Abstract Objective To evaluate whether antimicrobial photodynamic therapy (aPDT) repairs bisphosphonate-related osteonecrosis of the jaw (BRONJ) modulated by the reduction of NF-kB protein in a murine model. Methodology Male Wistar rats (N=30) were divided into the following groups (n=6/group): negative control (NC); experimental osteonecrosis (ONE); ONE + photosensitizer (PS); ONE + photobiomodulation (PBM); and ONE + aPDT. Over 8 weeks, ONE was induced by zoledronic acid 250 µg/kg injections, except in the NC group, which received sterile 0.9% saline, followed by extraction of the lower left first molar. Red light laser irradiation (wavelength ~660 nm, power 50 mW, energy of 2 J, energy dose of 66.67 J/cm2 for 40 s) was performed once a week for 4 weeks. Methylene blue 0.3% was used as PS. The animals were euthanized and examined macroscopically for the presence of exposed bone and epithelial repair and microscopically by histochemical (hematoxylin-eosin and Masson’s trichrome staining) and immunohistochemical (anti-NF-kB) methods. Macroscopic and histomorphometric data were analyzed by one-way ANOVA and Tukey’s post-test (p<0.05). Results Mucosal repair, viable osteocytes, and NF-kB immunostaining were observed in the NC, ONE+PS, ONE+PBM, and ONE+aPDT groups. The ONE group showed no mucosal repair, showing empty lacunae and multifocal immunostaining for NF-kB. The ONE+PBM and ONE+aPDT groups had greater deposition of extracellular matrix and less necrotic bone tissue (p<0.05). Conclusion PBM and aPDT treatments for BRONJ were effective for bone and epithelial repair, in addition to reducing inflammation mediated by the decrease of NF-kB protein in the irradiated regions. Resumo O objetivo deste estudo foi avaliar a resposta epithelial imune, a colonização da Candida albicans em monocamadas celulares e sua virulência em resposta a infecções de culturas de biofilme multiespécie. Culturas de biofilme monoespécie de C. albicans e culturas mistas (C. albicans, Streptococcus mutans e Streptococcus sanguinis) foram utilizadas para infectar monocamadas de células HaCaT e FaDu por 12 h. Após a infecção, a expressão dos genes IL-18 e IL-34 foi medida para avaliar as respostas imunes das células epiteliais. A atividade da lactato desidrogenase (LDH) foi medida como um indicador de dano celular. A microscopia determinou a morfologia de C. albicans e a penetração das células fúngicas através da monocamada de queratinócitos. Monocamadas em que não houve infecção serviram como controles. Os dados foram analisados por um teste ANOVA one-way seguido pelo teste post-hoc de Tukey (α = 0,05). Os resultados demonstraram que a expressão gênica de IL-18 e IL-34 e a atividade de LDH foram (p < 0,05) reguladas positivamente para ambas as linhagens de células expostas a culturas de espécies mistas em comparação com C. albicans isoladamente. Abstract Experimental models that consider host-pathogen interactions are relevant for improving knowledge about oral candidiasis. The aim of this study was to assess the epithelial immune responses, Candida penetration of cell monolayers, and virulence during mixed species culture infections. Single species cultures of Candida albicans and mixed cultures (C. albicans, Streptococcus mutans, and Streptococcus sanguinis) were used to infect monolayers of HaCaT and FaDu ATCC HTB-43 cells for 12 h. After infection, IL-18 and IL-34 gene expression was measured to assess epithelial cell immune responses, and lactate dehydrogenase (LDH) activity was measured as an indicator of cell damage. Microscopy determined C. albicans morphology and penetration of fungal cells through the keratinocyte monolayer. Monolayers devoid of infection served as controls. Data were analyzed by an ANOVA one-way test followed by Tukey’s post-hoc test (α = 0.05). The results found that IL-18 and IL-34 gene expression and LDH activity were significantly (p < 0.05) upregulated for both cell lines exposed to mixed species cultures compared with C. albicans alone. Candida albicans yeast and hyphae were evident in C. albicans only infections. In contrast, monolayers infected by C. albicans, S. mutans, and S. sanguinis exhibited higher microbial invasion with several hyphal aggregates detected. The presence of streptococci in C. albicans infection enhances the virulence and pathogenicity of the fungus with associated increased immune responses and tissue damage. OBJETIVOS: analisar a associação entre a insegurança alimentar das famílias e estado nutricional de menores de cinco anos. MÉTODOS: estudo transversal, de 501 famílias com 697 menores de cinco anos, no município da Gameleira, Pernambuco. A avaliação da (in)segurança alimentar foi realizada através da Escala Brasileira de Insegurança Alimentar (EBIA). A classificação do estado nutricional foi feita a partir dos indicadores estatura/idade, peso/idade e índice de massa corporal (IMC), utilizando o padrão de crescimento infantil da OMS. Foram estudados a associação de indicadores socioeconômicos, de insegurança alimentar e variáveis biológicas da criança sobre o índice estatura/idade, utilizando-se análise de regressão linear multivariada. RESULTADOS: a insegurança alimentar foi caracterizada em quase 90% das famílias, sendo a forma grave mais prevalente. Verificou-se uma prevalência baixa de déficit de peso pelo IMC e elevadas de déficit estatura/idade. O modelo final desta análise mostrou que as variáveis renda familiar per capita, escolaridade materna e idade da criança influenciaram significativamente o estado nutricional, entretanto, a EBIA não se associou com o estado nutricional das crianças. CONCLUSÕES: é evidente a discrepância entre a prevalência de insegurança alimentar nas famílias e a frequência baixa de desnutrição nas crianças, expressando que os dois indicadores avaliam aspectos e situações bem diferenciadas.

OBJECTIVES: to analyze the association between the food insecurity of families and the nutritional status of children aged under five years. METHODS: a cross-sectional study covering 501 families and 697 children aged under five years in the municipality of Gameleira, in the Brazilian State of Pernambuco. Food insecurity was assessed using the Brazilian Food Insecurity Scale (EBIA). Nutritional status was classified according to height/age, weight/age and body mass index (BMI), using the infant growth standards of the WHO. The study investigated the association between socio-economic indicators, food (in)security and biological variables relating to the child such as height/age, using linear multivariate regression analysis. RESULTS: food insecurity was found in almost 90% of families, with the most severe form being the most prevalent. There was a low prevalence of underweight in terms of BMI but high levels of underweight in terms of height for age. The final model of this analysis showed that the variables household per capita income, level of schooling of mother, and age of child had a significant influence on the nutritional status of the child, although the EBIA was not significantly associated with this. CONCLUSIONS: there is a clear discrepancy between the prevalence of food insecurity in families and the low frequency of malnutrition in children, suggesting that the two indicators evaluate quite distinct features and situations.

Desenvolveu-se em três prisões e dois hospitais por meio de entrevistas individuais e grupos de discussão. Revelou que concepções de saúde e doença, hierarquização de riscos e estratégias de preservação no contexto carcerário relacionam-se às posições nessa organização social, aos conflitos e tensões ali existentes e aos pertencimentos grupais que reforçam identidades e antagonismos, refletindo-se nas práticas rotineiras e no acesso aos serviços. A negação da saúde como um direito dos presos e a restrição de sua autonomia contribuem para ações de saúde prescritivas. A tomada de consciência dos agentes de segurança penitenciária quanto às representações e práticas de saúde pode contribuir para a desconstrução de idéias estereotipadas e aumentar seu envolvimento nas ações de prevenção e assistência. A melhoria das condições de saúde dos detentos implica também mudanças nas condições de encarceramento.

The role of prison guards in limiting access to health services by inmates and the impact of their representations on daily health practices were investigated through an action-research program. The program aimed to produce knowledge and improve the guards' involvement in tuberculosis and HIV/AIDS control by raising their awareness. Developed in three prisons and two hospitals, the study included individual interviews and group discussions and showed that representations of health and illness, risk hierarchy, and preventive strategies are related to actors' status in the prison's social organization, the conflicts and tensions between them, and the sense of group belonging that reinforces identities and antagonisms. Such representations have a major influence on the guards' daily practices and inmates' access to health services. The denial of inmates' right to health and their limited autonomy contributed to the implementation of essentially prescriptive health actions. The guards' awareness of their own health representations and practices may contribute to the deconstruction of stereotypes and increase their involvement in prevention and care. Further, improvement of inmates' health depends on changes in prison conditions.

O objetivo deste estudo foi verificar em 129 pacientes com AIDS, necropsiados na Fundação de Medicina Tropical do Amazonas de 1996 a 2003, as causas do óbito, observando o grau de concordância entre os diagnósticos necroscópicos com os diagnósticos clínicos. A doença mais freqüente que causou o óbito foi tuberculose 28%, seguida de pneumonia bacteriana 17%, histoplasmose 13%, toxoplasmose 10%, pneumocistose 8%, criptococose 5%, sepse bacteriana 4% e 15% outras causas. A concordância entre o diagnóstico clínico antemortem e a necropsia foi de 51,9%. O principal órgão acometido foi o pulmão 82,2%. O tempo de sobrevivência após o diagnóstico laboratorial até o óbito variou entre um mês e 120 meses. A média de sobrevivência foi 15 dias e 56% morreram menos de um mês após o diagnóstico, 15 pacientes morreram na mesma data do diagnóstico. Esses resultados demonstram a importância da necropsia na causa mortis em pacientes com AIDS.

The aim of this study was to investigate the causes of death among 129 AIDS patients that were autopsied at the Tropical Medicine Foundation of Amazonas between 1996 and 2003. The degree of concordance between the autopsy diagnoses and the clinical diagnoses was observed. The disease that most frequently caused death was tuberculosis (28%), followed by bacterial pneumonia (17%), histoplasmosis (13%), toxoplasmosis (10%), pneumocystosis (8%), cryptococcosis (5%), bacterial sepsis (4%) and other causes (15%). The concordance between the clinical diagnosis before death and the autopsy was 51.9%. The main organ involved was the lungs (82.2%). The length of survival from the time of the laboratory diagnosis to death ranged from one month to 120 months. The mean length of survival was 15 days and 56% died less than one month after the diagnosis, while 15 patients died on the same day that they were diagnosed. These results show the importance of autopsies in elucidating the causes of death among AIDS patients.
