Resumo Hub Criativo do Beato é uma infraestrutura voltada à inovação digital da cidade de Lisboa, Portugal, que vem sendo constituída, desde 2016, por um processo que chamo de refuncionalização contínua e flexível de um antigo complexo industrial militar. Também a compreendo como âncora de reabilitação urbana e como dispositivo de representação para uma Lisboa que compete internacionalmente por atração de capitais em contexto de ascensão global da economia digital. Multiplicidade de ideias, experimentações, aberturas a mudanças e adaptações dão o tom dos projetos lá realizados. O artigo busca compreender criticamente os enlaces possíveis entre produção do urbano e economia digital. Abstract Hub Criativo do Beato is an infrastructure targeted at digital innovation in the city of Lisbon, Portugal, that has been constituted, since 2016, through a process that I call continuous and flexible repurposing of an old military-industrial complex. I also understand it as an anchor for urban rehabilitation and as a representation apparatus for a city that competes internationally to attract capital in the context of the global rise of the digital economy. A multiplicity of ideas, experiments, and openness to change and adaptations guide its projects. The article seeks to critically understand the connections between urban production and the digital economy. RESUMO No clima semiárido, o crescimento de mudas frutíferas é desafiado pelo estresse hídrico característico da região, o que pode restringir seu desenvolvimento e, consequentemente, afetar a produção. Nesse contexto, o objetivo foi avaliar o efeito do extrato pirolenhoso nas características de troca gasosa, pigmentos fotossintéticos, crescimento e biomassa de pitangueira sob diferentes intensidades de déficit hídrico. O experimento foi conduzido em casa de vegetação utilizando um delineamento em blocos casualizados em um esquema fatorial 3 × 2, com quatro repetições, correspondendo a períodos de déficit hídrico foram de 7 e 14 dias, além do controle (irrigação diária), no substrato sem e com extrato pirolenhoso. As plantas foram avaliadas quanto às características de trocas gasosas foliares, índices de clorofila, crescimento e biomassa seca. A aplicação de extrato pirolenhoso aos 7 dias de déficit hídrico elevou a taxa fotossintética, a eficiência de carboxilação e o índice de qualidade de Dickson das mudas de pitanga em 40,04, 42,85 e 41,51%, respectivamente, em comparação aos 14 dias de déficit hídrico. O extrato pirolenhoso foi efetivo como um atenuante do estresse hídrico nas mudas de pitanga, especialmente durante os primeiros sete dias de exposição ao estresse. ABSTRACT In the semi-arid climate, the growth of fruit seedlings is challenged by the water stress characteristic of the region, which can restrict their development and, consequently, affect production. In this context, the aim was to assess the effect of pyroligneous extract on gas exchange characteristics, photosynthetic pigments, growth, and biomass of pitanga under different intensities of water deficit. The experiment was conducted using a randomized block design in a 3 × 2 factorial scheme, with four replicates, corresponding to water deficit periods (7 and 14 days) and control (daily irrigation), in substrates with and without pyroligneous extract. The plants were evaluated for gas exchange characteristics, chlorophyll indices, growth, and dry biomass. Application of pyroligneous extract after 7 days of water deficit increased the photosynthetic rate, carboxylation efficiency and Dickson quality index of pitanga seedlings by 40.04, 42.85 and 41.51%, respectively, compared to 14 days of water deficit. Pyroligneous extract was effective as a water stress attenuator in pitanga seedlings, especially during the first seven days of exposure to stress. Resumo A leucemia linfoblástica aguda (LLA) é a causa mais comum de neoplasia em idade pediátrica. Excluindo os tumores musculoesqueléticos, a LLA é uma das neoplasias mais frequentemente associadas a sintomas osteomusculares na apresentação inicial. Muitas das crianças com LLA são inicialmente referenciadas à Ortopedia, pelo que é necessário um elevado grau de suspeição para evitar o atraso do diagnóstico. Os autores relatam dois casos de crianças com diagnóstico presumível de infeção osteoarticular cuja evolução clínica levou ao diagnóstico de doença linfoproliferativa.

Abstract Acute lymphoblastic leukemia (ALL) is the most common cause of malignancy in pediatric age. With the exception of musculoskeletal tumors, ALL is one of the malignancies most commonly associated with musculoskeletal symptoms at initial presentation. Some children with ALL are initially referred to Orthopedics for diagnostic evaluation, and a high index of clinical suspicion is required to avoid diagnostic delay. The authors report two cases of children initially diagnosed with a presumed osteoarticular infection, but whose clinical evolution led to the diagnosis of lymphoproliferative disease.

Abstract Myasthenia gravis is an autoimmune disease of the neuromuscular junction that is characterized by muscle weakness and fatigue. Dropped head syndrome is a rare complication of myasthenia gravis and is manifested by a head drop resulting from progressive weakness of the flexor and extensor muscles of the neck. Targeted treatment of myasthenia gravis is associated with a recovery of neck strength, thus reducing the negative impact on daily life caused by this disease. It is essential that all healthcare professionals recognize the association between these two entities. We present the clinical case of a 68-year-old female initially evaluated in primary health care for head drop. The diagnosis of myasthenia gravis was only made one month after the initial appointment when other symptoms such as hypophonia and dysphagia were already present. Treatment with intravenous immunoglobulin allowed the recovery of strength in the extensor muscles of the neck.

Resumo A miastenia gravis é uma doença autoimune da junção neuromuscular que se caracteriza por fraqueza muscular e fadiga. A síndrome de dropped head é uma complicação rara da miastenia gravis e manifesta-se pela queda cefálica decorrente da fraqueza progressiva dos músculos flexores e extensores do pescoço. O tratamento dirigido da miastenia gravis está associado a uma recuperação da força cervical, reduzindo assim o impacto negativo na vida diária provocado por esta entidade. Revela-se fulcral que todos os profissionais de saúde reconheçam a associação existente entre estas duas entidades. Apresenta-se o caso clínico de uma doente de 68 anos avaliada inicialmente nos cuidados de saúde primários por queda cefálica. O diagnóstico de miastenia gravis só foi realizado um mês após a consulta inicial quando já se acompanhava de outros sintomas como hipofonia e disfagia. O tratamento com imunoglobulina intravenosa permitiu a recuperação da força dos músculos extensores do pescoço.

Abstract The arrival of COVID-19 in Brazil had a marked impact on the health network as well as on the professionals involved, due to the significant number of cases and constantly updated information. In this context, the Cooperative Intelligence Platform for Primary Health Care - Picaps - is set up as a technological platform that assists by systematizing the communication and negotiation processes among actors, with a focus on science and technology. This article sets out to describe the processes of Picaps, which consist of a collaborative system conceived of as a public solution capable of generating innovations in the areas of Science, Technology and Health, in order to tackle COVID-19 in territories with populations in socially vulnerable situations in the Federal District (DF). It integrates data collection and processing, as well as the dissemination of information for its use, both at the governmental and societal levels. Picaps can be seen as a powerful government instrument to help societies across the country, especially those in vulnerable situations, capable of assisting in the use of cooperative intelligence in primary care and health surveillance actions to build innovative solutions such as rapid responses to face health crises in modern times.

Resumo A chegada da COVID-19 ao Brasil gerou grande impacto no sistema de saúde, bem como em seus profissionais, com o elevado número de casos e informações constantemente publicadas. Nesse contexto, criou-se a Plataforma de Inteligência Cooperativa para a Atenção Primária à Saúde (Picaps), uma plataforma tecnológica que ajuda a sistematizar os processos de comunicação e negociação de atores, tendo como foco a ciência e a tecnologia. O presente trabalho tem como objetivo descrever os processos da Picaps, sendo esta constituída por um sistema colaborativo concebido como solução pública capaz de gerar inovações nas áreas de ciência, tecnologia e saúde para o enfrentamento da COVID-19 no Distrito Federal (DF). Nela integram-se processos de coleta e tratamento de dados, além da disseminação de informações visando o seu uso tanto em nível governamental quanto em nível societal. A Picaps pode ser vista como um poderoso instrumento governamental para auxiliar sociedades espalhadas pelo país, sobretudo as que se encontram em situação de vulnerabilidade, com capacidade de nortear o uso da inteligência cooperativa em ações de atenção primária e vigilância em saúde para construção de soluções inovadoras como respostas rápidas para o enfrentamento de crises sanitárias em tempos contemporâneos.

Abstract Public Health Consortiums (PHC) in Brazil represent a strategy to enhance regionalization in the Public Health Care System (SUS in Portuguese) and State/interstate/intermunicipal cooperation. The establishment of regional polyclinics aims to improve access to services with greater technological concentration and closer to users’ homes. This study specifically aims to analyze the process of creating PHC and regional polyclinics in Bahia State, based on documental analysis, identifying aspects related to entering the political agenda that facilitate and hinder, the role of state administration, specificities of the composition of these PHC and regional polyclinics. A documental analysis was carried out of the minutes of the Comissão Intergestores Bipartite (CIB-BA) [Bipartite Administrative Commission] from 2015 to 2018 and of constitutive documents of the consortiums established in this state. The Ceará State experience inspired the creation of consortiums in Bahia, especially the structuring of polyclinics. Municipal administrators demonstrated a favorable position regarding the potential of consortiums with cooperation among municipalities. The role of the state government proved to be a facilitating condition, whereas the maintenance of the municipal counterpart in finance constituted a hindering element. The consortiums contributed to the regionalization of health in the state as well as expansion of access to specialized care.

Resumo Os consórcios públicos de saúde (CPS) representam uma estratégia para potencializar a regionalização no SUS e a cooperação interfederativa. A implantação de policlínicas regionais visa melhorar o acesso a serviços de maior densidade tecnológica, mais próximos ao município de origem dos usuários. Este estudo objetiva analisar o processo de implantação dos consórcios públicos de saúde e das policlínicas regionais na Bahia, com base em análise documental, identificando aspectos facilitadores e dificultadores relativos à entrada na agenda política, ao papel da gestão estadual e às especificidades da composição dos CPS e das policlínicas regionais no estado da Bahia. Realizou-se análise documental das atas da Comissão Intergestores Bipartite dos anos de 2015 a 2018 e de documentos constitutivos dos consórcios implantados na Bahia. A experiência do Ceará inspirou a criação dos consórcios na Bahia, sobretudo na estruturação das policlínicas. Verificou-se posicionamento favorável dos gestores municipais acerca da potencialidade do consórcio na cooperação entre municípios. A atuação do governo do estado revelou-se condição facilitadora, e a manutenção da contrapartida municipal no financiamento, como elemento dificultador. Os consórcios contribuíram para a regionalização da saúde no estado e a ampliação do acesso à atenção especializada.

ABSTRACT Mixture designs are becoming frequently used as a powerful tool in modern chemical research. Their versatility opens the door to countless applications, making it a must-have tool in any experimental environment. Mixture designs combined with mathematical regressions allow us to understand how component proportions affect a measured response leading to system optimization at minimum cost and providing an accurate description of system behaviour. Dedicated software is often used to perform data analysis in mixture design experiments, but many of them are only commercially available hindering the use of mixture designs by scientific groups. The availability of free software would make these designs even more attractive, allowing users to benefit from the creative potential of this toolset without the prohibitive cost of proprietary tools. RStudio and LibreOffice are some of the free software with the capability to perform the necessary data analysis accompanying mixture designs. To exemplify how mixture design analysis can be executed the book “Como Fazer Experimentos” was used as reference material. Abstract Objective Autism spectrum disorder (ASD) is characterized by persistent deficits in social communication and social interaction and by restricted and repetitive patterns of behavior. Some studies have shown that substances derived from Cannabis sativa improve the quality of life of children with ASD without causing serious adverse effects, thus providing an alternative therapeutic option. The objective of this study was to evaluate the efficacy and safety of a cannabis extract rich in cannabidiol (CBD) in children with ASD. Methods In this randomized, double-blind, placebo-controlled clinical trial, 60 children, aged from 5 to 11 years, were selected and divided into two groups: the treatment group, which received the CBD-rich cannabis extract, and the control group, which received the placebo. They both used their respective products for a period of 12 weeks. Statistical analysis was done by two-factor mixed analysis of variance (two-way ANOVA). Results Significant results were found for social interaction (F1,116 = 14.13, p = 0.0002), anxiety (F1,116 = 5.99, p = 0.016), psychomotor agitation (F1,116 = 9.22, p = 0.003), number of meals a day (F1,116 = 4.11, p = 0.04), and concentration (F1,48 = 6.75, p = 0.01), the last of which was only significant in mild ASD cases. Regarding safety, it was found that only three children in the treatment group (9.7%) had adverse effects, namely dizziness, insomnia, colic, and weight gain. Conclusion CBD-rich cannabis extract was found to improve one of the diagnostic criteria for ASD (social interaction), as well as features that often co-exist with ASD, and to have few serious adverse effects. The aim of this work was to extract and characterize pectin from lyophilized noni pulp (Morinda citrifolia Linn.) at three different pHs using different extractor agents and to evaluate anti-viral activity. The extraction of high yield (acid extraction) was subjected to sulfation procedure. Fourier transform infrared spectroscopy and proton nuclear magneticresonance analysis were used to calculate the degree of methylation of pectin from acid extraction and its sulfated derivative. The data obtained for the degree of methylation of noni pectin and sulfated pectin were 30.3 and 30.7, respectively. Pectin was subjected to acute oral toxicity testing in mice. The results showed that pectin had a pro-inflammatory effect, and sulfated pectin had an anti-inflammatory effect. Both pectins were also evaluated for herpes simplex virus activity in vitro, and although they successfully inhibited virus replication, the sulfated pectins were cytotoxic. Morinda Linn. ABSTRACT The Verbal Autopsy (VA) is a questionnaire about the circumstances surrounding a death. It was widely used in Brazil to assist in postmortem diagnoses and investigate excess mortality during the Coronavirus Disease 2019 (COVID-19) pandemic. This study aimed to determine the accuracy of investigating acute respiratory distress syndrome (ARDS) using VA. This is a cross-sectional study with prospective data collected from January 2020 to August 2021 at the Death Verification Service of Sao Luis city, Brazil. VA was performed for suspected COVID-19 deaths, and one day of the week was randomly chosen to collect samples from patients without suspected COVID-19. Two swabs were collected after death and subjected to reverse transcription-polymerase chain reaction (RT-PCR) for SARS-CoV-2 detection. Of the 250 cases included, the VA questionnaire identified COVID-19-related ARDS in 67.2% (52.98% were positive for COVID-19). The sensitivity of the VA questionnaire was 0.53 (0.45–0.61), the specificity was 0.75 (0.64–0.84), the positive predictive value was 0.81 (0.72–0.88), and the negative predictive value was 0.44 (0.36–0.53). The VA had a lower-than-expected accuracy for detecting COVID-19 deaths; however, because it is an easily accessible and cost-effective tool, it can be combined with more accurate methods to improve its performance. Abstract Objective: To analyze the therapeutic itineraries of survivors of Covid-19 after hospital discharge. Methodology: Qualitative study based on the method of thematic oral history, conducted with ten individuals who, after hospital discharge from a public reference service in the municipality of João Pessoa-PB, developed post-Covid condition and sought care. Individual semi-structured interviews were conducted and analyzed using the content analysis technique. Results and Discussion: Most participants had to pay for the care received after hospital discharge. This reality indicates that when users need immediate health assistance and do not find viable alternatives through the SUS (Brazil's public health system), they are forced to seek private healthcare. Conclusion: It is necessary to develop strategies that guarantee adequate access for individuals in the post-Covid condition to rehabilitation and other healthcare services within the SUS.

Resumo Objetivo: Analisar os itinerários terapêuticos de sobreviventes da Covid-19 pós-alta hospitalar. Metodologia: Estudo qualitativo pautado no método da história oral temática, realizado com dez indivíduos que pós-alta hospitalar de um serviço público de referência no município de João Pessoa-PB, desenvolveram a condição pós-Covid e buscaram cuidados. Foram realizadas entrevistas semiestruturadas individuais, analisadas pela técnica da análise de conteúdo. Resultados e Discussão: Após a alta hospitalar, a maioria dos participantes tiveram que custear os cuidados recebidos. Essa realidade denota que, quando os usuários necessitam de assistência imediata à saúde e não encontram alternativas viáveis pelo SUS, são coagidos a buscar a rede privada. Conclusão: Faz-se necessário desenvolver estratégias que garantam o acesso adequado de indivíduos na condição pós-Covid à reabilitação e a outros cuidados de saúde no âmbito do SUS.

RESUMO: Objetivo: Elaborar e validar instrumento de consulta de enfermagem à pessoa com pé diabético na atenção primária à saúde, fundamentado no modelo teórico das Necessidades Humanas Básicas de Wanda de Aguiar Horta. Método: Estudo metodológico, realizado em outubro de 2022 a junho de 2023, em Montes Claros - Minas Gerais - Brasil, em três etapas: revisão de escopo para identificação das características sociodemográficas, clínicas e necessidades psicobiológicas, psicossociais e psicoespirituais; elaboração do instrumento e validação de conteúdo por experts. Para análise dos dados, utilizou-se estatística descritiva e o Índice de Validade de Conteúdo igual ou superior a 0,80. Resultados: Validação de conteúdo realizada por 15 experts. Foram avaliados três itens do instrumento, obtendo-se o Índice de Validade de Conteúdo de 0,93 em objetivos, 0,97 na estrutura e apresentação e 1,00 na relevância. Conclusão: O instrumento contribuirá para qualificação da consulta de enfermagem à pessoa com pé diabético na atenção primária à saúde. ABSTRACT Objective: Develop and validate a nursing consultation instrument for people with diabetic foot in primary health care, based on Wanda de Aguiar Horta’s theoretical model of Basic Human Needs. Method: Methodological study, conducted from October 2022 to June 2023 in Montes Claros - Minas Gerais - Brazil, in three stages: scoping review to identify sociodemographic, clinical characteristics, and psychobiological, psychosocial, and psycho-spiritual needs; development of the instrument and content validation by experts. Descriptive statistics and a Content Validity Index equal to or greater than 0.80 were used for data analysis. Results: Content validation was performed by 15 experts. RESUMO JUSTIFICATIVA E OBJETIVOS: A pesquisa se origina da escassez de dados oficiais sobre a população em situação de rua e das desafiadoras realidades enfrentadas por esse grupo, como a invisibilidade social e o acesso precário aos serviços de saúde. O objetivo principal foi conduzir uma revisão narrativa para compreender a interconexão entre a dor crônica (DC), a depressão e distúrbios do sono nessa população, identificando fatores-chave que desempenham um papel nessas condições. CONTEÚDO: O estudo empregou uma revisão narrativa qualitativa, realizando uma busca abrangente nas bases de dados Web of Science e Scopus sem restrições temporais. Utilizou-se descritores em inglês relacionados à situação de rua, depressão e distúrbios do sono. Mais de 100 milhões de pessoas estão em situação de rua globalmente, sendo o Brasil especialmente impactado por esse fenômeno devido à crise econômica. A prevalência de DC nesse grupo está vinculada às condições precárias de vida, discriminação e exposição à violência. O prejuízo da qualidade do sono, a depressão e a relação bidirecional entre DC e transtornos depressivos são destacadas. A vulnerabilidade social, a falta de acesso a recursos de saúde e as adversidades das condições de vida contribuem para a complexidade dos desafios enfrentados por essa população em situação de rua, impactando negativamente na saúde geral. CONCLUSÃO: Pessoas em situação de rua enfrentam maior vulnerabilidade a lesões, depressão e distúrbios do sono. A relação entre dor e depressão é bidirecional, agravada pelas adversidades vivenciadas. ABSTRACT BACKGROUND AND OBJECTIVES: This research stems from the scarcity of official data regarding the homeless population and the challenging realities faced by this group, such as social invisibility and precarious access to healthcare services. The main objective is to conduct a narrative review to understand the interconnection between chronic pain (CP), depression, and sleep disorders in this population, identifying key factors that play a role in these conditions. CONTENTS: The study employs a qualitative narrative review, conducting a comprehensive search in the Web of Science and Scopus databases without temporal restrictions. It utilizes English descriptors related to homelessness, depression, and sleep disorders. Globally, over 100 million people are homeless, with Brazil particularly affected by this phenomenon due to the economic crisis. The prevalence of CP in this group is linked to precarious living conditions, discrimination, and exposure to violence. The impairment of sleep quality, depression, and the bidirectional relationship between CP and depressive disorders are highlighted. Social vulnerability, lack of access to health resources, and the adversities of living conditions contribute to the complexity of challenges faced by the homeless population, negatively impacting overall health. CONCLUSION: Individuals experiencing homelessness face increased vulnerability to injuries, CP, depression, and sleep disorders. Resumo Antecedentes Encefalites autoimunes (EAIs) são um grupo de doenças inflamatórias caracterizadas pela presença de anticorpos contra antígenos neuronais e gliais, que ocasionam sintomas psiquiátricos subagudos, queixas de memória e distúrbios anormais do movimento. A maioria dos pacientes é jovem, e o atraso no tratamento está associado a pior prognóstico. Objetivo Com o apoio da Academia Brasileira de Neurologia (ABN) e da Sociedade Brasileira de Neurologia Infantil (SBNI), desenvolvemos um consenso sobre o diagnóstico e o tratamento da EAIs no Brasil utilizando a metodologia Delphi. Métodos Um total de 25 especialistas, incluindo neurologistas e neurologistas infantis, foram convidados a participar. Resultados Os especialistas concordaram que os pacientes com critérios de possíveis EAIs devem ser submetidos ao rastreio de anticorpos antineuronais no soro e no líquido cefalorraquidiano (LCR) por meio das técnicas de ensaio baseado em tecidos (tissue-based assay, TBA, em inglês) e ensaio baseado em células (cell-based assay, CBA, em inglês). As crianças também devem ser submetidas ao rastreio de de anticorpo contra a glicoproteína da mielina de oligodendrócitos (anti-myelin oligodendrocyte glycoprotein, anti-MOG, em inglês). O tratamento deve ser iniciado dentro das primeiras 4 semanas dos sintomas, sendo as opções de primeira linha metilprednisolona combinada com imunoglobulina intravenosa (IGIV) ou plasmaférese. O tratamento de segunda linha inclui rituximabe e ciclofosfamida. Bortezomib e tocilizumab são opções de tratamento de terceira linha. A maioria das crises epilépticas nas EAIs são sintomáticas, e os fármacos anticrise podem ser desmamadas após a fase aguda. Em relação à encefalite antirreceptor de N-metil-D-aspartato (anti-N-methyl-D-aspartate receptor, anti-NMDAR, em inglês), os especialistas concordaram que agentes imunossupressores orais não devem ser usados. Os pacientes devem ser avaliados na fase aguda e pós-aguda mediante escalas funcionais e cognitivas, como Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Modified Rankin Scale (mRS), e Clinical Assessment Scale in Autoimmune Encephalitis (CASE). Conclusão Esta pesquisa oferece evidências tangíveis do manejo efetivo de pacientes com EAIs no sistema de saúde Brasileiro. Abstract Background Autoimmune encephalitis (AIE) is a group of inflammatory diseases characterized by the presence of antibodies against neuronal and glial antigens, leading to subacute psychiatric symptoms, memory complaints, and movement disorders. The patients are predominantly young, and delays in treatment are associated with worse prognosis. Objective With the support of the Brazilian Academy of Neurology (Academia Brasileira de Neurologia, ABN) and the Brazilian Society of Child Neurology (Sociedade Brasileira de Neurologia Infantil, SBNI), a consensus on the diagnosis and treatment of AIE in Brazil was developed using the Delphi method. Methods A total of 25 panelists, including adult and child neurologists, participated in the study. Results The panelists agreed that patients fulfilling criteria for possible AIE should be screened for antineuronal antibodies in the serum and cerebrospinal fluid (CSF) using the tissue-based assay (TBA) and cell-based assay (CBA) techniques. Children should also be screened for anti-myelin oligodendrocyte glucoprotein antibodies (anti-MOG). Treatment should be started within the first 4 weeks of symptoms. The first-line option is methylprednisolone plus intravenous immunoglobulin (IVIG) or plasmapheresis, the second-line includes rituximab and/or cyclophosphamide, while third-line treatment options are bortezomib and tocilizumab. Most seizures in AIE are symptomatic, and antiseizure medications may be weaned after the acute stage. In anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis, the panelists have agreed that oral immunosuppressant agents should not be used. Patients should be evaluated at the acute and postacute stages using functional and cognitive scales, such as the Mini-Mental State Examination (MMSE), the Montreal Cognitive Assessment (MoCA), the Modified Rankin Scale (mRS), and the Clinical Assessment Scale in Autoimmune Encephalitis (CASE). Conclusion The present study provides tangible evidence for the effective management of AIE patients within the Brazilian healthcare system.
