Abstract Objective: To provide guidelines on the coronavirus disease 2019 (COVID-19) vaccination in patients with immune-mediated rheumatic diseases (IMRD) to rheumatologists considering specific scenarios of the daily practice based on the shared-making decision (SMD) process. Methods: A task force was constituted by 24 rheumatologists (panel members), with clinical and research expertise in immunizations and infectious diseases in immunocompromised patients, endorsed by the Brazilian Society of Rheumatology (BSR), to develop guidelines for COVID-19 vaccination in patients with IMRD. A consensus was built through the Delphi method and involved four rounds of anonymous voting, where five options were used to determine the level of agreement (LOA), based on the Likert Scale: (1) strongly disagree; (2) disagree, (3) neither agree nor disagree (neutral); (4) agree; and (5) strongly agree. Nineteen questions were addressed and discussed via teleconference to formulate the answers. In order to identify the relevant data on COVID-19 vaccines, a search with standardized descriptors and synonyms was performed on September 10th, 2021, of the MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov, and LILACS to identify studies of interest. We used the Newcastle–Ottawa Scale to assess the quality of nonrandomized studies. Results: All the nineteen questions-answers (Q&A) were approved by the BSR Task Force with more than 80% of panelists voting options 4—agree—and 5—strongly agree—, and a consensus was reached. These Guidelines were focused in SMD on the most appropriate timing for IMRD patients to get vaccinated to reach the adequate covid-19 vaccination response. Conclusion: These guidelines were developed by a BSR Task Force with a high LOA among panelists, based on the literature review of published studies and expert opinion for COVID-19 vaccination in IMRD patients. Noteworthy, in the pandemic period, up to the time of the review and the consensus process for this document, high-quality evidence was scarce. Thus, it is not a substitute for clinical judgment.

ABSTRACT Background and aims: Treatment of hepatitis C with direct antiviral agents (DAA) is associated with almost 95% of sustained virological response. However, some patients need retreatment. In Brazil, it should be done according to the Ministry of Health guidelines, frequently updated to include newly available drugs. This study aimed to conduct a national survey about the characteristics and outcomes of retreatment of hepatitis C in previously non-responders to DAAs. Patients and methods: Institutions from all over the country were invited to participate in a national registry for retreatment, including information about clinical and epidemiological characteristics of the patients, type and outcomes of retreatment regimens. Only patients previously treated with interferon-free regimens were included. Results: As previous treatments the distribution was: SOF/DCV (56%), SOF/SIM (22%), 3D (11%), SOF/LED (6%) and SOF/RBV (5%). For retreatment the most frequently used drugs were SOF/GP (46%), SOF/DCV (23%) and SOF/VEL (11%). From 159 patients retreated, 132/159 (83%) had complete information in the registry and among them only seven patients were non-responders (SVR of 94.6%). All retreatments were well tolerated, without any serious adverse events or interruptions. Conclusion: The retreatment of patients previously non-responders to DAAs was associated with high rate of SVR in this sample of Brazilian patients. This finding allows us to conclude that the retreatment options available in the public health system in Brazil are effective and safe and are an important component of the strategy of elimination of hepatitis C in our country.

RESUMO Objetivo Investigar a tendência da mortalidade infantil por causas evitáveis em crianças menores de um ano em Rondônia de 2008 a 2018. Método Estudo epidemiológico de série temporal com dados dos sistemas de Informação sobre Mortalidade e sobre Nascidos Vivos, analisados pelo STATA® versão 11.0, tendência aferida por regressão linear de Prais-Winsten e autocorrelação com teste de Durbin e Watson. Resultados A taxa de mortalidade infantil foi de 14,57 óbitos/1.000 nascidos vivos, 9,14/1.000 por causas evitáveis. Óbitos tiveram decréscimo anual de 2,88% (IC95%:-4,67; -1,06). Entretanto, causas reduzíveis por ações adequadas de imunização, à mulher na gestação e crescimento fetal e no parto, tiveram taxas estáveis. Óbitos por causas evitáveis no período neonatal tardio estiveram em declínio (-11,69%; IC95%-19, 56;-3,05). Conclusão Gestores precisam qualificar o cuidado materno-infantil, considerando a atuação da equipe de profissionais na assistência ao ciclo gravídico-puerperal bem como melhorar a qualidade dos registros de mortalidade infantil na região.

ABSTRACT Objective To investigate the trend of infant mortality from preventable causes in children under one year of age in Rondônia from 2008 to 2018. Method Epidemiological study of time series with data from the Mortality and Live Birth Information Systems, driven by STATA® version 11.0, trend measured by Prais-Winsten linear regression and autocorrelation with Durbin and Watson test. Results The infant mortality rate was 14.57 deaths / 1,000 live births, 9.14 / 1,000 due to preventable causes. Deaths decreased by 2.88% annually (95% CI: - 4.67; -1.06). However, causes that can be reduced by running immunization actions, to women during pregnancy and fetal and non-childbirth growth, have stable rates. Deaths from preventable causes in the late neonatal period were declining (-11.69%; 95% CI -19.56; -3.05). Conclusion Managers need to qualify maternal and child care, considering the performance of the team of professionals in assisting the pregnant-puerperal cycle as well as improving the quality of infant mortality records in the region.

RESUMEN Objetivo Investigar la tendencia de la mortalidad infantil por causas prevenibles en menores de un año en Rondônia de 2008 a 2018. Método Estudio epidemiológico de series de tiempo con datos de los Sistemas de Información de Mortalidad y Nacidos Vivos, impulsado por STATA® versión 11.0, tendencia medida por regresión lineal de Prais-Winsten y autocorrelación con la prueba de Durbin y Watson. Resultados La tasa de mortalidad infantil fue de 14,57 defunciones / 1.000 nacidos vivos, 9,14 / 1.000 por causas prevenibles. Las muertes disminuyeron en un 2,88% anual (IC del 95%: -4,67; -1,06). Sin embargo, las causas que pueden reducirse ejecutando acciones de inmunización, a las mujeres durante el embarazo y el crecimiento fetal y no durante el parto, tienen tasas estables. Las muertes por causas prevenibles en el período neonatal tardío estaban disminuyendo (-11,69%; IC del 95% -19,56; -3,05). Conclusión Es necesario que los gestores califiquen la atención materno-infantil, considerando el desempeño del equipo de profesionales en la atención al ciclo gestante-puerperal y mejorando la calidad de los registros de mortalidad infantil en la región.

RESUMEN Objetivo: Este estudio tiene como objetivo: describir las intervenciones del enfermero de rehabilitación en términos de Kinesiterapia Respiratoria en los pacientes con COVID-19 sobre ventilación mecánica y oxigenación com membrana extracorpórea, analizando la respuesta en términos de parámetros gasométricos y ventilatorios antes y después de cada sesión. Método: Estudio de caso de un paciente de 63 años con antecedentes patológicos de obesidad y hipertensión arterial. Se recopilaron datos retrospectivos del proceso clínico, durante una permanência en UCI de 38 días. Resultados: Las técnicas más utilizadas en Kinesiterapia Respiratoria fueron, la aspiración de secreciones en circuito cerrado, vibraciones, descompresión repentina, aberturas costales selectivas, espiración manual forzada, ventilación dirigida y in-exsuflador mecánico. En todas las sesiones, hubo una mejoria en los volúmenes inspiratorio y espiratorio, así como en la compliance estática. El in-exsuflador mecánico se inició después de una semana de hospitalización, después de agotarse todas las intervenciones posibles, siendo que todos los resultados mostraron mejoras globales, sin eventos adversos. Conclusión: La implementación de intervenciones de kinesiterapia respiratoria estructuradas e individualizadas puede contribuir significativamente en la recuperación de un paciente en una situación crítica, habiendo sido utilizadas técnicas que aín no se han descrito para los casos de COVID-19 de manera efectiva.

RESUMO Objetivo: Este estudo tem como objetivo: descrever as intervenções do enfermeiro de reabilitação em termos de Cinesiterapia Respiratória na pessoa com COVID-19, com ventilação mecânica e em oxigenação extra-corpórea por membrana, analisando a resposta em termos de parâmetros gasométricos e ventilatórios antes e após cada sessão. Método: Estudo de caso de uma pessoa com 63 anos, com antecedentes de obesidade e hipertensão arterial. Realizou-se colheita de dados retrospetiva de processo clínico, referente a um período de internamento em UCI de 38 dias. Resultados: As técnicas mais utilizadas na Cinesiterapia Respiratória foram a aspiração de secreções em circuito fechado, vibrações, descompressão brusca, aberturas costais seletivas, expiração manual forçada, ventilação dirigida e in-exsuflador mecânico. Na globalidade das sessões, verificou-se uma melhoria nos volumes inspiratórios e expiratórios, assim como melhoria na compliance estática. Começou a usar-se o in-exsuflador mecânico ao fim de uma semana de internamento, depois de se esgotarem todas as intervenções possíveis, sendo que todos osoutcomesapresentaram melhorias globais, sem eventos de adversos. Conclusão: A implementação de intervenções de cinesiterapia respiratória estruturadas e individualizadas, podem contribuir significativamente na recuperação da pessoa em situação crítica, tendo-se usado com eficácia técnicas que não estavam descritas para casos COVID- 19.

ABSTRACT Aim: This study aims to describe rehabilitation nurse interventions in Respiratory Kinesitherapy in critically ill with COVID-19 patient undergoing venovenous extra-corporeal membrane oxygenation and its response concerning to gasometric and ventilatory parameters before and after each session. Methodology: Case study of a 63-year-old person with history of obesity and high blood pressure. Retrospective data were collected from the clinical process, referring to a 38-day ICU stay. Results: The most performed techniques used in Respiratory Kinesitherapy by rehabilitation nurses were suctioning through closed system, vibrations, sudden decompression, selective costal openings, forced manual expiration, directed ventilation and mechanical in-exhaler. Globally, all sessions showed that there was an improvement in inspiratory and expiratory volumes, as well as in static compliance. Machanical in-exhaler was used after a week of hospital stay, after all other possible interventions were performed, and all outcomes showed global improvements, without any adverse events. Conclusion: Implementation of structured and individualized respiratory kinesitherapy interventions can significantly benefit recovery of critical ill patients. Techniques that weren´t described for covid 19 cases were used effectively.

ABSTRACT Objective: To identify the causes and trends of hospitalizations due to primary care-sensitive conditions in children under five years in Rondônia, Brazil, from 2008 to 2017. Method: Time series. Secondary data on hospitalizations available in the Hospital Information System between January and March 2018 were used. The trend analysis was based on the Prais-Winsten linear regression model. Results: Infectious gastroenteritis was the main cause of hospitalization at all ages. Children under one year had the highest rates of hospital admissions, and the causes were lung disease, ear, nose and throat infection, kidney and urinary tract infection, with an increasing trend of prenatal and childbirth diseases. Hospitalizations due to epilepsy, skin infection and subcutaneous tissue showed an increasing trend at all ages. Conclusion: The high rates of hospital admissions for children reflect the fragility of the care network. This study contributes to the improvement of public child health policies in Primary Health Care.

RESUMEN: Objetivo: identificar las causas y tendencias de internaciones por condiciones sensibles a la atención básica en niños con menos de cinco años en Rondônia, Brasil, de 2008 a 2017. Método: serie temporal, con datos secundarios de internaciones del Sistema de Informaciones Hospitalarias, entre enero y marzo de 2018. El análisis de tendencia se basó en la regresión lineal de Prais-Winsten. Resultados: las gastroenteritis infecciosas fueron la principal causa de internación en todas las edades. Las mayores tajas ocurrieron en niños con menos de un año por enfermedades pulmonares, infección de oído, nariz y garganta, infección de riñones y tracto urinario, con tendencia creciente de las enfermedades en el prenatal y parto. Las internaciones por epilepsia, infección de piel y tejido subcutáneo presentaron tendencia creciente en todas las edades. Conclusión: las elevadas tajas de internaciones en niños reflecten la fragilidad de la red asistencial. Este estudio contribuye con políticas públicas de salud infantil en la Atención Básica en Salud.

Método: série temporal, com dados secundários de internações do Sistema de Informações Hospitalares, entre janeiro e março de 2018. A análise da tendência foi baseada na regressão linear de Prais-Winsten. Resultados: as gastroenterites infecciosas foram a principal causa de internação em todas as idades. As maiores taxas ocorreram em menores de um ano por doenças pulmonares, infecção do ouvido, nariz e garganta, infecção do rim e trato urinário, com tendência crescente das doenças no pré-natal e parto. As internações por epilepsia, infecção de pele e tecido subcutâneo tiveram tendência crescente em todas as idades. Conclusão: as elevadas taxas de internações em crianças refletem a fragilidade da rede assistencial. Este estudo contribui com as políticas públicas de saúde infantil na Atenção Primária em Saúde.

Abstract Problem Providing health care for children with congenital heart diseases remains a major challenge in low- and middle-income countries. Approach In October 2011, the Government of Paraíba, Brazil, established a paediatric cardiology network in partnership with the nongovernmental organization Círculo do Coração. A cardiology team supervised all network activities, using the Internet to keep in contact with remote health facilities. The network developed protocols for screening heart defects. Echocardiograms were performed by physicians under direct online supervision of a cardiologist; alternatively, a video recording of the examination was subsequently reviewed by a cardiologist. Cardiovascular surgeons came to a paediatric hospital in the state capital once a week to perform heart surgeries. Local setting Until 2011, the State of Paraíba had no structured programme to care for children with heart disease. This often resulted in missed or late diagnosis, with adverse health consequences for the children. Relevant changes From 2012 to 2014, 73 751 babies were screened for heart defects and 857 abnormalities were identified. Detection of congenital heart diseases increased from 4.09 to 11.62 per 1000 live births ( P< 0.001). Over 6000 consultations and echocardiograms were supervised via the Internet. Time to diagnosis, transfers and hospital stays were greatly reduced. A total of 330 operations were carried out with 6.7% (22/330) mortality. Lessons learnt Access to an echocardiography machine with remote supervision by a cardiologist improves the detection of congenital heart disease by neonatologists; virtual outpatient clinics facilitate clinical management; the use of Internet technology with simple screening techniques allows resources to be allocated more efficiently.

This cross-sectional, retrospective research used the technique of documental research. Its objective was to characterize the hospitalization of drug addicts in a rehabilitation unit. Data was collected from 350 patient records belonging to drug addicts, referring to 2010. Regarding the characterization of the subjects, the average age was 35.8 years, 39.4% were single, 67.3% had only studied at primary school level and 45.1% were unemployed. Alcohol was the motive for treatment in 45.1% of the patients; 49.4% were diagnosed as dependent on multiple drugs. Hospitalization was voluntary for 97.1%; 40.4% of those responsible were the parents; clinical discharge predominated in 55.4% and, after discharge, 86% were referred to community services. Among the participants, 71.7% had been hospitalized before and 29.9% had undertaken other types of treatment. The results demonstrate that to aim for permanent abstinence, it is essential to maintain treatment after the hospitalization.

Investigación transversal y retrospectiva que utilizó la técnica de investigación documental, cuyo objetivo fue caracterizar las internaciones de dependientes químicos en una unidad de rehabilitación. Fueron obtenidos datos de 350 prontuarios de dependientes químicos que se refieren al año de 2010. Cuanto a la caracterización de los sujetos, la media etaria fue de 35,8 años, 39,4% estaban solteros, 67,3% estudiaron hasta la enseñanza fundamental y 45,1% estaban desempleados. El álcohol motivó la búsqueda por tratamiento en 45,1% de los pacientes; 49,4% fueron diagnosticados por la dependencia de múltiplas drogas. La internación para 97,1% fue voluntaria; 40,4% de los responsables fueron los padres; el alta clínica prevaleció en 55,4% y, después del alta, 86% fueron encaminados a servicios extrahospitalares. Entre los participantes, 71,7% tuvieron internamientos anteriores y 29,9% realizaron otros tipos de tratamiento. Los resultados demuestran que es imprescindible la manutención del tratamiento después de la internación para la búsqueda de la abstinencia permanente.

Foram coletados dados de 350 prontuários de dependentes químicos, referentes ao ano de 2010. Quanto à caracterização dos sujeitos, a média etária foi de 35,8 anos, 39,4% estavam solteiros, 67,3% cursaram até o ensino fundamental e 45,1% eram desempregados. O álcool motivou a busca por tratamento em 45,1% dos pacientes; 49,4% foram diagnosticados pela dependência de múltiplas drogas. A internação para 97,1% foi voluntária; 40,4% dos responsáveis foram os pais; a alta clínica prevaleceu em 55,4% e, após alta, 86% foram encaminhados a serviços extra-hospitalares. Dentre os participantes, 71,7% tiveram internamentos anteriores e 29,9% realizaram outros tipos de tratamento. OBJETIVO: Verificar a qualidade higiênico-sanitária da água de consumo humano em propriedades rurais por meio da contagem de indicadores microbiológicos de potabilidade. MÉTODOS: Foram colhidas 180 amostras de água utilizada para consumo humano das fontes, reservatórios e ponto de consumo em 30 propriedades rurais, situadas na região Nordeste do Estado de São Paulo. Determinou-se o número mais provável de coliformes totais, Escherichia coli e o número de microrganismos mesófilos. Foi verificada a presença de medidas de proteção das fontes de abastecimento. RESULTADOS: Os resultados evidenciaram que 90% das amostras de água das fontes, 90% dos reservatórios e 96,7% de água de consumo humano, colhidas no período de chuvas, e 83,3%, 96,7% e 90%, daquelas colhidas respectivamente nos mesmos locais, durante a estiagem, estavam fora dos padrões microbiológicos de potabilidade para água de consumo humano. CONCLUSÕES: A água utilizada nas propriedades rurais foi considerada um importante fator de risco à saúde dos seres humanos que a utilizam. A adoção de medidas preventivas, visando à preservação das fontes de água, e o tratamento das águas já comprometidas são as ferramentas necessárias para diminuir consideravelmente o risco de ocorrência de enfermidades de veiculação hídrica.

OBJECTIVE: To assess the sanitary quality of drinking water in rural farms through counts of microbiological indicators. METHODS: A total of 180 drinking water samples from sources, reservoirs and water from site of consumption were collected in 30 rural farms located in the northeast region of the sate of São Paulo. The most probable number of total coliforms, Escherichia coli and mesophilic microorganisms were determined. Also, the presence of protection measures for water supplies wase verified. RESULTS: The study results showed that 90.0% of drinking water samples from sources, 90.0% from reservoirs, and 96.7% from sites of consumption, collected during the rainy season, and 83.3%, 96.7% and 90.0% of samples collected in dry season were below the quality control standards for drinking water. CONCLUSIONS: Drinking water in rural farms was considered a potential human health threat. Preventive measures for protecting water sources and water treatment are necessary to significantly reduce the occurrence of waterborne diseases.
