SUMMARY Aldosterone-secreting adrenocortical carcinomas (ACCs) are rare and usually present as large tumors. The only potentially curative treatment for ACC is surgical resection. However, surgery may be unfeasible in some patients who have multiple comorbidities or decline the procedure. We describe herein the case of a 44-year-old man with aldosterone-secreting ACC who declined surgery because of religious convictions. As a Jehovah’s Witness, the patient was concerned about requiring blood transfusion during surgery. Treatment with mitotane was started but interrupted due to hepatotoxicity. Subsequently, the patient was successfully treated with stereotactic ablative radiotherapy (SABR). After SABR, the patient had progressive improvement of pain, reduction in antihypertensive drugs, control of blood pressure and hypokalemia, normalization of serum aldosterone and renin levels, and reduction in tumor size and weight. To our knowledge, this is the first report of a patient with a pure aldosterone-secreting ACC who received SABR. The patient’s response was substantial, showing that SABR could be considered as an alternative local treatment for aldosterone-secreting ACCs when surgery is unfeasible. ABSTRACT Background: This study aimed to evaluate the effectiveness and safety of direct-acting antivirals (DAAs) for hepatitis C treatment by measuring sustained virologic response (SVR) and serious adverse events to help design effective interventions for reducing disease prevalence. Methods: This was a retrospective, observational, real-life study of patients with chronic hepatitis C receiving DAA treatment in the state of Ceará, Brazil. Data were collected in REDCap and analyzed using R® software by the Student's t, chi-square, and Fisher’s exact tests, with a significance level of 5%. Results: In this study, 1075 patients who were diagnosed with hepatitis C infection between October 2015 and October 2023 were included. The mean age of the participants was 56.6 ± 11 years and 60.2% were men. The sample included 51 HIV-infected patients (6.6%), 166 (15,4%) liver transplant recipients, 34 (3,1%) kidney transplant recipients, and 446 patients with cirrhosis (41.4%). The overall SVR rate was 96.4%. The sofosbuvir/daclatasvir/ribavirin regimen used in 354 (32.9%) patients achieved an SVR of 96%. The cure rate was 96.5%, with a lower SVR in patients with cirrhosis (93.4%) than in those with less severe fibrosis (97.9%) (p=0.0015). Serious adverse events associated with ribavirin use occurred in 3.5% of patients. Conclusions: DAA treatment for hepatitis C achieved SVR in real life in all patient profiles, including transplant recipients, HIV carriers, and patients with cirrhosis. Resumo Este estudo investigou o desenvolvimento humano de oito municípios cearenses rurais que possuem o Sistema Integrado de Saneamento Rural (SISAR) e 170 que não o possuem. Desenvolveu-se um estudo a partir de Indicadores de Desenvolvimento Humano Municipal (IDHM) que constam na plataforma Atlas do Desenvolvimento Humano, extraindo-se dados dos Censos Demográficos de 1991, 2000 e 2010. Contou-se com o auxílio do software R para realização das análises estatísticas. Os resultados encontrados para o IDHM geral mostraram uma diferença significativa entre as médias dos dois grupos, o que não ocorreu quando analisadas as dimensões renda, longevidade e educação. Em suma, verificou-se que, no geral, a implantação de sistemas de abastecimento de água contribui para o aumento dos níveis de desenvolvimento humano, mas, no que se refere às dimensões mais específicas, essa premissa não foi estatisticamente confirmada.

The study was developed based on the Municipal Human Development Indicators (MHDIs), which feature in the Atlas of Human Development, by drawing data from the 1991, 2000 and 2010 Demographic Censuses. The R software was used to carry out the statistical analyses. The results found for the general MHDI showed a significant difference between the means of the two groups, which did not occur when the income, longevity, and education dimensions were analyzed. In sum, it was verified that, in general, implementing water supply systems contributes to increasing human development levels, but as regards the more specific dimensions, this premise was not statistically confirmed.

RESUMO INTRODUÇÃO: Interação medicamentosa é uma importante causa de morbidade mundial. Apresenta especial importância em pacientes oncológicos, pois esses frequentemente estão em uso de polifarmácia, podendo haver interações entre os medicamentos e os quimioterápicos utilizados. OBJETIVO: Avaliar a interação medicamentosa entre a quimioterapia e outros medicamentos em pacientes oncológicos. MÉTODOS: Estudo transversal realizado em serviço ambulatorial de oncologia de um hospital público terciário. Foram incluídos 235 pacientes, identificando-se quais medicamentos eram utilizados por eles. Por meio do auxílio do banco de dados do MedScape e Epocrates, avaliaram-se as interações entre as medicações e os quimioterápicos, definindo sua frequência e dividindo sua gravidade da interação em leve, monitorização próxima e grave. RESULTADOS: Do total estudado, 161 pacientes apresentavam alguma interação medicamentosa, sendo nove tipos de interações leves, 23 tipos de interações com necessidade de monitorização próxima e dois tipos de interações graves. As interações mais frequentes foram entre fluoracil e leucovorin (32 casos) e ciclofosfamida e doxorrubicina (19 casos). As interações sérias foram entre aspirina e pemetrexed; e leucovorim e bactrim. CONCLUSÃO: No presente trabalho, interações medicamentosas foram frequentes, incluindo interações graves com potencial aumento de morbimortalidade. Assim, faz-se necessário que oncologistas tracem um plano terapêutico levando em consideração as possíveis interações medicamentosas entre a quimioterapia prescrita e demais medicações em uso pelos pacientes.

SUMMARY INTRODUCTION: Drug interaction is an important cause of global morbidity. It is of particular importance in cancer patients since they are often in use of polypharmacy, related to interactions between the drugs and the chemotherapeutics used. OBJECTIVE: To evaluate the drug interaction between chemotherapy and other drugs in cancer patients. METHODS: a cross-sectional study carried out in the outpatient oncology department of a public tertiary hospital. Two hundred thirty-five patients were included, and the drugs they were using were identified. Using the MedScape and Epocrates database, we evaluated the interactions between medications and chemotherapy by defining their frequency and dividing their severity from interaction into mild, close monitoring necessity and severe. RESULTS: 161 patients had some drug interaction. We identified 9 types of mild interactions, 23 types of interactions with close monitoring necessity, and 2 types of serious interactions. The most frequent interactions were between fluorouracil and leucovorin (32 cases) and cyclophosphamide and doxorubicin (19 cases). Serious interactions were between aspirin and pemetrexed; and leucovorin and Bactrim. CONCLUSION: In the present study, drug interactions were frequent, including serious interactions with a potential increase in morbidity and mortality. Thus, it is necessary for oncologists to draw up a therapeutic plan considering potential interactions between prescribed chemotherapy and current medications in use by patients.

Crianças com dificuldades na realização de tarefas rotineiras que requerem habilidades motoras podem ter problemas específicos de coordenação motora, atualmente denominados Transtorno do Desenvolvimento da Coordenação (TDC). No Brasil esse diagnóstico ainda é pouco comum, embora seja muito utilizado na literatura internacional. O objetivo deste estudo foi investigar a percepção de mães brasileiras sobre o desempenho nas atividades diárias de crianças com transtorno da coordenação motora. Foram realizadas cinco entrevistas, seguidas por análise de conteúdo. Os relatos evidenciaram que todas as informantes percebem alguma dificuldade de seus filhos na realização de atividades cotidianas e na relação com os colegas, sendo essas dificuldades acentuadas principalmente pela entrada na escola. Os relatos também destacaram o desconhecimento sobre os problemas de coordenação motora e possibilidades de auxílio existentes, apontando para a necessidade de mais informação e maior valorização das percepções maternas

Children who have difficulty doing daily tasks that require motor skills may have specific motor coordination problems, known nowadays as Developmental Coordination Disorder (DCD). In Brazil, this diagnosis is used very seldom although it is much used in the international literature. The aim of this study was to investigate the perception of Brazilian mothers concerning the daily performance of children with motor coordination disorder. Five interviews were conducted and submitted to content analysis. The narratives showed that all informants perceive difficulties in the performance of daily activities in their children and in the relationship with peers, and these difficulties are more evident with the entrance in school. These findings highlight the unawareness about motor coordination problems and opportunities for getting help, as well as reinforce the need for more information and to value mother's perceptions.

Niños con dificultades en el desempeño de las tareas rutinarias que requieren habilidades motoras pueden tener problemas específicos de coordinación, actualmente conocidos como trastorno de desarrollo de la coordinación (TDC). En Brasil este diagnóstico es infrecuente, aunque se usa ampliamente en la literatura internacional. El objetivo de este estudio fue investigar la percepción de las madres brasileñas en el desempeño diario de niños con trastorno de la coordinación motora. Cinco entrevistas se llevaron a cabo, seguido de análisis de contenido. Los informes muestran que los encuestados perciben cierta dificultad a sus hijos en la realización de las actividades diarias y las relaciones con los colegas, y estas dificultades acentuadas principalmente por la entrada de la escuela. Los informes también destacó la falta de conocimiento acerca de los problemas de coordinación y las posibilidades de ayuda existentes, apuntando la necesidad de más información y una mayor apreciación de las percepciones maternas.
