SUMMARY OBJECTIVE: Stroke-associated pneumonia is an infection that commonly occurs in patients with spontaneous intracerebral hemorrhage and causes serious burdens. In this study, we evaluated the validity of the Braden scale for predicting stroke-associated pneumonia after spontaneous intracerebral hemorrhage. METHODS: Patients with spontaneous intracerebral hemorrhage were retrospectively included and divided into pneumonia and no pneumonia groups. The admission clinical characteristics and Braden scale scores at 24 h after admission were collected and compared between the two groups. Receiver operating characteristic curve analysis was performed to assess the predictive validity of the Braden scale. Multivariable analysis was conducted to identify the independent risk factors associated with pneumonia after intracerebral hemorrhage. RESULTS: A total of 629 intracerebral hemorrhage patients were included, 150 (23.8%) of whom developed stroke-associated pneumonia. Significant differences were found in age and fasting blood glucose levels between the two groups. The mean score on the Braden scale in the pneumonia group was 14.1±2.4, which was significantly lower than that in the no pneumonia group (16.5±2.6), p<0.001. The area under the curve for the Braden scale for the prediction of pneumonia after intracerebral hemorrhage was 0.760 (95%CI 0.717–0.804). When the cutoff point was 15 points, the sensitivity was 74.3%, the specificity was 64.7%, the accuracy was 72.0%, and the Youden's index was 39.0%. Multivariable analysis showed that a lower Braden scale score (OR 0.696; 95%CI 0.631–0.768; p<0.001) was an independent risk factor associated with stroke-associated pneumonia after intracerebral hemorrhage. CONCLUSION: The Braden scale, with a cutoff point of 15 points, is moderately valid for predicting stroke-associated pneumonia after spontaneous intracerebral hemorrhage.

Abstract Objective The aim is to evaluate the diagnostic value of Activin A levels in serum and pleural fluid on Parapneumonic Pleural Effusion (PPE). Methods The authors collected serum and pleural fluid from 86 PPE and 37 Non-PPE (NPPE) patients. Including Activin A, levels of biomarkers such as Lactate Dehydrogenase (LDH), Procalcitonin (PCT), and C-Reactive Protein (CRP) were measured. All factors were calculated for association with days after admission. The diagnostic potential of biomarkers on PPE was considered by Receiver Operating Characteristic (ROC) curve analysis. Results Levels of Activin A in serum and pleural fluid of PPE patients were significantly higher than those of the NPPE patients. Moreover, concentrations of Activin A in pleural fluid showed a more obvious relevant days after admission. ROC curve analysis found that Activin A in pleural fluid had AUCs of 0.899 with 93% sensitivity and 84% specificity for PPE diagnosis. Conclusion Activin A in pleural fluid correlated with disease severity could act to diagnose PPE. . Abstract Background: Diabetic retinopathy (DR) is a specific microvascular complication arising from diabetes, and its pathogenesis is not completely understood. tRNA-derived stress-induced RNAs (tiRNAs), a new type of small noncoding RNA generated by specific cleavage of tRNAs, has become a promising target for several diseases. However, the regulatory function of tiRNAs in DR and its detailed mechanism remain unknown. Results: Here, we analyzed the tiRNA profiles of normal and DR retinal tissues. The expression level of tiRNA-Val was significantly upregulated in DR retinal tissues. Consistently, tiRNA-Val was upregulated in human retinal microvascular endothelial cells (HRMECs) under high glucose conditions. The overexpression of tiRNA-Val enhanced cell proliferation and inhibited cell apoptosis in HRMECs, but the knockdown of tiRNA-Val decreased cell proliferation and promoted cell apoptosis. Mechanistically, tiRNA-Val, derived from mature tRNA-Val with Ang cleavage, decreased Sirt1 expression level by interacting with sirt1 3’UTR, leading to the accumulation of Hif-1α, a key target for DR. In addition, subretinal injection of adeno-associated virus to knock down tiRNA-Val in DR mice ameliorated the symptoms of DR. Conclusion: tiRNA-Val enhance cell proliferation and inhibited cell apoptosis via Sirt1/Hif-1α pathway in HRMECs of DR retinal tissues.

ABSTRACT Purpose: To evaluate the influence of atractylenolide (Atr) III on sepsis-induced lung damage. Methods: We constructed a mouse sepsis model through cecal ligation and puncture. These mice were allocated to the normal, sepsis, sepsis + Atr III-L (2 mg/kg), as well as Atr III-H (8 mg/kg) group. Lung injury and pulmonary fibrosis were accessed via hematoxylin-eosin (HE) and Masson’s staining. We used terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) and flow cytometry for detecting sepsis-induced lung cell apoptosis. The contents of the inflammatory cytokines in lung tissue were measured via enzyme-linked immunosorbent assay (ELISA). Results: Atr III-H did not only reduce sepsis-induced lung injury and apoptosis level, but also curbed the secretion of inflammatory factors. Atr III-H substantially ameliorated lung function and raised Bcl-2 expression. Atr III-H eased the pulmonary fibrosis damage and Bax, caspase-3, Vanin-1 (VNN1), as well as Forkhead Box Protein O1 (FoxO1) expression. Conclusions: Atr III alleviates sepsis-mediated lung injury via inhibition of FoxO1 and VNN1 protein.

Abstract: Introduction and aim. Hyponatremia is common in patients with decompensated cirrhosis and is associated with increased mortality. Tolvaptan, a vasopressor V2 receptor antagonist, can increase free wáter excretion, but its efficacy and safety in cirrhotic patients remain unclear. Material and methods. We studied the usage and safety of tolvaptan in cirrhotic patients in a real-life, non-randomized, multicenter prospective cohort study. Forty-nine cirrhotic patients with hyponatremia were treated with tolvaptan 15 mg daily, and 48 patients not treated with tolvaptan in the same period served as controls. Improvement in serum sodium level was defined as an increase in serum sodium from < 125 to ≥ 125 mmol/L or from 125-134 to ≥ 135 mmol/L on day 7. Results. Twenty-three (47%) patients in the tolvaptan group and 17 (35%) in the control group had normal serum sodium on day 7 (p = 0.25). Serum sodium improved in 30 (61%) patients in the tolvaptan group and 17 (35%) patients in the control group (p = 0.011). Adverse events occurred in 46-47% of patients in both groups, and tolvaptan was not associated with worsened liver function. No patient with normal serum sodium on day 7 died within 30 days of treatment, whereas 16% of those with persistent hyponatremia died (p = 0.0019). Conclusion. In conclusion, short-term tolvaptan treatment is safe and can improve serum sodium level in cirrhotic patients with hyponatremia. Normalization of serum sodium level is associated with better survival.

Background: Prevalence of overweight and obesity in adolescents has been increasing during recent decades. The present study aimed to assess the prevalence of obesity in 14-18 years old Anhui Secondary school children, and provide more comprehensive guidelines for intervention programs. Methods: This cross-sectional study was conducted among 4788 children in secondary school, and date from the routine health survey was analyzed. Trained health workers took anthropometric measures, height and weight, at the school. Definition of children overweight and obesity children was based on the international age-and sex-specific body mass index reference standards proposed by the International Obesity Task Force and National Center for Health Statistics. Results: According to the IOTF standard the prevalence of overweight (including obesity) and obesity revealed decreasing trend with age in male (overweight, from 17.3% at 14-y to 5.3% at 18-y; obesity, from 3.8% at 14-y to 2.3% at 18-y). Depending on the NCHS references used, the overall prevalence of overweight (including obesity) and obesity in male aged 14 to 18 decreases from 16.4% to 11.7% and from 4.6% to 2.8%, respectively. Conclusion: The overweight (including obesity) and obesity from14 to 18 years old have a decreasing trend, and more attention should be taken to male students in China. These findings suggest secondary school and government need to design appropriate guidelines to keep healthy for adolescents in China.

Antecedentes: La Prevalencia de sobrepeso y obesidad en adolescentes ha ido en aumento durante los últimos decenios. El presente estudio pretende evaluar la prevalencia de la obesidad en los niños de la escuela secundaria de 14 a 18 años de edad Anhui, y proporcionar directrices más amplias para los programas de intervención. Métodos: Este estudio transversal se llevó a cabo en 4788 niños de escuela secundaria, y la fecha de la encuesta de salud de rutina fue analizado. Dependiendo de la NCHS referencias utilizadas, la prevalencia global de sobrepeso (obesidad) y la obesidad en hombres de entre 14 y 18 años de edad disminuye a partir de un 16,4% a 11,7% y del 4,6% al 2,8%, respectivamente. Conclusión: El sobrepeso (obesidad) y la obesidad de diciembre de los 14 a los 18 años de edad tienen una tendencia a la disminucion, y más atención debe ser tomado para estudiantes varones en China. Muitos estudos de expressão proteica após lesão cerebral traumática (LCT) identificam biomarcadores para determinação diagnóstica ou prognóstica do LCT. No presente estudo, foram investigados marcadores proteicos adicionais de LCT, através de um aparelho de impacto no fluxo e perfusão em ratos S-D. Eletroforese bidimensional em gel e espectrometria de massa foram utilizadas para identificar diferentes proteínas expressas. Após a análise proteômica, detectamos marcas de proteínas 405 e 371, com pH variando entre 3-10 no córtex de ratos sham e naqueles com contusão cerebral, respectivamente. Oitenta marcas proteicas foram expressas nos dois grupos e 20 destas proteínas foram identificadas. Este estudo validou o estabelecimento de biomarcadores de LCT e identificou potencial biomarcadores que poderão ser estudados em estudos futuros.

Many studies of protein expression after traumatic brain injury (TBI) have identified biomarkers for diagnosing or determining the prognosis of TBI. In this study, we searched for additional protein markers of TBI using a fluid perfusion impact device to model TBI in S-D rats. Two-dimensional gel electrophoresis and mass spectrometry were used to identify differentially expressed proteins. After proteomic analysis, we detected 405 and 371 protein spots within a pH range of 3-10 from sham-treated and contused brain cortex, respectively. Eighty protein spots were differentially expressed in the two groups and 20 of these proteins were identified. This study validated the established biomarkers of TBI and identified potential biomarkers that could be examined in future work.

Background: Overweight and obesity are epidemic worldwide. Our previous study found that the prevalence of overweight and obesity in primary students is high. Objective: we sought to estimate whether the prevalence of overweight and obesity among university students is still high in China. Methods: A cross-sectional study was designed to collect the routine health screening data for university students in 2013. The height and weight of students were measured, and BMI was calculated with height and weight, we estimated the underweight and obesity prevalence of university students using by two references [Working Group on Obesity references in China (2004) and World Health Organization (WHO) criteria (2000)]. Results: Depending on the China references used, the overall prevalence of underweight, overweight (including obesity) and obesity in male university students were 14.2%, 14.7% and 4.2%, respectively; in female university students were 27.5%, 4.4% and 0.6%, respectively. Depending on the WHO references used, the overall prevalence of underweight, overweight (including obesity) and obesity in male university students were 14.2%, 11.5% and 2.5%, respectively, in female university students were 27.5%, 2.4% and 0.3%, respectively. An interesting observation made was that the underweight prevalence of university students has an increasing trend, especially in female. Conclusion: The study showed that the prevalence of underweight in university students, especially in female students has become a critical health issue. Related department of school and government should pay more attention to student's physical health.

Antecedentes: El sobrepeso y la obesidad son epidemia en todo el mundo.Nuestro estudio anterior halló que la prevalencia de sobrepeso y obesidad en estudiantes de primaria es alto. Objetivo: Hemos tratado de estimar si la prevalencia del sobrepeso y la obesidad entre los estudiantes universitarios es todavía alta en China. Métodos: Un estudio transversal fue diseñado para recoger los exámenes de salud de rutina datos para estudiantes universitarios en 2013.La altura y el peso de los estudiantes fueron medidos, y el IMC se calcula con la altura y el peso, así como para estimar la prevalencia de insuficiencia ponderal y obesidad de la Universidad los estudiantes utilizando dos referencias [Grupo de trabajo sobre la obesidad referencias en China (2004) y la Organización Mundial de la salud (OMS) criterios (2000)]. Background: Overweight and obesity are epidemic worldwide. The purpose of this research is to estimate whether the prevalence of obesity among primary school children is high, and to identify its potential determinants to optimize the methods of prevention to combat further increases in childhood overweight. Methods: A cross-sectional study was designed to collect the routine health screening data for primary school children in Wannan area, China. Overweight and obesity status were determined using the International Obesity Task Force standard (IOTF) BMI cut-off points. Results: A total of 67956 subjects (36664 male and 31292 female) aged 5-14 years were recruited in this study. Depending on the references used (IOTF), the overall prevalence of overweight, including obesity of the subjects was 17.85% , the prevalence of overweight, including obesity was 22.9% in male subjects and 11.9% in female subjects, respectively. The overall prevalence of obesity was3.7%, the prevalence of obesity was 5.2 % in male subjects and 1.8% in female subjects, respectively. An interesting observation made was that the prevalence of overweight was high in male subjects. Conclusions: Overweight is prevalence among primary school children, especially in male children. The relate department of school and government should take some measure to reduce the prevalence of overweight and obesity.

El propósito de esta investigacion es estimar si la prevalencia de la obesidad entre los niños de la escuela primaria es alto, y a identificar sus posibles factores determinantes para optimizar los métodos de prevención para combatir nuevas subidas en sobrepeso infantil. Métodos: Un estudio transversal fue diseñado para recoger los exámenes de salud de rutina datos para la escuela primaria los niños de 5 a 14 años en Wannan Area, China, el sobrepeso y la obesidad se determinaron utilizando la International Obesity Task Force Standard (IOTF) IMC puntos de corte. Resultados: Un total de 67956 sujetos (36664 macho y 31239 hembra) de 5 a 14 años fueron reclutados en este estudio. A craniectomia descompressiva (CD) é amplamente utilizada para tratar a hipertensão intracraniana após trauma craniencefálico (TC) ou doença cerebrovascular. Vários estudos discutem as complicações deste procedimento, sendo a hidrocefalia uma das complicações mais frequentes. Fizemos uma revisão da literatura para avaliar a relação entre a CD e a hidrocefalia. Podem ocorrer numerosas complicações após a CD, incluindo aumento de volume por contusão ou hematoma, epilepsia e herniação do cortex cerebral através do acesso ósseo. Fístulas liquóricas através a incisão no couro cabeludo, infecções, hematomas subdurais, hidrocefalia e a “síndrome pós-trepanação”. Foram identificados vários fatores preditivos de hidrocefalia: a distância da CD em relação à linha média, a ocorrência de higroma, a idade, a gravidade da lesão, a hemorragia subaracnóidea ou intraventricular, o tempo decorrido até a craniectomia, as reoperações e o uso de plástica com dura-máter. Entretanto, há divergências entre os autores e o impacto da CD na hidrocefalia continua controvertido.

Decompressive craniectomy (DC) is widely used to treat intracranial hypertension following traumatic brain injury (TBI) or cerebral vascular disease. Many studies have discussed complications of this procedure, and hydrocephalus is a common complication of DC. To further evaluate the relationship between DC and hydrocephalus, a review of the literature was performed. Numerous complications may arise after DC, including contusion or hematoma expansion, epilepsy, herniation of the cortex through a bone defect, CSF leakage through the scalp incision, infection, subdural effusion, hydrocephalus and “syndrome of the trephined”. Several hydrocephalus predictors were identified; these included DC, distance from the midline, hygroma, age, injury severity, subarachnoid or intraventricular hemorrhage, delayed time to craniotomy, repeated operation, and duraplasity. However, results differed among studies. The impact of DC on hydrocephalus remains controversial.

The aim of this work was to study the plant regeneration and Agrobacterium-mediated transformation of Achyranthes bidentata using cotton EREBP gene. Results showed that the callus induction rate of stems from A. bidentata was the highest (100%) and bud was in approximately 70% of calli from stems. However bud differentiation rate of the callus from leaves and petioles was very low. Compared with ceftriaxone, 200mg/L cefotaxime could completely control Agrobacterium tumefaciens and had relatively less toxic action on the stems of A. bidentata. In addition, the induction rate of callus resistant to hygromycin was the highest when infected for 3 min and co-cultivated for 3 d. Six positive transgenic plants transformed with pCAMBIA1304-GhEREB2 expression vector were obtained and confirmed by PCR. The expression of target gene GhEREB2 was detected in five transgenic plants by RT-PCR. In brief, an efficient system of genetic transformation and plant regeneration was established for A. bidentata.

PROBLEM: The Chinese central government launched the Health System Reform Plan in 2009 to strengthen disease control and health promotion and provide a package of basic public health services. Village doctors receive a modest subsidy for providing public health services associated with the package. Their beliefs about this subsidy and providing public health services could influence the quality and effectiveness of preventive health services and disease surveillance. APPROACH: To understand village doctors'perspectives on the subsidy and their experiences of delivering public health services, we performed 10 focus group discussions with village doctors, 12 in-depth interviews with directors of township health centres and 4 in-depth interviews with directors of county-level Centers for Disease Control and Prevention. LOCAL SETTING: The study was conducted in four counties in central China, two in Hubei province and two in Jiangxi province. RELEVANT CHANGES: Village doctors prioritize medical services but they do their best to manage their time to include public health services. The willingness of township health centre directors and village doctors to provide public health services has improved since the introduction of the package and a minimum subsidy, but village doctors do not find the subsidy to be sufficient remuneration for their efforts. LESSONS LEARNT: Improving the delivery of public health services by village doctors is likely to require an increase in the subsidy, improvement in the supervisory relationship between village clinics and township health centres and the creation of a government pension for village doctors.

SITUACIÓN: En el año 2009, el gobierno central chino puso en marcha el plan de reforma del sistema sanitario con el objetivo de reforzar el control de las enfermedades y fomentar la salud, asà como proporcionar un paquete de servicios básicos de salud pública. Los médicos rurales perciben una modesta subvención por la prestación de los servicios de salud pública relacionados con dicho paquete. Sus opiniones acerca de dicha subvención y la prestación de los servicios de salud pública podrÃan influir en la calidad y la eficacia de los servicios sanitarios preventivos y de la vigilancia de las enfermedades. ENFOQUE: Con objeto de comprender los puntos de vista de los médicos rurales acerca de la subvención y sus experiencias en la prestación de servicios sanitarios, hemos llevado a cabo 10 debates con grupos focales de médicos rurales, 12 entrevistas en detalle con directores de centros de salud municipales y 4 entrevistas en detalle con directores de Centros para el Control y Prevención de las Enfermedades provinciales. MARCO REGIONAL: El estudio se llevó a cabo en cuatro condados del centro de China, dos de ellos en la provincia de Hubei y otros dos en la de Jiangxi. CAMBIOS IMPORTANTES: Los médicos rurales dan prioridad a los servicios médicos, pero hacen todo lo posible por gestionar su tiempo a fin de incluir los servicios de salud pública. PROBLÈME: Le gouvernement central chinois a lancé le plan de réforme du système de santé en 2009 pour renforcer la lutte contre les maladies et l'amélioration de la santé, et pour fournir un paquet de services de base en termes de santé publique. Les médecins de village reçoivent une subvention modeste pour offrir des services de santé publique liés à ce paquet. Leurs opinions sur cette subvention et la fourniture de services de santé publique pourraient avoir une influence sur la qualité et l'efficacité des services de santé préventive et sur la surveillance des maladies. APPROCHE: Pour comprendre les perspectives des médecins de village sur la subvention et leur expérience dans le domaine de la fourniture de services de santé publique, nous avons organisé 10 discussions de groupe cible avec des médecins de village, 12 entrevues approfondies avec des directeurs de centres de santé cantonaux et 4 entrevues approfondies avec des directeurs départementaux de centres pour la lutte et la prévention des maladies. ENVIRONNEMENT LOCAL: Létude a été effectuée dans quatre comtés du centre de la Chine, deux dans la province de Hubei et deux dans la province du Jiangxi. CHANGEMENTS SIGNIFICATIFS: Les médecins de village donnent la priorité aux services médicaux, mais ils font de leur mieux pour gérer leur temps de manière à inclure les services de santé publique. Both genetic background and diet have profound effects on plasma lipid profiles. We hypothesized that a high-carbohydrate (high-CHO) diet may affect the ratios of serum lipids and apolipoproteins (apo) differently in subjects with different genotypes of the SstI polymorphism in the apoCIII gene (APOC3). Fifty-six healthy university students (27 males and 29 females, 22.89 ± 1.80 years) were given a washout diet of 54% carbohydrate for 7 days, followed by a high-CHO diet of 70% carbohydrate for 6 days without total energy restriction. Serum triglyceride (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), apoB100, apoAI, and the APOC3 SstI polymorphism were analyzed. The ratios of serum lipids and apoB100/apoAI were calculated. At baseline, the TG/HDL-C ratio was significantly higher in females, but not in males, with the S2 allele. The differences in the TG/HDL-C ratio between genotypes remained the same after the washout and the high-CHO diet in females. When compared with those before the high-CHO diet, the TC/HDL-C (male S2 carriers: 3.13 ± 1.00 vs 2.36 ± 0.65, P = 0.000; male subjects with the S1S1 genotype: 2.97 ± 0.74 vs 2.09 ± 0.55, P = 0.000; female S2 carriers: 2.68 ± 0.36 vs 2.24 ± 0.37, P = 0.004; female subjects with the S1S1 genotype: 2.69 ± 0.41 vs 2.09 ± 0.31, P = 0.000) and LDL-C/HDL-C (male S2 carriers: 1.44 ± 0.71 vs 1.06 ± 0.26, P = 0.012; male subjects with the S1S1 genotype: 1.35 ± 0.61 vs 1.01 ± 0.29, P = 0.005; female S2 carriers: 1.18 ± 0.33 vs 1.00 ± 0.18, P = 0.049; female subjects with the S1S1 genotype: 1.18 ± 0.35 vs 1.04 ± 0.19, P = 0.026) ratios were significantly decreased after the high-CHO diet regardless of gender and of genotype of the APOC3 SstI polymorphism. However, in female S2 carriers, the TG/HDL-C (1.38 ± 0.46 vs 1.63 ± 0.70, P = 0.039) ratio was significantly increased after the high-CHO diet. In conclusion, the high-CHO diet has favorable effects on the TC/HDL-C and LDL-C/HDL-C ratios regardless of gender and of genotype of the APOC3 SstI polymorphism. Somehow, it enhanced the adverse effect of the S2 allele on the TG/HDL-C ratio only in females.
