RESUMO Contexto A COVID-19 compreende uma infecção respiratória decorrente da contaminação pelo vírus SARS-CoV-2, sendo a insuficiência respiratória aguda uma de suas principais características, levando a uma alta frequência de intubação orotraqueal (IOT), que por sua vez aumenta o risco para a disfagia. Uma vez que esta pode levar ao comprometimento pulmonar, conhecer a real ocorrência de disfagia em parte da população brasileira e suas associações permite o manejo clínico precoce e eficaz da equipe multidisciplinar em relação aos pacientes. Objetivo: Verificar a ocorrência de disfagia em pacientes adultos positivos para COVID-19 em dois hospitais brasileiros, referências no atendimento à pandemia. Métodos: Trata-se de um estudo prospectivo, observacional longitudinal, realizado em dois hospitais privados no Brasil, ambos referências no atendimento de pacientes com isolamento por coronavírus. Inicialmente os dados foram levantados por meio de consulta aos prontuários de cada paciente. Foram também coletadas informações sobre sexo, idade, doenças anteriores, teste de COVID-19 e período de IOT. Após a coleta de dados, foi realizada a avaliação fonoaudiológica clínica da deglutição de cada paciente por meio do Gugging Swallowing Screen (GUSS) adaptado, do ASHA NOMS e da Functional Oral Intake Scale (FOIS). Resultados Foram avaliados 129 participantes, com média de idade de 72 anos. De acordo com a escala GUSS, 9,3% dos pacientes apresentaram deglutição normal/funcional, enquanto 90,7% apresentaram disfagia, sendo esta de grau leve em 17,05%, moderado em 33,33% e grave em 37,98%. Quanto aos resultados do ASHA NOMS, a maioria (36,5%) dos pacientes encontrava-se no nível 1, que representa o paciente que não consegue receber alimentação por via oral, tendo a necessidade do uso de alimentação por sonda. Esse dado está de acordo com os resultados observados com a escala FOIS, em que a maioria dos pacientes (42,1%) foi classificada como nível I, quando a ingestão de alimentos ocorre exclusivamente por sondas, sem oferta por via oral. Dos 129 participantes, 59% deles necessitaram de IOT. Ao comparar o tempo de IOT e a gravidade da disfagia, encontrou-se diferença estatisticamente significante, sendo que quanto mais grave a disfagia, maior o tempo que o paciente permaneceu intubado. Conclusão Existe alta incidência de disfagia orofaríngea em pacientes com COVID-19, com maior gravidade durante períodos mais longos de IOT.

ABSTRACT Background COVID-19 comprises a respiratory infection resulting from contamination by SARS-CoV-2, with acute respiratory failure being one of its main characteristics, leading to a high frequency of orotracheal intubation (OTI), which in turn increases the risk for dysphagia. Since this can lead to pulmonary impairment, knowing the real occurrence of dysphagia in part of the Brazilian population and its associations allows early and effective clinical management of the multidisciplinary team in relation to patients. Objective To verify the occurrence of dysphagia in COVID-19-positive adult patients in two Brazilian reference hospitals in the care of the pandemic. Methods This was a prospective, longitudinal observational study carried out in two private hospitals in Brazil, both references in the care of patients with coronavirus isolation. Data were initially collected by consulting the medical records of each patient. Information was collected regarding sex, age, previous diseases, COVID-19 testing, and the OTI period. After data collection, the clinical speech-language assessment of swallowing for each patient was carried out using the adapted Gugging Swallowing Screen (GUSS), the ASHA NOMS and the Functional Oral Intake Scale (FOIS). Results A total of 129 participants were evaluated, with a mean age of 72 years. According to the GUSS scale, 9.3% of the patients presented normal/functional swallowing, while 90.7% presented dysphagia, with mild dysphagia in 17.05%, moderate dysphagia in 33.33%, and severe dysphagia in 37.98%. As for the results of the ASHA NOMS, the majority (36.5%) of the patients were at level 1, which represents the patient who is not able to receive his or her food supply orally, having the need to use tube feedings. This is in line with the results observed with the FOIS scale, whereby most patients (42.1%) were classified as Level I, when food intake occurs exclusively through feeding tubes, with no oral supply. Of the 129 participants, 59% of them required OTI. When comparing the time of OTI and the severity of dysphagia, there was a statistically significant difference, with more severe dysphagia, the longer the patient remained intubated. Conclusion: There is a high incidence of oropharyngeal dysphagia in patients with COVID-19, with increased severity during longer periods of OTI.

RESUMO Objetivos: investigar os resultados da Triagem Auditiva Neonatal (TAN) em neonatos cujas mães tiveram hipertensão e/ou diabetes mellitus na gestação. Métodos: revisão sistemática da literatura, sem restrição de ano e de idioma, guiada pelo protocolo PRISMA, realizada a partir de consulta às bases MEDLINE(PUBMED), LILACS(BVS), SCOPUS, WEB OF SCIENCE e EMBASE, utilizando-se os descritores neonatal screening AND hearing AND (hypertension OR diabetes mellitus). Foram excluídos estudos duplicados ou indisponíveis na íntegra. Resultados: foram encontrados 64 registros, dos quais5 foram incluídos para análise. Os artigos analisados foram publicados nos últimos 13 anos, originaram-se em diferentes países e apresentaram tanto delineamento retrospectivo (transversal, n=1; caso-controle, n=1) como prospectivo (transversal, n=1; caso-controle, n=1; coorte, n=1). Os achados dos estudos revisados demonstram divergências quanto à presença de alterações no resultado da TAN de recém-nascidos cujas mães apresentaram hipertensão e/ou diabetes gestacional. Conclusão: diante dos resultados contraditórios a respeito de alterações do resultado da TAN na presença de hipertensão e/ou diabetes na gestação, sugere-se a realização de estudos de coorte prospectivos, em diferentes contextos, a fim de isolar os fatores de confusão para perda auditiva e minimizar os vieses de aferição e seleção.

ABSTRACT Purpose: to investigate the results of the Neonatal Hearing Screening (NHS) in neonates whose mothers presented hypertension and/or diabetes mellitus in their pregnancy. Methods: a systematic review of the literature, without restriction of year and language, guided by the PRISMA protocol, that was carried out through a search in the MEDLINE (PUBMED), LILACS (BVS), SCOPUS, WEB OF SCIENCE and EMBASE databases, using neonatal screening AND hearing AND (hypertension OR diabetes mellitus) as descriptors. Studies that were duplicated or were unavailable were excluded. Results: 64 records were found, 5 being included for analysis. These articles had been published in the last 13 years, originated from different countries, and using a retrospective (cross-sectional, n=1, case-control, n=1) or prospective design (cross-sectional, n=1, case-control, n=1; cohort, n=1). The findings of the articles demonstrated differences regarding the presence of changes in the NHS outcomes of newborns whose mothers presented hypertension and/or diabetes during pregnancy. Conclusion: since the review showed contradictory results regarding changes in the NHS outcomes in the presence of mothers’ hypertension and/or diabetes during pregnancy, prospective cohort studies are needed, in different contexts, in order to isolate confounding factors for hearing loss and minimize measurement bias and selection.

RESUMO: Objetivo: avaliar as características orofaciais e a amamentação de recém-nascidos prematuros antes da alta hospitalar e verificar possíveis relações entre o sistema sensório motor orofacial e a amamentação. Métodos: trata-se de um estudo transversal, quantitativo e descritivo. A coleta de dados foi realizada no período de junho a outubro de 2014. Realizou-se em três etapas a pesquisa: busca de dados no prontuário, aplicação do protocolo de prontidão do prematuro para início da alimentação oral e aplicação do protocolo de avaliação da mamada em recém nascidos prematuros. Obteve-se uma amostra de conveniência em uma unidade de terapia intensiva de um hospital escola de Porto Alegre. Foram considerados critérios de inclusão: prematuridade; ter recebido atendimento fonoaudiológico, estar de alta hospitalar; indicação para aleitamento materno exclusivo; assinatura do termo de consentimento livre e esclarecido pelo responsável. Resultados: a amostra contou com 26 sujeitos, posteriormente 6 indivíduos foram excluídos. A maioria das características do sistema sensório motor orofacial dos prematuros estava adequada e a maior parte das categorias avaliadas na amamentação estava próxima ao escore máximo. Identificou-se que os prematuros com estado de consciência alerta apresentaram melhor posição mãe/recém-nascido durante o aleitamento materno (p= 0,043). Observou-se que quanto maior a idade gestacional corrigida melhor o escore final do prematuro na avaliação do sistema sensório motor orofacial (rs = 0,512; p= 0,021). Conclusões: elucidaram-se informações do sistema sensório motor orofacial e do aleitamento materno do prematuro. Bem como, foram encontradas relações entre o sistema sensório motor orofacial e a amamentação do prematuro.

ABSTRACT: Purpose: to evaluate oral characteristics and breastfeeding in premature newborns before hospital discharge and verify the relationship between the oral sensory motor system and breastfeeding. Methods: this was a cross-sectional, quantitative and descriptive study. Data collection was conducted from June to October 2014 and was carried out in three stages: Data collection from medical records, the clinical assessment of the oral sensory motor system and the evaluation of breastfeeding. A convenience sample was obtained from the intensive care unit of a university hospital in Porto Alegre. The inclusion criteria were: prematurity; receiving speech therapy, involvement in the hospital discharge process; recommendation of exclusive breastfeeding; have a free and informed consent form signed by a legal guardian. Results: the sample consisted of 26 subjects, of which 6 individuals were excluded. Most of the characteristics of the oral sensory motor system were adequate and most of the categories evaluated in breastfeeding were close to the maximum score. It was found that premature infants with an alert state of consciousness showed better mother / newborn positioning during breastfeeding (p = 0.043). It was observed that higher corrected gestational ages were associated with better final scores in evaluation of the oral sensory motor system (rs = 0.512, p = 0.021). Conclusions: the study elucidated some characteristics of the oral sensory motor system and of breastfeeding. Additionally, relationships were found between the oral sensory motor system and breastfeeding in premature infants.

Objetivos identificar e descrever as dúvidas e dificuldades das gestantes e puérperas em relação à amamentação, além de compará-las nos períodos pré-natal e puerperal. Métodos caracteriza-se por um estudo transversal, descritivo e comparativo, composto por dois grupos: gestantes e puérperas. Houve a aprovação do Comitê de Ética em Pesquisas / Santa Casa sob nº 23355. Para coleta elaborou-se questionário com perguntas sobre aleitamento materno. As variáveis foram analisadas por meio de associações entre as respostas dos grupos com a utilização dos testes Qui-Quadrado de Pearson, Exato de Fisher e T de Student. Resultados as puérperas apresentaram maior escore de conhecimento geral quando comparadas às gestantes (p = 0,001). Ao relacionar a idade com o conhecimento, quanto maior a idade da puérpera maior o percentual de conhecimento (r= 0, 283; p=0,011). Ao analisar as puérperas primíparas, observou-se que estas apresentavam mais queixas quando comparadas com as que já possuíam um ou mais filhos (p= 0,014). Conclusões gestantes, mulheres primíparas, adolescentes e jovens possuem mais dúvidas e dificuldades em relação ao aleitamento materno. A equipe assistencial deve estar preparada para que a gestante tenha o trato correto com suas mamas e conscientização sobre a amamentação natural, para assim chegar ao período puerperal mais segura e incentivada ao aleitamento exclusivo.

Purposes this research aimed to identify and describe the doubts and difficulties of pregnant and postpartum women about breastfeeding, and compare them on the pregnancy period and postpartum period. Methods it is characterized by a cross-sectional, descriptive, comparative, composed of two groups: pregnant and postpartum women. There was the approval of the Ethics Committee in Research / Santa Casa under nº 23355. For collect it drafted a questionnaire with questions about breastfeeding. The variables were analyzed by associations between the answers of the groups using the Chi-square test, Fisher’s exact and Student’s t. Results the postpartum women had higher knowledge scores about breastfeeding compared to pregnant women (p = 0,001). By linking the age with the knowledge, the higher the postpartum women age the higher their percentage of knowledge (r = 0, 283, p = 0,011). By analyzing the primiparous mothers, we found that these were more complaints compared to those who already have one or more children (p = 0,014). Conclusions pregnant women, primiparous women and teenager girls have more doubts and difficulties in relation to breastfeeding. The care team must be prepared for the pregnant woman has the right to treat her breast awareness and breast feeding, reaching the puerperal period safer and encouraged for exclusive breastfeeding.
