Multiple myeloma and Chagas disease: qPCR as a marker for preemptive antiparasitic therapy: a case reports series and review disease therapy

ABSTRACT Multiple myeloma (MM) associated with Chagas disease is rarely described. This disease and its therapy suppress T cell and macrophage functions and increase regulatory T cell function, allowing the increase of parasitemia and the risk of Chagas Disease Reactivation (CDR). We aimed to analyze the role of conventional (cPCR) and quantitative Polymerase Chain Reaction (qPCR) for prospective monitoring of T. cruzi parasitemia, searching for markers of preemptive antiparasitic therapy in MM patients with Chagas disease. Moreover, we investigated the incidence and management of hematological diseases and CDR both inside and outside the transplant setting in the MEDLINE database. We found 293 studies and included 31 of them. Around 1.9–2.0% of patients with Chagas disease were reported in patients undergoing Stem Cell Transplantation. One case of CDR was described in eight cases of MM and Chagas disease. We monitored nine MM and Chagas disease patients, seven under Autologous Stem Cell Transplantation (ASCT), during 44.56±32.10 months (mean±SD) using parasitological methods, cPCR, and qPCR. From these patients, three had parasitemia. In the first, up to 256 par Eq/mL were detected, starting from 28 months after ASCT. The second patient dropped out and died soon after the detection of 161.0 par Eq/mL. The third patient had a positive blood culture. Benznidazole induced fast negativity in two cases; followed by notably lower levels in one of them. Increased T. cruzi parasitemia was related to the severity of the underlying disease. We recommend parasitemia monitoring by qPCR for early introduction of preemptive antiparasitic therapy to avoid CDR. (MM function . ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Passion fruit woodiness disease, caused by the Cowpea aphid-borne mosaic virus (CABMV), is one of the leading phytosanitary challenges of passion fruit production. Passiflora cincinnata has been recognized for its potential in genetic improvement due to its highest resistance to CABMV and other phytopathogens. Metabolomic and histological alterations of P. cincinnata infected with CABMV were evaluated and searched for differential responses during the asymptomatic or symptomatic infection phases to correlate them with the mechanisms of metabolic defense. The metabolites of infected plants were analyzed by liquid chromatography-mass spectrometry. Based on the metabolomic profile, the times of infection were grouped into early or late infection phases. The metabolites related to CABMV infection were classified as alkaloids, saponins, phospholipids and acids. This study can assist agricultural institutions or farms in the early diagnosis and correct management of CABMV infection and contribute to the genetic improvement of the Passiflora genus against this disease.

RESUMO Objetivo: Avaliar a capacidade do Teste de Caminhada de 6 Minutos para predizer a melhora do estado funcional físico em longo prazo de pacientes sobreviventes à unidade de terapia intensiva. Métodos: Foram avaliados, de forma prospectiva, entre fevereiro de 2017 e agosto de 2018, em um ambulatório pós-unidade de terapia intensiva, 32 sobreviventes à unidade de terapia intensiva. Foram inscritos consecutivamente os pacientes com permanência na unidade de terapia intensiva acima de 72 horas (para admissões emergenciais) ou acima de 120 horas (para admissões eletivas) que compareceram ao ambulatório pós-unidade de terapia intensiva 4 meses após receberem alta da unidade de terapia intensiva. A associação entre a distância percorrida no Teste de Caminhada de 6 Minutos realizado na avaliação inicial e a evolução do estado funcional físico foi avaliada durante 8 meses, com utilização do Índice de Barthel. Resultados: A distância média percorrida no Teste de Caminhada de 6 Minutos foi significantemente mais baixa nos sobreviventes à unidade de terapia intensiva do que na população geral (405m versus 557m; p < 0,001). A idade (β = -4,0; p < 0,001) e a fraqueza muscular (β = -99,7; p = 0,02) se associaram com a distância percorrida no Teste de Caminhada de 6 Minutos. A distância percorrida no Teste de Caminhada de 6 Minutos se associou com melhora do estado funcional físico no período de 8 meses de acompanhamento desses pacientes (razão de chance para cada 10m: 1,07; IC95% 1,01 - 1,16; p = 0,03). A área sob a curva Característica de Operação do Receptor para predição da melhora funcional física pelo Teste de Caminhada de 6 Minutos foi de 0,72 (IC95% 0,53 - 0,88). Conclusão: O Teste de Caminhada de 6 Minutos, realizado 4 meses após a alta da unidade de terapia intensiva, predisse com precisão moderada a melhora do estado funcional físico de sobreviventes à unidade de terapia intensiva.
ABSTRACT Objective: To evaluate the ability of the 6-Minute Walk Test to predict long-term physical functional status improvement among intensive care unit survivors. Methods: Thirty-two intensive care unit survivors were prospectively evaluated from February 2017 to August 2018 in a post-intensive care unit outpatient clinic in Brazil. Individuals with intensive care unit stays > 72 hours (emergency admissions) or > 120 hours (elective admissions) attending the post-intensive care unit clinic four months after intensive care unit discharge were consecutively enrolled. The association between the 6-Minute Walk Test distance at baseline and physical functional status was assessed over 8 months using the Barthel Index. Results: The mean 6-Minute Walk Test distance was significantly lower in intensive care unit survivors than in the general population (405m versus 557m; p < 0.001). Age (β = -4.0; p < 0.001) and muscle weakness (β = -99.7; p = 0.02) were associated with the 6-Minute Walk Test distance. A 6-Minute Walk Test distance was associated with improvement in physical functional status over the 8-month follow-up (odds ratio for each 10m of 1.07; 95%CI 1.01 - 1.16; p = 0.03). The area under the Receiver Operating Characteristic curve for the 6-Minute Walk Test prediction of physical functional status improvement was 0.72 (95%CI 0.53 - 0.88). Conclusion: The 6-Minute Walk Test performed 4 months after intensive care unit discharge predicted long-term physical functional status among intensive care unit survivors with moderate accuracy.

Abstract In view of the complexity of the processes associated to water quality, this study objective is to identify the main factors related to spatial and seasonal variability in water courses of the Lobo Stream River Basin. To this, multivariate statistical techniques were used. Data collection for water quality and streamflow variables were carried out monthly, from May 2018 to April 2019, at 10 monitoring points along basin’s tributaries. The results show that, during the dry season, the main causes for water quality decrease are related to erosion process on the river margins, which is intensified by inadequate handling in livestock activities in some monitoring points. In the rainy season, the main causes are related to soil leaching in agricultural areas that increases the nitrogen compounds concentration and reduces water quality. However, in addition to this, it was noted that regardless the environmental conditions, the most impactful factor is the point pollution from the effluent discharge of Itirapina City sewage treatment plant, responsible for nutrient concentration increase, organic contamination, OD reduction, and, consequently, water quality deterioration. With this, the study shows how multivariate statistical analysis enables more relevant evaluation of water quality data variability and supports further studies in the basin.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

RESUMO: O Pragmatismo, como proposto por Charles S. Peirce, deve ser considerado um método para a produção teórica de pensamento que visa à construção de conceitos, a qual assume a forma de uma máxima lógica orientadora da conduta científica. Se um conceito é uma relação sígnica e a sua compreensão, um processo sígnico, encontramos na semiótica peirceana uma estratégia para discriminar as etapas da compreensão conceitual e avaliar os significados de termos e conceitos abstratos. Assim, o objetivo deste trabalho é explicar os princípios lógicos que governam os tipos de Signos que mais diretamente estão relacionados à construção de conceitos, a saber: o Símbolo-Remático, o Símbolo-Dicente e o Argumento. Tais signos correspondem à antiga divisão Termo, Proposição e Argumento, modificada de modo a ser aplicável aos signos em geral.
ABSTRACT: Pragmatism, as proposed by Charles S. Peirce, should be considered a method for the theoretical production of thought that aims the construction of concepts, which takes the form of a logical maxim guiding scientific conduct. If a concept is a sign relationship and its understanding a sign process, we find in Peircean semiotics a strategy to discriminate the stages of conceptual understanding and to evaluate the meanings of abstract terms and concepts. Therefore, the aim of this work is to explain the logical principles that govern the types of Signs that more directly are related to the construction of concepts, namely: the Rhematic Symbol, the Dicent Symbol and the Argument. Such signs correspond to the old division between Term, Proposition and Argument, modified in order to be applicable to the signs in general.

Introdução e objetivos: Micoses sistêmicas assumem grande importância nas situações de imunodepressão. A histoplasmose pode produzir diversas manifestações clínicas, podendo simular metástases. Objetivamos demonstrar a importância da confirmação histopatológica de lesão sugestiva de metástase em portadores de neoplasia maligna. Relatos dos casos: Dois pacientes com neoplasias em tratamento, nos quais tomografias de tórax de controle revelaram nódulos pulmonares compatíveis com metástases. As lesões foram biopsiadas, confirmando-se granulomas por histoplasmose. Conclusão: Em portadores de malignidade, aspectos clinicorradiológicos não se limitam à disseminação das neoplasias, sendo necessária a confirmação histopatológica de lesões sugestivas de metástases, excluindo-se outras doenças e evitando-se tratamentos inadequados.
Introduction and objectives: Systemic mycoses are of great importance in immunosuppression-associated conditions. Histoplasmosis can produce different symptoms that may simulate metastasis. We aim to demonstrate the importance of histopathological confirmation of lesions suggestive of metastases in patients with malignant neoplasm. Cases report: Two patients undergoing cancer treatment, whose chest computed tomography (CT) revealed pulmonary nodules, compatible with metastases. The lesions were biopsied, confirming histoplasmosis granuloma. Conclusion: In patients with cancer, clinical and radiological findings are not limited to metastatic dissemination of tumors, requiring histopathological confirmation of lesions suggestive of metastasis, excluding other diseases and preventing inappropriate treatment.

The detection of specific DNA sequences by polymerase chain reaction (PCR) has proved extremely valuable for the analysis of genetic disorders and the diagnosis of a variety of infectious disease pathogens. However, the application to the detection of Schistosoma mansoni is rare, despite a recommendation of the World Health Organization that a major focus of research on schistosomiasis should be on the development and evaluation of new strategies and tools for control of the disease. In this context, a few studies were published for the detection of the parasite in snails, monitoring of cercariae in water bodies, and diagnosis of human infection. The present minireview describes sensitive and specific PCR based systems to detect S. mansoni, indicating possible applications in the detection of snail infection, monitoring of transmission sites, and diagnosis of human infection.

Este trabalho notifica a ocorrência de um surto de esquistossomose aguda na praia de Porto de Galinhas, Pernambuco, Brasil, quando 662 pessoas tiveram diagnóstico positivo para Schistosoma mansoni. A infecção humana em massa ocorreu no feriado de 7 de setembro, quando chuvas pesadas provocaram a enchente do rio Ipojuca que invadiu as residências. A maioria dos casos agudos foi representada por famílias de residentes locais que tiveram exposição diária às cercárias durante três semanas, até que as águas baixassem. O inquérito malacológico aponta para a possibilidade dos caramujos vetores terem sido introduzidos na localidade com os aterros realizados em quintais, onde vieram a se reproduzir em criadouros peridomiciliares. A infecção dos moluscos se deu a partir de vazamentos de fossas. Os resultados da clínica e do laboratório apontam para cerca de 62% de casos agudos. Estudos complementares estão sendo realizados, para a melhor compreensão deste evento epidemiológico de significativa magnitude para os órgãos de saúde pública e impacto para a população afetada.
We recently confirmed several cases of acute schistosomiasis in Porto de Galinhas beach, Northeast Brazil. A total of 662 patients were diagnosed by parasitological and clinical examinations. The infection likely occurred during the September 7 national holiday, when heavy rainfall flooded the Ipojuca River and people were infected when the water covered their yards. Families were continuously exposed to infection for a period of three weeks until the water had completely dried up. Previous investigation suggests that snail vectors were introduced as a result of landfill in marshy areas. The swamp-flooding of such areas facilitated the emergence of slums surrounded by snail breeding sites. Heavy rainfall caused open-air sewage ditches to overflow, allowing for infection of snails by Schistosoma mansoni. Thus, continuous floods were responsible for the spread of human infection. Clinical and laboratory results identified 62% of acute cases of S. mansoni. Complementary studies are being conducted to define the impact and epidemiological meaning of the acute schistosomiasis outbreak.

Padrão epidemiológico da esquistossomose em comunidade de pequenos produtores rurais de Pernambuco, Brasil

Este artigo é parte de um estudo mais amplo que empregou abordagens epidemiológica e antropológica para procurar entender a produção e manutenção da esquistossomose em pequena comunidade de área endêmica de Pernambuco. No presente trabalho, realizamos um estudo de corte transversal para avaliação dos fatores de risco para esquistossomose, associando variáveis sócio-econômicas, sanitárias e comportamentais (quantificáveis), com a prevalência e intensidade da infecção. Uma análise estatística univariada selecionou três variáveis (faixa etária, escolaridade e número de contatos), que mostraram associação significativa com a positividade para S. mansoni. Os resultados da análise multivariada realizada a seguir permitiram a identificação dos fatores causais e das variáveis de confundimento, entre aquelas anteriormente selecionadas. O diagnóstico epidemiológico quantitativo é discutido criticamente, com embasamento no estudo qualitativo etnográfico, quanto às condições ambientais de risco (contaminação, transmissão) e práticas de risco (atividades econômicas e comportamentais).
This article is part of a larger study in which epidemiological and anthropological methods were used to help understand the production and maintenance of schistosomiasis in a small endemic area in Pernambuco State. A cross-sectional study identified several local risk factors for schistosomiasis, quantifying socioeconomic, sanitary, and behavioral variables and then relating them to the prevalence and intensity of schistosomiasis infection. Using univariate analysis, three variables (age group, schooling, and human/water contact) showed significant association with S. mansoni infection. Causal factors and confounding variables were identified through multivariate analysis. Quantitative epidemiological analysis is critically discussed regarding a qualitative ethnographic study in relation to environmental risk situations (contamination and transmission) as well as risk practices (economic and behavioral).

Formas hepatosplênicas da esquistossomose mansônica no interior do Nordeste do Brasil

Estudos de campo sobre a morbidade da esquistossomose mansônica foram realizados em algumas áreas rurais do interior do nordeste do Brasil durante o primeiro semestre de 1991. Estes estudos seccionais foram limitados à determinação do gradiente clinico da endemia. Foram examinadas todas as populações presentes neste levantamento feito casa-por-casa. Os resultados mostraram a redução da prevalência da forma mais grave da esquistossomose - a forma hepatosplênica - quando comparados com estudos semelhantes feitos nas mesmas áreas durante as décadas de 60 e 70. As taxas de prevalência das formas clínicas graves da doença foram deslocadas dos grupos de idade mais jovens para os grupos mais velhos.
Field studies on the morbidity of schistosomiasis mansoni were carried out in some rural endemic areas of Northeastern Brazil during the first semester of 1991. These cross-sectional studies were limited to the assessment of the gradient of clinical forms of the disease. The entire population present in a house-to-house survey was examined. Results have shown a reduction in the prevalence rates of the more severe form of schistosomiasis, the hepatosplenic form, when compared with previous similar studies made in the same areas during the 1960s and 1970s. Changes in prevalence rates of the severe clinical forms from the younger to the older age groups were observed.
