Abstract Background: Currently, there is no uniform and official terminology in Portuguese for reflectance confocal microscopy analysis, despite the increasing number of Brazilian dermatologists using this new tool. Objective: To present the terminology in Brazilian Portuguese for the description of reflectance confocal microscopy and establish the first Brazilian consensus on terms related to normal skin and cutaneous tumors. Methods: 10 Brazilian specialists from different institutions and states of Brazil were selected to evaluate the best corresponding terms in Portuguese for normal skin, melanocytic and non-melanocytic tumors. The terms used were translated from international consensuses in the English language. The modified Delphi method was used to create the consensus in 3 steps. Results: The terms considered the most appropriate in the Portuguese language to describe the findings of normal skin, melanocytic and non-melanocytic lesions in the reflectance confocal microscopy analysis were presented. Study limitations: The limitations of the present study include the number of participants and limited regional representation (only two of the five Brazilian regions were represented). Conclusion: This Brazilian consensus represents an opportunity for dermatologists and physicians specializing in cutaneous oncology to become familiar with reflectance confocal microscopy, propagating the technique in clinical and research environments to stimulate national and international publications on this subject. Abstract Objective To develop the second evidence-based Brazilian Society of Rheumatology consensus for diagnosis and treatment of lupus nephritis (LN). Methods Two methodologists and 20 rheumatologists from Lupus Comittee of Brazilian Society of Rheumatology participate in the development of this guideline. Fourteen PICO questions were defined and a systematic review was performed. Eligible randomized controlled trials were analyzed regarding complete renal remission, partial renal remission, serum creatinine, proteinuria, serum creatinine doubling, progression to end-stage renal disease, renal relapse, and severe adverse events (infections and mortality). The Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach was used to develop these recommendations. Recommendations required ≥82% of agreement among the voting members and were classified as strongly in favor, weakly in favor, conditional, weakly against or strongly against a particular intervention. Other aspects of LN management (diagnosis, general principles of treatment, treatment of comorbidities and refractory cases) were evaluated through literature review and expert opinion. Results All SLE patients should undergo creatinine and urinalysis tests to assess renal involvement. Kidney biopsy is considered the gold standard for diagnosing LN but, if it is not available or there is a contraindication to the procedure, therapeutic decisions should be based on clinical and laboratory parameters. Fourteen recommendations were developed. Target Renal response (TRR) was defined as improvement or maintenance of renal function (±10% at baseline of treatment) combined with a decrease in 24-h proteinuria or 24-h UPCR of 25% at 3 months, a decrease of 50% at 6 months, and proteinuria < 0.8 g/24 h at 12 months. Hydroxychloroquine should be prescribed to all SLE patients, except in cases of contraindication. Glucocorticoids should be used at the lowest dose and for the minimal necessary period. In class III or IV (±V), mycophenolate (MMF), cyclophosphamide, MMF plus tacrolimus (TAC), MMF plus belimumab or TAC can be used as induction therapy. For maintenance therapy, MMF or azathioprine (AZA) are the first choice and TAC or cyclosporin or leflunomide can be used in patients who cannot use MMF or AZA. Rituximab can be prescribed in cases of refractory disease. In cases of failure in achieving TRR, it is important to assess adherence, immunosuppressant dosage, adjuvant therapy, comorbidities, and consider biopsy/rebiopsy. Resumo Trata-se de um artigo original que aborda a força de trabalho (FT) em saúde no Brasil, suas comorbidades e alterações da saúde mental na pandemia da COVID-19. O estudo conduzido pela Fundação Oswaldo Cruz coletou dados por meio de questionário on-line com um total de 36.612 participantes: profissionais de saúde (PS, formação de nível superior), e trabalhadores invisíveis (TI, nível técnico). A prevalência de comorbidades no Brasil foi de 26,1% e 23,9%, a maior foi hipertensão arterial (27,4% e 31,9%), seguida da obesidade (18,4% e 15,1%), doenças crônicas respiratórias (15,7% e 12,9%), diabetes mellitus (10,3% e 10,4%), e depressão/ansiedade (9,1% e 11,7%), nos PS e TI, respectivamente. A região com maior frequência foi a sudeste onde concentra-se o maior contingente de trabalhadores. A FT acometida com alta carga de doenças crônicas não transmissíveis e exposta ao SARS-CoV-2, torna-se vulnerável para o adoecimento e morte. Sintomas mentais e intenso sofrimento psíquico foram relatados. Os resultados deste estudo nos permitem estimar o impacto na saúde física e mental, e nas condições de vida e de trabalho da FT. Abstract This is an original article that addresses the healthcare workforce (HW) in Brazil, as well as comorbidities and mental health changes during the COVID-19 pandemic. This study was conducted by the Oswaldo Cruz Foundation and collected data through an online questionnaires from a total of 36,612 participants, health professionals (HP, with higher education level), and invisible healthcare workers (IHW, with a technical mid-level education). The overall prevalence of comorbidities in Brazil was 26.1% and 23.9%; the highest was arterial hypertension (27.4% and 31.9%), followed by obesity (18.4% and 15.1%), chronic respiratory diseases (15.7% and 12.9%), diabetes mellitus (10.3% and 10.4%), and depression/anxiety (9.1% and 11.7%), in the HW and IHW, respectively. The region with the highest frequency was the southeast, where the largest contingent of workers is located. The HW, affected with a high burden of non-communicable chronic diseases and exposed to SARS-CoV-2, proved to be vulnerable to illness and death. Mental symptoms and intense psychological suffering have been reported. These results allow us to estimate the impacts upon physical and mental health, as well as upon living and working conditions of the HW. The health and life of workers, leading role in facing health challenges of the pandemic, are a high priority in public policies. Resumo Fundamento A síndrome de takotsubo (takotsubo) é uma forma de cardiomiopatia adquirida. Dados nacionais sobre essa condição são escassos. O Registro REMUTA é o primeiro a incluir dados multicêntricos dessa condição no nosso país. Objetivo Descrever as características clínicas, prognóstico, tratamento intra-hospitalar e mortalidade hospitalar e em 1 ano de seguimento. Métodos Estudo observacional, retrospectivo, tipo registro. Incluídos pacientes internados com diagnóstico de takotsubo ou que desenvolveram esta condição durante internação por outra causa. Os desfechos avaliados incluíram fator desencadeador, análise dos exames, uso de medicações, complicações e óbito intra-hospitalar e em 1 ano de seguimento. O nível de significância adotado foi de 5%. Resultados Foram incluídos 169 pacientes, em 12 centros no Estado do Rio de Janeiro. A idade média foi de 70,9 ± 14,1 anos e 90,5% eram do sexo feminino; 63% dos casos foram de takotsubo primário e 37% secundário. Troponina I foi positiva em 92,5% dos pacientes e a mediana de BNP foi de 395 (176,5; 1725). Supradesnivelamento do segmento ST esteve presente em 28% dos pacientes. A fração de ejeção do ventrículo esquerdo teve mediana de 40 (35; 48)%. Observamos taxa de 25,7% de ventilação mecânica invasiva e 17,4% de choque. Suporte circulatório mecânico foi utilizado em 7,7%. A mortalidade intra-hospitalar foi de 10,6% e a mortalidade ao final de 1 ano foi de 16,5%. Takotsubo secundário e choque cardiogênico foram preditores independentes de mortalidade. Conclusão Os resultados do REMUTA mostram que takotsubo não se trata de patologia benigna como se pensava, especialmente no grupo de takotsubo secundário que acarreta elevada taxa de complicações e de mortalidade. Abstract Background Takotsubo syndrome (TTS) is an acquired form of cardiomyopathy. National Brazilian data on this condition are scarce. The Takotsubo Multicenter Registry (REMUTA) is the first to include multicenter data on this condition in Brazil. Objective To describe the clinical characteristics, prognosis, in-hospital treatment, in-hospital mortality, and mortality during 1 year of follow-up. Methods This is an observational, retrospective registry study including patients admitted to the hospital with diagnosis of TTS and patients admitted for other reasons who developed this condition. Evaluated outcomes included triggering factor, analysis of exams, use of medications, complications, in-hospital mortality, and mortality during 1 year of follow-up. A significance level of 5% was adopted. Results The registry included 169 patients from 12 centers in the state of Rio de Janeiro, Brazil. Mean age was 70.9 ± 14.1 years, and 90.5% of patients were female; 63% of cases were primary TTS, and 37% were secondary. Troponin I was positive in 92.5% of patients, and median BNP was 395 (176.5; 1725). ST-segment elevation was present in 28% of patients. Median left ventricular ejection fraction was 40 (35; 48)%. We observed invasive mechanical ventilation in 25.7% of cases and shock in 17.4%. Mechanical circulatory support was used in 7.7%. In-hospital mortality was 10.6%, and mortality at 1 year of follow-up was 16.5%. Secondary TTS and cardiogenic shock were independent predictors of mortality. Conclusion The results of the REMUTA show that TTS is not a benign pathology, as was once thought, especially regarding the secondary TTS group, which has a high rate of complications and mortality. Abstract Although the conservation of tropical biodiversity depends on protected areas, there is still a very large ‘gap’ of knowledge on the flora of Brazilian reserves, especially in the Northeast region of Brazil. Field and herbarium surveys of the phanerogamic flora of the Ubajara National Park, located on the Brazilian Northeast, were made and analyses on phenology and dispersal syndromes were performed. 418 taxa (213 trees and shrubs, 100 terrestrial herbs, 68 climbing plants, 33 sub-shrubs, two epiphytes, one hemiparasite and one aquatic herb) were recorded. The most representative families were: Fabaceae, Malvaceae, Asteraceae, Rubiaceae and Euphorbiaceae. The annual flowering / fruiting peak hypothesis was not fully confirmed, therefore, the forest may be an important food resource for the fauna all year long (especially in the moister region). Zoochory was the predominant dispersal syndrome in the moister area, whereas, autochory and anemochory together, predominated in the drier area.

ABSTRACT Biological insecticides can be another pest control method used in integrated pest management; hence there is a demand for scientific information about these products to understand the appropriate application technique. The aim of this study was to evaluate the spray spectra data by different nozzle types using biological insecticide isolated and with adjuvants, as well the surface tension of these spray solutions. For droplet spectra data, a particle size analyzer was used and for surface tension of the spray solution, the method of pendant drop was used. Among the nozzles tested, ADI 11002 provided greater droplets median diameter volume, a lower percentage of droplets with diameters less than 100 μm and also the highest coefficient of uniformity. In general, the addition of adjuvants reduced the median droplet diameter and increased the proportion of drops susceptible to spray drift. By adding adjuvants to the spray solution there was a lower surface tension, which were dependent of the adjuvant chemical group. The Silwet L-77® adjuvant showed the lowest surface tension among the spray solutions evaluated.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Este artigo descreve a experiência de formação de coordenadores pedagógicos em Alimentação Escolar desenvolvida pelo Centro Colaborador em Alimentação e Nutrição do Escolar da Universidade Federal da Bahia. Utilizou-se da observação participante e análise de dados documentados em: filmagens, resumos de discussão grupal, avaliações processuais e finais e diários de campo. As experiências de formação foram em 2011, em dois municípios da Bahia e na capital de Sergipe, com 118 coordenadores pedagógicos de 79 municípios dos dois estados. Constatou-se que os objetivos da formação de sensibilizar e orientar os coordenadores para a transversalização do tema alimentação e nutrição no currículo escolar foram alcançados. A proposta educativa foi avaliada pelos participantes como "dinâmica", "produtiva" e "interativa", promovendo trocas de experiências. Embora considerem um tema relevante, os coordenadores pedagógicos não têm formação suficiente nos conhecimentos de alimentação e nutrição para subsidiar as suas práticas pedagógicas. Considera-se que a formação apresente limitações quanto a seu desdobramento no campo de trabalho, pois a sensibilização alcançada pode ser fragilizada na volta dos atores ao seu ambiente laboral por falta de uma formação permanente.
This article describes the experience of in-service training for pedagogical coordinators of the Brazilian School Nutrition Program (PNAE) developed by the Cooperation Center for Student Food and Nutrition of the Federal University of Bahia (CECANE-UFBA). Participant observation and analysis of data documented in films, group discussion summaries, procedural and final evaluations and field diaries are used. The formative experiences were in 2011, in two municipalities in Bahia and Sergipe states, involving 118 pedagogical coordinators from 79 municipalities in both states. It was revealed that the objective of raising awareness and guiding pedagogical coordinators for mainstreaming the theme of food and nutrition has been achieved. The educational proposal was evaluated by the coordinators as "dynamic," "productive" and "interactive," promoting the exchange of experiences among participants. Despite considering the theme relevant, the pedagogical coordinators do not have sufficient training about food and nutrition to back their teaching practices. It is considered that their training has some limitations on the impact in the fieldwork, as the knowledge acquired night be dissipated when the coordinators return to their workplace due to the lack of ongoing training.

The educational proposal was evaluated by the coordinators as "dynamic," "productive" and "interactive," promoting the exchange of experiences among participants. Despite considering the theme relevant, the pedagogical coordinators do not have sufficient training about food and nutrition to back their teaching practices. It is considered that their training has some limitations on the impact in the fieldwork, as the knowledge acquired night be dissipated when the coordinators return to their workplace due to the lack of ongoing training.

Molecular matching of red blood cells is superior to serological matching in sickle cell disease patients

OBJECTIVE: To evaluate the usefulness of DNA methods to provide a means to precisely genotypically match donor blood units for the antigen-negative type of 35 sickle cell disease patients<. METHODS: Red blood cell units were investigated for ABO, D, C, c, E, e, K, Fyª, Fy b, Jkª, Jk b, S, s, Diª and RH variants by performing a molecular array (Human Erythrocyte Antigen BeadChipTM, BioArray Solutions), polymerase chain reaction followed by restriction fragment length polymorphism analysis and sequencing of patient samples and donor units that had been serologically matched based on the ABO, Rh and K phenotypes and the presence of antibodies. RESULTS: Matches for 21 of 35 sickle cell disease patients presented discrepancies or mismatches for multiple antigens between the genotype profile and the antigen profile of their serologically-matched blood units. The main discrepancies or mismatches occurred in the RH, FY, JK and MNS systems. Eight Rh alloimmunized patients presented RHD and RHCE variants that had not been serologically identified. According to these results better matches were found for the patients with genotyped units and the patients benefited as shown by better in vivo red blood cell survival. CONCLUSION: Molecular matching is superior to serological matching in sickle cell disease patients, decreasing the risk of transfusion reactions, especially delayed transfusion reactions to existing alloantibodies and preventing alloimmunization.

A infectividade e a reprodução de Meloidogyne javanica e de Heterodera glycines, em cultivar suscetível e resistente de soja, diferiram, de acordo com a temperatura do solo. A 28ºC o número de massas de ovos, total de ovos, de fêmeas e a porcentagem de sucesso de parasitismo dos juvenis do segundo estádio (J2) em cada cultivar (suscetível e resistentes) de soja infestada com M. javanica foram, significativamente maiores, comparados com as demais temperaturas (32º C, 26º C, 24º C e 20º C). Entretanto, a soma de juvenis do terceiro e quarto estádios (J3 e J4) foi maior em todas as cultivares, quando a temperatura submetida às plantas inoculadas com M. javanica foi de 20º C comparada com as demais temperaturas. A capacidade reprodutiva da fêmea (ovos por fêmea) de M. javanica foi sempre maior na cultivar suscetível em cada temperatura testada, porém, significativamente maior na cultivar suscetível colocada a 32º C comparada com as demais temperaturas. Nas relações H. glycines e soja, as temperaturas testadas proporcionaram valores semelhantes para a população total (fêmeas e cistos) na cultivar resistente. Na cultivar suscetível, a temperatura de 28º C aumentou significativamente o número de fêmeas, total de fêmeas e cistos e a porcentagem de parasitismo de J2 de H. glycines, comparada com todas as demais temperaturas.
The infectivity and reproduction of Meloidogyne javanica and Heterodera glycines in susceptible and resistant soybean cultivars differs according to soil temperatures. At 28ºC the egg-mass number, total eggs, and females, and percentage of parasitism success of second stage juveniles (J2) in each cultivar (susceptible and resistants) of infested soybean with M. javanica were significantly greater when compared to other temperatures (32º C, 26º C, 24º C and 20º C). However, the sum of third and fourth juvenile stages (J3 and J4) was greater in all cultivars when M. javanica inoculated plants were submitted to 20ºC. The female reproductive capacity (eggs per female) of M. javanica was always greater on susceptible cultivars in each tested temperature, but significantly greater in the susceptible cultivars submitted to 32ºC as compared to other temperatures. On the interaction between H. glycines and resistant soybean cultivars, the different temperatures resulted in similar values for the total population (females + cysts no.) on the resistant cultivars. On susceptible cultivars 28º C increased significantly the number of females, total females + cysts and the percentage of J2 parasitism of H. glycines as compared to all other temperatures.

Atração e penetração de Meloidogyne javanica e Heterodera glycines em raízes excisadas de soja

Com vista ao estudo de atração e penetração de Meloidogyne javanica (Treub) Chitwood e Heterodera glycines (Ichinoe) em soja (Glycine max L.), desenvolveu-se uma técnica empregando-se segmento de raiz com 2cm de comprimento. Nos segmentos de raiz de soja infectados, observou-se que a penetração de juvenis de segundo estádio (J2) de M. javanica ocorre pela coifa seguida de migração entre os feixes vasculares do cilindro central. Juvenis de H. glycines penetraram, aproximadamente, 15mm da coifa. A região seccionada da raiz de soja atraiu três vezes mais J2 de M. javanica do que a região da coifa, mas esta não foi tão atrativa para J2 de H. glycines. A obstrução conjunta da coifa e do local seccionado reduziu (83%) a penetração de J2, tanto de M. javanica quanto de H. glycines. Quando apenas um desses locais foi obstruído, a outra extremidade livre compensou o processo atrativo. Portanto, as substâncias atrativas são liberadas por essas extremidades. A penetração de J2 de M. javanica foi maior no segmento de raiz quando comparada com a plântula intacta de soja. Entretanto, os J2 de H. glycines penetraram menos em segmentos de raiz e em plântulas sem folhas, quando comparados com plântulas intactas e com as seccionadas no colo. Portanto, na cultivar de soja "Embrapa 20", a atração e os locais de penetração de J2 de H. glycines e M. javanica são diferenciados. Esta técnica poderá ser útil nos estudos de atração e penetração de outros nematoides endoparasitas.
To study the attraction and penetration of Meloidogyne javanica (Treub) Chitwood and Heterodera glycines (Ichinoe) in soybean (Glycine max L.), a technique using 2-cm long root segments was developed. In infected soybean root segments penetration of second stage juveniles (J2) of M. javanica occured through the root cap following migration between the vascular bundles of the central cylinder. Juveniles of H. glycines penetrated about 15mm from the root cap. The cut region of the soybean root attracted three times more J2 of M. javanica when compared to the root cap, but was not as attractive to H. glycines J2. Sealing both root cap and cut region reduced (83%) the J2 penetration of M. javanica and H. glycines. When only one of these penetration sites was sealed the open site compensated attractiveness. These results show that the attractive substances are released by these two penetration sites. The M. javanica J2 penetration was higher in root segments when compared to whole soybean plants. However, H. glycines J2 penetrated less in root segments and in roots of seedlings without leaves when compared to roots of whole soybean plants and roots of plants without the aerial part. In short, attraction and infection sites of H. glycines and M. javanica J2s in soybean cultivar "Embrapa 20" are different and this technique may be useful in studies of attraction and penetration of other endoparasitic nematodes.
