Abstract Objective: To discuss the possible input function of artistic and cultural practices in the context of Harm Reduction (HR). Methods: Ethnographic research developed at the Ponto de Cultura of the Centro de Convivência "É de Lei", located in São Paulo-SP, which took place in two moments: in the first, in October 2016, participative observation of artistic and cultural activities was carried out. In the second, in November 2016, semi-structured interviews were carried out with professionals and regulars. The study identified and described artistic and cultural practices, as well as investigated their possible relationships with HR inputs in the production of the social bond. The discussion of the data was based on concepts at the interface between public health and psychoanalysis. Results and Discussion: The research characterized the input in HR as a promoter of a bond between the drug addict and whoever offers it with the potential to divert the subject from the established solitary relationship with the substance towards the creation of social ties that stimulates the exercise of their condition as a singular subject in culture. Conclusion: The study concluded that artistic and cultural practices can play the role of inputs in HR with regard to the establishment of social bonds.

Resumo Objetivo: Discutir a possível função de insumo das práticas artísticas e culturais no contexto da Redução de Danos (RD). Métodos: Pesquisa etnográfica desenvolvida no Ponto de Cultura do Centro de Convivência "É de Lei”, localizado em São Paulo-SP, em dois momentos: no primeiro, em outubro de 2016, realizou-se observação participante das atividades artísticas e culturais. No segundo, em novembro de 2016, realizaram-se entrevistas semiestruturadas com profissionais e frequentadores. O estudo identificou e descreveu práticas artísticas e culturais, bem como investigou suas possíveis relações com os insumos em RD na produção do laço social. A análise e a discussão dos dados foram embasadas por conceitos na interface entre a Saúde Coletiva e a Psicanálise. Resultados e Discussão: A pesquisa caracterizou o insumo em RD como promotor de vínculo entre o toxicômano e quem o oferta, com potencial para deslocar o sujeito da relação solitária estabelecida com a substância na direção dos laços sociais que possibilitam exercer sua condição de sujeito singular na cultura. Conclusão: O estudo considerou que as práticas artísticas e culturais podem exercer a função de insumos em RD no que concerne ao estabelecimento de laços sociais.

Resumo: A vigilância sentinela de síndrome gripal atua no Brasil identificando os vírus respiratórios de importância para a saúde pública circulantes no país, e começou a ser implementada em 2000. Recentemente, a pandemia de COVID-19 reforçou a importância da detecção precoce de novos vírus em circulação no território brasileiro. Assim, se faz oportuna uma análise do desenho da vigilância sentinela de síndrome gripal. Para tal, simulamos uma rede sentinela, identificando os municípios que fariam parte da rede segundo os critérios definidos no desenho da vigilância sentinela de síndrome gripal, e, a partir dos dados de casos testados de síndrome respiratória aguda grave (SRAG) de 2014 a 2019, sorteamos amostras para cada município sentinela por semana epidemiológica. O sorteio foi repetido mil vezes, obtendo-se a mediana e intervalo quantílico de 95% (IQ95%) da positividade para cada vírus por Unidade Federativa e semana epidemiológica. Segundo os critérios do desenho da vigilância sentinela de síndrome gripal, unidades sentinelas estariam em 64 municípios, distribuídas principalmente em capitais e suas zonas metropolitanas, o que preconizou 690 amostras semanais. O desenho apresentou boa sensibilidade (total de 91,65%, considerando o IQ95%) para a detecção qualitativa dos vírus respiratórios, mesmo os de baixa circulação. Porém, houve importante incerteza na estimativa quantitativa de positividade, chegando a, pelo menos, 20% em 11,34% das estimativas. Os resultados aqui apresentados visam auxiliar a avaliação e a atualização do desenho da rede sentinela. Estratégias para reduzir a incerteza nas estimativas de positividade precisam ser avaliadas, assim como a necessidade de maior cobertura espacial.

Abstract: The influenza-like illness (ILI) sentinel surveillance operates in Brazil to identify respiratory viruses of public health relevance circulating in the country and was first implemented in 2000. Recently, the COVID-19 pandemic reinforced the importance of early detection of the circulation of new viruses in Brazil. Therefore, an analysis of the design of the ILI sentinel surveillance is timely. To this end, we simulated a sentinel surveillance network, identifying the municipalities that would be part of the network according to the criteria defined in the design of the ILI sentinel surveillance and, based on data from tested cases of severe acute respiratory illness (SARI) from 2014 to 2019, we drew samples for each sentinel municipality per epidemiological week. The draw was performed 1,000 times, obtaining the median and 95% quantile interval (95%QI) of virus positivity by Federative Unit and epidemiological week. According to the ILI sentinel surveillance design criteria, sentinel units would be in 64 municipalities, distributed mainly in capitals and their metropolitan areas, recommending 690 weekly samples. The design showed good sensitivity (91.65% considering the 95%QI) for qualitatively detecting respiratory viruses, even those with low circulation. However, there was important uncertainty in the quantitative estimate of positivity, reaching at least 20% in 11.34% of estimates. The results presented here aim to assist in evaluating and updating the ILI sentinel surveillance design. Strategies to reduce uncertainty in positivity estimates need to be evaluated, as does the need for greater spatial coverage.

Resumen: La vigilancia centinela de la enfermedad tipo infuenza (ETI) funciona en Brasil para identificar los virus respiratorios de importancia para la salud pública que circulan en el país y comenzó a ser implementada en 2000. Recientemente, la pandemia de COVID-19 ha reforzado la importancia de la detección temprana de la circulación de nuevos virus en el territorio brasileño. Así, se hace oportuno un análisis del diseño de la vigilancia centinela de la ETI. Para ello, simulamos una red centinela identificando los municipios que formarían parte de la red según los criterios definidos en el diseño de la vigilancia centinela de la ETI y, a partir de los datos de casos testados de infección respiratoria aguda grave (IRAG) de 2014 a 2019, se extrajeron muestras para cada municipio centinela por semana epidemiológica. El sorteo se repitió 1.000 veces y se obtuvo la mediana y el intervalo cuantílico del 95% (IC95%) de la positividad por virus, por Unidad Federativa y semana epidemiológica. Según los criterios del diseño de la vigilancia centinela de la ETI, unidades centinelas estarían en 64 municipios, distribuidas principalmente en capitales y zonas metropolitanas de las capitales, preconizando 690 muestras semanales. El diseño presentó una buena sensibilidad (total de 91,65% considerando el IC95%) para la detección cualitativa de los virus respiratorios, incluso los de baja circulación. Sin embargo, hubo una importante incertidumbre en la estimación cuantitativa de la positividad, alcanzando al menos el 20% en el 11,34% de las estimaciones. Los resultados presentados aquí tienen como objetivo ayudar en la evaluación y actualización del diseño de la red centinela. Es necesario evaluar las estrategias para reducir la incertidumbre en las estimaciones de positividad, al igual que la necesidad de una mayor cobertura espacial.

RESUMO Objetivos: Analisar as notificações de morte encefálica (ME) recebidas pela Central de Transplantes (CET) do estado de Roraima (RR) de 2017 a 2021. Métodos: Trata-se de estudo transversal, quantitativo e observacional no qual foram analisadas todas as notificações recebidas pela CET no período de 2017 a 2021, resultando em 120 pacientes, dos seguintes centros de saúde: Hospital Geral de Roraima (HGR), Hospital Confederação Nacional das Cooperativas Médicas (UNIMED), Hospital da Criança Santo Antônio (HCSA), Hospital das Clínicas (HC) e Hospital Lotty Íris (HLI). Os dados foram coletados do sistema de gerenciamento de dados da central e repassados para a ficha de coleta desenvolvida pela pesquisadora. Resultados: Este estudo analisou 119 notificações após a exclusão de uma devido a dados conflitantes. Houve predominância do sexo masculino (58%), com trauma cranioencefálico sendo a causa mais frequente de ME. Ao todo, foram realizadas seis captações de órgãos, todas no HGR. Este estudo não encontrou associações significativas entre a captação de órgãos e sexo, idade, local de internação e ano. Conclusão: Há grandes desafios em relação à doação de órgãos em RR, com uma taxa de captação baixa ao longo de 5 anos. Apenas o HGR obteve sucesso em realizar captações de órgãos, diferentemente de outros hospitais notificadores. São necessárias melhorias, destacando a importância de se realizarem mais estudos para uma análise mais aprofundada dos dados. ABSTRACT Objectives: To analyze brain death (BD) notifications received by Central de Transplantes (CET) in the state of Roraima (RR) from 2017 to 2021. Methods: This is a cross-sectional, quantitative, and observational study, where all notifications received by CET from 2017 to 2021 were analyzed, resulting in 120 patients from health the following centers: Hospital Geral de Roraima (HGR), Hospital Confederação Nacional das Cooperativas Médicas (UNIMED), Hospital da Criança Santo Antônio (HCSA), Hospital das Clínicas (HC), and Hospital Lotty Íris (HLI). Resumo Sabe-se que deficiências de projeto no dimensionamento à força cortante são mais perigosas que as de flexão, pois rupturas por cisalhamento podem ocorrer de forma frágil e sem possibilidade de redistribuição dos esforços internos. Diferentemente do dimensionamento à flexão, o dimensionamento a solicitações cisalhantes por diferentes normas, podem gerar resultados significativamente distintos para um mesmo elemento, visto que os modelos de dimensionamento seguem em discussão por muitos anos. Este trabalho considera a avaliação do comportamento da ação das solicitações combinadas de flexão e força cortante em vigas de concreto armado para diferentes pares de momentos fletores e forças cortantes. Para tanto foi utilizada a verificação definida na ABNT NBR 6118, comparada a uma teoria mais aprimorada utilizada atualmente, a Teoria do Campo de Compressão Modificada (Modified Compression Field Theory - MCFT). Essa teoria é capaz de prever as relações entre carregamentos e deformações específicas, bem como a resistência à força cortante de seções com grande precisão, sendo parametrizada por diversos elementos ensaiados. Como a utilização dessa teoria não é prática para os cálculos manuais, foi utilizado o software Response-2000, desenvolvido na Universidade de Toronto por Evan C. Bentz. O programa permite a análise de vigas e pilares sujeitos a momentos, forças cortantes e cargas axiais, para qualquer tipo de geometria, propriedades de materiais e arranjo de armaduras, resultando em respostas precisas do comportamento das seções utilizando como base a MCFT. Abstract It is known that design deficiencies in shear design are more dangerous than bending, as shear failures can occur in a fragile way and without possibility of redistributing internal forces. Unlike bending design, designing for shear loads by different standards can generate significantly different results for the same element, as long as design models have been under discussion for many years. This paper analyses the evaluation of the behavior of the combined bending and shear loads in reinforced concrete beams for different pairs of these forces. For this purpose, the verification presented in ABNT NBR 6118 was used and compared to a more improved theory currently used, the Modified Compression Field Theory - MCFT. This theory is able to predict the relationships of specific loads and strains, as well as the shear strength of sections with great precision, being parameterized by several tested elements. As the use of this theory is not practical for manual calculations, the Response-2000 software, developed at the University of Toronto by Evan C. Bentz, was used. RESUMO Objetivo Verificar a adequação das oportunidades de ação no ambiente doméstico de crianças em risco de atraso no desenvolvimento e identificar fatores associados à sua frequência. Métodos Este estudo transversal incluiu 97 famílias que responderam ao Affordances in the Home Environment for Motor Development - Infant Scale (AHEMD-IS) para 3–18 meses (n=63) ou o Self Report (AHEMD-SR) para 18–42 meses (n=34). O teste de Mann-Whitney foi utilizado para identificar as diferenças entre as frequências de oportunidades de ação entre os grupos. A regressão linear múltipla foi utilizada para verificar a associação entre o sexo da criança, o estado civil, a escolaridade e nível socioeconômico da mãe, as idades da criança e da mãe, o número de residentes da casa, a renda per capita e os escores do AHEMD (α=0,05). Resultados A frequência das oportunidades de ação dos domicílios no AHEMD-IS variou de menos do que adequado a excelente; enquanto, no AHEMD-SR, a maior predominância foi de média frequência. A oferta de estímulos no AHEMD-IS foi significativamente maior. O maior nível socioeconômico e o número de moradores da casa foram associados a maiores oportunidades de ação. Conclusões Quanto maior o nível socioeconômico e o número de moradores, maiores são as oportunidades de ação nos domicílios das crianças em risco de atraso. Methods: The cross-sectional study included 97 families who responded to the Affordances in the Home Environment for Motor Development — Infant Scale (AHEMD-IS) for 3–18 months (n=63), or AHEMD – Self-Report (AHEMD-SR) for 18–42 months (n=34). The Mann-Whitney U test was used to identify the differences between the frequencies of affordances between the groups. Multiple linear regression was used to verify the association between the child’s sex, mother’s marital status, education, socioeconomic level, child and mother’s ages, house residents’ number, per capita income, and AHEMD scores (α=0.05). Results: The home affordances’ frequency in the AHEMD-IS ranged from less than adequate to excellent, while in the AHEMD-SR, the highest predominance was medium. The offer of stimuli in the AHEMD-IS was significantly higher. Higher socioeconomic level and house residents’ number were associated with greater affordances. Conclusions: The higher the socioeconomic level and house residents’ number, the greater the affordances in the homes of children at risk of delay. It is necessary to provide families with some alternatives to make their home environments richer in affordances that favor child development. Abstract Although Três Lagoas municipality is located in a rare and highly fragmented Atlantic Forest-Cerrado ecotone, there is scarcity of biodiversity data to guide conservation decisions. Therefore, this study aimed to compile the first bird checklist in 31 localities in this municipality to propose conservation strategies based on avian richness, conservation and migratory status. A total of 419 bird species were recorded, of which 412 were exclusive to the Atlantic Forest-Cerrado ecotone and seven to the Cerrado domain (C. minor, L. xenopterus, P. unicinctus, T. curucui, S. pileata, S. melanopis and S. luteola). According to the results, 12 species are included in the Near Threatened category, four are vulnerable (C. fasciolata, L. xenopterus, A. tricolor and S. hypoxantha) and one is endangered (U. coronata). Regarding migratory birds, 112 were recorded (95 intracontinental, 17 intercontinental). Since the majority of these species are recorded in grasslands, aquatic habitats and Seasonal Deciduous Alluvial Forest, they necessarily have highest priority for conservation and restoration. Thus, we recommend that: (1) areas 2, 11 and 24 be considered as both Important Bird Area and “Area de Proteção Ambiental”; (2) riparian forest restoration along the Paraná river act as a local ecological corridor among these areas, allowing the recolonization, genetic exchange and restoration of locally reduced populations of migratory and near/threatened bird species. Resumo Este trabalho teve como objetivo caracterizar alguns dos compostos bioativos do fruto da Eugenia punicifolia (Kunth) DC. para aprofundar o conhecimento do seu potencial funcional. Frutos maduros foram coletados da restinga de Maricá-RJ, Brasil. Os compostos bioativos foram analisados por cromatografia líquida de alta eficiência. Os teores de ácido ascórbico (74,14 mg 100-1 g-1), licopeno (504 µg g-1) e carotenoides totais (632 µg g-1) foram superiores aos de outras frutas ricas nesses compostos. Os frutos de E. punicifolia são uma excelente fonte de carotenoides e podem ser considerados uma boa fonte de ácido ascórbico (vitamina C). Além disso, sua composição química apresentou compostos fenólicos, como ácido gálico e antocianinas. Assim, essa fruta brasileira subutilizada se destaca como fonte de compostos bioativos, apresentando um bom potencial como alimento funcional, principalmente devido ao alto teor de licopeno. Abstract This work aimed to characterize some of the bioactive compounds of Eugenia punicifolia (Kunth) DC. fruit to enhance the knowledge of its functional potential. Ripe fruits were collected from the restinga of Maricá, in the state of Rio de Janeiro (RJ), Brazil. Bioactive compounds were analyzed by High Performance Liquid Chromatography (HPLC). Ascorbic acid (74.14 mg 100-1 g-1), lycopene (504 µg g-1) and total carotenoids (632 µg g-1) contents were superior to other fruits rich in these compounds. In fact, E. punicifolia fruits are an excellent source of carotenoids and can be considered a good source of ascorbic acid (vitamin C). Furthermore, its chemical composition has presented phenolic compounds like gallic acid and anthocyanins. Thus, this underutilized Brazilian fruit stands out as a source of bioactive compounds, presenting a good potential as a functional food, especially due to the high content of lycopene. Abstract Objective: To provide guidelines on the coronavirus disease 2019 (COVID-19) vaccination in patients with immune-mediated rheumatic diseases (IMRD) to rheumatologists considering specific scenarios of the daily practice based on the shared-making decision (SMD) process. Methods: A task force was constituted by 24 rheumatologists (panel members), with clinical and research expertise in immunizations and infectious diseases in immunocompromised patients, endorsed by the Brazilian Society of Rheumatology (BSR), to develop guidelines for COVID-19 vaccination in patients with IMRD. A consensus was built through the Delphi method and involved four rounds of anonymous voting, where five options were used to determine the level of agreement (LOA), based on the Likert Scale: (1) strongly disagree; (2) disagree, (3) neither agree nor disagree (neutral); (4) agree; and (5) strongly agree. Nineteen questions were addressed and discussed via teleconference to formulate the answers. In order to identify the relevant data on COVID-19 vaccines, a search with standardized descriptors and synonyms was performed on September 10th, 2021, of the MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov, and LILACS to identify studies of interest. We used the Newcastle–Ottawa Scale to assess the quality of nonrandomized studies. Results: All the nineteen questions-answers (Q&A) were approved by the BSR Task Force with more than 80% of panelists voting options 4—agree—and 5—strongly agree—, and a consensus was reached. These Guidelines were focused in SMD on the most appropriate timing for IMRD patients to get vaccinated to reach the adequate covid-19 vaccination response. Conclusion: These guidelines were developed by a BSR Task Force with a high LOA among panelists, based on the literature review of published studies and expert opinion for COVID-19 vaccination in IMRD patients. Noteworthy, in the pandemic period, up to the time of the review and the consensus process for this document, high-quality evidence was scarce. Thus, it is not a substitute for clinical judgment.

Abstract Background: Patients using immunosuppressive drugs may have unfavorable results after infections. However, there is a lack of information regarding COVID 19 in these patients, especially in patients with rheumatoid arthritis (RA). Therefore, the aim of this study was to evaluate the risk factors associated with COVID 19 hospitalizations in patients with RA. Methods: This multicenter, prospective cohort study is within the ReumaCoV Brazil registry and included 489 patients with RA. In this context, 269 patients who tested positive for COVID 19 were compared to 220 patients who tested negative for COVID 19 (control group). All patient data were collected from the Research Electronic Data Capture database. Results: The participants were predominantly female (90.6%) with a mean age of 53 ±12 years. Of the patients with COVID 19, 54 (20.1%) required hospitalization. After multiple adjustments, the final regression model showed that heart disease (OR =4.61, 95% CI 1.06–20.02. P < 0.001) and current use of glucocorticoids (OR =20.66, 95% CI 3.09–138. P < 0.002) were the risk factors associated with hospitalization. In addition, anosmia was associated with a lower chance of hospitalization (OR =0.26; 95% CI 0.10–0.67, P < 0.005). Conclusion: Our results demonstrated that heart disease and the use of glucocorticoids were associated with a higher number of hospital admissions for COVID 19 in patients with RA. RESUMO: Na espécie suína os tumores são pouco diagnosticados devido a curta vida útil dos animais de produção. Normalmente não causam sinais clínicos, sendo, muitas vezes, encontrados no momento do abate. Um reprodutor com dois anos de idade, da raça Large White, utilizado no manejo reprodutivo de uma granja, sem histórico de problemas pré-existentes, foi examinado em função de lesões na pele da bolsa escrotal. Foram coletadas amostras de segmentos de pele contendo lesões para o diagnóstico histopatológico e imunohistoquímico. Microscopicamente os nodos em bolsa escrotal consistiam em neoplasias mal demarcadas, altamente celulares, expansivas e multifocalmente invasivas composta por endoteliócitos imaturos organizados em formações neovasculares. As células tumorais eram pleomórficas, levemente ovais a fusiformes com citoplasma eosinofílico e núcleos hipercromáticos e com um a três nucléolos. Todos os nódulos analisados foram compatíveis com hemangiossarcoma. Para a quantificação da angiogênese tecidual, após a avaliação imunohistoquímica, as células neoplásicas imunoexpressaram os anticorpos monoclonais CD31 e Fator VIII, através da identificação de proteínas expressas na superfície das células endoteliais. Observou-se o marcador de proliferação celular Ki67 positivo em aproximadamente 10% das células neoplásicas, demonstrando um alto grau de malignidade. O hemangiossarcoma na espécie suína já foi identificado em vários órgãos e tecidos, mas, até o momento, nenhum trabalho demonstrou o diagnóstico na pele da bolsa escrotal, como reportado, ineditamente, neste estudo. Face o exposto, espera-se que este relato contribua para a frequência de neoplasias encontradas na espécie suína.

ABSTRACT: Tumors are rarely diagnosed in swine specie because of the short lifespan of production animals. Normally, these tumors do not present any clinical signs and are often detected at the time of slaughter. A 2-year-old Large White boar, used in the reproductive management of a farm and without a history of pre-existing problems, was examined for skin lesions on the scrotum. Samples were collected from skin segments containing lesions for histopathological and immunohistochemical diagnosis. Microscopically, the nodes in the scrotum pouch consisted of poorly demarcated, highly cellular, expansile, and multifocally invasive neoplasms, composed of immature endotheliocytes organized into neovascular formations. The tumor cells were pleomorphic, slightly oval to spindle-shaped, with eosinophilic cytoplasm and hyperchromatic nuclei with one to three nucleoli. All the nodules analyzed were compatible with hemangiosarcoma. After immunohistochemical evaluation, for the quantification of tissue angiogenesis the neoplastic cells immunoexpressed the CD31 monoclonal antibodies and factor VIII, through the identification of proteins expressed on the surface of endothelial cells. The Ki67 cell proliferation marker was positive in approximately 10% of the neoplastic cells, demonstrating a high degree of malignancy. Hemangiosarcoma in swine species has already been identified in several organs and tissues; however, to date, no study has demonstrated the diagnosis of this condition on the skin of the scrotum, as reported in this study. Therefore, it is expected that this report will contribute to the knowledge of the frequency of neoplasms in swine species.

Abstract Objective: To review the technical aspects of body composition assessment by dual-energy X-ray absorptiometry (DXA) and other methods based on the most recent scientific evidence. Materials and methods: This Official Position is a result of efforts by the Scientific Committee of the Brazilian Association of Bone Assessment and Metabolism (Associação Brasileira de Avaliação Óssea e Osteometabolismo, ABRASSO) and health care professionals with expertise in body composition assessment who were invited to contribute to the preparation of this document. The authors searched current databases for relevant publications. In this first part of the Official Position, the authors discuss the different methods and parameters used for body composition assessment, general principles of DXA, and aspects of the acquisition and analysis of DXA scans. Conclusion: Considering aspects of accuracy, precision, cost, duration, and ability to evaluate all three compartments, DXA is considered the gold-standard method for body composition assessment, particularly for the evaluation of fat mass. In order to ensure reliable, adequate, and reproducible DXA reports, great attention is required regarding quality control procedures, preparation, removal of external artifacts, imaging acquisition, and data analysis and interpretation.

Métodos: Essas diretrizes foram desenvolvidas com base em revisão bibliográfica e experiência clínica de um comitê de especialistas. Foram realizadas buscas PubMed/Medline; documentos selecionados não se restringiram a revisões sistemáticas, ensaios clínicos randomizados e estudos observacionais. Quando adequado, o perfil GRADE foi aplicado para graduá-los e o consenso de especialistas foi usado nos tópicos sem evidência científica. Também foram revisadas as recomendações pela Academia Americana de Pediatria, Associação Americana de Oftalmologia Pediátrica e Estrabismo, Academia Americana de Oftalmologia, Royal College of Ophthalmologist e Sociedade Canadense de Oftalmologia. O documento final foi aprovado pela Sociedade Brasileira de Oftalmologia Pediátrica e Sociedade Brasileira de Pediatria. Resultados: Os recém-nascidos devem ser submetidos ao teste do reflexo vermelho e inspeção dos olhos e anexos pelo pediatra dentro de 72 horas de vida ou antes da alta da maternidade. O teste do reflexo vermelho deve ser repetido pelo pediatra durante as consultas de puericultura pelo menos três vezes ao ano durante os primeiros 3 anos de vida. Se factível, um exame oftalmológico completo pode ser feito entre 6 a 12 meses de vida. Até os 36 meses de idade, os marcos visuais, função visual apropriada para a idade, fixação e alinhamento ocular também devem ser avaliados pelo pediatra ou médico da família. Pelo menos um exame oftalmológico completo deve ser realizados entre 3 e 5 anos de idade. O exame deve conter pelo menos inspeção dos olhos e anexos, avaliação da função visual apropriada para a idade, avaliação da motilidade e alinhamento ocular (testes de cobertura), refração sob cicloplegia e avaliação do fundo de olho dilatado. Conclusões: As diretrizes sobre a frequência da avaliação oftalmológica são ferramentas importantes para orientar os médicos sobre a melhor prática a fim de evitar problemas visuais tratáveis na infância, que poderiam comprometer seu desenvolvimento social, escolar e global, além de causar perda permanente da visão.

ABSTRACT Purpose: To provide guidance on the frequency and components of eye examinations for healthy children aged 0 to 5 years. Methods: These guidelines were developed based on the medical literature and clinical experience of an expert committee. PubMed/Medline searches were performed, with selected publications not restricted to systematic reviews, randomized controlled trials, or observational studies. The Grading of Recommendations Assessment, Development, and Evaluation profile was applied when suitable, and for issues without scientific evidence, recommendations were based on expert consensus. Recommendations by the American Academy of Pediatrics, American Association of Pediatric Ophthalmology and Strabismus, American Academy of Ophthalmology, Royal College of Ophthalmologists, and Canadian Ophthalmological Society were also reviewed. The final guideline document was approved by the Brazilian Pediatric Ophthalmology Society and by the Brazilian Pediatric Society. Results: Newborns must undergo the red reflex test and inspection of the eyes and adnexa by a pediatrician within 72 hours of life. The red reflex test should be repeated by the pediatrician during childcare consultations at least three times per year during the first 3 years of life. If feasible, a comprehensive ophthalmologic examination may be performed between 6 and 12 months of age. Until 36 months of age, the pediatrician should assess the infant’s visual development milestones, age-appropriate assessment of visual function, ocular fixation, and eye alignment. At least one comprehensive ophthalmologic examination should be performed at 3 to 5 years of age. The examination should minimally include inspection of the eyes and adnexa, age-appropriate visual function assessment, evaluations of ocular motility and alignment (cover tests), cycloplegic refraction, and dilated fundus. Conclusions: Guidelines concerning the frequency of ophthalmic assessment are important tools for directing physicians regarding best practices that avoid treatable vision problems that affect children’s development, school, and social performance and cause unnecessary permanent vision loss.
