luta dos movimentos sociais, e compõe o fluxo pela busca de cuidados na rede de urgência e emergência (UE). O objetivo deste artigo é analisar esse acesso no município de Boa Vista do Ramos e elaborar reflexões sobre o fluxo de usuários ribeirinhos em situação de UE. Trata-se de um estudo descritivo baseado em dados qualitativos, no qual o espaço foi incorporado à análise. As etapas consistiram em: levantamento de informações sobre a localização geográfica de lugares de interesse; entrevistas com os atores locais; organização e análise dos dados secundários e primários; e geração de mapas temáticos. A remoção dos pacientes em situação de UE em território boavistense não segue um padrão fixo, ou sequer similar. A fixação de equipes ribeirinhas completas e a sua permanência ininterrupta em área vem consolidando essas localidades como pontos de atenção à saúde. A seca e a cheia dos rios determinam caminhos, aproximando ou distanciando espacialmente. Frente à complexidade do cenário de vida, as decisões dos itinerários de cuidado estão atreladas às experiências prévias com o sistema de saúde local. O fluxo dos usuários diz muito sobre a imprevisibilidade das escolhas e das opções cotidianas.

Abstract Access to health services is a banner defended by social movements and informs the flow for seeking care in the urgent and emergency (UE) network. This article analyzes this access in the municipality of Boa Vista do Ramos, Amazonas, and reflects on the flow of UE riverside users. A descriptive study was conducted based on qualitative and geographical data. Research steps consisted of gathering geographic information on the locations of interest; interviewing local actors; organizing and analyzing secondary and primary data; and generating thematic maps. Removal of UE patients in Boa Vista does not follow any fixed pattern, or even a similar one. Establishment of full riverside teams and their uninterrupted stay in the area has consolidated these locations as health care points. The drought and flood of the rivers determine paths, approaching or distancing spatially. Given the complexity of the life scenario, decisions regarding care itineraries are linked to previous experiences with the local health system. User flow has much to inform about the unpredictability of everyday choices and options.

Resumo O cobrimento das armaduras tem a função de proteger física e quimicamente as armaduras de aço, através da qualidade do concreto e sua espessura, influenciando na durabilidade e vida útil. Este trabalho propôs variações do cobrimento baseados na classe de agressividade ambiental (CAA) de exposição dos concretos, considerando a resistência especificada em cada classe. Os traços foram submetidos aos ensaios acelerados de névoa salina e de carbonatação. Os testes foram mantidos por períodos até que o cobrimento de cada CAA fosse atingido, determinando o impacto no cobrimento das armaduras para cada CAA, mais amena ou mais severa. Cada um destes traços foi avaliado quanto às características físicas e mecânicas. Verificou-se que o cobrimento necessário às armaduras varia de maneira linear com a resistência à compressão dos traços, denotando a possibilidade de considerar mais de uma classe de resistência de concreto e o cobrimento nas diferentes CAA. Destaca-se que a diferença do cobrimento necessário em relação às CAA se torna maior quando utilizados concretos de classe superior a C30. Abstract The physical and chemical protection of steel reinforcement afforded by the cover is a consequence of the quality of the concrete and its thickness, with a long-term impact on durability and service life. This study proposed variations in cover based on environmental aggressivity class (EAC) of exposure considering concretes of prescribed compressive strength classes (C20/C25/C30/C40). To this end, concrete mixtures were subjected to accelerated testing in salt spray chambers or carbonation. Accelerated testing was kept for periods long enough to reach each EAC cover level. Thus, the effect of each EAC, from mild to severe, was determined on the physical and mechanical characteristics of each mixture. It was determined that the required reinforcement cover varied linearly with compressive strength. This denoted the possibility of using more than one class of concrete and cover for each EAC. It should be noted that the difference in required cover became greater when using concrete with a strength class higher than C30. Abstract Objective: To summarize data of clinical trials that used silver diamine fluoride (SDF) to prevent and treat caries lesions and dentinal hypersensitivity. Material and Methods: Six electronic databases were searched in May 2022. The concentration of SDF, type of usage (alone/combined), dentition, anterior/posterior teeth, tooth region, dental tissue, number of the treated surfaces, the intervention environment, participants' age, frequency and duration of SDF application, purpose, and outcome were the extracted variables. The type of study, year of publication, authors, journals, and country were also investigated. Results: From 8860 articles, S3 were selected. Most were randomized (n=38), that applied 38% SDF (n=43), alone (n=44), on multiple surfaces (n=44), only in dentin (n=36), of the crown (n=46) of anterior and posterior (n=36) primary teeth (n=39). The studies were preferably carried out outside the clinic (n=3l), only in children (n=33), with reapplication of SDF (n=30), but did not inform the duration of application (n= 19). SDF was most used to treat (n=46) only caries lesions (n=50). They were published between 2001 and 2022, mainly in the Journal of Dentistry (n=10). China (n=19) and Lo E.GM (n=19) were the countries and authors that published the most, respectively. Conclusion: The silver diamine fluoride 38% alone was most used to treat caries lesions in the dentin of the crown of all primary teeth, preferably applied on multiple surfaces, requiring re application, and outside the clinic. RESUMO OBJETIVO Analisar o impacto do projeto estadual de pesquisa-ação nos indicadores de imunização (coberturas vacinais – CV, homogeneidade de cobertura vacinal – HCV, taxa de abandono – TA e classificação de risco) antes e após a intervenção em municípios e Gerências Regionais de Saúde/Superintendências Regionais de Saúde (GRS/SRS) prioritários. MÉTODOS O projeto estadual de pesquisa-ação foi um ensaio clínico comunitário, do tipo antes-depois, realizado em 212 municípios pertencentes a oito GRS/SRS do estado de Minas Gerais, Brasil. Compuseram a amostra do estudo as SRS/GRS com tendência decrescente para a cobertura vacinal de rotina em crianças menores de 1 ano, no período de 2015 a 2020. Neste estudo, foram utilizados dados secundários de CV e TA de 10 imunobiológicos recomendados para crianças menores de 2 anos, no período de janeiro a dezembro de 2021 (período pré-intervenção, anterior ao projeto estadual de pesquisa-ação) e de janeiro a dezembro de 2022 (período pós-intervenção). ABSTRACT OBJECTIVE Analyze the impact of the state research-action project on immunization indicators (vaccination coverage – VC, homogeneity of vaccination coverage – HVC, dropout rate – DR, and risk rating) before and after the intervention in municipalities and priority Regional Health Administrations/Regional Health Superintendencies (RHA/RHS). METHODS The state research-action project was a before-after community clinical trial conducted in 212 municipalities belonging to eight RHA/RHS in the state of Minas Gerais, Brazil. Resumo Realizou-se este estudo com o objetivo de avaliar a funcionalidade nutricional do complexo xilanolítico produzido a partir de fungos da linhagem Aspergillus japonicus var. aculeatus UFMS 48.136, oriundo do bioma Cerrado/Pantanal sul mato-grossense em comparação à xilanase comercial, em dietas de suínos. Foram utilizados dezesseis suínos machos, com peso inicial de 64,23 ± 10,5 kg, distribuídos em delineamento experimental de blocos ao acaso, com quatro dietas: controle, formulado de acordo com as recomendações nutricionais; controle negativo, formulado com redução de 100 Kcal / kg de energia metabolizável (EM); controle negativo + xilanase Cerrado / Pantanal; controle negativo + xilanase comercial; com quatro repetições cada. Abstract The objective of this study was to evaluate the nutritional functionality of the xylanolytic complex produced from Aspergillus japonicus var. aculeatus UFMS 48.136 isolated from the Cerrado/ Pantanal biome in Mato Grosso do Sul, compared to commercial xylanase, in swine diets. Sixteen barrows were used, with an initial weight of 64.23 ± 10.5 kg, distributed in a randomized block experimental design, with four diets: control, formulated according to nutritional recommendations; negative control, formulated with a reduction of 100 Kcal / kg of metabolizable energy (ME); negative control + xylanase Cerrado / Pantanal; negative control + commercial xylanase; with four repetitions each. Objetivo: avaliar a usabilidade do aplicativo móvel ROBOVID para educação em saúde acerca da COVID-19. Método: estudo metodológico com abordagem quantitativa aplicado, desenvolvido com 21 adultos, por meio de formulário eletrônico. Objective: to evaluate the usability of the ROBOVID mobile application for health education about COVID-19. Method: methodological study with an applied quantitative approach, developed with 21 adults, using an electronic form. Usability was assessed using the System Usability Scale. The Statistical Package for the Social Sciences software was used to analyze the data. Results: the overall average score on the System Usability Scale questionnaire was 87.3, which classifies the usability of the ROBOVID application as the best achievable. Furthermore, the application has high scores for ease of memorization, user satisfaction and ease of getting to know the system, as well as excellent scores for absence of errors and speed in carrying out the tasks set. ABSTRACT Background: The current treatments for Chagas disease (CD) include benznidazole and nifurtimox, which have limited efficacy and cause numerous side effects. Triazoles are candidates for new CD treatments due to their ability to eliminate T. cruzi parasites by inhibiting ergosterol synthesis, thereby damaging the cell membranes of the parasite. Methods: Eleven synthetic analogs of the kinase inhibitor SRPIN340 containing a triazole core (compounds 6A-6K) were screened in vitro against the Tulahuen strain transfected with β-galactosidase, and their IC50, CC50, and selectivity indexes (SI) were calculated. Compounds with an SI > 50 were further evaluated in mice infected with the T. cruzi Y strain by rapid testing. Results: Eight compounds were active in vitro with IC50 values ranging from 0.5-10.5 µg/mL. The most active compounds, 6E and 6H, had SI values of 125.2 and 69.6, respectively. These compounds also showed in vivo activity, leading to a reduction in parasitemia at doses of 10, 50, and 250 mg/kg/day. ABSTRACT Exacerbated inflammation and coagulation are a hallmark of COVID-19 severity. Extracellular vesicles (EVs) are intercellular transmitters involved in inflammatory conditions, which are capable of triggering prothrombotic mechanisms. Since the release of EVs is potentially associated with COVID-19-induced coagulopathy, the aim of this study was to evaluate changes in inflammation- and hypercoagulability-related EVs during the first month after symptom onset and to determine whether they are associated with disease severity. Blood samples of patients with mild or severe forms of the disease were collected on three occasions: in the second, third and fourth weeks after symptom onset for the quantification by flow cytometry of CD41A (platelet glycoprotein IIb/IIIa), CD162 (PSGL-1), CD31 (PECAM-1) and CD142 cells (tissue factor). Analysis of variance (ANOVA) with repeated measures, Kruskal-Wallis and correlation tests were used. Eighty-five patients were enrolled, 71% of whom had mild disease. Seventeen uninfected individuals served as controls. Compared to controls, both mild and severe COVID-19 were associated with higher EV-CD31+, EV-CD41+ and EV-CD142+ levels. All EV levels were higher in severe than in mild COVID-19 only after the third week from symptom onset, as opposed to C-reactive protein and D-dimer levels, which were higher in severe than in mild COVID-19 earlier during disease progression. EV levels were also associated with C-reactive protein and D-dimer levels only after the third week of symptoms. In conclusion, EVs expressing CD41A, CD31, TF, and CD162 appear as late markers of COVID-19 severity. Abstract As historical waste has accumulated in dams, the interest in studying the feasibility of reusing mining tailings has grown, offering both environmental and economic advantages. However, given the unique characteristics of each site, conducting site-specific assessments is crucial. Building on a detailed evaluation of the Cocoruto Dam in Nova Lima, Minas Gerais, this study presents a technical feasibility and financial viability analysis of three potential reprocessing scenarios. The primary objective was to assess the viability and potential benefits of reusing Au tailings. The proposed methods involve grinding, calcination and leaching, with designs adaptable for existing metallurgical plants. Considering the economic potential of Au in these inactive mine tailings (with an average grade of 0.95 mg/kg and a total resource of 3,350.55 tonnes), multiple factors were analyzed to determine the feasibility of Au production from this source. Analyzed variables included: cut-off grades and tonnage for each scenario; net present value (NPV) calculated with an 8% annual discount rate; a general slope angle of 20 degrees; three possible final pit configurations; different annual production rates (400,000 t, 800,000 t and 1,000,000 t), and various Au ounce values ($1300/oz, $1500/oz, and $1700/oz). Abstract: This article provides a genealogical analysis of the inclusion of “child aggressiveness” and its variations in the Diagnostic and Statistical Manual of Mental Disorders (DSM) throughout its editions.
