Abstract To date, hydroxyurea is the only effective and safe drug that significantly reduces morbidity and mortality of individuals with Sickle cell disease. Twenty years of real-life experience has demonstrated that hydroxyurea reduces pain attacks, vaso-occlusive events, including acute chest syndrome, the number and duration of hospitalizations and the need for transfusion. The therapeutic success of hydroxyurea is directly linked to access to the drug, the dose used and adherence to treatment which, in part, is correlated to the availability of hydroxyurea. This consensus aims to reduce the number of mandatory exams needed to access the drug, prioritizing the requesting physician's report, without affecting patient safety. date disease reallife real life attacks vasoocclusive vaso occlusive events syndrome transfusion which part physicians physician s report safety

Aqui, descrevemos a justificativa, o desenho e o status do estudo. ABSTRACT Background: Driving pressure has been suggested to be the main driver of ventilator-induced lung injury and mortality in observational studies of acute respiratory distress syndrome. Whether a driving pressure-limiting strategy can improve clinical outcomes is unclear. Objective: To describe the protocol and statistical analysis plan that will be used to test whether a driving pressure-limiting strategy including positive end-expiratory pressure titration according to the best respiratory compliance and reduction in tidal volume is superior to a standard strategy involving the use of the ARDSNet low-positive end-expiratory pressure table in terms of increasing the number of ventilator-free days in patients with acute respiratory distress syndrome due to community-acquired pneumonia. Methods: The ventilator STrAtegy for coMmunIty acquired pNeumoniA (STAMINA) study is a randomized, multicenter, open-label trial that compares a driving pressure-limiting strategy to the ARDSnet low-positive end-expiratory pressure table in patients with moderate-to-severe acute respiratory distress syndrome due to community-acquired pneumonia admitted to intensive care units. We expect to recruit 500 patients from 20 Brazilian and 2 Colombian intensive care units. They will be randomized to a driving pressure-limiting strategy group or to a standard strategy using the ARDSNet low-positive end-expiratory pressure table. In the driving pressure-limiting strategy group, positive end-expiratory pressure will be titrated according to the best respiratory system compliance. Outcomes: The primary outcome is the number of ventilator-free days within 28 days. The secondary outcomes are in-hospital and intensive care unit mortality and the need for rescue therapies such as extracorporeal life support, recruitment maneuvers and inhaled nitric oxide. Conclusion: STAMINA is designed to provide evidence on whether a driving pressure-limiting strategy is superior to the ARDSNet low-positive end-expiratory pressure table strategy for increasing the number of ventilator-free days within 28 days in patients with moderate-to-severe acute respiratory distress syndrome. Here, we describe the rationale, design and status of the trial. ABSTRACT Several studies have reported on the presence of litter on beaches and in the oceans, but this represents the smallest part of the litter that ends up in the sea. Little is known about the litter that settles on the ocean floor, especially near coastal regions. Thus, this study quantified, classified and determined possible sources of subtidal benthic marine litter input off the coast of Rio de Janeiro, Brazil. Different sites were selected in Guanabara Bay, an area with the greatest industrial and population concentration in the state of Rio de Janeiro, and on the coasts of Costa do Sol and Costa Verde, where tourism represents the main economic activity. Material was collected by scuba diving at depths of between 5 and 15 meters at 31 sites distributed among the study areas. A total of 1, 209 items was found, with Guanabara Bay having the highest average concentration (92.40 items/km2), followed by Costa do Sol and Costa Verde (14.15 items/km2 and 9.91 items/km2, respectively). Plastic materials are the most representative solid waste (55.08%), followed by metals (14.97%) and fishing gear (12.48%). Results indicated that local activities affected the composition of the marine litter on Costa do Sol and Costa Verde, whereas in Guanabara Bay, anthropogenic material carried by rivers influenced the marine litter composition at certain locations in addition to local activities. Environmental education actions associated with tourism and fishing activities and better waste management are necessary to minimize the presence of litter in subtidal benthic environments. oceans sea floor regions Thus quantified Janeiro Brazil activity 1 3 areas 20 found 92.40 9240 92 40 (92.4 itemskm2 itemskm , km2 km items/km2) 14.15 1415 14 (14.1 items/km 991 9 91 9.9 respectively. respectively . respectively) 55.08%, 5508 55.08% 55 08 (55.08%) 14.97% 1497 97 (14.97% 12.48%. 1248 12.48% 12 48 (12.48%) environments 2 92.4 924 4 (92. 14.1 141 (14. 99 9. 550 55.08 0 (55.08% 14.97 149 (14.97 124 12.48 (12.48% 92. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. The importance of the CTFB is evidenced by the elevated number of citations in the scientific literature in diverse areas of biology, law, anthropology, education, forensic science, and veterinary science, among others. publications problem uptodate up date classifications context exception (CTFB http//fauna.jbrj.gov.br/, httpfaunajbrjgovbr http //fauna.jbrj.gov.br/ , jbrj gov br (http://fauna.jbrj.gov.br/) 2015 Brazil 80 specialists 1 2024 133691 133 691 133,69 125138 125 138 125,13 82.3%, 823 82 3 (82.3% 102000 102 000 102,00 7.69%, 769 7 69 (7.69% 11000 11 11,00 . 3,567 3567 567 (3,56 2,292 2292 2 292 (2,29 1,833 1833 833 (1,83 1,447 1447 447 (1,44 1000 1,00 831 (83 628 (62 606 (60 520 (52 50 users science health biology law anthropology education others http//fauna.jbrj.gov.br/ faunajbrjgovbr //fauna.jbrj.gov.br (http://fauna.jbrj.gov.br/ 201 8 202 13369 13 133,6 12513 12 125,1 82.3% (82.3 10200 10 00 102,0 7.69% 76 6 (7.69 1100 11,0 3,56 356 56 (3,5 2,29 229 29 (2,2 1,83 183 83 (1,8 1,44 144 44 (1,4 100 1,0 (8 62 (6 60 52 (5 5 http//fauna.jbrj.gov.br (http://fauna.jbrj.gov.br 20 1336 133, 1251 125, 82.3 (82. 1020 0 102, 7.69 (7.6 110 11, 3,5 35 (3, 2,2 22 (2, 1,8 18 (1, 1,4 14 4 ( 82. Abstract Objective: To descriptively analyze the epidemiological data, clinical stage, and outcomes of oropharyngeal squamous cell carcinoma in the state of São Paulo, Brazil, and to estimate the influence of clinical stage and treatment type on overall and disease-free survival. Methods: We retrospectively analyzed epidemiological data from the São Paulo Cancer Center Foundation database relative to patients with oropharyngeal squamous cell carcinoma diagnosed between 2004 and 2014 in the state of São Paulo. Univariate and multivariate Cox regression analyses were performed to assess factors associated with the outcomes. A forward stepwise selection procedure was used. Survival curves were estimated by the Kaplan-Meier method and compared by the Gehan-Breslow-Wilcoxon test. Results: A total of 8075 individuals with oropharyngeal squamous cell carcinoma were identified. Of these, 86.3% were diagnosed at an advanced stage and 13.7% at an early stage. Only 27.2% of patients were treated surgically, whereas 57.5% were treated medically. Patients undergoing surgery had longer overall survival than those receiving medical treatment in both early- and advanced-stage oropharyngeal squamous cell carcinoma. However, there was no significant difference in disease-free survival between surgical and medical treatment. Conclusion: No significant difference in disease-free survival between medical and surgical treatment suggests similar complete remission rates with both approaches. Patients receiving medical treatment had shorter overall survival, which may be due to complications from chemotherapy and radiotherapy. However, we cannot confirm this relationship based on the data provided by the São Paulo Cancer Center Foundation. Prospective studies are warranted to assess whether the lower overall survival rate in patients receiving medical treatment is secondary to complications from chemotherapy and radiotherapy.
ABSTRACT The phenomenon called infodemia refers to the increase in the volume of information on a specific topic, which multiplies rapidly in a short period of time, and has stood out in the context of the health crisis triggered by the COVID-19 pandemic. Too much information can trigger feelings of fear, anxiety, stress, and other conditions of mental distress. The study aims to describe the profile of exposure to information about COVID-19 and its repercussions on the mental health of elderly Brazilians. This is a cross-sectional study carried out with 1924 elderly Brazilians. Data were collected through a web-based survey sent to the elderly via social networks and email, from July to October 2020. The results of the descriptive analysis of the data show that most of the elderly were aged between 60 and 69 years (69.02%), female (71.26%), married (53.79%) and white (75.57%). About 21.67% (n = 417) concluded their graduation, 19.75% (380) concluded their specialization and 16.63% (320) concluded their master's or doctoral degrees. Television 862 (44.80%) and social networks 651 (33.84%) were reported as frequent sources of exposure to news or information about COVID-19. Participants indicated that television (46.47%; n = 872), social networks (30.81%; n = 575) and radio (14.48%; 251) affected them psychologically and/or physically. Receiving fake news about COVID-19 on television (n = 482; 19.8%) and on social media (n = 415; 21.5%) mainly resulted in stress and fear. The disseminated information contributes to awareness, but also affects physically and/or psychologically many elderly people, mainly generating fear and stress.

Abstract In Brazil, rabies occurs mainly within an urban cycle, in which dogs and bats are reservoirs. This paper aims to report the occurrence of rabies in Callithrix sp. in Niterói, Rio de Janeiro, Brazil. In June 2019 a hybrid specimen was referred for diagnosis. The Direct Fluorescent Antibody, Mouse Inoculation, and Polymerase Chain Reaction tests were positive. A phylogenetic analysis was compatible with antigenic variant 3, characteristic of Desmodus rotundus. New studies should be undertaken to elucidate the real role of callitrichids in the urban rabies cycle.

Abstract Schistosomiasis is a neglected disease that affects millions of people around the world, being common in the state of Maranhão. A total of 225 rodents of the Holochilus sciureus species from the Western Lowland Maranhão were studied, of which 144 animals (64%) exhibited Schistosoma eggs in their feces samples. Macroscopic lesions characterized as well-defined whitish areas on the liver and spleen surfaces were observed. Histopathological examination revealed multifocal granulomas in the esophagus, liver, spleen, pancreas and duodenum, with structures compatible with Schistosoma mansoni eggs, as well as severe hepatic micro-vacuolar degeneration, multifocal and coalescent, with proliferation of random bile ducts and associated epithelial hyperplasia to areas of fibrosis. Adult forms of the parasite were observed in the blood vessels of the portal space. The lungs exhibited moderate and diffuse interstitial pneumonia with intralesional S. mansoni eggs. In the kidneys, hyaline cylinders were observed in the pelvis and diffuse hemorrhage. In conclusion, H. sciureus displays a pathological picture similar to human being. This rodent plays a role as sentinel in Baixada Maranhense.

ABSTRACT This study aimed to evaluate associations among final weight (FW), average daily gain (ADG), scrotal circumference (SC), and visual score (VS) of beef cattle in performance tests on pasture or in feedlots. Genetic parameters for FW, ADG, SC, and VS of young Nellore bulls performance-tested on pasture or in feedlots were evaluated by mixed model. The performance test and final age were considered as fixed effects and additive genetic and residual effects were considered as random effects. Additive genetic and residual variances for final weight and average daily gain were smaller on pasture than in feedlots. There was no difference between genetic and residual variances and heritability for scrotal circumference on pasture or in feedlots. Genetic variance and heritability for visual score on pasture were smaller than those in feedlots. The posterior means (and highest posterior density intervals with 90% of samples (HPD90) in parentheses) for heritability were 0.46 (0.42; 0.50) and 0.49 (0.41; 0.55) for FW, 0.25 (0.22; 0.29) and 0.25 (0.19; 0.30) for ADG, 0.56 (0.51; 0.61) and 0.60 (0.51; 0.68) for SC, and 0.31 (0.27; 0.34) and 0.42 (0.36; 0.48) for VS on pasture or in feedlots, respectively. The genetic correlations (posterior means with HPD90 in parentheses) were 0.74 (0.69; 0.79) and 0.67 (0.58; 0.77) between FW and ADG; 0.49 (0.43; 0.55) and 0.60 (0.53; 0.68) for FW and SC; 0.79 (0.75; 0.83) and 0.85 (0.80; 0.90) for FW and VS; 0.37 (0.29; 0.46) and 0.34 (0.19; 0.50) for ADG and SC; 0.65 (0.59; 0.71) and 0.74 (0.64; 0.84) for ADG and VS; and 0.46 (0.39; 0.52) and 0.53 (0.44; 0.62) for SC and VS, obtained on pasture or in feedlots, respectively. The genetic and residual (co)variances of growth, scrotal circumference, and visual score of beef cattle vary across environments; however, genetic and residual correlation and efficiency of correlated response among these traits remain constant on pasture or in feedlots.

Abstract The objective of this study was to develop and propose a methodology to evaluate the quality of different sampling grids. In addition, it allows us to choose the sampling grid that better suits one or a set of variables. The structure and magnitude of the spatial dependence were characterized by semivariogram. It allowed us to apply validation techniques as a base to create an index for evaluating the grid quality, and to develop an indicator that points out the best sampling grid. To test the proposed methodology, an experiment was performed at the Brejão farm, in Brazil. We have developed and compared twenty sampling grids, which were applied to four soil variables sampled in georeferenced locations. An accuracy index (AI), a precision index (PI) and the optimum grid indicator (OGI) were developed and proposed, which allowed us to choose the best grid (grid 5) among the sampling grids.
