ABSTRACT Congenital heart disease (CHD) affects eight to ten out of every 1,000 births, resulting in approximately 23,057 new cases in Brazil in 2022. About one in four children with CHD requires surgery or other procedures in the first year of life, and it is expected that approximately 81% of these children with CHD will survive until at least 35 years of age. Professionals choosing to specialize in CHD surgery face numerous challenges, not only related to mastering surgical techniques and the complexity of the diseases but also to the lack of recognition by medical societies as a separate subspecialty. Furthermore, families face difficulties when access to services capable of providing treatment for these children. To address these challenges, it is essential to have specialized hospitals, qualified professionals, updated technologies, sustainable industry, appropriate financing, quality assessment systems, and knowledge generation. The path to excellence involves specialization across all involved parties. As we reflect on the importance of Pediatric Cardiovascular Surgery and Congenital Heart Diseases establishing themselves as a subspecialty of Cardiovascular Surgery, it is essential to look beyond our borders to countries like the United States of America and United Kingdom, where this evolution is already a reality. This autonomy has led to significant advancements in research, education, and patient care outcomes, establishing a care model. By following this path in Brazil, we not only align our practice with the highest international standards but also demonstrate our maturity and the ability to meet the specific needs of patients with CHD and those with acquired childhood heart disease. ABSTRACT Objective: To evaluate the prevalence of sharps accidents among dental students in southwest Goiás state, Brazil, and further survey their knowledge of biosafety and post-injury management. Material and Methods: A cross-sectional analytical observational study was carried out in 2018 following the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) guidelines. The study population included dental students in the 4th and 10th course semester. A pre-formulated self-administered questionnaire containing 14 objective questions was used for data collection. Mean and standard deviation values were calculated. Results: A total of 308 responses were obtained. Overall, 15.9% of the respondents reported having previously experienced accidents with sharps. Most dental students who claimed to know the biological risks to which they are exposed were in the 5th and 8th course semesters, and 67.2% of them reported knowing how to proceed in the event of a sharp accident. Conclusion: A low prevalence of sharps accidents has been reported, and dental students are considered to have a good knowledge of biosafety. RESUMO Este estudo teve como objetivo analisar as características físicas dos ovos de galinhas caipiras var. Suras. Foram analisados um total de 100 ovos, onde diversos parâmetros físicos, tais como pesagem do ovo inteiro, medição do diâmetro equatorial e altura do ovo, classificação da cor da casca, miragem para verificação da presença de fendas na casca, determinação da altura da câmara de ar, classificação da cor da gema, verificação da presença de manchas de carne e manchas de sangue, medição da largura e comprimento do albúmen, medição do diâmetro da gema, pesagem da gema, e pesagem da casca. Foram posteriormente determinados o peso do albúmen por diferença entre o peso do ovo e pesos da casca e gema, determinação percentual dos diferentes constituintes do ovo, shape index (índice de forma) e shell index (índice de casca). A análise estatística foi realizada pelo GLM programa SAS (SAS Institute). O resultado comparando as médias de cada parâmetro estudado demonstrou que os ovos da variedade Sura em ambas as classes comerciais (Grande e Pequenos), possuem formato mais arredondado, com casca marrom clara e gema mais alaranjada. ABSTRACT This study aimed to analyze the physical characteristics of eggs from free-range hens of the Sura variety. One hundred eggs underwent assessment of various physical parameters, namely, whole egg weight, equatorial diameter, egg height, shell color, presence of cracks via candling, air chamber height, yolk color, presence of flesh and blood spots, albumen width and length, yolk diameter, yolk weight, and shell weight. Additional analyses included determining the albumen weight (calculated by subtracting the weights of the shell and yolk from the total egg weight), percentages of egg components, shape index, and shell index. Statistical analyses were conducted using GLM SAS software (SAS Institute). The results revealed distinct physical traits; eggs from the Sura variety, regardless of size class (large or small), typically have a more rounded shape, light brown shells, and more orange yolks. SESQUITERPENES ALKALOIDS FROM Maytenus distichophylla (CELASTRACEAE). The phytochemical study of the roots of Maytenus distichophylla Mart. ex Reissek (Celastraceae), resulted in the isolation of the seven sesquiterpenes pyridine alkaloids euojaponine F (1), euojaponine K (2), ebinifoline W-I (3), wilforine (4), maiteine (5), euojaponine A (6) and ebinifoline E-II (7). The structures of compounds were established using spectroscopic techniques such as 1D and 2D NMR. All compounds were subjected to an in silico study through of a predictive model and then submitted to in vitro testing against S. epidermidis, P. aeruginosa, C. parapsilosis and C. albicans for evaluation of their antimicrobial activity. Abstract Background: Systemic inflammatory responses mimicking infectious complications are often present in surgical patients. Methods: The objective was to assess the association between withholding early antimicrobial therapy while investigating alternative diagnoses and worse outcomes in nonseptic patients with suspected nosocomial infection in a retrospective cohort of critically ill surgical patients. The initiation of antibiotic therapy within 24 h of the suspicion of infection was defined as the Early Empirical Antibiotic strategy (EEA) group and the initiation after 24 h of suspicion or not prescribed was defined as the Conservative Antibiotic strategy (CA) group. Primary outcome was composite: death, sepsis, or septic shock within 14 days. Main exclusion criteria were sepsis or an evident source of infection at inclusion. Results: Three hundred and forty patients were eligible for inclusion (74% trauma patients). Age, sex, reason for hospital admission, SAPS3 score, SOFA score, and use of vasopressors or mechanical ventilation were not different between the groups. Within 14 days of inclusion, 100% (130/130) of EEA patients received antibiotics compared to 57% (120/210) of CA patients. After adjusting for confounding variables, there was no association between primary outcome and the groups. In a post hoc subgroup analysis including only patients with a posteriori confirmed infection (by microbiological cultures), delay in initiation of adequate antimicrobial therapy was independently associated with the primary outcome (Odds Ratio = 1.19 per day of delay; 95% CI 1.05–1.37). Conclusions: Withholding early empiric antibiotic therapy was not associated with progression of organ dysfunction within 14 days in nonseptic surgical patients with suspected nosocomial infection without an obvious source. Resumo Defeitos de grandes segmentos traqueais podem apresentar sério problema clínico, devido à ausência de substitutos na cirurgia reconstrutiva. Os biomateriais poliméricos estão entre os mais utilizados na medicina. Entretanto, a implantação de um biomaterial desencadeia uma série de eventos biológicos e a biocompatibilidade do material é de extrema importância para a medicina regenerativa. Objetivou-se com este estudo avaliar o uso da copoliamida associada ao elastômero termoplástico (PCTPE) em próteses impressas em 3D para reparação de defeitos parciais da traqueia de coelhos. Foram utilizados 16 coelhos machos da raça Nova Zelândia Branco, submetidos à criação de um defeito parcial de traqueia na região ventral do 4º ao 8º anel traqueal. Os animais foram subdivididos (n=4) de acordo com o momento das eutanásias, programadas com intervalo de sete (G7), quinze (G15), trinta (G30) e sessenta dias (G60). A análise histopatológica por hematoxilina e eosina (HE) revelou que o implante 3D de PCTPE apresentou reação do tipo corpo estranho e a inflamação persistiu até os 60 dias de avaliação. Abstract Large segmental tracheal defects can pose a serious clinical challenge owing to the lack of suitable substitutes for reconstructive surgery. Polymeric biomaterials are widely used in medicine. However, the implantation of biomaterials triggers a series of biological events, and material biocompatibility is of paramount importance in regenerative medicine. The objective of this study was to evaluate the use of a copolyamide associated with thermoplastic elastomer (PCTPE) in 3D-printed prostheses for repairing partial tracheal defects in rabbits. Sixteen male New Zealand White rabbits were used, and partial tracheal defects were created in the ventral region from the 4th to the 8th tracheal ring. The animals were subdivided into groups (n=4) based on the time of euthanasia, scheduled at seven days (G7), 15 days (G15), 30 days (G30), and 60 days (G60). Histopathological analysis with hematoxylin and eosin staining revealed that the 3D implant of PCTPE exhibited a foreign body reaction, and inflammation persisted for up to 60 days. Histochemistry with picrosirius red revealed a tendency for a greater amount of type I collagen to accumulate in the early stages of inflammation, whereas type III collagen was predominant in later evaluation periods. RESUMO A couve-de-folhas é uma das hortaliças folhosas mais cultivadas no Brasil, porém seu melhoramento ainda é limitado. Portanto, é crucial desenvolver pesquisas que explorem a diversidade genética dos germoplasmas disponíveis, visando estabelecer cruzamentos promissores nas populações estudadas. O objetivo deste estudo foi avaliar a divergência genética entre progênies de meios-irmãos de couve-de-folhas obtidas por seleção recorrente e selecionar as progênies mais promissoras para compor uma população recombinante utilizando métodos multivariados. Como tratamentos, foram avaliadas plantas obtidas a partir de sementes de 25 progênies de meio-irmãos pré-selecionadas e três cultivares comerciais. Para produzir essas sementes, clones das progênies foram submetidos a recombinação em 2018, com subsequente seleção de sementes de plantas com desempenho agronômico superior. Foram avaliadas doze características quantitativas e quinze qualitativas em cada parcela, totalizando vinte e sete características avaliadas. O estudo da divergência genética foi realizado através de variáveis canônicas para os dados quantitativos. O método UPGMA foi aplicado a dados quantitativos, qualitativos e combinados, utilizando-se a distância genética de Mahalanobis. As três primeiras variáveis canônicas explicaram 73,75% da variação total acumuladas quando comparadas às cultivares comerciais, e 55,29% quando considerado exclusivamente as progênies meios-irmãos. As progênies de meios-irmãos P1, P2, P3, P4, P5, P9, P11, P15, P16, P18, P20, P22, P23, P24 e P25 foram as mais divergentes e devem ser priorizadas na composição da população recombinante para o terceiro ciclo de seleção recorrente, pois além de variação genética, também apresentam características de interesse ao programa de melhoramento genético da couve. ABSTRACT Kale is one of the most widely cultivated leafy vegetables in Brazil, but its breeding genetic is still limited. Therefore, it is crucial to develop research that explores the genetic diversity of the available germplasms, aiming to establish promising crosses in the studied populations. The present study aimed to evaluate the genetic divergence between half-sibling progenies of kale, obtained through recurrent selection, using multivariate methods to select the most promising progenies for forming a recombinant population. The plants obtained from the seeds of 25 pre-selected half-sibling progenies and three commercial cultivars were evaluated as treatment groups. These seeds were produced in 2018 by subjecting the clones of the progenies to recombination, followed by the selection of appropriate seeds from plants with superior agronomic performance. In the present study, plants in each plot were evaluated for twelve quantitative and fifteen qualitative traits. Genetic divergence was studied based on canonical variables to obtain quantitative data. The UPGMA method was applied to the quantitative, qualitative, and combined data, and Mahalanobis genetic distance was adopted. The first three canonical variables explained 73.75% of the total cumulative variation compared to the commercial cultivars and 55.29% when considering the half-sibling progenies exclusively. The half-sibling progenies P1, P2, P3, P4, P5, P9, P11, P15, P16, P18, P20, P22, P23, P24, and P25 were the most divergent and should, therefore, be prioritized when composing the recombinant population for the third cycle of recurrent selection because, in addition to genetic variation, these progenies also present characteristics of interest to the kale genetic breeding program. Abstract Given escalating environmental concerns and stricter regulations, there’s a pressing need for sustainable materials. Eco-composites, which blend natural fibers with polymer matrices for reinforcement, are gaining traction. This study stands out by employing Australian royal palm stem fibers within a polystyrene matrix for eco-composites. The research involved assessing thermal stability, quantifying lignin/carbohydrate content, and measuring moisture levels. Scanning electron microscopy was utilized to examine fiber structure. Results from evaluations of thermal and mechanical properties underscore the high-performance reinforcement potential of Australian royal palm fibers, suggesting their suitability for various eco-composite applications. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. RESUMO Esse estudo objetivou investigar os efeitos da manipulação da altura da rede em jogos reduzidos de 4 x 4 no desempenho tático-técnico de atletas profissionais femininas de voleibol. Participaram oito atletas femininas profissionais, que jogaram 4 x 4 em três alturas de rede: 2,15 m, 2,24 m e 2,33 m. As ações técnico-táticas (Desempenho Específico por Ação, Desempenho Específico por Componente e Desempenho Geral) foram analisadas através do IAD-VB. ABSTRACT This study investigated the effects of manipulating the net height in 4 vs. 4 small-sided games on the tactical-technical performance of professional female volleyball athletes. Eight professional female athletes participated in this study, playing 4 vs. 4 small-sided games at three net heights: 2.15 m, 2.24 m, and 2.33 m. RESUMO A deficiência de alfa-1 antitripsina (DAAT) é uma herança genética autossômica codominante, relativamente rara, que resulta em concentração reduzida de alfa-1 antitripsina (AAT) no soro e, portanto, redução na atividade antielastase nos pulmões e aumento do risco de enfisema pulmonar, cirrose hepática e paniculite necrotizante. Resulta de diferentes mutações no gene SERPINA1 levando a mudanças na glicoproteína AAT, que podem alterar a sua concentração, conformação e/ou função. Infelizmente, o subdiagnóstico é muito comum, e é possível que apenas 10% dos casos estejam diagnosticados. A variante Z é a deficiência mais comum, e estima-se que mais de 3 milhões de pessoas em todo o mundo tenham combinações de alelos associadas à deficiência grave de AAT. É necessária a determinação da concentração sérica de AAT e a identificação de variantes alélicas por fenotipagem ou genotipagem. É fundamental o acompanhamento da função pulmonar, principalmente por espirometria, pois essa informa sobre a progressão da doença. A densitometria pulmonar parece ser a medida mais sensível da progressão do enfisema, mas não deve ser usada no acompanhamento de pacientes na prática clínica de rotina. O tratamento geral é semelhante ao indicado para pacientes com DPOC não causada por DAAT. ABSTRACT Alpha-1 antitrypsin deficiency (AATD) is a relatively rare genetic disorder, inherited in an autosomal codominant manner, that results in reduced serum AAT concentrations, with a consequent reduction in antielastase activity in the lungs, as well as an increased risk of diseases such as pulmonary emphysema, liver cirrhosis, and necrotizing panniculitis. It results from different mutations in the SERPINA1 gene, leading to changes in the AAT glycoprotein, which can alter its concentration, conformation, and function. Unfortunately, underdiagnosis is quite common; it is possible that only 10% of cases are diagnosed. The most common deficiency is in the Z variant, and it is estimated that more than 3 million people worldwide have combinations of alleles associated with severe AATD. Serum AAT concentrations should be determined, and allelic variants should be identified by phenotyping or genotyping. Abstract Background Patients with immune-mediated rheumatic diseases (IMRDs) have been prioritized for COVID-19 vaccination to mitigate the infection severity risks. Patients with rheumatoid arthritis (RA) are at a high risk of severe COVID-19 outcomes, especially those under immunosuppression or with associated comorbidities. However, few studies have assessed the safety of the COVID-19 vaccine in patients with RA. Objective To evaluate the safety of vaccines against SARS-CoV-2 in patients with RA. Methods This data are from the study “Safety and Efficacy on COVID-19 Vaccine in Rheumatic Diseases,” a Brazilian multicentric prospective phase IV study to evaluate COVID-19 vaccine in IMRDs in Brazil. Adverse events (AEs) in patients with RA of all centers were assessed after two doses of ChAdOx1 (Oxford/AstraZeneca) or CoronaVac (Sinovac/Butantan). Stratification of postvaccination AEs was performed using a diary, filled out daily and returned at the end of 28 days for each dose. Results A total of 188 patients with RA were include, 90% female. CoronaVac was used in 109 patients and ChAdOx1 in 79. Only mild AEs were observed, mainly after the first dose. The most common AEs after the first dose were pain at the injection (46,7%), headache (39,4%), arthralgia (39,4%), myalgia (30,5%) and fatigue (26,6%), and ChAdOx1 had a higher frequency of pain at the injection (66% vs 32 %, p < 0.001) arthralgia (62% vs 22%, p < 0.001) and myalgia (45% vs 20%, p < 0.001) compared to CoronaVac. The more common AEs after the second dose were pain at the injection (37%), arthralgia (31%), myalgia (23%), headache (21%) and fatigue (18%). Arthralgia (41,4% vs 25%, p = 0.02) and pain at injection (51,4% vs 27%, p = 0.001) were more common with ChAdOx1. No serious AEs were related. With Regard to RA activity level, no significant difference was observed between the three time periods for both COVID-19 vaccines. Conclusion In the comparison between the two immunizers in patients with RA, local reactions and musculoskeletal symptoms were more frequent with ChAdOx1 than with CoronaVac, especially after the first dose. ABSTRACT Cauda Equina Syndrome (CES) is a severe neurological emergency resulting from the compression of the lumbosacral nerve roots, potentially causing motor and sensory deficits, as well as sphincter dysfunctions. Early surgical intervention is essential to prevent permanent neurological sequelae. Traditionally, open laminectomy has been the treatment of choice for decompression. Still, minimally invasive techniques, such as endoscopic discectomy, are gaining popularity due to lower morbidity, smaller incisions, and faster recovery. This study reports three cases of patients with CES treated with endoscopic discectomy. Two cases were caused by a large, extruded disc herniation and one by an intracanal facet cyst. Pain intensity was assessed using the Visual Analog Scale (VAS), and functional disability was measured with the Oswestry Disability Index (ODI) in the preoperative period and at follow-ups of 1, 6, and 12 months. All three patients showed significant improvement in symptoms, with pain reduction and functional improvement. At 12 months, the patients maintained their functional and neurological recovery without reported complications. The endoscopic technique proved to be a safe and effective method for treating CES in these cases, offering early recovery and fewer postoperative complications. RESUMEN El síndrome de cola de caballo (SCC) es una emergencia neurológica grave resultante de la compresión de las raíces nerviosas lumbosacras, lo que puede ocasionar déficits motores, sensoriales y disfunciones esfinterianas. La intervención quirúrgica precoz es fundamental para evitar secuelas neurológicas permanentes. Tradicionalmente, la laminectomía abierta ha sido el enfoque de elección para la descompresión, pero las técnicas mínimamente invasivas, como la discectomía endoscópica, están ganando popularidad debido a su menor morbilidad, incisiones más pequeñas y recuperación más rápida. Este estudio presenta tres casos de pacientes con SCC tratados con discectomía endoscópica. Dos de los casos fueron causados por una hernia discal extrusa volumétrica y uno por un quiste facetario intracanal. La intensidad del dolor se evaluó mediante la Escala Visual Analógica (EVA) y la discapacidad funcional mediante el Oswestry Disability Index (ODI) en el período preoperatorio y en los seguimientos de 1, 6 y 12 meses. Los tres pacientes mostraron una mejora significativa en los síntomas, con reducción del dolor y mejora funcional. Este estudo relata três casos de pacientes com SCE tratados com discectomia endoscópica. Dois dos casos foram causados por hérnia discal extrusa volumosa e um por cisto facetário intracanal. A intensidade da dor foi avaliada pela Escala Visual Analógica (EVA) e a incapacidade funcional pelo Oswestry Disability Index (ODI) no período pré-operatório e em seguimentos de 1, 6 e 12 meses. Os três pacientes apresentaram melhora significativa dos sintomas, com redução da dor e melhora funcional. Aos 12 meses, os pacientes mantiveram a recuperação funcional e neurológica, sem complicações relatadas. A técnica endoscópica mostrou-se uma técnica segura e eficaz no tratamento da SCE nos casos relatados, oferecendo recuperação precoce e menos complicações pós-operatórias. ABSTRACT The objective of this article was to present an uncommon case of fully endoscopic resection of an osteoid osteoma in the S1 vertebra in a patient with lower back pain and no neurological symptoms. The patient was a 26-year-old male with significant lower back pain, worsening at night and improving with the use of non-steroidal anti-inflammatory drugs (NSAIDs). Lumbar imaging revealed an osteoblastic lesion near the right pedicle in the S1 vertebral body, with no evident nidus or contrast uptake. A biopsy guided by computed tomography confirmed the diagnosis of osteoid osteoma. The patient was treated with a fully endoscopic transcorporeal resection of S1 via a posterolateral extra-foraminal approach. The surgical technique was detailed, and the patient showed immediate clinical symptom improvement. Although there are several surgical techniques for the resection of spinal tumors, there is no consensus in the literature regarding the best surgical technique or conservative treatments. However, fully endoscopic techniques are emerging as a promising method for both diagnosis and treatment, potentially reducing complications and procedural morbidity. RESUMEN El objetivo de este artículo fue presentar un caso inusual de resección totalmente endoscópica de un osteoma osteoide en la vértebra S1 en un paciente con dolor lumbar sin síntomas neurológicos. El paciente era un hombre de 26 años con dolor lumbar significativo, que empeoraba por la noche y mejoraba con el uso de antiinflamatorios no esteroideos (AINE). Las imágenes lumbares revelaron una lesión osteoblástica en el cuerpo vertebral de S1, cerca del pedículo derecho, sin nidus evidente y sin captación de contraste. Se realizó una biopsia guiada por tomografía computarizada, que confirmó el diagnóstico de osteoma osteoide. El paciente fue tratado con una resección transcorporal totalmente endoscópica de S1 mediante un abordaje posterolateral extra-foraminal. La técnica quirúrgica fue descripta en detalle, y el paciente mostró una mejora clínica inmediata de los síntomas. Aunque existen varias técnicas quirúrgicas para la resección de tumores espinales, no hay consenso en la literatura sobre la mejor técnica quirúrgica o sobre los tratamientos conservadores. RESUMO O objetivo desse artigo foi apresentar um caso incomum de ressecção totalmente endoscópica de um osteoma osteóide na vértebra S1 em um paciente com dor lombar sem sintomas neurológicos. Paciente masculino de 26 anos com dor lombar baixa importante, com piora noturna e melhora com uso de anti-inflamatórios não esteróides (AINE). Exames de imagem lombar evidenciaram lesão osteoblástica no corpo de S1 próximo do pedículo direito, sem nidus evidente e sem captação por contraste. Foi realizada biópsia guiada por tomografia computadorizada, confirmando o diagnóstico de osteoma osteóide. O paciente foi tratado com ressecção totalmente endoscópica transcorpórea de S1 por via póstero-lateral extra-foraminal. A técnica cirúrgica foi descrita detalhadamente e o paciente apresentou melhora clínica imediata dos sintomas. Embora existam diversas técnicas cirúrgicas para ressecção de tumores na coluna, não há consenso na literatura sobre a melhor técnica cirúrgica ou sobre tratamentos conservadores. Entretanto, a técnica totalmente endoscópica vem surgindo como um método promissor, tanto para diagnóstico quanto para tratamento, com possibilidade de redução de complicações e morbidade do procedimento.
