RESUMO Objetivo: identificar o conhecimento produzido em pesquisas nacionais e internacionais das alterações de fala e de linguagem na Esclerose Lateral Amiotrófica, quanto tipo de pesquisa e abordagens em diferentes áreas. Métodos: revisão integrativa realizada em bases de dados, utilizando-se os descritores Amyotrophic Lateral Sclerosis, Dysarthria, Language Disorders, Speech Production Measurement e Speech Disorders. Os critérios de inclusão abrangeram artigos que abordassem alterações motoras de fala e de linguagem de 2013 a 2018, excluindo-se duplicações, categorizando-se os válidos para análise. Resultados: selecionou-se 83 artigos, após triagem dos títulos e resumos. Foi encontrada grande produção científica de diferentes países e áreas, principalmente Fonoaudiologia e Neurologia, na maioria pesquisas clínicas (65,06%), com foco principal em alterações motoras da fala (42,16%), alterações motoras de fala e linguagem, cognição e comportamento (27,71%) e alterações de linguagem (12,06%). Conclusão: as pesquisas encontradas foram, na maioria, clínicas e com objetivo determinar diagnóstico das alterações em diferentes áreas do conhecimento. No que se refere à comunicação, foram encontrados poucos estudos no Brasil e os internacionais voltavam-se à alta tecnologia. Os resultados confirmaram o caráter heterogêneo doença, que evidencia, além de comprometimento motor da fala, prejuízo cognitivo, comportamental e de linguagem.

ABSTRACT Purpose: to identify the knowledge produced in national and international researches on speech and language disorders in Amyotrophic Lateral Sclerosis, regarding the type of research and approach in different areas. Methods: an integrative review performed on databases, using the following descriptors: Amyotrophic Lateral Sclerosis, Dysarthria, Language Disorders, Speech Production Measurement and Speech Disorders. The inclusion criteria covered articles that addressed motor speech and language disorders from 2013 to 2018, excluding duplications, and categorizing valid articles for analysis. Results: 83 articles were selected, after screening the titles and abstracts. A large scientific production from different countries and areas, mainly Speech Therapy and Neurology, was found. Most of them was clinical research (65.06%), with a main focus on speech motor disorders (42.16%), speech and language motor disorders, cognition and behavior (27.71%), and language disorders (12.06%). Conclusion: researches found were mostly clinical and aimed at determining the diagnosis of disorders in different areas of knowledge. With regard to communication, few studies have been found in Brazil, and international studies addressed high technology. The results confirmed the heterogeneous nature of the disease, which shows, in addition to motor impairment of speech, cognitive, behavioral and language impairments.

RESUMO Objetivo Avaliar a inteligibilidade de fala e disartria, correlacionando com a avaliação funcional da ELA. Método Estudo quantitativo-descritivo aprovado pelo CEP, sob nº CAAE 62912416.4.0000.5404, constituído por 19 pessoas com ELA, esporádica ou familiar. Para coleta, aplicou-se o Protocolo de Disartria e Escala de Avaliação Funcional da Esclerose Lateral Amiotrófica (ALSFRS-Re). Para análise, foi utilizada escala visual analógica para inteligibilidade de fala e medidas de resumo e correlação dos instrumentos pelo Coeficiente de Spearman com nível de significância de 5%. Resultados A inteligibilidade de fala está comprometida (41,37±39,73) em graus variados com correlação positiva com o grau geral de disartria (p=<,0001) e com todos os parâmetros de fala analisados, indicando impacto na deterioração da fala do grupo estudado. Há correlação negativa entre inteligibilidade de fala e resultados das seções bulbar – fala e deglutição (p=0,0166), braço – atividades com membro superior (p=0,0064) e perna – atividades com membro inferior (p=0,0391). Os parâmetros de respiração (p=0,0178), fonação (p=0,0334) e ressonância (p=0,0053) apresentaram correlação negativa com o item “fala” do ALSFRS-Re. Conclusão Os achados mostram prejuízo da inteligibilidade de fala e disartria e evidenciam respiração, fonação e ressonância como importantes marcadores da progressão da doença. Uma avaliação criteriosa e precoce da produção motora oral permite melhor gerenciamento das alterações na ELA.

ABSTRACT Purpose To evaluate speech intelligibility and dysarthria, correlated to the functional assessment of Amyotrophic Lateral Sclerosis (ALS). Methods Quantitative-descriptive study approved by REC under No. CAAE 62912416.4.0000.5404, comprised of 19 individuals with sporadic or familiar ALS. Data were collected using the Dysarthria Protocol and the Revised Amyotrophic Lateral Sclerosis Functional Rating Scale (ALSFRS-Re). We used visual analogue scale (VAS) to assess speech intelligibility and summary measures; and Spearman's coefficients of correlation for the instruments with significance level of 5%. Results Speech intelligibility is compromised (41.37±39.73) in varied degrees with positive correlation with the general degree of dysarthria (p=<.0001), and with all the analyzed speech parameters, indicating impact on the speech deterioration of the studied group. There is negative correlation between speech intelligibility and the results of the bulbar sections – speech and deglutition (p=0.0166), arm – activities with the upper limb (p=0.0064) and leg – activities with the lower limb (p=0.0391). Breathing (p=0.0178), phonation (p=0.0334) and resonance (p=0.0053) parameters showed a negative correlation with the item “speech” of the ALSFRS-Re. Conclusion Results show impaired speech intelligibility and dysarthria, and evidence breathing, phonation and resonance as important markers of the disease progression. A thorough and early evaluation of the oral motor production allows for a better management of alterations in ALS.

RESUMO Objetivo: Investigar a participação e funcionalidade de crianças e adolescentes com alterações de fala utilizando a Classificação Internacional de Funcionalidade, Incapacidade e Saúde. Método: Pesquisa descritiva-analítica de abordagem qualitativa-quantitativa com 30 participantes com alteração de fala (CAF) e 30 com desenvolvimento típico de fala (DTF). Para a coleta de dados, realizaram-se entrevistas semiestruturadas com os participantes, estudo de prontuário, observação dos participantes quanto aos aspectos de fala e aplicação de questionário estruturado com os responsáveis. Os dados coletados foram utilizados para seleção de qualificadores das categorias da Classificação Internacional de Funcionalidade, Incapacidade e Saúde. Para comparação dos qualificadores entre grupos, utilizou-se o Teste Mann-Whitney, e a análise de conteúdo temática, para as entrevistas. Resultados: Os participantes do grupo CAF relataram maior grau de dificuldade do que o grupo DTF. As alterações de fala repercutiram significativamente em Funções do Corpo (articulação e fluência), Atividades e Participação (conversa, relacionamentos, realização de rotinas e lidar com estresse) e Fatores Ambientais (atitudes de familiares, amigos e conhecidos). Conclusão: Os resultados evidenciam o impacto das alterações de fala e suas implicações sociais para estas crianças e adolescentes. A CIF permitiu compreender a saúde em complexidade e integralidade, possibilitando assim o planejamento de estratégias para amenizar o impacto dessas alterações individual e coletivamente. Podendo ser base, futuramente, para a criação de políticas públicas e ações que poderão melhorar a qualidade de vida e promover a saúde dessa população.

ABSTRACT Purpose: To investigate participation, and functionality of children and adolescents with speech disorders using the International Classification of Functioning, Disability and Health (ICF). Method: Descriptive and analytical research of qualitative and quantitative approach. The sample was composed of 30 participants with speech disorders and 30 with typical speech development. For data collection, it was conducted semi-structured interviews with the participants, medical record review, observation of speech aspects’ participants, and analyzed study of medical records. The collected data were used to qualify the framework codes. The Mann-Whitney Test was used for comparison between groups, and thematic content analysis for the interviews. Results: Participants with speech disorders reported more difficulties than the participants with typical speech development. Speech disorders significantly impacted on the Body Functions (articulation and fluency), Activities and Participation (conversation, relationships, carrying out the routine and handling of stress) and Environmental Factors (attitudes of family, friends and acquaintances). Conclusion: The results show the impact of speech disorders and their social consequences for these children and adolescents. The ICF allowed us to comprehend health in all its complexity and integrality, making possible to plan strategies to soften the disorders impact in an individual and collective perspective. Then, ICF can be used, in the future, for elaboration of public policies and actions that will improve the quality of life and promote the health of this population.

ABSTRACT Purpose: to discuss the implementation and use of the International Classification of Functioning, Disability and Health (ICF) and International Classification of Functioning, Disability and Health, Children and Youth Version (ICF-CY) among children and adolescents, within the Augmentative and Alternative Communication (AAC) field. Methods: an integrative literature review. PubMed, Web of Science and VHL databases were searched for papers published between 2006 and 2017 that reported on the use of ICF and ICF-CY within the AAC context. Eighteen papers were reviewed and sorted into: Category i) papers which reported on the use of the ICF or ICF-CY with people who rely on AAC; and Category ii) theoretical papers or papers that used the ICF and ICF-CY to organize the results. Results: papers used the frameworks with different purposes, including the characterization of the children and their environment, goal setting and measurement of the results of therapeutic intervention. The papers drew on all elements, however, Activities and Participation were the components most used. Parents or caregivers were most commonly consulted in classifying the children’s and young people’s profiles of functioning, followed by the Educators and speech and language pathologists. Conclusion: classifications have shown advantages when used in the AAC field. Therefore, it is necessary to provide training in order for professionals to implement them in services.

RESUMO Objetivo Interessa descrever o processo de implementação da Classificação Internacional de Funcionalidade, Incapacidade e Saúde (CIF) em um Centro Especializado em Reabilitação fundamentado na abordagem biopsicossocial de saúde. Método Trata-se de pesquisa-ação, descritiva, analítica e longitudinal. O processo de implementação no serviço abrangeu quatro etapas: a) Capacitação para uso da CIF; b) Construção de checklists pela equipe; c) Aplicação dos checklists com usuários do serviço; e d) Construção de banco de dados. Foi elaborado um checklist para cada setor envolvido e um banco de dados incluindo informações do resultado da avaliação e reavaliação dos usuários. Resultados Os achados indicam maior resolutividade em todos os setores no período estudado e que a capacitação foi fundamental para operacionalização da CIF. A construção de instrumentos com base na realidade do serviço foi essencial para atender às demandas locais e de cada setor. Conclusão A CIF possibilitou maior prática da abordagem biopsicossocial a partir do envolvimento dos profissionais na sua operacionalização, com evidências de resolutividade do serviço, além de visibilidade e organização do processo de trabalho.

ABSTRACT Purpose Describe the implementation process of the International Classification of Functioning, Disability and Health (ICF) in a Specialized Rehabilitation Center based on the biopsychosocial approach to health. Methods This is a descriptive, analytical, longitudinal study. The ICF implementation process in the healthcare center encompassed four stages: a) training on the use of the ICF; b) preparation of checklists by the team; c) collection of relevant data based on the checklist from the healthcare center users; and d) construction of a database. Results A checklist was constructed for each sector involved, and the database included user information and the ICF results during evaluation and reevaluation. The findings indicate higher problem-solving capacity in all sectors throughout the study period, and that training was crucial to operationalize the ICF. Preparation of the instruments based on the reality of the healthcare center was essential to meet local demands and those of each sector. Conclusion The ICF enabled greater practice of the biopsychosocial approach based on the engagement of the professionals in its operationalization, with evidence of healthcare problem-solving capacity and visibility and organization of the work process.

RESUMO Objetivo Conhecer facilitadores e barreiras no uso da Comunicação Suplementar e Alternativa (CSA) na percepção de familiares/cuidadores e fonoaudiólogos que atuam com pessoas usuárias de CSA nos contextos familiar, social e terapêutico. Método Estudo transversal quantitativo e qualitativo com 20 fonoaudiólogos (GF) e 20 familiares e/ou cuidadores de usuários de CSA (GFC). Realizou-se a coleta de dados por meio de entrevistas semiestruturadas com questionários específicos para cada grupo, videogravadas e transcritas para análise. Os dados foram categorizados, conforme critérios de relevância e repetição, em dois eixos temáticos principais: barreiras e facilitadores, contendo categorias e subcategorias. Resultados Ambos os grupos apontaram como barreiras: custo dos materiais, utilização de outras formas de comunicação, familiar como intérprete, dificuldades linguístico-cognitivas e de aceitação de uso da CSA pelos usuários, falta de compreensão/conhecimento da CSA pelos familiares e inibição da fala na percepção dos familiares. Ambos indicaram como facilitadores: utilização de recursos de alta tecnologia, versatilidade/disponibilidade, envolvimento/adesão familiar, uso contextualizado, importância do ambiente terapêutico, equipe e estratégias utilizadas. Conclusão Os principais interlocutores na implementação da CSA, profissionais ou familiares, podem se constituir como barreiras quando dificultam os usuários de exercer sua autonomia na comunicação ou facilitadores quando incentivam e utilizam a CSA com os usuários.

ABSTRACT Purpose To investigate the facilitators and barriers involved in the use of Augmentative and Alternative Communication (AAC) by people with complex communication needs in social and therapeutic environments under family members/caregivers and speech-language pathologists (SLP) perceptions. Methods This is a transversal quantitative and qualitative study with 20 family members/caregivers (FCG) and 20 SLP (SLPG). The data was collected by semistructured interviews with specific questionnaires for each group; recorded and transcribed for further analysis. Data was categorized in thematic axes, categories and subcategories, using recurrent and salient criteria. The two most important topics were: barriers and facilitators. Results Both groups indicate as barriers: high material cost, using other ways to communicate, family member as interpreter, language and cognitive deficits, acceptance of AAC for users and family members, lack of comprehension of AAC and family perceptions of AAC as speech suppress. As facilitators, both groups indicate the use of high technology, versatility and availability of AAC systems, family adherence and engagement, contextualized use of AAC outside of therapeutic contexts and the importance of therapeutic setting and team support. Conclusion In this way, the main interlocutors in AAC implementation, professionals or family members can be barriers when they make it difficult for users to exercise their autonomy in communication, or facilitators when they encourage and use AAC with users.

RESUMO Objetivo O Acidente Vascular Encefálico é um importante problema de Saúde Pública, com alto índice de mortes e sequelas, tais como alterações de linguagem. As mudanças conceituais da saúde, ao longo do tempo, levaram à incorporação de aspectos funcionais e sociais como propõe a Organização Mundial de Saúde na Classificação Internacional de Funcionalidade, Incapacidade e Saúde. O objetivo foi avaliar e classificar aspectos de linguagem, funcionalidade e participação de pessoas pós-Acidente Vascular Encefálico com base conceitual da Classificação Internacional de Funcionalidade, Incapacidade e Saúde e caracterizar o perfil sociodemográfico dos participantes. Método A coleta de dados ocorreu pela aplicação de instrumento clínico para avaliação de linguagem, participação e funcionalidade em cinquenta indivíduos, com base na Classificação Internacional de Funcionalidade, Incapacidade e Saúde. Resultados A idade variou de 32 a 88 anos, sendo a maioria do gênero masculino. Em relação a funções do corpo, os participantes referiram maior dificuldade em funções da memória; nos aspectos de atividade e participação, verificou-se maior dificuldade em recreação e lazer; e nos fatores ambientais, os profissionais de saúde foram indicados como facilitadores para a maioria dos participantes. Conclusão Os resultados mostram o impacto das dificuldades de linguagem na vida das pessoas pós-Acidente Vascular Encefálico e reafirmam a aplicabilidade da Classificação Internacional de Funcionalidade, Incapacidade e Saúde como importante instrumento complementar à avaliação de linguagem, funcionalidade e participação numa abordagem integral e humanizada, em prol do aprimoramento da assistência à saúde no atendimento ambulatorial desse grupo.

ABSTRACT Purpose Cerebrovascular accident is an important Public Health problem because of the high rates of mortality and sequelae such as language disorders. The conceptual health changes have led to the incorporation of functional and social aspects in the assessments as proposed by the World Health Organization in the International Classification of Functioning, Disability and Health. The purpose was to evaluate and classify language aspects, functionality and participation of post-stroke individuals based on the concepts of the International Classification of Functioning, Disability and Health and characterize the sociodemographic profile of participants. Methods Data collection was carried out through the application of a clinical instrument to evaluate language, participation and functionality in fifty individuals based on the International Classification of Functioning, Disability and Health. Results The age of the participants varied between 32 and 88 years, and the majority were elderly men. Among body functions, the participants reported more difficulties in “memory functions”. As for activity and participation, more difficulties were reported in “recreation and leisure”. As for environmental factors, the component “healthcare professionals” was indicated as a facilitator by the majority of participants. Conclusion The results show the impact of language difficulties in the lives of post-stroke adults and reinforce the applicability of the International Classification of Functioning, Disability and Health as an important complementary tool for assessing language, functionality and participation in a comprehensive and humane approach, towards the improvement of health assistance in ambulatory care.

RESUMO Objetivo Investigar a percepção de familiares acerca das condições linguísticas e da participação social de crianças e adolescentes com alterações de fala/linguagem utilizando a Classificação Internacional de Funcionalidade, Incapacidade e Saúde – Versão Crianças e Jovens (CIF-CJ). Método Pesquisa de abordagem quali-quantitativa, na qual se realizou levantamento dos prontuários de 24 crianças/adolescentes, em acompanhamento fonoaudiológico, e entrevistas com seus familiares. Foi feita análise descritiva dos perfis dos participantes e categorização das respostas, utilizando a CIF-CJ. Resultados Todos os familiares abordaram diversos aspectos de fala/linguagem categorizados pela CIF-CJ. Inicialmente, trataram-nos como um problema orgânico, categorizado no componente de Funções e Estruturas do Corpo. A maioria referiu diferentes repercussões das alterações de fala/linguagem em domínios, como, lidar com estresse e atividade do falar, qualificados de leve a grave. Os participantes relataram Fatores Ambientais categorizados como facilitadores, em atitudes da família imediata, e como barreiras, em atitudes sociais. Conclusão Os achados, utilizando-se a CIF-CJ, evidenciam que as alterações de fala/linguagem das crianças e adolescentes, na percepção dos familiares, são compreendidas, inicialmente, na dimensão do corpo. Contudo, pautados numa abordagem mais ampla de saúde, os achados, em Atividades e Participação e Fatores Ambientais, demonstram ampliação do olhar dos participantes quanto às alterações de fala e linguagem. Os resultados reiteram a importância do uso da CIF-CJ como instrumento de análise na atenção à saúde, ao incorporar aspectos de funcionalidade e participação, proporcionando subsídios para a construção de projetos terapêuticos singulares, em uma abordagem mais ampla de saúde do grupo estudado.

ABSTRACT Purpose To investigate the perception of family members regarding linguistic conditions and social participation of children and adolescents with speech and language impairments using the International Classification of Functioning, Disability and Health - Children and Youth Version (ICF-CY). Methods Quali-quantitative approach research, in which a survey of medical records of 24 children/adolescents undergoing speech-language therapy and interviews with their family members was conducted. A descriptive analysis of the participants’ profiles was performed, followed by a categorization of responses using the ICF-CY. Results All family members mentioned various aspects of speech/language categorized by the ICF-CY. Initially, they approached it as an organic issue, categorized under the component of Body Functions and Structures. Most reported different repercussions of the speech-language impairments on the domains, such as dealing with stress and speaking, qualified from mild to severe. Participants reported Environmental Factors categorized as facilitators in the immediate family’s attitudes and as barriers in the social attitudes. Conclusion These findings, according to the use of the ICF-CY, demonstrate that the children/adolescents’ speech-language impairments, from the families’ perception, are primarily understood in the body dimension. However, guided by a broader approach to health, the findings in the Activities and Participation and Environmental Factors demonstrate a broader understanding of the participants of the speech-language impairments. The results corroborate the importance of using the ICF-CY as a health care analysis tool, by incorporating functionality and participation aspects and providing subsidies for the construction of unique therapeutic projects in a broader approach to the health of the group studied.

RESUMOObjetivo Caracterizar e analisar o perfil dos usuários atendidos em um Grupo de Avaliação e Prevenção de Alterações de Linguagem (GAPAL) de uma clínica-escola.Métodos Pesquisa retrospectiva de banco de dados do GAPAL, do primeiro semestre/2002 ao primeiro semestre/2013. Foram analisados 261 usuários, quanto à idade, gênero, procedência, encaminhamentos e queixas apresentadas.Resultados Houve maior incidência de queixas relacionadas à linguagem no gênero masculino e na faixa etária de 25 a 30 meses de idade. A cidade com maior procedência de usuários foi Campinas. Quanto aos encaminhamentos, a maior parte foi realizada por profissionais de saúde, em sua maioria pediatras, sendo o relato mais ocorrente o de queixa de atraso de linguagem.Conclusão O estudo possibilitou a caracterização da população assistida, reiterando a importância de ações de promoção e prevenção das alterações de linguagem, principalmente junto às famílias. Também evidenciou limitações e dificuldades quanto ao registro dos dados, indicando a necessidade de readequação dos registros para nortear ações de (re)organização e gestão dos serviços na clínica de linguagem, tendo em vista o aprimoramento das ações de fonoaudiologia para esse grupo populacional.

ABSTRACTPurpose To characterize and analyze the population profile of an evaluation and prevention group of language disorders (GAPAL) in a clinic school.Methods It was a retrospective study from related database of GAPAL from the period of the 1st half/ 2002 through 1st half/ 2013. 261 users were analyzed by age, gender, hometown, referrals and complaints.Results It was found a higher occurrence of complaints related to language in males and aged 25-30 months. The city of origin of most users was Campinas. In terms of referrals, the major part of them was made from health professionals, mostly pediatricians, who reported language delay.Conclusion The study enable to characterize the population who are assisted by this service and reaffirm the importance of language promotion and prevention actions, especially with families. However, this analysis also showed limitations and difficulties in the recording of data, ponting the need to readequate database, so they can be taken as a guide for (re) organization actions and management of services in clinical speech and language with a view to the improvement of Speech and Language Pathology for this population group.

Resumo:OBJETIVOS:verificar a percepção de graduandos de fonoaudiologia quanto à condução de grupos de familiares e analisar o processo de escolha, condução das atividades e a repercussão do grupo no processo terapêutico.MÉTODOS: trata-se de estudo de corte transversal e abordagem qualitativa, cuja amostra se constituiu de 19 alunos do quarto ano de graduação. Para a coleta de dados, foram realizados três grupos focais com participantes distintos, utilizando-se questões norteadoras. Os grupos foram gravados para posterior transcrição e análise segundo critérios de repetição e relevância e criação de categorias conforme objetivos da pesquisa.RESULTADOS: os achados evidenciam que diversos aspectos emergiram durante a condução dos grupos na percepção dos graduandos, os quais envolvem a mudança de sentimentos, o desenvolvimento do olhar analítico, o conhecimento do setting terapêutico como espaço de reconstrução de sentidos e subjetividades, o significado das atividades como recursos necessários para o alcance dos objetivos, a supervisão como um espaço esclarecedor de dúvidas, a autoavaliação da atuação na dinâmica grupal, e a contribuição de informações compartilhadas em grupo no processo terapêutico do sujeito em atendimento.CONCLUSÃO: a escolha, desenvolvimento e condução das atividades com familiares, discutidas na interface entre a Fonoaudiologia e a Terapia Ocupacional, agregam valores durante a formação e evidenciam a importância do processo para os alunos. Considerar os familiares, por meio dos grupos, no processo de aprendizagem ainda na graduação pode contribuir para uma formação em uma perspectiva de atenção integral e humanizada.

Abstract:PURPOSE:to verify the perception of undergraduates in Speech and Language Pathology regarding the management of family groups and to analyze the selection process, the management of activities and the impact of groups in the therapeutic process.METHODS: we design a qualitative cross-sectional study. The sample consisted of 19 fourth-year students. In relation to data collection, we performed three focus groups with different participants, by using guiding questions. The groups were recorded for subsequent transcription and analysis, according to the criteria of repetition and relevance. Categories were established following the objectives of the research.RESULTS: different issues emerged during the group discussions from the undergraduates' perception. The discussions led to: the change of feelings; the development of an analytical view; knowledge about the therapeutic setting as a space to reconstruct meanings and subjectivities; the meaning of activities as resources to achieve objectives; the supervision as a space to clarify questions; the self-assessment of the individual performance in the context of group dynamics; and the contribution of information shared in the group during the therapeutic process of the subject.CONCLUSION: understood on the interface between Speech and Language Pathology and Occupational Therapy, the selection, development and management of the activities with family members aggregate values during undergraduate training, and also corroborate the importance of this process for undergraduate students. By using groups, including family members early in the learning process during undergraduation may contribute to a professional training focused on integral and humane care.

Objetivo Investigar e comparar a qualidade de vida de afásicos fluentes e não fluentes. Métodos Trata-se de pesquisa prospectiva, quantitativa, transversal, cuja amostra se constituiu de 11 sujeitos afásicos (5 não fluentes, usuários de comunicação suplementar e/ou alternativa e 6 fluentes, não usuários de comunicação suplementar e/ou alternativa. A coleta de dados foi realizada por meio da aplicação de um questionário específico de qualidade de vida, entrevista estruturada e aplicação da escala de Rankin modificada. Resultados Na comparação dos grupos estudados, os afásicos não fluentes apresentaram escores de Rankin e de qualidade de vida menores do que os fluentes e as maiores diferenças referiram-se aos domínios de linguagem e função do membro superior. Os domínios mais prejudicados pelo acidente vascular cerebral foram linguagem, relações sociais e modo de pensar, para os afásicos não fluentes, e comportamento, relações sociais e modo de pensar, para os fluentes. Todos os sujeitos relataram que sua qualidade de vida piorou após o acidente vascular cerebral, sendo que linguagem e cuidados pessoais foram apontados como os aspectos que mais mudaram, após o episódio lesional. Conclusão Os achados mostram relação entre gravidade da afasia e funcionalidade do indivíduo, indicando que, no geral, os afásicos não fluentes apresentam qualidade de vida pior do que os fluentes. As diferenças não são homogêneas nos diversos domínios de qualidade de vida.

Purpose To investigate and to compare quality of life (QOL) in fluent and non-fluent aphasics. Methods This is a prospective, quantitative, and transversal study. We included 11 stroke patients with aphasia (five non-fluent aphasics augmentative and alternative communication users and six fluent aphasics). Data was gathered from the Stroke Specific Quality of Life Scale (SS-QOL), a structure interview, and The Modified Rankin Scale. Results The non-fluent aphasics presented poorer Rankin and quality of life than the fluent aphasics. The major difference occurred in the fields of language and upper extremity function. The three most affected domains in non-fluent aphasics were language, social roles, and thinking, whereas in the fluent aphasics were personality, social roles, and thinking. All the subjects referred a worse quality of life after stroke. The domains of language and self-care were identified as the most affected after stroke. Conclusion This study demonstrated that, in general, non-fluent aphasics have lower quality of life than fluent aphasics. However, this difference is not homogeneous among the several quality of life domains. Additionally, this research evidences a relationship between aphasia severity and individual functionality, implying impairment in quality of life, especially for non-fluent aphasics.

Objetivos : verificar as formas de comunicação e desempenho linguístico-cognitivo de afásicos a partir da Comunicação Suplementar e/ou Alternativa (CSA) e conhecer sua percepção.Métodos : pesquisa de abordagem qualitativa de corte longitudinal, aprovada pelo CEP, com amostra de 5 sujeitos afásicos não fluentes. A coleta de dados ocorreu por meio dos prontuários dos sujeitos, registros em vídeo do acompanhamento fonoaudiológico com a CSA e dos seus depoimentos acerca da utilização da CSA.Resultados : os sujeitos utilizam diversas formas de comunicação para se expressar além de fazerem maior uso da CSA, necessitando menor apoio de gestos próprios ao longo do estudo, o que facilitou a comunicação com o outro e diminuiu o uso de adivinhações e tentativas frustradas de conversas. O uso da CSA repercutiu no aumento da produção oral. Os sujeitos referem gostar de utilizar os recursos da CSA e que estes contribuem de alguma forma na sua comunicação.Conclusão: os resultados evidenciam que o apoio da CSA contribui para que os sujeitos estudados pudessem assumir seus lugares como falantes, superando suas dificuldades de linguagem. Deste modo, favoreceu que eles se colocassem como sujeitos linguísticos e sociais. As atividades dialógicas e contextualizadas, bem como a mediação do interlocutor, favoreceram o processo de (re)significação de seus enunciados. Portanto, a CSA, nesse contexto, mostra-se como importante recurso mediador e facilitador do processo linguístico nas afasias não fluentes com repercussão na produção oral.

Purpose : verify the communication forms and linguistic-cognitive performance of aphasics from an Augmentative and Alternative Communication (AAC) perspective and to evaluate their perception.Methods : this is a longitudinal research with qualitative approach, approved by the Ethic and Research Committee. The sample includes 5 non-fluent subjects with aphasia. Data was collected analyzing the subjects’ files, video records of speech and language therapy focused on AAC, and the subjects opinions about it.Results : subjects used several ways of communication to express themselves as well as increased their use of AAC. In doing so, they demanded less gestures support during the study, which facilitated their communication and diminished guessing and frustrated conversation attempts. The use of AAC resulted in the increase of oral production. Subjects referred that they enjoyed using AAC and that it contributed to their communication in some way.Conclusion : results showed that AAC support contributed to subjects so they could assume their position as speakers, overcoming their language difficulties. Therefore it has facilitated them to assume themselves as linguistic and social subjects. Dialogic and contextualized activities, as well as the interlocutor mediation, facilitated the process of (re)signification of their enunciations. Therefore, regarding this context, AAC is an important support that mediates and facilitates the linguistic process on non-fluent aphasias with repercussions on oral production.

OBJETIVO: Investigar a qualidade de vida dos cuidadores de crianças ou adolescentes com alterações de fala e linguagem de acordo com a perspectiva deles. MÉTODOS: Participaram dois grupos, totalizando 40 sujeitos. O Grupo 1 foi composto por 20 cuidadores de crianças ou adolescentes com alterações de fala e linguagem entre 4 e 17 anos, pareados por idade com o Grupo Controle ou 2, que incluiu 20 de crianças ou adolescentes sem queixas de fala e linguagem. Para a coleta de dados, foram aplicadas duas questões abertas e um instrumento da Organização Mundial de Saúde, traduzido e adaptado para o português - World Health Organization Quality of Life Scale (WHOQOL) Abreviado. Os resultados de tal instrumento foram submetidos à análise estatística, e as perguntas abertas estavam sendo analisadas qualitativamente. RESULTADOS: Na distribuição das mudanças de fala e linguagem, havia: gagueira (35%), alterações de linguagem oral sem (35%) e com causas neurológicas (30%). Na análise dos escores do WHOQOL-abreviado, encontraram-se diferenças da qualidade de vida nos domínios físico (1,1%), psicológico (0,5%) e relações sociais (1,8%). O Grupo 1 apresentou qualidade de vida mais insatisfatória; e nas abertas, boa e razoável. Já o Grupo 2 mostrou ótima e boa. A rotina clínica e os filhos foram mencionados como fatores que dificultam o autocuidado no Grupo 1. CONCLUSÕES: O menor escore do Grupo 1 no WHOQOL-abreviado foi compatível com achados das questões abertas, mostrando que aspectos como rotina de atendimentos clínicos e dificuldades de compreensão influenciam a qualidade de vida dos cuidadores. Os resultados reafirmam a necessidade de que, além de cuidadores, sejam cuidados, entendendo-os como um grupo digno de ações de saúde direcionadas aos mesmos.

PURPOSE: To investigate the quality of life of caregivers of children and adolescents presenting speech and language disorders from their own perspective. METHODS: Two groups participated, adding up to 40 subjects. Group 1 was composed by 20 caregivers of 4 to 17 years-old children or adolescents with speech and language disorders, paired by age with the Control Group or 2 that included 20 caregivers of children or adolescents with no speech and language complaints. Data collection was done using: two open questions and the World Health Organization instrument, which was translated and adapted to Portuguese language - the World Health Organization Quality of Life Scale (WHOQOL-BREF). The results were submitted to statistical analysis and the open questions were qualitatively analyzed. RESULTS: The language disorders distribution showed: stuttering (35%), non-neurological (35%), and neurological oral language disorders (30%). In the analysis of the WHOQOL-BREF scores, there were quality of life differences regarding the physical (1.1%), psychological (0.5%), and social relationships (1.8%) domains. Group 1 presented the most dissatisfying quality of life. Concerning the open questions, it presented good and reasonable characteristics and Group 2, good and very good. The clinical routine and children were mentioned as factors that hamper self-care in Group 1. CONCLUSIONS: The lowest score of Group 1 in the WHOQOL-BREF was consistent with the open questions results, showing that aspects such as clinical attendance routine and comprehension difficulties influence caregivers' quality of life. The results corroborate that they should be assisted, since they are a group that deserves healthy actions directed to them.

TEMA: compreender a experiência das pessoas que gaguejam, dentro e fora do contexto de tratamento, conduz a melhores resultados. OBJETIVO: investigar como a gagueira afeta a qualidade de vida de crianças e adolescentes que gaguejam. MÉTODO: o Overall Assessment of the Speaker's Experience of Stuttering - School-Age (OASES-S) foi utilizado para avaliar o impacto da gagueira e o Protocolo do Perfil da Fluência para severidade da gagueira. RESULTADOS: os grupos estudados experienciam impacto negativo moderado conforme medido pelo OASES-S. Os resultados evidenciam tendência à correlação positiva entre grau de severidade e impacto da gagueira na qualidade de vida. CONCLUSÃO: uma melhor compreensão do impacto da gagueira nas faixas etárias estudadas propicia direção necessária para desenvolvimento de tratamentos da gagueira e pesquisa dos resultados do tratamento.

BACKGROUND: understanding the experience of people who stutter, both in and out treatment, will lead to improved outcomes. AIM: to investigate how stuttering affects the quality of life of children and adolescents who stutter. METHOD: the Overall Assessment of the Speaker's Experience of Stuttering - School-Age (OASES-S) was used to assess the impact of stuttering and the Fluency Profile Protocol was used to stuttering severity. RESULTS: these age groups do experience moderate negative impact as measured by the OASES-S. The results showed a tendency toward a positive correlation between severity and the impact of stuttering on quality of life. CONCLUSION: a better understanding of the impact of stuttering in these age groups provides a needed guide for the development of stuttering treatments and treatment outcomes research.

Este trabalho volta-se ao estudo da Comunicação Suplementar e/ou Alternativa (CSA) na afasia sob uma perspectiva discursiva, assumindo caráter original e fundamental. Interessa investigar processos de significação de afásicos não fluentes, usuários de CSA por meio de estudo de caso longitudinal e qualitativo de cinco sujeitos com idades, dificuldades linguísticas e tipos de afasia variados, acompanhados em grupo fonoaudiológico. Os dados foram gravados em vídeo de 2006 a 2008, analisando-se episódios relevantes aos objetivos da investigação. Os resultados evidenciaram que a CSA possibilita processos de significação diversos. Os sujeitos realizaram traduções linguísticas, em termos jakobsonianos: do símbolo à palavra, do gesto à palavra, do desenho à palavra e da circulação entre sistemas verbais e não-verbais. A CSA mostrou-se eficaz nas atividades linguísticas dos sujeitos estudados, produzindo efeitos na qualidade de sua interação social.

This work focuses on the study of Augmentative and Alternative Communication (AAC) in aphasia from a discursive perspective, assuming an original and fundamental character. It is of interest to investigate meaning attribution processes of non-fluent aphasic patients who use AAC through a qualitative longitudinal case report of five subjects with varied ages, linguistic deficits, and aphasia types, attended in a Speech-Language Pathology therapeutic group. Data were video-recorded from 2006 to 2008, and relevant episodes to the aims of the present investigation were analyzed. The results evidenced that AAC enables various meaning attribution processes. The subjects performed language translations, in jakobsonian terms: from symbol to word, from gesture to word, from drawing to word and the circulationbetween verbal and non-verbal systems. AAC was effective in the linguistic activities of the studied subjects, affecting the quality of their social interaction.
