Gasto catastrófico em crianças com deficiência visual: estudo transversal com cuidadores no Rio de Janeiro, Brasil

Resumo: O cuidado de um filho com deficiência visual pode vir a afetar a renda do cuidador e, por sua vez, a renda da família. Sob essa realidade, há de se considerar o gasto catastrófico consequente do aumento de despesas e da redução de renda, seja pelo desemprego, pela redução do número de horas trabalhadas ou pela dificuldade de (re)inserção no mercado de trabalho. Perante esse cenário, o objetivo principal deste estudo foi estimar o gasto catastrófico atribuído ao cuidador de crianças cegas ou com baixa visão em centros de referência em educação para cegos, oftalmologia e saúde infantil localizados no Município do Rio de Janeiro, Brasil, identificando quais fatores estão associados a uma maior ou menor prevalência desse gasto. Observou-se que 53,3% dos cuidados de crianças com cegueira comprometem 40% ou mais da renda. Entre os cuidadores de crianças com baixa visão, o gasto catastrófico é mais ameno, comprometendo no mínimo 40% da renda para 36,8% dos cuidadores. Os fatores associados à maior prevalência de gasto catastrófico foram idade do cuidador, número de moradores na residência, maior escolaridade, menor renda domiciliar, reformas na residência, plano de saúde, aquisição de empréstimos, venda de bens, quantidade de unidades de saúde que a criança recebe tratamento e parentesco do cuidador principal. A carga que recai sobre os cuidadores de crianças com deficiência visual indica uma situação de vulnerabilidade que mostra a necessidade de acesso aos mecanismos de proteção financeira e social, por meio de políticas que sejam capazes de atender esse grupo.
Resumen: El cuidado de un niño con discapacidad visual puede impactar los ingresos del cuidador y, a su vez, de la familia. En este escenario, es necesario considerar el gasto catastrófico resultante del aumento de los gastos o la reducción de los ingresos, ya sea por desempleo, reducción del número de horas trabajadas o por la dificultad de inserción o reinserción en el mercado laboral. Ante esto, el objetivo principal de este estudio fue estimar el gasto catastrófico atribuido al cuidador de niños ciegos o con baja visión en centros de referencia en educación para ciegos, oftalmología y salud infantil, ubicados en el municipio de Río de Janeiro, Brasil, con el fin de identificar qué factores se asocian con una mayor o menor prevalencia de este gasto. Se observó que el 53,3% de los cuidados de niños con discapacidad visual comprometen más del 40% de los ingresos totales. Mientras tanto, el cuidado de niños con baja visión tiene un menor gasto catastrófico, comprometiendo menos del 40% de los ingresos según el 36,8% de los cuidadores. La mayor prevalencia de gasto catastrófico estuvo asociada a los siguientes factores: edad del cuidador, número de residentes en el hogar, mayor nivel de estudios, bajos ingresos familiares, remodelaciones en el hogar, seguro de salud, adquisición de préstamos, venta de bienes, cantidad de centros de salud en las que el niño acude al tratamiento y parentesco del cuidador principal. Los cuidadores de niños con discapacidad visual enfrentan una situación de vulnerabilidad, lo que apunta a una necesidad de acceder a acciones de protección financiera y social mediante políticas dirigidas a esta población.
Abstract: Caring for a visually impaired child can affect the caregiver’s income and, in turn, the family’s. Catastrophic spending resulting from increased expenses and reduced income must be taken into account, whether due to unemployment, a reduction in the number of hours worked or the difficulty of entering or reentering the job market. Given this scenario, the main objective of this study was to estimate the catastrophic spending attributed to the caregiver of blind or low-vision children in reference centers for education for the blind, ophthalmology and child health located in the city of Rio de Janeiro, Brazil, identifying which factors are associated with a higher or lower prevalence of this expenditure. It was found that 53.3% of care for blind children involved 40% or more of their income. Among the caregivers of children with low vision, catastrophic spending is milder, compromising at least 40% of income for 36.8% of the caregivers. The factors associated with a higher prevalence of catastrophic spending were the caregiver’s age, the number of residents in the household, higher schooling, lower household income, renovations to the home, health insurance, taking out loans, selling assets, the number of health units where the child receives treatment and the relationship of the main caregiver. The burden placed on caregivers of visually impaired children indicates a situation of vulnerability that shows the need for access to financial and social protection mechanisms, through policies that are capable of serving this group.

ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Resumo: O cuidado de um filho com deficiência visual pode vir a afetar a renda do cuidador e, por sua vez, a renda da família. Sob essa realidade, há de se considerar o gasto catastrófico consequente do aumento de despesas e da redução de renda, seja pelo desemprego, pela redução do número de horas trabalhadas ou pela dificuldade de (re)inserção no mercado de trabalho. Perante esse cenário, o objetivo principal deste estudo foi estimar o gasto catastrófico atribuído ao cuidador de crianças cegas ou com baixa visão em centros de referência em educação para cegos, oftalmologia e saúde infantil localizados no Município do Rio de Janeiro, Brasil, identificando quais fatores estão associados a uma maior ou menor prevalência desse gasto. Observou-se que 53,3% dos cuidados de crianças com cegueira comprometem 40% ou mais da renda. Entre os cuidadores de crianças com baixa visão, o gasto catastrófico é mais ameno, comprometendo no mínimo 40% da renda para 36,8% dos cuidadores. Os fatores associados à maior prevalência de gasto catastrófico foram idade do cuidador, número de moradores na residência, maior escolaridade, menor renda domiciliar, reformas na residência, plano de saúde, aquisição de empréstimos, venda de bens, quantidade de unidades de saúde que a criança recebe tratamento e parentesco do cuidador principal. A carga que recai sobre os cuidadores de crianças com deficiência visual indica uma situação de vulnerabilidade que mostra a necessidade de acesso aos mecanismos de proteção financeira e social, por meio de políticas que sejam capazes de atender esse grupo. Abstract: Caring for a visually impaired child can affect the caregiver’s income and, in turn, the family’s. Catastrophic spending resulting from increased expenses and reduced income must be taken into account, whether due to unemployment, a reduction in the number of hours worked or the difficulty of entering or reentering the job market. Given this scenario, the main objective of this study was to estimate the catastrophic spending attributed to the caregiver of blind or low-vision children in reference centers for education for the blind, ophthalmology and child health located in the city of Rio de Janeiro, Brazil, identifying which factors are associated with a higher or lower prevalence of this expenditure. It was found that 53.3% of care for blind children involved 40% or more of their income. Among the caregivers of children with low vision, catastrophic spending is milder, compromising at least 40% of income for 36.8% of the caregivers. The factors associated with a higher prevalence of catastrophic spending were the caregiver’s age, the number of residents in the household, higher schooling, lower household income, renovations to the home, health insurance, taking out loans, selling assets, the number of health units where the child receives treatment and the relationship of the main caregiver. Resumen: El cuidado de un niño con discapacidad visual puede impactar los ingresos del cuidador y, a su vez, de la familia. En este escenario, es necesario considerar el gasto catastrófico resultante del aumento de los gastos o la reducción de los ingresos, ya sea por desempleo, reducción del número de horas trabajadas o por la dificultad de inserción o reinserción en el mercado laboral. Ante esto, el objetivo principal de este estudio fue estimar el gasto catastrófico atribuido al cuidador de niños ciegos o con baja visión en centros de referencia en educación para ciegos, oftalmología y salud infantil, ubicados en el municipio de Río de Janeiro, Brasil, con el fin de identificar qué factores se asocian con una mayor o menor prevalencia de este gasto. Se observó que el 53,3% de los cuidados de niños con discapacidad visual comprometen más del 40% de los ingresos totales. Mientras tanto, el cuidado de niños con baja visión tiene un menor gasto catastrófico, comprometiendo menos del 40% de los ingresos según el 36,8% de los cuidadores. La mayor prevalencia de gasto catastrófico estuvo asociada a los siguientes factores: edad del cuidador, número de residentes en el hogar, mayor nivel de estudios, bajos ingresos familiares, remodelaciones en el hogar, seguro de salud, adquisición de préstamos, venta de bienes, cantidad de centros de salud en las que el niño acude al tratamiento y parentesco del cuidador principal. Abstract The Santos Basin (SB) is the main petroliferous basin in the Brazilian continental margin and one of the most studied marine areas in Brazil. However, historical data suggest that new efforts should be carried out to acquire quantitative biological data, especially in the deep sea, to establish the baseline of essential ocean variables in different ecosystems for future monitoring programs. The Brazilian energy company Petrobras planned and executed 24 oceanographic cruises over a period of 2 years to assess the benthic (SANSED cruise) and pelagic (SANAGU cruise) systems of the SB (356 days at sea in 2019 and 2021/2022). These efforts were part of the Santos Project, which comprised a comprehensive environmental study aimed at investigating benthic and pelagic variables to characterize ecology, biogeochemistry, thermohaline properties of water masses, and ocean circulation patterns, geomorphology, and sedimentology, as well as organic and inorganic chemistry. Here we present the detailed sampling designs and the field methods employed on board, during the SB scientific cruises. All sampling protocols were based on standardized approaches. For the benthos analyses, triplicate sediment samples were performed using a GOMEX-type box corer (0.25 m²) or a large modified Van Veen grab (0.75 m²) at 100 stations ranging from 25 to 2400 m depth. At each station, 25 geochemical and physico-chemical parameters were analyzed in addition to micro-, meio-, and macrofauna and living foraminifera samples. For the pelagic system, 60 stations were selected to investigate the plankton community, ranging in size from pico- to macroplankton, through vertical, horizontal, and oblique net hauls (20, 200, and 500 μm mesh size), as well as 25 biogeochemical parameters collected with an aid of a CTD-rosette sampler. Part of this scientific information also serves the Regional Environmental Characterization Project (PCR-BS) in support of Petrobras’ Santos Basin drilling licensing process led by the Brazilian Environmental Agency – IBAMA. This project contributes to the sustainable development of the SB, in line with the guidelines of the United Nations Decade of Ocean Science for Sustainable Development. Resumen La agroindustria del aceite de oliva es un importante sector económico en los países mediterráneos, cuya producción va acompañada de la generación de enormes cantidades de residuos orgánicos. La reciente proliferación de olivares superintensivos, más rentables, genera una mayor demanda de agua para riego y está aumentando la producción de estos residuos. La conversión de los residuos de almazara en biochar mediante pirólisis, para su posterior uso como enmienda orgánica en suelos de olivar superintensivo, podría contribuir a solucionar ambos problemas, ya que el biochar tiene una alta porosidad y contenido en carbono orgánico de alta estabilidad. Sin embargo, los efectos de su aplicación en las propiedades del suelo y en la fisiología del olivo no son bien conocidos. Este estudio aborda los efectos de la aplicación de biochar de alperujo de almazara, de compost vegetal (enmienda orgánica tradicional) y de la mezcla de ambos sobre las propiedades de un suelo (Xerochrept) y en la fisiología de olivos de “arbequina”. Esta plantación de olivar superintensivo y riego deficitario está situada en la estación experimental del IRNAS-CSIC "La Hampa" (Coria del Río, Sevilla, España). Tanto el biochar como el compost redujeron la resistencia a la penetración e incrementaron el contenido en Carbono orgánico y la humedad del suelo. La aplicación de biochar resultó más eficaz para aumentar la capacidad de retención de agua y la humedad del sueloy el estatus hídrico de los olivos en la fase anterior a la cosecha sin incrementar las tasas de respiración (emisión de CO2) del suelo.
Abstract The olive oil agro-industry is an important economic sector in Mediterranean countries, whose production is accompanied by the generation of huge quantities of organic waste. The recent proliferation of super-intensive olive groves, more profitable, generates a greater demand of water for irrigation and is increasing the production of organic waste. The conversion of these olive mill wastes into biochar by pyrolysis, for subsequent use as an organic amendment in super-intensive olive grove soils, could contribute to solving both problems, since biochar has a high porosity and a highly stable organic carbon content. Nevertheless, the effects of its application on soils properties and plant physiology are still not well known. This study addresses the effects of the application of olive mill pomace (alperujo) biochar, 100% vegetable compost (traditionally used organic amendment) and the mix of both organic amendments on the properties of a Xerochrept soil and on the physiology of “arbequina” olive trees”. The super-intensive olive trees plantation, with irrigation deficit, is located in the IRNAS-CSIC field station "La Hampa" (Coria del Río, Seville, Spain). Both biochar and compost reduced soil penetration resistance and increased soil organic Carbon and moisture content. However, the application of biochar was more effective than the addition of green compost or the compost+biochar mixture in increasing soil moisture. Biochar application improved soil physical properties and plant water status at the pre-harvest phase without increasing soil respiration (CO2 emission) rates.

Abstract Jackfruit is an exotic fruit present in the Northeast region of Brazil, presenting a great economic potential for the region because of the internal and external commercialization of its fruit. Its seed is rich in carbohydrates, protein, minerals and vitamins, and is an alternative source of starch and for food. Germination is an easily accessible, inexpensive technology for the production of sprouts, improving the bioavailability of nutrients in the seed for consumption, such as proteins and bioactive compounds. The objective of this work was to evaluate the chemical, physical and physicochemical parameters of fresh and germinated jackfruit seeds. The germination of jackfruit seeds increased the contents of water, proteins, ascorbic acid, phenolic compounds and total sugars, in addition to reducing the content of starch, lipids, tannins and the parameters of color, luminosity, red (+ a*) and yellow (+b*) intensities. In relation to the physical properties of the seed, there was an increase in the real specific mass and in the unit mass. For apparent specific mass there was a reduction. Seed germination provided an increase in the levels of magnesium, iron and sodium, consequently reducing potassium, phosphorus, calcium, zinc, copper and manganese.
Resumo A jaca é um fruto exótico presente na região Nordeste do Brasil, apresentando um grande potencial econômico para região em face da comercialização interna e externa do seu fruto. Sua semente é rica em carboidratos, proteína, minerais e vitaminas, constituindo fonte alternativa de amido para alimentação. A germinação é uma tecnologia barata de fácil acesso para produção de brotos, melhorando a biodisponibilidade de nutrientes na semente para o consumo, a exemplo das proteínas e compostos bioativos. O objetivo do trabalho foi avaliar os parâmetros químicos, físicos e físico-químicos das sementes de jaca in natura e germinadas. A germinação das sementes de jaca proporcionou o aumento nos teores de água, proteínas, ácido ascórbico, compostos fenólicos e açúcares totais, além de reduzir os conteúdo de amido, lipídios, taninos e os parâmetros de cor, a luminosidade, intensidade de vermelho (+a*) e de amarelo (+b*). Em relação às propriedades físicas da semente, ocorreu acréscimo na massa específica real e na massa unitária. Para massa específica aparente houve redução. A germinação das sementes proporcionou aumento nos teores de magnésio, ferro e sódio, consequentemente ocorrendo redução de potássio, fósforo, cálcio, zinco, cobre e manganês.

RESUMEN La morfea lineal en golpe de sable es una entidad dermatológica caracterizada por la inflamación esclerosante y progresiva del tejido cutáneo en la región frontal y/o en el cuero cabelludo. La cefalea y las crisis convulsivas son dos de los síntomas extracutáneos más frecuentes y están causados por el crecimiento subyacente de la lesión. Es importante un diagnóstico temprano para frenar la progresión e intentar evitar las complicaciones secundarias, principalmente neurológicas. El diagnóstico se basa en el cuadro clínico y el estudio histológico, que permite la confirmación definitiva. El tratamiento de elección es la terapia combinada con corticoides orales y metotrexato. Aun con el tratamiento farmacológico adecuado, esta patología puede presentar un curso recidivante y dejar secuelas a largo plazo. Se presenta el caso de una niña en quien se realizó un diagnóstico rápido de esta enfermedad, a pesar de un cuadro clínico inespecífico. Fue tratada con metotrexato oral con buena respuesta, sin efectos secundarios.
ABSTRACT Linear morphea in coup de sabre is a dermatological entity characterized by progressive, sclerosing inflammation of the skin tissue in the frontal region and on the scalp. Headache and seizures are two of the most frequent extracutaneous symptoms and they are caused by the growth of the lesion towards underlying structures. An early diagnosis is important to stop cranial progression and try to avoid secondary complications, mainly neurological. The diagnosis is relied on compatible clinical signs and a pathological study that allows a definitive confirmation. The treatment of choice is combination therapy with oral corticosteroids and methotrexate. Despite an adequate pharmacological treatment, this pathology can present a recurrent course and cause long-term sequelae. We present the case of a girl who was diagnosed quickly, despite a not very noticeable symptoms. She has been treated with oral methotrexate with a good response, without side effects.

Abstract: In recent decades, the growing obesity epidemic in Mexico has become one of the most important public health challenges faced by the country. With support from the World Obesity Federation, we formed a working group in 2021 to identify and summarize priority actions that Mexico can take to face this epidemic. More than 1 000 health professionals joined the development and discussion process. Recommendations from previously published, high-level documents and guidelines were taken into account. In commemoration of World Obesity Day 2022, this statement is presented as input for health care professionals to develop actions to address obesity. The statement includes 10 recommendations that include population-level and individual-level actions. It emphasizes the importance of social participation, comprehensive interventions with a person- centered perspective, planetary sustainability, on improving education and communication campaigns, as well as fostering a built environment that promotes active living, and shielding prevention and control efforts from conflicts of interest. The statement calls for obesity to be treated seriously, based on scientific evidence, in a timely and comprehensive manner, employing a life-course and ethical approach that does not perpetuate weight stigma in society.

Resumen Objetivo: Describir el grado de satisfacción y el impacto sociolaboral y medioambiental de los pacientes de un programa de seguimiento remoto de marcapasos. Método: Estudio observacional prospectivo que incluyó 160 pacientes del programa de seguimiento remoto entre 2016 y 2017. Se pasó una encuesta de satisfacción a dichos pacientes y se cuantificó la disminución del CO2 emitido al reducir el número de visitas. Resultados: Los pacientes acudían acompañados (86%) y en coche (66%) la mayoría de las veces, y mostraron un grado de satisfacción «bueno o excelente» en un 96%. Se estima un ahorro de emisión de CO2 de casi un 10% por ciclo de seguimiento. Conclusiones: El seguimiento remoto de marcapasos implantado en nuestra unidad de arritmias reduce el impacto sociolaboral, económico y medioambiental.
Abstract Objective: To describe the degree of satisfaction and social, occupational and environmental impact of patients on our remote pacemaker monitoring programme run. Method: Prospective observational study including 160 patients on the remote pacemaker monitoring programme between 2016 and 2017. We handed out a satisfaction survey and quantified the decrease in CO2 emitted by reducing the number of visits. Results: The patients attended visits accompanied (86%) and by car (66%) most of the time. 96% of respondents said their degree of satisfaction was “good or excellent”. We estimated a saving in CO2 emissions of almost 10% for each remote monitoring cycle. Conclusions: The remote monitoring of pacemakers implemented by our arrhythmia unit reduces the social, occupational, financial and environmental impact.

Abstract Objectives: To evaluate seroconverted asymptomatic COVID-19 in pediatric Autoimmune Rheumatic Diseases (ARDs) patients and to identify the risk factors related to contagion. Methods: A cross-sectional study was conducted in March 2021, before vaccination of children and adolescents in Brazil, including 77 pediatric ARDs patients, followed at a tertiary hospital and 45 healthy controls, all of them without a previous diagnosis of COVID-19. Data was obtained by a questionnaire with demographic data, symptoms compatible with COVID-19 over the previous year, and contact with people with confirmed COVID-19. Patient’s medical records were reviewed to access data regarding disease and current medications. A qualitative immunochromatographic SARS-CoV-2 test was performed on all participants. Results: Patients and controls were similar in terms of female gender (70.1% vs. 57.8%, p = 0.173), age (14 vs. 13 years, p = 0.269) and SARS-CoV-2 positive serology (22% vs. 15.5%, p = 0.481). 80.5% of rheumatic patients were in use of immunosuppressive drugs: 27.3% of them used corticosteroids (33.3% in high doses), and 7.8% on immunobiologicals. No statistical differences were found between positive (n = 17) and negative serology (n = 60) patients regarding demographic/socioeconomic data, contact with people with confirmed COVID-19, use and number of immunosuppressive drugs, use and dose of corticosteroids, use of hydroxychloroquine and immunobiological drugs (p > 0.05). Conclusions: Pediatric rheumatic disease patients were infected at the same rate as healthy ones. Neither the underlying pathology nor its immunosuppressive treatment seemed to interfere with contagion risk.

Abstract This study evaluated some biological activities of extracts from Abuta selloana. The gastroprotective potential was determined against ethanol/HCl- and indomethacin-induced gastric ulcers, whereas the antinociceptive effect was evaluated by acetic acid-induced abdominal contortions in mice. The cytotoxicity activity was measured against human cancer cell lines: U251 (glioma), MCF-7 (breast cancer) and NCI-H460 (lung cancer). The radical scavenger potential was verified; and preliminary phytochemical analyses were performed. The phytochemical screening revealed higher levels of phenolic compounds in all extracts. Moreover, the methanolic extract from pulp fruit (MEPu), peel fruit (MEPe), branches (MEB) and leaves (MEL) scavenged the DPPH radical at 100 µg/mL. Besides, only MEL presented GI50 < 30 µg/mL in all tested cells. Besides, MEPu, MEPe, MEB or MEL at 10 mg/kg (i.p) reduced the abdominal contortions at 47.22%, 63.31%, 84.59% and 37.76%, respectively. The MEPu, MEPe, MEB and MEL reduced the ethanol/HCl- and indomethacin- induced ulcer at 250 mg/kg (p.o). In conclusion, A. selloana had interesting biological activities; presenting the leaves as a promising source for compounds with cytotoxic potential, however, further studies should be performed to confirm its antitumoral activity. Besides, the whole plant can be an important source of bioactive compounds associated with gastroprotective and antinociceptive properties.

Abstract Centrosema coriaceum Benth belongs to Fabaceae family and have few studies of biological activity and chemical composition. Thus, the aims of this work were to determine chemical profile of the ethanolic extract of C. coriaceum leaves (CCE) by UFLC-QTOF-MS and to evaluate its in vitro biological potential. CCE showed MIC value of 1000 µg/mL against Candida glabrata (fungistatic effect) and high affinity in cell envelope by increasing cell permeability in nucleotide leakage, sorbitol and ergosterol assays. CCE showed antioxidant activity in all assays performed. For the anti-inflammatory and cytotoxicity activities, CCE, at all tested concentrations, significantly inhibited the production of nitric oxide and did not decrease J774A.1 cell viability below 70%. Finally, rutin, kaempferol-3O-rutinoside, caffeic acid, and sucrose were identified in CCE by UFLC-QTOF-MS. These results suggest, for the first time, that C. coriaceum has interesting antifungal, antioxidant, and anti-inflammatory activities.
