Abstract Background Chagas disease (CD) is a serious public health problem in Latin America. Benznidazole (BNZ) is used for the treatment of CD and, despite its wide use, little information is available about its toxicity and mechanisms of adverse drug reactions (ADR). Objectives To identify and classify clinical and laboratory adverse reactions caused by BNZ in terms of causality and severity. Methods Prospective cohort study from January 2018 to December 2021. Treatment follow-up included visits and biochemical tests (complete blood count, liver and kidney function tests) before, during and after treatment. ADR were classified according to causality and severity. In the statistical analysis, the significance level was set at p<0.05. Results Forty patients with chronic CD were included. A high prevalence of ADR was observed 161 ADR in 30 patients [90%]; of these, 104 (64.6%) were classified as possible and 57 (35.4%) as probable. The ADR were classified as moderate and mild. Of the 40 patients, nine (22.5%) discontinued treatment. ADR associated with treatment discontinuation and interventions were those that affected the dermatological system, central and peripheral nervous system and sense organs such as ageusia. Mild hematological and biochemical changes such as lymphopenia were observed after 30 days of treatment. Conclusion Many patients were able to complete the treatment even with ADR, which can be attributed to the successful follow-up strategy with symptomatic treatment and counseling, leading to patient’s awareness of symptoms and treatment adherence. Resumo Fundamento A Doença de Chagas (DC) representa um grave problema de saúde pública na América Latina. O Benznidazol (BNZ) é utilizado para o tratamento DC e, apesar do seu amplo uso, poucas informações estão disponíveis sobre sua toxicidade e mecanismos das Reações Adversas ao Medicamento (RAM). Objetivos Identificar e classificar as reações adversas clínicas e laboratoriais ocasionadas pelo uso do BNZ quanto à sua causalidade e gravidade. Métodos Estudo de coorte prospectivo realizado no período de janeiro de 2018 a dezembro de 2021. O acompanhamento do tratamento incluiu consultas e análises laboratoriais antes, 30 e 60 dias após o início do tratamento. As RAM foram classificadas quanto à causalidade e gravidade. Na análise estatística o nível de significância adotado foi p<0,05. Resultados Participaram do estudo 40 pacientes com DC crônica, observou-se alta prevalência de RAM com um total de 161 em 30 (90%) pacientes. Destas, 104 (64,6%) foram classificadas como possíveis e 57 (35,4%) como prováveis. As reações foram classificadas em moderadas e leves. Dos 40 pacientes, nove (22,5%) suspenderam o tratamento. As RAM associadas à interrupção e intervenções foram as que afetaram o sistema dermatológico, sistema nervoso central e periférico ou que culminaram em ageusia. Após 30 dias de tratamento, alterações hematológicas e bioquímicas leves foram observadas como linfopenia. Conclusão Apesar do elevado percentual de RAM, muitos pacientes foram capazes de completar o tratamento, o que se atribui ao êxito da estratégia de acompanhamento com intervenções de tratamento sintomático juntamente ao aconselhamento, levando à compreensão da sintomatologia e manutenção do tratamento. A cardiomiopatia hipertrófica (CMH) é uma forma de doença do músculo cardíaco de causa genética, caracterizada pela hipertrofia das paredes ventriculares. O diagnóstico requer detecção por métodos de imagem (Ecocardiograma ou Ressonância Magnética Cardíaca) de qualquer segmento da parede do ventrículo esquerdo com espessura > 15 mm, sem outra causa provável. A análise genética permite identificar mutações de genes codificantes de diferentes estruturas do sarcômero responsáveis pelo desenvolvimento da CMH em cerca de 60% dos casos, permitindo o rastreio de familiares e aconselhamento genético, como parte importante do manejo dos pacientes e familiares. Vários conceitos sobre a CMH foram recentemente revistos, incluindo sua prevalência de 1 em 250 indivíduos, não sendo, portanto, uma doença rara, mas subdiagnosticada. A vasta maioria dos pacientes é assintomática. Naqueles sintomáticos, a obstrução do trato de saída do ventrículo esquerdo (OTSVE) é o principal distúrbio responsável pelos sintomas, devendo-se investigar a sua presença em todos os casos. Naqueles em que o ecocardiograma em repouso ou com Manobra de Valsalva não detecta gradiente intraventricular significativo (> 30 mmHg), devem ser submetidos à ecocardiografia com esforço físico para detecção da OTSVE. Pacientes com sintomas limitantes e grave OTSVE, refratários ao uso de betabloqueadores e verapamil, devem receber terapias de redução septal ou uso de novas drogas inibidoras da miosina cardíaca. Por fim, os pacientes adequadamente identificados com risco aumentado de morta súbita podem receber medida profilática com implante de cardiodesfibrilador implantável (CDI).
La miocardiopatía hipertrófica (MCH) es una forma de enfermedad cardíaca de origen genético, caracterizada por el engrosamiento de las paredes ventriculares. El diagnóstico requiere la detección mediante métodos de imagen (Ecocardiograma o Resonancia Magnética Cardíaca) que muestren algún segmento de la pared ventricular izquierda con un grosor > 15 mm, sin otra causa probable. El análisis genético permite identificar mutaciones en genes que codifican diferentes estructuras del sarcómero responsables del desarrollo de la MCH en aproximadamente el 60% de los casos, lo que permite el tamizaje de familiares y el asesoramiento genético, como parte importante del manejo de pacientes y familiares. Varios conceptos sobre la MCH han sido revisados recientemente, incluida su prevalencia de 1 entre 250 individuos, por lo tanto, no es una enfermedad rara, sino subdiagnosticada. La gran mayoría de los pacientes son asintomáticos. En los casos sintomáticos, la obstrucción del tracto de salida ventricular izquierdo (TSVI) es el trastorno principal responsable de los síntomas, y su presencia debe investigarse en todos los casos. En aquellos en los que el ecocardiograma en reposo o la maniobra de Valsalva no detecta un gradiente intraventricular significativo (> 30 mmHg), deben someterse a ecocardiografía de esfuerzo para detectar la obstrucción del TSVI. Los pacientes con síntomas limitantes y obstrucción grave del TSVI, refractarios al uso de betabloqueantes y verapamilo, deben recibir terapias de reducción septal o usar nuevos medicamentos inhibidores de la miosina cardíaca. Finalmente, los pacientes adecuadamente identificados con un riesgo aumentado de muerte súbita pueden recibir medidas profilácticas con el implante de un cardioversor-desfibrilador implantable (CDI).
Hypertrophic cardiomyopathy (HCM) is a form of genetically caused heart muscle disease, characterized by the thickening of the ventricular walls. Diagnosis requires detection through imaging methods (Echocardiogram or Cardiac Magnetic Resonance) showing any segment of the left ventricular wall with a thickness > 15 mm, without any other probable cause. Genetic analysis allows the identification of mutations in genes encoding different structures of the sarcomere responsible for the development of HCM in about 60% of cases, enabling screening of family members and genetic counseling, as an important part of patient and family management. Several concepts about HCM have recently been reviewed, including its prevalence of 1 in 250 individuals, hence not a rare but rather underdiagnosed disease. The vast majority of patients are asymptomatic. In symptomatic cases, obstruction of the left ventricular outflow tract (LVOT) is the primary disorder responsible for symptoms, and its presence should be investigated in all cases. In those where resting echocardiogram or Valsalva maneuver does not detect significant intraventricular gradient (> 30 mmHg), they should undergo stress echocardiography to detect LVOT obstruction. Patients with limiting symptoms and severe LVOT obstruction, refractory to beta-blockers and verapamil, should receive septal reduction therapies or use new drugs inhibiting cardiac myosin. Finally, appropriately identified patients at increased risk of sudden death may receive prophylactic measure with implantable cardioverter-defibrillator (ICD) implantation.

ABSTRACT Acute disseminated encephalomyelitis is a rare, acute, inflammatory, and demyelinating disease of the central nervous system. Presumably associated in more than three quarters of cases by infections (viral, bacterial, or nonspecific) and immunizations or without any identifiable antecedent. It usually presents a monophasic course with onset of nonspecific symptoms in the prodromal phase and may evolve with multifocal neurological changes and even visual acuity loss. We describe a case of a 9-year-old boy with an initial picture of papillary edema caused by acute disseminated encephalomyelitis due to Bartonella henselae. Abstract In low and middle-income countries such as Brazil, most maternal deaths are related to hypertensive complications. Preeclampsia is the leading cause of maternal mortality and morbidity. Significant proportion is associated with the following factors: lack of identification of high-risk women, lack of adequate prevention, difficulty in maintaining a high-risk prenatal follow-up, delayed diagnosis, insecurity and low use of magnesium sulphate, delayed pregnancy interruption and lack of postpartum follow-up of these high-risk cases. Four major actions are proposed to minimize this alarming clinical picture and reduce the mortality rates due to preeclampsia, called the "4 P Rule" (Adequate Prevention – Vigilant Prenatal Care – Timely Delivery (Parturition) – Safe Postpartum). Sand flies play a crucial role as vectors of bacteria, viruses, and protists, with Leishmania being the most notable among them, transmitted to vertebrate hosts during blood feeding. Understanding the feeding behaviours of sand flies is imperative for gaining insights into their eco-epidemiological roles in the transmission of these infectious agents. This systematic review aimed to answer the question ‘What are the blood-feeding sources identified in Brazilian sand flies?’ to provide an analysis of their blood-feeding habits. The diverse range of at least 16 vertebrate orders identified as blood sources for 54 sand fly species across different geographic regions was summarised, and the factors potentially associated with the risk of bias in the included studies were analysed. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT Novel therapies for Alzheimer’s disease, particularly anti-amyloid drugs like lecanemab and donanemab, have shown modest clinical benefits but also significant risks. The present paper highlights the challenges of access to diagnosis, cost-effectiveness, safety, and the need for more representation of diverse populations in clinical trials. Recommendations include careful patient selection, risk-benefit analysis, and the importance of proven amyloid pathology for treatment. Future work involves further research on anti-amyloid therapies in Brazil and the development of more effective treatments for Alzheimer’s disease. ABSTRACT There is increasing interest in ultrasound-guided ablation treatments for thyroid diseases, including benign and malignant ones. Surgeons, radiologists, and endocrinologists carry out these treatments, and various organizations within these specialties have recently released multiple international consensus statements and clinical practice standards. The aim of the present consensus statement is to provide guidance, cohesion, and standardization of best practices for thermal ablation procedures of thyroid nodules. The statement includes the indications for these procedures, preprocedural evaluations, technical aspects of the procedures, posttreatment care, follow-up, complications, and training recommendations. ABSTRACT Corn silage is susceptible to losses through aerobic spoilage. Therefore, this study aimed to evaluate increasing sweet orange essential oil levels in chemical composition, in vitro degradability, losses, fermentation parameters, microbial count, and the aerobic stability of corn silage. The experiment was carried out in a completely randomized design with four treatment levels: Control (CON), ensiling corn without sweet orange essential oil; 200 mg kg–1,400 mg kg–1, and 600 mg kg–1 of ensiling mass with four replicates per treatment. Sweet orange essential oil affected both the dry matter (p = 0.035) and the organic matter (p = 0.021), presenting quadratic behavior. Sweet orange essential oil did not affect (p ≥ 0.05) in vitro dry matter degradability, in vitro neutral detergent fiber degradability, or gross energy. Sweet orange essential oil had a quadratic effect (p = 0.022) on the acetic acid concentration in corn silage. There was a tendency (p = 0.097) for sweet orange essential oil to reduce the ammoniacal nitrogen linearly. The levels of sweet orange essential oil did not influence losses by gases or effluents (p ≥ 0.05). We observed no effect of sweet orange essential oil on Lactic acid-producing bacteria populations or fungi (p ≥ 0.05). Furthermore, increasing sweet orange essential oil levels decreased temperature (p = 0.02) over time but did not influence pH (p = 0.404). Sweet orange essential oil does not affect in vitro degradability, fermentation parameters, nor microbial count in corn silage. ABSTRACT: Iguanids are susceptible to several bacterial infections, especially those caused by Gram-negative bacteria. The development of these diseases in reptiles is related to management, inadequate sanitary conditions, and immunosuppression. This study aims to describe the anatomopathological and microbiological aspects of a case of co-infection by Plesiomonas shigelloides, Citrobacter freundii and Aeromonas jandaei diagnosed in a free-living green iguana (Iguana iguana). Macroscopically, the lesions were mainly located in skeletal muscle, myocardium, small intestine, and liver, characterized by white-yellowish, multifocal, friable, irregular areas associated with necrosis and hemorrhage. In the histopathological analysis, basophilic bacillary structures corresponding to bacterial aggregates were observed in the skeletal muscle, myocardium, hepatic parenchyma, kidney, stomach and small intestine associated with areas of thrombosis, necrosis and hemorrhage. The diagnosis of sepsis by P. shigelloides, C. freundii and A. jandaei was confirmed by matrix-assisted laser desorption/ionization time of flight (MALDI-TOF) associated with the anatomopathological and microbiological findings observed in this case. RESUMO: Iguanídeos são suscetíveis a diversas infecções bacterianas, principalmente aquelas causadas por bactérias Gram-negativas. O desenvolvimento destas enfermidades em répteis está relacionado a manejos, condições sanitárias inadequadas e imunossupressão. Este estudo tem por objetivo descrever os aspectos anatomopatológicos e microbiológicos de um caso de coinfecção por Plesiomonas shigelloides, Citrobacter freundii e Aeromonas jandaei diagnosticado em uma iguana verde de vida livre (Iguana iguana). Macroscopicamente, as lesões localizavam-se principalmente em musculatura esquelética, miocárdio, duodeno e fígado, caracterizando-se por áreas branco-amareladas, multifocais, friáveis, e irregulares associadas à necrose e hemorragia. No histopatológico foram vistas estruturas bacilares basofílicas correspondentes a agregados bacterianos em músculo esquelético, miocárdio, parênquima hepático, rim, estômago e intestino delgado associados a áreas de trombose, necrose e hemorragia. Abstract Background: Theoretical models are characterized as a guiding thread for research. However, they are rarely used and reported in depth by researchers, which can negatively impact scientific quality and reproducibility. Objective: Identify the use profile and level of depth of theories and/or theoretical models, methodological and reporting qualities of systematic reviews (SR) with or without meta-analysis, and to correlate the use of theories/models with the methodological and reporting qualities. Method: Cross-sectional study, carried out from May to December 2019, composed of RS from the journals Revista de Saúde Pública (RSP) and Cadernos de Saúde Pública (CSP). There were no language restrictions. The selection of articles and data extraction was carried out in duplicate. To assess the use of theories, the questionnaire developed by Cabrera was used; to assess methodological quality, the Assessment of Multiple Systematic Reviews (AMSTAR-2); and for the report, the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Analysis was performed using statistical tests, at the level of significance p<0.05. Results: Forty-five SR were included (22 RSP and 23 CSP). Only 24.4% of SR used theories/models and, of these, 54.5% only mentioned (Level 1) and 45.5% described (Level 2), there was no use in depth (Levels 3 and 4). The SRs presented limitations in methodological and reporting quality: only 37.5% of the items in AMSTAR-2 were attended by most of the SRs, and 66.6% of those in PRISMA. Positive correlations were observed between the use of theories and methodological and reporting qualities. Conclusion: The use of theories/theoretical models may reflect better methodological and reporting quality of SR. Resumo Introdução: Modelos teóricos são visualizados como fio condutor das pesquisas. Entretanto, são pouco utilizados e relatados em profundidade pelos pesquisadores, o que pode impactar negativamente na qualidade e reprodutibilidade científica. Objetivos: Identificar o perfil de utilização e o nível de profundidade de Teorias e/ou Modelos Teóricos, qualidades metodológica e de relato de Revisões Sistemáticas (RS) com ou sem meta-análise, e correlacionar a utilização de teorias/modelos às qualidades metodológica e de relato. Método: Estudo transversal, realizado de maio a dezembro de 2019, composto por RS dos periódicos Revista de Saúde Pública (RSP) e Cadernos de Saúde Pública (CSP). Não houve restrições de idiomas. A seleção dos artigos e a extração dos dados foram realizadas em duplicata. Para avaliação do uso de teorias, utilizou-se questionário desenvolvido por Cabrera; qualidade metodológica, AMSTAR-2; e relato, PRISMA. Análise foi realizada por testes estatísticos, ao nível de significância p<0,05. Resultados: Foram incluídas 45 RS (22 RSP e 23 CSP). Apenas 24,4% das RS utilizaram teorias/modelos e, destas, 54,5% apenas citaram (Nível 1) e 45,5% descreveram (Nível 2). Não foi observada utilização em profundidade (Níveis 3 e 4). As RS apresentaram limitações na qualidade metodológica e de relato: apenas 37,5% dos itens do AMSTAR-2 foram atendidos pela maioria das RS e 66,6% aos do PRISMA. Abstract Plants used in traditional medicine offer an affordable new alternative in tissue repair therapy. This study aimed to evaluate the effectiveness of the 5% Rhizophora mangle cream compared to the 5% dexpanthenol cream in healing open surgical wounds on the upper eyelid. A total of 18 patients were submitted to the experiment and divided into 2 groups with 9 patients each who used topically and daily 5% dexpanthenol cream (control group) or 5% R.mangle cream (intervention group) for 7 days. Clinical, morphometric and histomorphometric analyses of wounds and surgical procedures for skin removal were performed. In the morphometric analysis, all wounds treated with R.mangle and dexpanthenol creams showed complete macroscopic scars, without inflammatory signs and infection free. The skin hydration values in pre and post application periods of the cream were 43.82 ± 43.93 and 62.12 ± 67.40 respectively. The histomorphometric study showed lower values of epithelium distance in R. mangle group and higher in dexpanthenol group with significant difference between groups (p < 0.05). Abstract Background The oral cavity is a link between of external environment with gastrointestinal tract. Studies are controversial on the presence of Periodontal Disease (PD) and its association with Gastric Adenocarcinoma (GAC). Methods The authors performed a systematic review and meta-analysis to verify the association between PD and GAC. Six electronic databases were evaluated between 1961 and 2022. Titles and abstracts were reviewed independently according to the eligibility criteria, assessing full texts of selected studies. The quality of the included research was verified using the Newcastle-Ottawa Scale for case-control and cohort studies. Statistical analyses were performed based on fixed and/or random effects models to calculate the summarized Relative Risk (RR) and its 95 % Confidence Interval (95 % CI). Results There were 639 studies, of which nine articles were included (3 case-controls and 6 cohorts). Overall, the authors identified 1,253 cases of GAC 2,501 controls in case-control studies, and 1,631 patients with GAC enrolled in cohort studies. Patients presenting PD increased the risk of developing GAC by 17 % (RR=1.17; 95 % CI 1.03‒1.32), which remained regardless of the diagnostic method for PD, i.e., clinical examination (RR = 1.19; 95 % CI 1.14‒1.24) and self-report (RR = 1.34; 95 % CI 1.06‒1.69). Moreover, Asian patients (RR=1.17; 95 % CI 1.00‒1.36) with PD had a higher risk of having GAC than American and European patients (RR = 1.18; 95 % CI 0.84‒1.66). Conclusions The presence of PD the risk of GAC suggesting that its infectious-inflammatory process of PD may be related to GAC development. Further investigations on the oral-gastric microbiota and its role in the carcinogenesis of gastric cancer should be carried out, and the screening of patients with potential risk for GAC should be considered in the clinical practice of dentists. tract (PD .
