Resumo Introdução: A trombocitemia essencial (TE) é uma condição pouco comum e assintomática na maioria dos casos. No entanto, podem surgir sintomas como cefaleias, síncope, vertigem, dor torácica, eritromelalgia e distúrbios visuais transitórios. Os eventos hemorrágicos e tromboembólicos são os grandes riscos desta patologia. O objetivo da partilha deste caso clínico é alertar para uma patologia rara que pode passar despercebida e causar eventos potencialmente graves. Descrição do caso: Sexo feminino, 43 anos, raça caucasiana, antecedentes de hepatite B crónica, sem antecedentes cirúrgicos nem medicação habitual, incluindo método contracetivo hormonal. Recorreu a consulta com queixas de enfartamento precoce, dispepsia e epigastralgia com três dias de evolução, negando alterações do trânsito intestinal, características das fezes, náuseas ou vómitos. Exame físico e endoscopia digestiva alta recente sem alterações de relevo. Foi consultado o estudo analítico que a utente realizava regularmente no hospital, verificando-se que o valor do número de plaquetas se encontrava consistentemente a rondar os 500.000/uL desde há três anos. Pediu-se ecografia abdominal que descrevia trombose parcial da veia porta e do seu ramo esquerdo. A doente foi medicada com enoxaparina 60 mg 12h/12 h. Foi realizado angio-TC abdominal, estudo pró-trombótico e estudo genético e iniciou varfarina 5 mg 1x/dia, inicialmente sobreposta com enoxaparina 60 mg 12/12h. O angio-TC viria a confirmar a trombose da veia porta e confirmou-se positiva a mutação JAK2 V617F. Assumido o diagnóstico de TE e iniciado tratamento citorredutor com hidroxiureia 500 mg em dias alternados. Comentário: Este caso chama a atenção pelo excesso de plaquetas secundário à TE, ainda que ligeiramente acima do valor de referência, ter originado uma trombose da veia porta. Pretende-se alertar os médicos de família para a importância de realizar um estudo atento, valorizando pequenas variações sustentadas fora dos intervalos da normalidade e fazer uma referenciação correta e atempada quando necessário.
Abstract Introduction: Essential thrombocythemia (ET) is an asymptomatic rare condition in most cases. Nonetheless, symptoms like headache, syncope, vertigo, thoracic pain, erythromelalgia, and transient visual disturbances may surge. The major concerns of this pathology are hemorrhagic and thromboembolic events. The aim of sharing this case report is to alert for the existence of this rare pathology that can go unnoticed and cause potentially serious events. Case description: Female, 43-year-old, Caucasian. Background of chronic B hepatitis, without surgical background nor usual medication, including the contraceptive pill. She appealed to a medical appointment with an early feeling of fullness when eating, dyspepsia, and epigastric pain in the last three days. She denied changes in intestinal habits, feces characteristics, nausea, or vomiting. Physical examination and recent upper endoscopic study were normal. The hospital medical record was examined, verifying that platelet levels were consistently rounding 500,000/uL for the past three years. An abdominal ultrasonography was requested, and it described a partial portal vein and left branch thrombosis. The patient started enoxaparin 60 mg every 12 hours. It was conducted an abdominal angiotomography, prothrombotic, and genetic study, including the JAK2 V617F mutation. The patient started warfarin 5 mg one time daily, initially overlaid with enoxaparin 60 mg 12h/12h. The abdominal angiotomography would confirm the portal vein thrombosis and the JAK2 V617F mutation study was positive. It was assumed the diagnosis of ET and was initiated cytoreductive therapy with hydroxyurea 500 mg on alternated days. Comment: This case report shows that the excessive platelet count, secondary to ET, even if slightly above the reference value, caused a portal vein thrombosis. We aim to alert family physicians to the importance of doing a careful study, valuing little sustained variations out of the normal intervals, and referring correctly and promptly when necessary.

BACKGROUND Leishmania tarentolae is a non-pathogenic species found in lizards representing an important model for Leishmania biology. However, several aspects of this Sauroleishmania remain unknown to explain its low level of virulence. OBJECTIVES We reported several aspects of L. tarentolae biology including glycoconjugates, proteolytic activities and metabolome composition in comparison to pathogenic species (Leishmania amazonensis, Leishmania braziliensis, Leishmania infantum and Leishmania major). METHODS Parasites were cultured for extraction and purification of lipophosphoglycan (LPG), immunofluorescence probing with anti-gp63 and resistance against complement. Parasite extracts were also tested for proteases activity and metabolome composition. FINDINGS Leishmania tarentolae does not express LPG on its surface. It expresses gp63 at lower levels compared to pathogenic species and, is highly sensitive to complement-mediated lysis. This species also lacks intracellular/extracellular activities of proteolytic enzymes. It has metabolic differences with pathogenic species, exhibiting a lower abundance of metabolites including ABC transporters, biosynthesis of unsaturated fatty acids and steroids, TCA cycle, glycine/serine/threonine metabolism, glyoxylate/dicarboxylate metabolism and pentose-phosphate pathways. MAIN CONCLUSIONS The non-pathogenic phenotype of L. tarentolae is associated with alterations in several biochemical and molecular features. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT. This research evaluated the effects of diets containing or not containing deactivated soybean sums on productive performance, carcass attributes, carcass ultrasound, and feed cost in confined heifers with 1/2 Nellore-Angus blood. We used blood from 60 1/2 Nellore-Angus heifers aged 2 or 25 years with a body weight of 404.62 kg. A completely randomized design was used, in which the animals were divided into 2 experimental groups: Cont (concentrated for semi feedlot without added deactivated soybean) and Bocchi (concentrated for semi feedlot with the addition of deactivated soybeans). There was no significant difference in the final weight between the experimental groups or for the consumption of concentrate in the evaluation periods. However, heifers supplemented with deactivated soybean in the diet showed greater daily weight gain than did those in the Cont group, as well as greater carcass weight, arrobas gain, and carcass yield. Bocchi concentrate was less expensive than Cont (U$ 7.26 ton-1). The addition of deactivated soybean for finishing cattle is indicated because it provided the animals with higher energy intake and positively influenced the productive performance as a final weight gain greater than that of the feed without adding the deactivated soybean in addition to lower production cost. Abstract: Background: Patients referred for palliative care (PC) need the application of assessment instruments, such as the level of functionality. Objective: Identify the inpatient's sociodemographic and clinical characteristics and characterize their functional capacity. Methodology: A descriptive, longitudinal study of adult cancer patients in a CP unit (n=53) from March to June of 2022. The instruments FIM®, ECOG® and PPS® were applied at admission, on the 14th day of hospitalization and at discharge. Results: The 53 patients evaluated have an average age of 71 years, most of whom are male (64.2%) and have gastric carcinomas (17.0%). On admission, the patients who have reduced FIM values also have reduced PPS and ECOG values. From admission to the 14th day, there is a worsening of functionality. No patient received intervention from the Rehabilitation Nurse. Conclusion: The results demonstrate a decrease in functionality, requiring professionals to continuously monitor and readjust the advanced care plan.
Resumo: Enquadramento: Os doentes referenciados para cuidados paliativos (CP) carecem da aplicação de instrumentos de avaliação, tais como o nível de funcionalidade. Objetivo: Identificar as características sociodemográficas e clínicas do doente internado e caracterizar a sua capacidade funcional. Metodologia: Estudo descritivo, longitudinal de adultos oncológicos internados em CP (n=53) de Março a Junho de 2022. Os instrumentos MIF®, ECOG® e PPS®, foram aplicados na admissão, no 14º dia de internamento e na alta. Resultados: Os 53 doentes avaliados apresentam uma idade média de 71 anos, sendo a maioria masculinos (64,2%) e com carcinomas gástricos (17,0%). Na admissão, os doentes que apresentam valores da MIF reduzidos, apresentam também valores de PPS e ECOG diminutos. Da admissão ao 14º dia, verifica-se um agravamento da funcionalidade. Nenhum doente obteve intervenção por parte do Enfermeiro de Reabilitação. Conclusão: Os resultados demonstram um decréscimo da funcionalidade, exigindo dos profissionais um acompanhamento contínuo e um reajuste do plano avançado de cuidados.

RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. RESUMO Introdução: As fraturas expostas, apesar de pouco comuns, têm custos que superam todos os outros motivos das internações. Sua epidemiologia é de fundamental importância para planejar o tratamento e definir prioridades. Objetivos: Avaliar prospectivamente o perfil epidemiológico das fraturas expostas e o grau de satisfação do atendimento inicial. Métodos: Estudo epidemiológico, prospectivo, descritivo, observacional, em amostra de conveniência das fraturas expostas. Avaliaram-se aspectos epidemiológicos quantitativos e qualitativos das fraturas expostas e o grau de satisfação com o atendimento inicial. Resultados: Foram atendidos 124 pacientes com 155 fraturas expostas. Desses, 88% eram do sexo masculino com média de idade 43 anos (± 42,99); não branco (56,72%); casado (52,41%); com baixo nível de instrução (51,60%); agricultor, autônomo, pedreiro ou industriário (51,60%); com ganho mensal de até dois salários-mínimos (87%); hígidos (76,13%); vítimas de acidentes trabalhistas (39,51%) nos ossos das mãos (58,02%); especialmente do lado esquerdo (55%); atendidos entre quinta-feira e sábado (50%); no período diurno (77%). Esses pacientes mostraram elevado nível de satisfação com o atendimento inicial realizado (98%). Conclusões: As fraturas expostas se relacionaram com homens hígidos, em torno de 43 anos, baixo grau de instrução e baixa renda, nos membros superiores, no período diurno de quinta a sábado. A maioria ficou satisfeita com o atendimento prestado. Nível de Evidência II, Estudo Epidemiológico, Prospectivo, Descritivo e Observacional.
ABSTRACT Introduction: Open fractures, although uncommon, with trauma have costs that exceed all other reasons for hospitalizations. Its epidemiology has fundamental importance to plan treatment and define priorities. Objective: To assess prospectively the epidemiological profile of open fractures and the degree of satisfaction with initial care. Methods: Epidemiological, prospective, descriptive, observational study was carried out in a convenience sample of open fractures. Quantitative, qualitative, and epidemiological aspects regarding open fractures were evaluated, as well as the degree of satisfaction with the initial care. Results: 124 patients treated with 155 open fractures. 88% were male; mean age 43 years (± 42.99); non-white (56.72%); married (52.41%); low level of education (51.60%); farmer, self-employed, bricklayer, industrialist (51.60%); with monthly earnings of up to 2 minimum wages (87%); healthy (76.13%); victims of labor accidents (39.51%) in bones of the hands (58.02%); 55% on the left side; attended between Thursday to Saturday (50%); work shift 6 a.m.-6 p.m. (77%). There was high level of satisfaction with the initial care provided (98%). Conclusion: Open fractures were related to healthy men, 43 years old, low education and low income, predominant in upper limbs, at 6 a.m. to 6 p.m., from Thursday to Saturday. Abstract The use of Echinacea purpurea (EP), a plant native from North America, is widely diffused throughout the world, presenting many pharmacological applications, mainly for the treatment of infections of respiratory and urinary tracts. Due to the widespread commercialization of EP-based products, an effective evaluation of their pharmacological properties is essential to assure efficacy during clinical use. In this study, in vitro tests were performed to evaluate the antimicrobial activity of dried extracts of EP by the microdilution method. In addition, a phagocytosis model was employed to assess the immunomodulatory potential of the extracts. The increase in reactive oxygen species production, as well as the intracellular proliferation rate of Cryptococcus gatti after phagocytosis by macrophages in the presence of EP dried extracts were also evaluated. RESUMO Objetivo: Identificar as estratégias e os efeitos de intervenções conduzidas por agentes comunitários de saúde (ACS) nos níveis de atividade física (AF) em crianças e adolescentes. Fontes de dados: Em agosto de 2020, uma revisão sistemática, delineada de acordo com os itens da lista de verificação PRISMA, foi desenvolvida por meio de buscas em sete bases de dados eletrônicas e em listas de referências. Foram procurados estudos originais, sem restrição quanto ao ano de sua publicação, escritos em espanhol, inglês e português, que desenvolveram intervenções implementadas por ACS envolvendo conteúdos teóricos e/ou práticas de AF com foco em crianças e/ou adolescentes na faixa entre os 3 e os 19 anos de idade. Síntese dos dados: Dos 2.321 estudos inicialmente avaliados, foram incluídos oito, direcionados a amostras com características mais específicas (clínicas, étnicas e/ou socioeconômicas). Em todos os estudos, ACS receberam treinamento prévio para a condução de atividades educativas. Em três ensaios não controlados foram observados resultados positivos envolvendo indicadores de AF moderadas, vigorosas e redução da inatividade física. Complementarmente, dois resultados positivos foram encontrados na redução do comportamento sedentário. Conclusões: Mesmo que a maior parte das intervenções incluídas não tenha apresentado efeitos significativos no aumento dos níveis de AF, a evidência disponível reforça o papel dos ACS como uma importante estratégia de diálogo entre os serviços de saúde e as comunidades mais vulneráveis e sugere maior articulação desses profissionais nas ações desenvolvidas no contexto escolar.
ABSTRACT Objective: To identify the strategies and effects of interventions carried out by community health workers (CHW) on physical activity (PA) levels in children and adolescents. Data source: In August 2020, a systematic review, designed according to the PRISMA checklist items, was conducted by searches in seven electronic databases and in reference lists. Original studies were searched without restriction with regard to year of publication; they were written in Spanish, English or Portuguese and examined interventions implemented by CHW, involving theoretical and/or practical contents of PA, with a focus on children and/or adolescents between three and 19 years of age. Data synthesis: Of the 2,321 studies initially retrieved, eight were included, targeting samples with more specific characteristics (e.g., clinical, ethnic and/or socioeconomic). In all studies, CHW were trained to lead educational activities. In three non-controlled trials, positive results were observed, involving indicators such as moderate and vigorous PA and physical inactivity reduction. Also, two positive results were found in reducing sedentary behavior. Conclusions: Even though most of the interventions included did not have a significant effect on increasing PA levels, the available findings reinforce the role of CHW as an important strategy for dialogue between health services and the most vulnerable communities, and they suggest a greater articulation of these professionals in the actions developed in the school context.

Resumo Introdução: A síndroma de febre de origem indeterminada (FOI) clássica é definida pela presença de temperatura corporal superior a 38,3 °C com duração superior a três semanas, sem diagnóstico definido, apesar de investigação apropriada durante três consultas em ambulatório ou três dias em internamento hospitalar. Implica a realização de uma avaliação criteriosa pelo médico de família, da qual deve resultar uma marcha diagnóstica adequada. Entre as múltiplas etiologias possíveis, a infeção pelo vírus de imunodeficiência humana (VIH) é uma entidade a ter em conta, nomeadamente na presença de fatores de risco para a mesma. Descrição do Caso: Mulher de 47 anos, raça caucasiana, com antecedentes pessoais de neoplasia do colo do útero submetida a conização há quatro anos. É fumadora, sem hábitos alcoólicos ou toxicofílicos conhecidos. Divorciada, integra uma família monoparental desde 2005, sem vida sexual ativa há cerca de oito anos. Recorre a consulta de doença aguda por quadro de mal-estar geral, cansaço, febre de predomínio vespertino e tosse seca com várias semanas de evolução. Após as primeiras etapas de investigação, a etiologia mantinha-se indeterminada, apesar da recorrência dos sintomas. Perante o quadro clínico de síndroma de FOI foi realizada investigação etiológica mais alargada, da qual se destaca serologia positiva para VIH-1. A utente foi referenciada para consulta de infeciologia e medicada com antirretrovirais e terapêutica dirigida a pneumocistose, uma doença definidora de síndroma de imunodeficiência adquirida (SIDA). Este diagnóstico teve um impacto profundo a nível pessoal, familiar e social, com necessidade de seguimento em consulta de psicologia e psiquiatria. Comentário: A investigação etiológica da síndroma de FOI neste caso clínico conduziu a um diagnóstico inesperado de SIDA, o que sensibilizou a equipa clínica para a importância do rastreio atempado de VIH, para a correta abordagem da síndroma de FOI e para a importância do acompanhamento longitudinal pelo médico de família, de acordo com o diagnóstico estabelecido.
Abstract Introduction: Fever of unknown origin (FUO) syndrome is defined as the presence of temperature higher than 38.3 °C lasting more than three weeks, with failure to reach a diagnosis despite a correct investigation in three outpatient visits or three days in the hospital. It implies a criterion evaluation for general practitioners, leading to a proper diagnostic investigation. Between the large spectrum of possible etiologies, the HIV infection is a possibility to consider, especially if there are associated risk factors. Case Description: Female, 47 years old, Caucasian, with a history of neoplasm of the cervix submitted to conization four years ago. She is a smoker. Divorced, she belongs to a mono-parental family since 2005, her last partner was eight years ago. The patient came to an urgent consultation with symptoms of malaise, fatigue, evening fever, and dry cough, for several weeks. After the first steps of investigation, the etiology remained undefined, despite the recurrence of symptoms. Given the clinical condition of FUO syndrome, it was performed a wider etiologic investigation revealed HIV-1 positive serology. The patient was referred to an infectious diseases hospital appointment and started on antiretroviral treatment and therapy directed to pneumocystosis, and acquired immunodeficiency syndrome (AIDS)-defining disease. This diagnosis had a profound influence at a personal, family, and social level with the need for follow-up in psychology and psychiatry consultation. Commentary: The etiologic investigation of FUO syndrome, in this clinical case, has conducted to an unexpected diagnosis of AIDS, which has sensitized the health team to the importance of a timely HIV screening, for the correct approach of the FOI syndrome and for the importance of longitudinal monitoring by the family physician.

Associated factors with physical activity counseling among Brazilian Family Health Strategy workers

Resumo O estudo objetivou determinar a prevalência e os fatores associados ao aconselhamento para atividade física entre trabalhadores da Estratégia de Saúde da Família. Trata-se de estudo transversal com amostra de 591 trabalhadores de Equipes de Saúde da Família de João Pessoa-PB, Brasil. Aconselhamento para atividade física foi definido como qualquer recomendação direcionada a aumentar os níveis de atividade física dos usuários conduzida por pelo menos seis meses. Foram considerados os seguintes fatores: tempo de trabalho, quantidade de atendimentos, educação permanente, barreiras percebidas, autoeficácia, atitude, autoavaliação de saúde, nível de atividade física e estado nutricional. A prevalência de aconselhamento foi de 46,3%, sendo maior entre médicos (74,5%; IC95%: 59,6-85,2) e enfermeiros (60,3%; IC95%: 48,0-71,4) em relação aos agentes comunitários de saúde (42,9%; IC95%: 38,2-47,7) e técnicos de enfermagem (31,5%; IC95%: 20,2-45,4). Profissionais com autoavaliação positiva de saúde, sem percepção de barreiras, com atitude positiva e alta autoeficácia, apresentaram maior chance de realizar aconselhamento. Conhecimentos e ações sobre os fatores associados ao aconselhamento podem ajudar a ampliar o envolvimento dos profissionais em iniciativas de educação em saúde.
Abstract This study aimed to determine the prevalence and associated factors with physical activity counseling among Brazilian Family Health Strategy workers. This is a cross-sectional study conducted with a random sample of 591 health workers who work in the Family Health Teams of João Pessoa-PB, Brazil. Counseling for physical activity was defined as any advising targeted for increasing patients’ physical activity levels conducted for at least six months. The following factors were considered: time working in health care units, amount of daily attendance, continuing education, perceived barriers, self-efficacy, attitude, self-rated health, physical activity level and nutritional status. Prevalence of counseling was 46.3%, being higher among physicians (74.5%; 95%CI: 59.6-85.2) and nurses (60.3%; 95%CI: 48.0-71.4) compared to community health workers (42.9%; 95%CI: 38.2-47.7) and nurses assistants (31.5%; 95%CI: 20.2-45.4). The results showed health professionals with positive self-rated health, without perception of barriers, having a positive attitude and high self-efficacy were more likely to perform physical activity counseling. Knowledge and actions on factors associated with physical activity counseling can help broaden the involvement of primary health care providers in health education.

ABSTRACT Objectives: To assess the efficacy of hydroxychloroquine in combination with azithromycin in terms of clinical and biochemical outcomes in adult patients with COVID-19 hospitalized for acute respiratory distress syndrome (ARDS), and to describe the occurrence of adverse events. Method: Retrospective comparative study, based in a quaternary private hospital in Rio de Janeiro, Brazil, involving 193 adult patients hospitalized for mild and moderate COVID-19 related ARSD, analyzing treatment efficacy based on clinical and biochemical outcomes. Results: The active group comprised 101 (52.3%) patients using hydroxychloroquine associated with azithromycin and the control group 92 (47.7%) patients who did not take these medications. Median age was 59 (47–70) in the active group and 65 (47−77) in the control group (p < 0.05). Patients in the control group had greater extent of pulmonary involvement on baseline chest CT scans (p < 0.05). All other baseline variables (BMI, comorbidities, previous use of medications and biochemical assessments) were similar between groups. In the medication group, 25% (25 out of 101) were admitted to the ICU, compared to 21% (19 out of 92) in the control group (p > 0.05). No difference in mortality, duration of non-invasive oxygen use or duration of hospitalization was seen between groups. The therapeutic regimen was well tolerated, with only eight (7.9%) patients presenting gastrointestinal symptoms and eight (7.9%) patients withdrawn treatment due to QTc prolongation. Conclusions: Patients treated with hydroxychloroquine combined with azithromycin and the control group had similar clinical outcomes. This therapeutic regimen was considered ineffective in hospitalized patients with mild to moderate COVID-19 related ARDS and was associated with few non-severe adverse events.

Abstract Potent signaling agents stimulate and guide pulp tissue regeneration, especially in endodontic treatment of teeth with incomplete root formation. Objective This study evaluated the bioactive properties of low concentrations of extracellular matrix proteins on human apical papilla cells (hAPCs). Methodology Different concentrations (1, 5, and 10 µg/mL) of fibronectin (FN), laminin (LM), and type I collagen (COL) were applied to the bottom of non-treated wells of sterilized 96-well plates. Non-treated and pre-treated wells were used as negative (NC) and positive (PC) controls. After seeding the hAPCs (5×103 cells/well) on the different substrates, we assessed the following parameters: adhesion, proliferation, spreading, total collagen/type I collagen synthesis and gene expression (ITGA5, ITGAV, COL1A1, COL3A1) (ANOVA/Tukey; α=0.05). Results We observed greater attachment potential for cells on the FN substrate, with the effect depending on concentration. Concentrations of 5 and 10 µg/mL of FN yielded the highest cell proliferation, spreading and collagen synthesis values with 10 µg/mL concentration increasing the ITGA5, ITGAV, and COL1A1 expression compared with PC. LM (5 and 10 µg/mL) showed higher bioactivity values than NC, but those were lower than PC, and COL showed no bioactivity at all. Conclusion We conclude that FN at 10 µg/mL concentration exerted the most intense bioactive effects on hAPCs.

Resumo O objetivo deste estudo foi investigar o nível de exposição à infecção por Leishmania em cães errantes de uma área de transmissão intensa de leishmaniose visceral no estado de Pernambuco. Amostras de sangue de 178 cães foram avaliadas por testes sorológicos e moleculares: teste imunocromatográfico (IC), ensaio imunoenzimático (ELISA), teste de imunofluorescência de anticorpos (RIFI), reação em cadeia da polimerase convencional e quantitativa (cPCR e qPCR). Os valores de positividade obtidos com os testes sorológicos foram 71,4% (127/178), 70,2% (125/178) e 50,6% (90/178) com IC, ELISA e RIFI, respectivamente. E com 38,8% (69/178) dos cães mostraram-se simultaneamente positivos aos três testes. Os valores de positividade obtidos por cPCR e qPCR foram 20,2% (36/178) e 38,8% (69/178), respectivamente. E com 11,8% (21/178) dos cães foram positivos em ambos os testes moleculares. No geral, 87,1% (155/178) foram positivos para anticorpos anti-Leishmania spp. e/ou material genético de Leishmania spp. A positividade a um ou mais testes foi estatisticamente associada à linfonodomegalia, lesões cutâneas, linfocitose, anemia e hiperproteinemia. Os resultados do estudo revelam um alto nível de exposição à Leishmania spp. em cães errantes em uma área de transmissão intensa de leishmaniose visceral humana, sugerindo que eles podem desempenhar um papel como reservatórios no ciclo de transmissão dessa zoonose.
Abstract The aim of this study was to investigate the level of exposure to Leishmania infection in stray dogs in an area of intense visceral leishmaniasis transmission in the state of Pernambuco, Brazil. Blood samples from 178 dogs were analyzed using serological and molecular assays: rapid immunochromatographic test (ICT), enzyme-linked immunosorbent assay (ELISA), immunofluorescence antibody test (IFAT), and conventional and quantitative polymerase chain reaction (cPCR and qPCR). Positivity values obtained with serological tests were 71.4% (127/178), 70.2% (125/178), and 50.6% (90/178) using ICT, ELISA, and IFAT, respectively, with 38.8% (69/178) of the dogs were simultaneously positive for all three tests. The positivity values obtained with cPCR and qPCR were 20.2% (36/178) and 38.8% (69/178), respectively, with 11.8% (21/178) testing positive in both molecular assays. Overall, 87.1% (155/178) were positive for anti-Leishmania spp. antibodies and/or Leishmania spp. DNA. Positivity to one or more tests was statistically associated with lymphadenomegaly, skin lesions, lymphocytosis, anemia and hyperproteinemia. The results of this study revealed a high level of exposure to Leishmania in stray dogs in an area of intense human visceral leishmaniasis transmission, suggesting that dogs play a role as reservoirs in the transmission cycle of this zoonosis.

BACKGROUND During routine Coronavirus disease 2019 (COVID-19) diagnosis, an unusually high viral load was detected by reverse transcription real-time polymerase chain reaction (RT-qPCR) in a nasopharyngeal swab sample collected from a patient with respiratory and neurological symptoms who rapidly succumbed to the disease. Therefore we sought to characterise the infection. OBJECTIVES We aimed to determine and characterise the etiological agent responsible for the poor outcome. METHODS Classical virological methods, such as plaque assay and plaque reduction neutralisation test combined with amplicon-based sequencing, as well as a viral metagenomic approach, were performed to characterise the etiological agents of the infection. FINDINGS Plaque assay revealed two distinct plaque phenotypes, suggesting either the presence of two severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) strains or a productive coinfection of two different species of virus. Amplicon-based sequencing did not support the presence of any SARS-CoV-2 genetic variants that would explain the high viral load and suggested the presence of a single SARS-CoV-2 strain. Nonetheless, the viral metagenomic analysis revealed that Coronaviridae and Herpesviridae were the predominant virus families within the sample. This finding was confirmed by a plaque reduction neutralisation test and PCR. MAIN CONCLUSIONS We characterised a productive coinfection of SARS-CoV-2 and Herpes simplex virus 1 (HSV-1) in a patient with severe symptoms that succumbed to the disease. Although we cannot establish the causal relationship between the coinfection and the severity of the clinical case, this work serves as a warning for future studies focused on the interplay between SARS-CoV-2 and HSV-1 coinfection and COVID-19 severity.
