ABSTRACT Introduction: The analysis of patients submitted to heart surgery at three assessment times has been insufficiently described in the literature. Objective: To analyze chest expansion, maximum inspiratory pressure (MIP), maximum expiratory pressure (MEP), distance traveled on the six-minute walk test (6MWT), and quality of life in the preoperative period, fourth postoperative day (4th PO), and 30th day after hospital discharge (30th-day HD) in individuals submitted to elective heart surgery. Methods: A descriptive, analytical, cross-sectional study was conducted with 15 individuals submitted to elective heart surgery between 2016 and 2020 who did not undergo any type of physiotherapeutic intervention in Phase II of cardiac rehabilitation. The outcome variables were difference in chest expansion (axillary, nipple, and xiphoid), MIP, MEP, distance on 6MWT, and quality of life. The assessment times were preoperative period, 4th PO, and 30th-day HD. Results: Chest expansion diminished between the preoperative period and 4th PO, followed by an increase at 30th-day HD. MIP, MEP, and distance traveled on the 6MWT diminished between the preoperative period and 4th PO, with a return to preoperative values at 30th-day HD. General quality of life improved between the preoperative period and 4th PO and 30th-day HD. An improvement was found in the social domain between the preoperative period and the 30th-day HD. Conclusion: Heart surgery causes immediate physical deficit, but physical functioning can be recovered 30 days after hospital discharge, resulting in an improvement in quality of life one month after surgery. OBJECTIVES: To identify the clinical discriminative value and determinants of arterial stiffness in individuals with type 2 diabetes mellitus (T2DM). METHODS: This prospective cohort study included 51 individuals (53.57±9.35 years) diagnosed with T2DM (stage glucose≥126 mg/dL; diagnostic time: 87.4±69.8 months). All participants underwent an initial evaluation of personal habits, medications, and history; arterial stiffness assessment by carotid-femoral pulse wave velocity (cfPWV) using SphygmoCor; and blood laboratory analysis. A statistical analysis was performed using SPSS software, and values of p≤0.05 were considered significant. RESULTS: A cut-off cfPWV value of 7.9 m/s was identified for T2DM [Sensitivity (SE): 90% and Specificity (SP): 80%]. A subgroup analysis revealed higher glycated hemoglobin (Hb1Ac) (p=0.006), obesity (p=0.036), and dyslipidemia (p=0.013) than those with cfPWV ≥7.9 m/s. Multivariate analysis identified higher stage glucose (p=0.04), Hb1Ac (p=0.04), hypertension (p=0.001), and dyslipidemia (p=0.01) as determinant factors of cfPWV; positive and significant correlation between cfPWV and glucose (r=0.62; p=0.0003) and Hb1Ac (r=0.55; p=0.0031). CONCLUSIONS: In T2DM, an indicator of the discriminative value of arterial stiffness was cfPWV of 7.9 m/s. Clinical findings and comorbidities, such as hypertension, glucose, poor glycemic control, and dyslipidemia, were associated with and were determinants of arterial stiffness in T2DM. Reinforcement of monitoring risk factors, such as hypertension, dyslipidemia, and glycemic control, seems to be essential to the process of arterial stiffening. Confirmation of this discriminative value in larger populations is recommended.

RESUMO O teste exercício cardiopulmonar (CPX) é uma metodologia não invasiva de avaliação global da integridade dos ajustes fisiológicos durante o exercício físico. Como alternativa, a avaliação da capacidade funcional foi realizada por meio de fórmulas preditivas. No entanto, esse método de avaliação pode ter resultados diferentes quando usado em uma população de homens saudáveis e pacientes com insuficiência cardíaca (IC) crônica. Compararam-se os valores de consumo de oxigênio (VO2) obtidos e estimados durante o CPX no limiar anaeróbio ventilatório (LAV) e no pico do exercício para homens saudáveis e pacientes com IC crônica. Cinquenta e seis homens foram divididos em 3 grupos: (1) 18 eram jovens saudáveis (GJ) (27±6,01 anos); (2) 14 eram idosos saudáveis (GE) (61±6,3 anos); e, (3) 24 com IC crônica (HFG) (53±13,6 anos). Todos foram submetidos ao CPX em cicloergômetro para determinação do VO2 no LAV e no pico do exercício. Posteriormente, a estimativa do VO2 foi realizada na potência do LA e no pico do exercício por meio de uma fórmula de predição para exercício físico em cicloergômetro. Os valores de VO2 obtidos e a carga estimada foram comparados. O VO2 estimado foi significativamente maior que o VO2 obtido no GE e no HFG (16,9±1,8 vs. 13,1±2,1mL/kg/min e 12±6,9 vs. 8,7±2,5mL/kg/min, respectivamente). Por fim, não houve diferença nos valores de VO2 estimados e obtidos para o GJ (22,6±5,5 vs. 23,1±8,7mL/kg/min, respectivamente). A fórmula de predição superestimou os valores de VO2 para idosos e pacientes com IC crônica. Porém, no GJ os valores de VO2 se mostraram semelhantes para a fórmula de predição e o obtido durante o CPX em cicloergômetro.

ABSTRACT Cardiopulmonary exercise testing (CPX) is a noninvasive method for assessing physiological changes during physical exercise. Functional capacity has been evaluated using prediction equations. However, this evaluation method may yield different outcomes when applied to a healthy male population and patients with chronic heart failure (HF). This study aimed to compare the estimated and obtained values of oxygen consumption (VO2) during CPX both at the ventilatory anaerobic threshold (VAT) and at peak exercise for healthy men and HF patients. For that, 56 men were divided into 3 groups: (1) 18 young and healthy (YG) (27±6.01 years); (2) 14 healthy older adults (OG) (61±6.3 years); and (3) 24 chronic HF patients (HFG) (53±13.6 years). CPX in cycle ergometer was administered to all individuals for determining VO2 at the VAT and peak exercise. Then, VO2 was estimated at the two moments using a prediction equation, and estimated values were compared to those obtained. Estimated VO2 was significantly higher than obtained VO2 in OG (16.9±1.8 vs. 13.1±2.1mL/kg/min) and HFG (12±6.9 vs. 8.7±2.5mL/kg/min). We found no difference between estimated and obtained VO2 for the YG (22,6±5,5 vs. 23,1±8,7mL/kg/min). The prediction equation overestimated VO2 values for older adults and HF patients. However, the YG obtained similar values than those estimated.

RESUMEN La prueba de ejercicio cardiopulmonar (CPX) es una metodología no invasiva para evaluar la integridad global de los ajustes fisiológicos durante la práctica de ejercicio físico. Se realizó como alternativa la evaluación de la capacidad funcional por medio de fórmulas predictivas. Sin embargo, este método de evaluación puede presentar resultados distintos cuando se utiliza en una población de hombres sanos y de pacientes con insuficiencia cardíaca crónica (IC). Se compararon los valores de consumo de oxígeno (VO2) obtenido y estimado durante la CPX en el umbral anaeróbico ventilatorio (LAV) y en la cima del ejercicio para hombres sanos y pacientes con IC crónica. Se dividieron a 56 hombres en 3 grupos: i) 18 eran jóvenes sanos (GJ) (27±6,01 años); ii) 14 eran ancianos sanos (GE) (61±6,3 años); iii) 24 tenían IC crónica (HFG) (53±13,6 años). Todos se sometieron a CPX en un cicloergómetro para determinar el VO2 en LAV y en la cima del ejercicio. Posteriormente, la estimación de VO2 se realizó sobre la potencia de LA y en la cima del ejercicio mediante una fórmula predictiva del ejercicio físico en un cicloergómetro. Los valores de VO2 obtenidos y la carga estimada se compararon. El VO2 estimado fue significativamente mayor que el VO2 obtenido en GE y HFG (16,9±1,8 vs. 13,1±2,1ml/kg/min y 12±6,9 vs. 8,7±2,5ml/kg/min, respectivamente). No hubo diferencias en los valores de VO2 estimado y obtenido para el GJ (22,6±5,5 vs. 23,1±8,7ml/kg/min, respectivamente). La fórmula predictiva sobrestimó los valores de VO2 para ancianos y pacientes con IC crónica. Sin embargo, en GJ los valores de VO2 fueron similares para la fórmula predictiva y la obtenida durante el CPX en cicloergómetro.

Abstract Aim: to evaluate the acute effects of expiratory positive airway pressure on cardiac autonomic modulation in chronic obstructive pulmonary disease patients during spontaneous breathing and slow deep breathing. Methods: 17 patients were evaluated. The R-R intervals were collected (Polar® S810i) during spontaneous breathing (10 minutes) and slow deep breathing (4 minutes), with and without 5 cmH2O expiratory positive airway pressure. Stable signals were analyzed by Kubios®. Heart rate variability indices were computed in time domain and in frequency domain. Results: Expiratory positive airway pressure application affected low frequency (spontaneous breathing: 62.5±4.1 vs slow deep breathing: 28.2±4.2, p<0.001) and high frequency (spontaneous breathing: 37.4±17.3 vs slow deep breathing: 58.9±18.1, p<0.001). Interactions were observed between expiratory positive airway pressure effect and slow deep breathing effect for low frequency (p<0.001), high frequency (p<0.001) and low frequency/high frequency ratio (p<0.001). When patients were stratified by disease’s severity, we identified a significant low frequency reduction (p<0.001) and high frequency increase (p<0.001) for all stages when slow deep breathing was associated with expiratory positive airway pressure. Conclusion: A 5 cmH2O expiratory positive airway pressure during spontaneous and slow deep breathing can elicit an acute response, resulting in a cardiac autonomic control improvement in moderate-to-very severe patients.

RESUMO Objetivo Estabelecer ponto de corte entre as variáveis clínicas e funcionais para avaliar a prevalência de sarcopenia e dinapenia em pacientes com Doença Pulmonar Obstrutiva Crônica (DPOC), além de analisar o impacto da Disfunção Muscular Esquelética (DME) nestas variáveis. Métodos Realizado estudo transversal com pacientes diagnosticados com DPOC para determinar sarcopenia ou dinapenia por meio do índice de baixa massa muscular e Força de Preensão Palmar (FPP). Avaliadas a função pulmonar, força muscular respiratória e capacidade funcional (CF). A precisão das variáveis na determinação dos pontos de corte previstos para as doenças em questão foi obtida a partir da curva Receiver Operating Characteristic (ROC) e de uma análise bidirecional da variância. Resultados Ao final da análise, obtiveram-se 20 pacientes com DPOC estratificados por sarcopenia (n = 11) e dinapenia (n = 07). O grupo com sarcopenia apresentou menor massa magra e menor pressão inspiratória máxima (PImáx), diminuição da Força de Preensão Palmar (FPP) e redução da CF (p<0,050). O grupo com dinapenia apresentou redução da PImáx, menor FPP e menor distância percorrida no teste Incremental Shuttle Walk (ISWT) (p<0,050). Foram encontrados pontos de corte no Volume Expiratório Forçado (VEF1) durante o primeiro segundo na PImáx, na Pressão Expiratória Máxima (PEmáx) e no ISWT, o que possibilitou identificar sarcopenia ou dinapenia nestes pacientes. A partir destes resultados, foi possível encontrar coexistência das condições (efeito DME) na DPOC: redução da distância no ISWT (p = 0,002) e % ISWT (p = 0,017). Conclusões Em pacientes com DPOC moderada a muito grave, a sarcopenia pode ser prevista pelo VEF1 (% previsto) <52, PImáx <73 cm H2O, PEmáx <126 cm H2O e distância percorrida de <295 metros no ISWT. Já a dinapenia pode ser prevista pelo VEF1 <40%, PImáx <71 cm H2O, PEmáx <110 cm H2O e distância de <230 metros percorrida no ISWT.

ABSTRACT Objective To establish a cut-off point for clinical and functional variables to determinate sarcopenia and dynapenia in COPD patients, and to analyze the impact of skeletal muscle dysfunction (SMD) on these variables. Methods Cross-sectional study, screened COPD patients for sarcopenia or dynapenia through low muscle mass and hand grip strength (HGS). Clinical variables: pulmonary function, respiratory muscle strength and functional capacity (FC). The precision of the variables in determining points of predictive cut-off for sarcopenia or dynapenia were performed using the Receiver Operating Characteristic curve and two-way analysis of variance. Results 20 COPD patients stratified for sarcopenia (n = 11) and dynapenia (n = 07). Sarcopenia group presented lower lean mass and lower maximal inspiratory pressure (MIP), decreased HGS, reduced FC (p<0.050). Dynapenia group presented reduced MIP, lower HGS and walked a shorter distance at Incremental shuttle walk test (ISWT) (p<0.050). We found cut-off points of forced expiratory volume in one second (FEV1), MIP and maximal expiratory pressure (MEP) and ISWT. It is possible to identify sarcopenia or dynapenia in these patients. We found the coexistence of the conditions (SMD effect) in COPD – reduction in the distance in the ISWT (p = 0.002) and %ISWT (p = 0.017). Conclusion In moderate to very severe COPD patients the sarcopenia could be predicted by FEV1 (%predicted) < 52, MIP < 73 cmH2O, MEP < 126 cmH2O and distance traveled of < 295 m in ISWT. Whereas dynapenia could be predicted by FEV1 < 40%, MIP < 71 cmH2O, MEP < 110 cmH2O and distance of < 230 m traveled in ISWT.

Resumo Contexto A doença pulmonar obstrutiva crônica (DPOC) apresenta uma complexa fisiopatologia e sua compreensão vem se modificando nos últimos anos, com atenção para as manifestações multissistêmicas, em especial a disfunção periférica e sua influência na intolerância ao exercício físico. Objetivo Avaliar o impacto da doença arterial periférica obstrutiva (DAOP) na força muscular periférica e na capacidade de exercício dos portadores de DPOC. Métodos Estudo transversal realizado com 35 portadores de DPOC, que foram avaliados pelo índice tornozelo-braquial, teste de força de preensão palmar (FPP), uma repetição máxima (1RM) de extensores e flexores de joelho, e a distância no incremental shuttle walking test (dISWT). Resultados Portadores de DPOC com DAP coexistente apresentaram menor FPP da mão dominante (33,00 versus 26,66 kgf, p = 0,02) e pior desempenho no dISWT (297,32 versus 219,41 m, p = 0,02) quando comparados aos portadores de DPOC sem DAP. Fortes correlações foram encontradas entre a medida da FPP e a dISWT (r = 0,78; p < 0,001) e a 1RM/extensão de joelho (r = 0,71; p = 0,03); entre a dISWT e a 1RM/extensão de joelho (r = 0,72; p = 0,02) e a 1RM/flexão de joelho (r = 0,92; p < 0,001). O modelo de regressão linear identificou que a variável dISWT explica isoladamente 15,3% do resultado do índice tornozelo braquial (p = 0,01). Conclusão Portadores de DPOC com DAOP coexistente apresentam maior perda da força muscular periférica e pior desempenho da capacidade de exercício quando comparados aos portadores de DPOC sem DAOP.

Abstract Background The pathophysiology of chronic obstructive pulmonary disease (COPD) is complex and understanding of it has been changing in recent years, with regard to its multisystemic manifestations, especially peripheral dysfunction and its influence on intolerance to exercise. Objectives To evaluate the relationship between peripheral arterial occlusive disease (PAOD) and peripheral muscle strength and exercise capacity in COPD patients. Methods We conducted a cross-sectional study of 35 patients with COPD who were evaluated with the Ankle-Brachial Index, handgrip strength test, 1 repetition maximum (1RM) of knee extensors and flexors, and distance covered in the incremental shuttle walking test (dISWT). Results COPD patients with coexisting PAOD had lower dominant handgrip strength test results (33.00 vs. 26.66 kgf, p = 0.02) and worse performance in the dISWT (297.32 vs. 219.41 m, p = 0.02) when compared to the COPD patients without PAOD. Strong correlations were found between the result of the handgrip strength test and both the dISWT (r = 0.78; p < 0.001) and the 1RM/knee extension (r = 0.71; p = 0.03); and also between the dISWT and both the 1RM/knee extension (r = 0.72; p = 0.02) and the 1RM/knee flexion (r = 0.92; p < 0.001). The linear regression model showed that the dISWT variable alone explains 15.3% of the Ankle-Brachial Index result (p = 0.01). Conclusion COPD patients with PAOD exhibit reduced muscle strength and lower exercise capacity than COPD patients without PAOD.

RESUMO A frequência cardíaca sofre variações durante o ciclo respiratório, fenômeno conhecido como arritmia sinusal respiratória. A manobra para acentuação da arritmia sinusal respiratória (M-ASR) consiste em manter ventilação educada com uma frequência respiratória de seis ciclos por minuto com relação tempo inspiração/expiração (TI:TE) de 1:1. Este estudo tem como objetivo avaliar o comportamento do volume minuto, da fração expirada de CO2 (FeCO2 infere sobre PaCO2) e do controle autonômico da frequência cardíaca durante a M-ASR com duração maior do que 90s. Foram avaliados 16 homens jovens saudáveis (de 18 a 25 anos). Todos foram orientados a realizar inspirações e expirações lentas com duração de 10 segundos por ciclo, TI:TE de 1:1 e consequente frequência respiratória de seis incursões por minuto, durante quatro minutos. Durante a avaliação foi coletada a frequência cardíaca (FC) batimento a batimento por meio de um cardiofrequencímetro, o volume minuto (VM) e a FeCO2 através de um ergoespirômetro. Para análise estatística empregou-se ANOVA one-way (com post-hoc de Tukey) ou teste de Kruskal-Wallis (com post-hoc de Dunn) quando conveniente (p<0,05). Durante a M-ASR, a FeCO2, o VM e os índices do domínio do tempo da variabilidade da frequência cardíaca (VFC) não sofreram alterações significativas ao longo do tempo. A realização da M-ASR em jovens saudáveis, por mais de 90 segundos, pode ser executada com segurança, sem o risco de hipocapnia e sem a interferência das alterações de FeCO2 nos índices do domínio do tempo da análise de VFC da M-ASR.

ABSTRACT Heart rate (HR) fluctuate during the respiratory cycle. This phenomenon is known as respiratory sinus arrhythmia. The deep breathing test is to keep a paced breathing in six breathing per minute and I:E relationship 1:1. The purpose of this study is to access minute volume, expired fraction of carbon dioxide (EFCO2) and autonomic control of heart rate during deep breathing test longer than 90 seconds. Sixteen young healthy male (18 - 25 years old) were assessed. The subjects were instructed to perform inspirations and expirations with duration of 10 seconds per cycle, I:E = 1:1, and consequently respiratory rate of 6 cycles per minute, for about four minutes with one minute after and before, totaling six minutes. HR was recorded beat-to-beat using a cardio frequencimeter; MV and EFCO2 was measured and recorded using a mobile ergoespirometer. To analyse statistics differences, ANOVA one way (Tuckey post-hoc) and Kruskall Wallis (Dunn post-hoc) were used (p<0.05). When deep breathing test in course, EFCO2, MV and time domain heart rate variability shows no statistics difference over time. To perform deep breathing test in young healthy male, longer than 90 seconds, can be safety, without risks of hypocapnia and no interference from EFCO2 changes in time domain heart rate variability analysis of M-RSA.

RESUMEN La frecuencia cardíaca sufre oscilaciones durante el ciclo respiratorio, fenómeno conocido como arritmia sinusal respiratoria. La maniobra para acentuación de la arritmia sinusal respiratoria (M-ASR) consiste en mantener ventilación educada con frecuencia respiratoria de seis ciclos por minuto con relación al tiempo inspiración/espiración (TI:TE) de 1:1. En este estudio se propone a evaluar la conducta del volumen minuto, de la fracción espirada de CO2 (FeCO2 infiere sobre el PaCO2) y el control autonómico de la frecuencia cardíaca durante la M-ASR con duración mayor de 90s. Se evaluaron 16 varones jóvenes sanos (de 18 a 25 años de edad). Se les orientaron para que realizasen inspiraciones y espiraciones pausadas de 10 segundos de duración por ciclo, TI:TE de 1:1, y consecuente frecuencia respiratoria de seis incursiones por minuto, durante cuatro minutos. Durante la evaluación se recolectaron la frecuencia cardíaca (FC), latido a latido a través de un monitor de frecuencia cardíaca, el volumen minuto (VM) y la FeCO2 mediante un ergoespirómetro. Para el análisis estadístico se empleó ANOVA one-way (con post-hoc de Tukey) o test de Kruskal-Wallis (con post-hoc de Dunn) cuando necesario (p<0,05). Durante la M-ASR, la FeCO2, el VM y los índices del dominio del tiempo para la variabilidad de la frecuencia cardíaca (VFC) no presentaron alteraciones significativas al largo del tiempo. Puede ejecutarse con seguridad la realización de la M-ASR en jóvenes sanos por más de 90 segundos, sin riesgo de hipocapnia y sin interferencia de las alteraciones de la FeCO2 en los índices del dominio del tiempo para analizar la VFC de la M-ASR.

BACKGROUND: Elastic resistance bands (ERB) combined with proprioceptive neuromuscular facilitation (PNF) are often used in resistance muscle training programs, which have potential effects on peripheral muscle strength. However, the effects of the combination of ERB and PNF on respiratory muscle strength warrant further investigation. OBJECTIVES: The assessment of the effects of PNF combined with ERB on respiratory muscle strength. METHOD: Twenty healthy, right-handed females were included. Subjects were randomized to either the resistance training program group (TG, n=10) or the control group (CG, n=10). Maximal expiratory pressure (MEP) and inspiratory pressure (MIP) were measured before and after four weeks of an upper extremity resistance training program. The training protocol consisted of upper extremity PNF combined with ERB, with resistance selected from 1 repetition maximum protocol. RESULTS: PNF combined with ERB showed significant increases in MIP and MEP (p<0.05). In addition, there were significant differences between the TG and CG regarding ∆MIP (p=0.01) and ∆MEP (p=0.04). CONCLUSIONS: PNF combined with ERB can have a positive impact on respiratory muscle strength. These results may be useful with respect to cardiopulmonary chronic diseases that are associated with reduced respiratory muscle strength.

INTRODUÇÃO: As modificações da frequência cardíaca (FC) durante a transição repouso-exercício podem ser caracterizadas por meio da aplicação de cálculos matemáticos simples, como: deltas 0-10 e 0-30s para inferir sobre o sistema nervoso parassimpático, e delta e regressão linear aplicados no intervalo 60-240s para inferir sobre o sistema nervoso simpático. Assim, o objetivo deste estudo foi testar a hipótese de que indivíduos jovens e de meia-idade apresentam diferentes respostas da FC em exercício de intensidade moderada e intensa, com diferentes cálculos matemáticos. MÉTODOS: Homens aparentemente saudáveis, sendo sete de meia-idade e 10 jovens, foram submetidos a testes de carga constante de intensidade moderada e intensa. Foram calculados os deltas da FC nos períodos de 0-10s, 0-30s e 60-240s e a regressão linear simples no período de 60 a 240s. Os parâmetros obtidos na análise de regressão linear simples foram: intercepto e inclinação angular. Utilizou-se o teste Shapiro-Wilk para verificar a distribuição dos dados e o teste t não pareado para comparação entre os grupos. O nível de significância estatística considerado foi 5%. RESULTADOS: O valor do intercepto e do delta 0-10s foi menor no grupo meia-idade nas duas cargas e a inclinação do ângular foi menor no grupo meia-idade no exercício moderado. CONCLUSÃO: Os indivíduos jovens apresentam retirada vagal de maior magnitude no estágio inicial da resposta da FC durante exercício dinâmico em carga constante nas intensidades analisadas e maior velocidade de ajuste da resposta simpática em exercícios moderados.

BACKGROUND: Changes in heart rate during rest-exercise transition can be characterized by the application of mathematical calculations, such as deltas 0-10 and 0-30 seconds to infer on the parasympathetic nervous system and linear regression and delta applied to data range from 60 to 240 seconds to infer on the sympathetic nervous system. The objective of this study was to test the hypothesis that young and middle-aged subjects have different heart rate responses in exercise of moderate and intense intensity, with different mathematical calculations. METHODS: Seven middle-aged men and ten young men apparently healthy were subject to constant load tests (intense and moderate) in cycle ergometer. The heart rate data were submitted to analysis of deltas (0-10, 0-30 and 60-240 seconds) and simple linear regression (60-240 seconds). The parameters obtained from simple linear regression analysis were: intercept and slope angle. We used the Shapiro-Wilk test to check the distribution of data and the "t" test for unpaired comparisons between groups. The level of statistical significance was 5%. RESULTS: The value of the intercept and delta 0-10 seconds was lower in middle age in two loads tested and the inclination angle was lower in moderate exercise in middle age. CONCLUSION: The young subjects present greater magnitude of vagal withdrawal in the initial stage of the HR response during constant load exercise and higher speed of adjustment of sympathetic response in moderate exercise.

INTRODUÇÃO: A fisioterapia na fase I da reabilitação cardiovascular (FTCV) pode ser iniciada de 12 a 24 horas após o infarto agudo do miocárdio (IAM), no entanto, é comum o repouso prolongado no leito em razão do receio de instabilização do paciente. OBJETIVOS: Avaliar as respostas autonômicas e hemodinâmicas de pacientes pós-IAM submetidos ao primeiro dia de protocolo de FTCV fase I, bem como sua segurança. MATERIAIS E MÉTODOS: Foram estudados 51 pacientes com primeiro IAM não complicado, 55 ± 11 anos, 76% homens. Foram submetidos ao primeiro dia do protocolo de FTCV fase I, em média 24 horas pós-IAM. A frequência cardíaca (FC) instantânea e os intervalos R-R do ECG foram captados pelo monitor de FC (Polar®S810i) e a pressão arterial (PA) aferida pelo método auscultatório. A variabilidade da FC foi analisada nos domínios do tempo (RMSSD e RMSM dos iR-R em ms) e da frequência. A densidade espectral de potência foi expressa em unidades absolutas (ms²/Hz) e normalizada (un) para as bandas de baixa (BF) e alta frequência (AF) e pela razão BF/AF. RESULTADOS: O índice RMSSD, a AF e a AFun apresentaram redução na execução dos exercícios em relação ao repouso pré e pós-exercício (p < 0,05), a BFun e a razão BF/AF aumentaram (p < 0,05). A FC e a PA sistólica apresentaram aumento durante a execução dos exercícios em relação ao repouso (p < 0,05). Não foi observado qualquer sinal e/ou sintoma de intolerância ao esforço. CONCLUSÕES: O exercício realizado foi eficaz, pois promoveu alterações hemodinâmicas e na modulação autonômica nesses pacientes, sem ocasionar qualquer intercorrência clínica.

INTRODUCTION: Physical therapy during phase I of cardiac rehabilitation (CPT) can be started 12 to 24 hours after acute myocardial infarction (AMI), however, it is common to extend the bed rest due to fear of patient’s instability. OBJECTIVES: To assess the hemodynamic and autonomic responses to post-AMI patients when subjected to first day of phase I protocol of CPT, as well as their safety. MATERIALS AND METHODS: We studied 51 patients with first uncomplicated AMI, 55 ± 11 years, 76% men. The patients were subjected to first day protocol phase I CPT, on average, 24 hours after AMI. The Instantaneous heart rate (HR) and RR interval were acquired by HR monitor (Polar™S810i) and blood pressure (BP) checked by auscultation. HR variability was analyzed in the time (RMSSD and RMSM-Ri in ms) and frequency domains. Power spectral density was expressed in absolute (ms²/Hz) and normalized (nu) units for the bands of low (LF) and high frequencies (HF) and as LF/HF ratio. RESULTS: The RMSSD, HF and HFnu have reduced performance of the exercises in relation to rest and post-exercise (p < 0.05), LFnu and LF/HF ratio increased (p < 0.05). HR and systolic BP showed an increase during the execution of the exercises in relation to rest (p < 0.05). There were no any signs and/or symptoms of exercise intolerance. CONCLUSION: The exercise was effective, because it caused changes hemodynamic and autonomic modulation in these patients, without causing any medical complications.

CONTEXTUALIZAÇÃO: A magnitude das respostas cardiovasculares depende dos componentes estático e dinâmico bem como da duração e intensidade da contração realizada. OBJETIVO: Avaliar as respostas da frequência cardíaca (FC) frente a diferentes percentuais de contração isométrica em 12 pacientes (63±11,6 anos; média±dp) com doença da artéria coronária e/ou fatores de risco para ela, participantes de um programa de reabilitação cardíaca fase III. MÉTODOS: A variação da frequência cardíaca (ΔFC) foi avaliada durante as contrações voluntárias máximas (CVM; 5" e 10" de duração) e submáximas (CVSM; 30 e 60% da CVM-5, até exaustão muscular) de preensão palmar, utilizando-se um dinamômetro (hand grip). Adicionalmente, o RMSSD dos iR-R em ms (índice representante da modulação vagal cardíaca) foi calculado em repouso (pré-contração) nos últimos 30 segundos da CVSM e na recuperação (pós-contração). RESULTADOS: A ΔFC apresentou maiores valores em CVM-10 vs CVM-5 (17±5,5 vs 12±4,2 bpm, p<0,05) e no CVSM-60 vs CVSM-30 (19±5,8 vs 15±5,1 bpm, p<0,05). No entanto, os resultados para CVM-10 mostraram ΔFC similar quando comparados aos resultados obtidos para CVSM (p>0,05). RMSSD de repouso reduziu-se (p<0,05) durante a CVSM-30 (30%=29,9±17,1 vs 12,9±8,5ms) e CVSM-60 (60%=25,8±18,2 vs 9,96±4,2 ms), mas retornou aos valores basais quando a contração foi interrompida. CONCLUSÕES: Em pacientes com doença da artéria coronária e/ou fatores de risco para ela, a contração isométrica de baixa intensidade mantida por longos períodos de tempo apresenta os mesmos efeitos sobre as respostas da FC, quando comparada à contração isométrica de alta ou máxima intensidade, porém de breve duração.

BACKGROUND: The magnitude of cardiovascular responses is dependent on the static and dynamic components as well as the duration and intensity of the contraction performed. OBJECTIVE: To evaluate the heart rate responses to different percentages of isometric contractions in 12 patients (63±11.6 years) with coronary artery disease and/or risk factors for coronary artery disease that were participating in a phase III cardiac rehabilitation program. METHODS: Heart rate variation (ΔHR) was evaluated during maximum (MVC, five and ten seconds in duration) and submaximal (SMVC, 30 and 60% of MVC-5, until muscle exhaustion) voluntary contraction, using a handgrip dynamometer. Additionally, the representative index of cardiac vagal modulation (RMSSD index) was calculated at rest (pre-contraction), at the final 30 seconds of SMVC and during recovery (post-contraction). RESULTS: ΔHR showed higher values in MVC-10 versus MVC-5 (17±5.5 vs 12±4.2 bpm, p<0.05) and the SMVC-60 vs SMVC-30 (19±5.8 vs 15±5.1 bpm, p<0.05). However, results for CVM-10 showed similar ΔHR compared to results for CVSM (p> 0.05). RICVM at rest decreased (p<0.05) during SMVC-30 (30% = 27.9±17.1 vs 12.9±8.5 ms) and SMVC-60 (60% =25.8±18.2 vs 9.96±4.2 ms), but returned to the baseline values when the contraction was interrupted. CONCLUSIONS: In patients with coronary artery disease and/or risk factors for coronary heart disease, low intensity isometric contraction, maintained over long periods of time, presents the same effect on the responses of HR, compared to a high intensity or maximal isometric contraction of briefly duration.

O objetivo deste estudo foi avaliar o efeito do BiPAP®, através de máscara nasal, na tolerância ao exercício físico e no desempenho muscular respiratório em pacientes com diagnóstico clínico e espirométrico de DPOC, moderado/grave (VEF1 < 60% do previsto). Com VEF1/CVF <70% do previsto e idade média de 59,4±8,9 anos, dez pacientes com doença pulmonar obstrutiva crônica (DPOC) foram tratados com 30 minutos de BiPAP® (IPAP=10-15 e EPAP=4 cmH2O), em três sessões semanais, durante dois meses. Antes e após o tratamento mediu-se a espirometria, a força muscular inspiratória (PImax) e expiratória (PEmax) e a distância percorrida em seis minutos (TC6). Foram constatados aumentos significativos (Wilcoxon, p<0,05) na média da PImax (de -55±17 para -77±19 cmH2O), da PEmax (de 75±20 para 109±36 cmH2O) e da distância percorrida (de 349±67 para 448±75 metros). Com base nesses resultados conclui-se que o BiPAP® melhorou o desempenho muscular respiratório e a tolerância ao exercício físico nesses pacientes com DPOC.

This study aimed to assess the effect of BiPAP®, by nasal mask, on exercise tolerance and respiratory muscle strength in patients with a clinical and spirometric diagnosis of moderate/severe COPD (FEV1 < 60% of predicted). Ten patients of 59.4±8.9 years old, with FEV1/FVC <70% of predicted level, were treated with 30 minutes of BiPAP® (IPAP:10 and 15 cmH2O; EPAP:4 cmH2O), three days per week, during two months. Before and after the treatment, spirometry, inspiratory (MIP) and expiratory (MEP) muscle strength and the distance walked in six minutes (6MWT) were measured. We observed a significant increase (Wilcoxon, p<0.05) in the mean values of MIP (from -55±17 to -77±19, respectively), MEP (from 75±20 to 109±36, respectively) and walking distance (from 349±67 to 448±75). Based on these results, we concluded that BiPAP® improves respiratory muscle strength and exercise tolerance in these COPD patients.

El objetivo de esto estudio fue evaluar el efecto del BiPAP®, por medio de la máscara nasal, sobre la tolerancia al ejercicio físico y el desempeño muscular respiratorio en pacientes con diagnosis clínico y espirométrico de EPOC moderada/grave (VEF1 < 60% del previsible). Con VEF1/CVF <70% del previsible y edad promedia de 59,4±8,9 años, diez pacientes fueron tratados con 30 minutos de BiPAP® (IPAP=10-15 e EPAP=4 cmH2O), en tres sesiones semanales, durante dos meses. Antes y después del tratamiento, fue medida la espirometría, la fuerza muscular inspiratoria (PImax) y expiratoria (PEmax) y la distancia cubierta en seis minutos (TC6). OBJETIVO: Avaliar os níveis de L-carnitina livre no plasma, o estado nutricional, a função pulmonar e a tolerância ao exercício em pacientes com doença pulmonar obstrutiva crônica e verificar as correlações entre a composição corporal e as frações de L-carnitina no plasma. MÉTODOS: Quarenta pacientes entre 66,2±9 anos, com diagnóstico clínico de doença pulmonar obstrutiva crônica, foram divididos em dois grupos: G1, com índice de massa corporal menor que 20kg/m², e G2, com índice de massa corporal maior que 20kg/m². Foram mensurados os parâmetros espirométricos, a tolerância ao exercício no teste de caminhada, a força muscular respiratória, a composição corporal por meio da impedância bioelétrica e as dosagens da L-carnitina plasmática, através de amostras de sangue. RESULTADOS: Foram observados menores valores das variáveis espirométricas (p<0,01), da força muscular respiratória e dos níveis de L-carnitina nos pacientes do G1; porém, não houve diferença entre os grupos quanto à capacidade de realizar exercício físico dinâmico de baixa intensidade. Correlações significativas entre o percentual de gordura e os níveis de L-carnitina plasmática foram observadas nos pacientes (r=0,53 com p<0,002); sendo que, nos pacientes com índice de massa corporal menor que 18kg/m², essa relação aumentou (r<0,73 com p<0,01). CONCLUSÃO: Na doença pulmonar obstrutiva crônica, a desnutrição está associada tanto aos prejuízos da função pulmonar e da força muscular respiratória, quanto aos baixos níveis de L-Carnitina plasmática.

OBJECTIVE: The objective of this study was to evaluate the levels of free L-carnitine in the plasma, the nutritional condition, the pulmonary function, and the tolerance to exercising in patients with chronic obstructive pulmonary Disease, in order to verify the correlations between body composition and L-carnitine levels in the plasma. METHODS: Forty patients between 66.2±9 years of age, with clinical diagnostics of chronic obstructive pulmonary disease, were divided in two groups: G1, patients with body mass index of less than 20 kg/m², and G2, with Body Mass Index of more than 20 kg/m². There were evaluations of the spirometric variables; the exercise tolerance, through a six-minute walking test; the respiratory muscle strength; the body composition, through the bioelectric impedance; and the free L-carnitine levels in the plasma, through blood exams. RESULTS: The results showed lower values in G1 patients, for the spirometric variables (p<0.01), the respiratory muscle strength, and the L-carnitine levels; however, no difference between the groups was observed regarding the capability to perform low-intensity dynamic physical exercise. There were significant correlations between the fat percentages and the levels of L-carnitine in the plasma (r=0.3 and p<0.002); furthermore, in patients who presented body mass index less than 18kg/m², this relationship was increased (r<0,73 with p<0,01). CONCLUSION: In patients diagnosed with chronic obstructive pulmonary disease, the malnutrition is associated with damage of pulmonary function and reduction of respiratory muscle strength, as well as with lower levels of free plasmatic L-carnitine.

INTRODUÇÃO: Pacientes portadores de doença pulmonar obstrutiva crônica apresentam redução da tolerância ao exercício físico, principalmente devido à limitação ventilatória. A L-carnitina tem sido utilizada com o objetivo de melhorar a capacidade aeróbia de pacientes com doenças crônicas, porém não existem estudos em pacientes portadores de doença pulmonar obstrutiva crônica. OBJETIVO: Avaliar a influência da suplementação de L-carnitina, associada ao treinamento físico por seis semanas, três vezes por semana em pacientes portadores de doença pulmonar obstrutiva crônica. MÉTODO: A amostra foi constituída de 30 pacientes portadores de doença pulmonar obstrutiva crônica (69 ± 7 anos) com volume expiratório forçado no primeiro segundo < 65% do previsto, dividida em três grupos de 10 pacientes: grupo 1 com treinamento físico e suplementação com 2g/dia de L-carnitina, grupo 2 que recebeu treinamento físico e placebo e grupo 3 que não foi submetido a treinamento físico e recebeu 2g/dia de L-carnitina. Os pacientes foram submetidos a avaliação espirométrica, a teste de caminhada de seis minutos e à mensuração dos níveis plasmáticos de carnitina livre no inicio e no final do estudo. RESULTADOS: Foi constatado aumento significativo (p < 0,05) da distância percorrida no teste de caminhada de seis minutos somente nos pacientes dos dois primeiros grupos (de 421 ± 100 para 508 ± 80,7 e de 496 ± 78,7 para 526 ± 64,3 respectivamente). Além disso, com intensidade de exercício semelhante, a subida da freqüência cardíaca foi menor no grupo 1 quando comparado com o grupo 2. As variáveis espirométricas, a saturação da oxihemoglobina e a dispnéia não se alteraram em nenhum dos grupos estudados. Os valores de L-carnitina livre no plasma aumentaram somente nos pacientes do terceiro grupo (59,2 ± 13,8 para 102,3 ± 15,32mmol/L). CONCLUSÃO: A L-carnitina associada ao treinamento físico pode proporcionar maior tolerância ao exercício em pacientes com doença pulmonar obstrutiva crônica.

BACKGROUND: Patients with chronic obstructive pulmonary disease usually present intolerance to physical exertion due to ventilatory limitation. L-carnitine has been used to enhance aerobic capacity in patients with chronic diseases, but no study seems to be available for this patient population. OBJECTIVE: To evaluate the influence of L-carnitine supplementation (2 g/day) in chronic obstructive pulmonary disease patients undergoing physical training three times a week for six weeks. METHOD: Patients (mean age 69 ± 7 years, n = 30) with stable chronic obstructive pulmonary disease and < 65% of predicted forced expiratory volume in 1 second (FEV1) were separated into three groups of 10 patients each. Group 1 (G1, n = 10) received physical training and L-carnitine (2 g/day), group 2 (G2, n = 10) received physical training and placebo, and group 3 (G3, n = 10) received only L-carnitine (2 g/day). Spirometry and a 6-minute walking distance test were performed before and after intervention. Plasma levels of free carnitine were measured at the beginning and end of the study. RESULTS: A significant increase in walking distance was found only in G1 and G2 (421 ± 100 to 508 ± 80.7 and 496 ± 78.7 to 526 ± 64.3 respectively). In addition, heart rates during physical training sessions were found to be significantly lower in G1 when compared to G2. There were no significant changes in spirometric variables, oxygen saturation or dyspnea in any group. Plasma levels of free L-carnitine were found to increase only in G3 (59.2 ± 13.8 to 102.3 ± 15.3 mmol/L). CONCLUSION: Oral L-carnitine supplementation combined with physical training may improve tolerance to physical exertion in chronic obstructive pulmonary disease patients.
