ABSTRACT Objective To describe and compare the clinical characteristics and outcomes of patients admitted to intensive care units during the first and second waves of the COVID-19 pandemic. Methods In this retrospective single-center cohort study, data were retrieved from the Epimed Monitor System; all adult patients admitted to the intensive care unit between March 4, 2020, and October 1, 2021, were included in the study. We compared the clinical characteristics and outcomes of patients admitted to the intensive care unit of a quaternary private hospital in São Paulo, Brazil, during the first (May 1, 2020, to August 31, 2020) and second (March 1, 2021, to June 30, 2021) waves of the COVID-19 pandemic. Results In total, 1,427 patients with COVID-19 were admitted to the intensive care unit during the first (421 patients) and second (1,006 patients) waves. Compared with the first wave group [median (IQR)], the second wave group was younger [57 (46-70) versus 67 (52-80) years; p<0.001], had a lower SAPS 3 Score [45 (42-52) versus 49 (43-57); p<0.001], lower SOFA Score on intensive care unit admission [3 (1-6) versus 4 (2-6); p=0.018], lower Charlson Comorbidity Index [0 (0-1) versus 1 (0-2); p<0.001], and were less frequently frail (10.4% versus 18.1%; p<0.001). The second wave group used more noninvasive ventilation (81.3% versus 53.4%; p<0.001) and high-flow nasal cannula (63.2% versus 23.0%; p<0.001) during their intensive care unit stay. The intensive care unit (11.3% versus 10.5%; p=0.696) and in-hospital mortality (12.3% versus 12.1%; p=0.998) rates did not differ between both waves. Conclusion In the first and second waves, patients with severe COVID-19 exhibited similar mortality rates and need for invasive organ support, despite the second wave group being younger and less severely ill at the time of intensive care unit admission.
ABSTRACT Objective: To describe clinical characteristics, resource use, outcomes, and to identify predictors of in-hospital mortality of patients with COVID-19 admitted to the intensive care unit. Methods: Retrospective single-center cohort study conducted at a private hospital in São Paulo (SP), Brazil. All consecutive adult (≥18 years) patients admitted to the intensive care unit, between March 4, 2020 and February 28, 2021 were included in this study. Patients were categorized between survivors and non-survivors according to hospital discharge. Results: During the study period, 1,296 patients [median (interquartile range) age: 66 (53-77) years] with COVID-19 were admitted to the intensive care unit. Out of those, 170 (13.6%) died at hospital (non-survivors) and 1,078 (86.4%) were discharged (survivors). Compared to survivors, non-survivors were older [80 (70-88) versus 63 (50-74) years; p<0.001], had a higher Simplified Acute Physiology Score 3 [59 (54-66) versus 47 (42-53) points; p<0.001], and presented comorbidities more frequently. During the intensive care unit stay, 56.6% of patients received noninvasive ventilation, 32.9% received mechanical ventilation, 31.3% used high flow nasal cannula, 11.7% received renal replacement therapy, and 1.5% used extracorporeal membrane oxygenation. Independent predictors of in-hospital mortality included age, Sequential Organ Failure Assessment score, Charlson Comorbidity Index, need for mechanical ventilation, high flow nasal cannula, renal replacement therapy, and extracorporeal membrane oxygenation support. Conclusion: Patients with severe COVID-19 admitted to the intensive care unit exhibited a considerable morbidity and mortality, demanding substantial organ support, and prolonged intensive care unit and hospital stay.

Abstract The main goal of the study was to determine the factors associated with dependence to perform instrumental activities of daily living in the elderly. A population-based case-control study was conducted, with 180 elderly people from Passo Fundo-RS, 2014. The cases were represented by 60 individuals aged ≥ 60 years, dependents to perform instrumental activities of daily living residents of the urban area of the city. The controls were represented by 120 individuals, not dependents to perform instrumental activities of daily living, residents of the urban area of the city. Crude and multivariate analysis using Poisson regression were performed to test the association between the outcome and the independent variables, estimating the crude and adjusted odds ratios (OR) and calculating the 95% confidence intervals respectively. All the variables with p ≤ 0.20 were included in the final model. Remained statistically significant after adjusted analysis: being aged 80 years or more (OR = 1.76; CI95%: 1.01-3.08), having studied from 1 to 4 years (OR = 2.36; CI95%: 1.35-4.14), being illiterate (OR = 2.98; CI95%: 1.52-5.84), having Parkinson’s disease (OR = 2.44; CI95%: 1.39-4.29) and the presence of cognitive impairment (OR = 1.88; CI95%: 1.30-2.72).

ABSTRACT This letter to the Editor aims to provide suggestions and recommendations for the management of urological conditions in times of COVID-19 crisis in Brazil and other low- and middle-income countries. It is important to highlight that one of the main characteristics of this pandemic is the oversaturation of the health system capacity, mostly due to a high demand for personal protective equipment (PPE), Hospital/ICU beds, as well as ventilators. In places with limited resources and where the health care systems are already saturated, such consideration is even more worrisome. Therefore, most worldwide authorities are recommending to avoid, as much as possible, patient’s elective visits to hospitals, as well as a judicious use of the operating room in order to mitigate the strain put on the health system. While efforts should be directed to the care of COVID-19 patients, other conditions (especially urgencies and oncological cases) must continue to be assisted. Thus, through a panel of experts, we have prepared a practical guide for urologists based on the recommendations from the main Urologic Associations, as well as data from the literature to support the suggested management. We will try to follow the standard guideline recommendations from the American Urological Association (AUA) and European Association of Urology (EAU), with the aim of pursuing the best outcomes possible. However, some recommendations were based on the consensus of the panel, taking into consideration the reality of developing countries and the unprecedented situation caused by the COVID-19 crisis.

ABSTRACT Introduction: Although the 2019 severe acute respiratory syndrome coronavirus 2 infection (SARS-CoV-2, COVID-19) pandemic poses new challenges to the healthcare system to provide support for thousands of patients, there is special concern about common medical emergencies, such as stroke, that will continue to occur and will require adequate treatment. The allocation of both material and human resources to fight the pandemic cannot overshadow the care for acute stroke, a time-sensitive emergency that with an inefficient treatment will further increase mortality and long-term disability. Objective: This paper summarizes the recommendations from the Scientific Department on Cerebrovascular Diseases of the Brazilian Academy of Neurology, the Brazilian Society of Cerebrovascular Diseases and the Brazilian Society of Neuroradiology for management of acute stroke and urgent neuro-interventional procedures during the COVID-19 pandemic, including proper use of screening tools, personal protective equipment (for patients and health professionals), and patient allocation.

Abstract In this work, CoCrMo-base alloys specimens were produced by additive manufacturing using selective laser melting (SLM) as well as CAD/CAM milling. For SLM specimens, spherical particles were laser processed at 1320°C under argon atmosphere and submitted to thermal stress relieving at 750°C–45min. Specimens from CoCrMo blocks were milled by CAD/CAM machining and sintered at 1300°C–60min. The materials from both techniques were characterized by relative density, dilatometry, SEM/BSE, OM and XRD analysis. The mechanical properties were determined by Vickers hardness and tensile tests. The specimens from both techniques exhibited single phase γCo (FCC) and CTE of 14 x10-6 °C-1. Relative density of 95.4%/ 85.6%, hardness values near 400HV/ 350HV and UTS of 905MPa/ 780MPa were measured for SLM and CAD/CAM specimens, respectively. Higher relative density is the main factor for increased mechanical properties of SLM specimens. On the other hand, both SLM and CAD/CAM specimens present properties in accordance with ISO-22674 recommendations, compatible with dental applications such as bridges containing four or more elements as well as fully dense pontics.

ABSTRACT New techniques for manufacturing of biomaterials using additive/substrate manufacturing such as melting laser sintering (SLM) or CAD/CAM milling use micrometric metal powders to build custom parts. CoCrMo alloys are commonly used as biomaterials due to biocompatibility, high corrosion resistance and good mechanical properties. In this work, Cr-Co-based powder was obtained by mechanical alloying of Cr-Co-Mo-W alloy, using different Ball/Powder-Ratios (BPR) and thermal treatments. The powders were deagglomerated and characterized by X-ray diffraction (XRD) and Scanning electron microscopy (SEM). Based on SEM micrographs, the particle size analysis was used to compare the particles spheroidizing degree, using Feret’s diameter. The XRD patterns shown Co(solid-solution) as crystalline phase. The results of particles size indicates a decrease of Feret’s diameter in BPR (Ball/Powder-Ratio) from 4:1 to 6:1 understanding that from this ratio there is no significant decrease of this relation but there is an average size increase with losses in morphological aspect. The comparison between atomized powders and powder developed in this work indicates that atomized products present relation close to one which indicates there is uniformity in the particles spherical shape. Comparing ground powders from 6:1 ratio (before and after thermal treatment) there is a considerable decrease of Feret’s diameter being it decreased from 1.75 to 1.4, an increase of about 46% in particles spheroidizing.

ABSTRACT: Problems with root-knot nematodes, caused by genus Meloidogyne, can be controlled through the introduction of resistance genes in commercial varieties of tomatoes. However, plants of the hybrid ‘Heinz 9992’ for industrial processing, carrying the Mi gene, were reported with their roots severely infected in experimental cultivation in Goiás State of Brazil with symptoms typical of galls caused by Meloidogyne spp. After dissecting the galls present in the root samples, the females of the nematodes were removed and afterwards, performed the analysis of the perineal pattern and the biochemical characterization by electrophoresis of isoenzymes, besides the pathogenicity test. The pathogen analyzed was identified as Meloidogyne incognita. The pathogenicity test confirmed the supplanting resistance of the tomato hybrid with the Mi gene by this virulent population of M. incognita.

ABSTRACT Objective: This study aims at analyzing retrospectively the clinical-functional and radiographic results of surgical treatment of the terrible elbow triad, with at least 12 months of postoperative follow-up evaluating elbow function. Methods: A group of patients for retrospective analysis from 2004 to 2015 was defined, in which 12 patients were studied. They underwent surgery due to fracture of the radial head, coronoid fracture, and elbow dislocation; they were evaluated by the Disabilities of the Arm, Shoulder and Hand (DASH) score, the degree of patient satisfaction, the degree of trauma energy, radiographic images, range of motion, and complications. Results: There was a higher incidence of Regan and Morrey type II coronoid process fractures; in relation to the injuries, nine patients had deinsertion of the brachialis. Half of the patients suffered a fall from their own height as the mechanism of trauma. The extent of elbow flexion and extension averaged 126.6 and 24.1 degrees, respectively; the averages for pronation and supination were 64.1 and 62.0 degrees, respectively. All patients presented muscle strength of grade IV or V. The mean DASH score was 14.3, the mean pain score was 2.5, and a majority of the patients were satisfied with the treatment. Conclusion: Despite the total loss of range of motion of the elbow, especially in extension, the treatment was satisfactory for most patients.

Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.
