RESUMO Contexto: Em estudos observacionais sobre a síndrome do desconforto respiratório agudo, sugeriu-se que a driving pressure é o principal fator de lesão pulmonar induzida por ventilador e de mortalidade. Não está claro se uma estratégia de limitação da driving pressure pode melhorar os desfechos clínicos. Objetivo: Descrever o protocolo e o plano de análise estatística que serão usados para testar se uma estratégia de limitação da driving pressure envolvendo a titulação da pressão positiva expiratória final de acordo com a melhor complacência respiratória e a redução do volume corrente é superior a uma estratégia padrão envolvendo o uso da tabela de pressão positiva expiratória final baixa do protocolo ARDSNet, em termos de aumento do número de dias sem ventilador em pacientes com síndrome do desconforto respiratório agudo devido à pneumonia adquirida na comunidade. Métodos: O estudo STAMINA (ventilator STrAtegy for coMmunIty acquired pNeumoniA) é randomizado, multicêntrico e aberto e compara uma estratégia de limitação da driving pressure com a tabela de pressão positiva expiratória final baixa do protocolo ARDSnet em pacientes com síndrome do desconforto respiratório agudo moderada a grave devido à pneumonia adquirida na comunidade internados em unidades de terapia intensiva. Esperamos recrutar 500 pacientes de 20 unidades de terapia intensiva brasileiras e duas colombianas. Eles serão randomizados para um grupo da estratégia de limitação da driving pressure ou para um grupo de estratégia padrão usando a tabela de pressão positiva expiratória final baixa do protocolo ARDSnet. No grupo da estratégia de limitação da driving pressure, a pressão positiva expiratória final será titulada de acordo com a melhor complacência do sistema respiratório. Desfechos: O desfecho primário é o número de dias sem ventilador em 28 dias. Os desfechos secundários são a mortalidade hospitalar e na unidade de terapia intensiva e a necessidade de terapias de resgate, como suporte de vida extracorpóreo, manobras de recrutamento e óxido nítrico inalado. Conclusão: O STAMINA foi projetado para fornecer evidências sobre se uma estratégia de limitação da driving pressure é superior à estratégia da tabela de pressão positiva expiratória final baixa do protocolo ARDSnet para aumentar o número de dias sem ventilador em 28 dias em pacientes com síndrome do desconforto respiratório agudo moderada a grave. ABSTRACT Background: Driving pressure has been suggested to be the main driver of ventilator-induced lung injury and mortality in observational studies of acute respiratory distress syndrome. Whether a driving pressure-limiting strategy can improve clinical outcomes is unclear. Objective: To describe the protocol and statistical analysis plan that will be used to test whether a driving pressure-limiting strategy including positive end-expiratory pressure titration according to the best respiratory compliance and reduction in tidal volume is superior to a standard strategy involving the use of the ARDSNet low-positive end-expiratory pressure table in terms of increasing the number of ventilator-free days in patients with acute respiratory distress syndrome due to community-acquired pneumonia. Methods: The ventilator STrAtegy for coMmunIty acquired pNeumoniA (STAMINA) study is a randomized, multicenter, open-label trial that compares a driving pressure-limiting strategy to the ARDSnet low-positive end-expiratory pressure table in patients with moderate-to-severe acute respiratory distress syndrome due to community-acquired pneumonia admitted to intensive care units. We expect to recruit 500 patients from 20 Brazilian and 2 Colombian intensive care units. They will be randomized to a driving pressure-limiting strategy group or to a standard strategy using the ARDSNet low-positive end-expiratory pressure table. In the driving pressure-limiting strategy group, positive end-expiratory pressure will be titrated according to the best respiratory system compliance. Outcomes: The primary outcome is the number of ventilator-free days within 28 days. The secondary outcomes are in-hospital and intensive care unit mortality and the need for rescue therapies such as extracorporeal life support, recruitment maneuvers and inhaled nitric oxide. Conclusion: STAMINA is designed to provide evidence on whether a driving pressure-limiting strategy is superior to the ARDSNet low-positive end-expiratory pressure table strategy for increasing the number of ventilator-free days within 28 days in patients with moderate-to-severe acute respiratory distress syndrome. RESUMEN Objetivo Evaluar las tendencias temporales y la magnitud de los indicadores de accidentes laborales entre los beneficiarios del Sistema de Seguridad Social en Brasil y sus regiones, durante el período de 2009 a 2019. Métodos Estudio de series temporales sobre los indicadores de accidentes de trabajo en las regiones de Brasil, en el período de 2009 a 2019; los datos fueron extraídos de las bases del Anuario Estadístico de Accidentes de Trabajo y del Anuario Estadístico de la Seguridad Social. Se utilizaron modelos de regresión lineal generalizada de Prais-Winsten para estimar las tendencias y los cambios porcentuales anuales y se obtuvieron sus intervalos de confianza del 95%. Resultados Hubo 7.253.923 accidentes laborales en el período analizado; la incidencia media de accidentes de trabajo fue de 16,28 por 1.000 vínculos, con una tendencia decreciente (VPA = 4,3%; IC95% -5,63; -3,26). Hubo desigualdades entre las regiones, pero la tendencia predominante fue de disminución en todo el país. ABSTRACT Objective To evaluate the temporal trend and magnitude of occupational accident indicators among Social Security beneficiaries in Brazil and its regions from 2009 to 2019. Methods A time series study was conducted on occupational accident indicators in the regions of Brazil, from 2009 to 2019. Data were retrieved from the Statistical Yearbook of Occupational Accidents and the Statistical Yearbook of Social Security. Prais-Winsten generalized linear regression models were used to estimate trends, and annual percentage change and their respective 95% confidence intervals were obtained. Results There were 7,253,923 occupational accidents during the study period. The average incidence rate was 16.3 per 1,000 employment relationships, with a decreasing trend (APC = 4.3%; 95%CI -5.63;-3.26). RESUMO Objetivo Avaliar tendência temporal e magnitude dos indicadores de acidentes do trabalho entre segurados da Previdência Social no Brasil e regiões de 2009 a 2019. Métodos Estudo de séries temporais dos indicadores de acidentes do trabalho nas regiões do Brasil, no período de 2009 a 2019. Os dados foram extraídos do Anuário Estatístico de Acidentes do Trabalho e do Anuário Estatístico da Previdência Social. Modelos de regressão linear generalizada de Prais-Winsten foram utilizados para estimar as tendências e obteve-se a variação percentual anual e os respectivos intervalos de confiança de 95%. Resultados Houve 7.253.923 acidentes do trabalho no período. A incidência média de acidentes foi 16,3 por mil vínculos de trabalho, com tendência decrescente (VPA = 4,3%; IC95% -5,63;-3,26). RESUMO O objetivo deste estudo é validar um instrumento de aferição da funcionalidade segundo a proposta conceitual da Classificação Internacional de Funcionalidade, Incapacidade e Saúde, para uso em pessoas pós-chikungunya. Este é um estudo de validação com indivíduos >17 anos, de ambos os sexos, em atendimento para manejo clínico da chikungunya. A coleta de dados foi realizada por meio de entrevistas coletando informações sobre funcionalidade (WHODAS), qualidade de vida (WHOQOL-bref) e sociodemográficas. A análise estatística usou o coeficiente alfa de Cronbach (consistência interna) e coeficiente de correlação de Spearman (validade convergente), médias e desvios-padrão para a determinação do perfil de qualidade de vida, com nível de significância de 5%. A amostra foi composta por 68 indivíduos. Os valores médios das pontuações dos instrumentos foram: 45,4 (±16,38) para o WHODAS e 12,1 (±2,10) para o WHOQOL-bref. O alfa de Cronbach do valor total foi de α=0,93; todos os domínios do WHODAS apresentaram valores acima de 0,75. O valor total do WHODAS 2.0 apresentou forte correlação com o domínio físico (r=−0,74) e moderada correlação com os domínios psicológico (r=−0,68) e social (r=−0,42) do WHOQOL-bref. Os resultados indicam que o WHODAS 2.0 é um instrumento válido para a mensuração da autopercepção de alteração da funcionalidade em pacientes acometidos pela chikungunya, capaz de fornecer dados que podem ajudar a construir um perfil de impacto da doença no perfil de funcionalidade dessa população.

ABSTRACT The aim of this study is to validate an assessment tool for functioning and health according to the conceptual proposal of the International Classification of Functioning, Disability and Health (ICF), for its use in post-chikungunya patients. This is a validation study with individuals >17 years old of both sexes. Data collection was performed through interviews, gathering information on functioning (WHODAS), quality of life (WHOQOL-bref) and socio-demographic data. Statistical analysis used the Cronbach alpha coefficient (internal consistency) and Spearman correlation coefficient (convergent validation), means and standard deviations, with significance level of 5%. The sample consisted of 68 individuals. The mean values of scores were: 45.4 (±16.38) for WHODAS and 12.1 (±2.10) for WHOQOL-bref. The Cronbach’s alpha of the total value was α=0.93; all WHODAS domains presented values ≥0.75. The total value of WHODAS 2.0 had strong correlation with the physical domain (r=−0.74) and moderate correlation with the psychological (r=−0.68) and social (r=−0.42) domains of the WHOQOL-bref. The results indicate that WHODAS 2.0 is a valid instrument for the measurement of self-perceived functioning alteration in patients affected by chikungunya.

RESUMEN El objetivo de este estudio es validar un instrumento de evaluación del funcionamiento de acuerdo con la propuesta conceptual de la Clasificación Internacional del Funcionamiento de la Discapacidad y de la Salud (CIF), para su uso en personas postchikungunya. Se trata de un estudio de validación con individuos >17 años, de ambos sexos, en atención para el manejo clínico de chikungunya. La recolección de datos se realizó mediante entrevistas, que reunieron informaciones sobre el funcionamiento (WHODAS), la calidad de vida (WHOQOL-bref) y los datos sociodemográficos. El análisis estadístico utilizó el coeficiente alfa de Cronbach (consistencia interna) y el coeficiente de correlación de Spearman (validación convergente), medias y desviaciones estándar para determinar el perfil de la calidad de vida, con un nivel de significación del 5%. La muestra consistió en 68 individuos. Los valores medios de las puntuaciones fueron: 45,4 (±16,38) para WHODAS; y 12,1 (±2,10) para WHOQOL-bref. El alfa de Cronbach del valor total fue α=0,93; Todos los dominios WHODAS presentaron valores superiores a 0,75. El valor total de WHODAS 2.0 tuvo una fuerte correlación con el dominio físico (r=−0,74) y una correlación moderada con los dominios psicológico (r=−0,68) y social (r=−0,42) del WHOQOL-bref. Los resultados indican que WHODAS 2.0 es un instrumento válido para medir la alteración del funcionamiento autopercibido en pacientes afectados por chikungunya, siendo capaz de proporcionar datos que pueden ayudar a construir un perfil de impacto de la enfermedad en el perfil de esta población.

Resumo Fundamento: A síndrome coronariana aguda (SCA) é responsável por elevados números de admissões e readmissões hospitalares, os quais estão associados ao aumento dos custos para o paciente e para o sistema de saúde, bem como à elevação nas taxas de mortalidade hospitalar. Objetivo: Investigar a reinternação entre pacientes com SCA e seus determinantes. Métodos: Trata-se de uma coorte retrospectiva de pacientes de ambos os sexos, adultos e idosos, diagnosticados com SCA. Foram avaliados, a partir dos registros dos hospitais locais públicos e privados de referência em cardiologia, a ocorrência de reinternação em até 1 ano após internação por SCA, o tempo entre as admissões e o uso de medicamentos no momento da reinternação. As variáveis categóricas foram associadas por meio do teste qui-quadrado, ou pelo teste exato de Fisher. Regressão logística múltipla foi utilizada para avaliar as variáveis preditoras da reinternação. Adotou-se como critério de significância estatística um valor de p < 0,05. Resultados: A ocorrência de reinternações foi de 21,46% (n = 115), e o período médio entre as internações foi de 122,74 (DP 112,14) dias. Os pacientes avaliados eram, em sua maioria, do sexo masculino (64,0%), com média de idade de 63,15 anos (DP 12,26). Sete por cento apresentaram óbito como prognóstico da reinternação, e 68,7% tiveram mais de uma reinternação em 1 ano. As causas cardiovasculares, entre elas a recorrência da SCA, foram as mais prevalentes entre as reinternações hospitalares. A assistência privada e o diagnóstico de insuficiência cardíaca congestiva (ICC) foram associados a reinternação após a regressão logística múltipla. Conclusão: Conclui-se que a SCA foi a principal causa de reinternação, sendo mais prevalente entre os usuários da rede suplementar de saúde. As reinternações associaram-se ao diagnóstico prévio de ICC e ao tipo de assitência à saúde.

Abstract Background: Acute coronary syndrome (ACS) is responsible for high rates of hospital admission and readmission, which are associated with increased costs for the patient and the health system, and increased in-hospital mortality rates. Objective: To evaluate readmission in patients with ACS and its determinants. Methods: This was a retrospective cohort study of adult and elderly patients with ACS, readmitted to public and private referral cardiology hospitals within one year after the first hospitalization for ACS. The occurrence of readmissions, the time elapsed from the first to the second admission, and the use of medications at admission were collected from the medical records. Associations between categorical variables were evaluated by the chi-square test or the Fisher's exact test. Multiple logistic regression was used to evaluate predictors for readmissions. A p < 0.05 was set as statistically significant. Results: Readmission rate was 21.5% (n = 115) and mean time between admissions was 122.7 ± 112.1 days. The patients were mostly men (64.0%), mean age of 63.15 ± 12.3 years. Among readmitted patients, 7% had a prognosis of "death", and 68.7% were readmitted more than once within a one-year period. The main reasons of readmission were cardiovascular diseases including ACS. Private health care and the diagnosis of congestive heart failure were associated with multiple logistic regression. Conclusion: ACS was the main cause of readmission, with higher prevalence among users of supplemental health care. Readmissions were associated with previous diagnosis of congestive heart failure and the type of health care provided.

Abstract Introduction The bone-anchored hearing aid (BAHA) is a bone conduction system that transmits the sound directly to the inner ear by surpassing the skin impedance and the subcutaneous tissue. It is indicated for patients with mixed, conductive and unilateral sensorineural hearing loss who did not benefit from conventional hearing aids (HAs). Although the benefits from BAHA are well demonstrated internationally, this field still lacks studies in Brazil. Objective To assess the auditory rehabilitation process in BAHA users through audiological, speech perception and tinnitus aspects. Methods Individuals with hearing loss were assessed before and after the implantation. The participants were subjected to pure tone audiometry in free field, functional gain audiometry, speech perception tests, tinnitus handicap inventory (THI) in open format, and to the visual analog scale (VAS). Results It was found that the participants benefited from the use of BAHA. The difference in the performance of the participants before and after the BAHA surgery was significant in terms of hearing acuity. There was no statistically significant difference in the speech perception tests. The tinnitus assessment showed that 80% of the participants scored slight tinnitus severity in THI after using a BAHA. Eighty percent of the participants classified their tinnitus as absent tomild in the VAS after the surgery. Conclusion Based on the results of the current study, we can conclude that the participants improved both the auditory perception and the tinnitus handicap.

RESUMO INTRODUÇÃO: O implante coclear tem a capacidade de medir o potencial de ação composto eletricamente evocado do nervo auditivo (ECAP). Para esta verificação utiliza-se uma medida chamada telemetria de respostas neurais. OBJETIVO: Analisar o potencial de ação composto evocado eletricamente, por meio da neurotelemetria de respostas neurais, em crianças usuárias de implante coclear bilateral. MÉTODO: Trata-se de um estudo analítico, prospectivo, de coorte histórica longitudinal. Foram recrutadas seis crianças, com idades entre de 1-4 anos, usuárias de implante coclear bilateral. Estas crianças foram avaliadas em cinco momentos durante o primeiro ano de uso do implante coclear. RESULTADOS: Houve diferença significativa no tempo de acompanhamento (p = 0,0082) e posição do eletrodo (p = 0,0019) na medida de T-NRT. Houve diferença significativa na interação entre tempo de acompanhamento e posição do eletrodo (p = 0,0143) na medida da amplitude das ondas N1-P1 entre os três eletrodos a cada tempo de acompanhamento. CONCLUSÃO: A mensuração do ECAP por meio da NRT nas crianças com implante coclear bilateral durante o primeiro ano de acompanhamento foi uma medida importante para apresentar o desenvolvimento bilateral da via auditiva periférica de forma sincronizada nesta população estudada.

ABSTRACT INTRODUCTION: The cochlear implant device has the capacity to measure the electrically evoked compound action potential of the auditory nerve. The neural response telemetry is used in order to measure the electrically evoked compound action potential of the auditory nerve. OBJECTIVE: To analyze the electrically evoked compound action potential, through the neural response telemetry, in children with bilateral cochlear implants. METHODS: This is an analytical, prospective, longitudinal, historical cohort study. Six children, aged 1-4 years, with bilateral cochlear implant were assessed at five different intervals during their first year of cochlear implant use. RESULTS: There were significant differences in follow-up time (p = 0.0082) and electrode position (p = 0.0019) in the T-NRT measure. There was a significant difference in the interaction between time of follow-up and electrode position (p = 0.0143) when measuring the N1-P1 wave amplitude between the three electrodes at each time of follow-up. CONCLUSION: The electrically evoked compound action potential measurement using neural response telemetry in children with bilateral cochlear implants during the first year of follow-up was effective in demonstrating the synchronized bilateral development of the peripheral auditory pathways in the studied population.

OBJETIVO: Avaliar as ações do Programa "Um Beijo Pela Vida", desenvolvido em um município do Nordeste brasileiro visando ampliar a adesão ao rastreamento do câncer de mama em mulheres cadastradas pela Estratégia Saúde da Família. MÉTODOS: Utilizou-se abordagem quantitativa para avaliar a cobertura das ações de rastreamento nas mulheres com 40 anos de idade ou mais. Foi realizada busca ativa das mulheres por agentes comunitários de saúde das nove Equipes de Saúde da Família do município. Os percentuais das mulheres submetidas ao exame clínico das mamas e à mamografia; de exames na faixa etária recomendada pelo Programa; da classificação das lesões mamográficas, de acordo com o BI-RADS®; das mulheres que receberam encaminhamento para diagnóstico e tratamento e o número de casos de câncer de mama detectados foram colhidos por meio de um questionário estruturado, analisados pelo programa EPI-INFO TM e confrontados a padrões previamente fixados. RESULTADOS: Foram incluídas 3.608 mulheres, correspondendo a 68,4% da população-alvo inscrita na Estratégia Saúde da Família. As taxas de cobertura do exame clínico das mamas para mulheres de 40 a 49 anos e da mamografia para mulheres de 50 a 69 anos foram de 58,9 e 56,7%, respectivamente. As mulheres com lesões mamográficas altamente suspeitas foram submetidas à punção por agulha fina ou por agulha grossa (100%). Seis casos novos de câncer foram detectados. Foram cumpridos 80% dos padrões analisados. CONCLUSÕES: A avaliação das ações do Programa aponta para sua adequação, considerando-se o grau de cumprimento das exigências previamente definidas.

PURPOSE: To evaluate the actions of the "Um Beijo Pela Vida" Program developed in a Brazilian Northeast city to increase adherence to breast cancer screening by women registered by the Brazilian Family Health Strategy for breast cancer screening. METHODS: A quantitative approach was used to evaluate the coverage of screening actions for aged 40 years-old or more. Community workers from the nine Family Health Teams of the town carried out an active search. The percentage of eligible women who were screened for breast cancer by clinical breast examination or mammography, mammogram classification according to BI-RADS®, women screened who were referred for further testing and treatment, and the number of breast cancers detected were collected by means of a structured questionnaire, analyzed with the EPI-INFO TM software and compared to previously defined patterns. RESULTS: 3,608 women were included, corresponding to 68.4% of the target population registered in the Brazilian Family Health Strategy. Coverage rates of clinical breast examination for women aged 40 to 49 years-old and of mammograms for women aged 50 to 69 years-old were 58.9 and 56.7%, respectively. All women with highly suspicious mammographic lesions were submitted to fine needle aspiration or core biopsy (100%). Six new cases of cancer were detected and 80% of the standards established for this evaluation were carried out. CONCLUSIONS: The evaluation of the actions of the Program suggests its adequacy considering the degree of fulfillment of the previously defined requirements.

Neste estudo foi analisada a riqueza, a abundância, os sítios de nidificação, o padrão de distribuição espacial, a densidade dos sítios potenciais disponíveis e dos sítios nidificados pelas espécies de abelhas sem ferrão do gênero Partamona e dos termiteiros arbóreos nidificados pelas abelhas, em uma área restrita de caatinga arbórea, em Milagres, Bahia, Brasil. Duas espécies ocorreram em simpatria, Partamona rustica (94,1%) e Partamona cupira (5,9%), ambas nidificando em termiteiros arbóreos da espécie Constrictotermes cyphergaster. Outras três espécies de termiteiros arbóreos ocorreram na área (Nasutitermes corniger, Nasutitermes macrocephalus e Microcerotermes sp.). Os térmitas arbóreos nidificaram principalmente na umburana (Commiphora leptophloeos). As espécies de abelhas do gênero Partamona têm como estratégia de nidificação na caatinga utilizar cavidades pré-existentes que consistem em ocos abertos em termiteiros arbóreos pelo periquito jandaia (Aratinga cactorum), para reprodução. Após o nascimento dos filhotes, o oco é abandonado e em seguida uma colônia de abelhas do gênero Partamona constrói ali o seu ninho, ocupando-o e isolando-o com geoprópolis, sendo freqüente a ocupação destes ocos por outros inquilinos.

The species richness, abundance, nesting sites, spatial distribution pattern, density of available substrates and substrates used for nesting by Partamona species were studied in a restricted area of a tropical dry forest/woodlands ecosystem called "arboreal caatinga" in Milagres, Bahia State, Brazil. Two species of the genus Partamona, occurred in the studied area, Partamona rustica (94.1%) and Partamona cupira (5.9%), both nested in arboreal termites nests of the Constrictotermes cyphergaster species. Other three species of arboreal termites' nests occurred in the area (Nasutitermes corniger, Nasutitermes macrocephalus and Microcerotermes sp.). The arboreal termites had nested mainly in "umburana" (Commiphora lepthophloeos). The nesting strategy of Partamona species consist in occupying preexisting cavities opened in arboreal termites nested by a parakeet called "jandaia" (Aratinga cactorum), for reproduction. After the birth of the younglings, the cavity is abandoned and after that a colony of Partamona bees occupies it and starts its nest construction, isolating it with mud and resin. Frequently these cavities are occupied by other inquilines.
