Many studies suggest that social capital, defined as those intangible resources of a society or community (trust, participation and reciprocity), that might facilitate collective action, can be associated with positive health effects. Aim: To explore the relationship between social capital an the level of mental health, in urban communities of Santiago, Chile. Material and methods: In a qualitative-quantitative cross-sectional design, two low income neighborhoods in the municipality of Puente Alto were selected. Interviews to key agents and focus groups, as well as surveys (407) to adults from a representative random sample of households, were conducted, measuring social capital, using a locally devised questionnaire and mental health using the General Health Questionnaire (GHQ-12 instrument). A qualitative analysis based on the «grounded theory» and a quantitative analysis through correlations and simple and logistic regression models were applied. Results: The quantitative analysis found an association between female gender, education and having a chronic disease, with low levels of mental health. At the same time, the trust component of social capital might be associated with a better mental health status. Qualitatively, all the components of social capital were identified as important for a better mental health. Conclusions: This study suggests the existence of a positive relationship between social capital and mental health. Developing trust in a community might be a useful tool to work in mental health at the community level

OBJECTIVE: To investigate the relation between decentralization and equity of resource allocation in Colombia and Chile. METHODS: The "decision space" approach and analysis of expenditures and utilization rates were used to provide a comparative analysis of decentralization of the health systems of Colombia and Chile. FINDINGS: Evidence from Colombia and Chile suggests that decentralization, under certain conditions and with some specific policy mechanisms, can improve equity of resource allocation. In these countries, equitable levels of per capita financial allocations at the municipal level were achieved through different forms of decentralization - the use of allocation formulae, adequate local funding choices and horizontal equity funds. Findings on equity of utilization of services were less consistent, but they did show that increased levels of funding were associated with increased utilization. This suggests that improved equity of funding over time might reduce inequities of service utilization. CONCLUSION: Decentralization can contribute to, or at least maintain, equitable allocation of health resources among municipalities of different incomes.

OBJETIVO: Investigar la relación entre la descentralización y la equidad de la distribución de los recursos en Colombia y Chile. MÉTODOS: Se utilizaron el criterio del "espacio decisional" y el análisis de los gastos y las tasas de utilización para realizar un análisis comparativo de la descentralización de los sistemas de salud en Colombia y Chile. RESULTADOS: La evidencia obtenida en estos dos países indica que la descentralización, siempre que se haga en determinadas condiciones y acompañada de algunos mecanismos de política específicos, puede propiciar una distribución más equitativa de los recursos. En los dos países se consiguió que las asignaciones financieras per cápita a nivel municipal alcanzaran niveles equitativos gracias a distintas formas de descentralización: uso de fórmulas de asignación, opciones idóneas de financiación local y fondos de equidad horizontal. Los resultados relativos a la equidad de la utilización de los servicios fueron menos coherentes, pero sí mostraron que el aumento del financiamiento se asociaba a una mayor utilización. Esto indica que un financiamiento más equitativo permitiría, con el tiempo, reducir las desigualdades en materia de utilización de los servicios. CONCLUSIÓN: La descentralización puede propiciar, o como mínimo preservar, una distribución equitativa de los recursos de salud entre municipios con distintos ingresos.

OBJECTIF: Etudier le lien entre la décentralisation et le degré d'équité dans la répartition des ressources au Chili et en Colombie. MÉTHODES: On a fait une analyse comparée de la décentralisation des systèmes de santé au Chili et en Colombie selon la méthode de la "marge de décision" et en mesurant l'équité d'après les dépenses et le taux d'utilisation. RÉSULTATS: Les données recueillies au Chili et en Colombie incitent à penser que, dans certaines conditions et grâce à certains mécanismes de décision, la décentralisation peut déboucher sur une répartition plus équitable des ressources. Dans ces pays, différentes formes de décentralisation (formules d'allocation des ressources, choix de financement judicieux au plan local et fonds horizontaux de redistribution) ont conduit à une répartition équitable des crédits par habitant au niveau municipal. Pour ce qui est de l'équité dans l'utilisation des services, les résultats sont moins homogènes mais montrent que le taux d'utilisation augmente avec le financement, ce qui semble indiquer qu'avec le temps, un financement plus équitable peut réduire les inégalités eu égard à l'utilisation des services. CONCLUSION: La décentralisation peut contribuer à une répartition équitable des ressources de santé entre des municipalités qui n'ont pas les mêmes revenus, ou tout au moins la maintenir.

Decentralization is often a major part of health reform policies. However, there have been few attempts to comparatively study the degree of decentralization and the effects of decentralization on equity of allocations to health, so we do not know how best to implement this reform. This article uses an innovative comparative analysis of the "decision space" that was allowed to local municipalities in the health reforms of Bolivia and Chile, two countries that have had several years of experience in implementing decentralization. The studies found that relatively little decision space was allowed to local authorities over key functions of health care systems. The studies also found that central authorities often reduce the decision space in order to direct more resources to health or to restrict local choice over human resources issues. The studies found that more equitable allocations of health funding were achieved through a common equalization fund for the municipalities in Chile and by forcing the assignment to health of a specific percentage of the central government transfers to municipalities in Bolivia.

La descentralización constituye a menudo una parte importante de las políticas de reforma del sector de la salud. Sin embargo, ha habido pocos intentos de investigar comparativamente el grado de descentralización y los efectos de esta sobre la equidad de las asignaciones a la salud, de modo que desconocemos la mejor manera de poner en práctica esta reforma. En este estudio se utiliza un innovador análisis comparativo del "espacio de decisión" que se les concedió a las municipalidades en las reformas de la salud en Bolivia y Chile, dos países que han tenido varios años de experiencia en la puesta en práctica de la descentralización. El estudio reveló que a las autoridades locales se les concedió un espacio de decisión relativamente pequeño en funciones clave de los sistemas de salud. Asimismo, se verificó que las autoridades centrales a menudo reducen el espacio de decisión para dirigir más recursos a la salud o para restringir la elección local en cuestiones de recursos humanos. También se observó que se consiguieron asignaciones más equitativas de los fondos destinados a la salud, en Chile mediante un fondo común de igualación para las municipalidades y en Bolivia forzando la asignación a la salud de un porcentaje específico de las transferencias del gobierno central a las municipalidades.
