A detecção de Leptospira pela técnica de PCR não havia sido descrita em serpentes. Este estudo investigou pelo teste de aglutinação microscópica (MAT) e PCR, a presença de anticorpos anti-Leptospira spp. e Leptospira spp., respectivamente, em serpentes peçonhentas e não peçonhentas de vida livre e de cativeiro. As serpentes foram divididas em três grupos para comparação: Grupo 1 (serpentes recém-chegadas da natureza - WS); Grupo 2 (serpentes em regime de cativeiro intensivo -IC) e Grupo 3 (serpentes em regime de cativeiro coletivo semi-extensivo - CC). Do total de 147 serpentes estudadas, 52 (35,4%) foram positivas para leptospirose pelo MAT, as quais 8 (15,4%) pertenciam ao Grupo 1 (WS), 34 (65,4%) ao Grupo 2 (IC) e 10 (19,2%) ao Grupo 3 (CC). Das espécies estudadas, a jararaca (Bothrops jararaca) apresentou maior soropositividade (66,7%, N=22/33). O sorovar mais prevalente foi o Hardjo prajtino (88,5%, N=46/52) e os títulos variaram de 100 a 3200. Leptospira interrogans foi revelada por PCR nos rins e no fígado de caiçaca (Bothrops moojeni) e de jararaca-pintada (Bothrops pauloensis), mostrando 100% e 93% de identidade, respectivamente. Futuros estudos devem ser realizados para melhor compreensão do papel das serpentes como reservatório de leptospiras na natureza.

Detection of Leptospira by PCR had not yet been described in snakes. This study investigated, by microscopic agglutination test (MAT) and PCR, the presence of antibodies to Leptospira spp. and Leptospira spp., respectively, in venomous and non-venomous wildlife and captivity snakes. All snakes were divided into three groups to be compared: Group 1 (wildlife snakes - WS); Group 2 (snakes in intensive captivity - IC), and Group 3 (collective semi-extensive captivity -CC). Of the 147 snakes studied, 52 (35.4%) were positive for leptospirosis by MAT, 8 (15.4%) belonging to Group 1 (WS), 34 (65.4%) to Group 2 (IC) and 10 (19.2%) to Group 3 (CC). Jararaca (Bothrops jararaca) presented the highest average titer (66.7%, N=22/33) among the three group studied, and Hardjo prajtino was the most prevalent serovar (88.5%, N=46/52), with titers varying from 100 to 3200. Leptospira interrogans was revealed by PCR in kidney and liver of caiçaca (Bothrops moojeni) and jararaca-pintada (Bothrops pauloensis), showing 100% and 93% identity respectively. Future studies should be carried out for better understanding of the role of snakes as a reservoir of Leptospira in nature.

Este estudo avaliou e comparou a microflora aeróbica da cavidade oral, cloaca e veneno de serpentes Crotalus durissus terrificus recém-capturadas da natureza e mantidas sob quarentena (WQ), mantidas em cativeiro coletivo (CC) e em cativeiro individual (IC). A eficácia de drogas antimicrobianas de agentes isolados foi também avaliada. Foram isolados microorganismos dos grupos I, II e III respectivamente: 29 (63.04%), 38 (90.48%) e 21 (42.86%) da cloaca; 15 (32.61%), 3 (7.14%) e 25 (51.02%) da cavidade oral, e finalmente 2 (4.35%), 1 (2.38%) e 3 (6.12%) do veneno. As bactérias mais frequentes foram Pseudomonas aeruginosa, Proteus vulgaris e Morganella morganii, com sensibilidade para amikacina, gentamicina, norfloxacina, sulfazotrina e tobramicina. Serpentes mantidas no cativeiro semi-aberto mostraram menor número de agentes infecciosos em cavidade oral, talvez devido ao ambiente de cativeiro com diferentes gradientes de temperatura, água corrente, ausência de manejo diário, ampla circulação de ar, possibilidade de movimentação pelos animais, limpeza diária e acesso ao Sol.

The study evaluated and compared the aerobic microbiota from the oral cavity, cloaca and venom of Crotalus durissus terrificus snakes, recently caught from the wild and kept under quarantine (WQ), individual captivity (IC) and collective captivity (CC). Antimicrobial drug effectiveness on isolated agents also was assayed. From group I, II and III were isolated, respectively, 29 (63.04%), 38 (90.48%) and 21 (42.86%) microorganisms from the cloaca; 15 (32.61%), 3 (7.14%) and 25 (51.02%) microorganisms from the oral cavity; and, 2 (4.35%), 1 (2.38%) and 3 (6.12%) microorganisms from venom. The most frequent bacteria were Pseudomonas aeruginosa, Proteus vulgaris and Morganella morganii, with sensitivity to amikacin, gentamicin, norfloxacin, sulfazotrin and tobramycin. Snakes kept in semi-open captivity exhibited the fewest microorganisms in oral cavities, perhaps due to the environment in captivity, with different temperature gradients, running water, absence of daily handling, circulating air, possibility of moving around, daily cleaning, and sunlight access.

Fibrin glue has been researched as an alternative method for tissue synthesis and is known for its capability to promote hemostasis at the application site, good approximation of wound edges and fast healing. The current study consisted in the application of fibrin glue derived from snake venom as treatment for experimental corneal ulcers. Twenty-one dogs had their corneas experimentally prepared through lamellar keratectomy (of standardized diameter and depth). Animals were divided into seven groups of three animals each. Six experimental groups were periodically evaluated and collection was carried out on the 1st, 3rd, 7th, 15th, 30th and 60th post-operative days, whereas one control group was evaluated throughout the experiment. Analyses consisted in the clinical evolution and in the histopathological study of all operated on eyes. Results indicated that fibrin glue was efficient in repairing keratectomy wounds in dogs and contributed to an earlier healing phenomenon, avoiding edema formation and keeping corneal clearness. The use of fibrin glue derived from snake venom showed to be easy to apply, feasible with animal models and of low cost, avoiding the lesion progress and allowing fast and appropriate corneal healing.

Rabies is considered a fatal disease once clinical symptoms have developed. The aim of this study was to evaluate epidemiological aspects and immune response in patients attacked by domestic and wild animals and subjected to post-exposure rabies treatment with equine serum and associated vaccine. Thirty-three patients were evaluated; they were between 13 and 65 years old, 75.8% were male and 24.2% female, and from the Botucatu neighborhood. Twenty healthy control individuals with the same age range were also studied. Specific antibodies to equine immunoglobulins and IFN-gamma, IL-2, IL-4, and IL-10 production were evaluated by ELISA. IgM, IgE, IgG and subclasses, and rabies virus antibodies serum levels were determined by nephelometry and seroneutralization methods, respectively. No anaphylactic or serum sickness allergic reactions were observed in patients after treatment. Anti-equine IgG levels were significantly higher than those of IgM after 14 and 28 days of treatment. Protective antibodies to rabies virus > 0.5 UI/ml were detected in 84.6% and 75% of patients at days 14 and 28, respectively. IFN-gamma, IL-2 and IL-10 levels in patients before and 48h after treatment were significantly higher than in controls suggesting that both Th1 and Th2 cells were activated in the patients. Serum IgM levels were higher at day 14, and IgG2 and IgE levels were higher at day 28 of treatment. These results suggest that post-exposure rabies treatment in humans induces significant alterations in patient immune response characterized by increased levels of cytokines, serum levels of specific rabies virus antibodies, and the equine serum components employed in the treatment.

Laboratory profile of young ovines was studied in order to evaluate and compare their antiserum production from natural and Cobalt-60 irradiated Crotalus durissus terrificus (C.d.t.) venoms. The parameters analyzed included complete blood count, and urea, creatinine, aspartate aminotransferase, total proteins, albumin and globulin serum measurements. Three groups of six animals each were used. Group 1 (G1) received natural C.d.t. venom; Group 2 (G2) received irradiated C.d.t. venom; and Group 3 (G3) was used as control and did not receive venom, only adjuvants, using seven venom inoculations. During the experimental period, animals were fortnightly weighed. According to clinical and weight evaluation, sheep in post-weaning phase showed no changes in their physiological profiles but had excellent weight gain. The parameters analyzed were not statistically different (p<5%) among the groups tested. The hyperimmunization process was successfully accomplished with the production of specific antibodies against Crotalus durissus terrificus venom. Results bring a new possibility of utilizing ovines in the commercial production of anticrotalic serum, which may be used to treat human and animal envenomation. Its production cost may be reduced by subsequent use of hyperimmunized sheep for human consumption.

ELISA was used to evaluate, accompany, and compare the humoral immune response of Swiss mice during hyperimmunization with native and Cobalt-60-irradiated (60Co) venoms of Bothrops jararaca, Bothrops jararacussu and Bothrops moojeni. Potency and neutralization were evaluated by in vitro challenges. After hyperimmunization, immunity was observed by in vivo challenge, and the side effects were assessed. The animals immunization with one LD50 of each venom occurred on days 1, 15, 21, 30, and 45, when blood samples were collected; challenges happened on the 60th day. Results showed that ELISA was efficient in evaluating, accompanying and comparing mouse immune response during hyperimmunization. Serum titers produced with natural venom were similar to those produced with irradiated venom. Immunogenic capacity was maintained after 60Co-irradiation. The sera produced with native venom showed neutralizing potency and capacity similar to those of the sera produced with irradiated venom. All antibodies were able to neutralize five LD50 from these venoms. Clinical alterations were minimum during hyperimmunization with irradiated venom, however, necrosis and death occurred in animals inoculated with native venom.

Rabies is a viral acute encephalitis of progressive and fatal outcome, particular of hot-blooded animals, and accidentally affecting men. Since it is a zoonosis with different animal species acting as a reservoir in the nature, this disease is a great public health problem in several countries in development. Prophylactic treatments for human rabies started in 1885 with Louis Pasteur, and developed in order to provide higher protection and lower incidence of side effects. Today, treatments of pre and post-exposure to the virus are well established, with excellent results of protection for individuals exposed to animals potentially contaminated by the rabies virus. These treatments consist of utilising the vaccine isolatedly or in combination with equine immunoglobulin, what contributes, in an important way, to the decrease in the number of cases of rabies.

This study evaluated the impact of multimedia in the information transfer of subjects specific to Tropical Diseases - Tetanus and Snake Envenoming. We evaluated the autonomous learning process of 76 fourth-year medical students at Botucatu School of Medicine of UNESP, using printed matter, video, and CD-ROM. The students were submitted to a specific test, which was repeated approximately one week later. They were divided into groups and received a kit containing a textbook, a video, and a CD-ROM. These materials were used for out-of-class study. Before the second test, the students gave a seminar,where they discussed and resolved their doubts with their professor. The results of the first test showed averagesbetween 4.27±1.41 and 6.41±1.61. The second test, given after the seminar, presented averages that increased to 8.41±0.76 and 9.52±0.42, significance a = 5%. At the end of the course, the students answered a questionnaire, which evaluated the material quality and acquired knowledge. The students concluded that multimedia was a more efficient and quick means for knowledge building than traditional teaching materials. They said that the active participation and interactivity with the CD-ROM were the major differences. The authors continue to study the associated use of printed matter, video, and CD-ROM as a faster alternative to the traditional method of information transfer, which may be of help in the knowledge building process in medical education.

The objective of this study was to detect the presence of local edema and hemorrhage caused by Crotalus durissus terrificus envenoming using magnetic resonance imaging (MRI). Six patients bitten by Crotalus durissus terrificus snakes were treated at the Emergency Unit and Tropical Diseases Unit of Botucatu School of Medicine, UNESP, São Paulo State, Brazil. After receiving specific serotherapy, the patients were submitted to MR of the bite site. Post contrast T1 and T2 spin-echo MRI were obtained revealing the following lesions: edema associated with hemorrhage in subcutaneous and muscular tissue (n=3), edema in subcutaneous tissue (n=2), and perimuscular hemorrhage (n=1). In this study, MR demonstrated a local effect of Crotalus durissus terrificus envenoming in inducing edema and hemorrhage, mainly in muscular tissues and perimuscular areas. These results indicate that Crotalus durissus terrificus venom cause a local muscular tissue damage in human envenoming represented by edema and hemorrhage.

The objective of this clinical study was to evaluate local tissue damage caused by Bothrops sp envenoming in relation to lesion type and damaged tissues using magnetic resonance imaging (MRI). Fifteen patients bitten by Bothrops snakes were treated at the Emergency Unit of the Tropical Diseases Unit at the University Hospital, Botucatu School of Medicine, UNESP, São Paulo State, Brazil. After receiving specific serotherapy, the patients were submitted to MR of the bite site. T1 spin-echo MRI were obtained revealing the following lesions: edema (n=9), edema associated with hemorrhage (n=5), and hemorrhage (n=1). Perimuscular areas (n=6) and subcutaneous tissues (n=5) were the most affected, followed by muscular tissues (n=4). It is important to mention that MRI did not show myonecrosis of the bite site, a widely reported finding in anatomical and histopathological experimental studies.

The authors report a case of bothropic envenoming in a male Cocker Spaniel. The animal was bitten in the ventral thoracic region, receiving treatment 4 hours later. Clinical examination revealed an extensive, painful and area of firm edema, absence of local or systemic hemorrhage, without evident neurological alterations. Clinical diagnosis was mild bothropic envenoming. Treatment consisted of 5 vials of polyvalent snake antivenom, two vials administered intravenously and three subcutaneously. Blood clotting time was always within normal values. Two days after envenoming, the animal showed hyperthermia and received enrofloxacin (5mg/kg/24h) for 10 days and ketoprofen (1mg/kg/24h) for 5 days. Seventy-two hours after envenoming, extensive subcutaneous, muscle fiber, and skin necrosis of approximately 10 cm in diameter was observed. After débridement of necrotic tissues, the area was cleaned with antiseptic solutions. Complete healing was observed 55 days after envenoming. The authors discuss whether heterologous serotherapy is effective in preventing tissue necrosis after bothropic envenoming.

This study evaluated the impact of printed matter, video, and multimedia on the learning/teaching process in Tropical Diseases. Eighty-four of 90 fourth-year medical students at Botucatu School of Medicine of UNESP were evaluated. The students received a kit containing a textbook, a video, and a CD-ROM on the Clinical Study of Tetanus to prepare a seminar on the subject. They were then asked to complete a questionnaire, which led to the following conclusions: 67.86% read the textbook, 91.66% watched the video, and 77.38% explored the CD-ROM. These results were obtained observing the total number of students using each different media. When asked which of these media contributed most, the CD-ROM came out on top. The authors stress that this learning teaching process motivated the students by opening possibilities for new teaching alternatives in medicine.

The authors have performed a literature review of surgical adhesives, such as cyanoacrylate, collagen gelatin, and fibrin glue. They have included different types of commercial and non-commercial fibrin sealants and have reported on the different components in these adhesives, such as fibrinogen, cryoprecipitate, bovine thrombin, and thrombin-like fraction of snake venom.
