Resumo O presente estudo teve como objetivo comparar os fatores associados à multimorbidade em idosos de 60 a 69 anos, em 2010 e 2021. Trata-se de um estudo transversal comparativo, composto por dois estudos transversais. Ambas as coletas de dados foram realizadas de forma individual por entrevistadores treinados e deu-se por inquérito domiciliar, no município de Coxilha-RS, Brasil. Utilizou-se para análise inferencial bivariada, exato de Fisher e qui-quadrado, e multivariada o teste de regressão de Poisson com variância robusta com nível de significância de p 0,05. Detectou-se que a prevalência de multimorbidade teve uma diminuição significativa, passando de 66,5% em 2010, para 41,6% em 2021. Ser dependente para atividades básicas e instrumentais da vida diária associou-se a maior prevalência para a multimorbidade no ano de 2010, contudo no ano de 2021 o ser dependente para atividades instrumentais, não saber ler/escrever e não trabalhar apresentou maior prevalência para a multimorbidade. Abstract The present study aimed to compare the factors associated with multimorbidity in older adults aged 60 to 69 years, in 2010 and 2021. This is a comparative cross-sectional study, comprised of other two cross-sectional studies. Both data collections were individually conducted by trained interviewers through household surveys in the municipality of Coxilha-RS, Brazil. Bivariate inferential analysis was conducted using Fisher's exact test and chi-square test, while multivariate analysis employed Poisson regression with robust variance with a significance level of p 0.05. It was observed that the prevalence of multimorbidity significantly decreased, decreasing from 66.5% in 2010 to 41.6% in 2021. Dependency for basic and instrumental activities of daily living was associated with higher prevalence of multimorbidity in the year 2010. However, in 2021, being dependent on instrumental activities, being unable to read/write, and being unemployed showed higher prevalence for multimorbidity. Resumo Objetivo Verificar a prevalência e os fatores associados à polifarmácia excessiva em pessoas idosas institucionalizadas. Método Estudo transversal com 478 pessoas idosas residentes em instituições de longa permanência para idosos. A variável dependente foi polifarmácia excessiva, definida como o uso concomitante de dez ou mais medicamentos. As variáveis independentes incluíram informações sociodemográficas e de saúde. Utilizou-se a regressão de Poisson com variância robusta para analisar o efeito das variáveis independentes em relação ao desfecho. Resultados A prevalência de polifarmácia excessiva foi de 29,3%, associada à cardiopatia (RP=1,40; IC95% 1,03-1,91), diabetes mellitus (RP=1,52; IC95% 1,15-2,01), depressão (RP=1,42; IC95% 1,08-1,87), internação hospitalar no último ano (RP=1,36; IC95% 1,02-1,80) e ao uso de medicamento potencialmente inapropriado para idosos (RP=2,13; IC95% 1,60-2,83). Conclusão A polifarmácia excessiva foi frequente entre pessoas idosas institucionalizadas. Os resultados sugerem que as doenças prevalentes entre pessoas idosas, a hospitalização e o uso de medicamentos potencialmente inapropriados são fatores para o uso de polifarmácia excessiva nessa população. Esses achados podem instruir ações com vistas à otimização da farmacoterapia prescrita às pessoas idosas.

Abstract Objective To verify the prevalence and factors associated with excessive polypharmacy in institutionalized older people. Method Cross-sectional study with 478 older people living in long-term care facilities. The dependent variable was excessive polypharmacy which is defined as the concomitant use of ten or more medications. The independent variables included sociodemographic and health information. The Poisson regression with robust variance was used to analyze the effect of the independent variables compared to the outcome. Results The prevalence of excessive polypharmacy was 29.3% associated with heart disease (PR=1.40; 95%CI 1.03-1.91), diabetes mellitus (PR=1.52; 95%CI 1.15- 2.01), depression (PR=1.42; 95%CI 1.08-1.87), hospitalization in the last year (PR=1.36; 95%CI 1.02-1.80), and the use of potentially inappropriate medication for older people (PR=2.13; 95%CI 1.60-2.83). Conclusion Excessive polypharmacy was frequent among institutionalized older people. The results suggest that prevalent diseases among older people, hospitalization, and the use of potentially inappropriate medications are reasons for the use of excessive polypharmacy by this population. Said findings can guide actions aimed at optimizing the pharmacotherapy prescribed to older people.

RESUMO Os anticoagulantes orais usados no pós-operatório de artroplastia de quadril para prevenção de eventos tromboembólicos geram dúvidas a respeito da efetividade tromboprofilática e da redução de riscos hemorrágicos. Para isso, esta revisão sistemática tem como objetivo avaliar o uso de anticoagulantes orais para prevenção de eventos tromboembólicos no pós-operatório de pacientes submetidos a artroplastia de quadril. Os métodos usados foram pesquisas nas bases de dados indexadas do PubMed, BVS e periódicos da Capes de setembro de 2015 a junho de 2016, dos últimos dez anos, completos, livres e nos idiomas inglês e português. Os resultados apresentaram alguns casos de embolia pulmonar, trombose venosa profunda e sangramentos; apesar disso, os NACOs foram considerados, pelos estudos citados, eficazes na prevenção de eventos tromboembólicos. Os três medicamentos estudados mostraram-se importantes na prevenção de eventos tromboembólicos, mas os melhores resultados profiláticos foram obtidos com Rivaroxaban 10 mg, uma vez ao dia, com duração entre 30 e 35 dias com anticoagulantes orais e 28 a 42 dias com antiagregante plaquetário.

ABSTRACT Oral anticoagulants are being used in the postoperative period of hip arthroplasty to prevent of thromboembolic events, create doubts as to the effectiveness of thromboprophylaxis and reduce the risk of hemorrhage. This systematic revision is aimed at evaluating the use of oral anticoagulants in the prevention of thromboembolic events in the postoperative period of patients undergoing hip arthroplasty. Research with descriptors found on PubMed, BVS, and the CAPES portal for medical journal publications from September 2015 to June 2016, from the last ten years (2005-2015), complete, free, and written in Portuguese and in English were the methods used. The results of the studies showed some cases of pulmonary embolism, deep vein thrombosis, and bleeding; even then, the NOACs were effective in preventing thromboembolic events. There is no consensus regarding the prophylaxis method for these events, which is why the challenge is to obtain high levels of prevention while minimizing the adverse effects. The most studied oral anticoagulant was rivaroxaban (67%). The three drugs that were studied have shown to be effective in preventing thromboembolic events, but the best results were obtained with rivaroxaban 10 mg, one tablet daily; treatment duration ranged from 30 to 35 days with oral anticoagulants and from 28 to 42 days with anti-platelet drugs.

Resumo Objetivo: verificar a associação entre multimorbidade em idosos e variáveis sociodemográficas, autopercepção de saúde e polifarmácia. Método: Estudo de corte transversal, para a coleta de dados utilizou-se o questionário Saúde, Bem-estar e Envelhecimento. A amostra foi constituída de 676 indivíduos com 60 anos de idade ou mais residentes em municípios de pequeno porte do norte do estado do Rio Grande do Sul, Brasil. A variável dependente foi a multimorbidade, isto é, a ocorrência de duas ou mais doenças crônicas não transmissíveis no mesmo indivíduo. As variáveis independentes foram as características demográficas, socioeconômicas e relativas à saúde. O modelo de regressão de Poisson bruta e ajustada foi utilizado para analisar o efeito das variáveis independentes em relação ao desfecho e p foi considerado significativo quando <0,05. Resultados: Dos idosos entrevistados, 45% apresentam multimorbidade, 51,1% relatam autopercepção de saúde ruim/muito ruim e 37,1% faz uso de polifarmácia. Após análise ajustada à ocorrência de multimorbidade, apresentou-se associação às variáveis: percepção de saúde negativa. RP=1,15; (IC95%; 1,09 - 1,22) e uso de polifarmácia RP=1,29; (IC95%; 1,22 - 1,35). Conclusão: A multimorbidade pode interferir negativamente na autopercepção de saúde dos idosos contribuindo para o aumento do consumo de medicamentos.

Abstract Objective: to verify the association between the multimorbidity of the elderly and sociodemographic variables, self-perception of health and polypharmacy. Method: a cross-sectional study was performed. The research data was collected using the Health, Well-Being and Aging questionnaire. The sample was composed of 676 people aged 60 years or more, who were residents of small towns in the north of the state of Rio Grande do Sul, Brazil. The dependent variable was multimorbidity, that is, the occurrence of two or more chronic non-communicable diseases in the same person. The independent variables were demographic, socioeconomic and health-related characteristics. Poisson’s raw and robust regression model was used to analyze the effect of the independent variables in relation to the outcome and p was considered significant when <0.05. Result: among the elderly interviewed, 45% presented multimorbidity, 51.1% reported a self-perception of poor/very poor health and 37.1% used polypharmacy. After the analysis was adjusted to the occurrence of multimorbidity, association with the following variables was found: health perception (regular/poor/very poor) PR=1.15 (CI95%; 1.09 - 1.22) and use of polypharmacy PR=1.29 (CI95%; 1.22 - 1.35). Conclusion: Multimorbidity may interfere negatively in the self-perception of health of the elderly contributing to increased medicine consumption.
