Introduction: unintentional injuries are one of the leading causes of morbidity and mortality in the paediatric population and can be prevented if the risk factors that make them more likely are identified and addressed. The degree of parental supervision is one of the factors that may affect the occurrence of unintentional injuries. The aim of our study was to compare the degree of parental supervision estimated subjectively by health care professionals and the degree measured objectively using a questionnaire validated for this purpose. The ultimate objective was to determine the need to use questionnaires to act more efficiently to address risk factors. Material and methods: we conducted an observational, cross-sectional and analytic study to assess the agreement between the degree of parental supervision perceived by paediatricians and the degree measured using the Parental Supervision Attributes Profile Questionnaire (PSAPQ), which has been adapted and validated for use in Spanish translation. Results: we found no correlation between the agreement between the degree of parental supervision perceived by paediatricians and the degree measured using the PSAPQ. Conclusions: validated methods need to be used to objectively measure the risk factors involved in unintentional injuries in children, including the degree of parental supervision, in order to avoid perception biases and address these factors as efficiently as possible.

Introducción: los accidentes infantiles son una de las mayores causas de morbimortalidad en la edad pediátrica, y pueden ser prevenibles si se conoce y se actúa sobre los factores de riesgo que los propician. El grado de supervisión parental es un factor que puede influir, entre otros, en la aparición de estos. El objetivo de este estudio ha sido la comparación entre el grado de supervisión parental atribuido de forma subjetiva por los profesionales sanitarios, y el medido de forma objetiva mediante la utilización de un cuestionario validado para este fin. La finalidad es valorar la necesidad de utilizar cuestionarios para actuar de forma más eficiente frente a los factores de riesgo. Material y métodos: estudio observacional, trasversal y analítico, para comparar la concordancia entre el grado de supervisión parental estimado por los pediatras de Atención Primaria y el medido de forma objetiva mediante el Parental Supervision Attributes Profile Questionnaire (PSAPQ), adaptado y validado a lengua española. Resultados: se ha demostrado que no existe concordancia entre el grado de supervisión parental expresado de forma subjetiva por parte de los profesionales sanitarios y el medido de forma objetiva mediante la utilización del cuestionario PSAPQ. Conclusiones: es necesaria la utilización de metodología validada para la medición de forma objetiva de los diversos factores de riesgo de accidentes infantiles, entre ellos el grado de supervisión, para de esta forma evitar sesgos de percepción y actuar de la forma más eficiente posible.

Abstract Introduction: One of the purposes in the research in attention deficit/hyperactivity disorder (ADHD) is identify whether there is a relationship between gender and its diagnosis in ADHD in children. Objectives: The objectives were to identify whether there is: (1) A relationship between subtypes in the diagnosis of ADHD according to the diagnostic and statistical manual of mental disorders version IV (DSM-IV) and gender, and (2) a subtype proportion in the diagnosis of ADHD according to gender. Method: Retrospective study of 297 clinical records of children diagnosed with ADHD of a Pediatric Service in a public hospital. Results: (1) In the diagnosis according to the three subtypes of ADHD (DSM-IV), there is a higher proportion of ADHD of the inattentive type in female and a greater proportion in ADHD of the combined type in male regardless of the number of children according to gender. The results were statistically significant (test Chi-square 3 subtypes: p ≤ 0.001) and also were in the inattentive and combined subtypes of the analyzed author; (2) in the male gender, the diagnosis of ADHD was higher (73.40%) than among females (26.60%). The result was statistically significant as well as those obtained by the author that corresponded to the present study. Conclusions: In the diagnosis in ADHD and gender, a significant proportion of more male diagnosed with ADHD and with the combined subtype were obtained compared to the female and their inattentive subtype. The high rate in male gender with the combined subtype because of their behavioral characteristics probably represents a trigger for this higher proportion of children diagnosed with ADHD with respect to the female gender.

Resumen Introducción: Uno de los propósitos en la investigación en el trastorno por déficit de atención/hiperactividad (TDAH) es identificar si existe relación entre el género y el diagnóstico de TDAH en niños. Objetivos: Los objetivos fueron identificar si existe: 1) una relación entre en los subtipos en el diagnóstico del TDAH según el Manual Diagnóstico y Estadístico de los Trastornos Mentales versión IV (DSM-IV) y el género, y 2) una proporción del subtipo en el diagnóstico de TDAH según el género. Metodología: Estudio retrospectivo de 297 historias clínicas de niños con diagnóstico de TDAH de un Servicio de Pediatría en un hospital público. Resultados: 1) en el diagnóstico según los tres subtipos del TDAH (DSM-IV) se obtiene una mayor proporción en TDAH de tipo inatento en niñas y una mayor proporción en TDAH de tipo combinado en niños, independientemente del número de niños según el género. Los resultados fueron estadísticamente significativos (test chi cuadrado 3 subtipos: p ≤ 0.001) y también lo fueron en los subtipos inatento y combinado del autor analizado; 2) en el género masculino es mayor el diagnóstico de TDAH (73.40%) con respecto al género femenino (26.60%); el resultado fue estadísticamente significativo, como también los obtenidos por el autor, que se correspondieron con el presente estudio. Conclusiones: En el diagnóstico del TDAH y el género, se obtuvo una proporción de niños diagnosticados con TDAH significativamente mayor y con el subtipo combinado en comparación con las niñas y su subtipo inatento. La tasa elevada de niños con el subtipo combinado por sus características en la conducta probablemente represente un disparador para esta mayor proporción de niños con diagnóstico de TDAH con respecto al género femenino.

The aim of this study was to validate and cross-culturally adapt to Spanish the questionnaire Parental Supervision Attributes Profile Questionnaire (PSAPQ) as a method of measuring the degree of parental supervision. Methods: Forward and back-translation methodology was applied using 4 bilingual spanish-english people to obtain version 1.0 of the questionnaire. Subsequently, the questionnaire was reviewed by a committee of experts, obtaining version 1.1, which was provided to 149 parents of children between 2-5 years old, randomly obtained, who attended consultation for the well child visits in several health centers in the province of Valencia. For the statistical study, the internal consistency was analyzed using Cronbach’s α test and the test-retest reliability using Pearson correlations. Results: A very good internal consistency was obtained, with Cronbach’s α values greater than 0.7 in three of the four subscales that make up the test, with the remaining being 0.68. The reliability obtained was excellent, with values with Pearson correlations close to or higher than 0.7 for all subscales. Conclusions: The PSAPQ translated and validated into spanish shows very good psychometric results with respect to the original, so it can be said that an adequate instrument has been obtained to objectively measure one of the possible risk factors of child accidents

RESUMEN Fundamentos: Los accidentes infantiles son la causa prevenible más importante de morbi-mortalidad en edad pediátrica. El grado de supervisión parental es un factor que puede influir, entre otros, en su aparición. El objetivo de este estudio fue validar y adaptar transculturalmente al español el cuestionario Parental Supervision Attributes Profile Questionnaire (PSAPQ) como método de medida del grado de supervisión de los padres a sus hijos. Métodos: Se empleó una metodología de traducción/retrotraducción mediante 4 personas bilingües (español-inglés), obteniéndose la versión 1.0 del cuestionario. Posteriormente, fue revisada por un comité de expertos, obteniéndose la versión 1.1, que se proporcionó a 149 padres de niños de entre 2-5 años, reclutados de forma aleatoria, que acudían a las revisiones del programa de salud infantil de diversos centros de salud de la provincia de Valencia. En el estudio estadístico se analizó la consistencia interna mediante el test alfa de Cronbach y la fiabilidad test-retest mediante correlaciones de Pearson. Resultados: Se obtuvo buena consistencia interna, con valores de alfa de Cronbach mayores de 0,7 en tres de las cuatro subescalas que forman el test. En la restante se consiguió un valor de 0,68. En cuanto a la fiabilidad, se obtuvieron correlaciones de Pearson cercanas o superiores a 0,7 para todas las subescalas. Conclusiones: El PSAPQ traducido y validado al idioma español muestra muy buenos resultados psicométricos respecto al original, por lo que se puede afirmar que se ha obtenido un instrumento adecuado para medir objetivamente uno de los posibles factores de riesgo de accidentes infantiles.

Background: A favorable effect over development of degenerative diseases is derived of an adecuate intake of fruit and vegetables, mainly due to their antioxidant compounds Objectives: The goal of this study was to test the effect in vivo over oxidant status and inflammation in obese children of a novel food product made of dried apples enriched with mandarin juice by vacuum impregnation. Methods: A four-week intervention study was conducted in 41 obese children (> 2 standard deviation score-body mass index). Participants were instructed to follow their usual diet supplemented with 40 g/day of the developed product. Anthropometric parameters were determined including body mass index, waist circumference and estimations of body fat percentage using bioelectrical impedance. Dietary intake was assessed by questionnaire. Metabolic risk factors (blood pressure, lipid profile, glucose and insulin resistance) were recorded. To determine oxidant status, plasma total antioxidant capacity and 8-hydroxydeoxyguanosine, as marker of oxidative damage to DNA, were investigated. High-sensitive C-reactive protein, tumor necrosis factor-α, and interleukins 6 and 1-α were measured as inflammatory markers. Measurements were collected at baseline and at the end of the intervention period. Results: Significant improvement in systolic blood pressure and lipid profile after intervention period was noted. A significant increase in the antioxidant capacity of plasma (ABTS and FRAP assays) and reductions in DNA oxidative damage and inflammatory markers were also found. Conclusion: Overall, adding the product to the diet contributes to ameliorate oxidant and inflammatory status in obese children and several risk factors for atherosclerosis.

Antecedentes: Una adecuada ingesta de vegetales previene el desarrollo de enfermedades degenerativas, principalmente debido a sus compuestos antioxidantes. Objetivo: Evaluamos el efecto in vivo en los niños obesos de un nuevo producto alimenticio hecho de manzanas deshidratadas enriquecidas con zumo de mandarina mediante impregnación a vacío. Métodos: Estudio prospectivo longitudinal de cuatro semanas de duración. Se estudiaron 41 niños obesos que suplementaron su dieta habitual con 40 g/día del producto desarrollado. Se determinaron parámetros antropométricos (índice de masa corporal, circunferencia de la cintura) y estimación de la de grasa corporal con impedancia bioeléctrica. La ingesta dietética se evaluó por cuestionario. Se registraron factores de riesgo metabólico (presión sanguínea, perfil lipídico, glucosa y resistencia insulínica). El estado oxidante se investigó mediante la capacidad antioxidante total del plasma y la 8-hydroxideoxiguanosina (marcador de daño oxidativo al ADN) y como marcadores de inflamación valoramos la proteína C-reactiva ultrasensible, el factor de necrosis tumoral-α y las interleukinas 6 y 1-α. Las mediciones se recogieron al inicio y al final del período de intervención. Resultados: Encontramos una mejoría significativa en la presión arterial sistólica y en el perfil lipídico después del período de intervención. Igualmente demostramos un aumento significativo de la capacidad antioxidante del plasma, una reducción del daño oxidativo del ADN y de los marcadores inflamatorios. Conclusión: La adición a la dieta del producto elaborado con manzana deshidratada, y enriquecido con zumo de mandarina mediante impregnación al vacío, contribuye a mejorar el estado oxidante e inflamatorio en los niños obesos, así como diversos factores de riesgo cardiometabólico.
