ABSTRACT Introduction: This study evaluated outcomes and risk factors for COVID-19 in 91 Brazilian multiple myeloma (MM) patients between April 2020 and January 2022. Results: Of the 91 MM patients diagnosed with COVID-19, 64% had comorbidities and 66% required hospitalization due to COVID-19, with 44% needing ventilatory support and 37% intensive care. Age (OR 2.02; 95%CI 1.02 – 7.7) and hypertension OR 4.5; 95%CI 1.3 – 15.5) were independently associated with hospitalization and certain MM therapies (corticosteroids and monoclonal drugs) were associated with ventilatory support (OR 4.3; 95%CI 1.3 – 14 and OR 5.7; 95%CI 1.8 – 18, respectively), while corticosteroids and immunomodulatory drugs were linked to ICU admission (OR 5.1; 95% CI 1.4 – 18 and OR 3.4; 95%CI 1.1 – 10, respectively). The overall mortality rate was 30%, with the highest rate observed in the ICU (73%). Additionally, the ECOG performance status was linked to increased mortality (OR 11.5; 95%CI 1.9 – 69). The MM treatment was delayed in 63% of patients who recovered from COVID-19. Conclusions: The findings highlight the need for preventing COVID-19 and prioritizing vaccination among MM patients, as they have high rates of severe outcomes in the event of COVID-19. It is also essential to monitor the potential clinical impacts of COVID-19 on MM patients in the long-term. Given the limited resources available in treating MM patients in Brazil during the COVID-19 pandemic, outcomes might be worse in this population. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. The importance of the CTFB is evidenced by the elevated number of citations in the scientific literature in diverse areas of biology, law, anthropology, education, forensic science, and veterinary science, among others. publications problem uptodate up date classifications context exception (CTFB http//fauna.jbrj.gov.br/, httpfaunajbrjgovbr http //fauna.jbrj.gov.br/ , jbrj gov br (http://fauna.jbrj.gov.br/) 2015 Brazil 80 specialists 1 2024 133691 133 691 133,69 125138 125 138 125,13 82.3%, 823 82 3 (82.3% 102000 102 000 102,00 7.69%, 769 7 69 (7.69% 11000 11 11,00 . 3,567 3567 567 (3,56 2,292 2292 2 292 (2,29 1,833 1833 833 (1,83 1,447 1447 447 (1,44 1000 1,00 831 (83 628 (62 606 (60 520 (52 50 users science health biology law anthropology education others http//fauna.jbrj.gov.br/ faunajbrjgovbr //fauna.jbrj.gov.br (http://fauna.jbrj.gov.br/ 201 8 202 13369 13 133,6 12513 12 125,1 82.3% (82.3 10200 10 00 102,0 7.69% 76 6 (7.69 1100 11,0 3,56 356 56 (3,5 2,29 229 29 (2,2 1,83 183 83 (1,8 1,44 144 44 (1,4 100 1,0 (8 62 (6 60 52 (5 5 http//fauna.jbrj.gov.br (http://fauna.jbrj.gov.br 20 1336 133, 1251 125, 82.3 (82. 1020 0 102, 7.69 (7.6 110 11, 3,5 35 (3, 2,2 22 (2, 1,8 18 (1, 1,4 14 4 ( 82. ABSTRACT In 2022, an outbreak of monkeypox is being reported in non-endemic areas, with unusual clinical manifestations. The detailed clinical description of the first patient that received the diagnosis of monkeypox in Brazil is reported here, whose clinical manifestations can easily lead to misdiagnosis of sexually transmitted infections. A 41 years old male presented to an emergency room with a vesicular rash with eight days of evolution. He had traveled to Portugal and Spain and reported non-penetrative sexual involvement with three different male individuals. On the third day of symptoms, he sought medical care and received empirical treatment directed to sexually transmitted infections. As the symptoms did not improve, he sought medical attention at an infectious disease referral center presenting, on admission, an ulcerated penile lesion with central necrotic crusts, a disseminated pleomorphic skin rash and an oropharyngeal ulcer. The monkeypox diagnosis was suspected due to the characteristics of the lesions and the history of intimate contact with casual partners, and it was later confirmed by sequencing the almost complete monkeypox genome. The patient was hospitalized for pain control, which required opiate administration. He developed a secondary bacterial infection on the penile lesions, which were treated with oral antibiotics. He was discharged after 14 days, with lesions in process of re-epithelialization. Given the current outbreak, we must consider the possibility of monkeypox in patients with suggestive lesions, anywhere on the body (including the genitals), added to an epidemiological link or history of intimate contact with strangers or casual partners.

ABSTRACT Twenty-seven children aged seven months to 5 years were inadvertently vaccinated with a COVID-19 vaccine, the CoronaVac (Sinovac, China), an inactivated SARS-CoV-2 vaccine, in two different cities of Sao Paulo State, Brazil. After the event, these children were monitored by local pediatricians and serum samples were collected at the first visit and 30 days after vaccination and tested for SARS-CoV-2 S1 serology with Ortho total IgG anti-S1 protein and Cpass, an ACE2 receptor binding domain inhibition assay. Only one child had a mild symptom after vaccination, with no other adverse events documented up to the 30 days follow-up. Of 27 children tested 3-9 days after vaccination, 5 (19%) had positive serology suggesting a previous natural SARS-CoV-2 infection, with all 19 tested on day 30 after vaccination and presenting with positive tests, with an increment of antibody titers in those initially positive. A low Cpass binding inhibition was observed in the first collection in 11 seronegative cases, with high titers among those anti-S1 positive. All children showed an important increase in antibody titers on day 30. The event allowed the documentation of a robust serological response to one dose of CoronaVac in this small population of young children, with no major adverse effects. Although it was an unfortunate accident, this event may contribute with future vaccine strategies in this age group. The data suggest that CoronaVac is safe and immunogenic for children.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

Abstract Introduction/Objectives: Repair of post-dissection thoraco-abdominal aortic aneurysms (PD-TAAA) is a complex challenge. Choosing the correct approach to manage these aneurysms is not straightforward as both open and endovascular strategies are valid. Our aim was to analyze and compare the results of PD-TAAA treated by endovascular or open surgery. Methods: A seven-year (January-2013 and May-2020) single-center retrospective cohort study of patients with PD-TAAA treated by endovascular (group-1) or open (group-2) surgery was conducted. Primary outcome was in-hospital mortality. Secondary outcomes were organ lesion, in-hospital infections, length of stay (LOS), endoleaks, branch occlusions, re-interventions and mortality during follow-up. Results: Twenty-one patients (15-men) were treated: 8 in group-1 and 13 in group-2. The mean age was lower in group-2 [68 (SD:11) versus 48 (SD:12), p=0.004]. Three patients had connective tissue disease (CTD). Group-1 patients had a higher ASA score (p<0.001). In group-1, debranching and TEVAR were performed in 2 patients and custom-made fenestrated/branched-endografts were used in 6. In group-2, there was one thoracic aorta interposition graft and reconstruction involving the visceral arteries ocurred in 12 patients. Seven cases were operated using the Crawford technique with visceral patch, and branched grafts were used in 3 patients with CTD. Intercostal arteries were revascularized in 5 patients. In-hospital mortality was 12% (1 patient) in group-1 and 15% (2 patients) in group-2, LogRank=0.9. The LOS was longer in group-2 (p=0.033), and there was a tendency for a longer stay in intensive care unit in this group. No difference was observed in spinal cord ischemia, acute kidney injury or re-interventions. There were more post-operative infections in group-2 (12% versus 69%, p = 0.017). During follow-up [median 15 months (IQR:55)], there was no mortality after discharge. In group-1, 14% had type-II-endoleaks, without aneurysmal sac dilation. Branch permeability during follow-up was 100% in group-1 and 95% in group-2, LogRank=0.3. Conclusion: Endovascular and open surgery of PD-TAAA allowed treatment of a wide variety of patients in this cohort. Patients treated by the endovascular surgery were older and had higher surgical risk but without repercussions on the outcomes. Open surgery was associated with longer hospital stay and more postoperative complications.

ABSTRACT The pharmacological management of asthma has changed considerably in recent decades, as it has come to be understood that it is a complex, heterogeneous disease with different phenotypes and endotypes. It is now clear that the goal of asthma treatment should be to achieve and maintain control of the disease, as well as to minimize the risks (of exacerbations, disease instability, accelerated loss of lung function, and adverse treatment effects). That requires an approach that is personalized in terms of the pharmacological treatment, patient education, written action plan, training in correct inhaler use, and review of the inhaler technique at each office visit. A panel of 22 pulmonologists was invited to perform a critical review of recent evidence of pharmacological treatment of asthma and to prepare this set of recommendations, a treatment guide tailored to use in Brazil. The topics or questions related to the most significant changes in concepts, and consequently in the management of asthma in clinical practice, were chosen by a panel of experts. To formulate these recommendations, we asked each expert to perform a critical review of a topic or to respond to a question, on the basis of evidence in the literature. In a second phase, three experts discussed and structured all texts submitted by the others. That was followed by a third phase, in which all of the experts reviewed and discussed each recommendation. These recommendations, which are intended for physicians involved in the treatment of asthma, apply to asthma patients of all ages.

ABSTRACT Hexapods, commonly known as insects, are a neglected taxonomic group in the Fernando de Noronha archipelago, with unanswered questions about their species richness and the ecological processes in which they are involved (e.g., colonization, introduction, establishment, and extinction). Herein, we provide an updated Hexapod checklist with current nomenclatural combinations. The entomofauna of the Fernando de Noronha archipelago is currently composed of 453 species in 21 orders. The orders, and their respective number of species, are: Blattaria (9), Coleoptera (118), Collembola (29), Dermaptera (3), Diplura (1), Diptera (134), Embioptera (1), Hemiptera (29), Hymenoptera (59), Isoptera (2), Lepidoptera (25), Mantodea (1), Neuroptera (3), Odonata (5), Orthoptera (11), Phasmatodea (1), Phthiraptera (6), Psocoptera (3), Siphonaptera (1), Thysanoptera (10), and Zygentoma (2). The archipelago has 263 new taxon records (family + genera + species). Thirty-eight species (3.39%) were described from local specimens and most of them are likely endemic species. This study more than doubles our knowledge (from the previous 190 records) of the entomofauna in this large Brazilian archipelago. This study also provides a baseline for studies on its conservation status and for implementing future environmental management programs.

RESUMO Objetivo: O minimally invasive repair of pectus excavatum (MIRPE, reparo minimamente invasivo do pectus excavatum) é um tratamento cirúrgico do PE.
ABSTRACT Objective: Minimally invasive repair of pectus excavatum (MIRPE) is a surgical treatment for PE. During the procedure, a specialized introducer is used to tunnel across the mediastinum for thoracoscopic insertion of a metal bar. There have been reported cases of cardiac perforation during this risky step. The large introducer can be a dangerous lever in unskilled hands. We set out to determine the safety and feasibility of using regular instruments (i.e., not relying on special devices or tools) to create the retrosternal tunnel during MIRPE. Methods: This was a preliminary study of MIRPE with regular instruments (MIRPERI), involving 28 patients with PE. We recorded basic patient demographics, chest measurements, and surgical details, as well as intraoperative and postoperative complications. Results: Patients undergoing MIRPERI had Haller index values ranging from 2.58 to 5.56. No intraoperative complications occurred. Postoperative complications included nausea/vomiting in 8 patients, pruritus in 2, and dizziness in 2, as well as atelectasis, pneumothorax with thoracic drainage, pleural effusion, and dyspnea in 1 patient each. Conclusions: In this preliminary study, the rate of complications associated with MIRPERI was comparable to that reported in the literature for MIRPE. The MIRPERI approach has the potential to improve the safety of PE repair, particularly for surgeons that do not have access to certain special instruments or have not been trained in their use.

ABSTRACT The hydrogen is a fuel with a high energy density. Studies have sought to verify the potential of different residues and configurations of reactors in the production of biohydrogen. This study evaluated the influence of the organic loading rate on the production of biohydrogen from cassava starch wastewater (CSW) in anaerobic sequencing batch biofilm reactors (AnSBBR) operated in a fed-batch. In the tests, the organic loading rates of 9.0; 13.5 and 18.0 gCT L−1 d−1 were used, at cycle times (CT) of 4; 4 and 2 h (tests I, II and III, respectively). The reactor was inoculated with sludge from a pilot anaerobic reactor used in the treatment of CSW, heat treated (heated at 90°C for 15 min). With the increase of the organic load (condition I to II) with the same cycle time (CT: 4 h), there were increases in the hydrogen molar productivity (PrM) and hydrogen yield in relation to applied total carbohydrate (RMCA) and removed (RMCR). However, with the increase in the organic loading rate in which the CT was reduced from 4 to 2 h (condition II to III), there was a decrease in PrM and RMCA and RMCR. The best performance of the reactor was verified in condition II (organic loading rate: 13.5 gTC L− 1 d−1; CT: 4 h), in which the PrM was 35.8 molH2 m−3 d−1 and RMCA and RMCR were 2.24 and 3.67 molH2 kgTC−1, respectively.

Abstract: This subject sought to analyze Chilean physical education students’ social representations by exploring what physical education means to them. A non-experimental, cross-sectional and mixed-method design was used. The sample was made up of 200 students from the pedagogy in physical education degree in 12 Chilean universities, who were selected under purposive non-probability criteria. The results show that the defining words with the greatest semantic weight are “health”, “sport”, “motor skills”, and “movement”. The study concludes that social representations in this field are linked to health and well-being, and relate to movement and the study of human motor skills, providing pedagogical value through aspects such as physical activity, recreation, and sport.
ABSTRACT This study aimed to propose a clustering methodology with bootstrap resampling using the Additive Main Effects and Multiplicative Interaction Analysis (AMMI) to contribute to better prediction of phenotypic stability of genotypes and environments. It also aims to analyze the genetic divergence in the assessment of soybean lines, identify genotypes with high-yielding characteristics, with control of chewing and sucking insect pests, and cluster similar genotypes for the traits evaluated. A total of 24 experiments were conducted in randomized blocks, with two replications subdivided in experimental groups with common controls. AMMI with principal component analysis indicated that PC1 and PC2 were significant, explaining 83.9% of the sum of squares of the interaction. The first singular axis of AMMI analysis captured the highest percentage of "pattern" and, with subsequent accumulation of the dimensions of the axes, there was a decrease in the percentage of "pattern" and an increase in "noise". The Euclidean distance between genotype scores was used as the dissimilarity measure and clusters were obtained by the hierarchical method of Ward. Genotypes 97-8011, 97-8029, 97-8050 and IAS-5 had the best performance and are promising for recommendation purposes, with the greatest stability and best performance on grain yield.

Abstract The scope of this article is to determine the distribution and frequency of consumption of anxiolytic benzodiazepines and the correlation between consumption and demographic, epidemiological, economic and social characteristics. It is an ecological study with a sample of 27 state capitals. Data collection was performed through the ANVISA database for the dispensation of Alprazolam, Bromazepam, Clonazepam, Diazepam and Lorazepam in 2010-2012, the 2010 Demographic Census (IBGE), DATASUS and Medical Demographic Research. Descriptive statistical analysis and multiple linear regression analyses were performed for data analysis. The northern region has capitals with the lowest and the southeast has capitals with the highest average consumption of these products. The average consumption for the population of all capitals was 3.60 DHD. Alprazolam is the drug most dispensed by pharmacies and private drugstores with average 2.00 DHD for the capitals. Multiple linear regression analysis showed that 76% of the variation was explained by population density (β = 0.310 p = 0.045) and percentage of physicians (β = 0.507 p = 0.016). The consumption of short half-life anxiolytics has been on the increase, mainly in the cities of greater population density and concentration of physicians.
