ABSTRACT Objective: To assess dentists' knowledge from Minas Gerais, Brazil, about dentoalveolar trauma (DT) and their experiences during the COVID-19 pandemic. Material and Methods: An online questionnaire with 34 questions was applied to collect personal data, professional training, self-assessment of experience/knowledge about DT, experience in care provided during the social distancing, and knowledge/conduct. The specific responses were evaluated based on the guidelines of the International Association of Dental Traumatology (IADT). Descriptive analysis and Pearson's chi-square test were performed. The level of significance was set at p≤0.05. Results: Most professionals (97.7%) had received information on DT, and only 4.6% of the participants considered their knowledge poor or very poor. However, 92.7% felt the need for more information on the subject. Regarding experiences during the pandemic, 55.7% provided trauma care during that period. Forty percent of the consultations were performed in person, and 33.3% of the professionals noted an increase in cases during the pandemic; 56.6% reported that the frequency did not change. The overall mean number of correct answers about DT was 5.29±2.11, indicating an acceptable level of knowledge. The mean percentage of hits for the specific questions was 44.1%. Conclusion: Although the level of knowledge of the dentists evaluated was acceptable, some aspects were deficient, with the need for more information about the IADT guidelines. BACKGROUND Leishmania tarentolae is a non-pathogenic species found in lizards representing an important model for Leishmania biology. However, several aspects of this Sauroleishmania remain unknown to explain its low level of virulence. OBJECTIVES We reported several aspects of L. tarentolae biology including glycoconjugates, proteolytic activities and metabolome composition in comparison to pathogenic species (Leishmania amazonensis, Leishmania braziliensis, Leishmania infantum and Leishmania major). METHODS Parasites were cultured for extraction and purification of lipophosphoglycan (LPG), immunofluorescence probing with anti-gp63 and resistance against complement. Parasite extracts were also tested for proteases activity and metabolome composition. FINDINGS Leishmania tarentolae does not express LPG on its surface. It expresses gp63 at lower levels compared to pathogenic species and, is highly sensitive to complement-mediated lysis. This species also lacks intracellular/extracellular activities of proteolytic enzymes. It has metabolic differences with pathogenic species, exhibiting a lower abundance of metabolites including ABC transporters, biosynthesis of unsaturated fatty acids and steroids, TCA cycle, glycine/serine/threonine metabolism, glyoxylate/dicarboxylate metabolism and pentose-phosphate pathways. ABSTRACT Purpose: Acute-on-chronic liver failure (ACLF) is a leading cause of death in cirrhotic patients. This study aims to describe the outcomes of in-patients with ACLF at a liver transplantation (LT) center in Brazil. Methods: Retrospective study analyzing patient data from 2017 to 2022. Re-transplant cases and patients without previous chronic liver disease were excluded. The ACLF diagnosis was based on the European Association for the Study of the Liver-Chronic Liver Failure criteria and assessments repeated on days 3 and 7 after the initial diagnosis. Results: Among 381 patients, 10.49% (n = 40) were diagnosed with ACLF. Bacterial infection was the most common precipitating factor (45%). Kidney failure occurred in 65% of the cases. The 28-day mortality rate was 35% and varied according to ACLF severity at diagnosis, from single organ failure (ACLF-1) at 22% to three organ failures (ACLF-3) at 60%. Eighteen patients (45%) were transplanted with a 100% 28-day survival rate. For ACLF-3 cases at diagnosis (n = 15), the 28-day and 1-year survival rates with a transplant (n = 4) were 100% and 80%, respectively, and without transplant (n = 11), 10 and 0%, respectively. Conclusions: ACLF was associated with high mortality rates. LT was an effective therapeutic option, particularly for ACLF-3 cases. Abstract Purpose To review studies that have intervention in reading with impacts on phonological awareness in children with autism spectrum disorder. Research strategies Searches took place until February 2021 in Cochrane, Embase, ERIC (Education Resources Information Center), LILACS (Latin American and Caribbean Health Sciences Literature), PubMed/Medline, Scopus, Web of Science and gray literature databases. Selection criteria The review included experimental studies with preschoolers and schoolchildren with ASD. Two independent reviewers selected the studies and, in case of disagreement, a third reviewer was consulted. Data analysis Joanna Briggs Institute checklists were used for risk of bias. A random effects meta-analysis was performed and the certainty of the evidence was assessed using the GRADE tool. Results Eight studies with some impact on phonological awareness were reviewed. The risk of bias was low and moderate. The certainty of the evidence was low for randomized trials and very low for non-randomised trials. Comparison of pre- and post-therapy on the Preschool Literacy Test (TOPEL) showed that children with ASD improved phonological awareness, with a mean difference between baseline and post-therapy of 6.21 (95% CI = 3.75-8.67; I2 = 0%). Conclusion Shared reading and software activities with words and phrases can alter phonological awareness. ABSTRACT Objective: To evaluate the risk factors for healthcare-related infections during the COVID-19 pandemic in intensive care units, to investigate the impact of COVID-19 on Central Line-Associated Bloodstream Infection, Catheter-Associated Urinary Tract Infection, and ventilator-associated pneumonia, and to describe healthcare-associated infections in the waves of the COVID-19 pandemic. Methods: This nested case-control study was conducted in a 137-bed adult medical/surgical intensive care unit at a private hospital in São Paulo, Brazil, between January 11, 2019, and May 21, 2022. Case patients were identified using the Nosocomial Infection Control Committee database and control patients were identified using the intensive care unit's EPIMED system. For the analysis of risk factors, the chi-square test, multiple logistic regression model, and Kaplan-Meier method were used to identify independent risk factors, considering p<0.05. Results: The Case Group consisted of 189 healthcare-associated infections, including ventilator-associated pneumonia (61.4%), Central Line-Associated Bloodstream Infection (30.1%), Catheter-Associated Urinary Tract Infection (8.5%), and the Control Group consisted of 6,834 patients. The independent risk factors were COVID-19 infection (OR=2.84, 95%CI=1.92–4.23, p<0.01), length of stay in intensive care greater than 14 days (OR=3.15, 95%CI=1.95–5.14, p<0.01), length of hospital stay greater than 30 days (OR=3.64, 95%CI=2.44–5.51, p<0.01), and patients who were in the third wave (OR=1.72, 95%CI=1.05–2.91, p=0.04). Ventilator-associated pneumonia was the only healthcare-related infection for which COVID-19 infection was an independent risk factor (OR=3.32, 95%CI=1.92–5.94, p<0.01). Conclusion: COVID-19 and length of hospital stay were independent risk factors for healthcare-associated infections, and only ventilator-associated pneumonia was affected by the COVID-19 pandemic. RESUMO Objetivos: mapear os fatores associados à elevação do nível de lactato no pós-operatório de cirurgia cardíaca com uso de circulação extracorpórea. Métodos: trata-se de uma revisão de escopo realizada em dezembro de 2022, em dez fontes de dados. Foi elaborada conforme as recomendações do Instituto Joanna Briggs e do checklist Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews. ABSTRACT Objectives: to map the factors associated with increased lactate levels in the postoperative period of cardiac surgery using extracorporeal circulation. Methods: this is a scoping review carried out in December 2022, across ten data sources. It was prepared in accordance with the recommendations of the Joanna Briggs Institute and the Preferred Reporting Items for Systematic Reviews and Meta Analyses Extension for Scoping Reviews checklist. Results: the most recurrent findings in studies regarding the factors responsible for the increase in lactate were: tissue hypoperfusion, cardiopulmonary bypass time and use of vasoactive drugs. In 95% of studies, increased lactate was related to increased patient mortality. Conclusions: discussing the causes of possible complications in cardiac surgery patients is important for preparing the team and preventing complications, in addition to ensuring quality recovery. ABSTRACT Background: The current treatments for Chagas disease (CD) include benznidazole and nifurtimox, which have limited efficacy and cause numerous side effects. Triazoles are candidates for new CD treatments due to their ability to eliminate T. cruzi parasites by inhibiting ergosterol synthesis, thereby damaging the cell membranes of the parasite. Methods: Eleven synthetic analogs of the kinase inhibitor SRPIN340 containing a triazole core (compounds 6A-6K) were screened in vitro against the Tulahuen strain transfected with β-galactosidase, and their IC50, CC50, and selectivity indexes (SI) were calculated. Compounds with an SI > 50 were further evaluated in mice infected with the T. cruzi Y strain by rapid testing. Results: Eight compounds were active in vitro with IC50 values ranging from 0.5-10.5 µg/mL. The most active compounds, 6E and 6H, had SI values of 125.2 and 69.6, respectively. These compounds also showed in vivo activity, leading to a reduction in parasitemia at doses of 10, 50, and 250 mg/kg/day. At doses of 50 and 250 mg/kg/day, parasitemia was significantly reduced compared to infected untreated animals, with no significant differences between the effects of 6E and 6H. Abstract Background: Loxoscelism refers to a set of clinical manifestations caused by the bite of spiders from the Loxosceles genus. The classic clinical symptoms are characterized by an intense inflammatory reaction at the bite site followed by local necrosis and can be classified as cutaneous loxoscelism. This cutaneous form presents difficult healing, and the proposed treatments are not specific or effective. This study aimed to evaluate the protective effect of mesenchymal stromal cells-derived secretome on dermonecrosis induced by Loxosceles intermedia spider venom in rabbits. Methods: Sixteen rabbits were distributed into four groups (n = 4). Except for group 1 (G1), which received only PBS, the other three groups (G2, G3, and G4) were initially challenged with 10 μg of L. intermedia venom, diluted in 100 μL of NaCl 0.9%, by intradermic injection in the interscapular region. Thirty minutes after the challenge all groups were treated with secretome, except for group 2. Group 1 (G1-control group) received intradermal injection (ID) of 60 μg of secretome in 0.15 M PBS; Group 2 (G2) received 0.9% NaCl via ID; Group 3 (G3) received 60 μg of secretome, via ID and Group 4 (G4), received 60 μg of secretome by intravenous route. Rabbits were evaluated daily and after 15 days were euthanized, necropsied and skin samples around the necrotic lesions were collected for histological analysis. Results: Rabbits of G1 did not present edema, erythema, hemorrhagic halo, or necrosis. In animals from G2, G3, and G4, edema appeared after 6h. However, minor edema was observed in the animals of G2 and G3. Hemorrhagic halo was observed in animals, six hours and three days after, on G2, G3, and G4. Macroscopically, in G4, only one animal out of four had a lesion that evolved into a dermonecrotic wound. No changes were observed in the skin of the animals of G1, by microscopic evaluation. All animals challenged with L. intermedia venom showed similar alterations, such as necrosis and heterophilic infiltration. However, animals from G4 showed fibroblast activation, early development of connective tissue, neovascularization, and tissue re-epithelialization, indicating a more prominent healing process. Conclusion: These results suggest that secretome from mesenchymal stromal cells cultured in a xeno-free and human component-free culture media can be promising to treat dermonecrosis caused after Loxosceles spiders bite envenoming. RESUMO Os frutos da juçara (Euterpe edulis Martius), importante palmeira nativa do ecossistema da Mata Atlântica, exibe uma rica diversidade de microrganismos, incluindo bactérias lácticas (LAB). As LAB são altamente capazes de sintetizar exopolissacarídeos (EPS), que são biopolímeros de carboidratos extracelulares com propriedades tecnofuncionais e, consequentemente, ampla aplicabilidade. No presente estudo, informações tecnofuncionais da dextrana produzida por L. pseudomesenteroides JF17 foram obtidas através da determinação de suas propriedades físico-químicas, reológicas, de retenção de água, de retenção de óleo e emulsificantes. A dextrana-JF17 era composta principalmente por carboidratos (87,87% ± 1,45%, p/p), juntamente com 48,86% ± 1,2% (p/p) de fibras solúveis e baixo teor de proteínas (2,98% ± 0,49%, p/p). A dextrana exibiu alta capacidade de retenção de água (470,89% ± 39,67%, v/v) e de retenção de óleo (89,945% ± 4,16%, v/v), com valores próximos aos relatados para outros EPS microbianos. As soluções de dextrana-JF17 com concentrações de 0,5%, 1,0% e 2,0% (p/v) exibiram o comportamento típico de fluido pseudoplástico não newtoniano. A capacidade emulsificante da dextrana-JF17 foi superior (26,73 ± 0,31 mL de óleo/g de dextrana) à da goma guar (15,45 ± 1,33 mL de óleo/goma). Além disso, a dextrana-JF17 apresentou uma atividade emulsificante de aproximadamente 65% (p/v) em 24 horas, indicando uma forte capacidade de estabilização de emulsões. ABSTRACT Juçara fruits (Euterpe edulis Martius), an important palm tree native to the Atlantic Forest ecosystem, exhibit a rich diversity of microorganisms, including lactic acid bacteria (LAB). LAB are highly capable of synthesizing exopolysaccharides (EPS), which are extracellular carbohydrate biopolymers with techno-functional properties, and consequently, wide applicability. In the present study, the technical-functional information of the dextran produced by L. pseudomesenteroides JF17 was obtained by determining its physicochemical, rheological, water-holding, oil-holding, and emulsifying properties. Dextran-JF17 mainly comprised carbohydrates (87.87% ± 1.45%, w/w), along with 48.86% ± 1.2% (w/w) of soluble fibers and a low protein content (2.98% ± 0.49%, w/w). The dextran exhibited high water-holding (470.89% ± 39.67%, w/v) and oil-holding (89.945% ± 4.16%, w/v) capacities, with values close to those reported for other microbial EPS. The dextran-JF17 solutions with concentrations 0.5%, 1.0%, and 2.0% (w/v) exhibited the typical pseudoplastic non-Newtonian fluid behavior. The emulsifying capacity of dextran-JF17 was higher (26.73 ± 0.31 mL of oil/g dextran) than that of guar gum (15.45 ± 1.33 mL oil/gum). In addition, dextran-JF17 presented an emulsifying activity of approximately 65% (w/v) within 24 h, indicating a strong emulsion stabilization capacity. Collectively, these results suggested that dextran-JF17 is a good candidate for application as a viscosity, stabilizing, and emulsifying agent in the food and pharmaceutical industries. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT The aim of this study was to compare the in vitro activity of delafloxacin with other fluoro-quinolones against bacterial pathogens recovered from inpatients with osteomyelitis, Acute Bacterial Skin and Skin-Structure Infections (ABSSSI). In total, 100 bacterial isolates (58 % Gram-negative and 42 % Gram-positive) recovered from inpatients between January and April 2021, were reidentified at species level by MALDI-TOF MS. Antimicrobial susceptibility testing was conducted using the broth microdilution method and the detection of bio-film formation was assessed through the microtiter plate assay. The screening for mecA was carried out by PCR, while mutations in the Quinolone Resistance Determining Regions (QRDR), specifically gyrA and parC, were analyzed using PCR followed by Sanger sequencing. Results showed that delafloxacin exhibited greater in vitro potency (at least 64-times) than the other tested fluoroquinolones (levofloxacin and ciprofloxacin) when evaluating Staphylococcus aureus (MIC50 ≤0.008 mg/L) and coagulase-negative Staphylococcus (MIC50 0.06 mg/L). Furthermore, delafloxacin (MIC50 0.25 mg/L) was at least 4 times more potent than other tested fluoroquinolones (MIC50 1 mg/L) against P. aeruginosa. No difference in delafloxacin activity (MIC50 0.03 mg/L) was observed against Enterobacter cloacae when compared with ciprofloxacin (MIC50 0.03 mg/L). Despite presenting low activity against K. pneumoniae isolates (22.2 %), delafloxacin exhibited twice the activity compared to both levofloxacin and ciprofloxacin. Delafloxacin also exhibited a strong activity (71.4 %–85.7 %.) against biofilm producing bacterial pathogens tested in this study. Interestingly, 82.14 % of the staphylococci tested in this study harbored mecA gene. In addition, the gyrA and parC genes in fluoroquinolone-resistant Gram-negative isolates displayed different mutations (substitutions and deletions). Resumo A Hipereosinofilia (HE) é definida como uma contagem de eosinófilos superior a 1500 células/microL no sangue periférico em dois exames, realizados com intervalo mínimo de um mês e/ou confirmação anatomopatológica de HE, com eosinófilos compreendendo mais de 20% de todas as células nucleadas da medula óssea. A Síndrome hipereosinofílica (SHE) indica a presença de HE com comprometimento de órgãos por ação eosinofílica, podendo ser classificada como primária (ou neoplásica), secundária (ou reativa) e idiopática. O comprometimento cardíaco ocorre em até 5% dos casos na fase aguda e em 20% na fase crônica da doença, variando de casos oligossintomáticos até miocardite aguda fulminante ou cardiomiopatia restritiva crônica (endomiocardite de Loeffler). No entanto, o grau de disfunção cardíaca não se correlaciona diretamente com o grau de eosinofilia. O envolvimento cardíaco na SHE ocorre em três fases: necrótica inicial, trombótica e necrótica final. Pode se manifestar como insuficiência cardíaca, arritmias e fenômenos tromboembólicos. Abstract Hypereosinophilia (HE) is defined as an eosinophil count exceeding 1500 cells/microL in peripheral blood in two tests, performed with an interval of at least one month and/or anatomopathological confirmation of HE, with eosinophils comprising more than 20% of all nucleated cells in the bone marrow. Hypereosinophilic syndrome (HES) indicates the presence of HE with organ involvement due to eosinophil action, which can be classified as primary (or neoplastic), secondary (or reactive), and idiopathic. Cardiac involvement occurs in up to 5% of cases in the acute phase and 20% of the chronic phase of the disease, ranging from oligosymptomatic cases to fulminant acute myocarditis or chronic restrictive cardiomyopathy (Loeffler endomyocarditis). However, the degree of cardiac dysfunction does not directly correlate with the degree of eosinophilia. The cardiac involvement of HES occurs in three phases: initial necrotic, thrombotic, and finally necrotic. It can manifest as heart failure, arrhythmias, and thromboembolic phenomena. The diagnosis of cardiopathy is based on multimodality imaging, with an emphasis on the importance of echocardiography (echo) as the primary examination. RESUMO Objetivos: avaliar os fatores sociocognitivos determinantes na adesão às precauções padrão pelos profissionais de enfermagem na prática assistencial na pandemia de COVID-19 no Brasil. Métodos: estudo transversal analítico, realizado com 9.039 profissionais de enfermagem do Brasil, através de um formulário eletrônico contendo variáveis sociodemográficas, de formação e laborais dos participantes, e a versão brasileira do Standard Precautions Questionnaire. Utilizou-se estatística descritiva e inferencial pelo software estatístico R. Resultados: os participantes reconhecem precauções padrão como medidas eficazes para reduzir infecções e relataram intenção de segui-las. A capacitação quanto às precauções padrão foi evidenciada como facilitador da adesão (4,72; DP: 0,73), e problemas relacionados a materiais (3,78; DP: 1,45) foi um dificultador. Conclusões: entre os fatores determinantes, a organização apresentou maior pontuação, seguida da intenção de seguir. A identificação dos fatores facilitadores e dificultadores possibilita desenvolver estratégias interventivas visando fortalecer a segurança do paciente e diminuir os riscos ocupacionais entre profissionais. ABSTRACT Objectives: to assess the socio-cognitive factors determining adherence to standard precautions by nursing professionals in care practice during the COVID-19 pandemic in Brazil. Resumo O contexto do Sistema Único de Saúde é atravessado por diversas iniciativas formadoras direcionadas aos profissionais de saúde, aos gestores e à sociedade civil organizada, dentre as quais destacam-se os processos relacionados à temática do monitoramento e avaliação em saúde. Contudo, pouco se observam iniciativas nessa área voltadas para o controle social do referido sistema. O objetivo deste artigo é analisar a trajetória formativa e pedagógica do curso de formação em monitoramento e avaliação para o controle social no Sistema Único de Saúde, de modo a delinear reflexões e apontamentos pertinentes a processos formativos destinados a pessoas conselheiras e apoiadoras técnicas atuantes nos espaços de participação social em saúde. Trata-se de um artigo com orientação informativa, referenciada, dialogada e crítica, subdividido em três seções. Abstract The context of the Unified Health System is crossed by various training initiatives aimed at health professionals, managers, and organized civil society, among which the processes related to health monitoring and evaluation stand out. However, few initiatives in this area have been aimed at social control of this system. This article aims to analyze the formative and pedagogical trajectory of the training course in monitoring and evaluation for social control in the Unified Health System to outline reflections and notes pertinent to formative processes aimed at councilors and technical supporters working in spaces of social participation in health. This is an informative, referenced, dialogued and critical article, subdivided into three sections. Resumen El contexto del Sistema Único de Salud está atravesado por diversas iniciativas de formación dirigidas a profesionales sanitarios, gestores y sociedad civil organizada, entre las que destacan los procesos relacionados con el control y la evaluación sanitaria. Sin embargo, son pocas las iniciativas en este ámbito dirigidas al control social del sistema. El objetivo de este artículo es analizar la trayectoria formativa y pedagógica del curso de capacitación en monitoreo y evaluación para el control social en el Sistema Único de Salud, con el fin de delinear reflexiones y apuntes pertinentes a procesos formativos dirigidos a consejeros y técnicos de apoyo que trabajan en espacios de participación social en salud. Se trata de un artículo informativo, referenciado, dialogado y crítico, subdividido en tres secciones.
