OBJETIVO: Avaliar as repercussões da ressecção pulmonar sobre a função pulmonar e a qualidade de vida (QV) de pacientes com câncer de pulmão primário ou metastático. MÉTODOS: Estudo de coorte prospectivo que incluiu todos os pacientes que realizaram ressecção pulmonar por neoplasia no Hospital A. C. Camargo entre setembro de 2006 e março de 2007. Os pacientes foram avaliados no pré-operatório e após seis meses do procedimento cirúrgico através de espirometria. Após seis meses de pós-operatório, os pacientes responderam a um questionário de QV geral (Medical Outcomes Study 36-item Short-form Health Survey) e um específico para sintomas respiratórios (Saint George's Respiratory Questionnaire). Os valores de QV obtidos foram comparados a valores de uma população geral e aos de uma população de portadores de DPOC. RESULTADOS: Foram incluídos 33 pacientes (14 homens e 19 mulheres), com idade entre 39 e 79 anos. Todos os pacientes, tabagistas ou não, apresentaram piora significativa da função pulmonar. Observamos uma redução de aproximadamente 5% na média dos escores do questionário de QV geral em comparação àquela da população geral. Houve uma redução de 50-60% nos vários domínios do questionário específico para sintomas, quando comparado aos resultados da população geral, e um aumento de aproximadamente 20%, quando comparado aos resultados da população com DPOC. CONCLUSÕES: Existe impacto direto da ressecção pulmonar na deterioração da função pulmonar e na QV com ênfase nos aspectos diretamente ligados à função pulmonar. Cabe ressaltar a importância da avaliação da função pulmonar destes pacientes no pré-operatório para se estimar sua evolução pós-cirúrgica.

OBJECTIVE: To evaluate the effects that pulmonary resection has on pulmonary function and quality of life (QoL) in patients with primary or metastatic lung cancer. METHODS: This was a prospective cohort study involving all patients submitted to pulmonary resection for cancer between September of 2006 and March of 2007 at the A. C. Camargo Hospital in São Paulo, Brazil. Patients underwent spirometry in the preoperative period and at six months after the surgical procedure. After a postoperative period of six months, the patients completed an overall QoL questionnaire (the Medical Outcomes Study 36-item Short-form Health Survey) and another one, specific for respiratory symptoms (the Saint George's Respiratory Questionnaire). The scores obtained in our study were compared with those previously obtained for a general population and for a population of patients with COPD. RESULTS: We included 33 patients (14 males and 19 females), ranging in age from 39 to 79 years. All of the patients, smokers and nonsmokers alike, presented significant worsening of pulmonary function. The mean scores on the overall QoL questionnaire were approximately 5% lower than those obtained for the general population. The scores of various domains of the symptom-specific QoL questionnaire were 50-60% lower than those obtained for the general population and approximately 20% higher than those obtained for the population with COPD. CONCLUSIONS: Pulmonary resection has a direct negative impact on pulmonary function and QoL, especially on the QoL related to aspects directly linked to pulmonary function. We highlight the importance of preoperative assessment of pulmonary function in patients undergoing pulmonary resection, in order to predict their postoperative evolution.

INTRODUCTION: It is known that mechanical ventilation and many of its features may affect the evolution of inspiratory muscle strength during ventilation. However, this evolution has not been described, nor have its predictors been studied. In addition, a probable parallel between inspiratory and limb muscle strength evolution has not been investigated. OBJECTIVE: To describe the variation over time of maximal inspiratory pressure during mechanical ventilation and its predictors. We also studied the possible relationship between the evolution of maximal inspiratory pressure and limb muscle strength. METHODS: A prospective observational study was performed in consecutive patients submitted to mechanical ventilation for > 72 hours. The maximal inspiratory pressure trend was evaluated by the linear regression of the daily maximal inspiratory pressure and a logistic regression analysis was used to look for independent maximal inspiratory pressure trend predictors. Limb muscle strength was evaluated using the Medical Research Council score. RESULTS: One hundred and sixteen patients were studied, forty-four of whom (37.9%) presented a decrease in maximal inspiratory pressure over time. The members of the group in which maximal inspiratory pressure decreased underwent deeper sedation, spent less time in pressure support ventilation and were extubated less frequently. The only independent predictor of the maximal inspiratory pressure trend was the level of sedation (OR=1.55, 95% CI 1.003 - 2.408; p = 0.049). There was no relationship between the maximal inspiratory pressure trend and limb muscle strength. CONCLUSIONS: Around forty percent of the mechanically ventilated patients had a decreased maximal inspiratory pressure during mechanical ventilation, which was independently associated with deeper levels of sedation. There was no relationship between the evolution of maximal inspiratory pressure and the muscular strength of the limb.

Os tumores de células germinativas primários do mediastino podem corresponder de 10% a 20% das neoplasias mediastinais. Ocorrem devido à transformação maligna de elementos germinativos sem evidência de um foco gonadal primário. Podem ser classificados de acordo com suas características morfológicas em: teratomas, seminomas, não seminomas (tumores do saco vitelino, carcinomas embrionários e coriocarcicoma), e tumores de células germinativas combinados sem componentes teratomatosos. O objetivo dessa revisão é analisar os diferentes tipos de tumores de células germinativas primários do mediastino no que diz respeito aos seus aspectos clínicos bem como as suas diferentes formas de abordagens terapêuticas.

Primary mediastinal germ cell tumors account for approximately 10% to 20% of all neoplasms of the mediastinum. It is generally accepted that they represent the malignant transformation of germinal elements without a gonadal focus. These tumors can be classified as teratomas, seminomas, nonseminomatous (yolk sac tumor, embryonal carcinoma, choriocarcinoma) and combined germ-cell tumors without teratomatous components. This review purposes to discuss the different primary mediastinal germ-cell tumors, their clinical presentation and options for treatment.

CONTEXTO E OBJETIVO: As abordagens terapêuticas atuais para o câncer de mama metastático não asseguram aumento na sobrevida. Sendo assim, qualidade de vida passa a ser um dos principais objetivos do tratamento. Apesar disso, desconhecemos a influência dos tratamentos atualmente utilizados na qualidade de vida dessa população. O objetivo do estudo foi avaliar a influência do tratamento oncológico na qualidade de vida de portadoras de câncer de mama metastático. TIPO DE ESTUDO E LOCAL: Estudo prospectivo da variação da qualidade de vida de portadoras de câncer de mama metastático tratadas em um centro médico terciário. MÉTODOS: Realizamos um levantamento prospectivo da influência dos tratamentos oncológicos, incluindo quimioterapia, tratamento hormonal, radioterapia, cirurgia e suporte clínico, na qualidade de vida de pacientes tratadas no Centro de Tratamento e Pesquisa Hospital do Câncer, em São Paulo. Utilizamos para isso o Medical Outcomes Study's Short Form-36 (SF-36) e o Inventário de Depressão de Beck (IDB), aplicados em três ocasiões: antes do início, na 6ª e na 12ª semanas após o início do tratamento. RESULTADOS: Encontramos melhora de qualidade de vida na amostra avaliada (n = 40), expressa pela variação dos índices do SF-36 (p = 0,002) e do IDB (p = 0,004). Os componentes do SF-36 que apresentaram ganhos mais significativos foram dor, aspecto social e saúde mental. Como esperado, as pacientes com piores Performance Status (PS) e com sintomas secundários presentes apresentaram maiores ganhos em sua qualidade de vida que as de bom PS e as assintomáticas (p < 0,001). Da mesma forma, aquelas que receberam mais de uma modalidade terapêutica em comparação com as que só receberam um tipo de tratamento (p = 0,038). CONCLUSÕES: Em nosso meio, a abordagem atual para o câncer de mama metastático mostrou-se capaz de melhorar a qualidade de vida das pacientes, especialmente das que se apresentam com sintomas secundários à doença de base ou com baixo PS.

CONTEXT AND OBJECTIVE: In metastatic breast cancer cases, the currently available therapeutic approaches provide minimal improvement in survival. As such, quality of life (QOL) becomes one of the main objectives of treatment. It is not known whether current treatments derived from trials improve QOL. The aim was to evaluate changes in QOL among metastatic breast cancer patients receiving treatment derived from trials. DESIGN AND SETTING: Prospective observational QOL survey in a tertiary cancer center. METHODS: To evaluate the influence of current treatments on patients' QOL, the Medical Outcomes Study Short Form-36 (SF-36) and the Beck Depression Inventory (BDI) were applied on three occasions: before starting treatment and at the 6th and 12th weeks, to consecutive metastatic breast cancer patients over a one-year period. RESULTS: We found an improvement in QOL in the sample evaluated (n = 40), expressed by changes in the overall SF-36 score (p = 0.002) and the BDI (p = 0.004). Taken individually, the SF-36 components Pain, Social Functioning and Mental Health also improved significantly. Patients with worse initial performance status and secondary symptoms displayed greater improvement than those with better initial performance status and asymptomatic disease (p < 0.001). Patients who received more than one type of therapy showed larger gains than those given only one type (p = 0.038). CONCLUSIONS: In our environment, current metastatic breast cancer treatments can improve QOL, especially among symptomatic patients and those with low performance status.

OBJETIVO: A ventilação mecânica invasiva é associada a complicações, portanto sua abreviação é desejada. O desbalanço entre o aumento da carga sobre os músculos inspiratórios, a diminuição da força e a resistência muscular é determinante na dependência da ventilação. A baixa resistência muscular pode ser causada por atrofia muscular, pela doença grave ou pelo uso de corticoesteróides. O treinamento da musculatura inspiratória pode aumentar ou preservar a resistência. O objetivo principal do estudo foi testar a hipótese que o treinamento da musculatura inspiratória desde o início da ventilação iria abreviar o desmame da ventilação e diminuir a taxa de reintubação. Como objetivo secundário descrevemos a evolução da pressão inspiratória máxima com e sem treinamento da musculatura inspiratória. MÉTODOS: Estudo prospectivo e aleatorizado em unidade de tratamento intensivo Clínico-Cirúrgica. Doze pacientes treinaram a musculatura inspiratória duas vezes ao dia e treze não treinaram (controle). O treinamento foi realizado ajustando a sensibilidade do ventilador, baseando-se na pressão inspiratória máxima. Os pacientes tiveram sua pressão inspiratória máxima verificada diariamente. RESULTADOS: A duração do desmame (31 ± 22 controle e 23 ± 11 horas grupo treinamento; p=0.24) não foi estatisticamente diferente. A pressão inspiratória máxima do grupo controle teve leve tendência ao aumento, enquanto o grupo treinamento teve leve tendência à diminuição. CONCLUSÃO: Em pacientes graves, o treinamento da musculatura inspiratória desde o início da ventilação mecânica não abreviou o desmame, nem diminuiu a reintubação. A pressão inspiratória máxima tendeu a manter-se constante ao longo da ventilação mecânica, com ou sem o treinamento inspiratório aplicado.

PURPOSE: Invasive mechanical ventilation is associated with complications, and its abbreviation is desirable. The imbalance between increased workload, decreased inspiratory muscle strength and endurance is an important determinant of ventilator dependence. Low endurance may be present due to respiratory muscle atrophy, critical illness, or steroid use. Specific inspiratory muscle training may increase or preserve endurance. The objective of the study was to test the hypothesis that inspiratory muscle training from the beginning of mechanical ventilation would abbreviate the weaning duration and decrease reintubation rate. As a secondary objective, we described the evolution of inspiratory muscle strength with and without inspiratory muscle training. METHODS: Prospective, randomized clinical trial in an adult clinical-surgical intensive care unit. Twelve patients trained the inspiratory muscles twice a day, and 13 patients did not (control). Training was performed adjusting the sensitivity of the ventilator based on the maximal inspiratory pressure. Patients underwent daily surveillance of the maximal inspiratory pressure. RESULTS: The weaning duration (31 ± 22 hr, control and 23 ± 11 hr, training group; P = .24) and reintubation rate (5 control and 3 training group; P = .39) were not statistically different. The maximal inspiratory pressure of the control group showed a trend toward a modest increase. In contrast, the training group showed a small decrease (P = .34). CONCLUSIONS: In acute critically ill patients, inspiratory muscle training from the beginning of mechanical ventilation neither abbreviated the weaning duration, nor decreased the reintubation rate. Inspiratory muscle strength tended to stay constant, along the mechanical ventilation, with or without this specific inspiratory muscle training.

OBJETIVO: Menos de 20% dos fumantes crônicos desenvolverá cancer. As características do hábito de fumar, particularmente o nível de dependência à nicotina, não foram avaliadas inteiramente como um marcador do risco. MÉTODOS: Estudo caso-controle de pacientes voluntários, registrados prospectivamente em um programa de cessação de tabagismo em um hospital de cancer. Para cada caso de cancer, pacientes da mesma idade e sexo foram selecionados. O inventário de depressão de Beck, um instrumento validado para diagnóstico de estado depressivo e depressão clínica e o questionário de tolerância de Fagerstron , que é usado para determinar o grau de dependência e tem boa correlação com níveis de nicotina, foram aplicados. Idade na admissão ao estudo, sexo, número de maços-anos fumados foram avaliados também. RESULTADOS: De maio de 1999 a maio de 2002, 56 pacientes de câncer (caso) e 85 controles pareados (controle) foram identificados na população estudada . Não houve diferença quanto ao número de maços-ano. O questionário de tolerância de Fagerstron foi significativamente mais elevado nos pacientes com câncer (7.5 ± 1.9) comparado aos controles (6.3 ± 2.0). Encontramos um questionário de tolerância de Fagerstron > 7 em 73.2% dos casos de câncer, contra 43.5% dos controles (p=0.001). A proporção de pacientes deprimidos foi mais elevada no grupo do cancer (37.5% x 17.6%). A regressão logística, ajustada para a idade e o consumo do tabaco, apontou que uma contagem de questionário de tolerância de Fagerstron > 7 tem uma razão de chance para câncer de 3.45 (CI 95% 1.52 - 7.83, p = 0.003). CONCLUSÃO: Resultado no questionário de tolerância de Fagerstron maior que 7 foi identificado como um fator de risco para cancer em fumantes com consumo similar do tabaco.

BACKGROUND: Less than 20% of lifetime smokers will ever develop cancer. Smoking habits characteristics, particularly the level of nicotine dependence level, were not fully evaluated as a marker of risk. METHODS: Case-control study of voluntary patients prospectively enrolled in a smoking cessation program in a cancer hospital. For each cancer case, patients of the same age and sex were selected. The Beck Depression Inventory, an instrument for the diagnosis of depressive mood and clinical depression, and the Fagerström Test Questionnaire, a questionnaire that has a good correlation with nicotine levels, used to determine the degree of dependence on nicotine, were applied. Age on admission to the study, sex, and number of pack-years were also evaluated. RESULTS: From May 1999 to May 2002, 56 cancer patients (case) and 85 matching controls (control) were identified in the population studied. There was no difference regarding pack-years. Fagerström Test Questionnaire was significantly higher in patients with cancer (7.5 ± 1.9) compared to controls (6.3 ± 2.0). We found a Fagerström Test Questionnaire > 7 in 73.2% of the cancer cases versus 43.5% of the controls (p=0.001). The proportion of depressed patients was higher in the cancer group (37.5% x 17.6%). Logistic regression adjusted for age and tobacco consumption disclosed that Fagerström Test Questionnaire score > 7 has an odds ratio for cancer of 3.45 (95% CI 1.52 - 7.83, p = 0.003). CONCLUSION: Fagerström Test Questionnaire higher than 7 was identified as a risk factor for cancer in smokers with similar tobacco consumption.

INTRODUÇÃO: A adenosina é um nucleosídeo com potente ação vasodilatadora. Apesar de seus efeitos sobre o território arterial pulmonar ser conhecido, seu efeito sobre o território capilar/venoso ainda não foi descrito. OBJETIVO: Estudar o comportamento das resistências arterial e venosa da circulação pulmonar de pacientes com hipertensão pulmonar primária antes e depois da administração de adenosina. MÉTODO: Foram estudados sete pacientes com hipertensão pulmonar primária que apresentaram resposta positiva durante o teste agudo com adenosina. Resistência arterial e resistência venosa foram determinadas pela estimativa da pressão capilar pulmonar realizada através da análise da curva de decaimento da pressão arterial pulmonar, antes e depois da administração da adenosina. RESULTADOS: Após a administração da adenosina, houve um aumento do índice cardíaco (1,71 ± 0,23 para 2,72 ± 0,74 L.min-1.m-2) com concomitante diminuição da resistência vascular pulmonar (2.924 ± 1.060 para 1.975 ± 764 dina.s.cm-5.m-2), sem variações significativas da pressão arterial pulmonar média (75,6 ± 16,8 para 78,1 ± 18,8 mmHg), da pressão de oclusão da artéria pulmonar (15,3 ± 1,5 para 15,4 ± 1,9 mmHg) e da pressão capilar pulmonar (43,8 ± 5,8 para 44,5 ± 4,9 mmHg). A proporção entre a resistência arterial e a resistência vascular pulmonar total também apresentou variação não significativa (50 ± 15 para 49 ± 17%). Estes achados sugerem que a adenosina teve ação não somente sobre o território arterial mas também sobre o território capilar/venoso. CONCLUSÃO: Os autores concluem que o mecanismo de ação da adenosina não é restrito ao território arterial da circulação pulmonar e que este tipo de análise, através da determinação da pressão capilar pulmonar, pode ser útil no estudo das diversas drogas que agem sobre a circulação pulmonar.

BACKGROUND: The nucleoside adenosine is a potent vasodilator. Although its effect on the pulmonary arteries is well known, its influence on capillaries and veins has yet to be described. OBJECTIVE: To evaluate the pre- and post-administration effects of adenosine on arterial and venous resistance in the pulmonary circulation of patients with primary pulmonary hypertension. METHOD: The study involved 7 patients with primary pulmonary hypertension and presenting a positive response to adenosine on the acute test. Before and after adenosine administration, arterial and venous resistances were determined by estimating pulmonary capillary pressure through analysis of pulmonary artery pressure decay curves. RESULTS: Following adenosine administration, there was an increase in the cardiac index (from 1.71 ± 0.23 to 2.72 ± 0.74 L/min-1/m-2) and a decrease in pulmonary vascular resistance (from 2924 ± 1060 to 1975 ± 764 dynes/s/cm-5/m-2) with no significant variations in mean pulmonary artery pressure (pre: 75.6 ± 16.8 mmHg; post: 78.1 ± 18.8 mmHg), pulmonary wedge pressure (pre: 15.3 ± 1.5 mmHg; post: 15.4 ± 1.9 mmHg) and pulmonary capillary pressure (pre: 43.8 ± 5.8 mmHg; post: 44.5 ± 4.9 mmHg). The ratio between arterial resistance and total pulmonary vascular resistance also presented a less than significant variation (pre: 50 ± 15%; post: 49 ± 17%). These findings suggest that adenosine affects the capillaries and veins as well as the arteries. CONCLUSION: We can conclude that the adenosine mechanism is not restricted to the arterial aspect of the pulmonary circulation, and that analysis of pulmonary capillary pressure could prove useful in the study of various drugs that affect the pulmonary circulation.

OBJETIVO: Analisar o resultado e fatores prognósticos de patients com CPNPC admitidos em uma única instituição e correlacionar os dados com o sistema atual de estadiamento. MÉTODO: Setecentos e trinta e sete pacientes com diagnóstico de CPNPC foram admitidos ao Hospital do Cancer A. C. Camargo entre 1990 e 2000. Todos os pacientes foram incluídos em um banco de dados contínuo prospectivo e seus dados foram analisados. Após o estadiamento, uma equipe multidisciplinar estabeleceu decisões sobre o manejo adequado para o caso. Variáveis analisadas incluíram idade, sexo, tipo histológico, índice de Karnofsky, perda de peso, estadio clínico, estadio cirúrgico, quimioterapia, radioterapia e taxa de sobrevida. RESULTADOS: 75,5% dos pacientes eram do sexo masculino. A distribuição dos tipos histológicos foi carcinoma espino celular 51,8%, adenocarcinoma 43,% e carcinoma indiferenciado de grandes células 5,1%. A maior parte apresentou perda de peso significativa e um Karnofsky Index de 80%. O estadiamento clínico foi IA 3,8%, IB 9,2%, IIA 1,4%, IIB 8,1%, IIIA 20,9%, IIIB 22,4%, IV 30,9%. A ressecção total do tumor foi possível em 24,6% dos casos. A distribuição do estadiamento cirúrgico foi IA 25,3%, IB 1,4%, IIA 1,4%, IIB 17,1%, IIIA 16,1%, IIIB 20,3%, IV 11,5%. Quimioterapia e radioterapia também foram consideradas opções terapêuticas. A sobrevida global de 5 anos em nosso estudo foi de 28%, sendo a sobrevida mediana de18,9 meses. CONCLUSÕES: CPNPC é uma doença que requer atenção especial, devido aos altos índices de morbi-mortalidade. Melhor prognóstico está associado à ressecção completa do tumor, com dissecção de linfonodos. Todavia, isso só é possível em estadios clínicos mais precoces.

OBJECTIVE: To analyze surgical and pathological parameters and outcome and prognostic factors of patients with nonsmall cell lung cancer (NSCLC) who were admitted to a single institution, as well as to correlate these findings to the current staging system. METHOD: Seven hundred and thirty seven patients were diagnosed with NSCLC and admitted to Hospital do Cancer A. C. Camargo from 1990 to 2000. All patients were included in a continuous prospective database, and their data was analyzed. Following staging, a multidisciplinary team decision on adequate management was established. Variables included in this analysis were age, gender, histology, Karnofsky index, weight loss, clinical stage, surgical stage, chemotherapy, radiotherapy, and survival rates. RESULTS: 75.5% of patients were males. The distribution of histologic type was squamous cell carcinoma 51.8%, adenocarcinoma 43.1%, and undifferentiated large cell carcinoma 5.1%. Most patients (73%) presented significant weight loss and a Karnofsky index of 80%. Clinical staging was IA 3.8%, IB 9.2%, IIA 1.4%, IIB 8.1%, IIIA 20.9%, IIIB 22.4%, IV 30.9%. Complete tumor resection was performed in 24.6% of all patients. Surgical stage distribution was IA 25.3%, IB 1.4%, IIB 17.1%, IIIA 16.1%, IIIB 20.3%, IV 11.5%. Chemotherapy and radiotherapy were considered therapeutic options in 43% and 72%, respectively. The overall 5-year survival rate of nonsmall cell lung cancer patients in our study was 28%. Median survival was 18.9 months. CONCLUSIONS: Patients with NSCLC who were admitted to our institution presented with histopathologic and clinical characteristics that were similar to previously published series in cancer hospitals. The best prognosis was associated with complete tumor resection with lymph node dissection, which is only achievable in earlier clinical stages.
