RESUMO Este artigo desvenda as estratégias de legitimidade moral utilizadas pelos três maiores frigoríficos brasileiros. A análise foi realizada a partir de declarações prestadas nos relatórios de sustentabilidade dos frigoríficos. Utilizando análise de conteúdo e a teoria da legitimidade, com foco nas estratégias de legitimidade moral, foi possível identificar e relacionar as declarações com as estratégias propostas pelo autor. Observa-se que as organizações dedicam maior atenção em ganhar legitimidade moral, principalmente, oferecendo demonstrações simbólicas, incorporando-se a instituições legitimadas, e, definindo metas. O artigo contribui para a compreensão das estratégias de legitimidade moral utilizadas pelos frigoríficos e quais práticas de sustentabilidade são divulgadas por empresas em busca da legitimidade moral. This article unveils the moral legitimacy strategies employed by the three largest Brazilian meatpacking companies. The analysis was conducted based on statements provided in the sustainability reports of these meatpacking companies. Using content analysis and legitimacy theory, with a focus on moral legitimacy strategies, it was possible to identify and correlate the statements with the strategies proposed by the author. It is observed that organizations pay greater attention to gaining moral legitimacy, primarily through symbolic demonstrations, aligning themselves with legitimized institutions, and setting goals. The article contributes to understanding the moral legitimacy strategies used by meatpacking companies and which sustainability practices are promoted by these companies in pursuit of moral legitimacy. El análisis se llevó a cabo a partir de declaraciones proporcionadas en los informes de sostenibilidad de estos frigoríficos. Utilizando análisis de contenido y la teoría de la legitimidad, con un enfoque en las estrategias de legitimidad moral, fue posible identificar y correlacionar las declaraciones con las estrategias propuestas por el autor. RESUMO Contexto: Em estudos observacionais sobre a síndrome do desconforto respiratório agudo, sugeriu-se que a driving pressure é o principal fator de lesão pulmonar induzida por ventilador e de mortalidade. Não está claro se uma estratégia de limitação da driving pressure pode melhorar os desfechos clínicos. Objetivo: Descrever o protocolo e o plano de análise estatística que serão usados para testar se uma estratégia de limitação da driving pressure envolvendo a titulação da pressão positiva expiratória final de acordo com a melhor complacência respiratória e a redução do volume corrente é superior a uma estratégia padrão envolvendo o uso da tabela de pressão positiva expiratória final baixa do protocolo ARDSNet, em termos de aumento do número de dias sem ventilador em pacientes com síndrome do desconforto respiratório agudo devido à pneumonia adquirida na comunidade. Métodos: O estudo STAMINA (ventilator STrAtegy for coMmunIty acquired pNeumoniA) é randomizado, multicêntrico e aberto e compara uma estratégia de limitação da driving pressure com a tabela de pressão positiva expiratória final baixa do protocolo ARDSnet em pacientes com síndrome do desconforto respiratório agudo moderada a grave devido à pneumonia adquirida na comunidade internados em unidades de terapia intensiva. Esperamos recrutar 500 pacientes de 20 unidades de terapia intensiva brasileiras e duas colombianas. Eles serão randomizados para um grupo da estratégia de limitação da driving pressure ou para um grupo de estratégia padrão usando a tabela de pressão positiva expiratória final baixa do protocolo ARDSnet. No grupo da estratégia de limitação da driving pressure, a pressão positiva expiratória final será titulada de acordo com a melhor complacência do sistema respiratório. Desfechos: O desfecho primário é o número de dias sem ventilador em 28 dias. Os desfechos secundários são a mortalidade hospitalar e na unidade de terapia intensiva e a necessidade de terapias de resgate, como suporte de vida extracorpóreo, manobras de recrutamento e óxido nítrico inalado. Conclusão: O STAMINA foi projetado para fornecer evidências sobre se uma estratégia de limitação da driving pressure é superior à estratégia da tabela de pressão positiva expiratória final baixa do protocolo ARDSnet para aumentar o número de dias sem ventilador em 28 dias em pacientes com síndrome do desconforto respiratório agudo moderada a grave. ABSTRACT Background: Driving pressure has been suggested to be the main driver of ventilator-induced lung injury and mortality in observational studies of acute respiratory distress syndrome. Whether a driving pressure-limiting strategy can improve clinical outcomes is unclear. Objective: To describe the protocol and statistical analysis plan that will be used to test whether a driving pressure-limiting strategy including positive end-expiratory pressure titration according to the best respiratory compliance and reduction in tidal volume is superior to a standard strategy involving the use of the ARDSNet low-positive end-expiratory pressure table in terms of increasing the number of ventilator-free days in patients with acute respiratory distress syndrome due to community-acquired pneumonia. Methods: The ventilator STrAtegy for coMmunIty acquired pNeumoniA (STAMINA) study is a randomized, multicenter, open-label trial that compares a driving pressure-limiting strategy to the ARDSnet low-positive end-expiratory pressure table in patients with moderate-to-severe acute respiratory distress syndrome due to community-acquired pneumonia admitted to intensive care units. We expect to recruit 500 patients from 20 Brazilian and 2 Colombian intensive care units. They will be randomized to a driving pressure-limiting strategy group or to a standard strategy using the ARDSNet low-positive end-expiratory pressure table. In the driving pressure-limiting strategy group, positive end-expiratory pressure will be titrated according to the best respiratory system compliance. Outcomes: The primary outcome is the number of ventilator-free days within 28 days. The secondary outcomes are in-hospital and intensive care unit mortality and the need for rescue therapies such as extracorporeal life support, recruitment maneuvers and inhaled nitric oxide. Conclusion: STAMINA is designed to provide evidence on whether a driving pressure-limiting strategy is superior to the ARDSNet low-positive end-expiratory pressure table strategy for increasing the number of ventilator-free days within 28 days in patients with moderate-to-severe acute respiratory distress syndrome. ABSTRACT The pear tree (Pyrus spp.) is a fruit tree of temperate climate, so it needs a certain number of hours under cold temperatures to overcome its natural dormancy. It can be cultivated in the tropics, but few regions have cold enough winters to overcome the natural dormancy of pear trees. For this purpose, it is necessary to select cultivars with less need for cold conditions combined with the use of bud-inducing to sprouting and flowering products. This study aimed to test doses of hydrogenated cyanamide (Dormex) to break dormancy and induce bud sprouting in different pear cultivars under subtropical conditions. The branches of five pear tree cultivars, ‘Cascatense’, ‘Primorosa’, ‘Seleta’, ‘Packham’s Triumph’, and ‘William’s’, were sprayed with Dormex (a commercial product containing 52% hydrogen cyanamide, CH2N2) at 1, 2, 3, 4, or 5% or with water (control). The trees were evaluated at two, four, six, eight, 10, and 12 weeks after the application of the treatments in two consecutive years. The European pear trees ‘William’s’ and ‘Packham’s Triumph’ showed low budding rates compared to hybrid pear trees. Hybrid pear showed better performance in subtropical regions, with higher budding rates, than European pear. CH2N2 efficiently induces bud sprouting of pear, with especially 4 and 5% Dormex. RESUMO Contexto: Nas últimas décadas, a ecoendoscopia evoluiu de uma ferramenta diagnóstica para uma plataforma para diversas intervenções terapêuticas. Vários avanços tecnológicos surgiram desde o último Consenso Brasileiro, demandando uma revisão e atualização das recomendações baseadas nas melhores evidências científicas. Métodos: Um grupo de 32 ecoendoscopistas renomados selecionou oito tópicos relevantes para serem discutidos a fim de gerar questões clínicas. Em seguida, foi realizada uma revisão da literatura para responder a essas perguntas com base nas evidências mais atualizadas. Resultados: Trinta e três tópicos foram formulados e votados pelos especialistas para alcançar um consenso. O Sistema de Oxford foi utilizado para classificar o nível de evidência. Conclusão: Há evidências moderadas para sustentar que a forma da agulha, calibre ou técnica de aspiração não influenciam no rendimento da amostragem tecidual guiada por ultrassom endoscópico (USE) de lesões sólidas pancreáticas. Há evidências moderadas para sustentar o uso da biópsia transcutânea guiada por USE da parede cística para diferenciar entre neoplasias císticas mucinosas e não mucinosas. Existem poucas evidências para apoiar o tratamento guiado por USE de varizes gástricas. Há um alto nível de evidência para sustentar que a drenagem biliar guiada por USE e a CPRE apresentam resultados semelhantes em pacientes com obstrução biliar maligna distal. Há um alto nível de evidência para o uso da USE no diagnóstico de cistos pancreáticos neoplásicos e na detecção de necrose antes de indicar a drenagem. Há evidências moderadas para sustentar a preferência pela gastroenterostomia guiada por USE em relação ao stent duodenal para obstrução maligna da saída gástrica em pacientes com expectativa de vida superior a 2 meses. ABSTRACT Background: In the past decades, endoscopic ultrasound has developed from a diagnostic tool to a platform for many therapeutic interventions. Various technological advancements have emerged since the last Brazilian Consensus, demanding a review and update of the recommendations based on the best scientific evidence. Methods: A group of 32 renowned echoendoscopists selected eight relevant topics to be discussed to generate clinical questions. After that, a literature review was conducted to answer these questions based on the most updated evidence. Results: Thirty-three statements were formulated and voted on by the experts to reach a consensus. The Oxford System was used to grade the level of evidence. Conclusion: There is moderate evidence to support that the needle shape, gauge, or aspiration technique does not influence the yield of endoscopic ultrasound (EUS)-guided tissue sampling of pancreatic solid lesions. There is moderate evidence to support using EUS-TTNB of the cyst wall to differentiate between mucinous and non-mucinous cystic neoplasms. There is little evidence to support the EUS-guided treatment of gastric varices. RESUMO Objetivo Avaliar a associação entre padrão de consumo alimentar e gordura visceral materna. Métodos Estudo observacional realizado durante consulta pré-natal com gestantes de Porto Alegre, Brasil. O padrão de consumo alimentar gestacional foi estimado através de recordatório alimentar habitual, questionando sobre todos os alimentos consumidos em um dia de semana normal e suas respectivas quantidades. Os depósitos de gordura foram aferidos por ultrassonografia. A correlação entre variáveis simétrica foi avaliada por correlação de Pearson e a acorrelação entre variáveis assimétrica foi avaliada por correlação de Spearman. A associação entre as variáveis foi avaliada por regressão linear multivariada, sendo as variáveis com distribuição assimétricas transformadas em logarítimo. ABSTRACT Objective To assess the association between the pattern of dietary intake and visceral fat deposits. Methods This is an observational study with pregnant women during a prenatal appointment in Porto Alegre, Brazil. The gestational dietary intake was assessed using a usual food recall, questioning every food consumed on a usual weekday, and their respective amounts. Fat deposits were measured by ultrasonography. The correlation between symmetric variables was assessed using Pearson's correlation and the correlation between asymmetric variables was assessed using Spearman's correlation. The association between variables was assessed using multivariate linear regression, in which variables with asymmetric distribution were transformed into logarithms. Results A total of 154 pregnant women were included in the sample; the mean visceral fat thickness was 44.17 mm±15.03 and the median daily total energy intake was 2,149.25 kcal [1,676.53-3,051.72]. A significant positive correlation (r=0.165) was found between visceral fat thickness and the consumption of processed culinary ingredients (p=0.049). Conclusion Our findings suggest that the consumption of processed culinary ingredients may be positively associated with visceral fat deposits in pregnant women. RESUMO Objetivo: Descrever os indicadores de resultado da implementação de uma cascata de cuidados da infecção latente por Mycobacterium tuberculosis em pessoas vivendo com o vírus da imunodeficiência humana. Método: Estudo transversal realizado com pessoas vivendo com HIV no período de 2022 a 2024 em serviço de referência de Campo Grande, Mato Grosso do Sul. Foi feito após a implementação do seguinte processo de trabalho: Identificação de pessoas em risco para investigação de Infecção Latente da Tuberculose (ILTB); Teste para ILTB; Exclusão de casos de TB ativa; Tratamento para ILTB; Adesão ao tratamento; e Conclusão do tratamento. Os dados foram analisados por estatística descritiva, teste Qui-quadrado e teste exato de Fisher. Resultados: Foram acompanhadas 735 pessoas. Desse total, 29,6% tinham indicação de iniciar o tratamento de infecção latente, das quais 32,5% iniciaram. A taxa de conclusão do tratamento foi maior no grupo que usou o esquema encurtado (90,2% versus 71,8%; p-value < 0,05, 7,8% com Isoniazida e 2% com Rifampicina). Houve uma (0,9%) reação adversa grave. Conclusão: A implementação da cascata de cuidados demonstrou que um terço dos pacientes tinha indicação de tratamento e ampliou o acesso ao tratamento preconizado. A taxa de conclusão do tratamento foi maior com o esquema encurtado. As reações adversas foram infrequentes. ABSTRACT Objective: To describe the outcome indicators of implementing a cascade of care for latent Mycobacterium tuberculosis infection in people living with the human immunodeficiency virus. Method: Cross-sectional study, carried out with people living with HIV, from 2022 to 2024, in a reference service in Campo Grande, Mato Grosso do Sul. RESUMEN Objetivo: Describir los indicadores de resultados de la implementación de una cascada de atención para la infección latente por Mycobacterium tuberculosis en personas que viven con el virus de la inmunodeficiencia humana. Método: Estudio transversal, realizado con personas que viven con VIH, de 2022 a 2024, en un servicio de referencia en Campo Grande, Mato Grosso do Sul. ABSTRACT The application of rock dust (RD) in agricultural soils has shown the capacity to improve soil conditions for plant growth. However, the successful use of RD requires understanding its effects on weathering in soil surfaces and its possible products. In this study, we present the results of an exploratory test in which a RD had its weathering intensified in a soil column by temperature (45 °C) and intense leaching (1,339.2 mm in 24 weeks), in the absence of plants. There was a significant increase in amorphous phases of iron (Fe) and particularly of aluminum (Al) to the amount of 0.13 g kg –1 for Fe and 0.16 g kg –1 for Al in the 0-0.05 m layer of the column and 0.22 g kg –1 for Al in the 0.05-0.10 m layer. These values account for 6 % of the total Fe, and 27 % of the total Al added as RD in the column. Spectral features in the visible-near infrared-short wave infrared (VIS-NIR-SWIR) and mid-infrared (MIR) range, as well as X-ray diffraction (XRD) scans, were inconclusive because of the small signal-noise ratio of the newly precipitated phases. Selective chemical methods targeted to specific soil pools (precipitated, adsorbed, and leached) were more sensitive to capture the new amorphous phases resulting from the RD dissolution. This study contributes to understanding the effects of RD on the availability of ions and the possibility of amorphous oxides precipitating in the soil. In addition, it helps identify other effects associated with using RD, such as changes in the pH, cation exchange capacity (CEC), and nutrient availability. RESUMO Introdução: A empatia, um elemento primordial do humanismo, tem sido alvo de crescente interesse no ensino médico. No contexto das profissões de saúde, a empatia no atendimento ao paciente é um atributo que envolve a compreensão das experiências, da dor, do sofrimento e das preocupações do paciente combinados com a capacidade de comunicar esse entendimento e uma intenção de ajudar. Apesar do desenvolvimento de práticas que estimulam a empatia nos estudantes de Medicina, um desafio presente na vida acadêmica é mensurar essa habilidade. Objetivo: Este estudo teve como objetivos elaborar e validar uma escala brasileira de empatia no contexto do atendimento clínico. Método: Trata-se de estudo-piloto de construção e validação de uma escala psicométrica, realizado em cinco etapas: 1. definição das dimensões do construto baseada em revisão da literatura; 2. itens submetidos à análise teórica com juízes especializados no tema, objetivando identificar a pertinência do item dentro do construto; 3. realização de um pré-teste com a população-alvo visando avaliar o entendimento dos itens; 4. validação da escala com aplicação a 207 estudantes de Medicina brasileiros; 5. aplicação de testes estatísticos que auxiliam na validação da escala. Resultado: Ao final do estudo, a escala de empatia elaborada continha 21 itens, com respostas em escala de Likert, distribuídos em dois fatores: compreensão empática e ação empática, que apresentaram boa confiabilidade proposta (> 0,842) e boa consistência interna (H-latente > 0,879 e H-observado > 0,864) com total de variância explicada de 44,95%. Conclusão: A aplicação da escala de empatia clínica em estudantes de Medicina resultou em um instrumento que atendeu aos critérios de adequação semântica e cultural, e revelou evidências preliminares de validade. ABSTRACT Introduction: Empathy, which is a crucial element of humanism, has been the subject of increasing interest in medical education. In the context of the healthcare professions, empathy in patient care is an attribute that involves understanding the patient’s experiences, pain, suffering, and concerns combined with the ability to communicate that understanding and an intention to help. Despite the development of practices that encourage empathy in medical students, a challenge in academic life is measuring this ability. Objective: To develop and validate a Brazilian scale of empathy in the context of clinical care. Method: Pilot study of construction and validation of a psychometric scale, carried out in five stages: (1) Definition of the dimensions of the construct based on a literature review; (2) Items submitted to theoretical analysis with judges specialized in the subject, aiming to identify the pertinence of the item within the construct; (3) carrying out a pre-test with the target population to assess the understanding of the items (4) validation of the scale with application to 207 Brazilian medical students (5) application of statistical tests that help in the validation of the scale. Result: At the end of the study, the created Empathy Scale contained 21 items, with Likert scale responses, distributed in two factors: empathic understanding and empathic action, which presented good proposed reliability (>0.842) and good internal consistency (latent H >0.879 and observed H >0.864) with a total explained variance of 44.95%. RESUMO Este trabalho considera o aspecto probabilístico da organização do texto a partir da instanciação. Cada instância manifesta de texto se associa à tipologia textual, tanto pelo contexto - como se registra amplamente na literatura - quanto pela configuração linguística mais provável, que é o foco deste trabalho. Descrever como a organização de probabilidade e frequência, assim como os mecanismos pelos quais as frequências emergem, motiva uma configuração linguística e determina como um texto é instanciado torna-se passo necessário para incluir propriedades quantitativas na modelagem. O artigo parte de um corpus de procedimentos, Subcorpus1 - experimentos científicos e receitas; e Subcorpus2 - referente à atividade social de “assar bolo”. Em seguida, identifica o perfil metafuncional dos textos e estabelece regras de associação entre os sistemas para identificar sua chavicidade. Por fim, aplica a metodologia da dinâmica de sistemas. ABSTRACT The paper departs from instantiation to explore text in relation to the quantitative properties of language - more specifically how probability plays a part in the matching of an instance and a particular socially-oriented use. Such matching not only links instantiation and stratification, but also helps explaining how different context configurations (patterns of register and genre) are accordingly related to steps in subpotentialization. The fact that language is an inherently probabilistic system may yield a quantitative account of this process if a more complete modelling of language is to be obtained. As part of our exploration, we compiled a corpus of procedures and applied dynamics modelling to tease out quantitative properties of instances associated with context. We described some properties of instantiation - reweighting, association, and dynamics -, which determine the probabilities of an instance configuration, as they also behave following register variation. A toxina botulínica tem sido apresentada como alternativa terapêutica em estudos nacionais e internacionais, sendo necessária uma compilação e destaque dos principais estudos que possam subsidiar a prática clínica. Assim, o objetivo foi apresentar uma revisão de escopo sobre a utilização terapêutica da toxina botulínica para o tratamento de cicatrizes queloides. Método: A revisão foi realizada através da estratégia PICO e utilizando o Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews. Foi realizada nas bases de dados PubMed/ Medline, Biblioteca Virtual em Saúde e SciELO, considerando estudos do período de 2016 até setembro de 2021. Resultados: Foram encontrados 34 artigos no geral relacionados ao tema. Após filtragem e seleção, a revisão foi construída com apoio de 5 artigos. Os estudos variaram entre coorte, relatos de caso, ensaio clínico randomizado e caso-controle. Foi possível observar como principais resultados a ação a curto prazo da toxina botulínica na redução de queloides, maior efetividade na redução dos sintomas e possibilidades de utilização clínica para diferentes populações e manifestações clínicas. Conclusão: O mecanismo de ação da toxina botulínica pode facilitar o tratamento de queloides e redução de sintomas, sendo necessários estudos mais robustos para definição de protocolos cínicos de gestão de cicatrizes. ABSTRACT Introduction: Visible scars can cause problems, whether aesthetic, psychological, functional, or social, mainly of great extension and volume, such as keloids. The discovery of new treatments for keloids is not easy, given the presence of some methodological and ethical obstacles, and it is an area that is little explored. Botulinum toxin has been presented as a therapeutic alternative in national and international studies, requiring a compilation and highlighting of the main studies that can support clinical practice. Thus, the objective was to present a scoping review on the therapeutic use of botulinum toxin for the treatment of keloid scars. Method: The review was carried out using the PICO strategy and using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews. It was carried out in the PubMed/Medline, Virtual Health Library, and SciELO databases, considering studies from 2016 to September 2021. Results: Overall, 34 articles related to the topic were found. After filtering and selection, the review was constructed with the support of 5 articles. The studies varied between cohorts, case reports, randomized clinical trials, and case-control. It was possible to observe as main results of the short-term action of botulinum toxin in reducing keloids, greater effectiveness in reducing symptoms, and possibilities of clinical use for different populations and clinical manifestations. Conclusion: The mechanism of action of botulinum toxin can facilitate the treatment of keloids and reduce symptoms, requiring more robust studies to define effective scar management protocols. RESUMO Em janeiro de 2022, o Rio Carmo (um afluente do Rio Doce) foi afetado por um evento hidrológico extremo, com um tempo de retorno de 50 anos. Esse evento foi 50% mais intenso do que o associado a uma das maiores falhas de barragem de rejeitos já registradas, que ocorreu neste mesmo rio. Isso resultou no despejo de rejeitos de mineração contaminados no Rio Doce, por meio do Rio Carmo, em uma confluência em forma de Y. Com objetivo de entender a função dos afluentes na recuperação do rio Doce, foram realizadas simulações hidrodinâmicas e de transporte de sedimentos, a partir da versão bidimensional do modelo HEC-RAS, na confluência do rio do Carmo com o rio Piranga. Nossos resultados, com base em modelagem numérica, sensoriamento remoto e medições in situ, apontaram que grandes quantidades de sedimentos foram retidas nas margens do rio e nos bancos de pedra, o que torna esta confluência um local importante para a retenção de sedimentos contaminados, com influências significativas para a qualidade de água a jusante. Sendo assim, essa confluência merece uma atenção especial da engenharia Hídrica a fim de evitar que os sedimentos retidos nesta área sejam transportados a jusante durante os eventos extremos nos próximos anos. ABSTRACT On January 2022, the Carmo River (a tributary of the Doce River) was affected by an extreme hydrological event, with 50-yr return interval. This event was 50% higher than that associated to one of the largest tailing dam failures ever reported that took place in the Gualaxo do Norte River. This delivered to Doce River, through the Carmo River, in a Y-shaped confluence, the contaminated tailing slurry. In order to understand the role of tributaries in the recovery of the Doce River, 2D hydrodynamic and 2D sediment transport models were applied at the confluence of the Carmo River with the Piranga River. Our results, based on hydraulic modeling, remote sensing, and in-situ measurements, indicated that significant amounts of sediments were trapped on the riverbanks and stone banks, making this confluence an important site for the retention of contaminated sediments, influencing sediment budgets and downstream water quality. Therefore, this confluence deserves special attention from water engineering in order to prevent the sediments retained in this area from being transported downstream during extreme events in the coming years. Abstract There is growing interest in using cannabinoids across various clinical scenarios, including pain medicine, leading to the disregard of regulatory protocols in some countries. Legislation has been implemented in Brazil, specifically in the state of São Paulo, permitting the distribution of cannabinoid products by health authorities for clinical purposes, free of charge for patients, upon professional prescription. Thus, it is imperative to assess the existing evidence regarding the efficacy and safety of these products in pain management. In light of this, the São Paulo State Society of Anesthesiology (SAESP) established a task force to conduct a narrative review on the topic using the Delphi method, requiring a minimum agreement of 60% among panelists. The study concluded that cannabinoid products could potentially serve as adjuncts in pain management but stressed the importance of judicious prescription. Nevertheless, this review advises against their use for acute pain and cancer-related pain. In other clinical scenarios, established treatments should take precedence, particularly when clinical protocols are available, such as in neuropathic pain. Only patients exhibiting poor therapeutic responses to established protocols or demonstrating intolerance to recommended management may be considered as potential candidates for cannabinoids, which should be prescribed by physicians experienced in handling these substances. RESUMO Contexto A traqueobronquite associada ao ventilador é uma condição comum entre pacientes ventilados invasivamente em unidades de terapia intensiva, para a qual se desconhece atualmente a melhor estratégia de tratamento. Desenhamos o estudo VATICAN (Ventilator-Associated Tracheobronchitis Initiative to Conduct Antibiotic Evaluation) para avaliar se uma estratégia de tratamento antibiótico de espera vigilante não é inferior ao tratamento antibiótico de rotina para traqueobronquite associada ao ventilador em relação aos dias sem ventilador mecânico. Métodos O VATICAN é um estudo randomizado, controlado, aberto e multicêntrico de não inferioridade. Os pacientes com suspeita de traqueobronquite associada ao ventilador sem evidência de pneumonia associada ao ventilador ou instabilidade hemodinâmica devido a uma provável infecção serão designados para uma estratégia de espera vigilante, sem administração profilática de antimicrobianos contra traqueobronquite associada ao ventilador e prescrição de antimicrobianos somente em casos de pneumonia associada ao ventilador, sepse ou choque séptico, ou outro diagnóstico infeccioso, ou para uma estratégia de tratamento antimicrobiano de rotina por 7 dias. O desfecho primário será o número de dias sem ventilador mecânico em 28 dias, e um desfecho secundário importante será a sobrevida sem pneumonia associada ao ventilador. Por meio de uma estrutura de intenção de tratar com análise de sensibilidade por protocolo, a análise do desfecho primário abordará a não inferioridade com margem de 20%, o que se traduz em uma diferença de 1,5 dia sem ventilador. Outras análises seguirão uma estrutura de análise de superioridade. Conclusão O VATICAN seguirá todos os padrões éticos nacionais e internacionais. O objetivo é publicar o estudo em um periódico geral de alta visibilidade e apresentá-lo em conferências de cuidados intensivos e doenças infecciosas para divulgação. ABSTRACT Background Ventilator-associated tracheobronchitis is a common condition among invasively ventilated patients in intensive care units, for which the best treatment strategy is currently unknown. We designed the VATICAN (Ventilator-Associated Tracheobronchitis Initiative to Conduct Antibiotic Evaluation) trial to assess whether a watchful waiting antibiotic treatment strategy is noninferior to routine antibiotic treatment for ventilator-associated tracheobronchitis regarding days free of mechanical ventilation. Methods VATICAN is a randomized, controlled, open-label, multicenter noninferiority trial. Patients with suspected ventilator-associated tracheobronchitis without evidence of ventilator-associated pneumonia or hemodynamic instability due to probable infection will be assigned to either a watchful waiting strategy, without antimicrobial administration for ventilator-associated tracheobronchitis and prescription of antimicrobials only in cases of ventilator-associated pneumonia, sepsis or septic shock, or another infectious diagnosis, or to a routine antimicrobial treatment strategy for seven days. The primary outcome will be mechanical ventilation-free days at 28 days, and a key secondary outcome will be ventilator-associated pneumonia-free survival. Through an intention-to-treat framework with a per-protocol sensitivity analysis, the primary outcome analysis will address noninferiority with a 20% margin, which translates to a 1.5 difference in ventilator-free days. Other analyses will follow a superiority analysis framework. Conclusion The VATICAN trial will follow all national and international ethical standards. We aim to publish the trial in a high-visibility general journal and present it at critical care and infectious disease conferences for dissemination. These results will likely be immediately applicable to the bedside upon trial completion and will provide information with a low risk of bias for guideline development.
