Implications of clinical simulation in motivation for learning: scoping review learning

ABSTRACT Objective: To identify, synthesize, and analyze the scientific knowledge produced regarding the implications of using clinical simulation for undergraduate nursing or medical students' motivation for learning. Methods: The search for articles was conducted between July 28 and August 3, 2022, on the PubMed/MEDLINE, Scopus, Web of Science, and SciELO databases. The following was used for the search: P - undergraduate students attending Nursing or Medicine courses; C - motivation for learning, and C - skills and clinical simulation laboratory. The following research question guided the study: “What are the implications of clinical simulation on the motivation for learning of undergraduate students of nursing and medicine?” Of the 1,783 articles found, 13 were included in the sample for analysis. All stages of the selection process were carried out by two independent evaluators. The results were presented as charts and a discursive report. Results: The studies analyzed indicated the beneficial effects of clinical simulation on students' motivation, in addition to other gains such as competencies, technical and non-technical skills, knowledge, belonging, autonomy, clinical judgment, critical and reflective thinking, self-efficacy and decreased anxiety, self-management, and improvements in learning and learning climate. Conclusion: Clinical simulation provides a positive learning environment favorable to the development of technical and interpersonal skills and competencies, and raising the level of motivational qualities. ABSTRACT In 2000, Cleide Costa published a paper presenting the state of knowledge of the Neotropical Coleopte ra, with a focus on the Brazilian fauna. Twenty-four years later, thanks to the development of the Coleoptera section of the Taxonomic Catalog of the Brazilian Fauna (CTFB - Catálogo Taxonômico da Fauna do Brasil) through the collaboration of 100 coleopterists from all over the globe, we can build on Costa’s work and present an updated overview of the state of knowledge of the beetles from Brazil. There are currently 35,699 species in 4,958 genera and 116 families known to occur in the country, including representatives of all extant suborders and superfamilies. Our data show that the Brazilian beetle fauna is the richest on the planet, concentrating 9% of the world species diversity, with some estimates accounting to up to 15% of the global total. The most diverse family in numbers of genera is Cerambycidae (1,056 genera), while in number of species it is Chrysomelidae (6,079 species). Conotrachelus Dejean, 1835 (Curculionidae) is the most species-rich genus, with 570 species. The French entomologist Maurice Pic is the author who has contributed the most to the naming of species recorded from Brazil, with 1,794 valid names in 36 families, whereas the Brazilians Ubirajara R. Martins and Maria Helena M. Galileo are the only ones among the top-ten authors to have named species in the 21st century. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Introduction : Cerebral venous sinus thrombosis (CVST) is a well-known, although underestimated, cause of stroke in childhood. Its diagnosis requires a high index of suspicion, a correct interpretation of neuroim aging studies and an interrelation between clinicians and radiologists. The clinical features, risk factors and neuroimaging of children under 15 years of age with CVST were analyzed. Methods : multicenter, retrospective, descriptive, study of a consecutive series of cases of children under 15 years of age, who were admitted due to CVST between January 1st, 2010, and March 1st, 2022. Results : The study included 51 patients: 39% with acute symptoms and 59% with subacute symptoms. Newborns predominantly presented encephalopathic symptoms and seizures, while children exhibited signs of intracranial hypertension (ICH). Risk factors were 96 identified in 90% of the cases. Magnetic resonance with angiography in venous time confirmed the diagnosis in 80% of the patients, with the straight sinus being the most affected in newborns and the lateral sinus in chil dren. Hemorrhagic complications were found in 30.5%, being more frequent in newborns. Anticoagulation was initiated in 82% without complications. Sequelae were present in 44.4% of newborns and 37.9% of children, being more frequent and severe in newborns. Conclusions : To make an early diagnosis, it is essen tial to consider CVST in newborns with encephalopathic symptoms and/or seizures, and in children with signs of ICH in the presence of predisposing or triggering conditions.
Resumen Introducción : La trombosis de venas y senos venosos cerebrales (TVSC) constituye una causa conocida, aun que subestimada de ictus en la infancia. Su diagnóstico requiere un alto índice de sospecha, una correcta in terpretación de la neuroimagen e interrelación entre el clínico y el radiólogo. Objetivo : Analizar las manifestaciones clínicas, fac tores de riesgo y neuroimagen de recién nacidos (RN) y niños menores de 15 años con TVSC. Métodos: Estudio descriptivo, retrospectivo, multi céntrico, de una serie consecutiva de casos de menores de 15 años que ingresaron por TVSC entre el 1 de enero del 2010 y el 1 de marzo de 2022. Resultados : El estudio incluyó 51 pacientes: 39% con síntomas agudos y 59% subagudos. En los RN predomi naron síntomas encefalopáticos y convulsiones, mien tras en los niños elementos de hipertensión endocranea na (HTEC). Se identificaron factores de riesgo en el 90% de los casos. La resonancia magnética con angiografía en tiempo venoso confirmó el diagnóstico en el 80%, siendo el seno recto el más afectado en RN y el seno lateral en niños. Se encontraron complicaciones hemorrágicas en 30.5%, siendo más frecuentes en los RN. Se inició anti coagulación en el 82% sin complicaciones. Las secuelas estuvieron presentes en 44.4% de RN y 37.9% de niños, siendo más frecuentes y graves en los RN. Conclusiones : Para realizar un diagnóstico precoz es fundamental pensar en TVSC en RN con síntomas en cefalopáticos y/o convulsiones y en mayores con clínica de HTEC en presencia de enfermedades predisponentes o desencadenantes.

Abstract Bartonella are rodent-borne bacteria that cause varied human etiologies. Studies on synanthropic rodents are rare, causing gaps in epidemiological knowledge. We tested bloodclot samples from 79 rats from an urban slum in Salvador, Brazil through PCR targeting gltA gene. Nine samples (11.4%) were positive: six had 100% identity with Bartonella sp. isolate JF429580 and 99.5% with B. queenslandensis strain AUST/NH8; three were 100% identical to isolate JF429532 and 99.7% to B. tribocorum. This is the second report on urban rat Bartonella indicating bacterial circulation at detectable rates. Uma revisão sistemática e metanálise referente ao período de 1930 a 2017, realizada por meio de busca em seis plataformas: PubMed, Web of Science, Scopus, Lilacs, Embase e Cochrane. Resultados. A pesquisa encontrou 77 publicações, das quais 53 (68%) eram do período de 2000 a 2017. Dos 77 estudos, 62 foram incluídos na análise, da América do Norte (11, equivalente a 17%), América Central (9, equivalente a 14%) e América do Sul (42, equivalente a 67%), e 22 estudos foram realizados em áreas urbanas. A prevalência da leptospirose nos 62 estudos analisados correspondeu a 28% (IC 95% [23, 32]). Os países com maior prevalência foram os Estados Unidos da América (41%), a Colômbia (29%) e o Brasil (21%). Os sorovares mais frequentes encontrados foram Icterohaemorrhagiae (43 de 77 publicações, equivalente a 55%), Canicola (35, equivalente a 45%), Pomona (28, equivalente a 36%) e Grippotyphosa (26, equivalente a 33%). Conclusões. Há variabilidade nas espécies e sorovares de Leptospira, que têm distribuição heterogênea nas Américas e alta prevalência em alguns países, o que destaca a necessidade de ações para controlar a doença.

En la búsqueda se encontraron 77 publicaciones, de las que 53 (68%) eran del periodo 2000-2017. En el análisis se incluyeron 62 de los 77 estudios, correspondientes a América del Norte (11, 17%), Centroamérica (9, 14%) y América del Sur (42, 67%), y 22 estudios correspondientes a zonas urbanas. La prevalencia de la leptospirosis en los 62 estudios analizados fue del 28% (IC del 95% [23, 32]). Los países con mayor prevalencia fueron Estados Unidos de América (41%), Colombia (29%) y Brasil (21%). Las serovariedades más frecuentes fueron icterohaemorrhagiae (43 de 77 publicaciones, 55%), canicola (35, 45%), pomona (28, 36%) y grippotyphosa (26, 33%). Conclusiones. Se observa variabilidad de especies y serovariedades de Leptospira, con una distribución heterogénea en las Américas y una elevada prevalencia en algunos países, lo que pone de manifiesto la necesidad de adoptar medidas para controlar la enfermedad.

The most frequent serovars found were Icterohaemorrhagiae (43 of 77 publications, 55%), Canicola (35, 45%), Pomona (28, 36%), and Grippotyphosa (26, 33%). Conclusions. There is variability of Leptospira species and serovars with heterogenous distribution throughout the Americas, with high prevalence in some countries, highlighting the need for action to control the disease.

Abstract Direct oral anticoagulants have emerged as the drugs that have changed the man agement of the antithrombotic treatment in the last 15 years. Their advantages, like a more friendly way of anticoagulation and their lower risk of bleeding, especially in the brain, have positioned these new anticoagu lants as the first drug of choice in the two most frequent indications of anticoagulation, atrial fibrillation, and the venous thromboembolic disease. However, not all the patients can receive these agents, not all the direct oral anticoagulants have the same characteristics, and most importantly, not all the diseases with an indication of an anticoagulant drug can be treated with them. Therefore, it is mandatory that all the faculties involved in the management of these drugs must know them in depth, to decide the best treatment for the patient. This position paper, from a group of experts in anticoagulation in Argentina, can help the general practitioner in the daily use of direct oral anticoagulants based on the new evidence and the experience of a wide group of professionals. The way we relate to the anticoagulant treatment has changed in the last years. The doctors who work with them must also do so.
Resumen Los anticoagulantes orales directos han surgido como una de las herramientas que ha cambiado el manejo de la enfermedad trombótica en los últimos 15 años. Sus ventajas, desde el punto de vista de la facilidad de uso y menor riesgo de sangrado, especialmente de sangrado cerebral, han posicionado a estos nuevos anticoagulantes como la primera alternativa de tratamiento en las dos indicaciones más frecuentes en que necesitamos estas drogas, la fibrilación auricular y la enfermedad tromboembólica venosa. Sin embargo, no todos los pacientes pueden recibir estos agentes, no todos los anticoagulantes directos tienen las mismas pro piedades y fundamentalmente, no todas las enfermedades con indicación de un anticoagulante pueden tratarse con ellos;con lo cual es necesario que todos los profesionales que están involucrados en el manejo de estos medicamentos estén obligados a conocerlos en profundidad, para poder decidir el mejor tratamiento en cada caso particular. Este documento de posición de expertos de diferentes especialidades de Argentina, presenta lineamientos para el uso correcto de los anticoagulantes directos en base a nueva evidencia y a la experiencia de uso de un amplio grupo de profesionales. La forma de relacionarnos con el tratamiento anticoagulante ha cambiado. Los médicos que trabajamos con ellos también debemos hacerlo.

A Systematic Review of the geographic distribution of pathogenic Leptospira serovars in the Americas, 1930-2017

Abstract Leptospirosis is an important public health problem caused by Leptospira. The objective is to characterize the geographic distribution of pathogenic leptospira serovars in the Americas through a systematic review of the literature between 1930-2017. Searches were conducted in six scientific databases (PubMed, Web of Science, Embase, Lilacs, Scopus and Cochrane). We included studies conducted unambiguously in the Americas, that investigated infection of Leptospira in humans and animals in their natural environments with serovar identification. 283 articles were included, of which 69 were studies in humans, 86 in wild animals, and 182 in domestic animals. Most of them conducted in Brazil (104, 36.7%) and in rural environments (158, 55.8%). Bovines, equines and dogs where the most frequently studied domestic species. However, a large diversity including 80 species of wild animals were studied. Icterohaemorrhgiae, Canicola, Pomona and Grippotyphosa were the most common serovars, described in 46 (16.2%), 38 (13.3%), 32 (11.3%) and 26 (9%) of the articles, respectively. The Results indicate a large concentration of studies in Latin America, with emphasis on Brazil, in wild mammals and three main domestic animal groups. Our results emphasize the need for studies that delve into the relationships of the epidemiological cycle, environment, and health.

RESUMO: A hipomagnesemia é uma das principais causas de morte em vacas de corte em pastagem em países do Hemisfério Sul, como Argentina, Austrália e Nova Zelândia. Aqui, revisamos a literatura sobre hipomagnesemia em bovinos de corte e descrevemos um surto no Uruguai. A doença ocorreu no final do outono, afetando vacas Aberdeen Angus e Aberdeen Angus x Hereford de 6-11 anos de idade em boas condições corporais, lactantes, multíparas e pastando em campos naturais que foram melhorados com fertilização e plantio de azevém e aveia. Aproximadamente 40 de 225 vacas foram afetadas e 24 (10,7%) morreram. O diagnóstico presuntivo de hipomagnesemia foi estabelecido com base na morte súbita, sinais clínicos neuromusculares agudos, dados epidemiológicos e a resposta à administração de Mg. O diagnóstico foi confirmado pela detecção de baixos níveis de Mg no soro (0,47-0,57mmol/L), humor vítreo (0,47-0,80mmol/L), humor aquoso (0,66mmol/L) e líquido cefalorraquidiano (0,59mmol/L). O componente mais importante da dieta correspondia a pastagens de azevém e aveia de rápido crescimento, as quais apresentaram altas concentrações de K (3,48%), N (4,38%) e P (0,94%), que sugerem hipomagnesemia secundária. Além disso, a relação K/(Ca+Mg) foi de 2,38 em forragens de azevém e aveia (valor de referência: 2,2) e 0,15 no solo (valor de referência: 0,09), que representam fatores de risco para hipomagnesemia. Em conclusão, a hipomagnesemia é uma causa importante de mortalidade em gado de corte em pastagens naturais melhoradas no Uruguai e pode ser facilmente prevenida pela correta suplementação sazonal de Mg.
ABSTRACT: Hypomagnesemia is a major cause of death in grazing beef cows in countries of the Southern Hemisphere such as Argentina, Australia and New Zealand. Here we review the literature on hypomagnesemia in beef cattle and describe an outbreak in Uruguay. The disease occurred in late autumn, affecting 6 to 11-year-old Aberdeen Angus and Hereford x Aberdeen Angus lactating multiparous cows in good body condition, grazing on natural grasslands that had been improved by fertilization and sowing of seeds of ryegrass and oat. Approximately 40 out of 225 cows were affected and 24 (10.7%) died. A presumptive diagnosis of hypomagnesemia was established based on sudden death, acute neuromuscular clinical signs, epidemiological data, and the response to Mg administration. The diagnosis was confirmed by detecting low Mg concentrations in serum (0.47-0.57mmol/L), vitreous humor (0.47-0.80mmol/L), aqueous humor (0.66mmol/L) and cerebrospinal fluid (0.59mmol/L). The largest component of the diet corresponding to fast-growing and exuberant forages of ryegrass and oat had high concentrations of K (3.48%), N (4.38%) and P (0.94%), suggesting secondary hypomagnesemia. In addition, the K/(Ca+Mg) ratio was 2.38 in forages of ryegrass and oat (reference value: 2.2), and 0.15 in the soil (reference value: 0.09), which represent risk factors for hypomagnesemia. In conclusion, hypomagnesemia is an important cause of mortality in beef cattle grazing improved natural grasslands in Uruguay and it can be easily prevented by correct seasonal Mg supplementation.

A systematic literature review of leptospirosis outbreaks worldwide, 1970–2012

ABSTRACT Objective. This review describes the geographic and temporal distribution of, detection methods for, and other epidemiological features of published leptospirosis outbreaks, with the aim of informing efforts to standardize outbreak-reporting practices. Methods. We conducted a systematic review of leptospirosis outbreaks reported in the scientific literature and ProMED during 1970–2012. Predefined criteria were used to identify and classify outbreaks and a standard form was used to extract information. Results. During 1970–2012, we identified 318 outbreaks (average: 7 outbreaks/year; range: 1–19). Most outbreaks were reported in the Latin America and the Caribbean region (36%), followed by Southern Asia (13%), and North America (11%). Most outbreaks were located in tropical and subtropical ecoregions (55%). Quality classification showed that there was clear description of laboratory-confirmed cases in 40% of outbreaks. Among those, the average outbreak size was 82 cases overall (range: 2–2 259) but reached 253 cases in tropical/subtropical ecoregions. Common risk factors included outdoor work activities (25%), exposure to floodwaters (23%), and recreational exposure to water (22%). Epidemiologic investigation was conducted in 80% of outbreaks, mainly as case interviews. Case fatality was 5% overall (range: 0%–60%). Conclusions. Outbreak reporting increased over the study period with outbreaks covering tropical and non-tropical regions. Outbreaks varied by size, setting, and risk factors; however, data reviewed often had limited information regarding diagnosis and epidemiology. Guidelines are recommended to develop standardized procedures for diagnostic and epidemiological investigations during an outbreak and for reporting.
RESUMEN Objetivo. Describir la distribución geográfica y temporal, los métodos de detección y otras características epidemiológicas de los brotes de leptospirosis publicados con el fin de fundamentar los esfuerzos tendientes a estandarizar las prácticas empleadas en la notificación de brotes. Métodos. Se llevó a cabo una revisión sistemática de los brotes de leptospirosis notificados en la bibliografía científica y en ProMED entre 1970 y 2012. Se utilizaron criterios predefinidos para identificar y clasificar los brotes y se empleó un formulario estándar para extraer la información. Resultados. Entre 1970 y 2012 se identificaron 318 brotes (promedio: 7 brotes/año; rango: 1-19), la mayoría de ellos en América Latina y el Caribe (36%), región seguida por Asia meridional (13%) y América del Norte (11%). La mayoría de los brotes se localizaron en ecorregiones tropicales y subtropicales (55%). La clasificación cualitativa reveló que en el 40% de los brotes había una clara descripción de los casos confirmados por laboratorio. Entre ellos, el tamaño promedio del brote fue de 82 casos (rango: 2-2259 casos) pero alcanzó los 253 casos en ecorregiones tropicales o subtropicales. Entre los factores de riesgo frecuentes figuraban las actividades laborales al aire libre (25%), la exposición a agua proveniente de inundaciones (23%) y la exposición a agua con fines recreativos (22%). En el 80% de los brotes se realizaron investigaciones epidemiológicas, principalmente entrevistas de casos. La mortalidad específica de los casos fue del 5% (rango: 0%-60%). Conclusiones. La notificación de brotes aumentó durante el período de estudio, y los brotes abarcaron regiones tropicales y no tropicales. Los brotes fueron diferentes en cuanto a su tamaño, el entorno y los factores de riesgo; sin embargo, los datos examinados con frecuencia incluían una información limitada respecto del diagnóstico y la epidemiología. Se recomiendan directrices para elaborar procedimientos estandarizados para las investigaciones diagnósticas y epidemiológicas durante un brote y para su notificación.

RESUMO: Descreve-se um surto de hipomagnesemia em bovinos leiteiros da raça Holandês com alimentação a base de pastagens de aveia (Avena sativa) em crescimento no Uruguai, nos meses de maio a julho (outono-inverno) de 2017. De um rebanho de 270 vacas em ordenha, nove (3,3%) morreram. As nove vacas tinham entre dois e nove anos, estavam entre a primeira e a sexta lactação e a produção diária era de 15,8 a 31,4 litros de leite. Duas vacas que apresentaram sialorreia, espasmos musculares, decúbito lateral, debilidade, opistótono e posteriormente, estado comatoso foram eutanasiadas e necropsiadas. Não foram observadas lesões macroscópicas ou histológicas significantes. Uma vaca com sinais clínicos, não tratada, e oito de 14 vacas sem sinais clínicos, do mesmo rebanho, apresentaram baixos níveis séricos de Mg (menos de 0,7mmol/L). Adicionalmente, as duas vacas com sinais clínicos e seis das 14 vacas sem sinais clínicos apresentaram baixos níveis de Ca sérico. A relação do risco tetanizante K/(Ca+Mg) de duas pastagens de aveia, silagem de milho e ração concentrada foi estimada em 5; 10; 7,73; 2,15 e 0,85, respectivamente. Uma relação K/(Ca+Mg) maior de 2,2 é um indicador do potencial tetanizante da forragem, A diferença entre o aporte e os requerimentos dos minerais foi estabelecida constatando-se deficiência diária de Mg (-0,36g/dia), Na (-25,2g/dia) e Ca (-9,27g/dia). O potássio (K) (184,42g/dia) e o fósforo (P) (12,81g/dia) estavam em excesso. A dieta foi reformulada para corrigir as deficiências e a doença foi controlada mediante a administração diária de 80g de MgO, 80g de CaCO3 e 30g de NaCl por vaca. Conclui-se que a hipomagnesemia é uma doença de importância crescente em gado leiteiro no Uruguai e que deve ser prevenida mediante a formulação correta da dieta.
ABSTRACT: An outbreak of hypomagnesemia is reported in Holstein dairy cattle grazing lush oat (Avena sativa) pasture in Uruguay. Nine of 270 (3.3%) cows died in May-July (autumn-winter) 2017. These nine cows were from 2 to 9-years-old (1st-6th lactation), with 22 to 194 days of lactation and 15.8 to 31.4L of daily milk production. Two cows with acute sialorrhea, muscle spasms, lateral recumbency, weakness, opisthotonos, and coma, were euthanized and necropsied. No significant macroscopic or histological lesions were found. One untreated clinically-affected cow and eight out of 14 clinically healthy cows of the same group under similar management and production conditions had low serum levels of Mg (lower than 0.7mmol/L). Secondarily, both clinically affected cows and six out of 14 healthy cows had low serum Ca levels. The K/(Ca+Mg) ratio of two oat forages, corn silage, and ration was 5.10, 7.73, 2.45, and 0.85, respectively. A K/(Ca+Mg) ratio lower than 2.2 represents a risk for hypomagnesemia. The difference between the contribution-requirement of minerals in the diet was established and a daily deficiency of Mg (-0.36g/day), Na (-25.2g/day) and Ca (-9.27g/day) was found, while K (184.42g/day) and P (12.81g/day) were in excess. The diet was reformulated to correct the deficiencies and the disease was controlled by the daily administration of 80g of magnesium oxide, 80g of calcium carbonate and 30g sodium chloride per cow. It is concluded that hypomagnesemia is a cause of mortality in dairy cattle in Uruguay, and that the condition can be prevented by appropriate diet formulation.

RESUMO: Descreveram-se carcinomas de células escamosas (CCE) nas orelhas, pálpebras e olhos em cinco ovinos da raça Frisona Milchschaf em dois rebanhos de criação extensiva no Uruguai. A prevalência nos rebanhos foi de 2,12% e 2,53%. Macroscopicamente se observaram lesões focais ou multifocais, com aumento de volume, nodulares, verrucosas ou em placas, com ocasionais ulcerações e hemorragias. Apresentavam aspecto verrucoso, por vezes com superfície ulcerada e firmes ao corte. De acordo com as características histológicas da neoplasia, as lesões foram classificadas em CCEs bem diferenciados em três casos e moderadamente diferenciados em dois casos. Esta raça é nativa da Alemanha, caracterizada pela face deslanada e pele branca despigmentada. Na Alemanha, há menores níveis de radiação solar do que no Uruguai e seu sistema de produção é mais intensivo, com permanência dos animais confinados durante longos períodos. O aumento do tempo de exposição a níveis mais elevados de radiação solar pode ser um fator predisponente para o desenvolvimento de CCEs nesta raça em sistemas extensivos de produção no Uruguai. Neste País há mais radiação solar e os sistemas produtivos fornecem menos proteção contra a luz solar, em relação ao sistema de produção alemão. A alta frequência de CEE deveria ser levada em consideração para quem decide introduzir no Uruguai e outros países da região. raças europeias com a face despigmentada.
ABSTRACT: This study described five cases of squamous cell carcinoma (SCC) in the pinnae, eyelids, and eyes of Friesian Milchschaf sheep from two flocks raised under extensive conditions in Uruguay. The prevalence values for both flocks were 2.12% and 2.53%, respectively. Macroscopically, single or multiple, nodular, verrucous, or plaque-like neoplasms, with occasional ulceration and bleeding, were observed in the affected areas. Histologically, well- and moderately-differentiated SCCs were diagnosed in three and two cases, respectively. The Milchschaf breed is native of Germany, and its individuals present white non-pigmented skin with the face and legs clear of fleece, thus adapted to areas with lower solar radiation. Additionally, in the German breeding systems, animals remain indoors for long periods. The increased exposure time to higher levels of solar radiation of the Uruguayan extensive production systems may be a predisposing factor for the development of SCC in this sheep breed. In Uruguay, there is more solar radiation and the production systems provide less protection against sunlight compared with the German production system. The high frequency of SCC should be considered prior to introducing European breeds with non-pigmented skin in Uruguay and other countries of the region.

Caída en la producción láctea, signos respiratorios agudos y muertes asociadas a estrés calórico en bovinos lecheros de Uruguay

Resumen En marzo de 2017 se registró un episodio clínico de estrés calórico en un tambo de vacas Holando en el suroeste de Uruguay. Tras la ingesta de la ración matutina, 6 de 189 vacas lactantes manifestaron disnea, ortopnea y respiración con la boca abierta. Cinco vacas se recuperaron, una murió durante el episodio y otra había muerto la noche anterior. No se encontraron lesiones en el tracto respiratorio. Los índices de temperatura y humedad (ITH) ambiental máximos fueron 83,5; 83,5 y 83,0, los días 27 y 28 de febrero y 1 de marzo, indicando una ola de calor, con estrés calórico moderado. No se detectaron ergoalcaloides en la ración. Los signos de comienzo abrupto, la ausencia de lesiones en el tracto respiratorio y los ITH moderadamente elevados por tiempo prolongado, indicaron un episodio agudo de estrés calórico con desenlace fatal. En el análisis retrospectivo se observó que el episodio clínico estuvo precedido de una reducción de la producción láctea desde el 15 de febrero al 2 de marzo, seguido de una recuperación parcial de la producción entre el 3 y el 15 de marzo, en asociación con una reducción del ITH. Este reporte marca la necesidad de implementar medidas adecuadas para prevenir y controlar el estrés calórico durante el verano en tambos de Uruguay.
Summary On March 2017, a clinical episode of heat stress in Holstein cows was diagnosed in a dairy farm from southwest Uruguay. After the consumption of the morning feed, 6 of 189 cows presented dyspnea, orthopnea, and open-mouth breathing. One cow died during the manifestation of these clinical signs, the other five cows recovered a few minutes afterwards. Another cow died the night before. There were no lesions in the respiratory system. The maximum Temperature Humidity Index (THI) recorded on February 27-28 and March 1 was 83.5, 83.5 and 83.0, respectively, indicating a heat wave and moderate heat stress. No ergoalkaloids were detected upon toxicological evaluation of the feed. The sudden onset of clinical signs, the lack of lesions in the respiratory tracts, and the moderately elevated THI registered for a prolonged period, indicated an acute clinical episode of heat stress with fatal outcome. The retrospective analysis of the daily THI and average milk production revealed that the clinical episode was preceded by a marked subclinical reduction of milk production from February 15 to March 2, associated with a high THI registered in that period. This was followed by a partial recovery of milk production between March 3 and March 15, in association with a concomitant reduction of the THI and, therefore, of heat stress. This report highlights the need for applying adequate mitigation strategies to prevent and control heat stress during summer in dairy cows in Uruguay.
