ABSTRACT To describe the effects of the 5% lidocaine patch on the symptomatic relief of Peruvian patients with postherpetic neuralgia. Case series report of adult patients with post-herpetic neuralgia admitted to and treated at the Air Force Central Hospital in Lima. Symptomatic relief was defined as the difference between the specified pain score before and after patch treatment. The pain score was evaluated by the visual analogue scale and the Pain Assessment in Advanced Dementia scale, for patients with dementia. Twenty-two patients with postherpetic neuralgia (mean age 74.4 ( 12.40 years) were recruited, 17 of whom began topical therapy with lidocaine following oral systemic treatment. The main reasons for the decision to change were a persistent pain and the adverse effects of the preceding therapy. A statistically significant decrease in pain symptoms was observed after the use of the lidocaine patch (p<0.01), and no adverse reactions to the patch were reported during the follow-up period. A statistically significant pain reduction found after the use of topical treatment. New studies that overcome the limitations of this and others previously published, are suggested.
RESUMEN Describir los efectos del parche de lidocaína al 5 % en el alivio sintomático de pacientes peruanos con neuralgia posherpética Reporte de una serie de casos de pacientes adultos con neuralgia posherpética, admitidos y tratados en el Hospital Central de la Fuerza Aérea de Lima, Perú. El alivio sintomático del dolor por neuralgia posherpética fue definido como la diferencia entre la puntuación del dolor especificado antes y después del tratamiento con el parche. La puntuación fue evaluada mediante la escala visual analógica y la escala Pain Assessment in Advanced Dementia para los pacientes con demencia. Se recolectaron 22 casos de pacientes con neuralgia posherpética (edad media de 74,4 ( 12,40 años), 17 de los cuales iniciaron la terapia tópica con lidocaína luego del tratamiento sistémico por vía oral. Las principales razones para el cambio fueron dolor persistente y efectos adversos de la terapia precedente. Se observó una disminución estadísticamente significativa del dolor y de los síntomas colaterales después del uso del parche de lidocaína (p < 0,01). No se reportó reacción adversa alguna al final del seguimiento. Se encontró reducción estadísticamente significativa del dolor posterior al inicio del tratamiento tópico. Se sugiere realizar nuevos estudios que superen las limitaciones de este y de otros previamente publicados.

Resumen El grado de calidad de piezas fabricadas por inyección de termoplásticos se puede establecer por el peso del producto, apariencia y mínimos defectos. Adicionalmente, las condiciones del proceso de inyección pueden inducir un efecto en el desempeño mecánico de las piezas inyectadas, donde los esfuerzos residuales de la pieza inyectada generados en el procesamiento pueden promover la formación de grietas o fallas prematuras del producto durante su uso al aplicarle una carga o fuerza externa. El propósito del presente trabajo es evaluar el efecto que genera el procesamiento por inyección en una pieza inyectada en el desempeño mecánico de esta. Para tal fin, se inyectaron probetas bajo diferentes condiciones de proceso y posteriormente estas fueron sometidas a pruebas mecánicas de flexión, algunas piezas fueron llevadas a un tratamiento térmico de recocido “annealing”, con el fin de obtener una relajación molecular que permita reducir sus esfuerzos internos o residuales generados durante el proceso de inyección. Adicionalmente, se hizo una evaluación cualitativa de la distribución de esfuerzos residuales en las piezas inyectadas mediante la técnica de fotoelasticidad, para complementar y validar los resultados de las pruebas mecánicas. Se halló en los resultados de las muestras inyectadas a temperatura de molde de 50 ºC una mayor liberación de esfuerzos residuales de acuerdo con las imágenes de fotoelasticidad analizadas y un mayor porcentaje de incremento en la resistencia a la flexión de estas piezas al aplicar el tratamiento térmico. Lo que podría sugerir una mayor generación de esfuerzos residuales a esta temperatura de molde con respecto a la temperatura de molde recomendada de 80 ºC. El efecto de tratamiento térmico mejora significativamente el desempeño mecánico de todas las piezas evaluadas a las diferentes condiciones de proceso.
Abstract The quality of parts manufactured by thermoplastics injection molding can be evaluated based on product weight, appearance, and presence of defects. Furthermore, the conditions of the injection process can significantly influence the mechanical performance of the molded parts. Residual stresses generated during plastic processing may contribute to the formation of cracks or premature failures when the product is subjected to external loads or forces. This study aims to evaluate the effects of the injection process on an injected part and how they affect its mechanical performance. For this purpose, specimens were injected under different process conditions and, subsequently, employed in a. Some parts were also subjected to annealing (a thermal treatment) to induce molecular relaxation, which helps to reduce internal or residual stresses generated during the injection process. Additionally, a qualitative evaluation of the distribution of residual stresses in the injected parts was conducted using the s technique to complement and validate the results of the mechanical tests. The results of the specimens injected at a mold temperature of 50°C showed a greater release of residual stresses, as indicated in photoelasticity images analyzed here, and a noticeable increase in flexural strength if the heat treatment had been applied. This suggests that more residual stresses are produced at 50°C than at the recommended mold temperature of 80°C. The heat treatment significantly improved the mechanical performance of all the parts injected in this study under different processing conditions.

RESUMO Processo de salinização do solo afeta negativamente o crescimento e o rendimento de culturas economicamente valiosas, como a soja. Com o objetivo de minimizar os danos causados pelo estresse salino e melhorar o desempenho das culturas, novas tecnologias estão sendo desenvolvidas. Neste contexto, o uso de bioestimulantes na agricultura pode oferecer uma alternativa para mitigar os efeitos deletérios da salinidade. O extrato da alga Ascophyllum nodosum e substâncias húmicas (ácidos fúlvicos) são comumente empregados. Portanto, o estudo teve como objetivo avaliar a ação de um bioestimulante composto pelo extrato da alga marinha A. nodosum e ácidos fúlvicos aplicados em diferentes estádios fenológicos sobre parâmetros biométricos, bioquímicos e fisiológicos da soja (Glycine max) submetida à irrigação com água salina. O delineamento experimental utilizado foi inteiramente casualizado em esquema fatorial 3 x 2 + 2, referente a três aplicações do bioestimulante em diferentes estádios fenológicos da soja: V3, V3R1 e V3R1R4, dois níveis de condutividade elétrica da água de irrigação (CEa): 0,5 (S0) e 5,0 dS m-1 (S1), dois controles adicionais, um com controle positivo (PC) e outro com controle negativo (NC). O experimento contou com cinco repetições e uma planta por parcela. Os resultados deste estudo sugerem que a aplicação do bioestimulante pode ser uma alternativa na mitigação dos efeitos nocivos do estresse salino em soja. ABSTRACT Soil salinization negatively affects the growth and yield of economically valuable crops such as soybeans. New technologies are being developed to minimize the damage caused by saline stress and improve crop performance. Therefore, the use of biostimulants in agriculture may offer an alternative method to mitigate the deleterious effects of salinity. Extracts of the seaweed, Ascophyllum nodosum and humic substances (fulvic acids) are commonly used. Therefore, this study aimed to evaluate the action of a biostimulant composed of A. nodosum seaweed extract and fulvic acid applied at different phenological stages on the biometric, biochemical, and physiological parameters of soybeans (Glycine max) subjected to irrigation with saline water. The experimental design used was completely randomized in a 3 × 2 + 2 factorial scheme, referring to three applications of the biostimulant at different soybean phenological stages: V3, V3R1, and V3R1R4, two levels of electrical conductivity of irrigation water (ECw): 0.5 (S0) and 5.0 dS m-1 (S1), two additional controls, one with a positive control (PC) and the other with a negative control (NC). Abstract Beyond the cultural and historical richness that identifies Cuetzalan and Tlatlauquitepec, there is the biological diversity it houses. The dominant type of vegetation is the cloud forest, which is at risk of disappearing due to the transformation to land cultivation, coffee plantations, and tourism, causing the extirpation of the species that live there, such as ferns, lycopods, and lichens. This study aimed to document the lichen richness species of cloud forest; we conducted field trips and opportunistic non-quantitative recollections. We identified 110 lichen species in 52 genera. Most of the species were crustose lichens (38%) and foliose lichens (51%). The genus Parmotrema was mesodiverse with 23 species, while 33 genera were monospecific in presenting only one species each, such as Bacidia, Baeomyces, Bulbothrix, Dermatocarpon, and Teloschistes. We report to Allographa chlorocarpa, A. rufopallida, Leucodermia guzmaniana, and Pyxine pyxinoides that had not previously been recorded in the Puebla state. Instead of the results obtained, there is still much work to inventory the lichen thoroughly. It is necessary to design effective strategies in which lichens are included in priority areas for conservation since they are an important component of ecosystems.
Resumen Más allá de la riqueza cultural e histórica que identifica a Cuetzalan y Tlatlauquitepec, está la diversidad biológica que alberga. El tipo de vegetación dominante es el bosque mesófilo de montaña, el cual se encuentra en riesgo de desaparecer debido a la transformación a tierras de cultivo, plantíos de café y al turismo, lo que provoca la extirpación de las especies que ahí habitan, tales como helechos, licopodios y líquenes. El objetivo de este estudio fue documentar la riqueza liquénica del bosque mesófilo de montaña para lo cual se hicieron recorridos de campo y una recolecta oportunista no cuantitativa. Se identificaron 110 especies de líquenes en 52 géneros. La mayoría de las especies fueron líquenes costrosos (38%) y líquenes foliosos (51% líquenes). El género Parmotrema fue mesodiverso con 23 especies, mientras que 33 géneros fueron monoespecíficos al presentar cada uno de ellos una sola especie, tal es el caso de Bacidia, Baeomyces, Bulbothrix, Dermatocarpon, y Teloschistes. Se reporta a Allographa chlorocarpa, A. rufopallida, Leucodermia guzmaniana y a Pyxine pyxinoides como nuevos registros para el estado de Puebla. A pesar de los resultados obtenidos, aún falta mucho por hacer para completar el inventario de los líquenes. Es necesario diseñar estrategias efectivas en las que se incluya en las áreas prioritarias para la conservación a los líquenes, ya que son un componente importante de los ecosistemas.

Resumo Uma lista de peixes recentes e fósseis da Ordem Characiformes é apresentada e acredita-se que a lista esteja completa até 2022. Ao todo, 47 colaboradores verificaram e forneceram informações sobre táxons de sua especialidade. A lista está organizada em ordem alfabética por família e em seguida gênero e inclui todos os sinônimos disponíveis. De um total de 3.087 nomes de grupos de espécies e 527 nomes de grupos de gêneros, 2.334 espécies são atualmente tratadas como válidas e atribuídas a 301 gêneros e 24 famílias. Este total inclui 25 espécies fósseis, das quais 21 são incluídas em oito famílias recentes e quatro são Incertae sedis em Characiformes, mas não inclui cinco espécies baseadas em fósseis que foram incorretamente consideradas como Characiformes. O status de 79 espécies nominais permanece não resolvido. Quando há duas hipóteses concorrentes sobre a validade de um nome, uma delas é escolhida e a interpretação alternativa é fornecida na seção de comentários do nome em questão. Abstract A checklist of recent and fossil fishes of the Order Characiformes is presented herein and believed to be complete through 2022. A total of 47 collaborators checked and provided information about their taxa of expertise. The list is arranged in alphabetical order by family and then genus and includes all available synonyms. From a total of 3,087 species group names and 527 genus group names, 2,334 species are currently treated as valid, and assigned to 301 genera and 24 families. This total includes 25 species known only from the fossil record, of which 21 are assigned to eight extant families and four are Incertae sedis in Characiformes, but does not include five species based on fossil which were wrongly treated in Characiformes. The status of 79 nominal species remains unresolved. When there are two competing hypotheses about the validity of a name, one of them is chosen and the alternate interpretation is provided in the remark section of that name. Distributional summaries are presented as numbers that correspond to areas for which there is a record of occurrence of the species. RESUMO Objetivo: Determinar as indicações cirúrgicas de enxertia óssea da glenoide associadas aos melhores arcos de movimento no pós-operatório. Métodos: De acordo com o Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA), 14 artigos de um total de 1.567, selecionados em busca eletrônica, foram escolhidos para a revisão sistemática. Os estudos incluídos foram subdivididos de acordo com os critérios de indicação da cirurgia: indicações somente radiológicas (grupo R), indicações radiológicas associadas a indicações clínicas (grupo R + C) e indicações artroscópicas (grupo A). Os dados avaliados foram os arcos de movimento do ombro. Resultados: Em relação aos arcos de movimento, os grupos que apresentaram a maior quantidade de resultados estatisticamente positivos foram o A – parâmetros elevação, perda de rotação lateral em adução e rotação medial em abdução – e o R – parâmetros rotação lateral em adução e perda de rotação lateral em adução. O grupo R + C apresentou a maior quantidade de resultados estatisticamente negativos nos parâmetros elevação, rotação lateral em abdução, perda de rotação lateral em adução e rotação medial em abdução. ABSTRACT Objective: to determine the surgical indications for glenoid bone grafting associated with better postoperative ranges of motion. Methods: This systematic review was conducted according to PRISMA. The included studies were subdivided according to the criteria used to indicate glenoid bone graft surgery: group for radiological indications only (Group R), group for radiological indications associated with clinical indications (Group R + C), and group for arthroscopic indications (Group A). The extracted and evaluated data were the range of motion of the shoulder. Results: in the electronic search conducted in October 2022, 1567 articles were selected. After applying the inclusion criteria, 14 articles were selected for the systematic review. Regarding the ranges of motion, group A had the highest number of statistically positive results together with group R. Group A showed positive results in elevation parameters, loss of lateral rotation in adduction, and medial rotation in abduction. Group R showed positive results in lateral rotation in adduction and loss of lateral rotation in adduction. On the other hand, Group R + C was the one that presented the highest number of statistically negative results, in the following parameters: elevation, lateral rotation in abduction, loss of lateral rotation in adduction, and medial rotation in abduction. Conclusion: the subgroups presented variable results in the evaluated parameters; however, the groups with arthroscopic and radiological indications showed the highest number of positive results, with the latter group showing the best results regarding lateral rotation. SUMMARY Aldosterone-secreting adrenocortical carcinomas (ACCs) are rare and usually present as large tumors. The only potentially curative treatment for ACC is surgical resection. However, surgery may be unfeasible in some patients who have multiple comorbidities or decline the procedure. We describe herein the case of a 44-year-old man with aldosterone-secreting ACC who declined surgery because of religious convictions. As a Jehovah’s Witness, the patient was concerned about requiring blood transfusion during surgery. Treatment with mitotane was started but interrupted due to hepatotoxicity. Subsequently, the patient was successfully treated with stereotactic ablative radiotherapy (SABR). After SABR, the patient had progressive improvement of pain, reduction in antihypertensive drugs, control of blood pressure and hypokalemia, normalization of serum aldosterone and renin levels, and reduction in tumor size and weight. To our knowledge, this is the first report of a patient with a pure aldosterone-secreting ACC who received SABR. The patient’s response was substantial, showing that SABR could be considered as an alternative local treatment for aldosterone-secreting ACCs when surgery is unfeasible. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract The effect of manufacturing production on Gross Domestic Product (GDP) growth and on total and manufacturing productivity, as well as the dynamic effect created in the economy by the industrial sector, under Kaldor’s view, is a topic under debate due to the patterns presented in deindustrialisation and industrial productivity in developed countries. However, developing countries such as some in the Latin American region have presented different patterns. This article aims to study the validity of Kaldor’s Laws in the largest economies in Latin America and compare results with the case of European economies. For this, an econometric methodology of static and dynamic panel data is used in a sample of 12 countries in the time period between 1992 and 2021. The study yields particular results such as a small coefficient between the growth of manufacturing and its productivity, but a dynamic effect among the two variables.
Resumen El efecto de la producción manufacturera sobre el crecimiento del producto interno bruto (PIB) y sobre la productividad total y manufacturera, así como el efecto dinámico creado en la economía por el sector industrial, en la visión de Kaldor, es un tema en debate debido a los patrones presentados en la desindustrialización y la productividad industrial en países desarrollados. Sin embargo, los países en desarrollo, como algunos de América Latina, han presentado patrones diferentes. Este artículo tiene como objetivo estudiar la validez de las leyes de Kaldor en las economías más grandes de América Latina y comparar los resultados con el caso de economías de Europa. Para ello, utilizamos una metodología econométrica de datos de panel estáticos y dinámicos en una muestra de 12 países en el periodo de tiempo comprendido entre 1992 y 2021. El estudio arroja resultados particulares, como un pequeño coeficiente entre el crecimiento de la manufactura y su productividad, pero un efecto dinámico entre las dos variables.

ABSTRACT INTRODUCTION: Neuromyelitis Optica Spectrum Disorders (NMOSD) is a group of inflammatory, autoimmune, and demyelinating disorders. Its hallmark behavior is characterized by recurrent episodes of optic neuritis and longitudinally extensive transverse myelitis, among other clinical manifestations. Chronic therapy is based primarily in immunosuppressive therapies such as azathioprine (AZA), mycophenolate mofetil (MMF), or rituximab (RTX). The goal of this study is to perform a comparative analysis of response rates to chronic treatment with either AZA or RTX. MATERIALS AND METHODS: A retrospective observational analytic study was designed with an initial cohort of 69 patients with a diagnosis of NMOSD. After application of the inclusion and exclusion criteria a total of 59 patients were finally included in the analysis. RESULTS: The RTX group had an improved functional status when compared to the AZA group; in the latter this feature worsened after a one-year follow-up. There was also a comparable safety profile between the two groups with a significantly greater adherence to RTX regimes. DISCUSSION: The findings of the current study as to the benefits of RTX in comparison to AZA are similar to the results of previous studies. CONCLUSION: These results favor the use of RTX as maintenance treatment of NMOSD, because of its greater benefit mainly in the improvement in functional status of patients, as well as a greater adherence to treatment.
RESUMEN INTRODUCCIÓN: El espectro de trastornos de neuromielitis óptica (NMOSD) es un grupo de enfermedades desmielinizantes, inflamatorias y autoinmunes, caracterizadas por episodios recurrentes de neuritis óptica y mielitis transversa longitudinal extensa, entre otras manifestaciones clínicas. Su tratamiento crónico se basa en el uso de terapias inmunosupresoras como azatioprina (AZA), micofenolato mofetilo (MFM) o rituximab (RTX). El objetivo del presente estudio es realizar un análisis comparativo de la respuesta al tratamiento con AZA o RTX. MATERIALES Y MÉTODOS: Se realizó un estudio observacional, analítico, retrospectivo, en el cual se incluyeron inicialmente 69 pacientes con diagnóstico confirmado de NMOSD. Tras aplicar los criterios de inclusión y exclusión 59 pacientes fueron incluidos en el análisis final. RESULTADOS: En el grupo de RTX se evidenció una mejoría importante en el estado funcional en comparación con el grupo de AZA, en el que se vio un empeoramiento de este al año de seguimiento. El perfil de seguridad fue similar entre ambos grupos, con una adherencia significativamente superior en el grupo de RTX. DISCUSIÓN: Los hallazgos del presente estudio respecto a las ventajas del uso de RTX sobre AZA se encuentran en concordancia con resultados de estudios previos reportados en la literatura. CONCLUSIONES: Los resultados respaldan el uso de RTX sobre AZA como terapia de mantenimiento para pacientes con NMOSD, al estar asociado principalmente con una mejoría notable en la funcionalidad de los pacientes, al igual que una mayor adherencia al tratamiento.

Outcomes of kidney autotransplantation technique in the treatment of different vascular disorders: an updated narrative review

Abstract Introduction: Kidney autotransplantation (KAT) is a versatile surgical technique used to treat multiple diseases affecting the kidney. This procedure allows the reconstruction of the renal artery or vein with optimal surgical exposure reducing warm ischemia time compared to open direct repair. Although infrequently reported nowadays and despite the growing use of endovascular treatments, KAT still has a place in treating complex kidney vascular conditions. Methods: We performed a literature review using the MEDLINE®, SCOPUS, and Web of Science databases with the combination of the terms: “kidney autotransplantation,” “renal artery,” “renal vein,” “aorta,” “aneurysm,” and “nutcracker syndrome.” All relevant English-language studies published between January 1990 and June 2022 reporting the outcomes of the KAT technique in vascular disorders were included. Results: A total of 32 articles were included. Ex-vivo repair and KAT is widely described in treating renal artery aneurysms (RAA), with a reported technical success rate of 99.9%, a complication rate ranging from 6.9 to 9.4%, and a graft loss rate of up to 4.1%. The use of the laparoscopic technique for harvesting the kidney was variable throughout the series published, and heterotopic KAT was performed in most treated patients. Ex-vivo repair and KAT was successfully used after failed endovascular therapy and is a viable option in women of childbearing age with bilateral complex RAA. Concerning renal artery stenosis associated with different etiologies (fibromuscular dysplasia, Takayasu's arteritis, and neurofibromatosis type 1), significant reduction of blood pressure and the number of anti-hypertensive drugs have been reported in most patients after KAT. Results of arterial hypertension improvement associated with RAA after KAT are more variable. The use of KAT in treating nutcracker syndrome is reported with good results, even after left renal vein transposition or venous stenting failure. Other reports exist on the successful use of KAT for treating renal vein aneurysms and as an adjunct in open surgical or hybrid aortic reconstructions. Conclusion: Renal autotransplantation is a useful technique in the treatment of kidney vascular disorders with good overall results reported in the literature. It may be an essential adjunct in open surgical or hybrid treatments for abdominal and thoracoabdominal aortic diseases. Defining the patients who would benefit from this technique as a preferential treatment choice in different renal vessel disorders would be necessary.

Abstract Objectives The present study aimed to evaluate the measurement invariance of a general measure of the perception of governmental responses to COVID-|19 (COVID-SCORE-10) in the general population of 13 Latin American countries. Methods A total of 5780 individuals from 13 Latin American and Caribbean countries selected by non-probabilistic snowball sampling participated. A confirmatory factor analysis was performed and the alignment method was used to evaluate invariance. Additionally, a graded response model was used for the assessment of item characteristics. Results The results indicate that there is approximate measurement invariance of the COVID-SCORE-10 among the participating countries. Furthermore, IRT results suggest that the COVID-SCORE-10 measures with good psychometric ability a broad spectrum of the construct assessed, especially around average levels. Comparison of COVID-SCORE-10 scores indicated that participants from Cuba, Uruguay and El Salvador had the most positive perceptions of government actions to address the pandemic. Thus, the underlying construct of perception of government actions was equivalent in all countries. Conclusion The results show the importance of initially establishing the fundamental measurement properties and MI before inferring the cross-cultural universality of the construct to be measured. Abstract This study aims to explore the behavior of mechanical stimuli and their effect on growth plate shape under different physiological scenarios and loading conditions. A linear elastic, isotropic, and homogeneous model was implemented to model the epiphyseal progressive ossification and the development of the secondary ossification center. Results shed light on the role of mechanical stimulus, suggesting that maximum shear stress, hydrostatic stress, and von Mises stress may contribute to the morphological changes of the growth plate. This model is a useful tool to improve orthopedic therapies focused on pathologies that imply abnormal bone growth under abnormal mechanical conditions.

Resumen La detección de bordes toma importancia en los sistemas de procesamiento de imágenes para el diagnóstico asistido por ordenador, donde se analizan los cambios bruscos en la intensidad de los píxeles para obtener información rápida y precisa sobre las regiones de interés para el especialista. Se desarrolló un método para el realce de caracteristicas y detección de bordes en imágenes médicas utilizando procesamiento de imágenes analizando el histograma de distribución de píxeles y la operación de gradiente morfológico. Se utilizaron imágenes del conjunto de datos MINI MIAS y del conjunto de datos COVID-CT. El método se basa en procesamiento de imágenes y se aplica a las imágenes de mamografía y TAC de tórax, donde los pasos de filtrado de desenfoque se acompañan de filtrado de gradiente morfológico, además de obtener el umbral para detectar el borde mediante el análisis del punto de máxima concentración de píxeles según el histograma de distribución. El procesamiento se presenta en una interfaz gráfica de usuario desarrollada en lenguaje Python. El método se valida mediante la comparación con otras técnicas de detección de bordes como el Algoritmo Canny, y con métodos de aprendizaje profundo como el Holistically-Nested Edge Detection. El método propuesto mejora la calidad de la imagen tanto en mamografías como en TAC en comparación con otras técnicas. También presenta el mejor rendimiento teniendo en cuenta la detección de bordes internos y externos, así como un tiempo medio de respuesta de 1.054 segundos y 2.63 % de requerimiento de la Unidad Central de Procesamiento. El sistema desarrollado se presenta como una herramienta de apoyo para su uso en procesos de diagnóstico asistido por ordenador debido a su alta eficiencia en la detección de bordes.
Abstract Edge detection takes importance in image processing systems for computer-aided diagnosis, where sharp changes in pixel intensity are analyzed to obtain fast and accurate information about regions of interest to the specialist. A method for feature enhancement and edge detection in medical images was developed using image processing by analyzing the pixel distribution histogram and morphological gradient operation. Images from the MINI MIAS dataset and the COVID-CT dataset were used. The method is based on image processing and is applied to mammography and chest CT images, where blur filtering steps are accompanied by morphological gradient filtering, in addition to obtaining the threshold for edge detection by analyzing the point of maximum pixel concentration according to the distribution histogram. The processing is presented in a graphical user interface developed in Python language. The method is validated by comparison with other edge detection techniques such as the Canny Algorithm, and with deep learning methods such as Holistically-Nested Edge Detection. The proposed method improves image quality in both mammograms and CT scans compared to other techniques. It also presents the best performance considering internal and external edge detection, as well as an average response time of 1.054 seconds and 2.63 % of Central Processing Unit requirement. The developed system is presented as a support tool for use in computer-aided diagnosis processes due to its high efficiency in edge detection.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;
