Abstract Interventions based on pain education and self-management are dependent on factors such as attention, memory, concentration, and executive function. Objective To explore the relationship between cognitive performance and the variables pain intensity, central sensitization, catastrophizing, and hypervigilance in women diagnosed with chronic pain-related TMD. Methodology This is a cross-sectional study. A total of 33 women (mean age: 38±4.6 years; range: 18 to 66 years) with chronic pain-related TMD (myalgia and/or arthralgia) diagnosed according to the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD). Specific questionnaires were used to evaluate cognitive performance, overall pain intensity, central sensitization, hypervigilance, and pain catastrophizing. The data were analyzed using Pearson’s correlation coefficient and backward stepwise multiple linear regression (statistical significance at 5% alpha). Results Approximately 53% of the study sample showed decreased cognitive performance. High central sensitization, hypervigilance, and pain catastrophizing were observed. A significant negative correlation was observed between cognitive performance and hypervigilance (p=.003, r=−.49), cognitive performance and catastrophizing (p<.001, r=−.58), and cognitive performance and pain intensity (p<.001, r=−.58). Regarding the partial regression coefficients, only catastrophizing and pain intensity showed statistical significance (t=−2.12, p=.043; t=−2.64, p=.014, respectively), indicating a significant role in explaining cognitive performance at the sample. Conclusion High pain intensity and the presence of catastrophic thoughts regarding pain can predict impaired cognitive performance in women with chronic pain-related TMD. RESUMO Objetivo: Investigar a incidência de disfunção temporomandibular dolorosa (DTM) e Síndrome de Burnout (SB) em estudantes de Odontologia, com base na percepção subjetiva de sinais e sintomas, e verificar se há uma relação de coexistência entre estas duas condições. Métodos: 57 estudantes (8º período, média de idade de 23 anos), 39 mulheres e 18 homens, responderam ao Inventário de Burnout de Maslach – Versão para Estudantes (MBI-SS), ProDTMMulti e foram avaliados quanto à presença de DTM (DC/TMD). Resultados: A incidência de DTM foi de 22% e de SB de 8,7%. Dentre os estudantes com SB, 80% apresentavam diagnóstico de DTM, bem como maior percepção subjetiva de sinais e sintomas de DTM e maior tempo de duração da dor. Estes estudantes também apresentaram escores mais elevados quanto à percepção de sinais e sintomas em situações que envolviam as funções orofaciais. Conclusão: Parece haver uma relação clínica de coexistência entre a SB e a DTM dolorosa na amostra estudada, estudantes de Odontologia com SB e elevados escores no MBI-SS estão mais propensos a apresentarem DTM dolorosa.

ABSTRACT Objective: To investigate the incidence of painful temporomandibular disorders (TMD) and Burnout Syndrome (BS) in Dentistry students, based on the subjective perception of signs and symptoms, and to verify if there is a coexistence relation between these conditions. Methods: 57 students (8th period of the dentistry course, 23 years mean age), 39 women and 18 men, responded to the Maslach Burnout Inventory Questionnaire – Students Survey (MBI-SS), ProTMDMulti and were evaluated for the presence of TMD diagnosis (DC/TMD). Results: The incidence of TMD was 22% and BS was 8.7%. Among the students with BS, 80% had a TMD diagnosis, as well as a greater subjective perception of TMD signs and symptoms and a longer duration of pain. They also showed higher scores of signs/symptoms perception for situations involving orofacial function. Conclusion: There seems to be a clinical coexistence between the BS and painful TMD in the studied sample, Dentistry students with BS and high scores on the MBI-SS are more likely to have painful TMD.

RESUMO JUSTIFICATIVA E OBJETIVOS: A busca por serviços de atendimento a pacientes com disfunção temporomandibular e dor orofacial tem crescido nas últimas décadas em função do maior acesso à informação e maior demanda por tratamento. O objetivo deste estudo foi analisar o perfil de atendimento a pacientes com disfunção temporomandibular e dor orofacial de um serviço universitário ao longo de 10 anos. MÉTODOS: Foram coletados dados quantitativos do atendimento de disfunção temporomandibular e dor orofacial da Faculdade de Odontologia de Ribeirão Preto a partir do Sistema de Informatização de Clínicas Odontológicas da Universidade de São Paulo (ROMEU) relativos a um período de 10 anos (2006 a 2016): total de pacientes atendidos, altas, faltas, encaminhamentos para outras disciplinas, placas oclusais, orientações de automanejo, terapias fonoaudiológica e fisioterapêutica, sessões de laserterapia de baixa intensidade e exames de eletromiografia. Os dados foram analisados de forma descritiva. RESULTADOS: Houve um aumento no número de pacientes atendidos ao longo dos anos, além de um aumento das altas e redução de encaminhamentos para outras disciplinas. Foi também verificado um aumento crescente dos procedimentos de educação em dor e do número de atendimentos fonoaudiológicos e fisioterapêuticos. As placas oclusais acompanharam o aumento de atendimentos. CONCLUSÃO: Ao longo de 10 anos houve um aumento de atendimentos, uma crescente valorização de práticas voltadas para educação em dor, orientações de automanuseio e atuação multidisciplinar, além disso a placa oclusal se mostrou uma opção terapêutica ainda bastante utilizada na odontologia.

ABSTRACT BACKGROUND AND OBJECTIVES: The search for healthcare services for the treatment of temporomandibular disorders and orofacial pain has increased in the last decades due to the higher access to information and demand for treatment. The objective of this study was to analyze the profile of the service of caring for patients with temporomandibular disorders and orofacial pain of a university service over 10 years. METHODS: The quantitative data on temporomandibular disorders and orofacial pain care were collected from the School of Dentistry of Ribeirão Preto, from the Sistema de Informatização de Clínicas Odontológicas da Universidade de São Paulo (ROMEU) for a period of 10 years (2006 to 2016). The data included the total of patients assisted, discharges, absences, referrals to other disciplines, occlusal splints, self-management orientations, speech therapy and physiotherapy, low-intensity laser therapy sessions and electromyography tests. The data were descriptively analyzed. RESULTS: There was an increase in the number of patients assisted over the years, in addition to an increase in discharges and reduction of referrals to other disciplines. There was also increased use of pain education procedures and in the number of speech therapy and physiotherapy provided. The occlusal splints followed the increase in the number of patients assisted. CONCLUSION: Over 10 years there was an increase in the number of patients assisted, an increasing appreciation of the practices directed at pain education, self-management orientations, and multidisciplinary action. Also, the occlusal splint was shown to be a therapeutic option still widely used in dentistry.

RESUMO JUSTIFICATIVA E OBJETIVOS: A disfunção temporomandibular e a síndrome de Burnout são influenciadas por fatores emocionais. O objetivo deste estudo foi analisar de forma prospectiva as dimensões da síndrome de Burnout, e sua relação com a percepção dos sinais e sintomas de disfunção temporomandibular em uma amostra de estudantes de graduação em Odontologia. MÉTODOS: A amostra foi composta por alunos de graduação em odontologia (23±2.5 anos), os quais responderam aos questionários Inventário de Burnout de Maslach - versão estudantes e o ProDTMMulti. Os questionários foram respondidos anualmente, considerando-se o primeiro, segundo e terceiro ano do curso (2013/2014/2015). A análise estatística comparou as dimensões da síndrome de Burnout entre os gêneros e testou a sua correlação com o escore total do ProDTMMulti, bem como a variação entre os escores das dimensões dessa síndrome ao longo dos anos por gênero. RESULTADOS: Considerando a amostra total houve aumento na frequência da zona de risco para a síndrome de Burnout com o passar dos anos. Observou-se diferença significativa entre homens e mulheres para exaustão emocional nos três anos avaliados (p=0,002, p=0,003, p=0,01, respectivamente) e ainda para a descrença no último ano (p=0,02), sendo mais altos para as mulheres. Houve correlação significativa dos escores do ProDTMMulti com a exaustão emocional nas mulheres no primeiro e segundo ano avaliados (p=0,0004, p=0,04). CONCLUSÃO: Parece existir uma correlação entre a exaustão emocional e a percepção de sinais e sintomas de disfunção temporomandibular no gênero feminino e uma tendência de aumento dos escores das dimensões da síndrome de Burnout com a progressão do curso.

ABSTRACT BACKGROUND AND OBJECTIVES: Temporomandibular disorder and Burnout syndrome are influenced by emotional factors. This study aimed at prospectively evaluating Burnout syndrome dimensions and their relationship with temporomandibular disorder signs and symptoms in a sample of dentistry graduating students. METHODS: Sample was made up of dentistry graduating students (23±2.5 years), who have answered the Maslach Burnout Inventory questionnaire, students survey, and the ProDTM-Multi. Questionnaires were answered once a year, considering first, second and third year of the course (2013/2014/2015). Statistical analysis has compared Burnout syndrome dimensions between genders and has tested their correlation with total ProDTMMulti score, as well as variations between scores of this syndrome dimensions along the years by gender. RESULTS: Considering total sample, there has been increase in risk zone frequency for Burnout syndrome as years went by. There has been significant difference between males and females for emotional exhaustion in the three evaluated years (p=0.002, p=0.003, p=0.01, respectively) and also for disbelief in the last year (p=0.02), being higher for females. There has been significant correlation of ProTD-MMulti scores and emotional exhaustion among females in the first and second evaluated years (p=0.0004, p=0.04). CONCLUSION: It seems to be correlation between emotional exhaustion and perception of temporomandibular signs and symptoms among females, and a trend to Burnout syndrome dimensions scores increase as course progressed.

<p>Falhas podem ocorrer em conexões de implantes dentários, em especial em hexágonos externos (EH). Devido à ocorrência de deformação nesta porção dos implantes, este estudo objetivou avaliar os níveis de deformação de conexões EH submetidas ao torque interno. Dois tipos de implantes foram utilizados: grupo N e grupo S. Foram aplicados torques de 0, 32, 45, 60 e 80 Ncm nos implantes do grupo N e torques de 0, 30, 40, 60 Ncm e infinito nos implantes do grupo S. Medidas referentes à distância interna (ID), área interna (AI) e área externa (AE) foram obtidas por meio de fotos digitais analisadas em software. A análise estatística foi feita pelo teste de Scott-Knott. Os resultados demonstraram que quanto maior o torque aplicado, maior a alteração de todas as dimensões avaliadas em ambos os grupos. No grupo S, torques iguais ou superiores a 40 Ncm e 30 Ncm causaram maior deformação na AE e AI respectivamente, enquanto no grupo N, torques iguais ou superiores a 60 Ncm e 32 Ncm causaram maior deformação na AE e AI respectivamente. Os níveis de deformação foram maiores no grupo S em comparação ao grupo N. Nossos resultados indicam que a AI, a AE e a DI do EH podem ser influenciadas pelos diferentes torques internos.</p>

<p>Failures may occur in the connections of dental implants, especially in external hexagon (EH). Due to the deformations in this portion of implants, this study aimed to evaluate the levels of deformation of EH connections subjected to internal toque. Two types of implants were used: N group and S group. Torques of 0, 32, 45, 60 and 80 Ncm were applied to the N group, and torques of 0, 30, 40, 60 and infinite Ncm were applied to the S group implants. The internal distance (ID), internal area (IA) and external area (EA) of the EH were obtained from digital pictures, which were analyzed by a specific software. Statistical analysis was performed by the Scott-Knott test. The results showed that the higher the torque applied, the greater were the changes in the evaluated dimensions in both groups. In the S group, torque levels equal or greater than 40 Ncm and 30 Ncm caused greater deformation of EA and IA respectively, while in the N group, torque levels equal or greater than 60 Ncm and 32 Ncm caused greater deformation of EA and IA respectively. Levels of deformation were greater in the S group as compared with the N group. These findings suggest that the IA, EA and ID of the EH may be affected by different internal torque levels.</p>

OBJECTIVE: To evaluate the effects of the levels of glycemic control on the frequency of clinical complications following invasive dental treatments in type 2 diabetic patients and suggest appropriate levels of fasting blood glucose and glycated hemoglobin considered to be safe to avoid these complications. METHOD: Type 2 diabetic patients and non-diabetic patients were selected and divided into three groups. Group I consisted of 13 type 2 diabetic patients with adequate glycemic control (fasting blood glucose levels <140 mg/dl and glycated hemoglobin (HbA1c) levels <7%). Group II consisted of 15 type 2 diabetic patients with inadequate glycemic control (fasting blood glucose levels >140 mg/dl and HbA1c levels >7%). Group III consisted of 18 non-diabetic patients (no symptoms and fasting blood glucose levels <100 mg/dl). The levels of fasting blood glucose, glycated HbA1c, and fingerstick capillary glycemia were evaluated in diabetic patients prior to performing dental procedures. Seven days after the dental procedure, the frequency of clinical complications (surgery site infections and systemic infections) was examined and compared between the three study groups. In addition, correlations between the occurrence of these outcomes and the glycemic control of diabetes mellitus were evaluated. RESULTS: The frequency of clinical outcomes was low (4/43; 8.6%), and no significant differences between the outcome frequencies of the various study groups were observed (p>0.05). However, a significant association was observed between clinical complications and dental extractions (p = 0.02). CONCLUSIONS: Because of the low frequency of clinical outcomes, it was not possible to determine whether fasting blood glucose or glycated HbA1c levels are important for these clinical outcomes.

The Lucia jig is a technique that promotes neuromuscular reprogramming of the masticatory system and allows the stabilization of the mandible without the interference of dental contacts, maintaining the mandible position in harmonic condition with the musculature in normal subjects or in patients with temporomandibular dysfunction (TMD). This study aimed to electromyographically analyze the activity (RMS) of the masseter and temporal muscles in normal subjects (control group) during the use of an anterior programming device, the Lucia jig, in place for 0, 5, 10, 20 and 30 minutes to demonstrate its effect on the stomatognathic system. Forty-two healthy dentate individuals (aged 21 to 40 years) with normal occlusion and without parafunctional habits or temporomandibular dysfunction (RDC/TMD) were evaluated on the basis of the electromyographic activity of the masseter and temporal muscles before placement of a neuromuscular re-programming device, the Lucia jig, on the upper central incisors. There were no statistically significant differences (p < 0.05) in the electromyographic activity of the masticatory muscles in the different time periods. The Lucia jig changed the electromyographic activity by promoting a neuromuscular reprogramming. In most of the time periods, it decreased the activation of the masticatory muscles, showing that this device has wide applicability in dentistry. The use of a Lucia jig over 0, 5, 10, 15, 20 and 30 minutes did not promote any statistically significant increase in muscle activity despite differences in the data, thus showing that this intra-oral device can be used in dentistry.

A perda de dentes posteriores pode provocar o desequilíbrio do sistema estomatognático. As próteses parciais removíveis terapêuticas atuam como terapia reversível e não dispendiosa para os pacientes com desgastes dentais severos. Este trabalho apresenta um tratamento com as próteses parciais removíveis terapêuticas (overlay) em um paciente de 55 anos, do gênero masculino que apresentava atrição severa nos dentes superiores e inferiores, dor na articulação temporomandibular e dimensão vertical de oclusão reduzida. O tratamento consistiu em restabelecer a dimensão vertical de oclusão utilizando o Jig de Lucia, próteses parciais removíveis com reconstrução dos dentes desgastados e sem preparos. Esta terapia pode ser utilizada como uma alternativa de tratamento para a obtenção de estética, função e estabilidade oclusal em pacientes com desgastes dentais severos.

Loss of posterior teeth may cause an imbalance in the stomatognathic system. Overlay removable partial dentures (ORPD) are a reversible and relatively inexpensive treatment for patients with severely worn teeth. This paper presents a treatment with ORPD in a 55-year-old male patient who had severe attrition in the maxillary and mandibular teeth, temporomandibular joint pain and reduced vertical dimension of occlusion (VDO). The treatment consisted in the reestablishment of the VDO using Lucia’s jig, fabricating removable partial denture with reconstruction of the worn teeth without preparation. This therapy can be used as an alternative treatment to provide esthetic, function and stable occlusion in patients with severely worn teeth.

A integridade estética/funcional do tecido periodontal pode ser comprometida pela perda do elemento dental. Os implantes imediatos tornaram-se uma opção viável na manutenção da arquitetura periodontal, desde que haja a compatibilidade com o alvéolo e a possibilidade da contaminação local ser totalmente eliminada. O objetivo deste estudo foi descrever o procedimento de instalação de implantes imediatos na região anterior de maxila usados na reposição de dentes com lesões crônicas periapicais, discutindo a relação entre o procedimento e lesões periapicais. A remoção dos dentes condenados (11, 12 e 21) foi feita de forma conservadora a fim de preservar a estética e arquitetura periodontal. Um segundo acesso cirúrgico foi obtido a nível apical, permitindo: o debridamento da loja cirúrgica, orientação visual dos implantes e o preenchimento com enxerto ósseo bovino. A loja óssea foi coberta por uma membrana absorvível e uma prótese parcial removível provisória foi instalada e após 6 meses os implantes foram reabilitados. Após 3 anos de reabilitação, os implantes se apresentam em condições satisfatórias de estética/função, sugerindo que a instalação de implantes imediatos combinados com a regeneração tecidual guiada em lesões periapicais crônicas podem ser indicados para repor dentes perdidos devido a lesões periapicais crônicas com história de insucesso endodôntico na maxila anterior.

The esthetics and functional integrity of the periodontal tissue may be compromised by dental loss. Immediate implants became a viable option to maintain the periodontal architecture because of their anatomic compatibility with the dental socket and the possibility of eliminating local contamination. This article describes the procedure of immediate implant placement in the anterior maxilla replacing teeth with chronic periapical lesions, which were condemned due to endodontic lesions persisting after failed endodontic treatment and endodontic surgery, and discusses the relationship between the procedure and periapical lesions. Surgical removal of hopeless teeth 11, 12 and 21 was performed conservatively in such a way to preserve the anatomy and gingival esthetics. A second surgical access was gained at the apical level, allowing the debridement of the surgical chamber for elimination of the periapical lesion, visual orientation for setting of the implants and filling of the surgical chamber with xenogenous bovine bone graft. After this procedure, the bone chamber was covered with an absorbent membrane and the healing screws were positioned on the implants. Later, a provisional partial removable denture was installed and the implants were inserted after 6 months. After 3 years of rehabilitation, the implants present satisfactory functional and esthetic conditions, suggesting that immediate implant placement combined with guided bone regeneration may be indicated for replacing teeth lost due to chronic periapical lesions with endodontic failure history in the anterior maxilla.

Os sinais e sintomas de DTM podem contribuir na redução da força de mordida e da atividade muscular. O objetivo deste trabalho foi comparar a força de mordida em indivíduos com prótese total com DTM (Grupo CDTM) e sem DTM (Grupo SDTM). O Grupo CDTM (n=9) era composto por indivíduos que utilizavam as mesmas próteses totais há mais de 10 anos. O Grupo SDTM (n=9) por indivíduos com próteses totais satisfatórias quanto aos requisitos estruturais e funcionais, observados pelo profissional e relatados pelo paciente. O índice de Helkimo foi utilizado para análise do índice de disfunção. A força de mordida máxima foi medida na região de molares com gnatodinamômetro. Os resultados mostraram que o Grupo CDTM apresentou valor numérico médio de força de mordida menor que o Grupo SDTM, porém sem diferença estatisticamente significante entre os grupos avaliados (p<0,05) sugerindo que os sinais e sintomas da DTM e as condições estruturais das próteses não afetaram os resultados da força de mordida.

The signs and symptoms of temporomandibular dysfunction (TMD) may contribute to reduce bite force and muscular activity. The aims of this study were to compare bite force in complete denture wearers with TMD (TMD group) and without TMD (healthy group).The TMD group consisted of 9 individuals, who had worn a maxillary and a mandibular complete removable denture for more than 10 years. The healthy group consisted of 9 participants who wore dentures and had satisfactory interocclusal and maxillomandibular relationship. Helkimo Index was used to analyze the dysfunction level. Maximum bite force was measured using a digital dynamometer with capacity of 100 kgf and adapted to oral conditions.The TMD group presented smaller mean bite force values than the healthy group, though without statistical significance (p>0.05). This outcome suggests that the TMD signs and symptoms and the structural conditions of the dentures did not affect the maximal bite force of complete denture wearers.
