ABSTRACT BACKGROUND: Radiation therapy (RT) is a standard treatment for non-metastatic breast cancer and is associated with acute and late toxicities. Intensity-modulated RT (IMRT) may decrease toxicity and is convenient for patients. OBJECTIVES: To assess the efficacy and safety of IMRT in women with early stage breast cancer. DESIGN AND SETTING: Systematic review study; Multi-institutional centers. METHODS: Seven databases were searched. Randomized controlled trials (RCT) comparing IMRT with any “non-IMRT” strategies were included. Primary outcomes were local control and acute toxicity. Cochrane Handbook was use to plan and conduct the review, and PRISMA 2020 was used to report results. RESULTS: Five RCT involving 2,556 women (n = 1,283 IMRT; n = 1,274 control arm) were included. Baseline characteristics were similar between trials and arms. Local relapse-free survival rates were not different (hazard-ratio [HR] 0.62; 95%confidence interval [CI] -0.38 to 1.62; P > 0.05); however, IMRT reduced the overall acute toxicity (RR 0.69, 95%CI 0.58 to 0.82; P < 0.00001) and acute moist desquamation (risk-ratio [RR] 0.71, 95%CI 0.60 to 0.82; P < 0.00001). Lymphedema and pneumonitis rates, and survival outcomes were not affected by IMRT. The 2-year telangiectasia rate was decreased with IMRT (RR 0.66, 95%CI 0.47 to 0.93; P = 0.02); however, edema, pain, pigmentation, or fibrosis remained unaffected. IMRT did not improve cosmesis. CONCLUSIONS: IMRT improved acute toxicity and lowered telangiectasia rates, without affecting oncological and aesthetic outcomes. Abstract Objective To compare the effectiveness of inhaled Magnesium Sulfate associated with Salbutamol versus Inhaled Salbutamol alone in patients with moderate and severe asthma exacerbations. Method Clinical, prospective and randomized study with patients between 3 and 14 years of age divided into two groups: one to receive inhaled salbutamol associated with magnesium sulfate (GSM), the other to receive inhaled salbutamol alone (GS). The sample consisted of 40 patients, 20 patients in each group. Severity was classified using the modified Wood-Downes score, with values between 4 and 7 classified as moderate and 8 or more classified as severe. Results Post-inhalation scores decreased both in patients who received salbutamol and magnesium and in those who received salbutamol alone, with no statistically significant difference between the groups. Abstract Gastrointestinal tuberculosis is an uncommon entity, in which clinical presentation can be widely variable, from mild and nonspecific symptoms to an acute abdomen and gastrointestinal bleeding. Gastric involvement by Mycobacterium tuberculosis is rare, especially when it occurs without other recognized infectious foci - primary gastric tuberculosis - with only a few reported cases. Endoscopic findings can be very heterogeneous, from areas of hyperemia to pseudotumor lesions. We present a case of primary gastric tuberculosis in an immuno-competent patient, in which the absence of an epidemiological context and nonspecificendoscopic findings led to a delay in the diagnosis. Bite-on-bite biopsies proved to be essential, allowing to obtain samples from deeper layers of the sub-mucosa where M. tuberculosis was identified. This case aimed to increase awareness for this entity, especially in endemic countries or regions with a high prevalence of tuberculosis since the diagnosis is based mainly on a high index of suspicion.

Resumo A tuberculose gastrointestinal é uma entidade pouco comum, com uma apresentação clínica amplamente variável, desde sintomas ligeiros e inespecíficos até quadros de abdómen agudo e hemorragia digestiva. O envolvimento gástrico pelo Mycobacterium tuberculosis é raro, especialmente quando ocorre sem outros focos infeciosos reconhecidos - tuberculose gástrica primária -, havendo apenas alguns casos descritos na literatura. Os achados endoscópicos podem ser muito heterogéneos, variando desde áreas de mucosa hiperemiada até lesões pseudo-tumorais. Apresentamos o caso de uma doente imunocompetente com diagnóstico de tuberculose gástrica primária, em que a ausência de um contexto epidemiológico e achados endoscópicos inespecíficos conduziram a um atraso no diagnóstico. As biópsias sobre biópsias mostraram ser essenciais para o diagnóstico, pois permitiram obter amostras de camadas mais profundas da submucosa do antro gástrico onde foi identificado o agente infecioso. Este caso pretende sensibilizar para existência desta entidade, especialmente em países endémicos ou regiões com alta prevalência de tuberculose, uma vez que o seu diagnóstico implica um elevado grau de suspeição.

Abstract Introduction: The association of hepatitis delta virus (HDV) infection with positive autoantibodies and autoimmune features has been known for decades. However, to date, very few cases of clinical autoimmune hepatitis (AIH) have been reported in association with HDV infection, most of them being in the context of treatment with peginterferon. Case Report: This case refers to a 46-year-old woman born in Guinea-Bissau who moved to Portugal in 2018 to investigate complaints of diffuse abdominal discomfort and nausea. Her initial work-up, including laboratory and liver histology, was consistent with type 1 AIH. She had HBe antigen-negative chronic hepatitis B virus infection with negative DNA and also a positive total anti-HDV antibody, with negative IgM and undetectable RNA. Therefore, after initiating prophylactic tenofovir difumarate, she was started on prednisolone followed by azathioprine, which was later stopped due to presumed hepatotoxicity. Repeated histology showed signs of viral superinfection, and she was treated with acyclovir due to a positive herpes simplex IgM, with HDV RNA remaining negative. A third flare in transaminases prompted the introduction of mycophenolate mofetil (MMF) after a thorough exclusion of additional causes of liver disease. About 6 months later, during another bout of hepatitis, HDV RNA was finally positive and classified as genotype 5. MMF was stopped, and, considering a contraindication to interferon, the patient was offered therapy with bulevirtide, which she refused for personal reasons as she is currently living in her home country. Discussion: This is a challenging case of autoimmune or “autoimmune-like” hepatitis, probably induced by chronic HDV infection. High suspicion of HDV was essential because, had the case been interpreted as refractory AIH, with escalation of immunosuppression, a more severe course of the viral infection might have ensued. Recently, HDV suppression with bulevirtide was shown to reverse autoimmune liver disease. We hypothesize that the same could have happened to our patient, had she accepted this treatment.

Resumo Introdução: A associação da infeção pelo vírus da hepatite delta (VHD) com a presença de autoanticorpos e outros aspetos de autoimunidade é conhecida desde há várias décadas. Contudo, até à data, muito poucos casos de hepatite autoimune (HAI) clínica foram reportados em relação com a infeção VHD, sendo a maioria destes no contexto de terapêutica com interferão peguilado. Caso clínico: O caso refere-se a uma mulher de 46 anos natural da Guiné-Bissau, que se mudou para Portugal em 2018 para investigação de queixas de desconforto abdominal difuso e náuseas. A avaliação laboratorial inicial e a histologia hepática foram compatíveis com HAI tipo 1. A doente apresentava também infeção crónica a VHB (vírus da hepatite B) antigénio HBe negativa, com DNA negativo, e anti-VHD (vírus da hepatite delta) total positivo, com IgM negativo e RNA indetetável. Assim, após início de tenofovir difumarato profilático, foi iniciada terapêutica com prednisolona seguida de azatioprina, que posteriormente se interrompeu por presumível hepatotoxicidade. Uma segunda biópsia mostrou aspetos de superinfeção viral e como tal a doente foi tratada com aciclovir, tendo em conta IgM positivo para Herpes Simplex, mantendo-se o RNA VHD negativo. Um terceiro flare de transaminases motivou o início de micofenolato de mofetil, após extensa investigação e exclusão de outras causas de doença hepática. Cerca de 6 meses mais tarde, durante novo episódio de hepatite, o RNA VHD revelou-se finalmente positivo e este foi classificado como genotipo 5. O MMF foi suspenso e, considerando a contra-indicação para interferão, foi proposto à doente tratamento com bulevirtide, que esta recusou, alegando motivos pessoais, visto estar atualmente a residir no seu país de origem. Discussão: Este é um caso desafiante de hepatite autoimune, ou autoimune-like, provavelmente induzida pela infeção crónica pelo VHD. Um elevado índice de suspeição para VHD foi essencial porque, se o caso tivesse sido interpretado como HAI refratária, com incremento de imunossupressão, poderia ter-se verificado um agravamento da hepatite viral. Recentemente, foi reportado que a supressão do VHD pelo bulevirtide pode reverter a doença hepática autoimune. Questionamo-nos se o mesmo poderia ter sucedido com a nossa doente, caso esta tivesse aceite este tratamento.

Las políticas de salud pública apuntan a brindar acceso equitativo a los servicios de salud como un derecho fundamental. En 2022, el Ministerio de Salud creó un incentivo financiero para promover la actividad física (IAF) en la Atención Primaria de Salud del SUS. El presente trabajo tuvo como objetivo presentar el panorama nacional del primer año de implementación del IAF y analizar si el puntaje de priorización municipal, creado considerando la disponibilidad presupuestaria existente, fue un criterio efectivo para una mayor equidad en la distribución de los recursos en los diferentes períodos de implementación (sin y con metas). Se trata de un estudio descriptivo con análisis del número absoluto y relativo de municipios y unidades de salud aprobadas, unidades de salud que recibieron recursos y los montos pagados. Para analizar la equidad se calcularon medidas de desigualdades absolutas y relativas entre unidades de salud, agrupadas en cuartiles según el puntaje de priorización municipal. Se demostró que el 74% de los municipios y el 16,7% de las unidades de salud elegibles fueron aprobados por el IAF. El porcentaje de unidades de salud aprobadas que recibieron recursos no superó el 37,6%, siendo pagados R$ 18,05 millones (período sin metas) y R$ 10,10 millones (con metas). Hubo una reducción del 68,9% en las unidades de salud que recibieron recursos luego de establecer metas. Se revelaron importantes desigualdades en la recepción de recursos, con un mayor porcentaje de unidades de salud ubicadas en municipios con menor prioridad. Por tanto, los criterios adoptados por el Ministerio de Salud fueron insuficientes para garantizar la equidad en la asignación de recursos para promover la actividad física.

As políticas públicas de saúde têm como objetivo proporcionar acesso equitativo aos serviços de saúde como um direito fundamental. Em 2022, o Ministério da Saúde criou um incentivo financeiro para a promoção da atividade física (IAF) na Atenção Primária à Saúde do SUS. O presente trabalho teve como objetivos apresentar o panorama nacional do primeiro ano de implementação do IAF e analisar se a nota de priorização municipal, criada considerando a disponibilidade orçamentária existente, foi um critério efetivo para maior equidade na distribuição dos recursos nos diferentes períodos de implementação (sem e com metas). Trata-se de um estudo descritivo com análises do número absoluto e relativo de municípios e unidades de saúde homologadas, de unidades de saúde que receberam recursos e dos valores pagos. Para analisar a equidade, foram calculadas as medidas de desigualdades absolutas e relativas entre as unidades de saúde, agrupadas em quartis conforme a nota de priorização municipal. Foi demonstrado que 74% dos municípios e 16,7% das unidades de saúde elegíveis foram homologadas ao IAF. O percentual de unidades de saúde homologadas que receberam recursos não passou de 37,6%, sendo pagos R$18,05 milhões (período sem metas) e R$10,10 milhões (com metas). Houve uma redução de 68,9% nas unidades de saúde que receberam recursos após o estabelecimento de metas. Foram reveladas importantes desigualdades no recebimento de recursos, com maior percentual de unidades de saúde localizadas em municípios com menor prioridade. Public health policies aim to provide equitable access to healthcare services as a fundamental right. In 2022, the Ministry of Health created a financial incentive to promote physical activity (IAF) in SUS Primary Health Care. The objectives of this study were to present the national panorama of the first year of implementation of the IAF and to analyze whether the municipal prioritization score, created considering the existing budget availability, was an effective criterion for greater equity in the distribution of resources in the different implementation periods (without and with targets). This is a descriptive study analyzing the absolute and relative number of municipalities and health units approved, the number of health units that received funds and the amounts paid out. To analyze equity, measures of absolute and relative inequalities between health units were calculated, grouped into quartiles according to the municipal prioritization score. The percentage of approved health units that received funds was no more than 37.6%, with R$18.05 million paid out (period without targets) and R$10.10 million (with targets). There was a 68.9% reduction in the number of health units that received funds after the targets were set. Important inequalities in the receipt of resources were revealed, with a higher percentage of health units located in municipalities with lower priority. Thus, the criteria adopted by the Ministry of Health were insufficient to guarantee equity in the allocation of resources to promote physical activity.

ABSTRACT Objective: To define a reference chart comparing pressure drop vs. flow generated by a set of arterial cannulae currently utilized in cardiopulmonary bypass conditions in pediatric surgery. Methods: Cannulae from two manufacturers were selected considering their design and outer and inner diameters. Cannula performance was evaluated in terms of pressure drop vs. flow during simulated cardiopulmonary bypass conditions. The experimental circuits consisted of a Jostra HL-20 roller pump, a Quadrox-i pediatric oxygenator (Maquet Cardiopulmonary AG, Rastatt, Germany), and a custom pediatric tubing set. The circuit was primed with lactated Ringer’s solution only (first condition) and with human packed red blood cells added (second condition) to achieve a hematocrit of 30%. Cannula sizes 8 to 16 Fr were inserted into the cardiopulmonary bypass circuit with a “Y” connector. The flow was adjusted in 100 ml/min increments within typical flow ranges for each cannula. Pre-cannula and post-cannula pressures were measured to calculate the pressure drop. Results: Utilizing a pressure drop limit of 100 mmHg, our results suggest a recommended flow limit of 500, 900, 1400, 2600, and 3100 mL/min for Braile arterial cannulae sizes 8, 10, 12, 14, and 16 Fr, respectively. For Medtronic DLP arterial cannulae sizes 8, 10, 12, 14, and 16 Fr, the recommended flow limit is 600, 1100, 1700, 2700, and 3300 mL/min, respectively. Conclusion: This study reinforces discrepancies in pressure drop between cannulae of the same diameter supplied by different manufacturers and the importance of independent translational research to evaluate components’ performance. Abstract Through quantitative exploratory research, the present study analyzed the amount foreseen in the Federal Budget and the amounts paid (nominal and deflated) for programs and actions to promote body practices and physical activities (Health Academy Program and the Federal Incentive for Physical Activity in Primary Health Care) from 2019 to 2022. Resumo Por meio de pesquisa quantitativa de caráter exploratório, o presente estudo teve o objetivo de analisar o orçamento e o financiamento federal de programas e ações de promoção das práticas corporais e atividades físicas no Sistema Único de Saúde (SUS) de 2019 a 2022 (Programa Academia da Saúde e o Incentivo Federal de Custeio da Atividade Física na Atenção Primária). Foram analisados e calculados os valores per capita, pela população coberta pela atenção primária e por participante de programas públicos. Os recursos efetivamente pagos foram de 3,31% a 15,06% menores dos que os aprovados no orçamento (nominal), e também foram identificados os baixos valores (máximos) anuais, independentemente se nominal ou deflacionado per capita (R$ 0,21 a 0,30) por população coberta pela atenção primária (R$ 0,25 a 0,40) e por participante (R$ 10,61 a 14,61).
