Urea is the most widely used nitrogen fertilizer worldwide. However, ammonia volatilization, resulting from applying urea to the soil surface, causes economic and environmental losses; thus, urease inhibitors have been developed to mitigate these losses. In this work, the anti-ureolytic activity of Schiff’s base 4-(3-hydroxybenzylideneamino) phenol (3B4) and its amine-derived (3B4a) was evaluated. The most promising urease inhibitor in soil was 3B4 (55.0 ± 3.9% inhibition), with comparable results to N-(butyl) thiophosphoric triamide (p = 0.659). In the in vitro analysis (Canavalia ensiformis), the results of anti-ureolytic activity were similar, 22.6 ± 6.9% for 3B4 and 24.2 ± 9.6% for 3B4a. Biophysical interaction studies were also carried out through molecular docking studies and molecular fluorescence spectroscopy. These studies showed that both substances are preferentially competitive inhibitors, with the interaction between 3B4a and urease forming a more stable complex. In the analysis by Fourier transform infrared spectroscopy, no interaction was observed when 3B4 or 3B4a was mixed with urea (1:1) for 48 h, providing evidence of compatibility. A scalable total synthesis for deoxyrhapontigenin (DRG) was successfully accomplished using low-cost a-resorcylic acid as a starting material. Our approach initiates with the protection of phenolic groups using methoxymethyl, resulting in the synthesis of a derivative of the expensive 3,5-hydroxybenzaldehyde, a critical component for Wittig and Perkin approaches in stilbenoid synthesis. Additionally, the corresponding olefin for Heck reaction and formal synthesis of resveratrol were produced in over 85% yield. The synthesis of deoxyrhapontigenin was accomplished in seven steps from the initial carboxylic acid, yielding a product with an average purity higher than 98%, as confirmed by quantitative nuclear magnetic resonance (NMR) analysis. In this investigation, we synthesized magnetic nanoparticles coated with silica and functionalized with p-sulfonic acid calix[4]arene (CX4), to be used as heterogeneous acid catalysts for acetal synthesis. We evaluated the catalytic efficiency of these nanoparticles for acetal synthesis using various aldehydes and glycerol under microwave irradiation at atmospheric pressure, with and without solvents. Two distinct synthesis methods, Pechini (Fe/pch/Si/CX4) and precipitation (Fe/ppt/Si/CX4), resulted in variations in the physical and chemical properties of the solids produced. Structural characterization involved thermogravimetry analysis (TG), X-ray diffraction (XRD), Mössbauer spectroscopy, vibrating sample magnetometry, scanning and transmission electron microscopy (SEM and TEM), infrared spectroscopy (FTIR), and titration for acidic site quantification. Fe/ppt/Si/CX4 exhibited goethite phases, while Fe/pch/Si/CX4 showed a substantial proportion of maghemite. Both catalysts demonstrated significant activity in acetal synthesis, achieving approximately 80% conversion without solvents. The increase in food production to meet the demand of the world’s growing population is largely conditioned by the efficiency of agricultural fertilizers. The development of relatively low-cost urease inhibitors and more environmentally friendly ones is a challenge for agriculture, especially because urea is the most used nitrogen fertilizer globally. The pyroligneous acid (PA), a by-product of wood charcoal production, stimulates plant growth, increases crop productivity, improves microbial metabolism, and induces plant tolerance to (a)biotic stresses. This work investigated the in vitro urease inhibitory potential of tree PAs of distinct plant biomass origin and production process. Coconut (Cocus nucifera) shell PA showed the highest urease inhibitory activity, followed by black wattle (Acacia mearnsii) PA and eucalyptus (Eucalyptus urograndis, a hybrid of Eucalyptus grandis and Eucalyptus urophylla). The same sequence was observed for the content of phenols and humic substances (humic acids + fulvic acids). This fact probably justifies the greater inhibitory activity, since both humic substances and some phenols normally present in PA, such as hydroquinone, have already been reported in the literature as urease inhibitors. In this study, the interaction between benzothiazole (BTA, concentration of a drug required for 50% inhibition in vitro (IC50) = 0.77 mM) and benzimidazole (BIA, IC50 = 2.14 mM) with urease was quantitatively assessed, using UV-Vis, molecular fluorescence, and circular dichroism. The results showed that both compounds interact with urease by a static fluorescence quenching mechanism with a non-fluorescent complex formation. The main forces responsible for stabilizing the supramolecular complex between BTA and urease were hydrophobic while, for BIA, van der Waals interactions and hydrogen bonds were the main ones. Urease conformation changes due to the interaction process were analyzed by circular dichroism and synchronous fluorescence. Besides, a competitive assay with substrate and inhibitors was used to evaluate the preferential urease site of interaction with BTA and BIA. Our experimental and theoretical studies supported that both, BTA and BIA, are mixed-inhibitors of ureases with a slight preference to the active site of such enzymes. Finally, both BTA and BIA showed to possess anti-Helicobacter pylori (one reference strain and six clinical isolates) activity, presenting minimal inhibitory concentration (MIC) values ranging from 38-150 and 20-164 µM, respectively. The urease inhibitors omeprazole and hydroxyurea showed MIC values in the range of 46-185 µM and 1683-> 3366 µM, respectively.

RESUMEN Objetivo: Comprender, a partir del pensamiento fenomenológico de Martin Heidegger, el Ser que sufre una enfermedad coronaria y desarrolla una lesión por presión. Método: Investigación cualitativa, fenomenológica basada en los fundamentos teórico-filosófico-metodológicos de Martin Heidegger. El discurso de nueve entrevistados fue obtenido de octubre a diciembre de 2015, en Ceará. Resultados: Seis unidades fueron reveladas: experimentar dificultades; hacer frente al tratamiento; desconocimiento de la enfermedad cardíaca; recibir apoyo; experimentar cambios resultantes de la enfermedad; y mantener la fe en Dios. La vida se aprehendió en una cotidianidad inauténtica, en el chatear, la curiosidad y la ambivalencia. Atrapados por el vigor de haber sido, tienen angustia, sostenidos por la fe en Dios, siendo-con-los-otros, en un movimiento de solicitud. Conclusión: El fenómeno interfiere en la vida cotidiana de pacientes y familias, haciéndoles vulnerables. Existe la necesidad de que la enfermería reflexione sobre esa experiencia e incorpore cuidados que lleguen hacia la existencia humana. ABSTRACT Objective: To understand, using Martin Heidegger's phenomenological framework, the Being who experiences a heart disease and develops a pressure injury. Method: Qualitative, phenomenological study using Martin Heidegger's theoretical-philosophical-methodological framework. Nine participants were interviewed at their residences, from October to December 2015, Ceará. Results: Six meaning units were revealed: experiencing difficulties; dealing with the treatment of pressure wounds; not knowing the cardiac disease; being supported by family and friends; experiencing changes caused by disease; and maintaining faith in God. RESUMO Objetivo: Compreender, a partir do pensamento fenomenológico de Martin Heidegger, o Ser que vivencia uma coronariopatia e desenvolve uma lesão por pressão. Método: Qualitativo, com base nos pressupostos de Martin Heidegger. Participaram nove depoentes e tais dados foram analisados com base na compreensão vaga e mediana, bem como na hermenêutica compreensiva. Resultados: Foram reveladas as seguintes unidades de significado: vivenciar dificuldades; enfrentar tratamento da lesão por pressão; desconhecer o adoecimento cardíaco; receber apoio de familiares e amigos; vivenciar mudanças advindas do adoecimento; e manter a fé em Deus. RESUMO Objetivo: identificar sintomas de ansiedade e fatores associados entre profissionais de saúde durante pandemia da COVID-19. Método: estudo transversal, realizado com 1.354 profissionais da saúde, cujos dados socioprofissionais, sintomas psicológicos e físicos de ansiedade foram coletados via Google Forms entre 17 e 21 de maio de 2020, no nordeste brasileiro, e analisados pelo teste de Qui-Quadrado de Pearson e regressão logística. Resultados: Obteve-se associação entre sintomas psicológicos e idade (p=0,036), sexo feminino (p=0,015), bem entre sintomas físicos e idade (p=0,000), prestou atendimento a pacientes com COVID-19 (p=0,003), se o profissional apresentou sintomas da COVID-19 (p=0,000), com convívio social com pessoas sintomáticas (p=0,028) e sexo (p=0,000). Conclusão: o estudo pode subsidiar melhor compreensão da vivência do profissional de saúde em relação à pandemia da COVID-19 para elaboração de políticas de cuidado dos profissionais, bem como incentivar novos estudos, tendo em vista a urgência, unicidade e magnitude da situação vivida.

ABSTRACT Objective: to identify anxiety symptoms and associated factors among health professionals during the COVID-19 pandemic. Method: a cross-sectional study carried out with 1,354 health professionals, whose socio-professional data and psychological and physical anxiety symptoms were collected via Google Forms between May 17th and May 21st, 2020, in northeastern Brazil, and analyzed using Pearson’s Chi-Square test and logistic regression. Results: there was an association between psychological symptoms and age (p=0.036) and female gender (p=0.015), as well as between physical symptoms and age (p=0.000), having provided care for patients with COVID-19 (p=0.003), if the professional presented COVID-19 symptoms (p=0.000), with social interaction with symptomatic people (p=0.028) and gender (p=0.000). Conclusion: the study may support a better understanding of the health professionals’ experience in relation to the COVID-19 pandemic for developing policies for the care of professionals, as well as encourage further studies, considering the urgency, uniqueness and magnitude of the situation experienced.

ABSTRACT Objective: Adipose tissue-derived stromal/stem cells (ASCs) and vitamin D have immunomodulatory actions that could be useful for type 1 diabetes (T1D). We aimed in this study to investigate the safety and efficacy of ASCs + daily cholecalciferol (VIT D) for 6 months in patients with recent-onset T1D. Materials and methods: In this prospective, dual-center, open trial, patients with recent onset T1D received one dose of allogenic ASC (1 x 106 cells/kg) and cholecalciferol 2,000 UI/day for 6 months (group 1). They were compared to patients who received chol-ecalciferol (group 2) and standard treatment (group 3). Adverse events were recorded; C-peptide (CP), insulin dose and HbA1c were measured at baseline (T0), after 3 (T3) and 6 months (T6). Results: In group 1 (n = 7), adverse events included transient headache (all), mild local reactions (all), tachycardia (n = 4), abdominal cramps (n = 1), thrombophlebitis (n = 4), scotomas (n = 2), and central retinal vein occlusion at T3 (n = 1, resolution at T6). Group 1 had an increase in basal CP (p = 0.018; mean: 40.41+/-40.79 %), without changes in stimulated CP after mixed meal (p = 0.62), from T0 to T6. Basal CP remained stable in groups 2 and 3 (p = 0.58 and p = 0.116, respectively). Group 1 had small insulin requirements (0.31+/- 0.26 UI/kg) without changes at T6 (p = 0.44) and HbA1c decline (p = 0.01). At T6, all patients (100%; n = 7) in group 1 were in honeymoon vs 75% (n = 3/4) and 50% (n = 3/6) in groups 2 and 3, p = 0.01. Conclusions: Allogenic ASC + VIT D without immunosuppression was safe and might have a role in the preservation of β-cells in patients with recent-onset T1D. ClinicalTrials.gov: NCT03920397.

RESUMO Objetivo O objetivo deste estudo foi avaliar a cinética diafragmática, a função respiratória e a dosagem sérica de leptina e citocinas inflamatórias (IL-6 e TNF-α) em três grupos clínicos: obeso, asmático e saudável. Métodos Estudo clínico-exploratório realizado com 73 jovens (12-24 anos, sendo 42,5% do sexo masculino) alocados em três grupos: obesidade (GO, n = 33), índice de massa corporal (IMC z-score) ≥ + 2 e asmáticos leves controlados (GA, n = 26), classificados pela GINA, e grupo controle saudável (GC, n = 14). Os participantes foram submetidos à ultrassonografia diafragmática, espirometria, pressão respiratória máxima, níveis séricos de leptina e níveis de IL-6 e TNF-α em hemocultura total. Resultados A espessura do diafragma foi maior no GO em comparação ao GA e GC (2,0 ± 0,4 vs 1,7 ± 0,5 e 1,6 ± 0,2, respectivamente, com p < 0,05). A ventilação voluntária máxima (VVM) foi significativamente menor no GO e GA em relação ao GC (82,8 ± 21,4 e 72,5 ± 21,2 vs 102,8 ± 27,3, respectivamente, com p < 0,05). O GO tem a maior taxa de leptina entre todos os grupos (com os outros dois grupos, p < 0,05). Os três grupos tinham níveis semelhantes de TNF-α e IL-6. Conclusão A hipertrofia muscular encontrada no diafragma de indivíduos obesos pode ser justificada pelo aumento do trabalho respiratório imposto pela condição crônica da doença. Esse aumento de espessura não ocorreu em asmáticos leves controlados. Os marcadores IL-6 e TNF-α não detectaram evidências de inflamação muscular, embora fosse esperado que a leptina estivesse alterada em indivíduos obesos. Pacientes obesos e asmáticos apresentaram menor resistência pulmonar do que os saudáveis.

ABSTRACT Objective The aim of this study was to assess the diaphragm kinetics, respiratory function, and serum dosage of leptin and inflammatory cytokines (IL-6 and TNF-α) in three clinical groups: obese, asthmatic, and healthy. Methods This is a clinical exploratory study performed on 73 youths (12-24 years of age, 42.5% male) allocated into three groups: obesity (OG, n=33), body mass index (BMIz-score) ≥ +2, asthmatic (AG, n=26) controlled mild asthmatics, classified by GINA, and Healthy Control Group (CG, n=14). The participants were subjected to diaphragmatic ultrasound, spirometry, maximal respiratory pressure, serum leptin levels, and IL-6 and TNF-α whole blood cell culture levels. Results Diaphragm thickness was higher in OG in comparison to AG and CG (2.0±0.4 vs 1.7±0.5 and 1.6±0.2, both with p<0.05). Maximal voluntary ventilation (MVV) was significantly lower in OG and AG in relation to the CG (82.8±21.4 and 72.5±21.2 vs 102.8±27.3, both with p<0.05). OG has the highest leptin rate among the groups (with the other two groups had p<0.05). All groups had similar TNF-α and IL-6 levels. Conclusion The muscular hypertrophy found in the diaphragm of the obese individuals can be justified by the increase in respiratory work imposed by the chronic condition of the disease. Such increase in thickness did not occur in controlled mild asthmatics. The IL-6 and TNF-α markers detected no evidence of muscle inflammation, even though leptin was expected to be altered in obese individuals. Both obese and asthmatic patients had lower pulmonary resistance than the healthy ones.

Abstract Pereskia grandifolia Haworth (PGH) and Pereskia aculeata Miller (PAM) are recognized sources of proteins; dietary fiber; vitamins and minerals make this plant leaves, raw, cooked, and braised, an important ally against protein and micronutrient deficiencies. One of the main problems is the presence of antinutritional factors that may interfere in the digestibility and bioavailability of some nutrients. The objective was to evaluate the amino acid profile and the chemical score of the raw leaves and the effects of heating media and time on the total dietary fiber, minerals, trypsin inhibition, oxalic acid and tannins of leaves of PGH and PAM. The samples had similar amino acid profiles and total dietary fiber. With regard to antinutritional compounds, heating the leaves of PGH led to a decrease in trypsin inhibition, primarily after the first minutes of wet cooking. Oxalic acid and tannins predominated in both species. Considering the interaction with time, the variables related to iron and zinc minimized the tannin responses in PGH and PAM, respectively. Heating media and times interfered with the chemical components present in the leaves of Pereskia species and led to high antinutrient retention after heat treatment.

RESUMO O carcinoma hepatocelular (CHC) é uma das principais causas de mortalidade relacionada a câncer no Brasil e no mundo. A Sociedade Brasileira de Hepatologia (SBH) publicou em 2015 suas primeiras recomendações sobre a abordagem do CHC. Desde então, novas evidências sobre o diagnóstico e tratamento do CHC foram relatadas na literatura médica, levando a diretoria da SBH a promover uma reunião monotemática sobre câncer primário de fígado em agosto de 2018 com o intuito de atualizar as recomendações sobre o manejo da neoplasia. Um grupo de experts foi convidado para realizar uma revisão sistemática da literatura e apresentar uma atualização baseada em evidências científicas visando que pudesse nortear a prática clínica multidisciplinar do CHC. O texto resultante foi submetido a avaliação e aprovação de todos membros da SBH através de sua homepage. O documento atual é a versão final que contêm as recomendações atualizadas e revisadas da SBH.

ABSTRACT Hepatocellular carcinoma (HCC) is one of the leading causes of cancer-related mortality worldwide. The Brazilian Society of Hepatology (SBH) published in 2015 its first recommendations about the management of HCC. Since then, new data have emerged in the literature, prompting the governing board of SBH to sponsor a single-topic meeting in August 2018 in São Paulo. All the invited experts were asked to make a systematic review of the literature reviewing the management of HCC in subjects with cirrhosis. After the meeting, all panelists gathered together for the discussion of the topics and the elaboration of updated recommendations. The text was subsequently submitted for suggestions and approval of all members of the Brazilian Society of Hepatology through its homepage. The present manuscript is the final version of the reviewed manuscript containing the recommendations of SBH.

Resumo O objetivo foi analisar a distribuição espacial da prevalência de excesso de peso e sua correlação com insegurança alimentar, aquisição de alimentos por tipo de processamento e Índice de Desenvolvimento Humano (IDH), no Brasil. Estudo ecológico, com dados de três inquéritos do Instituto Brasileiro de Geografia e Estatística (IBGE). Utilizou-se técnicas de estatística espacial para análise univariada e bivariada. A prevalência de excesso de peso encontrada para o Brasil foi de 34,2% (IC95% 33,8-34,6%). O excesso de peso apresentou autocorrelação espacial moderada (0,581; p = 0,01) e maiores prevalências nos estados das regiões Sul, Sudeste e Centro-Oeste. As prevalências de excesso de peso apresentaram correlação espacial positiva e moderada com o IDH (0,605; p< 0,05), proporção de aquisição domiciliar de produtos alimentícios ultraprocessados (0,559; p< 0,05); e correlação negativa e moderada com a prevalência de domicílios em insegurança alimentar (-0,561; p< 0,05). Conclui-se que a ocorrência de excesso de peso encontrava-se desigualmente distribuída no território brasileiro. As prevalências mais elevadas foram encontradas nos estados do Sudeste, Sul e Centro-Oeste, associadas a maiores valores de IDH e proporções de aquisição de produtos alimentícios ultraprocessados.

Abstract The aim of this study was to analyze the spatial distribution of excess weight in Brazil and its correlation with household food insecurity, purchase of foods by type of processing, and Human Development Index (HDI). An ecological study was undertaken using data from three surveys conducted by the Brazilian Institute of Geography and Statistics. Spatial analysis techniques were used to perform univariate and bivariate analysis. The prevalence of excess weight was 34.2% (CI 95% 33.8-34.6%). Excess weight showed a moderate and significant spatial autocorrelation (0.581; p = 0.01), with higher prevalence in states in the South, Southeast and Center-West regions. A positive moderate spatial correlation was shown between the prevalence of excess weight and HDI (0.605; p < 0.05) and purchase of ultra-processed foods (0.559; p < 0.05), while a negative moderate spatial correlation was observed between prevalence of excess weight and household food insecurity (-0.561; p < 0.05). It can be concluded that there is an unequal distribution of excess weight across Brazil. The highest prevalence rates were found in states in the Southeast, South, and Center-West regions, associated with higher HDI values and higher ultra-processed food purchases as a proportion of overall household food purchases.

Alzheimer disease (AD) is a complex disease related to multiple pathogenic mechanisms. A strategy to develop effective drugs is based on the so-called multi-target directed ligands (MTDL) by using hybrid compounds. So, in the present study, we have designed and synthesized two hybrids, containing the indanone-piperidine moiety of donepezil, a drug approved for the treatment of AD, and the lipoic acid scaffold, an antioxidant compound endowed with neuroprotective effects. One hybrid was synthesized in four steps with 42% global yield, and the other hybrid in six steps with 19% global yield. The latter hybrid displayed moderate inhibitory activity against human acetylcholinesterase (hAChE) and greater activity against human butyrylcholinesterases (hBuChE). The selectivity for hBuChE was further rationalized by theoretical study. Importantly, the second hybrid showed a good antioxidant activity, exhibiting better ability in scavenging 2,2-diphenyl-1-picrylhydrazyl (DPPH) radicals than lipoic acid.

Xanthenones were synthesized via one-pot tricomponent reaction, under solvent-free conditions, using aldehydes, phenolic and cyclic 1,3-dicarbonyl compounds. Natural organic acids (NOAs), compounds present in many living metabolisms, were used as potential green catalysts. NOA are considered to be more eco-friendly and user-friendly alternative to traditional methodologies. Optimization studies showed that oxalic acid was the best NOA catalyst for such reaction furnishing the xanthenones with up to 93% of yield. Theoretical calculations were performed to evaluate this reaction mechanism and regioselectivity. The results showed that the regiospecificity of this three-component reaction is kinetically and thermodynamically controlled by the addition of β-naphthol C2, instead of C10, to the aldehyde. Our results also disclosed two xanthenones as novel calcium channels blockers. Eco-friendly reaction conditions, easy workup procedure, short reaction times and good yields are some of the advantages of our methodology.
