Efectividad de las terapias de medicina alternativa para el tratamiento complementario de pacientes adultos con hipertensión arterial: una visión general de revisiones sistemáticas

Abstract This study aimed to evaluate the efficacy of nonpharmacological interventions beyond the known and effective lifestyle interventions, as an adjunct to the treatment of adult patients with hypertension, a significant risk factor for fatal cardiovascular diseases. The evaluated interventions included alternative therapies, like traditional Chinese medicine and acupuncture (three studies), mind-body therapy (10 studies) and dietetic/nutraceutical supplements (10 studies). The results suggest that some of these therapies may be promising adjuncts to antihypertensive treatment. Among the mentioned therapies, auricular acupressure, tai chi, yoga, qigong and meditation stand out. In addition, certain nutraceuticals and phytotherapeutic agents like omega-3 fatty acids, coenzyme Q10, vitamin C, probiotics, resveratrol, soy isoflavones, garlic extract and extra virgin olive oil may be recommended as part of a healthy diet, like the Mediterranean or DASH diet, known for its impact on cardiovascular risk. In conclusion, as far as public policies aimed at the comprehensive treatment of chronic noncommunicable diseases are concerned, these therapies, as part of nonpharmacological management, can effectively complement hypertension treatment, providing patients with additional options for controlling their blood pressure and improving their cardiovascular health. However, more research is needed to confirm their long-term efficacy and safety.
Resumen Este estudio se dirigió a evaluar la eficacia de intervenciones no farmacológicas más allá de las conocidas y efectivas intervenciones en el estilo de vida como complemento al tratamiento de pacientes adultos con diagnóstico de hipertensión arterial, factor de riesgo significativo para enfermedades cardiovasculares mortales. Entre las intervenciones evaluadas, se incluyeron las terapias alternativas, como la medicina tradicional china y la acupuntura (tres estudios), las terapias mente-cuerpo (diez estudios) y la suplementación con productos dietéticos/nutracéuticos (diez estudios). Los resultados sugieren que algunas de estas terapias pueden ser prometedoras como complemento al tratamiento antihipertensivo. Entre las terapias mencionadas, se destacan la acupresión auricular, el taichi, el yoga, el qigong y la meditación. Además, ciertos nutracéuticos y fitoterapéuticos, como los ácidos grasos omega 3, la coenzima Q10, la vitamina C, los probióticos, el resveratrol, las isoflavonas de soja, el extracto de ajo y el aceite de oliva extravirgen, pueden ser recomendados como parte de una dieta saludable, como la dieta mediterránea o DASH, reconocida por su impacto en el riesgo cardiovascular. En conclusión, en lo que respecta a las políticas públicas dirigidas al manejo integral de las enfermedades crónicas no transmisibles, estas terapias, dentro del manejo no farmacológico, pueden complementar eficazmente el tratamiento de la hipertensión arterial, brindando a los pacientes opciones adicionales para controlar su presión arterial y mejorar su salud cardiovascular. Sin embargo, se necesita más investigación para confirmar su eficacia y seguridad a largo plazo.

ABSTRACT In 2000, Cleide Costa published a paper presenting the state of knowledge of the Neotropical Coleopte ra, with a focus on the Brazilian fauna. Twenty-four years later, thanks to the development of the Coleoptera section of the Taxonomic Catalog of the Brazilian Fauna (CTFB - Catálogo Taxonômico da Fauna do Brasil) through the collaboration of 100 coleopterists from all over the globe, we can build on Costa’s work and present an updated overview of the state of knowledge of the beetles from Brazil. There are currently 35,699 species in 4,958 genera and 116 families known to occur in the country, including representatives of all extant suborders and superfamilies. Our data show that the Brazilian beetle fauna is the richest on the planet, concentrating 9% of the world species diversity, with some estimates accounting to up to 15% of the global total. The most diverse family in numbers of genera is Cerambycidae (1,056 genera), while in number of species it is Chrysomelidae (6,079 species). Conotrachelus Dejean, 1835 (Curculionidae) is the most species-rich genus, with 570 species. The French entomologist Maurice Pic is the author who has contributed the most to the naming of species recorded from Brazil, with 1,794 valid names in 36 families, whereas the Brazilians Ubirajara R. Martins and Maria Helena M. Galileo are the only ones among the top-ten authors to have named species in the 21st century. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Resumo A onicomicose é a doença mais comum que afeta a unidade ungueal e representa pelo menos 50% de todas as doenças ungueais. Além disso, a Candida albicans é responsável por aproximadamente 70% das onicomicoses causadas por leveduras. Nesse estudo, foi investigado o efeito antifúngico dos enantiômeros (R) e (S)-citronelal, bem como seu mecanismo de ação preditivo sobre C. albicans de onicomicoses resistentes ao voriconazol. Para este propósito, foram aplicadas técnicas in vitro de microdiluição em caldo e docking molecular de forma preditiva e complementar para os mecanismos de ação. Os principais resultados deste estudo indicam que C. albicans foi resistente ao voriconazol e sensível aos enantiômeros (R) e (S)-citronelal na dose de 256 e 32 µg/mL respectivamente. Além disso, houve aumento da concentração inibitória mínima (CIM) dos enantiômeros na presença do sorbitol e do ergosterol, indicando que estas moléculas possivelmente afetem a integridade da parede e da membrana celular de C. albicans. O docking molecular com proteínas chave da biossíntese e manutenção da parede celular e da membrana plasmática fúngica, demonstraram a possibilidade do (R) e (S)-citronelal interagir com duas importantes enzimas: 1,3-β-glucan sintase e lanosterol 14α-demetilase. Abstract Onychomycosis is the most common disease affecting the nail unit and accounts for at least 50% of all nail diseases. In addition, Candida albicans is responsible for approximately 70% of onychomycoses caused by yeasts. This study investigated the antifungal effect of (R) and (S)-citronellal enantiomers, as well as its predictive mechanism of action on C. albicans from voriconazole-resistant onychomycoses. For this purpose, in vitro broth microdilution and molecular docking techniques were applied in a predictive and complementary manner to the mechanisms of action. The main results of this study indicate that C. albicans was resistant to voriconazole and sensitive to the enantiomers (R) and (S)-citronellal at a dose of 256 and 32 µg/mL respectively. In addition, there was an increase in the minimum inhibitory concentration (MIC) of the enantiomers in the presence of sorbitol and ergosterol, indicating that these molecules possibly affect the integrity of the cell wall and cell membrane of C. albicans. Molecular docking with key biosynthesis proteins and maintenance of the fungal cell wall and plasma membrane demonstrated the possibility of (R) and (S)-citronellal interacting with two important enzymes: 1,3-β-glucan synthase and lanosterol 14α-demethylase. A fluidized bed reactor was used to study the degradation of the surfactant linear alkylbenzene sulfonate (LAS). The reactor was inoculated with anaerobic sludge and was fed with a synthetic substrate supplemented with LAS in increasing concentrations (8.2 to 45.8 mg l-1). The removal efficiency of 93% was obtained after 270 days of operation. Subsequently, 16S rRNA gene sequencing and phylogenetic analysis of the sample at the last stage of the reactor operation recovered 105 clones belonging to the domain Bacteria. These clones represented a variety of phyla with significant homology to Bacteroidetes (40%), Proteobacteria (42%), Verrucomicrobia (4%), Acidobacteria (3%), Firmicutes (2%), and Gemmatimonadetes (1%). A small fraction of the clones (8%) was not related to any phylum. Such phyla variety indicated the role of microbial consortia in degrading the surfactant LAS.

The removal of linear alkylbenzene sulfonate (LAS) was evaluated in a UASB reactor using short-chain alcohols (ethanol and methanol) and complex co-substrate (yeast extract). Using only methanol and ethanol as co-substrates resulted in removal of LAS between 30 and 41%. At the end, addition of a complex substrate (yeast extract) increased the removal of LAS to 50%. During the assay, water supply aeration increased the volatile fatty acid of the effluent (70 mg HAc.L-1) and decreased the removal of LAS (from 40 to 30%). According to the fluorescence in situ hybridization (FISH) results, the amount of Archaea decreased due to water supply aeration (from 64 to 48%). Furthermore, addition of complex co-substrate increased the total anaerobic bacteria and methanogenic archaea content (three and four log units, respectively), which were estimated using the most probable number technique.

Aloysia gratissima (Gill. et Hook) Tronc. (Verbenaceae) is native to South America with folk therapeutic applications for a wide range of diseases. The polyphenolic and carotenoid profile, toxicity, and antioxidant activity of aqueous extract of Aloysia gratissima were investigated. HPLC analyses showed high amounts of ferulic acid, trans-cinnamic acid and p-coumaric acid, and also trans-β- carotene and lutein which fluctuated throughout the seasons. Furthermore, the extract investigated not only exerted antioxidant activity but also inhibited lipid peroxidation. Toxicity was achieved only at the highest dose tested. Therefore, A. gratissima is a potential species for medicinal purposes.

Elementos traço e majoritários foram determinados em mexilhão dourado (Limnoperna fortunei) coletados no canal São Gonçalo (Rio Grande-RS, Brasil). Digestão assistida por microondas em sistema fechado foi utilizada para decomposição das amostras com subsequente determinação elementar por técnicas espectrométricas. Os resultados mostraram que o tecido de mexilhão contém Ag, Al, As, B, Ba, Ca, Cd, Co, Cr, Cu, Fe, Hg, Li, Mg, Mn, Mo, P, Pb, S, Sn, Sr, Ti, V e Zn, enquanto que na casca foram encontrados os mesmo elementos, exceto Ag e Hg, evidenciando seu potencial como biomarcador. Neste sentido, estes resultados podem ser utilizados para estabelecer um panorama inicial e contribuir para futuros estudos relacionados a contaminantes inorgânicos na área estudada.
Major and trace element content was determined in golden mussel (Limnoperna fortunei) collected in the São Gonçalo Channel (Rio Grande City, Brazil). A microwave-assisted digestion procedure in closed vessels was applied to mussel decomposition and subsequent determination of elements by spectrometric techniques. Results showed that the mussel tissue contains Ag, Al, As, B, Ba, Ca, Cd, Co, Cr, Cu, Fe, Hg, Li, Mg, Mn, Mo, P, Pb, S, Sn, Sr, Ti, V and Zn, while the same elements (except Ag and Hg) were quantified in its shell, demonstrating its potential as a biomarker. In this sense, these results can be used to establish an initial view and to contribute to further studies related to element contamination in the area under study.

Anaerobic degradation of linear alkylbenzene sulfonate in fluidized bed reactor

An anaerobic fluidized bed reactor was used to assess the degradation of the surfactant linear alkylbenzene sulfonate (LAS). The reactor was inoculated with sludge from an UASB reactor treating swine wastewater and was fed with a synthetic substrate supplemented with LAS. Sand was used as support material for biomass immobilization. The reactor was kept in a controlled temperature chamber (30±1 ºC) and operated with a hydraulic retention time (HRT) of 18 h. The LAS concentration was gradually increased from 8.2±1.3 to 45.8±5.4 mg.L-1. The COD removal was 91%, on average, when the influent COD was 645±49 mg.L-1. The results obtained by chromatographic analysis showed that the reactor removed 93% of the LAS after 270 days of operation.

Introdução:A OMS definiu como objectivo principal do tratamento da obesidade, uma redução de peso de 10 a 15%, para diminuição do risco cardiovascular global e comorbilidades associadas. Objectivos: Caracterizar e analisar a eficácia a curto e longo-prazo, segurança e tolerabilidade do balão intra-gástrico (BIG) no tratamento da obesidade. Métodos: Estudo retrospectivo do tratamento com 210 BIGs, em 194 doentes, entre Outubro/2000 e Junho/2007. A maioria dos doentes apresentava obesidade mórbida (IMC médio 47,8Kg/m2), com comorbilidades significativas. O tempo médio de permanência do BIG foi 7 meses. Resultados: No final do tratamento, constatou-se uma redução ponderal média de 12,9%. Obteve-se “sucesso clínico” (redução ponderal ≥10%), na maioria dos casos (60,5%). Em 2 grupos de 52 doentes hipertensos e 30 diabéticos, registou-se melhoria destas comorbilidades durante o tratamento em 61,5% e 66,7%, respectivamente. Não se registaram complicações significativas associadas ao BIG. Analisando a longo-prazo (12 meses após remoção) um coorte de 91 doentes, a redução média de peso foi 5,6%, sendo significativamente superior (>2 vezes) nos doentes com mais de 50 anos. Conclusões: O BIG é seguro e apresenta uma taxa de sucesso clínico satisfatória a curto-prazo, com melhoria das principais comorbilidades. A longo-prazo, foi mais eficaz nas faixas etárias mais avançadas.
Background: The main goal proposed by the WHO in obesity treatment, in order to reduce cardiovascular risk and related comorbidities, is a 10-15% weight-loss. Aims: To characterize and analyze short and long-term efficacy, safety and tolerability of BioEnterics® Intragastric Balloon (BIB) in obesity treatment. Patients and Methods: Retrospective study on data of 210 BIB procedures in 194 patients, between October/2000 and June/2007. Most patients presented with morbid obesity (mean BMI 47,8Kg/m2) and heavy comorbidities. Average BIB stay was 7 months. Results: On BIB removal, the mean weight-loss was 12,9%. “Clinical success” (weight-loss ≥10%) was achieved in most cases (60,5%). In two groups of 52 hypertensive and 30 diabetic patients, there was a 61,5% and 66,7% improvement in these comorbidities, respectively, during treatment. There were no major complications associated to BIB. On longterm analysis (12 month after BIB removal) of a 91 patients cohort, global weight-loss was 5,6%, being significantly higher (>2-fold) in patients over 50 years-old. Conclusions: BIB is safe, shows satisfactory short-term cli nical success rate, with improvement in co-morbidities. On long-term, higher efficacy was seen in older patients.

Um experimento em casa de vegetação foi conduzido entre novembro de 2005 e abril de 2006, na UNESP, campus de Jaboticabal, SP, objetivando estudar a produção de massa seca, a distribuição e o acúmulo de macronutrientes por Ipomoea nil, uma importante infestante de culturas anuais e perenes no Brasil. As plantas foram cultivadas em vasos de 7 litros com substrato de areia e irrigadas diariamente com solução nutritiva de Hoagland & Arnon. O delineamento experimental foi inteiramente casualizado, com quatro repetições. Os tratamentos corresponderam às épocas de avaliação, em intervalos de 14 dias, iniciandose 21 dias após a emergência (DAE). Em cada avaliação, as plantas de quatro vasos foram analisadas quanto ao acúmulo de massa seca e à nutrição mineral. I. nil apresentou pequeno acúmulo de massa seca e de macronutrientes no início da fase experimental. Esses acúmulos se intensificaram após 77 DAE, atingindo o máximo valor teórico aos 123, 122, 124, 121, 119, 121 e 125 DAE, para massa seca, N, P, K, Ca, Mg e S, respectivamente. Folhas e caules foram os principais órgãos responsáveis pelo acúmulo de massa seca por plantas de I. nil. K e N foram os macronutrientes extraídos em maior quantidade por essa espécie.
A greenhouse experiment was carried out from November 2005 through April 2006, at UNESP, Jaboticabal, SP, Brazil, to study dry mass production and macronutrient distribution and accumulation by Ipomoea nil, an important weed of annual and perennial crops in Brazil. The plants were cultivated in 7 liter pots, with sand substrate daily irrigated with Hoagland & Arnon nutrient solution. The experimental design was completely randomized with four replications. The treatments corresponded to the evaluation times, at 14 day intervals, beginning 21 days after emergence (DAE). In each evaluation, the plants from four pots were analyzed for dry mass accumulation and mineral nutrition. I. nil showed small dry mass and macronutrient accumulations at the beginning of the experimental stage. These accumulations intensified after 77 DAE, reaching the maximum theoretical value at 123, 122, 124, 121, 119, 121 and 125 DAE, for dry mass, N, P, K, Ca, Mg and S, respectively. Leaves and stems were the main organs responsible for dry mass accumulation by I. nil plants. K and N were the macronutrients extracted at higher amount by this species.

Temperature-dependent photoluminescence spectra of GaAsSb/AlGaAs and GaAsSbN/GaAs single quantum wells under different excitation intensities

The mechanism for low-temperature photoluminescence (PL) emissions in GaAsSb/AlGaAs and GaAsSbN/GaAs strained-layer single quantum wells (SQWs), grown by molecular-beam epitaxy, is studied in detail, using PL spectroscopy as a function of temperature and excitation intensity. In all samples, the PL peak energy as well as the full width at half maximum (FWHM), as a function of temperature, present anomalous behaviors, i.e., the PL peak energy shows a successive red/blue/redshift (S-shaped behavior) and the FWHM shows a successive blue/red/blueshift ("inverted S-shaped curve") with increasing temperature. At sufficiently low excitation intensity and in a narrow temperature interval (50 - 80 K), the nitrogen-containing samples present two clear competitive PL peaks. The low-energy PL mechanism (8 - 80 K) is dominated by localized PL transitions, while the high-energy PL mechanism is dominated by the ground state (e1-hh1) PL transition. Additionally, these PL peaks show different temperature dependence with the low-energy PL peak, showing a stronger redshift than the high-energy PL peak. A competition process between localized and delocalized excitons is used to discuss these PL properties.

Pattern of mycobacterial antigen detection in leprosy

Foi realizada reação imunohistoquímica (Peroxidase anti-peroxidase-PAP) em 52 biópsias de pele de pacientes com hanseníase com o objetivo de verificar quais os padrões antigênicos das micobactérias e estudar a sensibilidade deste método como auxílio diagnóstico da hanseníase. Foram encontrados cinco padrões diferentes, classificados como bacilar, granular, vesicular, citoplasmático e depósitos, de acordo com a característica do material antigênico. Os depósitos (material finamente particulado) apareceram com maior frequencia confirmando a sensibilidade da imunohistoquímica na detecção de pequenas quantidades de antígeno, cujo material não é mais visto por colorações histoquímicas usuais.
Immunohistochemistry reaction (Peroxidase anti-peroxidase - PAP) was carried out on fifty-two skin biopsies from leprosy patients with the purpose to identify the antigenic pattern in mycobacteria and to study the sensitivity of this method. Five different patterns were found: bacillar, granular, vesicular, cytoplasmatic and deposits, classified according to the antigenic material characteristics. Deposits (thinely particulate material) appeared more frequently, confirming the immunohistochemistry sensitivity to detect small amounts of antigens even when this material is not detected by histochemical stainings.
