Resumen La experiencia a partir de la cual reflexionaremos está anclada en el Área Farmacia Asistencial de la Facultad de Ciencias Bioquímicas y Farmacéuticas de la Universidad Nacional de Rosario. En 2022 implementamos un proyecto de extensión que habilitó una asignatura electiva y tareas de investigación. El objetivo es promover la reflexión a partir de la experiencia de integración de funciones de enseñanza, extensión e investigación, en un territorio priorizado por la Universidad. Trabajamos en un barrio del distrito oeste de Rosario: en el Club Social y Deportivo 20 amigos, un Centro de Salud y una farmacia comunitaria. Promovimos el abordaje crítico de la utilización de medicamentos: realizamos entrevistas e intervenciones en el marco de Atención Farmacéutica y relevamos eventos adversos supuestamente atribuibles a la vacunación e inmunización. Nuestro horizonte es generar estrategias de integración del trabajo territorial y académico para estimular una perspectiva humanizadora y comprometida con la realidad social.

Resumo A experiência a partir da qual refletiremos está ancorada na Área de Farmácia Assistencial da Faculdade de Ciências Bioquímicas e Farmacêuticas da Universidade Nacional de Rosário. Em 2022 implementamos um projeto de extensão que habilitou uma disciplina optativa e áreas de pesquisa. O objetivo é promover a reflexão a partir da experiência de integração das funções de ensino, extensão e pesquisa em um território priorizado pela Universidade. Atuamos em um bairro da zona oeste de Rosário: no Clube Social e Esportivo 20 Amigos, um Centro de Saúde e uma farmácia comunitária. Promovemos uma abordagem crítica da utilização de medicamentos: realizamos entrevistas e intervenções no âmbito da Assistência Farmacêutica e levantamos eventos adversos presumivelmente atribuíveis à vacinação e imunização. Nosso horizonte é gerar estratégias de integração do trabalho territorial e acadêmico para estimular uma perspectiva humanizadora e comprometida com a realidade social.

Abstract The experience from which we will reflect is anchored in the Assistance Pharmacy Area of the Faculty of Biochemical and Pharmaceutical Sciences of the National University of Rosario. In 2022 we implemented an extension project that enabled an elective subject and research tasks. The objective is to promote reflection based on the experience of integrating teaching, extension and research functions in a territory prioritized by the University. We worked in a neighborhood in the western district of Rosario: in the 20 Amigos Social and Sports Club, a Health Center and a community pharmacy. We promoted a critical approach to medication use: we conducted interviews and interventions within the framework of Pharmaceutical Care and we reported adverse events supposedly attributable to vaccination and immunization. Our horizon is to generate integration strategies of territorial and academic work to stimulate a humanizing perspective and committed to social reality.

Abstract Difficulties in guaranteeing the supply of specific drugs for the treatment of human immunodeficiency virus / acquired immunodeficiency syndrome (HIV/AIDS) are recurrent given the advancement of adjustment policies. In Argentina, the gear that sustains compliance with public health obligations is permeable to crisis. The problem focuses on the most acute crisis in Argentina in December 2001, from Oszlak and O'Donnell´s perspective. Objective: to reconstruct the processes of discussion, generation and implementation of public policies from the National Health Ministry -specifically in its interaction with international agencies- in order to guarantee the supply of antiretrovirals (ARV) to all people living with HIV/AIDS of the country. Qualitative approach, documents were used: four dossiers of the medication management of the National Health Ministry and minutes of Commissions. Semi-structured individual interviews were conducted with key actors according to their role during the health emergency. A category of the first dimension is presented: Strategies implemented by the State to guarantee the acquisition of ARV, category: international agencies. A data matrix was constructed to analyze the information. The reconstruction showed that the problem had an international agenda and that the commitments had to be fulfilled despite the crisis. Tensions and contradictions are found within the framework of economic, social and cultural rights, taking into account the progressivity and non-regressivity of rights. States have double international pressure: agencies that monitor indicators rights and in the other side, lending agencies that define the priorities of recipient countries. Institutionally weak States, with low autonomy and in crisis generate programmatic vulnerability. Access to medicines is essential to achieve the right to health and it is a constant problem in emerging countries.

Resumen Las dificultades para garantizar el suministro de medicamentos específicos para el tratamiento del virus de inmunodeficiencia humana / síndrome de inmunodeficiencia adquirida (VIH/sida) son recurrentes ante el avance de políticas de ajuste. En Argentina, el engranaje que sostiene el cumplimiento de obligaciones en salud pública es permeable a las crisis. El problema se focaliza en el momento más agudo de la crisis de diciembre de 2001, y se trabajó desde la perspectiva de Oszlak y O´Donnell. Objetivo: reconstruir los procesos de discusión, generación e implementación de políticas públicas desde el Ministerio de Salud de la Nación -específicamente en su vínculo con las agencias internacionales-, con el fin de garantizar el suministro de antirretrovirales (ARV) a todas las personas viviendo con VIH/sida del país. Con un abordaje cualitativo, se utilizaron cuatro expedientes de la gestión de medicamentos del Ministerio de Salud de la Nación, actas de comisiones y se realizaron entrevistas individuales semiestructuradas a actores clave según su rol durante la emergencia sanitaria. Se presenta una categoría de la primera dimensión: estrategias implementadas por el Estado para garantizar la adquisición de ARV, categoría agencias internacionales. Se construyó una matriz de datos para analizar la información. La reconstrucción muestra que la problemática tenía agenda internacional y que los compromisos debían cumplirse a pesar de la crisis. En el marco de los derechos económicos, sociales y culturales, se encuentran tensiones y contradicciones, teniendo en cuenta la progresividad y no regresividad de los derechos. Los estados tienen doble presión internacional: organismos que monitorean indicadores de derechos y organismos de préstamo que definen en qué pueden gastar los países receptores. Los estados débiles institucionalmente, con baja autonomía y en crisis generan vulnerabilidad programática. El acceso a medicamentos es fundamental para alcanzar el derecho a la salud y es una problemática constante en países emergentes.

In accordance with the National AIDS Law, the Argentinian State is committed to supply antiretroviral medication (ARV) free of charge. Due to the complexity of the National ARV Program management process, and the fact that people with HIV/AIDS (PLHA) are the beneficiaries of the Program, it is considered relevant to ascertain their opinion. The aim of this work is to examine the supply of ARV by the National HIV/AIDS Program, from the perspective of PLHA, in a public hospital of the city of Rosario, in relation to: 1) value of the Program and the information received from it; 2) waiting times; 3) availability of ARV to continue the treatment. It is a qualitative study with interviews during January and February 2007 consisting of structured questions and patient criteria selection, in the presence of a psychologist. 15 PLHA were interviewed and gave their considered opinion about the Program and its background. They acknowledge difficulties in the continuity of ARV treatment and the consequences with respect to death and quality of life. The Program shows the coexistence between characteristics of a policy guaranteed by the state and others from programs that respond to emergencies. It shows the need to rethink the Program as a genuine health policy, considering ARV as essential medicine.

El Estado argentino se compromete, mediante la Ley Nacional de SIDA, a suministrar gratuitamente los ARV. Reconociendo la complejidad que envuelve el proceso de gestión de ARV desde Programa Nacional y dando por sentado que las PVVS son el fin último del mismo, se piensa que es fundamental indagar en sus opiniones. El objetivo es explorar el suministro de ARV desde la perspectiva de las PVVS, en un hospital público de Rosario, respecto a: 1) valoración del Programa y de la información recibida, 2) tiempos de espera, 3) la provisión en relación con la disponibilidad de ARV para sostener el tratamiento. Estudio de abordaje cualitativo, con entrevistas durante enero y febrero de 2007, se realizan en presencia de una psicóloga. Se definen criterios de selección de los entrevistados y ejes para las entrevistas. Las PVVS entrevistadas son 15 y opinan con autoridad respecto al Programa, señalando problemas en el suministro de ARV. Reconocen dificultades del sostenimiento en el tiempo en la toma de ARV y sus consecuencias en relación a la muerte y a la calidad de vida. El Programa se caracteriza por convivencia de rasgos que responden a políticas garantizadas por el Estado y a programas que responden a la emergencia. Aparece la necesidad de repensar el Programa como política de salud genuina, considerando a los ARV como medicamentos esenciales.
