Resumen La relación entre el consumo de bebidas endulzadas con sacarosa y la reducción en la ingesta de alimentos ha sido objeto de estudio en el ámbito de la salud pública y la nutrición. A medida que el consumo de estas bebidas ha aumentado en las últimas décadas, se ha observado un aumento de los problemas de salud relacionados con la alimentación. En este trabajo se analiza el efecto que tiene la ingesta crónica de agua endulzada con sacarosa sobre el consumo de alimento balanceado, el perfil lipídico, la resistencia a la insulina, y en algunos marcadores del estado nutricional como el peso, la proteína sérica total, albúmina, hemoglobina y creatinina en ratas Wistar macho. El agua endulzada con sacarosa al 12% se administró a un grupo de 9 ratas durante 8 semanas. Un grupo control de 9 ratas bebieron agua natural. El consumo de alimentos entre ambos grupos y los marcadores bioquímicos se analizaron al final del tratamiento. Las concentraciones séricas de glucosa, lípidos, proteínas totales, albúmina, creatinina y hemoglobina se midieron mediante espectroscopia. Los resultados mostraron que el grupo de ratas que consumieron agua endulzada con sacarosa ingirieron menos alimento balanceado; consumieron una mayor cantidad de líquidos y calorías. Sin embargo, el consumo de agua endulzada no modificó el perfil lipídico. Los resultados de marcadores nutricionales tales como peso, creatinina, proteínas totales, albúmina y hemoglobina mostraron que el consumo crónico de sacarosa al 12% no ocasionaba un estado de desnutrición a largo plazo por menor consumo de alimento balanceado.

Resumo A relação entre o consumo de bebidas açucaradas com sacarose e a redução na ingestão de alimentos tem sido objeto de estudo na área de saúde pública e na nutrição. Como o consumo destas bebidas tem crescido nas últimas décadas, foi observado um aumento dos problemas de saúde relacionados com a alimentação. No presente trabalho foi analisado o efeito da ingestão crônica de água açucarada com sacarose sobre o consumo de alimento balanceado, perfil lipídico, resistência à insulina, bem como em alguns marcadores do estado nutricional como peso, proteína sérica total, albumina, hemoglobina e creatinina em ratos Wistar machos. Água adoçada com sacarose a 12% foi administrada a um grupo de 9 ratos durante 8 semanas, com um grupo de 9 ratos que beberam água natural como grupo controle. O consumo alimentar entre os dois grupos, bem como os marcadores bioquímicos, foram analisados no final do tratamento. As concentrações séricas de glicose, lipídios, proteínas totais, albumina, creatinina e hemoglobina foram medidas por espectroscopia. Os resultados mostraram que o grupo de ratos que consumiu água adoçada com sacarose ingeriu menos alimento balanceado; Eles consumiram maior quantidade de líquidos e calorias. Entretanto, o consumo de água adoçada não modificou o perfil lipídico. Os resultados dos marcadores nutricionais como peso, creatinina, proteína total, albumina e hemoglobina mostraram que o consumo crônico de sacarose a 12% não causava estado de desnutrição no longo prazo devido ao menor consumo de alimentos balanceados.

Abstract The relationship between the consumption of sucrose-sweetened beverages and the reduction in the intake of foods has been the subject of study in the field of public health and nutrition. As consumption of these beverages has increased in recent decades, an increase in diet-related health problems has been observed. In the present work, the effect of chronic intake of water sweetened with 12% sucrose on balanced food consumption, lipid profile, insulin resistance, as well as on some markers of nutritional status such as weight, total serum, protein, albumin, hemoglobin and creatinine in male Wistar rats is analysed. Water sweetened with sucrose was administered to a group of 9 rats for 8 weeks, with a group of 9 rats that drank natural water as a control. Food consumption between both groups, as well as blood chemistry markers, were analysed at the end of treatment. Serum concentrations of glucose, lipids, total protein, albumin, creatinine, and hemoglobin were measured by spectroscopy. The results showed that the group of rats that consumed water sweetened with sucrose ingested less balanced food; they consumed a greater amount of fluids and calories. However, consumption of sweetened water did not modify the lipid profile. The results of nutritional markers such as weight, creatinine, total protein, albumin and hemoglobin show that chronic consumption of 12% sucrose does not cause a state of long-term malnutrition due to lower consumption of balanced food.

ABSTRACT Despite being subject to lower AIDS-related mortality rates and having a higher life expectancy, patients with HIV are more prone to develop non-AIDS events. A low CD4+/CD8+ ratio during antiretroviral therapy identifies people with heightened immune senescence and increased risk of mortality. In clinical practice, finding determinants of a low CD4+/CD8+ ratio may be useful for identifying patients who require close monitoring due to an increased risk of comorbidities and death. We performed a prospective study on the evolution of the CD4+/CD8+ ratio in 60 patients infected with HIV (80% males), who were subjected to two different antiretroviral regimens: early and deferred therapy. The initial CD4+/CD8+ ratio was ≤1 for 70% of the patients in both groups. Older age, CD4+ cell count at inclusion, Nadir CD8+T-cell count, and Initial CD4+/CD8+ ratio ≤ 1 were risk factors for lack of ratio recovery. In the multivariate analysis, a CD4+/CD8+ ratio > 1 at the start of the treatment was found to be a determinant factor in maintaining a CD4+/CD8+ ratio > 1. The nadir CD4+T-cell count was lower in the deferred therapy group (p=0.004), and the last CD4+/CD8+ ratio ≤1 was not associated with comorbidities. Ratio recovery was not associated with the duration of HIV infection, time without therapy, or absence of AIDS incidence. A greater improvement was observed in patients treated early (p=0.003). In contrast, the slope of increase was slower in patients who deferred treatment. In conclusion, the increase in the CD4+/CD8+ ratio occurred mostly for patients undergoing early strategy treatment and its extension did not seem to be related to previous HIV-related factors. Abstract Introduction Adolescents represent an important demographic percentage in the studied population and in Emergency Departments (ED). It is imperative that health professionals and services are prepared to address this population. This work aims to characterize adolescents at the ED of a Brazilian private tertiary hospital. Methods The study was an observational, retrospective longitudinal cohort that included 37,450 visits of patients aged 10 to 21 years of age, between January 2018 and June 2022 in the ED of a private tertiary hospital. The study evaluated the reason for the consultation, diagnosis, need for hospitalization, the medical professional responsible for the care, severity, and paying source of care. Results 53.7% were female. Mean age was 16.2y for girls and 15.6y for boys (p < 0.005). The most common complaints were flu-like symptoms (17.4%), sore throat (8.2%), fever (6.7%) and limb trauma (6.3%). Flu-like symptoms were the main consultations caused in all age groups and genders. 36.8% were attended by a general practitioner, 35.8% by a pediatrician, 15.1% by orthopedics and 5.6% by surgeons. The hospitalization rate was 5.5%. There was a strong correlation between age and hospitalization rate (correlation coefficient [r = 0.93]; p < 0.001). The most prevalent diagnoses in hospitalizations were acute abdomen (12.7%) and trauma (9.4%). 78.2% of the consultations were classified as “not urgent”. There was a strong correlation between age and severity (r = 0.86; p < 0.001). 92.7% of the consultations were paid by medical insurance. Conclusion In this study, flu-like symptoms were the single main reason for adolescents to search for immediate health care, in every age subgroup and gender, but represented a small risk for hospital admission. Limb trauma was more common in younger male teenagers. Acute abdominal pain and trauma were the most frequent causes of hospital admissions. There was a significant and strong correlation between age and both admission rate and severity. ED. . Resumo Fundamento O implante percutâneo de bioprótese valvar aórtica (TAVI) consolidou-se como opção terapêutica da estenose aórtica de grau importante. Dados sobre as características evolutivas dos procedimentos e dos resultados obtidos com a técnica ao longo da última década, em escala nacional, são desconhecidos. Objetivos Analisar a tendência temporal referente ao perfil demográfico, características dos procedimentos e desfechos hospitalares de pacientes submetidos a TAVI na Rede D’Or São Luiz. Métodos Registro observacional envolvendo 29 instituições nacionais. Comparou-se características dos procedimentos realizados de 2012 a 2017 (Grupo 1) e de 2018 a 2023 (Grupo 2). Foram considerados significantes os resultados com valor de p < 0,05. Resultados Foram analisados 661 casos, 95 pertencentes ao Grupo 1 e 566 ao Grupo 2. A média de idade foi 81,1 anos. Observou-se no Grupo 1 maior prevalência de pacientes em classe funcional III ou IV e escore de risco > 8%. Foi mais frequente o emprego de anestesia geral, monitorização ecocardiográfica transesofágica e via de acesso por dissecção. Maior taxa de sucesso do procedimento (95,4% versus 89,5%; p = 0,018) foi aferida em implantes efetivados a partir de 2018, assim como menor mortalidade (3,9% versus 11,6%; p = 0,004) e necessidade de marcapasso definitivo (8,5% versus 17,9%; p = 0,008). Conclusões A análise temporal de 10 anos do Registro TAVIDOR demonstra uma queda na complexidade clínica dos pacientes. Além disso, o avanço para técnicas de implante minimalistas, somadas à evolução tecnológica dos dispositivos, podem ter contribuído para desfechos favoráveis dentre aqueles cujo implante ocorreu no último quinquênio. Abstract Background Transcatheter aortic valve implantation (TAVI) has established itself as the preferential strategy to approach severe aortic stenosis. Information on procedural improvements and nationwide results obtained with the technique throughout the past decade are unknown. Objectives To assess the temporal variation of the demographic profile, procedural characteristics, and in-hospital outcomes of patients undergoing TAVI procedures at the Rede D’Or São Luiz. Methods Observational registry comprising 29 national institutions, comparing the characteristics of the TAVI procedures performed from 2012 to 2017 (Group 1) to those performed from 2018 to 2023 (Group 2). The statistical significance level adopted was p < 0.05. Results This study assessed 661 patients, 95 in Group 1 and 566 in Group 2, with a mean age of 81.1 years. Group 1 patients had a higher prevalence of New York Heart Association functional class III or IV and STS risk score > 8%. In addition, they more often underwent general anesthesia, transesophageal echocardiographic monitoring, and access through femoral dissection. Group 2 patients had a higher success rate of the TAVI procedure (95.4% versus 89.5%; p = 0.018), lower mortality (3.9% versus 11.6%; p = 0.004), and less often needed permanent pacemaker implantation (8.5% versus 17.9%; p = 0.008). Conclusions The 10-year temporal trends analysis of the TAVIDOR Registry shows a reduction in patients’ clinical complexity over time. Furthermore, the advance to minimalistic implantation techniques, added to the technological evolution of the devices, may have contributed to the favorable outcomes observed among those whose implantation occurred in the last 5 years studied. SUMMARY OBJECTIVE: The aim of this study was to evaluate the prevalence and risk factors related to metabolic dysfunction-associated steatotic liver disease in inflammatory bowel disease patients. METHODS: This is a cross-sectional study conducted on adults with inflammatory bowel disease from 2019 to 2021. Metabolic dysfunction-associated steatotic liver disease encompasses patients with steatosis and at least one cardiometabolic risk factor. Patients with alcohol consumption ≥20 g/day, chronic liver diseases, or methotrexate use were excluded. RESULTS: Almost 140 patients were included: 67.1% were female, with a mean age of 49.7±13.7 years, and 63.6% had Crohn's disease. The mean duration of inflammatory bowel disease was 9.7±7.9 years. Metabolic dysfunction-associated steatotic liver disease was observed in 44.3% and advanced liver fibrosis was excluded in 63.5% by Fibrosis-4. Patients with metabolic dysfunction-associated steatotic liver disease were older (p = 0.003) and had a higher number of metabolic syndrome components (2.9±1.1 versus 1.6±1.0; p<0.001), greater abdominal circumference (p<0.001), and body mass index (p<0.001). The only factor related to inflammatory bowel disease associated with metabolic dysfunction-associated steatotic liver disease was disease duration (11.6±9.5 versus 8.3±6.2; p = 0.017). A higher number of metabolic syndrome components and obesity increase by 2.2 times and an altered waist circumference by 2.6 times the occurrence of metabolic dysfunction-associated steatotic liver disease. CONCLUSION: A high prevalence of metabolic dysfunction-associated steatotic liver disease was observed in patients with inflammatory bowel disease, with the main risk factors being associated with metabolic syndrome predicting it, but not with inflammatory bowel disease features and/or its treatment. ABSTRACT Introduction: Hemotropic mycoplasmas are widely distributed in several regions of the world. The aim of the present work was to determine the presence of hemotropic mycoplasma species in cattle, buffaloes, dogs and ticks in Cuba. Methods: They were analyzed thirty-nine buffalo and 41 cattle blood samples to investigate the presence of Mycoplasma wenyonii and 'Candidatus Mycoplasma haemobos'. They were analyzed and 391 dog blood samples and 247 tick pools to detect the presence of Mycoplasma haemocanis and 'Candidatus Mycoplasma haematoparvum'. In all cases it was used the species-specific TaqMan® real-time PCR assay. Results: For M. wenyonii 53 animals were positive and 33 for 'Ca. Mycoplasma haemobos'; in coinfection with M. wenyonii, with similar prevalence in cattle and buffalo. 17.9% of dog samples were positive for at least one hemoplasma species, with 15.1% positive for M. haemocanis, 4.4% for 'Ca. Mycoplasma haematoparvum', and 1.5% co-infected. The ticks were identified as Rhipicephalus sanguineus sensu lato; none of the pools were positive for the canine mycoplasma species under study. It was demonstrated he presence of five different haplotypes for M. haemocanis and four haplotypes for 'Ca. M. haematoparvum'; one of these haplotypes showed 100 % identity with one isolated from Grenada, which was associated with cases of zoonotic exposure. Conclusions. These results constitute the first molecular evidence of infection by hemoplasma species in cattle and buffaloes in Cuba and the Caribbean and the first molecular report of the occurrence of single infections and co-infections caused by M. haemocanis and 'Ca. M. haematoparvum' in dogs in Cuba. For the first time is evident the circulation of a genotype of ‘Ca. M. haematoparvum' with zoonotic potential, common between the Greater and Lesser Antilles of the Caribbean.

RESUMEN Introducción: Los micoplasmas hemotrópicos se encuentran ampliamente distribuidos en varias regiones del mundo. El presente trabajo tuvo como objetivo determinar la presencia de especies de micoplasmas hemotrópicos en bovinos, bufalinos, perros y garrapatas de Cuba. Métodos: Se analizaron 39 muestras de sangre de búfalos y 41 bovinos para investigar la presencia de Mycoplasma wenyonii y 'Candidatus Mycoplasma haemobos'; 391 muestras de sangre de perros y 247 pools de garrapatas para detectar la presencia de Mycoplasma haemocanis y 'Candidatus Mycoplasma haematoparvum' mediante ensayos de PCR en tiempo real TaqMan® (especie específicos). Resultados: Para M. wenyonii 53 animales fueron positivos y 33 para 'Ca. Mycoplasma haemobos'; en coinfección con M. wenyonii, con prevalencias similares en bovinos y bufalinos. El 17,9 % de las muestras de perros fueron positivas para al menos una especie de hemoplasma, con el 15,1 % positivo para M. haemocanis, el 4,4 % para 'Ca. Mycoplasma haematoparvum', y el 1,5 % coinfectados. Las garrapatas se identificaron como Rhipicephalus sanguineus sensu lato; y ninguno de los pools fue positivo a las especies de micoplasmas caninos en estudio. Se demostró la presencia de 5 haplotipos diferentes para M. haemocanis y 4 haplotipos para 'Ca. M. haematoparvum'; uno de estos haplotipos mostró 100 % de identidad con un aislado de Granada, que se asoció con casos de exposición zoonótica. Conclusiones: Estos resultados constituyen la primera evidencia molecular de la infección por especies de hemoplasmas en bovinos y bufalinos de Cuba y el Caribe y el primer reporte molecular de la ocurrencia de infecciones simples y coinfecciones causadas por M. haemocanis y 'Ca. M. haematoparvum' en perros de Cuba. Por primera vez se evidencia la circulación de un genotipo de 'Ca. M. haematoparvum' con potencial zoonótico, común entre las Antillas mayores y menores del Caribe.

RESUMEN Las invasiones biológicas han demostrado de forma inequívoca ser una de las primordiales causas mundiales del detrimento de la biodiversidad. El objetivo de la investigación consistió en proponer un sistema de actividades socioculturales en los poblados pesqueros de Isabela de Sagua y Caibarién, en la provincia de Villa Clara Cuba, para contribuir al conocimiento de las Especies Exóticas Invasoras, y sus efectos indeseables a los ecosistemas marinos cubanos y a la salud humana y en particular de la especie Pterois volitans, conocida como Pez León. Para el desarrollo de esta investigación, se emplearon métodos de recopilación de información y procesamiento de estas. La investigación representó una manera novedosa de apropiarse de nuevos conocimientos sobre la especie P. volitans por parte de los habitantes de dichos poblados pesqueros. En las entrevistas realizadas, se pudo constatar que algunos pobladores poseían ciertos conocimientos sobre el Pez León, no así de los efectos nocivos que puede provocar este animal a los ecosistemas marinos cubanos y a la salud humana. Se concluye que la invasión del Pez León es un recordatorio de cómo una especie foránea puede establecerse y provocar efectos nocivos.

ABSTRACT Biological invasions have unequivocally demonstrated to be one of the main global causes of biodiversity detriment. The objective of the research consisted in proposing a system of socio-cultural activities in the fishing villages of Isabela de Sagua and Caibarién, in the province of Villa Clara Cuba, to contribute to the knowledge of Invasive Alien Species, and their undesirable effects on Cuban marine ecosystems and on human health and in particular on the species Pterois volitans, known as lionfish. For the effective development of this research, information gathering and processing methods were used to approach the multilateral study of the research object. The research represented a novel way of appropriating new knowledge about the species P. volitans on the part of the inhabitants of these fishing villages. In the interviews conducted, it was found that some villagers had some knowledge about the lionfish, but not about the harmful effects that this animal can cause to Cuban marine ecosystems and human health. It is concluded that the lionfish invasion is a reminder of how a foreign species can become established and cause harmful effects.

Abstract The iAtlantic Project has established an international collaborative strategy to improve mapping and characterization of deep and open ocean ecosystems in understudied regions of the Atlantic and evaluate their health. In December 2022, the first iAtlantic expedition in the South Atlantic set off to map and explore seafloor ecosystems in the Santos Basin slope (200-1,000 m depths) in collaboration with the Petrobras ‘Santos Basin - Regional Characterization Project.’ The 17-day ‘iAtlantic_BR10-Petrobras’ cruise was conducted on board the research vessel NPqHOc Vital de Oliveira (Brazilian Navy) and performed (a) water column structure characterization, (b) seafloor morphology mapping, (c) description of benthic habitats and communities by seafloor imagery and biological/ geological sampling, and (d) ex-situ experiments to assess the functioning of sedimentary ecosystems and their responses to climate-related environmental changes. ABSTRACT State parks are integral protection units created to protect ecosystems. However, anthropic activities may have been previously performed before their creation, forming transitional areas. Studies that evaluate the modifications of edaphic properties in these environments are scarce. The aim of this study was to evaluate the changes in soil properties and litter stock in areas of eucalyptus-forest ecotones. Litter stock (Stocklitter) and total soil organic carbon content and stock (TOCsoil), weighted mean diameter (MWD), origin of aggregates (biogenic or physicogenic) and respective content of total organic carbon (TOCBio and TOCPhy), total organic carbon (TOCAG), soil organic matter chemical fractions [fulvic acid (FAC), humic (HAC), and humin (HUMC)], and particulate, mineral-associated, free light and intra-aggregate light (POC, MAOC, FLFC, and ILFC) soil organic matter, fertility, and glomalin-related soil protein (GRSP) fractions were evaluated in aggregates (layer 0.00-0.10 m) in areas of eucalyptus-forest ecotone (Eco), preserved forest fragment (For), and eucalyptus plantation (Euc), in Nova Baden State Park, Lambari–MG. In the Euc and Eco areas, the highest Stocklitter content was found. Higher values of TOCsoil, TOCAG, GRSP, and MWD were observed in the Euc area. In the biogenic aggregates, the highest content of TOCBio, HAC, HUMC, POC, MAOC, FLFC, and ILFC was determined in detriment of the physicogenic ones. The dynamics of edaphic properties in the Eco area showed greater similarity with the For area than in the Euc area. RESUMO OBJETIVO Estimar a soroprevalência de anticorpos do vírus SARS-CoV-2 em escolares de quatro a 14 anos de idade residentes no município de São Paulo, segundo variáveis clínicas, demográficas, epidemiológicas e sociais, durante o período de fechamento das escolas como medida de controle da covid-19. MÉTODOS Realizou-se um inquérito sorológico em setembro de 2020 com amostra aleatória estratificada por tipo de rede de ensino (pública municipal, pública estadual e privada). Foi coletada amostra de sangue venoso utilizando-se o teste de imunoensaio de fluxo lateral da fabricante Wondfo para detecção de anticorpos totais contra o vírus SARS-CoV-2. Aplicaram-se questionários semiestruturados para o levantamento de dados clínicos, demográficos, sociais e epidemiológicos. RESULTADOS A soroprevalência de anticorpos do vírus SARS-CoV-2 em escolares foi de 16,6% (IC95% 15,4–17,8). O estudo encontrou soroprevalências mais elevadas na rede pública municipal (18,5%; IC95% 16,6–20,6) e estadual (16,2%; IC95% 14,4–18,2) em relação à rede privada (11,7; IC95% 10,0–13,7) e entre escolares da raça/cor preta e parda (18,4%; IC95% 16,8–20,2) e no estrato social mais vulnerável (18,5%; IC95% 16,9–20,2). A pesquisa identificou menores soroprevalências nos escolares que relataram seguir as medidas recomendadas de proteção contra a covid-19. CONCLUSÃO A soroprevalência de anticorpos contra o vírus SARS-CoV-2 atinge principalmente os escolares socialmente mais vulneráveis. Este estudo pode contribuir para embasar políticas públicas que reforcem a importância da suspensão das aulas presenciais e da necessidade de estratégias de medidas de proteção e acompanhamento do status sorológico daqueles que ainda não foram contemplados no calendário vacinal. ABSTRACT OBJECTIVE To estimate seroprevalence of SARS-CoV-2 antibodies in schoolchildren aged 4 to 14 years living in the city of São Paulo, according to clinical, demographic, epidemiological, and social variables, during the school closure period as a measure against covid-19 spread. METHODS A serological survey was made in September 2020 with a random sample stratified by school system (municipal public, state public and private) type. A venous blood sample was collected using the Wondfo SARS-CoV-2 Antibody Test (lateral flow method) for detection of total SARS-CoV-2 virus antibodies. Semi-structured questionnaires were applied to collect clinical, demographic, social, and epidemiological data. RESULTS Seroprevalence of SARS-CoV-2 antibodies in schoolchildren was of 16.6% (95%CI 15.4–17.8). The study found higher seroprevalence in the municipal (18.5%; 95%CI 16.6–20.6) and state (16.2%; 95%CI 14.4–18.2) public school systems compared to the private school system (11.7; 95%CI 10.0–13.7), among black and brown students (18.4%; 95%CI 16.8–20.2) and in the most vulnerable social stratum (18.5 %;95%CI 16.9–20.2). Lower seroprevalence was identified in schoolchildren who reported following the recommended protective measures against covid-19. CONCLUSION Seroprevalence of SARS-CoV-2 antibodies is found mainly in the most socially vulnerable schoolchildren. This study can contribute to support public policies that reinforce the importance of suspending face-to-face classes and developing strategies aimed at protective measures and monitoring of the serological status of those who have not yet been included in the vaccination schedule. Abstract Objective COVID-19 is associated with an elevated risk of thromboembolism and excess mortality. Difficulties with best anticoagulation practices and their implementation motivated the current analysis of COVID-19 patients who developed Venous Thromboembolism (VTE). Method This is a post-hoc analysis of a COVID-19 cohort, described in an economic study already published. The authors analyzed a subset of patients with confirmed VTE. We described the characteristics of the cohort, such as demographics, clinical status, and laboratory results. We tested differences amid two subgroups of patients, those with VTE or not, with the competitive risk Fine and Gray model. Results Out of 3186 adult patients with COVID-19, 245 (7.7%) were diagnosed with VTE, 174 (5.4%) of them during admission to the hospital. Four (2.3% of these 174) did not receive prophylactic anticoagulation and 19 (11%) discontinued anticoagulation for at least 3 days, resulting in 170 analyzed. During the first week of hospitalization, the laboratory most altered results were C-reactive protein and D-dimer. Patients with VTE were more critical, had a higher mortality rate, worse SOFA score, and, on average, 50% longer hospital stay. Conclusion Proven VTE incidence in this severe COVID-19 cohort was 7.7%, despite 87% of them complying completely with VTE prophylaxis. Abstract The Santos Basin (SB) is the main petroliferous basin in the Brazilian continental margin and one of the most studied marine areas in Brazil. However, historical data suggest that new efforts should be carried out to acquire quantitative biological data, especially in the deep sea, to establish the baseline of essential ocean variables in different ecosystems for future monitoring programs. The Brazilian energy company Petrobras planned and executed 24 oceanographic cruises over a period of 2 years to assess the benthic (SANSED cruise) and pelagic (SANAGU cruise) systems of the SB (356 days at sea in 2019 and 2021/2022). These efforts were part of the Santos Project, which comprised a comprehensive environmental study aimed at investigating benthic and pelagic variables to characterize ecology, biogeochemistry, thermohaline properties of water masses, and ocean circulation patterns, geomorphology, and sedimentology, as well as organic and inorganic chemistry. Here we present the detailed sampling designs and the field methods employed on board, during the SB scientific cruises. All sampling protocols were based on standardized approaches. For the benthos analyses, triplicate sediment samples were performed using a GOMEX-type box corer (0.25 m²) or a large modified Van Veen grab (0.75 m²) at 100 stations ranging from 25 to 2400 m depth. At each station, 25 geochemical and physico-chemical parameters were analyzed in addition to micro-, meio-, and macrofauna and living foraminifera samples. For the pelagic system, 60 stations were selected to investigate the plankton community, ranging in size from pico- to macroplankton, through vertical, horizontal, and oblique net hauls (20, 200, and 500 μm mesh size), as well as 25 biogeochemical parameters collected with an aid of a CTD-rosette sampler. Part of this scientific information also serves the Regional Environmental Characterization Project (PCR-BS) in support of Petrobras’ Santos Basin drilling licensing process led by the Brazilian Environmental Agency – IBAMA. This project contributes to the sustainable development of the SB, in line with the guidelines of the United Nations Decade of Ocean Science for Sustainable Development. Introducción. En España, el ejercicio de cualquier especialidad médica exige disponer del título de especialista. El acceso a la formación como especialista se realiza a través de la prueba MIR. Esta prueba es obligatoria para poder optar a una plaza de formación en cualquier especialidad médica. El presente artículo realiza un estudio psicométrico tanto a través de la teoría clásica de los test como de la teoría de la respuesta al ítem de las pruebas MIR de 2020 y 2021 (de 175 y 200 preguntas, respectivamente). Material y métodos. La base de datos utilizada en el presente trabajo contiene las respuestas de un total de 4.810 opositores que se examinaron en 2020 y 3.888 que lo hicieron en 2021. La información disponible se analizó haciendo uso de indicadores como el alfa de Cronbach, la fórmula número 21 de Kuder y Richardson, el índice de dificultad, el índice de dificultad con corrección de los efectos del azar, el índice de discriminación, el índice de correlación biserial puntual, así como los índices de dificultad y discriminación según la teoría de respuesta al ítem. Resultados. Las pruebas MIR de 2020 y 2021 presentan una alta fiabilidad, con variaciones pequeñas de un año a otro en el comportamiento de las distintas asignaturas y bloques de preguntas en lo relativo a los índices utilizados en el presente estudio. Conclusiones. Los resultados obtenidos en el presente estudio permiten afirmar que el examen MIR es un examen objetivo, de dificultad y discriminación adecuadas, así como estable de una convocatoria a otra.

Introduction. In Spain, in order to practise any medical speciality, it is necessary to have qualified as a specialist. Access to training as a specialist is gained through the MIR test. This test is mandatory to qualify for a training position in any medical specialty. This article carries out a psychometric study of the 2020 and 2021 MIR tests (with 175 and 200 questions, respectively) through both the classical test theory and the item response theory. Material and methods. The database used in this study contains the responses from a total of 4,810 individuals who took the exam in 2020 and 3,888 who did so in 2021. The information available was analysed using indicators such as Cronbach's alpha, the formula number 21 of Kuder and Richardson, the difficulty index, the difficulty index corrected for random effects, the discrimination index and the point biserial correlation index. The difficulty and discrimination indices were also used, according to item response theory. Results. The MIR tests of 2020 and 2021 present high reliability, with only slight variations from one year to another in the behaviour of the different subjects and question blocks in terms of the indices used in the present study. Conclusions. The results obtained in the present study allow us to affirm that the MIR exam is objective, of appropriate difficulty and discrimination, as well as stable from one year to another.

Introducción. En España, para ejercer como especialista de cualquier especialidad médica es necesario tener la titulación correspondiente. Para poder acceder a la formación como especialista es obligatorio superar la prueba MIR. En este artículo se ha realizado un análisis psicométrico de la prueba MIR de 2022, celebrada en enero de 2023, utilizando tanto la teoría clásica de los test como la teoría de respuesta al ítem. Material y métodos. Se hizo uso de un subconjunto formado por 3.229 médicos que se presentaron a la prueba MIR de 2022 y que introdujeron su plantilla de respuestas en una aplicación informática puesta a su disposición para tal efecto. Los datos se analizaron mediante la teoría clásica de los test y la teoría de la respuesta al ítem. Concretamente, se calcularon el alfa de Cronbach, la fórmula número 21 de Kuder y Richardson, el índice de dificultad, el índice de dificultad con corrección de los efectos del azar, el índice de discriminación, el índice de correlación biserial puntual, así como los índices de dificultad y discriminación según la teoría de respuesta al ítem. Resultados. La prueba MIR de 2022, al igual que las de años anteriores, presenta una alta fiabilidad. Se analizan tanto los resultados globales de la prueba como detallados por asignaturas y bloques de asignaturas. Conclusión. Los resultados obtenidos en el presente estudio permiten afirmar que el examen MIR es un examen objetivo, de dificultad y discriminación adecuadas, así como relativamente estable de una convocatoria a otra.

Introduction. In Spain, to practice as a specialist in any medical specialty, it is necessary to have the corresponding qualification. In order to access training as a specialist, it is mandatory to pass the MIR test. In this article, a psychometric analysis of the 2022 MIR test, held in January 2023, has been carried out using both classical test theory and item response theory. Materials and methods. A subset made up of 3,229 doctors who took the MIR test in 2022 and who entered their response template in a computer application made available for this purpose. Data were analyzed using classical test theory and item response theory. Specifically, Cronbach's alpha, Kuder and Richardson's formula number 21, the difficulty index, the difficulty index corrected for chance effects, the discrimination index, the biserial-point correlation index, as well as the difficulty and discrimination indices according to item response theory were calculated. Results. The 2022 MIR test, like those of previous years, shows high reliability. Both the overall results of the test and those detailed by subjects and subject blocks are analyzed. Conclusion. The results obtained in this study allow us to affirm that the MIR exam is an objective exam, of adequate difficulty and discrimination, as well as relatively stable from one call to another.
