Abstract The informal caregiver provides non-remunerated permanent or regular care to dependent older adults. This qualitative study aimed to identify the perceptions of informal caregivers about motivations, needs, and benefits of caring for dependent older adults. It was conducted with ten Portuguese informal caregivers, based on an instrument with questions about the care provided to older adults and their perceptions about performing this role. The results revealed the following motivations for care: proximity and trust relationship, duty of care, more available family members, home proximity, lack of vacancies, high cost of shelter institutions, and older adults’ desire to remain in their homes. The primary care activities for older adults are hydration, hygiene, food, therapeutic administration, companionship, emotional support, comfort, entertainment, and promoting autonomy and dignity. The needs identified by the caregivers were home, social security, and the caregiver’s employer support, financial help, psychological support, and training to care for the older adults. The benefits of informal care for dependent older adults were prompt family support, physical and emotional security, affection, and companionship. Resumo O cuidador informal presta cuidados permanentes ou regulares a pessoas idosas em situação de dependência, sem remuneração. Objetivou-se identificar as percepções de cuidadores informais sobre motivações, necessidades e benefícios do cuidado ao idoso dependente. Estudo qualitativo realizado com 10 cuidadoras informais portuguesas, a partir de um instrumento com questões sobre o cuidado prestado ao idoso e suas percepções sobre o exercício dessa função. Os resultados revelaram como motivações para o cuidado: relação de proximidade e confiança, dever de cuidar, familiar mais disponível, proximidade da residência, inexistência de vagas e elevado custo das instituições de acolhimento e desejo do idoso permanecer na sua habitação. Os principais cuidados aos idosos são: hidratação, higiene, alimentação, administração terapêutica, companhia, apoio emocional, conforto, entretenimento, promoção da autonomia e dignidade. As necessidades identificadas pelas cuidadoras foram: apoio domiciliário, da segurança social e da entidade empregadora do cuidador, ajuda financeira, suporte psicológico e capacitação para cuidar do idoso. ABSTRACT Introduction: Primary immunodeficiencies (PID) manifest as increased susceptibility to infectious diseases and are associated with significant morbimortality. COVID-19 has a variable clinical presentation, with unpredictable impact on immunocompromised patients. Knowing how this population responds to vaccination may be important to adjust future approaches. Objectives: To analyse immunological response to anti-COVID-19 vaccination in patients with PID. Methods: Prospective study of a population of patients with PID, followed in an Immunoallergology Unit, who were vaccinated against COVID-19. Clinical records were consulted to assess demographic, clinical, and laboratory data. Determination of total immunoglobulin G (IgG) and IgG SARS-CoV-2 before vaccination and serial measurement of IgG anti-spike after vaccination was performed. Results: Twenty patients were included, 55% female, with a mean age of 36,8±13,1 years old, 60% with common variable immunodeficiency (CVID), 30% selective antibody deficiency, and 10% X-linked agammaglobulinemia (XLA). In CVID patients, the median CD19 basal count was 180 cells/μL (IQR, 60;372). Twelve patients with CVID/XLA were under replacement treatment with subcutaneous gammaglobulin. All patients presented negative pre-vaccinal serology, with a median total IgG of 419 mg/dL (IQR, 238;896); IgG anti-spike after the primary vaccinal scheme was positive in 75% [median 796 AU/mL (IQR, 107;3772)]. Concerning CVID subpopulation: IgG anti-spike was positive in 67% after the primary vaccinal scheme, 75% before the booster dose, and 100% afterward. Lower counts of CD19 cells correlated with lower levels of IgG anti-spike (95% CI=0.117-0.898; p=0.023). No patient reported significant adverse reactions to vaccination. Conclusions: Anti-COVID-19 vaccination showed good response and safety in patients with PID. In the CVID subpopulation, more severe cases with lower counts of CD19 cells presented lower antibody titles after vaccination. Booster dose enhanced immune response, suggesting the need for additional inoculations to ensure sustained protection against COVID-19.

RESUMO Fundamentos: As imunodeficiências primárias (IDP) manifestam-se por suscetibilidade aumentada a doenças infeciosas e estão associadas a elevada morbimortalidade. A COVID-19 tem apresentação clínica variável, com impacto imprevisível nos doentes imunocomprometidos. Conhecer a resposta à vacinação desta população poderá ser importante para adequar medidas no futuro. Objetivos: Analisar a resposta imunológica à vacinação contra a COVID-19 em doentes com IDP. Métodos: Estudo prospetivo de uma população de doentes com IDP, seguidos numa unidade de Imunoalergologia, vacinados contra a COVID-19. Consulta dos processos clínicos para colheita de dados demográficos, clínicos e laboratoriais. Determinação de imunoglobulina G (IgG) total e IgG SARS-CoV-2 antes da vacinação e doseamentos seriados de IgG anti-spike após vacinação. Resultados: Incluídos 20 doentes, 55% do sexo feminino, com idade média 36,8±13,1 anos, 60% com imunodeficiência comum variável (IDCV), 30% deficiência seletiva de anticorpos e 10% agamaglobulinemia ligada ao X (ALX). Nos doentes com IDCV, a contagem basal de células CD19 mediana foi 180 células/μL (IIQ, 60;372). Realizavam terapêutica de substituição com gamaglobulina subcutânea 12 doentes com IDCV/ALX. A serologia pré-vacinal foi negativa em todos os doentes, a IgG total mediana foi 419 mg/dL (IIQ, 238;896) e a IgG anti-spike após esquema vacinal primário foi apositiva em 75% [mediana 796 AU/mL (IIQ, 107;3772)]. Na subpopulação com IDCV: a IgG anti-spike foi positiva em 67% após esquema vacinal primário, 75% antes da dose de reforço e 100% após reforço. Níveis mais baixos de células CD19 correlacionaram-se com valores inferiores de IgG anti-spike (95% IC=0,117-0,898; p=0,023). Nenhum doente reportou reações adversas significativas à vacinação. Conclusões: A vacinação contra a COVID-19 apresentou boa resposta e segurança nos doentes com IDP. Na subpopulação com IDCV, formas mais graves com contagens reduzidas de células CD19 apresentaram títulos mais baixos de anticorpos após vacinação. A dose de reforço potenciou a resposta imune, sugerindo a necessidade de inoculações adicionais para conferir proteção sustentada contra a COVID-19.

Abstract Human myiasis is the infestation of humans with dipterous larvae. Cutaneous myiasis is the most common form and is one of the most common travel-associated skin diseases. The condition should be suspected when furuncle or boil-like skin lesions are observed in patients travelling from endemic regions. Herein is described the case of a toddler who had travelled from Guinea-Bissau seven days earlier and presented with a six-day history of painful furuncle-like swellings on the left thigh and scrotum. Two larvae were collected from one of the lesions, and entomological analysis confirmed the diagnosis of cutaneous myiasis. Complete larvae extraction is the primary treatment goal, and adequate preservation of the maggots is essential for public health analysis.

Resumo A miíase humana é a infestação de humanos por larvas de dípteros. A forma cutânea é a mais frequente apresentação da doença e também uma das doenças dermatológicas do viajante mais comuns. Deve haver suspeita de miíase cutânea quando são observadas lesões do tipo furúnculo ou bolha em doentes que estiveram numa região endémica. É descrito o caso de um rapaz de 17 meses oriundo da Guiné-Bissau que se apresentou com lesões do tipo furúnculo dolorosas na coxa esquerda e escroto com seis dias de evolução. Foram recuperadas duas larvas numa das lesões, cuja análise entomológica confirmou o diagnóstico de miíase cutânea. A extração completa das larvas é o principal objetivo do tratamento e a sua correta preservação para análise é fundamental numa perspetiva de saúde pública.

Abstract Neuroendocrine tumors (NETs) are the most common primary lung neoplasms. Clinical manifestations can be subtle, and chest radiograph usually shows non-specific findings, often leading to delayed or inaccurate diagnosis. Despite their indolent nature, NETs have metastatic potential and can cause significant morbidity. Early diagnosis is critical for optimal management, as surgical resection can be curative. The challenge is to correctly diagnose this rare entity while avoiding unnecessary testing. Patients with suspected NETs should be referred to a tertiary hospital for multidisciplinary evaluation, and bronchoscopy should not be delayed. Herein is reported the case of an adolescent with asthma who presented with nonspecific respiratory and constitutional symptoms. The persistence of symptoms and imaging alterations despite multiple treatments for recurrent pneumonia raised clinical suspicion of a more serious underlying condition, leading to the diagnosis of bronchial NET.

Resumo Os tumores neuroendócrinos (TNEs) são as neoplasias pulmonares primárias mais comuns. O diagnóstico pode ser desafiante, pois as manifestações clínicas são geralmente subtis e as alterações imagiológicas inespecíficas. Apesar da sua natureza indolente, os TNEs têm potencial metastático e podem causar morbilidade significativa. O diagnóstico precoce é fundamental para uma abordagem otimizada, uma vez que a ressecção cirúrgica pode ser curativa. O desafio é estabelecer o diagnóstico desta entidade rara evitando avaliações e exames complementares desnecessários. Perante suspeita de TNE, o doente deve ser referenciado para um hospital terciário para avaliação multidisciplinar e realização de broncoscopia. É descrito o caso de um adolescente com asma que se apresentou com sintomas respiratórios e constitucionais inespecíficos. A persistência dos sintomas e alterações imagiológicas apesar da realização de múltiplos tratamentos por pneumonia recorrente motivou a continuação da investigação etiológica e possibilitou o estabelecimento do diagnóstico de TNE endobrônquico.

Abstract Objective: To evaluate the success rate of high-flow nasal cannula (HFNC) therapy using an adapted obsolete mechanical ventilator (MV), Optiflow™ and Vapotherm™ in newborns (NBs). Method: This was a retrospective observational study conducted in the neonatal intensive care unit (NICU). The sample comprised NBs who underwent HFNC therapy due to ventilatory dysfunction, for weaning from non-invasive ventilation (NIV), or post-extubation. The three groups, stratified according to gestational age (GA) and birth weight, and corrected GA and weight at the beginning of HFNC use, were as follows: Optiflow ™, Vapotherm ™, and obsolete Mechanical Ventilator (MV) adapted for high flow therapy. Subsequently, the NBs were divided into a success group (SG) and a failure group (FG). HFNC success was defined as a therapy duration exceeding 72 h. Results: A total of 209 NBs were evaluated, with 31.1 % using HFNC due to ventilatory dysfunction, 2.4 % after extubation, and 66.5 % after NIV weaning. HFNC success rate was observed in 90.9 % of the NBs, with no difference between equipment types (Vapotherm ™, Optiflow ™, and adapted VM). Conclusion: Different types of HFNC equipment are equally effective when used in neonatology for respiratory dysfunction, as a method of weaning from NIV and post-extubation. Abstract Background: The European Society of Cardiology guidelines recommend an LDL-cholesterol (LDL-C) < 55 mg/dL for patients with established cardiovascular disease. While the Friedewald equation to estimate LDL-C is still widely used, the newer Martin-Hopkins equation has shown greater accuracy. Objectives: We aimed to assess: A) the proportion of patients reaching LDL-C goal and the therapies used in a tertiary center; B) the impact of using the Martin-Hopkins method instead of Friedewald’s on the proportion of controlled patients. Methods: A single-center cross-sectional study including consecutive post-myocardial infarction patients followed by 20 cardiologists in a tertiary hospital. Data was collected retrospectively from clinical appointments that took place after April 2022. For each patient, the LDL-C levels and attainment of goals were estimated from an ambulatory lipid profile using both Friedewald and Martin-Hopkins equations. A two-tailed p-value of < 0.05 was considered statistically significant for all tests. Results: Overall, 400 patients were included (aged 67 ± 13 years, 77% male). Using Friedewald’s equation, the median LDL-C under therapy was 64 (50-81) mg/dL, and 31% had LDL-C within goals. High-intensity statins were used in 64% of patients, 37% were on ezetimibe, and 0.5% were under PCSK9 inhibitors. Combination therapy of high-intensity statin + ezetimibe was used in 102 patients (26%). Applying the Martin-Hopkins method would reclassify a total of 31 patients (7.8%). Among those deemed controlled by Friedewald’s equation, 27 (21.6%) would have a Martin-Hopkins’ LDL-C above goals. Conclusions: Less than one-third of post-myocardial infarction patients had LDL-C within the goal. Applying the Martin-Hopkins equation would reclassify one-fifth of presumably controlled patients into the non-controlled group. Resumo Fundamento: As diretrizes da Sociedade Europeia de Cardiologia recomendam um nível de colesterol LDL (LDL-C) < 55 mg/dL para pacientes com doença cardiovascular estabelecida. Embora a fórmula de Friedewald ainda seja amplamente utilizada para estimar o LDL-C, a fórmula mais recente de Martin-Hopkins mostrou maior precisão. Objetivos: Nosso objetivo foi avaliar: A) a proporção de pacientes que atingiram a meta de LDL-C e as terapias utilizadas em um centro terciário; B) o impacto da utilização do método de Martin-Hopkins em vez do método de Friedewald na proporção de pacientes controlados. Métodos: Estudo transversal monocêntrico, incluindo pacientes consecutivos pós-infarto do miocárdio, acompanhados por 20 cardiologistas, em um hospital terciário. Abstract Ctenocephalides felis felis is a relevant and widely distributed ectoparasite that acts as a vector of disease-causing pathogens. Moreover, it is responsible for economic losses due the use of harmful chemicals to the environment and that favor the emergence of insecticide-resistant populations. Research on entomopathogenic nematodes may open up an alternative route to the insect chemical control. The present study aimed to evaluate the killing efficacy of Heterorhabditis bacteriophora (HP88) against C. felis larvae in 400 μL, 600 μL and 1000 μL of suspension containing 120, 160 and 200 infective juveniles/larva and 600 μL of suspension containing the same concentrations of Heterorhabditis indica (LPP30), divided into two groups (absence and presence of diet) and a control group with three replications containing only distilled water. In the bioassay with H. bacteriophora, the groups in 600 μL of suspension showed higher mortality rates than those in the other tested volumes, which were above 80% at all concentrations. On the other hand, H. indica achieved mortality rates above 70% at all concentrations used. Resumo Ctenocephalides felis felis é um ectoparasito de relevância, ampla distribuição geográfica, vetor de patógenos causadores de doenças e responsável por perdas econômicas relacionadas ao uso de produtos químicos que causam impacto ambiental e aparecimento de populações resistentes. Pesquisas com nematoides entomopatogênicos podem ser uma alternativa ao controle químico desse inseto. O presente estudo objetivou avaliar a eficácia de Heterorhabiditis bacteriophora (HP88) em larvas de C. felis em 400 μL, 600 μL e 1000 μL de suspensão contendo 120, 160 e 200 juvenis infectantes/larva e 600 μL de suspensão contendo as mesmas concentrações de Heterorhabditis indica (LPP30), divididos em dois grupos (ausência e presença de dieta) e grupo controle com três repetições contendo apenas com água destilada. No bioensaio com H. bacteriophora observou-se que os grupos com 600 μL de suspensão apresentaram melhores percentuais de mortalidade em relação aos outros volumes testados, sendo estas acima de 80% para as três concentrações. Abstract The blend of butadiene and acrylonitrile copolymer (NBR) with natural poly-cis-isoprene (NR) shows increased resistance to swelling in solvents in comparison to the individual components. In aerospace, NBR rubber is used as thermal protection for rockets and shall not contain other polymers, even in low contents, otherwise, it can affect the protection performance and rocket safety by causing detachment of the elastomer/propellant interface; therefore, this investigation presents methodologies to determine the NR/NBR contents. This study explores different analytical techniques, such as Raman spectroscopy and Fourier transform infrared (FTIR) spectroscopy, in the mid-infrared (MIR) by reflection and in the near-infrared by reflectance (NIRA) modes, Furthermore, quantification strategies by univariate, bivariate and multivariate (chemometric) models are evaluated and compared. A proposed methodology, based on multivariate Raman microscopy with partial least squares regression (PLS), showed high linearity (R2 > 0.99) and low error (< 0.82 %). The validation of FT-MIR data for the CH3, which presented lower error (1.3%) than vinylidene band (6%), showed that both methodologies (reflection and NIRA reflectance) can be used for the quantification of NR in NR/NBR. ABSTRACT Introduction: The complexity of liver transplantation requires a highly qualified team, in which the social worker plays a crucial role in analyzing and intervening in the social situation of candidates. Objectives: To investigate the social profile of liver transplant candidates and relate it to the interventions and reflections made during the social assessment at the Hospital de Base Liver Transplant Unit. Methods: Based on the records of the Social Work, the information of the candidates evaluated between January 2019 and December 2020 was analyzed. This quantitative-qualitative, retrospective, descriptive and documentary study, with participant observation, was conducted from a dialectical perspective. Results: During the data collection period, 174 evaluations were obtained. Social profile: Average age 55.8 years, predominantly male (N=116; 66.7%), with a partner (N=129; 74.1%), living in municipalities in the state of São Paulo (N=124; 71.3%), incomplete primary education (N=68; 39.1%), low level of education (N=65; 37.4%), inactive in the job market (N=151; 86.8%), accessing social security benefits (N=120; 69%), positive acceptance of the transplant (N=158; 90.8%), nuclear family (N=120; 69%), offer of care and family adherence (N=172; 98.9%), partial access to medication (N=122; 70.1%), ease of access to the transplant center (N=157; 90.2%), per person family income of 1\2 to 2 minimum wages (N=107; 61.5%) and satisfactory/conserved housing standard and state of repair (N=157; 90.3%). Conclusion: The social profile of greater social vulnerability required more interventions in most of the 25 variables assessed, providing important elements for identifying and meeting the social needs of each individual. RESUMO Introdução: A complexidade do transplante de fígado requer uma equipe altamente qualificada, no qual o assistente social desempenha papel crucial para analisar e intervir na situação social dos candidatos. Objetivos: Investigar o perfil social dos candidatos ao transplante de fígado e relacioná-lo com as intervenções e reflexões efetuadas durante a avaliação social na Unidade de Transplante de Fígado do Hospital de Base. Métodos: Com base nos registros do Serviço Social, foram analisadas as informações dos candidatos avaliados entre janeiro de 2019 e dezembro de 2020. Este estudo quantitativo-qualitativo, retrospectivo, descritivo e documental, com observação participante, foi conduzido a partir de um panorama dialético. Resultados Durante o período de coleta de dados, obteve-se174 avaliações. Perfil social: média etária de 55,8 anos, predominância masculina (N=116; 66,7%), com companheiro/a (N=129; 74,1%), residentes em municípios do estado de São Paulo (124; 71,3%), ensino fundamental incompleto (N=68, 39,1%), baixo nível de instrução (N=65; 37,4%), inatividade no mercado de trabalho (N=151; 86,8%), acessando benefício da previdência social (N=120; 69%), positiva aceitação do transplante (N=158; 90,8%), família nuclear (N=120; 69%), oferta de cuidados e aderência familiar (N= 172;98,9%), acesso parcial a medicamentos (N=122; 70,1%), facilidade de acesso ao centro transplantador (N=157; 90,2%), renda per capita familiar de 1\2 a 2 salários mínimos (N=107; 61,5%) e padrão habitacional e estado de conservação satisfatório/conservado (N=157; 90,3%). Conclusão O perfil social de maior vulnerabilidade social exigiu mais intervenções na maioria das 25 variáveis avaliadas, fornecendo elementos importantes para a identificação e atendimento das necessidades sociais de cada indivíduo. Abstract As historical waste has accumulated in dams, the interest in studying the feasibility of reusing mining tailings has grown, offering both environmental and economic advantages. However, given the unique characteristics of each site, conducting site-specific assessments is crucial. Building on a detailed evaluation of the Cocoruto Dam in Nova Lima, Minas Gerais, this study presents a technical feasibility and financial viability analysis of three potential reprocessing scenarios. The primary objective was to assess the viability and potential benefits of reusing Au tailings. The proposed methods involve grinding, calcination and leaching, with designs adaptable for existing metallurgical plants. Considering the economic potential of Au in these inactive mine tailings (with an average grade of 0.95 mg/kg and a total resource of 3,350.55 tonnes), multiple factors were analyzed to determine the feasibility of Au production from this source. Analyzed variables included: cut-off grades and tonnage for each scenario; net present value (NPV) calculated with an 8% annual discount rate; a general slope angle of 20 degrees; three possible final pit configurations; different annual production rates (400,000 t, 800,000 t and 1,000,000 t), and various Au ounce values ($1300/oz, $1500/oz, and $1700/oz). Abstract As it is an infrequent etiology, the diagnosis of perianal tuberculosis is challenging, especially in the absence of a pulmonary focus. TB should be considered in the differential diagnosis of perianal ulcers, fistulas, abscesses, mainly in non-healing and recurrent anal lesions. Treatment with anti-TB agents can provide complete recovery. Furthermore, these lesions are often diagnosed later after complete histopathological and mycobacterial results, where the benefit of avoiding morbid multiple surgeries by effective anti-TB treatment is lost. We reported a rare case of an immuno-competent patient with perianal TB, which was the first manifestation of the disease. A fit-and-well man in his 20s presented a large perianal abscess. Unexpectedly, his chest X-ray showed a rounded hyper-transparency in the left lung. The abscess was drained. Posterior investigation with culture analysis from pus swabs and sputum revealed the presence of Mycobacterium tuberculosis complex infection. After completing the 6 months of oral administration of anti-TB drugs, the patient was asymptomatic. Resumo O cuidador informal presta cuidados permanentes ou regulares a pessoas idosas em situação de dependência, sem remuneração. Objetivou-se identificar as percepções de cuidadores informais sobre motivações, necessidades e benefícios do cuidado ao idoso dependente. Estudo qualitativo realizado com 10 cuidadoras informais portuguesas, a partir de um instrumento com questões sobre o cuidado prestado ao idoso e suas percepções sobre o exercício dessa função. Os resultados revelaram como motivações para o cuidado: relação de proximidade e confiança, dever de cuidar, familiar mais disponível, proximidade da residência, inexistência de vagas e elevado custo das instituições de acolhimento e desejo do idoso permanecer na sua habitação. Os principais cuidados aos idosos são: hidratação, higiene, alimentação, administração terapêutica, companhia, apoio emocional, conforto, entretenimento, promoção da autonomia e dignidade. As necessidades identificadas pelas cuidadoras foram: apoio domiciliário, da segurança social e da entidade empregadora do cuidador, ajuda financeira, suporte psicológico e capacitação para cuidar do idoso. Os benefícios do cuidado informal para o idoso dependente apontados foram: celeridade do apoio familiar, segurança física e emocional, afeto e companheirismo. Este estudo dá voz a cidadãos cruciais.

Abstract The informal caregiver provides non-remunerated permanent or regular care to dependent older adults. This qualitative study aimed to identify the perceptions of informal caregivers about motivations, needs, and benefits of caring for dependent older adults. It was conducted with ten Portuguese informal caregivers, based on an instrument with questions about the care provided to older adults and their perceptions about performing this role. The results revealed the following motivations for care: proximity and trust relationship, duty of care, more available family members, home proximity, lack of vacancies, high cost of shelter institutions, and older adults’ desire to remain in their homes. The primary care activities for older adults are hydration, hygiene, food, therapeutic administration, companionship, emotional support, comfort, entertainment, and promoting autonomy and dignity. The needs identified by the caregivers were home, social security, and the caregiver’s employer support, financial help, psychological support, and training to care for the older adults. The benefits of informal care for dependent older adults were prompt family support, physical and emotional security, affection, and companionship. This study gives voice to crucial citizens.

ABSTRACT The cultivation of beans (Phaseolus vulgaris L.) is of great importance to the national economy since they are grown in almost all regions of Brazil and the use of biostimulants based on phytohormones and plant vitamins can improve the yield of the crop. This work aimed to evaluate the effects of the application of the vitamin nicotinamide and phytohormones as biostimulants on bean plants. The experimental design was carried out in randomized blocks in a 2 x 5 factorial scheme with 4 replicates. The treatments consisted of the application of five doses of nicotinamide (0, 200, 400, 600 and 800 mg L-1) in the presence or absence of phytohormones containing auxin, gibberellin and cytokinin. The growth characteristics, production components and productivity of the bean grains were evaluated. The use of phytohormones and nicotinamide alone or together favored the growth, production components and grain productivity of the bean plant. The range of 366 to 469 mL L-1 nicotinamide achieved the highest values for the growth variables, production components and grain productivity of the bean plant, with 414 mL L-1 being the most suitable to achieve higher grain productivity.
