Abstract Introduction Benign paroxysmal positional vertigo (BPPV) is the peripheral vestibular dysfunction that most affects people worldwide, but its etiopathogenesis is still not fully understood. Considering the etiological diversity, some studies highlight the association between BPPV and thyroid diseases. Objective To investigate the association between thyroid diseases and BPPV. Data Synthesis Systematic review and meta-analysis of epidemiological studies searched in the PubMed, Web of Science, Embase, Cochrane Library, and Scopus databases. Studies that were fully available and investigated the association between BPPV and thyroid diseases were selected. The articles that composed the meta-analysis were analyzed using the dichotomous model, the Mantel-Haenszel statistical test, odds ratio (OR), and a 95% confidence interval (CI). Of the 67 articles retrieved from the databases, 7 met the eligibility criteria of the systematic review, and 4 had data necessary to perform the meta-analysis. Qualitative analysis revealed that the studies were conducted in the European and Asian continents. The predominant methodological design was the case-control type, and thyroid dysfunctions, hypothyroidism, and Hashimoto thyroiditis occurred more frequently. The meta-analysis showed no association between hypothyroidism and BPPV; however, there was a statistically significant relationship between Hashimoto thyroiditis and BPPV. Conclusion The meta-analysis results suggest a possible association between BPPV and Hashimoto thyroiditis. Nevertheless, we emphasize the need for further studies to elucidate the evidence obtained. Abstract Objective: To estimate the prevalence of vitamin D deficiency and severe deficiency in children and adolescents, in a large Brazilian sample. Methodology: Results of 413,988 25(OH)D measurements in children and adolescents aged 0 to 18 years collected between 01/2014 and 10/2018 were obtained from the database of a Clinical Laboratory. In this population, 25 hydroxyvitamin D concentrations below 20 ng/mL are considered deficient, and below 12 ng/mL as severe deficiency. All measurements were performed by immunoassay and the results were distributed by gender, age group, seasonality, and latitude. Results: The mean of 25(OH)D levels was 29.2 ng/mL with a standard deviation of 9.2 ng/mL. Of the total samples, 0.8% had a concentration < 12 ng/mL, and 12.5% of the samples had a concentration < 20 ng/mL, with a higher prevalence in females. Children under 2 years of age had the lowest prevalence. The effects of latitude and seasonality were quite evident. In samples of female adolescents from the southern region in winter, 36% of vitamin D deficiency and 5% of severe deficiency were found. Conclusion: In this large number of measurements of 25(OH)D in children and adolescents, 12.5% had a deficiency and 0.8% had severe deficiency. A greater deficiency was observed among adolescents, especially females, which raises questions about the need for supplementation during this period of life. Abstract Objective: To investigate the effectiveness of linezolid and vancomycin for the treatment of nosocomial infections in children under 12 years old. Data sources: This is a systematic review in which five randomized clinical trials about the effectiveness of linezolid and vancomycin, involving a total of 429 children with nosocomial infections, were evaluated. They were searched in scientific databases: PubMed, Bvs, and SciELO. Summary of findings: The main nosocomial infections that affected children were bacteremia, skin, and soft tissue infections followed by nosocomial pneumonia. Most infections were caused by Gram-positive bacteria, which all studies showed infections caused by Staphylococcus aureus, with methicillin-resistant S. aureus (MRSA) and methicillin-resistant coagulase-negative staphylococci strains being isolated. Both linezolid and vancomycin showed high therapeutic efficacy against different types of nosocomial infections, ranging from 84.4% to 94% for linezolid and 76.9% to 90% for vancomycin. Patients receiving linezolid had lower rates of rash and red man syndrome compared to those receiving vancomycin. However, despite the adverse reactions, antimicrobials can be safely administered to children to treat nosocomial infections caused by resistant Gram-positive bacteria. Conclusion: Both linezolid and vancomycin showed good efficacy in the treatment of bacterial infections caused by resistant Gram-positive bacteria in hospitalized children. However, linezolid stands out regarding its pharmacological safety. Importantly, to strengthen this conclusion, further clinical trials are needed to provide additional evidence. Abstract Objective: to carry out a scoping review with the purpose of mapping the scientific evidence on the use of the neutropenic diet in neutropenic pediatric cancer patients. Source of data: The scoping review protocol was prepared in accordance with the PRISMA-ScR and the checklist before the literature search was performed. Articles on nutritional management in adults or on the treatment of other diseases, and articles that were not in Portuguese or English and published before the year 2000, were excluded. Data were extracted based on the Cochrane Consumer and Communication Review Group form. Summary of the findings: Three hundred and forty scientific articles were identified, with the final sample of this review consisting of nine studies. Although the neutropenic diet has been part of the nutritional management of pediatric cancer patients for more than 20 years, there is still great variation in the criteria for indicating use and starting and discontinuing it, as well as in the nutritional composition of the diet. Furthermore, there is no consensus on the impact of using a neutropenic diet on different clinical and nutritional outcomes. Conclusion: In the absence of guidelines that standardize the use of a neutropenic diet in pediatric patients with neutropenia, there are heterogeneous approaches reported in the literature, even within the same institution. The available literature presents an absence of evidence on the use, viability, and effectiveness of the neutropenic diet in oncological children with neutropenia. More studies are needed to identify the real impact of the neutropenic diet on clinical and nutritional outcomes. Abstract Objective: To correlate the nasal anatomical characteristics of newborns with the dimensions of short binasal prongs. Method: Observational, cross-sectional study carried out in two hospitals in southern Brazil. The authors evaluated 1620 newborns with neonatal data and nasal dimensions. To measure the dimensions of the nasal region, the authors considered the width of the medial columella, the right nostril diameter, and the left nostril diameter. These data were correlated with the dimensions of two models of short binasal prongs. Results: Of the total newborns evaluated, 807 were female (49.8%), and 813 were male (50.2%). The majority were white (96.2%). The mean gestational age was 37.4 ± 2.9 weeks, ranging from 22 to 42 weeks. The birth weight was 2946.8 ± 699.3 g, ranging from 490.0 to 4740.0 g. Most of the nasal measures were significantly larger than both prong model measurements. Conclusion: The sizes of short binasal prongs available on the Brazilian market do not match the nasal anatomical characteristics of newborns. RESUMO Objetivo. Descrever a implementação e a atuação pioneira do comitê de farmacovigilância em vacinas do estado de São Paulo, Brasil, na análise de eventos supostamente atribuíveis à vacinação ou imunização (ESAVI) no período da pandemia de covid-19. Método. Trata-se de um estudo de caso retrospectivo, qualiquantitativo, com enfoque na atuação do comitê de farmacovigilância nos anos de 2021 e 2022. A partir dos registros das reuniões do comitê, foi descrito o modo de funcionamento do comitê e contabilizados os casos de ESAVI discutidos e resolvidos nas reuniões de especialistas. O número de doses de vacinas aplicadas no período de interesse, assim como o número de ESAVI notificados nesse período, foi obtido de sistemas de informação estaduais. Resultados. Em 55 encontros realizados em 2021 e 2022, foram discutidos 118 casos de ESAVI nas reuniões do comitê de farmacovigilância, majoritariamente relacionados às vacinas de covid-19. Contabilizou-se a aplicação de 126 778 252 doses de vacinas de covid-19 nesse período, tendo havido 42 893 notificações de ESAVI. Entre os casos elencados para discussão, 71,2% ocorreram após a primeira dose da vacina, 32,8% representaram eventos neurológicos e apenas 5% dos casos apresentaram associação causal com os imunizantes. Conclusão. O comitê de farmacovigilância teve um papel importante no sistema de vigilância de ESAVI no período analisado, auxiliando na avaliação de casos de maior complexidade, com potencial para contribuir positivamente no fortalecimento do programa de imunização, em especial no âmbito do sistema de vacinação segura.
ABSTRACT Objective. To describe the implementation and pioneering work of a vaccine pharmacovigilance committee in the state of São Paulo, Brazil, for the analysis of events supposedly attributable to vaccination or immunization (ESAVI) during the COVID-19 pandemic. Methods. This is a retrospective, mixed-methods (qualitative and quantitative) case study focusing on the work carried out by the pharmacovigilance committee in the years 2021 and 2022. The minutes of committee meetings were used to describe how the committee operates. ESAVI cases discussed and addressed at expert meetings were recorded. The number of vaccine doses applied during the period of analysis, as well as the number of ESAVIs reported within that period, was obtained from statewide information systems. Results. Over 55 pharmacovigilance committee meetings held in 2021–2022, 118 ESAVI cases—most related to COVID-19 vaccines—were discussed. A total of 126,778,252 doses of COVID-19 vaccines were administered during this period, with 42,893 ESAVI reported. Among the cases selected for discussion, 71.2% occurred after the first dose of vaccine, 32.8% represented neurological events, and only 5% were deemed to have a causal relationship with the vaccine. Conclusions. The pharmacovigilance committee played a significant role in the ESAVI surveillance system during the period of analysis by assisting in the evaluation of more complex cases. It shows potential to contribute positively to strengthening the immunization program, especially within the safe vaccination system framework.
RESUMEN Objetivo. Describir la puesta en marcha del comité de farmacovigilancia de vacunas del estado de São Paulo (Brasil) y su actuación pionera en el análisis de eventos supuestamente atribuibles a la vacunación o inmunización (ESAVI) durante la pandemia de COVID-19. Método. Se trata de un estudio de caso, retrospectivo, cualitativo y cuantitativo, centrado en la actuación del comité de farmacovigilancia en el período 2021-2022. A partir de las actas de las reuniones del comité, se describió su funcionamiento y se contabilizaron los casos de ESAVI examinados y sobre los que se llegó a una conclusión en las reuniones de expertos. El número de dosis de vacunas administradas en el periodo de interés y el número de ESAVI notificados en ese periodo se obtuvieron de los sistemas de información estatales. Resultados. En 55 reuniones celebradas en el período 2021-2022, se analizaron 118 casos de ESAVI en las reuniones del comité de farmacovigilancia, la mayoría de ellos relacionados con las vacunas contra la COVID-19. Durante este periodo se administró un total de 126 778 252 dosis de vacunas contra la COVID-19, y hubo 42 893 notificaciones de ESAVI. De los casos seleccionados para el examen por el comité, el 71,2% ocurrieron después de administrar la primera dosis de la vacuna, el 32,8% fueron eventos neurológicos y solo el 5% presentaron una asociación causal con los productos vacunales. Conclusiones. El comité de farmacovigilancia desempeñó un papel importante en el sistema de vigilancia de ESAVI en el periodo analizado y ayudó a evaluar los casos más complejos, y mostró su potencial de contribuir positivamente a fortalecer el programa de inmunización, sobre todo en el contexto del sistema de vacunación segura.

Resumo Fundamento: O Brasil é um país com diferentes biomas e desigualdades sociais. Existem poucos dados disponíveis sobre as diferenças regionais e o prognóstico da insuficiência cardíaca (IC) no país. Objetivo: O estudo Rosa dos Ventos tem como objetivo investigar as diferenças regionais e o prognóstico atual de pacientes com IC com fração de ejeção reduzida ou moderadamente reduzida no Brasil. Métodos: Este é um estudo prospectivo, multicêntrico, observacional, do tipo coorte que incluirá pacientes ambulatoriais com idade superior a 18 anos com IC e fração de ejeção < 50% em 30 centros privados distribuídos nas regiões brasileiras. Um total de 2500 pacientes serão incluídos entre junho de 2021 e outubro de 2023, com um período de 12 meses de seguimento. Coletaremos dados sobre status clínico e socioeconômico, prescrição médica e resultados de exames cardiológicos. Serão realizados telefonemas para o seguimento dos pacientes seis e 12 meses após a inclusão para coleta de informações sobre visitas ao departamento de emergência, internações e mortalidade. Conclusão: O estudo Rosa dos Ventos permitirá uma caracterização mais precisa da IC crônica no Brasil. Abstract Background: Brazil is a country with different biomes and social disparities. There are limited data available on regional differences and prognosis of heart failure (HF) in the country. Objective: The Rosa dos Ventos study aims to investigate regional differences and the current prognosis of HF outpatients with reduced or mildly reduced ejection fraction in Brazil. Methods: This is a prospective, multicenter, observational cohort study that will include outpatients older than 18 years with HF and an ejection fraction < 50% in 30 public and private centers distributed in all Brazilian regions. A total of 2,500 patients will be enrolled from June 2021 and October 2023, with a 12-month follow-up period. We will collect data on socioeconomic and clinical status, medical prescription and results of cardiology tests. Follow-up phone calls will be made at 6 and 12 months after inclusion to collect information regarding emergency room visits, hospitalization and mortality. Conclusion: The Rosa dos Ventos study will allow a more accurate characterization of chronic HF in Brazil. This initiative will provide relevant information for the development of effective management strategies to mitigate the impact of this condition on patients and the healthcare system. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. RESUMO Nos últimos anos, a precisão diagnóstica da doença de Alzheimer tem sido aprimorada pelo desenvolvimento de diferentes tipos de biomarcadores que indicam a presença dos processos neuropatológicos. Além de melhorar a seleção de pacientes para ensaios clínicos, os biomarcadores podem avaliar os efeitos de novos tratamentos nos processos patológicos. No entanto, há preocupação com o uso indiscriminado e mal fundamentado de biomarcadores, especialmente em indivíduos assintomáticos ou com declínio cognitivo subjetivo. As dificuldades na interpretação desses testes, os altos custos e o acesso desigual tornam esse cenário ainda mais desafiador no cuidado assistencial. Neste artigo, são apresentadas as recomendações do Departamento Científico de Neurologia Cognitiva e Envelhecimento da Academia Brasileira de Neurologia quanto ao uso racional e interpretação de biomarcadores da doença de Alzheimer na prática clínica. ABSTRACT In recent years, the diagnostic accuracy of Alzheimer’s disease has been enhanced by the development of different types of biomarkers that indicate the presence of neuropathological processes. In addition to improving patient selection for clinical trials, biomarkers can assess the effects of new treatments on pathological processes. RESUMO Novas terapias para a doença de Alzheimer, particularmente os anticorpos monoclonais antiamiloides como o lecanemab e o donanemab, demonstraram benefícios clínicos modestos, mas também riscos significativos. RESUMO O feijão-caupi (Vigna unguiculata L. Walp.) é uma cultura promíscua, nodulando com uma ampla gama de estirpes de rizóbio nativas do solo, o que frequentemente leva a baixas respostas de inoculação com rizóbios selecionados, reduzindo a eficiência dos inoculantes introduzidos. Objetivou-se avaliar a ocupação nodular da estirpe elite Bradyrhizobium pachyrhizi BR 3262 em dois genótipos de feijão-caupi (BRS Pujante e BRS Acauã), em solo com população de rizóbio estabelecida (superior a 103 g-1 de solo), em casa-de-vegetação. Para a ocupação de nódulos BR 3262, foi utilizada abordagem de PCR estirpe-específica, avaliando-se todos os 1.237 nódulos obtidos das regiões da coroa radicular e de toda a raiz (raízes secundárias) das plantas inoculadas. A inoculação de BR 3262 aumentou a massa seca da raiz e a nodulação, mas não a massa seca da parte aérea. ABSTRACT Cowpea (Vigna unguiculata L. Walp.) is a promiscuous crop, nodulating with a wide range of rhizobia strains native to the soil, what often leads to low inoculation responses with selected rhizobia, reducing the efficiency of introduced inoculants. This study aimed to assess the nodule occupancy of the Bradyrhizobium pachyrhizi BR 3262 elite strain in two cowpea genotypes (BRS Pujante and BRS Acauã), in soil with established rhizobial population (higher than 103 g-1 of soil), in a greenhouse. For the BR 3262 nodule occupancy, a strain-specific PCR approach was applied, assessing all the 1,237 nodules obtained from the root crown and the entire root (secondary roots) regions of the inoculated plants. The BR 3262 inoculation increased the root dry mass and nodulation, but not the shoot dry mass. Both genotypes showed a nodular occupancy above 80 % in the crown and secondary roots, indicating a high competitiveness and persistence in the soil. Vigna L Walp. RESUMO Este estudo objetivou caracterizar o perfil dos preceptores que iniciaram um curso de especialização em Preceptoria Multiprofissional na Área da Saúde. Trata-se de um estudo transversal de abordagem quantitativa. Ao início do curso (2022), os preceptores matriculados foram convidados a responder ao questionário eletrônico com questões de múltipla escolha, que abordavam dados pessoais, da formação e da atuação. Dos matriculados, 1.112 (71,23%) responderam ao questionário. A maioria é do sexo feminino (84,8%), tem entre 31 e 50 anos, e atua em programas de residência multiprofissional, no nível terciário de atenção à saúde. A área de graduação predominante é a enfermagem (35,61%), seguida pela fisioterapia e medicina. Houve uma maior proporção de preceptores da região Sudeste, seguida pelas regiões Nordeste e Norte. Apenas 35,25% cursaram residência, e 136 preceptores não cursaram residência nem outra pós-graduação. Poucos estão envolvidos em pesquisa (3,5%). A maioria não tem plano de carreira e não recebe bolsa complementar. Os resultados podem auxiliar na construção e no monitoramento de políticas públicas relacionadas à formação de preceptores e às residências em saúde, evidenciando necessidades como ampliação das oportunidades de formação, inclusive em residências, valorização da carreira do preceptor e incentivo à pesquisa. ABSTRACT This study aimed to characterize the profile of preceptors who started a specialization course in Multiprofessional Preceptorship in the Health Area. It is a cross-sectional study with a quantitative approach. At the beginning of the course (2022), enrolled preceptors were invited to respond to the electronic questionnaire with multiple-choice questions, which covered personal, training and performance data. Of the enrolled preceptors, 1112 (71.23%) responded to the questionnaire. The majority are female (84.8%), aged between 31 and 50, and work in multi-professional residency programs at the tertiary health care level. The predominant area of graduation is nursing (35.61%), followed by physiotherapy and medicine. There was a higher proportion of preceptors from the Southeast region, followed by the Northeast and North regions. Only 35.25% attended residency, and 136 preceptors did not attend residency or other postgraduate courses. Few are involved in research (3.5%). The majority declared that they did not have a career plan and did not receive a supplementary remuneration. RESUMO Introdução: O CrossFit® é um método de treinamento físico que visa promover a aptidão física por meio do desenvolvimento de componentes como capacidade aeróbia, força e resistência muscular. Os dados sobre comportamentos de bruxismo em praticantes de CrossFit® são escassos, mas estudos prévios mostraram aumento do comportamento de apertamento dentário durante as práticas de levantamento de peso. Objetivo: O presente estudo teve como objetivo avaliar os sinais clínicos de bruxismo em praticantes de CrossFit®. Métodos: O estudo foi composto por uma amostra de conveniência de praticantes de CrossFit® (n=57), de ambos os sexos, com idade entre 19 e 58 anos, e as variáveis de desfecho foram: Lista de Verificação de Comportamento Oral, Questionário Internacional de Atividade Física e Ferramenta Padronizada para Avaliação do Bruxismo. Os dados foram expressos em termos de valores absolutos e percentuais. Para análise estatística, foram utilizados os testes de correlação de Spearman e exato de Fisher, e considerou-se nível de significância de 5%. Resultados: A média de idade foi de 32,82 anos, com maioria feminina (63,15%). Os praticantes de CrossFit® relataram frequentemente apertar os dentes durante a prática do treinamento (61,40%) e apresentaram linha alba (82,45%), impressão labial (54,38%), impressão de língua (26,31%), exostose óssea (19,29%), desgaste dentário (61,40%) e lesões cervicais não cariosas (35,09%). Conclusões: Não foi encontrada correlação entre sinais clínicos de bruxismo, comportamentos orais e intensidade da atividade física; no entanto, os resultados sugerem que indivíduos que praticam o treinamento CrossFit® apresentam comportamentos orais que podem causar alterações morfofuncionais no sistema estomatognático, principalmente o hábito de apertar os dentes durante o treinamento. Além disso, os dados enfatizam a necessidade de educação em saúde bucal entre os praticantes de CrossFit®, sendo necessários mais estudos com amostras representativas. ABSTRACT Introduction: CrossFit® is a physical training method that aims to promote physical fitness through the development of components such as aerobic capacity, strength and muscular endurance. Data regarding bruxism behaviors in CrossFit® practitioners are scarce, but previous studies have shown increased dental clenching behavior during weightlifting practices. Objective: The present study aimed to evaluate clinical signs of bruxism in CrossFit® practitioners. Methods: The sample comprised a convenience sample of CrossFit® practitioners (n=57), of both genders, aged 19-58 years. Outcome variables were as follows: Oral Behavior Checklist, the International Physical Activity Questionnaire and the Standardized Tool for the Assessment of Bruxism. Data were expressed in terms of absolute values and percentages. Spearman’s correlation and Fisher exact tests were used for statistical analysis, and a significance level of 5% was considered. Results: The mean age was 32.82 years, with a female majority (63.15%). CrossFit® practitioners frequently reported clenching their teeth during training practice (61.40%) and presented linea alba (82.45%), lip impression (54.38%), tongue impression (26.31%), bone exostosis (19.29%), tooth wear (61.40%) and non-carious cervical lesions (35.09%). Conclusions: No correlation was found between clinical signs of bruxism, oral behaviors and physical activity intensity; however, the results suggest that individuals who practice CrossFit® training have oral behaviors that can cause morpho-functional changes in the stomatognathic system, especially the habit of clenching their teeth during training. In addition, data emphasize the need for dental health education among CrossFit® practitioners, and more studies with a representative sample are necessary.
