RESUMO: O objetivo deste relato é descrever o auxílio da elastografia por impulso de força de radiação acústica (ARFI) no diagnóstico de dois casos de tumores malignos da bainha dos nervos periféricos em canídeos. Ambos os pacientes apresentavam claudicação em membro torácico e já haviam sido tratados clinicamente por outros profissionais, sem resposta ao tratamento ou histórico de trauma. Ao exame físico, foi observada atrofia muscular em apenas um dos membros torácicos, nos dois casos. Foi realizado exame ultrassonográfico axilar. Na avaliação modo B, foi observada a presença de estrutura nodular, predominantemente hipoecogênica, heterogênea, irregular, não encapsulada e com reatividade adjacente, medindo aproximadamente 20.1 mm x 15.4 mm no primeiro caso e 18.3 mm x 10.7 mm no segundo caso. Foi realizada elastografia ARFI, demonstrando a presença de áreas de maior rigidez, com velocidade de ondas de cisalhamento média de 4.12 m/s (caso 1) e 4.35 m/s (caso 2), sugerindo malignidade. Em ambos os casos, foi realizada amputação do membro afetado e os nódulos removidos foram encaminhados para análise histopatológica, na qual se obteve o diagnóstico de tumor maligno da bainha de nervo periférico. ABSTRACT: This report described the use of acoustic radiation force impulse (ARFI) elastography in the diagnosis of two cases of canine malignant peripheral nerve sheath tumors. Both patients presented with lameness in the thoracic limb and no response to previous clinical treatment or trauma history. Physical examination revealed muscular atrophy in one of the thoracic limbs, in both dogs. B-mode axillary ultrasound showed the presence of a predominantly hypoechogenic, heterogeneous, irregular, nonencapsulated nodule with adjacent reactivity, measuring approximately 20.1 mm x 15.4 mm in the first case and 18.3 mm x 10.7 mm in the second. ARFI elastography was performed, showing areas of significant stiffness, with an average shear wave velocity of 4.12 m/s (case 1) and 4.35 m/s (case 2), suggesting malignancy. A transmissão do vírus Oropouche (OROV) para novas regiões, juntamente com um aumento de casos e o surgimento de formas graves, anteriormente não suspeitas, expressas preocupações de saúde pública. Uma fazendeira grávida de 40 anos, com 30 semanas de gestação, desenvolvimento de febre, mialgia e dor de cabeça, com infecção por OROV confirmada por RT-qPCR. as avaliações maternas e fetais não foram inicialmente concluídas. No entanto, na semana seguinte, um paciente notou diminuição dos movimentos fetais, e o ultrassom confirmou a morte fetal. O diagnóstico molecular detectou o RNA do OROV em vários espécimes fetais. Este caso de transmissão vertical ressalta a necessidade urgente de proteger as mulheres grávidas, incorporar o OROV no diagnóstico diferencial de doenças febris e investigar mais profundamente os potenciais mecanismos patogênicos do vírus.

RESUMO Contexto: Nas últimas décadas, a ecoendoscopia evoluiu de uma ferramenta diagnóstica para uma plataforma para diversas intervenções terapêuticas. Vários avanços tecnológicos surgiram desde o último Consenso Brasileiro, demandando uma revisão e atualização das recomendações baseadas nas melhores evidências científicas. Métodos: Um grupo de 32 ecoendoscopistas renomados selecionou oito tópicos relevantes para serem discutidos a fim de gerar questões clínicas. Em seguida, foi realizada uma revisão da literatura para responder a essas perguntas com base nas evidências mais atualizadas. Resultados: Trinta e três tópicos foram formulados e votados pelos especialistas para alcançar um consenso. O Sistema de Oxford foi utilizado para classificar o nível de evidência. Conclusão: Há evidências moderadas para sustentar que a forma da agulha, calibre ou técnica de aspiração não influenciam no rendimento da amostragem tecidual guiada por ultrassom endoscópico (USE) de lesões sólidas pancreáticas. Há evidências moderadas para sustentar o uso da biópsia transcutânea guiada por USE da parede cística para diferenciar entre neoplasias císticas mucinosas e não mucinosas. Existem poucas evidências para apoiar o tratamento guiado por USE de varizes gástricas. ABSTRACT Background: In the past decades, endoscopic ultrasound has developed from a diagnostic tool to a platform for many therapeutic interventions. Various technological advancements have emerged since the last Brazilian Consensus, demanding a review and update of the recommendations based on the best scientific evidence. Methods: A group of 32 renowned echoendoscopists selected eight relevant topics to be discussed to generate clinical questions. After that, a literature review was conducted to answer these questions based on the most updated evidence. Results: Thirty-three statements were formulated and voted on by the experts to reach a consensus. The Oxford System was used to grade the level of evidence. Conclusion: There is moderate evidence to support that the needle shape, gauge, or aspiration technique does not influence the yield of endoscopic ultrasound (EUS)-guided tissue sampling of pancreatic solid lesions. Resumo Este estudo avaliou a resistência de união (RU) de sistemas de pinos de fibra de vidro convencionais, anatomizados, ajustáveis e CAD/CAM. Quarenta raízes de caninos superiores foram enfraquecidas, tratadas endodonticamente e divididas em quatro grupos (n=10). Um slice de cada terço do canal radicular (cervical, média e apical) foi submetido ao teste de push-out e o padrão de falha foi analisado posteriormente. O slice mais apical foi submetido à análise da espessura da linha de cimentação por microscopia eletrônica de varredura (MEV). Os dados referentes à RU e espessura da linha de cimentação foram submetidos ao teste de normalidade de Shapiro-Wilk seguido dos testes de variância e Tukey. O padrão de falha foi expresso em porcentagem e submetido ao teste qui-quadrado. O pino convencional apresentou menor RU em comparação aos demais pinos de fibra de vidro (p<0,05). Todos os pinos de fibra de vidro apresentaram maior percentual de falhas adesivas à dentina. A MEV evidenciou linha de cimentação mais espessa para o pino convencional em relação aos demais grupos (p<0,05), além de lacunas e espaços vazios no material restaurador. Os pinos de fibra de vidro anatomizados, ajustáveis e fresados em CAD/CAM demonstraram RU superior e melhor adaptação à dentina radicular em comparação com o pino de fibra de vidro convencional. Abstract This study evaluated the bond strength (BS) of conventional, anatomized, adjustable, and CAD/CAM fiberglass post systems. Forty maxillary canine roots were weakened, endodontically treated, and divided into four groups (n=10). A slice from each of the three sections (cervical, middle, apical) of the root canal was subjected to the push-out test and the failure pattern was subsequently analyzed. The most apical slice was subjected to analysis of the cementation line thickness by scanning electron microscopy (SEM). Data regarding the BS and cementation line thickness were subjected to the Shapiro-Wilk normality test followed by the variance and Tukey tests. Data failure patterns were expressed as a percentage and submitted to the chi-square test. Conventional fiberglass posts had the lowest BD compared to the other fiberglass posts (p<0.05). All fiberglass posts exhibited a higher percentage of adhesive failures to dentin. The SEM demonstrated a thicker cementation line for the conventional post compared to the other groups (p<0.05), as well as gaps and voids in the cementing material. The anatomized, adjustable, and CAD/CAM milled fiberglass post systems demonstrated a superior BS and adaptation to the root dentin compared to the conventional fiberglass post. Abstract Background Hepatocellular carcinoma (HCC) is a prevalent tumor with high mortality rates. Computed tomography (CT) is crucial in the non-invasive diagnosis of HCC. Recent advancements in artificial intelligence (AI) have shown significant potential in medical imaging analysis. However, developing these AI algorithms is hindered by the scarcity of comprehensive, publicly available liver imaging datasets. Objectives This study aims to detail the tools, data organization, and database structuring used in creating HepatIA, a medical imaging annotation platform and database at a Brazilian tertiary teaching hospital. HepatIA supports liver disease AI research at the institution. Material and methods The authors collected baseline characteristics and CT scans of 656 patients from 2008 to 2021. The database, designed using PostgreSQL and implemented with Django and Vue.js, includes 692 CT volumes from a four-phase abdominal CT protocol. Radiologists made segmentation annotations using the OHIF medical image viewer, incorporating MONAI Label for pre-annotation segmentation models. The annotation process included detailed descriptions of liver morphology and nodule characteristics. Results The HepatIA database currently includes healthy individuals and those with liver diseases such as HCC and cirrhosis. The database dashboard facilitates user interaction with intuitive plots and histograms. Key patient demographics include 64% males and an average age of 56.89 years. The database supports various filters for detailed searches, enhancing research capabilities. Conclusion A comprehensive data structure was successfully created and integrated with the IT systems of a teaching hospital, enabling research on deep learning algorithms applied to abdominal CT scans for investigating hepatic lesions such as HCC. ABSTRACT The main social impact of toxoplasmosis stems from its ability to be vertically transmitted. Postnatally acquired infection is generally asymptomatic in approximately 70–90% of cases, making diagnosis often dependent on laboratory tests using serological methods to search for anti-T. gondii antibodies. This study aimed to investigate the ability of the VIDAS TOXO IgG avidity and VIDAS TOXO IgM assays to confirm recent toxoplasmosis. In total, 341 pregnant women with suspected acute toxoplasmosis were systematically monitored in the Program for Control of Congenital Toxoplasmosis in Minas Gerais State, Brazil. We conducted an observational analytical-descriptive cross-sectional study and grouped according to clinical and laboratory criteria as having acute or chronic toxoplasmosis. The VIDAS TOXO IgG avidity and VIDAS TOXO IgM assays were evaluated to investigate the capacity to identify acute infection. IgG avidity showed good performance in identifying acute toxoplasmosis when the IgG avidity index was lower than or equal to 0.1. Values greater than or equal to 3.16 according to the TOXO IgM kit were associated with a greater chance of acute infection. ABSTRACT Elephant grass is an allogamous perennial forage crop with asexual propagation, allowing plant breeders to explore heterosis and develop hybrids. However, selecting parents for diallel crossing schemes is a major hurdle in the development of hybrids. In addition, this perennial crop has several harvests, which increases the dataset size and the complexity of the statistical analyses. Here, we propose a diallel analysis based on linear mixed models with repeatability information to identify parents and hybrids, and the optimum number of harvests. We performed a complete diallel crossing of 11 parents with data from five harvests for morphological traits and two harvests for nutritive value traits. The diallel model with repeatability information aids in the estimation of the genetic effect of diallel analysis to infer the ideal number of harvests and genotype × harvest interaction. We observed that the specific combining ability (SCA) is more critical for morphoagronomic traits, while the general combining ability (GCA) has greater importance for traits related to nutritive value. Furthermore, we detected that five harvests were sufficient for the morphoagronomic traits with a coefficient of determination (R2) of 0.8; however, ten harvests were required for an R2 of 0.9. Abstract Background There is a remarkable variability in the frequency of HLA-B27 positivity in patients with spondyloarthritis (SpA), which may be associated with different clinical presentations worldwide. However, there is a lack of data considering ethnicity and sex on the evaluation of the main clinical and prognostic outcomes in mixed-race populations. The aim of this study was to evaluate the frequency of HLA-B27 and its correlation with disease parameters in a large population of patients from the Brazilian Registry of Spondyloarthritis (RBE). Methods The RBE is a multicenter, observational, prospective cohort that enrolled patients with SpA from 46 centers representing all five geographic regions of Brazil. The inclusion criteria were as follow: (1) diagnosis of axSpA by an expert rheumatologist; (2) age ≥18 years; (3) classification according to ASAS axial. The following data were collected via a standardized protocol: demographic data, disease parameters and treatment historical. Results A total of 1096 patients were included, with 73.4% HLA-B27 positivity and a mean age of 44.4 (±13.2) years. Positive HLA-B27 was significantly associated with male sex, earlier age at disease onset and diagnosis, uveitis, and family history of SpA. Conversely, negative HLA-B27 was associated with psoriasis, higher peripheral involvement and disease activity, worse quality of life and mobility. Conclusions Our data showed that HLA-B27 positivity was associated with a classic axSpA pattern quite similar to that of Caucasian axSpA patients around the world. Furthermore, its absence was associated with peripheral manifestations and worse outcomes, suggesting a relevant phenotypic difference in a highly miscegenated population. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Dark Septate Endophytic (DSE) fungi can benefit plants by optimizing nutrient uptake, biosynthesis of phytohormones-like compounds, and stress relief such as toxic metals. The objective was to characterize in vitro 57 strains of the DSE Periconia macrospinosa isolated from sugarcane roots, indicating the most promising in solubilizing phosphate sources, growing in different metal and vinasse contents, and producing molecules related to indoleacetic acid (IAA). The strains were from the Embrapa Agrobiology Fungi Collection. Over 35% of the strains solubilize calcium phosphate, highlighting A356 and A155. No strain solubilizes aluminum phosphate. Three strains did not grow in the presence of Cd (A333, A334, A163), but A226, A332, and A423 stand out showing high growth with Cd. All strains grew in the presence of Cu, Zn, and Vinasse. A163, A164, and A328 were even stimulated by Cu addition. A331 and A335 showed a marked growth decrease with Zn. All strains are highly adapted to grow in vinasse presence. A257 and A226 produced the highest amount of IAA. The most biotechnological potential strains are indicated by presenting high growth under Cd, Cu, Zn, and vinasse stress, associated with a high tolerance index to these pollutants, further calcium phosphate solubilization, or IAA production. RESUMO Avaliou-se a influência da concentração de cloreto de sódio na estabilidade dimensional da madeira de Eucalyptus urophylla x Eucalyptus grandis. Foram utilizados 90 corpos de prova, na qual foram saturados em béqueres contendo água destilada (controle) e solução de cloreto de sódio com concentrações de 2,5% e 10%. Depois de saturados, os corpos de prova foram submetidos à secagem até atingirem massa constante. As dimensões dos corpos de prova foram determinadas para cálculo dos inchamentos e contrações radiais e tangenciais. Madeiras de Eucalyptus urophylla x Eucalyptus grandis impregnadas com solução de cloreto de sódio nas concentrações de 2,5 e 10% obtiveram valores de inchamento tangencial semelhantes ao do controle. Para o inchamento radial, as madeiras impregnadas com solução de cloreto de sódio na concentração de 2,5% atingiram menores valores. A concentração da solução de cloreto de sódio influenciou a contração tangencial e radial das madeiras de Eucalyptus urophylla x Eucalyptus grandis, pois menores valores foram encontrados para as madeiras impregnadas com solução de cloreto de sódio nas concentrações de 10 e 2,5%. ABSTRACT Evaluated the influence of the concentration of sodium chloride in the dimensional stability of Eucalyptus urophylla x Eucalyptus grandis. It was used 90 samples, which were saturated in beakers containing distilled water (control) and sodium chloride solution concentrations of 2.5% and 10%. Once saturated, the samples were dried until constant mass. The dimensions of the samples were determined for calculate the swelling and radial and tangential contractions. Resumo Objetivo Avaliar a segurança e a reprodutibilidade da cirurgia para escorregamento da epífise femoral proximal (EEPF) com instabilidade por meio da técnica de Dunn modificada em uma coorte unicêntrica no Brasil. Métodos Analisamos de forma retrospectiva uma coorte de pacientes submetidos a esse procedimento por um único cirurgião especialista em preservação do quadril. Avaliamos os dados demográficos e os ângulos radiográficos quanto ao risco relativo (RR) de necrose avascular (NAV) por meio do modelo de regressão log-binomial com efeitos simples e aleatórios. Resultados Entre os 30 pacientes (30 quadris) com idade média de 11,79 anos no momento da cirurgia, havia 17 meninos e 18 quadris esquerdos. O procedimento ocorreu em média 11,5 dias após o escorregamento. O tempo médio de acompanhamento foi de 38 meses. O ângulo de Southwick pré-operatório foi, em média, de 60,69° contra 4,52° após o procedimento (p< 0,001). O maior ângulo de escorregamento pré-operatório foi associado ao desenvolvimento de NAV (RR: 1,05; intervalo de confiança de 95% [IC95%]: 1,02–1,07; p< 0,01). A frequência geral de NAV foi de 26,7%. De acordo com a Escala de Quadril de Harris (Harris Hip Score), a função foi boa ou excelente em 86% dos quadris sem complicações, e ruim em 87,5% dos casos com NAV. Não houve relação estatística entre sangramento epifisário e desenvolvimento de NAV (p= 0,82). Conclusão A técnica de Dunn modificada restaura o alinhamento femoral e a função articular após o EEPF com instabilidade na ausência de complicações. Além disso, mostrou-se passível de reprodução em nossa população, com frequência de necrose da cabeça femoral de 26%. Abstract Objective To evaluate the safety and reproducibility of the surgery for unstable slipped capital femoral epiphysis (SCFE) through the modified Dunn technique in a single center cohort from Brazil. Methods We retrospectively analyzed a cohort of patients submitted to this procedure by a single surgeon who was a hip preservation specialist. Demographic data and radiographic angles were evaluated for the relative risk (RR) of avascular necrosis (AVN) using a log-binomial regression model with simple and random effects. Results Among the 30 patients (30 hips) with a mean age of 11.79 years at the time of the operation, there were 17 boys and 18 left hips, which were operated on in a mean of 11.5 days after the slip. The mean follow-up was of 38 months. The preoperative Southwick angle averaged 60.69° against 4.52° postoperatively (p< 0.001). A larger preoperative slip angle was associated with the development of AVN (RR: 1.05; 95% confidence interval [95%CI]: 1.02–1.07; p< 0.01). The overall AVN rate was of 26.7%. Function was good or excellent in 86% of uncomplicated hips, and poor in 87.5% of the partients who developed AVN, as graded by the Harris Hip Score. There was no statistical relationship between epiphyseal bleeding and AVN development (p= 0.82). Conclusion The modified Dunn technique is associated with restoration of the femoral alignment and function after unstable SCFE, when uncomplicated. Moreover, it was shown to be reproducible in our population, with a rate of 26% of femoral head necrosis.
