RESUMEN Introducción: la experiencia del uso de la plasmaféresis terapéutica en enfermedades autoinmunes e inflamatorias permitió proponerla e indicarla en combinación con fármacos biotecnológicos en el paciente con COVID-19; enfermedad caracterizada por la liberación de citocinas no controlada que activa la respuesta proinflamatoria, síndrome de insuficiencia respiratoria aguda, eventos trombóticos y fallo multiorgánico. Objetivo: describir los resultados del empleo de la plasmaféresis terapéutica combinado con los fármacos biotecnológicos cubanos en pacientes con empeoramiento de la enfermedad por COVID-19. Metodología: se realizó la investigación durante la segunda oleada de pandemia de COVID-19 en el Hospital Militar “Manuel Fajardo Rivero” durante los meses marzo y abril del año 2021. El estudio de caso involucró a 16 pacientes mayores de 18 años de edad con reacción en cadena de la polimerasa con transcriptasa inversa positivo al coronavirus 2 del síndrome respiratorio agudo severo, con deterioro de su estado clínico y de laboratorio que fueron sometidos a tratamiento de plasmaféresis y fármacos biotecnológicos cubanos: Itolizumab, Jusvinza, HeberFERON® e Intacglobin®. Resultados: el 75% de los pacientes fueron clasificados potencialmente graves. La mitad de los enfermos recibió Itolizumab y al menos una sesión de plasmaféresis terapéutica. La respuesta fue favorable, el 100% negativizó la reacción en cadena de la polimerasa con transcriptasa inversa y el dímero D se normalizó con mayor rapidez, seguido de la proteína C reactiva. Se necesitaron menos sesiones de plasmaféresis para lograr la recuperación. Conclusiones: la plasmaféresis terapéutica en pacientes con COVID-19 fue un tratamiento complementario y prometedor; combinado con los biofármacos cubanos fue innovador y necesario para reducir la carga inflamatoria-viral y lograr la recuperación del paciente con agravamiento clínico de su estado.

ABSTRACT Introduction: The experience of using therapeutic plasmapheresis in autoimmune and inflammatory diseases allowed us to propose and indicate this therapy in combination with Cuban biotechnological drugs in patients with COVID-19; a disease characterized by the uncontrolled release of cytokines that activate the proinflammatory response, acute respiratory failure syndrome, thrombotic events and multiorgan failure. Objective: To describe the results of the use of therapeutic plasmapheresis combined with Cuban biotechnological drugs in patients with worsening of COVID-19 disease. Methodology: The research was carried out during the second wave of the COVID-19 pandemic at the "Manuel Fajardo Rivero" Army Military Hospital in Villa Clara, during the months of March and April 2021. Case study involving 16 patients over 18 years of age with positive PCR-RT for SARS-CoV-2, with deterioration in their clinical and laboratory condition who underwent plasmapheresis treatment and Cuban biotechnological drugs: Itolizumab, Jusvinza, HeberFERON® e Intacglobin®. Results: 75% of the patients were classified as potentially serious. Half of the patients received Itolizumab and at least one therapeutic plasmapheresis session. The response was favorable, 100% had negative PCR-RT, and D-dimer normalized more quickly, followed by C - reactive protein. Fewer plasmapheresis sessions were needed to achieve recovery. Conclusions: Therapeutic plasmapheresis in patients with COVID-19 was a complementary and promising treatment; combined with biopharmaceuticals, innovative and necessary to reduce the inflammatory and viral load and achieve the recovery of the patient with clinical worsening of his condition.

RESUMEN Introducción: La respuesta a las terapias en el cáncer de pulmón avanzado pudiera estar relacionada con determinados factores pronósticos como los índices inflamatorios. Objetivo: Evaluar la eficacia del anticuerpo monoclonal humanizado nimotuzumab en pacientes portadores de cáncer de pulmón no microcítico avanzado según índices inflamatorios. Método: Se realizó un estudio de evaluación longitudinal retrospectivo, en un universo de 498 pacientes mayores de 18 años, con diagnóstico citohistológico de cáncer de pulmón de células no pequeñas, en estadios avanzados, después de la primera línea de terapia oncológica, incluidos en ensayos clínicos multicéntricos promovidos por el Centro de Inmunología Molecular desde 2002 a 2018. Se aplicó la estadística descriptiva, se utilizó el software x-tile 3.6.1 para el test de Kaplan Meier; se consideraron diferencias significativas cuando p< 0,05. Resultados: En los pacientes analizados el nimotuzumab mostró beneficio terapéutico en el grupo de pacientes no progresores a la primera línea de tratamiento con quimioterapia o quimiorradioterapia, cuando tenían menor índice de neutrófilos/linfocitos (p= 0,017 y p= 0,027) y menor índice de plaquetas/linfocitos (p= 0,030 y p= 0,009). Conclusión: La selección de un paciente con menor índice inflamatorio beneficia la eficacia del tratamiento con el AcM humanizado nimotuzumab en el cáncer de pulmón avanzado no microcítico, la que se convierte en una herramienta predictiva de la respuesta al tratamiento.

ABSTRACT Introduction: The response to therapies in advanced lung cancer could be related to certain prognostic factors such as inflammatory indices. Objective: To evaluate the efficacy of the humanized monoclonal antibody nimotuzumab in patients with advanced non-small cell lung cancer according to inflammatory indices. Method: A retrospective longitudinal evaluation study was carried out in a universe of 498 patients older than 18 years, with a cytohistological diagnosis of non-small cell lung cancer, in advanced stages, after the first line of oncological therapy, including in multicenter clinical trials promoted by the Center for Molecular Immunology from 2002 to 2018. Descriptive statistics were applied, the x-tile 3.6.1 software was used for the Kaplan Meier test, significant differences were considered when p< 0,05. Results: In the patients analyzed, nimotuzumab showed therapeutic benefit in the group of patients who did not progress to the first line of treatment with chemotherapy or chemoradiotherapy, when they had a lower neutrophil-lymphocyte index (p= 0,017 and p= 0,027) and a lower platelet-lymphocyte index (p= 0,030 and p= 0,009). Conclusion: Selecting a patient with a lower inflammatory index benefits the efficacy of treatment with the humanized mAb nimotuzumab in advanced non-small cell lung cancer, which becomes a predictive tool for response to treatment.

ABSTRACT Introduction: Lung injury and STAT1 deficit can induce EGFR overexpression in SARS-CoV2 infected cells. Methods: Considering the role of EGFR in inflammation and fibrosis, a Phase I/II trial was done to evaluate the safety and preliminary effect of nimotuzumab, a humanized anti-EGFR antibody, in severe and high-risk moderate patients with COVID-19. Patients were able to receive other drugs included in the national protocol. Treatment consisted on nimotuzumab intravenous infusions, administered every 72 hours. Before launching the study, EGFR expression was confirmed by immunohistochemistry in 20 lung samples from COVID-19 deceased subjects. Results: They were included forty-one patients (31 severe/10 moderate) in the trial. The median age was 62 years and the main comorbidities were hypertension, diabetes, cardiovascular disease and obesity. Seven patients received one dose of nimotuzumab, 29 received 2 infusions while 5 subjects required 3 doses. The antibody was very safe. There were only 4 related adverse events in 2 subjects. Eight patients required invasive mechanical ventilation. The 14 day recovery rate was 82,9%. 90% in moderate patients and 80,64 % in severe patients. Inflammatory markers decreased overtime and interleukin-6 concentration diminished from 46,5 pg/ml to 14,51 pg/ml at day 7. None of the evaluated patients showed signs of fibrosis in the follow-up evaluation. After the use of nimotuzumab in 1,536 patients in the real-world scenario, a high recovery rate was achieved in severe and moderate patients (89,4% and 79.4%, respectively), which compares very favorably with the phase I/II trial data. Conclusions: our results suggest that nimotuzumab is a safe antibody that can reduce inflammation and avoid fibrosis in severe and moderate COVID-19 patients at high risk of aggravation.

RESUMEN Introducción: La lesión pulmonar y el déficit del transductor de señal y el activador de la transcripción 1 (STAT1) inducen la sobreexpresión de receptor del factor de crecimiento epidérmico (EGFR) en células infectadas con SARS-CoV-2. Métodos: Teniendo en cuenta el papel del EGFR en la inflamación y la fibrosis, se realizó un ensayo de fase I/II para evaluar la seguridad y el efecto del nimotuzumab, un anticuerpo anti-EGFR humanizado, en pacientes con COVID-19 graves y moderados. Los pacientes recibieron otros medicamentos incluidos en el protocolo nacional. El tratamiento consistió en infusiones intravenosas de nimotuzumab, administradas cada 72 h. Antes de iniciar el estudio, la expresión de EGFR se confirmó mediante inmunohistoquímica en 20 muestras de pulmón de sujetos fallecidos por COVID-19. Resultados: En el ensayo se incluyeron 41 pacientes (31 graves/10 moderados). La media de edad fue de 62 años y las principales comorbilidades fueron hipertensión, diabetes, enfermedad cardiovascular y obesidad. Recibieron una dosis de nimotuzumab 7 pacientes, 29 recibieron 2 infusiones mientras que 5 sujetos requirieron 3 dosis. El anticuerpo fue seguro. Solo se detectaron 4 eventos adversos en 2 sujetos. Requirieron ventilación mecánica invasiva 8 pacientes. La tasa de recuperación a los 14 días fue del 82,9 %: pacientes moderados 90 % y 80,64 % en pacientes graves. Los marcadores inflamatorios disminuyeron en el tiempo y la concentración de interleucina-6 disminuyó de 46,5 pg/ml a 14,51 pg/ml el día 7. Ninguno de los pacientes evaluados mostró signos de fibrosis en el seguimiento. Tras el uso del nimotuzumab en 1536 pacientes en las condiciones del mundo real, se logró una alta tasa de recuperación en los pacientes graves y moderados (89,4 y 79,4 %, respectivamente), lo cual se compara muy favorablemente con los datos del ensayo fase I/II. Conclusiones: estos resultados sugieren que el nimotuzumab es un anticuerpo seguro que puede reducir la inflamación y evitar la fibrosis en pacientes con COVID-19 graves y moderados.

ABSTRACT Introduction: COVID-19 can lead to a hyper-inflammatory state. CD6 is a glycoprotein expressed on mature T-lymphocytes, which is a crucial regulator of the T-cell activation. Itolizumab is an antibody targeting CD6. Clinical data in autoimmune diseases indicate that it lowers multiple cytokines primarily involving the Th1/Th17 pathway. Objective: to preliminarily assess the safety and impact of itolizumab on interleukin-6, pulmonary function and mortality of COVID-19 patients. Methods: Sixty-eight moderate, severe and critical patients were included in an expanded-access trial in Cuba. Itolizumab was administered together with other therapies included in the national protocol for SARS-CoV-2. Results: All patients completed the first infusion and 41 received 2 doses. Median age was 68 and 94 % have comorbidities. Itolizumab improved the lung function and was well tolerated; 3 subjects had related serious adverse events. Interleukin-6 decreased in individuals with high levels and did not change in those with lower concentration. 14-day lethality rate was 4 % and 18 % for moderate and severe patients. Although this was not a randomized study, preliminary data suggest that itolizumab reduced the death probability in comparison to controls. Time to treatment, neurological manifestations, biomarkers like NLR, neutrophil count and interleukin-6 were significantly associated with higher lethality. Conclusions. Itolizumab can disrupt the inflammatory cascade and might prevent the COVID-19 associated morbidity and mortality.

RESUMEN Introducción: La neumonía COVID-19 puede conducir a un estado hiperinflamatorio. El CD6 es una glicoproteína expresada en linfocitos T maduros que constituye un regulador crucial de la activación de las células T. El itolizumab es un anticuerpo monoclonal que reconoce el CD6. Los datos clínicos en enfermedades autoinmunes indican que reduce múltiples citocinas proinflamatorias. Objetivo: evaluar la seguridad y el impacto de itolizumab sobre la interleucina-6, la función pulmonar y la mortalidad de los pacientes con COVID-19. Métodos: Se incluyeron 68 pacientes moderados, severos y críticos en un ensayo de acceso expandido en Cuba. El itolizumab se administró junto con otras terapias incluidas en el protocolo nacional para el SARS-CoV-2. Resultados: Todos los pacientes completaron la primera infusión y 41, recibieron 2 dosis. La mediana de edad fue de 68 años y el 94 % era portador de comorbilidades. El itolizumab mejoró la función pulmonar y fue bien tolerado; 3 sujetos tuvieron eventos adversos graves relacionados. La interleucina-6 disminuyó en individuos con niveles altos y no se incrementó en aquellos pacientes con concentraciones más bajas. La tasa de letalidad a los 14 días fue del 4 % y del 18 % para los pacientes moderados y graves. Aunque este estudio no fue aleatorizado, los datos preliminares sugieren que itolizumab redujo la probabilidad de muerte en comparación con los controles. El tiempo de tratamiento, las manifestaciones neurológicas y los biomarcadores como la razón entre neutrófilos y linfocitos, recuento de neutrófilos y la interleucina-6 se asociaron significativamente con mayor letalidad. Conclusiones. El itolizumab puede interrumpir la cascada inflamatoria y prevenir la morbilidad y mortalidad asociada al COVID-19.

ABSTRACT This report corresponds to quality of life analysis of patient with non-small cell lung cancer included in the phase III clinical trials Evaluation of CIMAvaxEGF® vaccine in lung cancer. The quality of life was evaluate using the EORTC questionnaires QLQ-C30 y QLQ-C13, at the beginning and every 3 months. To compare the median between two groups the Mann-Whitney non-parametric test was used. To compare the baseline and different follows times the Wilcoxon non-parametric test was used. The QLQ-C30 questionnaire showed a benefit in terms of the quality of life for the CIMAvaxEGF® vaccine group on the functional scores (global, role and social) and symptoms of the disease (pain). The QLQ-LC13 questionnaire showed a benefit in terms of the quality of life for the CIMAvaxEGF® vaccine group on the symptoms scores (dyspnea, dysphagia, alopecia and chest pain). It is noted as significant that the hemoptysis decreases in the group vaccinated as well as the dysphagia, the cough and the dyspnea observing a worsening in the control group.

RESUMEN Este reporte corresponde al análisis de la calidad de vida de los pacientes que se incluyeron en el ensayo clínico fase III de evaluación de la vacuna CIMAvaxEGF® en cáncer de pulmón de células no pequeñas. La calidad de vida se evaluó empleando los cuestionarios EORTC QLQ-C30 y QLQ-C13, al inicio y cada 3 meses hasta el fallecimiento del paciente a criterio del investigador. Para comparar las medianas entre los dos grupos se utilizó la prueba no paramétrica de Mann-Whitney. Las comparaciones entre el nivel basal y los diferentes tiempos de seguimiento se realizaron a través de la prueba no paramétrica de Wilcoxon. El cuestionario QLQ-C30 evidenció un beneficio en cuanto a calidad de vida para el grupo vacunado con la vacuna CIMAvaxEGF® en las escalas funcionales (global, rol y social), en las escalas de síntomas de la enfermedad y del tratamiento (dolor) se observó que mejora la calidad de los mismos a favor de los pacientes tratados con la vacuna CIMAvaxEGF®. El cuestionario QLQ-C13, también evidenció ventajas para el grupo vacunado desde el punto de vista de beneficio clínico en los síntomas (disnea, disfagia, alopecia y dolor en el pecho). Se señala como significativo que disminuye la hemoptisis y la tos en el grupo vacunado, observándose un empeoramiento en el grupo control.

The clinical research does not finish with the registration of a new drug, but with the modification of the population health indexes. This paper set forth the experience of the Cuban biotechnology in cancer control, in order to achieve the reduction of mortality rate from malignant tumors. Biotechnology has four areas of remarkable impact in cancer control: first, the primary prevention by means of prophylactic vaccines; second, the techniques for early diagnosis; third, the stratification of patients according to the molecular prognostic or predictive biomarkers and fourth, the treatment of advanced disseminated cancer with therapeutic cancer vaccines and monoclonal antibodies that detect specific targets in tumors. The use of these therapies has entailed a shift of paradigm in the cancer therapy. The Cuban experience has been successful due to the existence of a high standard health system based on complete coverage and equal access, the development of the primary medical care- the core of the Cuban health system -and the advance of a national, innovative biotech industry with manufacturing capacity to meet the national demand and to export products. In spite of the advances, big technical and methodological challenges still remain. Post-registration scientific research would define the strategy to optimize the impact of the new products in the population's health, aimed at increasing the life expectancy of the Cuban people.

La investigación clínica no termina con el registro del nuevo medicamento, sino con la modificación de los indicadores de salud poblacional. Este artículo ilustra la experiencia de la biotecnología cubana en el propósito de contribuir a la reducción de la mortalidad por tumores malignos. En el control del cáncer, la biotecnología tiene cuatro espacios de impacto: el primero es en la prevención primaria mediante vacunas profilácticas, el segundo, consiste en las técnicas de diagnóstico precoz, el tercero es la estratificación de los pacientes mediante marcadores moleculares pronósticos o predictivos y el cuarto rol radica en el tratamiento de la enfermedad diseminada con vacunas terapéuticas y anticuerpos monoclonales que reconocen blancos específicos en los tumores. El uso de estas terapias ha traído consigo un cambio de paradigma del tratamiento del cáncer. La experiencia cubana ha sido exitosa debido a la existencia de un sistema de salud con altos estándares basado en la cobertura completa y equidad de acceso, el desarrollo del nivel primario de atención médica, centro de gravedad del sistema cubano y el desarrollo en las últimas tres décadas de una industria biotecnológica nacional, innovadora y con capacidad productiva para cubrir las necesidades nacionales de productos y exportar. A pesar de los avances, quedan grandes retos técnicos y metodológicos. La investigación científica posregistro permitirá trazar la estrategia para optimizar el impacto de los nuevos productos en la salud poblacional, encaminada a incrementar la esperanza de vida de los cubanos.

The results allowing the Cuban Regulatory Agency (CECMED) to grant the Sanitary Registration to the CIMAvax-EGF cancer vaccine for advanced non-small cell lung cancer (NSCLC) are shown. This was the first registration of a therapeutic vaccine in Cuba and also the first registration of a lung cancer vaccine in the world. Hence, a unique therapeutic vaccine is offered to lung cancer patients, which will increase survival and their quality of life. For this purpose, significant preclinical, clinical, regulatory, productive and negotiation challenges were to be faced. The results obtained in these fields led to 18 scientific papers published in high impact journals and 4 invention objects, generating several patents in Cuba and other countries. In the pre-clinical setting, immunogenicity, safety and anti-tumoral effects were demonstrated in different animal species. The clinical experience began in 1995. Up to now, five phase I-II clinical trials have concluded in Cuba, two phase II have also concluded, one in Cuba and another one in Canada and the UK, and a phase II-III trial with an optimized schedule as well as a phase III trial are currently in progress in Cuba. In the regulatory field, a fast-track registration strategy was designed and performed. It required novel regulatory conceptions to develop this unique product. A scalable, reproducible and controlled productive process was carried out, together with a quality system that ensured full GMP compliance. Funds for product development came from implementing a novel negotiation strategy: negotiation of intangibles.
