RESUMO: No Brasil, a ureia é o fertilizante nitrogenado mais utilizado para melhorar a produção de forragem. No entanto, seu uso excessivo pode causar impactos ambientais por meio de perdas de nitrogênio (N), como a volatilização da amônia (NH3). Portanto, o objetivo do presente estudo foi ajustar a volatilização de NH3 da ureia aplicada em pastos tropicais em três condições de chuva utilizando modelos matemáticos. Dados foram coletados de pastos de capim-marandu (Brachiaria brizantha) adubado com 50 kg N ha-1 em condições úmidas, intermediárias e secas. A volatilização da NH3 foi medida em cinco câmaras semiabertas durante 21 dias. Os modelos, linear, quadrático, exponencial, Gompertz, Groot e Richards foram testados para ajuste e estimativa da volatilização do NH3. Os modelos de Gompertz, Groot e Richards geraram predições semelhantes aos dados observados, com alto coeficiente de determinação, indicando um melhor ajuste dessas equações aos dados, com acurácia e precisão. No entanto, o modelo Groot foi selecionado devido ao menor erro quadrático médio das predições (0,29% de N total perdido como NH3). A maior volatilização de NH3 ocorreu em condições climáticas úmida, seguido por intermediária e seca (20,2; 17,0 e 11,3% de N total perdido como NH3, respectivamente). ABSTRACT: In Brazil, urea is the most used nitrogen (N) fertilizer to improve forage production. However, their excessive use can cause environmental impacts through N losses, such as ammonia (NH3) volatilization. Therefore, the current study adjusted and estimated the NH3 volatilization from urea applied on tropical pastures in three rainfall conditions using mathematical models. Data were collected from Marandu grass (Brachiaria brizantha) fertilized with 50 kg N ha-1 during wet, intermediate, and dry conditions. Ammonia volatilization was measured in five semi-open chambers for 21 days. The linear, quadratic, exponential, Gompertz, Groot, and Richards models were tested for fitting and estimating the NH3 volatilization. The Gompertz, Groot, and Richards models generated predictions similar to the observed data, with a high determination coefficient, indicating a better fit of these equations to data, with precision and accuracy. However, the Groot model was selected due to the lowest root mean square error of prediction (0.29 % total N lost as NH3). The greatest N loss as NH3 volatilization occurred in the wet, followed by intermediate and dry conditions (20.2, 17.0, and 11.3 % total N lost as NH3, respectively). Therefore, nitrogen losses as NH3 volatilization after application of 50 kg N ha-1, as urea source, are altered according to the weather conditions, reaching 20% of N added in the wet rainfall period. RESUMO Objetivo: Comparar e analisar parâmetros de função pulmonar e de mecânica respiratória entre escolares saudáveis e com fibrose cística (FC). Métodos: Estudo transversal que incluiu escolares saudáveis (GES) e com FC (GFC), com idades entre seis e 13 anos, provenientes de instituições de ensino e de um centro de referência da FC em Florianópolis/SC, Brasil, pareados por idade e sexo, respectivamente. Inicialmente, conduziu-se avaliação antropométrica para pareamento e caracterização de ambos os grupos e, no GFC, consultou-se prontuário médico para registro dos dados de colonização bacteriana, genótipo e gravidade da doença (Escore de Schwachman-Doershuk — ESD). Para a avaliação da função pulmonar, realizou-se espirometria e a avaliação da mecânica respiratória foi conduzida por meio do sistema de oscilometria de impulso. Resultados: Participaram 110 escolares, 55 em cada grupo. No GFC, 58,2% foram classificados pelo ESD como excelentes, 49,1% apresentaram genótipo ∆F508 heterozigoto e 67,3% eram colonizados por alguma patógeno. ABSTRACT Objective: To compare and analyze pulmonary function and respiratory mechanics parameters between healthy children and children with cystic fibrosis. Methods: This cross-sectional analytical study included healthy children (HSG) and children with cystic fibrosis (CFG), aged 6–13 years, from teaching institutions and a reference center for cystic fibrosis in Florianópolis/SC, Brazil. The patients were paired by age and sex. Initially, an anthropometric evaluation was undertaken to pair the sample characteristics in both groups; the medical records of CFG were consulted for bacterial colonization, genotype, and disease severity (Schwachman-Doershuk Score — SDS) data. Spirometry and impulse oscillometry were used to assess pulmonary function. Results: In total, 110 children were included, 55 in each group. In the CFG group, 58.2% were classified as excellent by SDS, 49.1% showed the ΔF508 heterozygotic genotype, and 67.3% were colonized by some pathogens. Statistical analysis revealed significant differences between both groups (p<0.05) in most pulmonary function parameters and respiratory mechanics. Conclusions: Children with cystic fibrosis showed obstructive ventilatory disorders and compromised peripheral airways compared with healthy children. These findings reinforce the early changes in pulmonary function and mechanics associated with this disease. Abstract The article aims to identify stage of the food supply chain (FSC) has the greatest food loss and waste (FLW), the factors that influence and economic, social and environmental impacts in Latin America countries. We carried out a scoping review of observational studies, case reports and interventional studies in January 2023. Searches were performed in scientific databases and hand-searching of reference lists. Data on the included studies were summarized with narrative synthesis. In total 16 articles met the inclusion criteria. The greatest FLW occur in the early and middle stages of the FSC, mainly during storage. The main causes were connected to financial, managerial and operational limitations related in harvesting techniques, storage and cooling facilities, infrastructure and marketing systems. Food waste (FW) is also a result of lack of appropriate storage facilities and efficient transport systems, market fluctuations and systems. Only one study presented results on the environmental impact of FW. There is a higher occurrence of food loss, characterized by decrease in the quantity and quality of food in the first three stages of FSC.

Resumo O objetivo do artigo é identificar etapa da cadeia de abastecimento de alimentos (CAA) com maior perda e desperdício de alimentos (PDA), os fatores que influenciam e os impactos econômicos, sociais e ambientais nos países da América Latina. Realizamos revisão de escopo de estudos observacionais, relatos de caso e estudos intervencionais em janeiro de 2023. As buscas foram realizadas em bases de dados científicas e busca manual de listas de referências. Os dados dos estudos incluídos foram resumidos com síntese narrativa. No total, 16 artigos atenderam aos critérios de elegibilidade. As maiores PDA ocorrem nas fases inicial e intermediária do CAA, principalmente durante o armazenamento. Principais causas das perdas eram financeiras, gerenciais e operacionais relacionadas a técnicas de colheita, instalações de armazenamento e resfriamento, infraestrutura e sistemas de comercialização. Desperdício é resultado da falta de instalações de armazenamento adequadas e sistemas de transporte eficientes, flutuações de mercado e de sistemas. Um estudo apresentou resultados sobre o impacto ambiental das perdas. Há maior ocorrência de perdas alimentares, caracterizadas pela diminuição da quantidade e qualidade dos alimentos nas etapas iniciais da CAA.

Abstract Background The SARS-CoV-2 outbreak has led to radical transformation in social, economic, and healthcare systems. This may lead to profound indirect consequences on clinical presentation and management of patients with ST-segment–elevation myocardial infarction. Objectives The objective of this study was to describe the characteristics, management, and outcomes of patients admitted with acute myocardial infarction with ST-segment elevation (STEMI), in two tertiary reference hospitals during the SARS-CoV-2 outbreak and compare them with patients admitted in the previous year. Methods We analyzed data from a multicenter STEMI registry from reference centers in the South Region of Brazil from March 2019 to May 2021. The beginning of the COVID-19 outbreak was considered to be March 2020 and compared to the same period in 2019. Only patients with STEMI submitted to primary percutaneous coronary intervention (PCI) were included in the analysis. Mortality rates were compared with chi-square test. All hypothesis tests had a two-sided significance level of 5%. Results A total of 1169 patients admitted with STEMI were enrolled in our registry, 635 of whom were admitted during the pandemic period. The mean age of our sample was 61.6 (± 12.4) years, and 66.7% of patients were male. Pain-to-door time and door-to-balloon time were longer during the pandemic period. However, there was no difference in mortality rates or major adverse cardiovascular outcomes (MACE). Conclusions We observed a stable incidence of STEMI cases in our registry during the SARS-CoV-2 outbreak with higher pain-to-door time and door-to-balloon time, without any influence on mortality rates however. Abstract Objective To measure the average time for the diagnosis and for the therapeutic prescription of Continuous Positive Airway Pressure (CPAP) at a hospital in Botucatu Medical School - State University São Paulo, UNESP. Method A retrospective observational study was carried out by collecting data from the electronic medical records of patients over 18-years of age, who had a diagnostic polysomnography testing scheduled between January and December 2017. Result Of the 347 patients eligible for the study, 94 (27.1%) missed follow-up and 103 (29.7%) had a referral for CPAP use. Until February 2021, only 37 (35.9%) of these patients had already acquired and were using the device, the remaining 66 (64.1%) were waiting or gave up the therapy. The mean value of the waiting time interval between the referral of the diagnostic test and its performance was equivalent to 197 days (6.5 months). The mean time between diagnostic polysomnography and CPAP prescription was 440-days (14.5-months), with a total mean time of 624 days (21-months). Conclusion As in other services, the diagnostic-therapeutic flow proved to be highly inefficient, with a long waiting period, difficult access to treatment and a high dropout rate. These findings highlight the need to establish new patient-centered strategies with measures that speed up the flow and facilitate access to CPAP, in order to reduce the morbidity and mortality associated with this condition. RESUMO Objetivo: Investigar o efeito do broncodilatador na mecânica respiratória e função pulmonar de crianças e adolescentes com fibrose cística. Métodos: Estudo transversal em crianças e adolescentes com fibrose cística clinicamente estáveis, com idade entre seis e 15 anos. Os participantes realizaram avaliações por meio do sistema de oscilometria de impulso e espirometria antes e 15 minutos depois da inalação de broncodilatador. Foi aplicado o teste Kolmogorov-Smirnov para verificar a distribuição da amostra. Para comparar os dados antes e depois do broncodilatador foram utilizados os testes t de Student e Wilcoxon. Resultados: Participaram do estudo 54 indivíduos com média de idade de 9,7±2,8 anos. Houve melhora estatisticamente significativa nos parâmetros do oscilometria de impulso e espirometria após a inalação com broncodilatador. ABSTRACT Objective: To investigate the effect of bronchodilator on the respiratory mechanics and pulmonary function of children and adolescents with cystic fibrosis. Methods: Cross-sectional study on clinically stable children and adolescents with cystic fibrosis aged from six to 15 years. Participants underwent impulse oscillometry and spirometry evaluations before and 15 minutes after bronchodilator inhalation. The Kolmogorov-Smirnov test was applied to verify the sample distribution, and the Student's t-test and Wilcoxon test were used to compare the data before and after bronchodilator inhalation. Results: The study included 54 individuals with a mean age of 9.7±2.8 years. The analysis showed a statistically significant improvement in impulse oscillometry and spirometry parameters after bronchodilator inhalation. However, according to the American Thoracic Society (ATS) and European Respiratory Society (ERS) recommendations (2020 and 2021), this improvement was not sufficient to classify it as a bronchodilator response. Método: Estudo transversal com 300 puérperas de uma maternidade de referência estadual, realizado de março de 2020 a janeiro de 2021. Foi estimada a frequência da peregrinação segundo características socioeconômicas e de assistência pré-natal. Análises com o Teste Qui-Quadrado de Pearson selecionaram variáveis para a regressão de Poisson ajustada. Resultados: A frequência de peregrinação anteparto por mais de um serviço de saúde foi de 34,3%. Não conhecer a maternidade de referência (1,16; IC95% 1,04–1,30) e não residir próximo à maternidade de referência (1,16; IC95%: 1,03–1,31) estiveram associados à ocorrência de peregrinação entre as mulheres. ABSTRACT Objective: To identify factors associated with antepartum pilgrimage in pregnant women in Fortaleza, Ceará, Brazil. Method: A cross-sectional study with 300 postpartum women from a state reference maternity hospital, carried out from March 2020 to January 2021. The frequency of pilgrimage was estimated according to socioeconomic characteristics and prenatal care. Analysis with Pearson’s chi-square test selected variables for adjusted Poisson regression. Results: The frequency of antepartum pilgrimage to more than one health service was 34.3%. Not knowing the reference maternity hospital (1.16; 95%CI: 1.04–1.30) and not living close to the reference maternity hospital (1.16; 95%CI: 1.03–1.31) were associated with the occurrence of pilgrimage among women. Personal characteristics and prenatal care were not associated. Conclusion: There was an association between antepartum pilgrimage and lack of knowledge of the reference maternity hospital and residence far from that maternity hospital, which requires better team communication and the guarantee of easier access to obstetric care services, through effective implementation of regionalization of maternal care. Método: Estudio transversal con 300 puérperas de una maternidad de referencia estatal, realizado de marzo de 2020 a enero de 2021. Se estimó la frecuencia de peregrinación según características socioeconómicas y de atención prenatal. Análisis con Prueba de Chi-Cuadrado de Pearson variables seleccionadas para regresión de Poisson ajustada. Resultados: La frecuencia de peregrinación anteparto a más de un servicio de salud fue del 34,3%. Desconocer la maternidad de referencia (1,16; IC95%: 1,04–1,30) y no vivir cerca de la maternidad de referencia (1,16; IC95%: 1,03–1,31) se asociaron con la ocurrencia de peregrinación entre las mujeres. Resumo Objetivo A disfunção executiva após traumatismo cranioencefálico pode levar à diminuição da funcionalidade devido ao dano direto ao lobo frontal ou ao rompimento de suas conexões com outras partes do cérebro. Este estudo objetivou comparar o desempenho de adultos com e sem traumatismo cranioencefálico em tarefas de função executiva. Método O estudo apresenta delineamento transversal descritivo de análise quantitativa. Participaram 43 sujeitos divididos em grupo clínico (N =23, idade M = 34.3 [10.86]) e controle (N = 20, idade M = 38.3 [10.7]). A bateria foi composta por tarefas tradicionais de função executiva e a Tarefa Ecológica do Hotel. Resultados Encontrou-se diferença significativa na performance dos grupos nas medidas de planejamento (número de tarefas [t = -3.06; p < 0.01] e escore de planejamento [t = -3.06; p < 0.01]), reforçando a vantagem do uso de instrumentos tradicionais e ecológicos na avaliação neuropsicológica de funções executivas. Abstract Objective Executive dysfunction after traumatic brain injury may lead to reduced daily functionality due to direct damage to the frontal lobe or to disruption of its connections to other brain regions. This study aimed to compare the performance of adults with and without traumatic brain injury on executive functions tasks. Method The present study had a descriptive transversal design and included 43 participants divided into a clinical group (N = 23, age M = 34.3 [10.86]) and a control group (N = 20, age M = 38.3 [10.7]). The battery consisted of traditional instruments and the Ecological Hotel Task. Results A significant difference was found in the performance of the groups in the planning measures (number of tasks [t = -3.06; p < 0.01] and planning score [t = -3.06; p < 0.01]), highlighting the advantages of using both the traditional performance paradigms and the ecological tasks in neuropsychological assessment of executive functions. RESUMO Objetivo mapear, por meio de revisão de escopo, evidências sobre a relação entre cognição e sono de indivíduos com esclerose múltipla. Estratégia de pesquisa foi realizada busca ativa em bases de dados e na literatura cinzenta, a fim de identificar estudos primários, com textos disponibilizados gratuitamente, sem limite temporal e de idioma. Critérios de seleção os dados foram analisados por meio de análises descritivas e da análise de conteúdo. Resultados treze publicações foram elegíveis. ABSTRACT Purpose to map, through a scoping review, evidence on the relationship between cognition and sleep in individuals with multiple sclerosis. Research strategy an active search of databases and gray literature was conducted to identify primary studies with freely available texts, without time and language limits. Selection criteria data were analyzed using descriptive analysis and content analysis. Results thirteen publications were eligible. Mainly cross-sectional studies were found, published in English, between 2016 and 2022, mostly concentrated in the northern hemisphere. The studies reviewed show that sleep disorders, whether identified objectively or subjectively, coexisted with cognitive decline in people with multiple sclerosis. ABSTRACT Background: This study investigated the self-rated general health, mental health, and work absenteeism among patients with laboratory-confirmed chikungunya. Methods: Telephone interviews were conducted with 63 patients ≥22 months after infection. Results: Patients who reported (N=42) or did not report (N=21) chronic arthralgia, defined by duration ≥90 days, had different frequencies for low scores for general health (68.3% vs. 30.0%, respectively; prevalence ratio, 95% confidence interval: 2.3, 1.1-4.6), symptoms of depression (31.7% vs. 15.0%; 2.1, 0.7-6.6), symptoms of anxiety (43.9% vs. 35.0%; 1.3, 0.6-2.5), and work absenteeism (76.5% and 40.0%; 1.9, 0.9-4.2). BACKGROUND In response to the coronavirus disease 2019 (Covid-19) pandemic, Brazil authorised the Astra Zeneca/Fiocruz vaccine in January 2021. As the Delta variant emerged in May 2021, interval between vaccine doses was adjusted. By September 2021, the Brazilian National Immunisation Program recommended a booster dose for individuals over 70, and later expanded the recommendation to all adults. OBJECTIVES Assess the equivalence of IgG antibody response against the Covid-19 S protein before and approximately 28 days after the third dose of a Covid-19 recombinant vaccine. Two groups received initial two doses with intervals of eight and 12 weeks. METHODS This is a phase IV clinical study, uncontrolled, non-randomised. The study proposes calculating the ratio of geometric means titres (GMT) 28 days after the third dose, with a target ratio of confidence interval (CI) between 0.77 and 1.3. FINDINGS In the primary endpoint, there was no equivalence between the eight- and 12-week intervals with a slight variation favouring the eight-week group. Post-third dose, both groups showed increases titres at 28 days, three months, six months and 12 months. Both groups responded similarly to Delta and Omicron BA.1, with a more significant increase for Delta. MAIN CONCLUSIONS The study showed strong and consistent immune response in all age groups receiving the Covid-19 recombinant vaccine. Resumo O objetivo do artigo é identificar etapa da cadeia de abastecimento de alimentos (CAA) com maior perda e desperdício de alimentos (PDA), os fatores que influenciam e os impactos econômicos, sociais e ambientais nos países da América Latina. Realizamos revisão de escopo de estudos observacionais, relatos de caso e estudos intervencionais em janeiro de 2023. As buscas foram realizadas em bases de dados científicas e busca manual de listas de referências. Os dados dos estudos incluídos foram resumidos com síntese narrativa. No total, 16 artigos atenderam aos critérios de elegibilidade. As maiores PDA ocorrem nas fases inicial e intermediária do CAA, principalmente durante o armazenamento. Abstract The article aims to identify stage of the food supply chain (FSC) has the greatest food loss and waste (FLW), the factors that influence and economic, social and environmental impacts in Latin America countries. We carried out a scoping review of observational studies, case reports and interventional studies in January 2023. Searches were performed in scientific databases and hand-searching of reference lists. Data on the included studies were summarized with narrative synthesis. In total 16 articles met the inclusion criteria. The greatest FLW occur in the early and middle stages of the FSC, mainly during storage. The main causes were connected to financial, managerial and operational limitations related in harvesting techniques, storage and cooling facilities, infrastructure and marketing systems. Food waste (FW) is also a result of lack of appropriate storage facilities and efficient transport systems, market fluctuations and systems. Only one study presented results on the environmental impact of FW. There is a higher occurrence of food loss, characterized by decrease in the quantity and quality of food in the first three stages of FSC. Resumo O objetivo desta pesquisa foi investigar a ocorrência de enterobactérias em passeriformes do gênero Sporophila, provenientes do tráfico ilegal de animais, e analisar sua resistência antimicrobiana. Um total de 35 aves, aparentemente saudáveis e provenientes de criações ilegais, foram encaminhadas ao Laboratório de Estudos Ornitológicos da Universidade Estadual do Ceará (Fortaleza, Ceará, Brasil) por órgãos ambientais. Amostras obtidas a partir de suabes cloacais foram coletadas de cada ave e submetidas a processamento microbiológico tradicional, utilizando técnicas padrão de cultivo e identificação bacteriana. O teste de sensibilidade aos antimicrobianos foi avaliado pelo método de disco de difusão. Das amostras examinadas, 23 (65,7%) apresentaram a presença de enterobactérias, sendo Escherichia coli (28,6%) e Serratia liquefaciens (25,7%) as mais frequentes. A resistência antimicrobiana referente aos isolados totais foi observada com mais frequência em relação à ciprofloxacina (28,1%), seguida por tetraciclina (25,0%) e enrofloxacina (18,8%). Especificamente em relação à Serratia liquefaciens, a taxa de resistência foi a mais altas, atingindo 66,6% dos isolados. Com base nos resultados obtidos, podemos concluir que as amostras cloacais das aves apresentaram isolamento de diversos membros da ordem Enterobacteriales, sendo E. coli e S. liquefaciens as mais frequentes. Também foi observada a ocorrência de resistência antimicrobiana a diversos fármacos utilizados, assim como casos de multiresistência, o que mostra que esse problema está presente em pássaros silvestres oriundo de manutenções ilegais. Isso ressalta a necessidade de considerar a presença de bactérias resistentes a antimicrobianos em programas de soltura, visando evitar a dispersão desses microrganismos no meio ambiente. Abstract We investigate the occurrence of enterobacteria and antimicrobial resistance in passerines of the genus Sporophila seized from illegal trafficking. A total of 35 birds, apparently healthy and from illegal breeding, were sent to the Laboratory of Ornithological Studies at the State University of Ceará (Fortaleza, Ceará, Brazil) by environmental agencies. Cloacal swabs were collected from each bird and subjected to traditional microbiological processing, using standard cultivation and bacterialidentification techniques. We conducted antimicrobial susceptibility testing using the disc-diffusion method. Twenty-three birds (65.7%) exhibited enterobacteria, with Escherichia coli (28.6%) and Serratia liquefaciens (25.7%) being the most common. Antimicrobial resistance in total isolates was observed most frequently in relation to ciprofloxacin (28.1%), followed by tetracycline (25.0%) and enrofloxacin (18.8%). The rate of resistance to Serratia liquefaciens was the highest (i.e., 66.6% of isolates). The birds’ cloacal samples revealed several members of the Enterobacterales order, with E. coli and S. liquefaciens being the most prevalent. We also observed antimicrobial resistance to several drugs, as well as cases of multi-resistance. Antimicrobial resistance is clearly present in wild birds resulting from illegal keeping. Abstract The objective of the present study was to investigate the frequency of oral leukoplakia and oral erythroplakia among young patients from three Brazilian reference centers in Oral and Maxillofacial Pathology. A retrospective study was carried out from 2011 to 2021 on 861 patients diagnosed with oral leukoplakia and oral erythroplakia. Demographic and clinicopathological data were evaluated. Fisher’s exact test was used to evaluate the association among sex, age, anatomical location, and histopathological diagnosis. A total of 83 (9.64%) cases involved young patients (aged <40 years). Among these, biopsy records were included in 31 (37.34%) cases, all of which received a clinical diagnosis of oral leukoplakia. Seventeen (54.84%) patients were female, mostly in their fourth decade of life (n = 22/70.97%), and their mean age at diagnosis was 32.61(± 5.21) years. Among informed cases, seven (22.58%) patients were smokers. The lateral border of the tongue (n = 9/29.03%) was the most affected site. In 13 (41.94%) cases, oral leukoplakias showed a homogeneous appearance. The mean size of the lesions was 1.47 cm (0.2–3.0 cm) and the mean time of disease progression was 64.37 (± 65.90) months. The histopathological analysis showed that 11 cases (35.48%) exhibited some degree of epithelial dysplasia. Acanthosis and/or hyperkeratosis were observed in 20 cases (64.52%). No significant associations were observed between sex and anatomical location, age and anatomical location, nor between sex and histological diagnosis (p > 0.05).
