RESUMO: O estudo foi realizado em um ovino, fêmea, de três anos de idade, da raça santa inês, procedente de uma propriedade localizada do município de Santo Antônio de Tauá, oeste do Pará. O animal acometido tinha histórico de exoftalmia no olho esquerdo, secreção nasal e dificuldade respiratória há mais de três meses. Ao exame físico o animal apresentava baixo escore corporal, apatia, assimetria facial unilateral, moderada exoftalmia unilateral esquerda, dispneia mista e exsudação nasal serossanguinolenta profusa unilateral. À necropsia, foi observada grande massa nasal, predominantemente branco-amarelada com áreas vermelho enegrecidas, de superfície irregular, finamente granular, úmida e friável. A massa invadia desde a junção mucocutânea do nariz esquerdo até as coanas, se infiltrava pelos seios nasais frontais, placa cribiforme e alcançava as meninges e a porção frontal do cérebro. À microscopia da superfície do epitélio nasal notou-se severa destruição dos cornetos e dos epitélios, evidenciada por extensas áreas de necrose com muitos debris celulares, ulceração e infecção bacteriana secundária. No sistema nervoso central (SNC) havia meningite piogranulomatosa com acentuada congestão de vasos, focos de necrose e inflamação granulomatosa. ABSTRACT: The study was conducted on a 3-year-old female Santa Inês sheep from a property located in the municipality of Santo Antônio de Tauá, west of Pará. The affected animal had a history of exophthalmos in the left eye, nasal discharge, and breathing difficulty for more than three months. On physical examination, the animal had a low body score, apathy, unilateral facial asymmetry, moderate unilateral left exophthalmos, mixed dyspnea and unilateral profuse serosanguinous nasal discharge. At necropsy, a large nasal mass was observed, predominantly yellowish-white with blackish-red areas, with an irregular, finely granular, moist, and friable surface. The mass invaded from the mucocutaneous junction of the left nostril to the choanae, infiltrated the frontal sinuses, the cribiform plate, and reached the meninges and the frontal portion of the brain. Microscopy of the surface of the nasal epithelium showed severe destruction of the turbinates and epithelia, evidenced by extensive areas of necrosis with a large amount of cellular debris, ulceration, and secondary bacterial infection. In the central nervous system there was pyogranulomatous meningitis with marked vessel congestion, as well as foci of necrosis and granulomatous inflammation. Immunohistochemistry showed intense staining of numerous intralesional hyphae for antibodies against Conidiobolus lamprauges. The diagnosis of granulomatous rhinitis of mycotic origin associated with the Conidiobolus lamprauges was based on epidemiological, clinical-pathological, and immunohistochemical data. RESUMO: O melão de polpa alaranjada honeydew apresenta elevado potencial nutricional e econômico, sendo amplamente apreciado como fruta doce e refrescante. Portanto, avaliar amostras comerciais de produtos tão relevantes para o mercado de frutas frescas é de grande importância. O objetivo deste estudo foi investigar a capacidade antioxidante, os parâmetros de qualidade e a composição química de melões do mercado brasileiro. Os valores de pH e ºBrix determinados para as amostras de melão foram 6,5 ± 0,30 e 9,2 ± 0,80 g/100 g (peso fresco), respectivamente, o que está de acordo com os resultados esperados para melões de alta qualidade. A análise cromatográfica revelou a sacarose (49% do açúcar total) como principal açúcar, seguida pela frutose (30%) e glicose (21%). Os carotenóides, que estão diretamente ligados à cor laranja característica, apresentaram teor total de 270,59 µg/g (peso seco). Além disso, o maior valor de b* comparado ao valor de a* indica a maior contribuição da cor amarela. Quanto ao potencial antioxidante, além de apresentar capacidade antioxidante para ambos os métodos aplicados, DPPH e FRAP, níveis variando de 16,40 ± 0,21 µmol equivalente de ácido ascórbico/g a 56,98 ± 0,80 µmol equivalente de Trolox/g (peso seco) foram encontrados nos extratos de melão. Ademais, os resultados foram maiores em termos de FRAP do que em DPPH. Desta forma, os resultados destacam os melões avaliados como fontes valiosas de compostos bioativos, como carotenóides e fenólicos, que contribuem para suas propriedades antioxidantes. ABSTRACT: Orange-fleshed non-netted honeydew melon has high nutritional and economic potential, being widely appreciated as a refreshing sweet fruit. Therefore, evaluating commercial samples of such relevant products for the fresh fruit market is of great importance. This study investigated the antioxidant capacity, quality parameters, and chemical composition of honeydew melons from the Brazilian market. The values of pH and °Brix determined for melon samples were 6.5 ± 0.30 and 9.2 ± 0.80 g/100 g (fresh weight), respectively, which agree with the results expected for high-quality melons. The chromatography analysis revealed sucrose (49% of total sugar) as the main sugar, followed by fructose (30%) and glucose (21%). Carotenoids, which are directly linked to the characteristic orange color, showed a total level of 270.59 µg/g (dry weight). Additionally, the higher b* value compared to a* value indicates the highest yellow contribution. Regarding the antioxidant assays, in addition to presenting antioxidant capacity for both methods applied, DPPH and FRAP, levels ranging from 16.40 ± 0.21 µmol ascorbic acid equivalent/g to 56.98 ± 0.80 µmol Trolox equivalent/g (dry weight) were reported in melon extracts. RESUMO O manejo dos níveis de condutividade elétrica é um dos fundamentos necessários para se obter maior produção e melhor qualidade de hortaliças. O objetivo deste trabalho foi avaliar o efeito da condutividade elétrica da solução nutritiva na cultura da couve-rábano cultivada em sistema semi-hidropônico. O delineamento experimental foi em blocos casualizados com cinco tratamentos associados à condutividade elétrica da solução nutritiva (1,31; 1,71; 2,37; 2,98 e 3,75 dS m-1) e quatro repetições. As plantas foram colhidas 78 dias após o transplante e avaliadas quanto ao crescimento, produção e qualidade pós-colheita. Solução nutritiva com condutividade elétrica (CE) de 1,31 dS m-1 proporciona maior produção de couve-rábano roxa. O aumento da CE reduziu linearmente a maioria das variáveis analisadas (área foliar, massa fresca de folhas, bulbos e parte aérea, massa seca de folhas e totais, volume e firmeza de bulbos), com perdas mais expressivas para as variáveis massa fresca de bulbos (50,54%) e volume de bulbos (57,37%). ABSTRACT Managing electrical conductivity levels is one of the fundamentals required to obtain greater production and better quality of vegetables. The aim of this study was to evaluate the effect of electrical conductivity of the nutrient solution on the kohlrabi crop cultivated in semi-hydroponic system. A randomized block design was used with five treatments associated with the electrical conductivity of the nutrient solution (1.31, 1.71, 2.37, 2.98, and 3.75 dS m-1) and four replications. The plants were harvested 78 days after transplanting and evaluated for growth, production, and postharvest quality. Nutrient solution with electrical conductivity (EC) of 1.31 dS m-1 promotes greater production of purple kohlrabi. The increase in EC reduced linearly most of the analyzed variables (leaf area, fresh mass of leaves, bulb and shoot, dry mass of leaves and total, bulb volume and firmness), with more significant losses for the bulb fresh mass (50.54%) and bulb volume (57.37%) variables. Resumen Objetivo Evaluar la estructura y el flujo de asistencia de recepción, clasificación de riesgo y emergencia obstétrica, en una maternidad pública en el contexto del COVID-19. Métodos Estudio tipo evaluación normativa y observacional en los sectores de Recepción (clasificación de riesgo y emergencia obstétrica) de una maternidad pública en Rio de Janeiro. La recopilación de datos se realizó de junio a agosto de 2020 durante 480 de observación directa no participante, con registros sistematizados en una checklist con variables relacionadas con la disponibilidad y conformidad de los recursos estructurales y con la conformidad del flujo de asistencia. Los datos se organizaron en planillas (Microsoft Excel® 2010) y se analizaron usando la estadística descriptiva. Resultados La evaluación de la estructura obtuvo un 80,3 % de disponibilidad, que tuvo como resultado una clasificación de alta disponibilidad, y un 91,1 % de conformidad, con una conformidad adecuada. A coleta de dados foi realizada de junho a agosto de 2020 por 480 horas de observação direta não participante, com registros sistematizados em checklist contendo variáveis relacionadas à disponibilidade e conformidade dos recursos estruturais, e à conformidade do fluxo assistencial. Abstract Objective Evaluate the structure and care flow - risk classification and obstetric emergency in a public maternity hospital in the context of COVID-19. Methods Normative and observational evaluation study in the Care sectors (Risk Classification and Obstetric Emergency) of a public maternity hospital in Rio de Janeiro. Data collection was carried out from June to August 2020 for 480 hours of non-participant direct observation, with records systematized in a checklist containing variables related to the availability and compliance of structural resources, and the compliance of the care flow. Data were organized into spreadsheets (Microsoft Excel® 2010) and analyzed using descriptive statistics. Results The framework assessment scored 80.3% availability, resulting in a high availability rating, and 91.1% compliance, achieving adequate compliance. The care flow showed 72.7% of total compliance; 9.1% of partial compliance and 18.2% of non-compliance, configuring high compliance. Conclusion Normative evaluation indicated high availability and compliance in structure and care flow in the studied scenarios. COVID19. ABSTRACT Purpose: To conduct a systematic literature review with meta-analysis to identify whether antibiotic prophylaxis after removal of the indwelling urinary catheter reduces posterior infections. Methods: A systematic literature review was conducted in the databases PubMed, Embase, Cochrane, Google Scholar, and Latin American and Caribbean Health Sciences Literature, using the keywords “antibiotics” AND “prostatectomy” AND “urinary catheter.” Results: Three articles were identified having the scope of our review, with 1,040 patients, which were subjected to our meta-analysis revealing a marginally significant decrease in the risk of urinary infection after indwelling urinary catheter removal (odds ratio–OR = 0.51; 95% confidence interval–95%CI 0.27–0.98; p = 0.04; I2 = 0%). No difference was found regarding the presence of bacteriuria (OR = 0.39; 95%CI 0.12–1.24; p = 0.11; I2 = 73%). Conclusions: In our meta-analysis, there was a significant decrease in urinary tract infection with antibiotic prophylaxis after indwelling urinary catheter removal following radical prostatectomy. Resumo Esta revisão tem como objetivo discutir os principais fatores envolvidos no desenvolvimento de folículos antrais iniciais até a dependência de gonadotrofinas. Essa fase folicular é caracterizada por intensa proliferação de células da granulosa, formação de uma cavidade preenchida por líquido, diferenciação morfológica das células do cumulus, células da granulosa murais e recrutamento de células da teca. A interação entre oócito, células da granulosa e da teca é determinante para o crescimento folicular e produção hormonal. Fatores de crescimento produzidos pelo oócito, fator de crescimento e diferenciação-9 (GDF-9) e proteína morfogenética óssea-15 (BMP-15), regulam a proliferação e diferenciação de células da granulosa, e o desenvolvimento da cavidade antral, bem como estimulam a produção de receptores do hormônio folículo estimulante (FSH) nas células da granulosa. Em resposta ao FSH, as células da granulosa secretam o peptídeo natriurético tipo C (CNP), que atua através de seu receptor para aumentar a produção de monofosfato de guanosina cíclico (GMPc) e consequentemente o desenvolvimento folicular. As células da granulosa também produzem o fator de crescimento semelhante à insulina 1 (IGF-1) e aumentam a atividade da enzima aromatase, o que resulta em maior sensibilidade às gonadotrofinas e esteroidogênese folicular. A ausência de sinalização do IGF-1 causa cessação do crescimento folicular no início do estágio antral. Muitos outros fatores locais estão envolvidos na regulação do desenvolvimento folicular. Abstract This review aims to discuss the main factors involved in the development of early antral follicles until gonadotropin dependence. This follicular phase is characterized by intense proliferation of granulosa cells, formation of a fluid-filled cavity, morphological differentiation of cumulus cells, mural granulosa cells and recruitment of theca cells. The interaction between oocyte, granulosa and theca cells is crucial for follicular growth and hormone production. Growth factors produced by the oocyte, such as growth and differentiation factor-9 (GDF-9) and bone morphogenetic protein-15 (BMP-15), regulate granulosa cell proliferation and differentiation and antral cavity development, as well as stimulate the production of follicle-stimulating hormone (FSH) receptors in granulosa cells. In response to FSH, granulosa cells secrete C-type natriuretic peptide (CNP), which acts through its receptor to increase cyclic guanosine monophosphate (cGMP) production and consequently follicular development. Granulosa cells also produce insulin-like growth factor-1 (IGF-1) and increase aromatase enzyme activity, which results in greater sensitivity to gonadotropins and follicular steroidogenesis. The absence of IGF-1 signaling causes cessation of follicular growth at the early antral stage. Many other local factors are involved in the regulation of follicular development. Therefore, this review brings relevant data for a better understanding of the mechanisms involved in the control of early antral follicle growth, emphasizing the role of endocrine and paracrine factors, the oocyte-granulosa cell interaction and the processes of follicular atresia. Resumo A osteoartrite do tornozelo (OAT) está associada a quadro álgico e limitação funcional variável, demandando tratamento clínico e eventual indicação cirúrgica quando as medidas conservadoras são inefetivas – a artrodese tem sido o procedimento de escolha, por reduzir a dor, restaurar o alinhamento articular e tornar o segmento estável, preservando a marcha. O presente estudo relata 3 casos (3 tornozelos) de pacientes do sexo masculino, com entre 49 e 63 anos de idade, portadores de OAT secundária, American Orthopaedic Foot and Ankle Society Ankle-Hindfoot Scale (AOFAS AHS, na sigla em inglês) pré-operatória de 27 a 39 pontos, tratados mediante artrodese tibiotalocalcaneana minimamente invasiva utilizando haste intramedular retrógrada bloqueada. A permanência hospitalar foi de 1 dia, e os pacientes foram autorizados para carga imediata com órteses removíveis para deambulação, conforme tolerado. O tratamento fisioterápico, introduzido desde o internamento, foi mantido, priorizando-se treino de marcha, ganho de força e propriocepção. Foi realizado acompanhamento clínico e radiográfico nas semanas 1, 2, 6, 12 e 24. Após evidências de consolidação (entre a 6ª e a 10ª semanas), as órteses foram retiradas. Um paciente queixou-se de dor no pós-operatório imediato e, ao final do 1° ano, apenas 1 paciente apresentou dor durante a reabilitação, resolvida completamente com analgésicos. Abstract Ankle osteoarthritis (AOA) is associated with pain and variable functional limitation, demanding clinical treatment and possible surgical indication when conservative measures are ineffective – arthrodesis has been the procedure of choice, because it reduces pain, restores joint alignment and makes the segment stable, preserving gait. The present study reports 3 cases (3 ankles) of male patients between 49 and 63 years old, with secondary AOA, preoperative American Orthopaedic Foot and Ankle Society Ankle-Hindfoot Scale (AOFAS AHS) of 27 to 39 points, treated by minimally invasive tibiotalocalcaneal arthrodesis using blocked retrograde intramedullary nail. Hospital stay was of 1 day, and the patients were authorized for immediate loading with removable ambulation orthotics, as tolerated. The physical therapy treatment, introduced since hospitalization, was maintained, prioritizing gait training, strength gain, and proprioception. Clinical and radiographic follow-up was performed at weeks 1, 2, 6, 12 and 24. After evidence of consolidation (between the 6th and 10th weeks), the orthotics were removed. One patient complained of pain in the immediate postoperative period and, at the end of the 1st year, only one patient presented pain during rehabilitation, which was completely resolved with analgesics. Currently, the patients do not present complaints, returning to activities without restrictions – one of them, to the practice of soccer and rappelling. Abstract The use of different medical imaging modalities is becoming more frequently engaged in medical applications. This is mainly due to the expansion of different computational processing techniques, which facilitates not only diagnosis but also the follow-up care during medical treatments. The use of anatomical medical images, acquired by computerized tomography (CT) or magnetic resonance imaging (MRI), can be registered with the functional images of the human body, obtained by other exams, such as infrared thermography and scintigraphy. Therefore, this combination of different imaging modalities is representing an innovative perspective in medicine. The methodology presented in this paper allows the imaging fusion and achieves new 2D images, which merge different modalities altogether. The image data acquired are aligned based on the affine registration, where at least three corresponding pairs of points are selected in both images. For this purpose, it was developed a script in MATLAB®. This processing allows the combination of different pairs of images, as it also applies transparencies into them. The results obtained can grant the visualization of the thyroid and other structures of the human anatomy simultaneously into a single aligned image. RESUMO: Objetivou-se, na construção deste trabalho, detectar Anaplasma marginale por meio do esfregaço sanguíneo e Nested PCR (nPCR) e comparar os resultados com os sinais clínicos apresentados pelos bezerros em uma propriedade leiteira localizada no município de Castanhal, região nordeste do estado do Pará (1°07’19,1”S e 47°53’53,0”W), Amazônia Oriental. Para isso, foram coletadas 192 amostras sanguíneas de 24 animais, divididos em três períodos: 1-20, 21-41 e 42-60 dias de idade. Foram realizados esfregaços sanguíneos e nPCR com iniciadores para o gene msp5. A prevalência de A. marginale foi de 61,46% (118/192) tanto para a técnica de esfregaço sanguíneo quanto para nPCR (msp5). A manifestação de sinais clínicos da anaplasmose também foi significativamente crescente ao longo do estudo (P < 0,0001), sendo menor em animais de 1 a 20 dias, mas expandindo-se entre os de 21 a 41 dias e 42 a 60 dias, esses sinais foram caracterizados por apatia, febre, perda de peso, diarreia, desidratação e mucosas hipocoradas. Quanto a avaliação das técnicas diagnósticas, não houve diferença significativa entre a detecção de A. marginale no esfregaço sanguíneo e na nPCR (P = 0,995), porém se observou aumento riquetsêmico do agente no 47º dia (P < 0,01) em ambos os testes, demonstrando, assim, um padrão de aumento da riquetsemia próximo ao linear ao longo dos 60 dias, com consequente diminuição do volume globular. Assim, demonstra-se que dos 24 animais estudados, 21 se infectaram em algum momento do período estudado, e não houve diferença significativa entre esfregaço sanguíneo e nPCR, em virtude, provavelmente, das parasitemias médias e altas, as quais estiveram diretamente relacionadas com os sinais clínicos e a diminuição do volume globular. ABSTRACT: This study detected Anaplasma marginale in calvesusing blood smears and nested PCR (nPCR) and to compare the results with the clinical signs presented by calves on a dairy farm in the municipality of Castanhal, located northeast of the state Pará (1°07’19.1”S and 47°53’53.0”W), eastern Amazon. To this end, 192 blood samples were collected from 24 animals at 1-20, 21-41 and 42-60 days of age. Blood smears and nPCR with primers for the msp5 gene were performed. The prevalence of A. marginale was 61.5% (118/192) for the blood smear technique and nPCR (msp5). The manifestation of clinical signs of anaplasmosis also increased significantly over the course of the study (P < 0.0001), being lower in animals aged 1-20 days, but increasing among those aged 21-41 and 42-60 days. These signs were characterized by apathy, fever, weight loss, diarrhea, dehydration, and hypochromic mucous membranes. Regarding the evaluation of the diagnostic techniques, no significant difference was observed in the detection of A. marginale between the blood smear and nPCR (P = 0.995), but the agent’s rickets increased on Day 47 (P < 0.01) in both tests, thereby demonstrating a near-linear pattern of increase in rickets over the 60 days, with a consequent decrease in globular volume. This shows that of the 24 animals studied, 21 were infected at some point during the study period. Additionally, there was no significant difference between blood smears and nPCR, probably due to medium and high parasitemia, which were directly related to the clinical signs and decrease in globular volume. Abstract The climatic and edaphic conditions of the semi-arid Brazilian Northeast region limit the production of several annual crops, including cotton. The aim of this study was to estimate the genetic parameters and select water stress tolerant cotton lines based on yield and fiber quality traits. Twenty cotton lines were evaluated in Alagoinha-PB over two years under rainfed conditions. Individual and joint analysis of variance was performed on the data. Genetic parameters were determined, and lines were selected using the selection index. The lines CNPA SA 2019-115, CNPA SA 2019-185, CNPA SA 2019-109, and CNPA SA 2019-165 were selected for further tests in the region for recommendation of new cultivars. ABSTRACT As part of the systematic study of the tribes Ditaxeae and Adelieae (Acalyphoideae-Euphorbiaceae), here we compile and present a taxonomic synopsis for the South American taxa of Argythamnia, Chiropetalum, and Philyra. We recognize 22 species for Argythamnia, including one herein described as a new species, 20 species and one variety for Chiropetalum, and a single species for Philyra. We also designate 22 lectotypes, a new synonymization and a status novum, in addition to keys for each genus, distribution maps for all taxa, analysis of collection density in South America for each genus, and taxonomic and ecological comments for all taxa. RESUMO Introdução: A empatia, um elemento primordial do humanismo, tem sido alvo de crescente interesse no ensino médico. No contexto das profissões de saúde, a empatia no atendimento ao paciente é um atributo que envolve a compreensão das experiências, da dor, do sofrimento e das preocupações do paciente combinados com a capacidade de comunicar esse entendimento e uma intenção de ajudar. Apesar do desenvolvimento de práticas que estimulam a empatia nos estudantes de Medicina, um desafio presente na vida acadêmica é mensurar essa habilidade. Objetivo: Este estudo teve como objetivos elaborar e validar uma escala brasileira de empatia no contexto do atendimento clínico. Método: Trata-se de estudo-piloto de construção e validação de uma escala psicométrica, realizado em cinco etapas: 1. definição das dimensões do construto baseada em revisão da literatura; 2. itens submetidos à análise teórica com juízes especializados no tema, objetivando identificar a pertinência do item dentro do construto; 3. realização de um pré-teste com a população-alvo visando avaliar o entendimento dos itens; 4. validação da escala com aplicação a 207 estudantes de Medicina brasileiros; 5. aplicação de testes estatísticos que auxiliam na validação da escala. Resultado: Ao final do estudo, a escala de empatia elaborada continha 21 itens, com respostas em escala de Likert, distribuídos em dois fatores: compreensão empática e ação empática, que apresentaram boa confiabilidade proposta (> 0,842) e boa consistência interna (H-latente > 0,879 e H-observado > 0,864) com total de variância explicada de 44,95%. ABSTRACT Introduction: Empathy, which is a crucial element of humanism, has been the subject of increasing interest in medical education. In the context of the healthcare professions, empathy in patient care is an attribute that involves understanding the patient’s experiences, pain, suffering, and concerns combined with the ability to communicate that understanding and an intention to help. Despite the development of practices that encourage empathy in medical students, a challenge in academic life is measuring this ability. Objective: To develop and validate a Brazilian scale of empathy in the context of clinical care. Method: Pilot study of construction and validation of a psychometric scale, carried out in five stages: (1) Definition of the dimensions of the construct based on a literature review; (2) Items submitted to theoretical analysis with judges specialized in the subject, aiming to identify the pertinence of the item within the construct; (3) carrying out a pre-test with the target population to assess the understanding of the items (4) validation of the scale with application to 207 Brazilian medical students (5) application of statistical tests that help in the validation of the scale.
