ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. RESUMO. O envelhecimento está geralmente associado a um declínio na cognição comparado à idade adulta. As alterações mais observadas são na atenção, percepção, memória de trabalho, memória de curto prazo, recordação livre e velocidade de processamento. Certas alterações cognitivas podem ocorrer no teste de Julgamento de Orientação de Linhas (JOL) em idosos. Objetivo: Descrever a diferença entre idade, sexo, escolaridade, Miniexame do Estado Mental (MEM) e Miniexame do Estado Mental em função da escolaridade (MEMFE), quando comparados ao teste de JOL em idosos. Avaliar o uso do teste de JOL no declínio cognitivo de idosos no Nordeste do Brasil. Métodos: Foi realizado um estudo transversal e randomizado com 280 idosos, entre 60 e 89 anos e média de idade de 69,4 (Dp±6,8 anos), 73,9% do sexo feminino residentes na cidade de Olinda, Estado de Pernambuco, Brasil. A idade foi estratificada a cada 5 anos entre 60 a 89 anos, e a escolaridade foi dividida em 4 subgrupos entre analfabetos a mais de 8 anos de escolaridade. Todos os participantes foram submetidos à análise de idade, sexo, escolaridade, fatores de risco, MEM, MEMFE e o teste de JOL. Resultados: Não houve diferença estatística entre os sexos. Porém, houve diferença estatística quando foi comparado o teste de JOL e idade (p<0,012), e escolaridade (p<0,001), e MEM (p<0,001) e MEMFE (p<0,001). Conclusão: Observamos que com ponto de corte de 18 pontos, o teste de JOL é indicado para avaliar alterações visoespaciais e visoperceptivas em idosos no Nordeste do Brasil. ABSTRACT. Aging is generally associated with a decline in cognition when compared to adulthood. The changes most seen are in attention, perception, working memory, short-term memory, free recall, and processing speed. Certain changes in the Judgment of Line Orientation (JLO) test may be present in elderly people. Objective: The objective of this study was to describe the difference among age, sex, schooling, Mini-Mental State (MMS), and Mini-Mental State Examination (MMSE) when compared to the JLO test in elderly people. The second is to assess the clinical use of the JLO test in elderly people's cognitive decline in Northeast Brazil. Methods: A cross-sectional, randomized study was carried out on 280 elderly people, between 60 and 89 years old, and the mean age was 69.4 (±6.8 years SD); 73.9% of the females lived in Olinda City, State of Pernambuco, Brazil. Age was stratified every 5 years between 60 and 89 years old, and schooling was divided into four subgroups between illiterate and more than 8 years of schooling. ABSTRACT The objective of this study was to examine the physicochemical composition and sensory properties of Boursin cheese made from the milk of goats fed diets with increasing levels of cassava chips replacing corn. Eight lactating Alpine goats were distributed into two 4 × 4 Latin squares, where they were subjected to diets in which 0, 33, 66, and 100% of corn was replaced with cassava chips. The cheeses were prepared with individual milk of each goat for physicochemical, yield, and texture analyses. Sensory analysis consisted of flavor and texture appraisal by 50 untrained tasters, using the difference-from-control test and the acceptance test. The cheeses from the four treatments did not differ in their fat, protein, and total solid contents; acidity; moisture; ash; pH; yield; or the texture parameters of gumminess, cohesiveness, hardness, and chewiness. RESUMO A afasia progressiva primária é uma síndrome clínica causada por uma neurodegeneração de áreas e redes neurais envolvidas na linguagem, geralmente no hemisfério esquerdo. O termo “afasia cruzada” denota uma disfunção adquirida de linguagem causada por uma lesão no hemisfério ipsilateral da mão dominante. Objetivo: Relatamos um caso de afasia cruzada em uma paciente de 60 anos, canhota, com um quadro clínico de afasia progressiva primária variante não fluente (idade de início=52), evidenciada por assimetria no SPECT cerebral à esquerda. Métodos: Foram realizados para o diagnóstico do caso: história clínica e familiar, o Inventário de Dominância de Edinburgh, a Escala de Atividades Funcionais de Pfeffer, o Miniexame do Estado Mental, o Teste das Trilhas, o Teste da Torre de Londres, a Avaliação Neuropsicológica Adequada às Demências e exames de neuroimagem. Resultados: A avaliação neuropsicológica mostrou comprometimento cognitivo severo, principalmente sobre a linguagem; a ressonância magnética do crânio mostrou sinais de involução córtico-subcortical, com predominância nos lobos frontal e temporal e a cintilografia cerebral por emissão de fóton único mostrou hipoperfusão moderada a severa no hemisfério cerebral esquerdo, incluindo o hipocampo. ABSTRACT Primary progressive aphasia is a clinical syndrome caused by neurodegeneration of areas and neural networks involved in language, usually in the left hemisphere. The term “crossed aphasia” denotes an acquired language dysfunction caused by a lesion in the ipsilateral hemisphere to the dominant hand. Objective: To describe a case of crossed aphasia in a 60-year-old left-handed patient with a non-fluent variant of primary progressive aphasia diagnosis (age of onset=52), evidenced by a left asymmetry on brain SPECT scan. Methods: Clinical and family history, the Edinburgh Handedness Inventory, Measurement of Functional Activities in Older Adults in the Community, the “Mini-Mental State Examination”, the Trail Making Test, the Tower of London, and the Neuropsychological assessment for dementia, and neuroimaging studies were carried out. Results: Neuropsychological assessment showed severe cognitive impairment, especially regarding language. The magnetic resonance imaging showed important signs of cortico-subcortical atrophy, with predominance in the frontal and temporal lobes. The single-photon emission computed tomography scan showed moderate to severe hypoperfusion in the left cerebral hemisphere, including the hippocampus. RESUMO Objetivo: Estimar a reprodutibilidade intra e interobservadores da avaliação da obliquidade pélvica (OP) nos pacientes com deformidades neuromusculares pelo método que usa as cristas ilíacas e pelo método que usa o platô superior de S1e avaliar se há relação entre os métodos. Métodos: Trinta radiografias panorâmicas digitalizadas de pacientes com paralisia cerebral e mielomeningocele acompanhados em ambulatório foram avaliadas por quatro examinadores, sendo dois cirurgiões de coluna experientes e doisfellows. Foram excluídas duas radiografias que impossibilitavam a análise. Todos os exames foram obtidos conforme protocolo de acompanhamento periódico, na posição sentada, em filme digitalizado e distância foco-filme de 110 cm. Resultados: Observou-se alta concordância intra e interobservadores tanto do método que usa as cristas ilíacas quanto do método que avalia o platô de S1. No entanto, não foi observada relação significativa entre os dois métodos. Conclusões: Os métodos avaliados apresentam boa reprodutibilidade e concordância entre os observadores. Verificou-se que, por conta da relação linear existente, é possível estimar o valor do método das cristas ilíacas conhecendo o valor obtido pelo método do platô de S1 multiplicado por 0,76. Não houve concordância entre o método de avaliação da OP pelas cristas ilíacas em comparação com o método de avaliação pelo platô de S1. Nível de evidência IV; Estudo transversal retrospectivo.
ABSTRACT Objective: To evaluate the intra- and inter-observer reproducibility of the evaluation of the pelvic obliquity (PO) in patients with neuro-muscular deformities via the method that uses the iliac crests and the method that uses the upper endplate of S1 and to determine whe ther there is a relationship between the methods. Methods: The digitized panoramic radiographies of thirty patients with cerebral paralysis or myelomeningocelein outpatient monitoring were evaluated by four examiners: two experienced spinal surgeons and two fellows. Two radiographs were excluded because analysis was impossible. All exams were obtained in accordance with the periodic monitoring protocol in the sitting position, using digitized film and a film-focus distance of 110 cm Results: High intra- and inter-observer agreement was observed both for method that uses the iliac crests and the method that evaluates the S1 endplate. However, no significant relationship between the two methods was observed. Conclusions: The methods evaluated had good reproducibility and agreement among the observers. It was confirmed that, on account of the existent linear relation, it is possible to estimate the value of the iliac crest method knowing the value obtained by the S1 plateau multiplied by 0.76. There was no agreement between the iliac crest and S1 plateau PO evaluation methods. Level of evidence IV; Retrospective cross-sectional study.

Se excluyeron dos radiografías porque imposibilitaban el análisis. Todos los exámenes se obtuvieron según el protocolo de acompañamiento periódico, en posición sentada, sobre película digitalizada y distancia foco-película de 110 cm. Resultados: Se observó una alta concordancia intra e interobservador tanto en el método que utiliza las crestas ilíacas como en el que evalúa la meseta de S1. Sin embargo no se observó ninguna relación significativa entre los dos métodos. Conclusiones: Los métodos evaluados presentaron buena reproducibilidad y concordancia entre los observadores. Se observó que, debido a la relación lineal existente, es posible estimar el valor del método de las crestas ilíacas conociendo el valor obtenido por el método demeseta de S1 multiplicado por 0,76. No hubo concordancia entre el método de evaluación de la OP por las crestas ilíacas en comparación conel método de evaluación por meseta de S1. ABSTRACT This study examined the replacement of maize and soybean meal with cassava chips and alfalfa grazing, respectively. Twelve lactating Anglo-Nubian goats were kept on a Panicum maximum cv. Tobiatã pasture. The experiment was laid out in a Latin square design in which the following diets were tested: ground maize + soybean meal, cassava chips + soybean meal, ground maize + alfalfa grazing, and cassava chips + alfalfa grazing. The evaluated variables were feed intake, daily weight gain, milk yield and composition, and feeding behavior of the goats as well as production costs. Cassava chips and grazed alfalfa influenced the intakes of dry matter, crude protein, neutral detergent fiber, and total digestible nutrients. However, milk yield, body weight, and body score did not change. There was no diet effect on the proportions of protein, solids-not-fat, somatic cell count, or urea nitrogen in the milk. Treatments influenced the levels of fat, lactose, and total solids in milk, with the highest fat levels achieved with diets containing alfalfa. Grazing, rumination, and idle times and time spent interacting with other goats were not influenced by diets. The evaluated feedstuffs improved feed efficiency and reduced production costs. Therefore, cassava chips and alfalfa can replace certain ingredients without impairing the production performance of goats, but rather improving the profit of the producer.

RESUMO. Relatamos o caso de um paciente do sexo masculino, 61 anos, com anacusia, síndrome cerebelar, mioclonia e sinais frontais. A ressonância magnética cerebral mostrou hiperintensidade estriada bilateral do fluid-attenuated inversion recovery (FLAIR) e difusão restrita no diffusion-weighted imaging (DWI) e áreas hipointensas correspondendo ao coeficiente de difusão aparente no córtex cerebral. A autópsia revelou imuno-histoquímica positiva para a proteína PrPSc. A doença de Creutzfeldt-Jakob que se apresenta com perda auditiva é incomum.
ABSTRACT. We reported a case of a 61-year-old male patient with anacusis, cerebellar syndrome, myoclonus, and frontal signs. The brain magnetic resonance imaging showed bilateral striated hyperintensity of the fluid-attenuated inversion recovery and restricted diffusion in the diffusion-weighted imaging and hypointense areas corresponding to the apparent diffusion coefficient in the cerebral cortex. The autopsy revealed positive immunohistochemistry for the PrPSc protein. Creutzfeldt–Jakob disease presenting with hearing loss is unusual.

One's first social contacts beyond his family happens at school. How children and adolescents settle themselves at school influences their social relations during adulthood. Through a social inclusion perspective, this qualitative and interdisciplinary study aimed to get to know social representations developed by students regarding Down's Syndrome adolescents. Its basic referential theory was the Social Representation Theory. Data was collected through participant observation and semi-structured interviews with students who shared their classroom with adolescents with Down´s Syndrome. Data collected was submitted to methodological procedures of Speech Analysis, dialectically with Social Representation Theory. It was observed that relation between students with and without Down's Syndrome is positive and that the latter worries about how the former is treated by other people. Therefore, they stand for equity in adolescents with Down's Syndrome´s social relations. Nevertheless, the idea of capacitism was unveiled in representations, which is a great obstacle for inclusion.
En la escuela, los primeros contactos sociales del individuo tienen lugar más allá del núcleo familiar. La forma en que los niños y adolescentes se establecen en la escuela influye en la forma en que construyen relaciones sociales en la edad adulta. A través de la perspectiva de la inclusión social, este estudio, de carácter cualitativo, con un enfoque interdisciplinario, tuvo como objetivo conocer las representaciones sociales elaboradas por pares escolares sobre el adolescente con Síndrome de Down. El marco teórico básico de esta investigación fue la Teoría de las Representaciones Sociales. La recolección de datos se realizó a partir de la observación participante y entrevistas semiestructuradas realizadas a estudiantes que asistían al aula con un adolescente con Síndrome de Down. Los contenidos recolectados fueron sometidos a los procedimientos metodológicos de Análisis del Discurso, basados en una dialéctica con la Teoría de las Representaciones Sociales. Se observó que las relaciones entre pares son positivas y que se preocupan por la forma en que la sociedad los trata, por lo que defienden la equidad en las relaciones sociales con la persona con Síndrome de Down. Sin embargo, también se desveló el fenómeno del capacitismo, un gran obstáculo para el desafío de la inclusión social.
Na escola acontecem os primeiros contatos sociais do indivíduo para além do núcleo familiar. O modo como as crianças e os adolescentes se estabelece na escola influencia como constroem as relações sociais na vida adulta. Através da perspectiva da inclusão social, esse estudo, de natureza qualitativa, com aporte interdisciplina, teve como objetivo conhecer as representações sociais elaboradas por pares escolares a respeito do adolescente com Síndrome de Down. O referencial teórico basilar para esta pesquisa foi a Teoria das Representações Sociais. A coleta de dados se deu com base na observação participante e em entrevistas semiestruturadas realizadas com estudantes que frequentavam sala de aula com uma adolescente com Síndrome de Down. Os conteúdos coletados foram submetidos aos procedimentos metodológicos da Análise do Discurso, a partir de uma dialética com a Teoria das Representações Sociais. Observou-se que as relações entre os pares são positivas e que se preocupam com o modo que a sociedade lida com eles, portanto, defendem a equidade nas relações sociais com a pessoa com Síndrome de Down. Entretanto, também desvelou-se o fenômeno do capacitismo, um grande obstáculo ao desafio da inclusão social.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

RESUMO Introdução: A pandemia de coronavírus iniciada na China em 2019 (COVID-19) não só causou problemas de saúde pública mas também trouxe grande sofrimento psíquico, principalmente aos médicos envolvidos no enfrentamento da doença ou àqueles do grupo de risco em isolamento social, e representa um desafio para a resiliência psicológica da população mundial. Estudos mostram que profissionais de saúde apresentaram sintomas psicológicos como depressão, ansiedade, insônia, estresse, entre outros. Objetivos: Investigar a qualidade do sono e a taxa de prevalência de transtornos do sono entre os médicos durante a pandemia do COVID-19, e identificar os fatores psicológicos e sociais associados ao quadro. Métodos: Estudo transversal com aplicação de questionário on-line a médicos do Brasil e inclusão de 332 participantes, dos quais 227 eram mulheres. Foram utilizados questionários com avaliação sociodemográfica, escala de impacto a eventos com modificações causadas pelo COVID-19, avaliação da qualidade do sono (PSQI), presença e gravidade da insônia (IGI), sintomas depressivos (PHQ-9) e ansiedade (TAG-7). Resultados: A maioria dos médicos (65,6%) apresentou alterações no sono, sendo a má qualidade do sono reportada por 73,1%. Sintomas depressivos estiveram presentes em 75,8%, e ansiedade em 73,4%. Conclusão: Nosso estudo constatou que mais de 70% dos médicos avaliados apresentavam comprometimento da qualidade do sono, caracterizando sintomas de insônia durante o surto de COVID-19. Os fatores relacionados incluíram ambiente de isolamento, preocupações com o surto de COVID-19 e sintomas de ansiedade e depressão. Intervenções especiais são necessárias para promover o bem-estar mental dos profissionais de saúde e implementar mudanças nesse cenário.
ABSTRACT Introduction: Coronavirus pandemic began in China in 2019 (COVID-19), causing not only public health problems but also great psychological distress, especially for physicians involved in coping with the virus or those of the risk group in social isolation, and this represents a challenge for the psychological resilience in the world population. Studies showed that health professionals had psychological symptoms such as depression, anxiety, insomnia, stress, among others. Objectives: To investigate the quality of sleep and the prevalence rate of sleeping disorders among physicians during COVID-19 pandemic, and identify the psychological and social factors associated with the condition. Methods: A cross-sectional study of an online questionnaire was applied for physicians in Brazil. Among the 332 participants included, 227 were women. Sociodemographic assessment was used in the questionnaire, as well as the scale of impact on the events of modifications caused by COVID-19, assessment on sleep quality (PSQI), presence and severity of insomnia (ISI), depressive symptoms (PHQ-9), and anxiety (GAD-7). Results: Most physicians (65.6%) had changes in sleep. Poor sleep quality was reported by 73.1%, depressive symptoms were present in 75.8%, and anxiety in 73.4%. Conclusion: Our study found that more than 70% of the physicians assessed had impaired sleep quality, characterizing insomnia symptoms during COVID-19 outbreak. Related factors included an environment of isolation, concerns about COVID-19 outbreak and symptoms of anxiety and depression. Special interventions are needed to promote health professionals’ mental well-being and implement changes in this scenario.

Performance, carcass traits, and meat fatty acid profile and quality of Anglo-Nubian kids fed diets supplemented with vegetable oils

ABSTRACT The objective of this study was to evaluate the inclusion of vegetable oils in goat kid diets on performance, carcass traits, and meat composition, quality, and fatty acid profile. Forty-eight Anglo Nubian kids were evaluated in a completely randomized experimental design with four treatments, namely, control diet and diets including 2.5% canola, sunflower, or soybean oils. The animals were slaughtered at 30 kg live weight and performance, carcass and meat quality, centesimal composition, and fatty acid profile were evaluated. The treatments did not influence carcass yield or meat color. Among the saturated fatty acids, only caprylic, palmitic, and heptadecanoic acids differed among the treatments, whereas total saturated fatty acids decreased in the sunflower oil treatment. Monounsaturated fatty acids were not influenced by lipid supplementation. Among the polyunsaturated fatty acids (PUFA), linoleic acid was highest in the sunflower oil-treated group. Total PUFA were higher for the soybean oil-supplemented group, but similar to that achieved with sunflower oil. The highest omega-6 content was obtained with dietary inclusion of sunflower oil, whereas conjugated linoleic acid, atherogenicity index, and hypocholesterolemic:hypocholesterolemic ratio improved with the inclusion of sunflower and soybean oils. Supplementation with vegetable oils improves the fatty acid profile of kid meat without affecting animal performance. Better results are obtained with sunflower and soybean oils; therefore, it is up to the producer to choose the diet according to its cost.

This article presents the history and construction of the National Health Survey (PNS) 2019, a household survey conducted in partnership with the Brazilian Institute of Geography and Statistics. The objective was to provide the country with information on the determinants, conditions and health needs of the Brazilian population. The expected sample was 108,525 households, considering a non-response rate of 20%. The questionnaire had three parts: (i) regarding the household; (ii) to all residents of the household, focusing on the collection of socioeconomic and health information; and (iii) aimed at the selected resident (15 years or more) for whom lifestyles, chronic diseases, violence, among other topics were investigated, and anthropometric measures (sub-sample) were measured. The PNS information will serve as a basis for the (re)formulation of health policies, as well as support for existing actions and programs of the Unified Health System.
Este artigo apresenta o histórico e a construção da Pesquisa Nacional de Saúde (PNS) 2019, inquérito de base domiciliar realizado em parceria com a fundação Instituto Brasileiro de Geografia e Estatística. O objetivo da PNS 2019 foi dotar o país de informações sobre os determinantes, condicionantes e necessidades de saúde da população brasileira. A amostra prevista foi de 108.525 domicílios particulares, considerando-se uma taxa de não resposta de 20%. Seu questionário continha três partes, orientadas para (i) o domicílio, (ii) todos os moradores do domicílio, com enfoque na coleta de informações socioeconômicas e de saúde, e (iii) o morador selecionado (idade ≥15 anos), sobre o qual investigou-se estilos de vida, doenças crônicas, violências, entre outros temas, e aferiu-se medidas antropométricas (subamostra). As informações da PNS 2019 servirão de base para a (re)formulação de políticas de saúde e subsídio a ações e programas existentes do Sistema Único de Saúde.

Resumo Contexto Implementar um programa para profilaxia de tromboembolismo venoso (TEV) em hospitais é uma recomendação de diretrizes internacionais e da Agency for Healthcare Research and Quality para segurança dos pacientes. O Programa TEV Safety Zone (TEVSZ) é um modelo que sugere avaliação sistemática do risco de TEV incorporada às rotinas do hospital com participação institucional e multidisciplinar continuada. Objetivos Levantar dados de implementação de iniciativas para profilaxia em hospitais brasileiros que iniciaram o Programa TEVSZ. Métodos Envio de questionário por correio eletrônico aos responsáveis pelos programas TEVSZ em hospitais visitados até julho de 2016. Resultados Dos 132 convites enviados, foram obtidas 68 respostas, sendo 50 (73,5%) completas. Em 61,5% dos hospitais participantes havia entre 100 e 250 leitos, e 65,4% tinham mais de 20 leitos de terapia intensiva; 61,5% referiam ter acreditação hospitalar, 86,3% tinham comissão de profilaxia de TEV e 58% tinham prontuários eletrônicos. As avaliações de risco de TEV pela diretriz brasileira, escores de Pádua ou Caprini eram feitas no prontuário eletrônico em 56,9% e como passo obrigatório em 45,1% dos casos. Em apenas 25% dos hospitais, a reavaliação do risco de TEV era solicitada antes da alta, e foram citadas várias barreiras no processo de implementação do TEVSZ. Conclusões O estudo mostra um panorama da implementação do TEVSZ em hospitais brasileiros. As avaliações sistemáticas de risco ainda não ocorrem na maioria dos pacientes. O reconhecimento de diversas barreiras no processo pode levar a novas estratégias para a adequação da profilaxia e segurança dos pacientes hospitalizados.
Abstract Background In common with other international guidelines, the Agency for Healthcare Research and Quality recommends implementation of venous thromboembolism (VTE) prophylaxis programs in hospitals as a measure for patient safety. The VTE Safety Zone Program (VTESZ) proposes a model for incorporation of systematic VTE risk-assessment into hospital routines, with continuing institutional and multidisciplinary participation. Objectives To evaluate implementation of VTE prophylaxis initiatives in Brazilian hospitals that have adhered to the VTESZ Program. Methods Questionnaires were e-mailed to VTESZ Program representatives at hospitals visited up to July 2016. Results Of the 132 invitations sent, 68 answers were obtained and 50 (73.5%) were complete. 61.5% of participating hospitals had between 100 and 250 beds, and 65.4% had more than 20 intensive care beds; 61.5% reported having hospital accreditation, 86.3% had VTE prophylaxis committees, and 58% had electronic medical records. VTE risk assessments using the Brazilian guidelines or the Padua or Caprini scores were noted on the electronic medical record in 56.9% and were a mandatory step in 45.1% of the cases. VTE risk reassessment was requested prior to discharge in only 25% of hospitals and several issues were cited that negatively affect the VTESZ implementation process. Conclusions This study provides an overview of implementation of VTESZ in Brazilian hospitals. Systematic risk assessment is not yet conducted for most patients. Recognition of various issues affecting the process may lead to new strategies for achieving adequate prophylaxis and safety of hospitalized patients.

RESUMO Sistemas de produção baseados em pastagens e alimentos alternativos podem ser utilizados para reduzir cerca de 20% os custos de produção do leite caprino.O objetivo deste trabalho foi avaliar a influência da substituição do milho e do farelo de soja do concentrado, pela raspa de mandioca e alfafa, respectivamente, sobre o consumo, fermentação e degradabilidade ruminal de cabras adultas Saanen confinadas, vazias, secas e canuladas no rúmen. O experimento foi conduzido no delineamento em quadrado latino, com as dietas: milho triturado e farelo de soja; raspa de mandioca e farelo de soja; milho triturado e alfafa; raspa de mandioca e alfafa. Foram avaliados: consumo, parâmetros ruminais, produção de ácidos graxos de cadeia curta (AGCC) e cinética da degradação das dietas. A inclusão da raspa de mandioca e da alfafa não influenciou o consumo e a produção de amônia. O pH ruminal e as produções de AGCC foram influenciados pelos tempos de colheita,apresentando um pH elevado (6,29) na primeira mensuração (7 h 00) e diminuindo em seguida, influenciando dessa forma as produções de AGCC ao longo do dia. O ácido acético e a produção total de AGCC foram influenciados pelas dietas, sendo os maiores valores para a dieta mandioca/alfafa (68,03 e 93,64 mM 100 mM-1, respectivamente) e os menores valores para a dieta milho/soja (55,40 e 76,03 mM 100 mM-1, respectivamente).A degradabilidade dos nutrientes foi influenciada pelas dietas, sendo que as com mandioca e mandioca/alfafa apresentaram as maiores degradabilidades efetivas(57,57 para 0.02 h-1 e 53,49 para 0.05 h-1; 57,25 para 0.02 h-1 e 53,26 para 0.05 h-1, respectivamente), demonstrando que a mandioca e a alfafa podem substituir o milho e o farelo de soja dos concentrados, sem alterar o consumo e o ambiente ruminal das cabras, melhorando a produção de ácido acético e a degradabilidade da dieta.
ABSTRACT Pasture-based production systems and alternative foods can be used to reduce goat milk production costs by around 20%.The objective of this study was to examine the effect of substituting the corn and soybean meal of the concentrate with cassava chips and alfalfa, respectively, on the feed intake, fermentation, and rumen degradability parameters of adult, dry, empty, rumen-cannulated Saanen goats reared in a feedlot. The experiment was conducted as a Latin square design in which the following diets were tested: ground corn and soybean meal; cassava chips and soybean meal; ground corn and alfalfa; and cassava chips and alfalfa. The following variables were evaluated: feed intake, rumen parameters, short-chain fatty acid (SCFA) production, and feed degradation kinetics. Inclusion of cassava chips and alfalfa did not influence feed intake or ammonia production. Rumen pH and SCFA production were influenced by the collection times, presenting a high pH (6.29) at the first measurement (07 h 00) and then decreasing, thus influencing SCFA production throughout the day.Acetic acid and total SCFA production were influenced by the diets, with the highest values for the cassava/alfalfa diet (68.03 and 93.64 mM 100 mM-1, respectively) and the lowest values for the corn/soybean diet (55.40 and 76.03 mM 100 mM-1, respectively).Nutrient digestibility was also influenced by diets, with those containing cassava and cassava/alfalfa, providing the greatest effective degradability (57.57 to 0.02 h-1 and 53.49 to 0.05 h-1; 57.25 to 0.02 h-1and 53.26 to 0.05 h-1, respectively). This finding demonstrates that cassava and alfalfa can substitute the corn and soybean meal of concentrates without changing feed intake or the ruminal environment of goats. Rather, this substitution improves acetic acid production and diet digestibility.

ABSTRACT We aimed to evaluate the inclusion of three sources of vegetable oil in the diet of lactating goats on production in 120 days of lactation and the effect of these sources and lactation stage on fortnightly composition and fatty acid profile of goat milk at 20, 50, 80, and 110 days of lactation. A completely randomized design was adopted and 32 Anglo Nubian goats were used, distributed in four treatments: control diet and diets with inclusion of 30 g/kg of dry matter of diet of canola, sunflower, or soybean oil. The dairy production was 182.75 kg, and there was no difference for treatments. Among the constituents, only urea nitrogen was influenced by treatment and presented lower content for control treatment. Day of lactation had an effect on lactose. Defatted dry extract and somatic cell count had a quadratic effect with minimum values around 100 and 33 days of lactation, respectively. The content of urea nitrogen, also with a quadratic effect, was higher at 93 days of lactation. For protein, there was an interaction between treatments and period and, at the end of lactation, its content was increased. The inclusion of vegetable oils promoted reduction in total saturated fatty acids (SFA) and increased the total content of monounsaturated fatty acids (MUFA) and conjugated linoleic acid. The proportions MUFA:SFA and PUFA:SFA, the atherogenicity and thrombogenicity indexes, and the relation hypocholesterolemic fatty acids:hypercholesterolemic fatty acids improved with oil addition in animal diets. The addition of vegetable oil to diets for lactating goats improve the fatty acid profile with no impairment on milk production and composition, and the milk from early stages of lactation has better nutritional quality of the lipid fraction.
