Resumo A violência doméstica contra a mulher é um problema do campo da saúde, que tem na Estratégia Saúde da Família (ESF) importante via de enfrentamento. Este estudo buscou compreender as práticas de cuidado com esse público, elaboradas por trabalhadores e trabalhadoras da ESF e do Núcleo Ampliado de Saúde da Família. A pesquisa foi marcada pelo advento da pandemia do coronavírus, que impossibilitou a presença no campo. Assim, foram utilizadas a entrevista semiestruturada on-line como principal fonte de dados, e a Análise Temática como estratégia de análise. Observa-se que a construção do cuidado se dá principalmente a partir da valorização das tecnologias leves e dos saberes práticos, que se manifestam principalmente por meio do que os participantes denominam “protocolos intuitivos”. O estudo aponta ainda o medo de represálias, a desvinculação de julgamentos morais, a implicação da mulher, o desconhecimento da rede de assistência e a ausência de protocolos como principais desafios a serem superados. Também revela a importância do protagonismo dos saberes elaborados no trabalho vivo em ato para a formulação das estratégias de cuidado, articuladas com a Política Nacional de Atenção Integral à Saúde da Mulher, além da necessidade de novas produções com essa perspectiva.

Abstract Domestic violence against women is a problem in the field of health, which has the Family Health Strategy (ESF) as an important way of coping. This study sought to understand the care practices for this public, elaborated by workers of the ESF and the Expanded Family Health Center. The research was marked by the advent of the coronavirus pandemic, which made it impossible to be present in the field, thus, the online semi-structured interview was used as the main source of data, and the Thematic Analysis as an analysis strategy. It is observed that the construction of care takes place mainly from the appreciation of light technologies and practical knowledge, which are manifested mainly through what the participants call “intuitive protocols”. The study also points out the fear of reprisals, the disengagement of moral judgments, the implication of the woman, the lack of knowledge about the assistance network and the absence of protocols as the main challenges to be overcome. It also reveals the importance of the protagonism of the knowledge elaborated in the live work in action for the formulation of care strategies, articulated with the National Policy of Integral Attention to Women's Health, in addition to the need for new productions with this perspective.

Resumo Fundamento: A reperfusão miocárdica é parte fundamental do tratamento para infarto agudo do miocárdio com supradesnivelamento de ST (IAMCSST) e é responsável por reduzir morbimortalidade no paciente acometido. No entanto, as taxas de reperfusão são geralmente mais baixas e as taxas de mortalidade mais altas em mulheres que em homens. Objetivos: Avaliar a prevalência do uso de terapias de reperfusão em mulheres e homens com IAMCSST nos hospitais com capacidade para realizar intervenção coronariana percutânea (ICP) no estado de Sergipe. Métodos: Trata-se de estudo transversal que utilizou dados do Registro VICTIM. Foram avaliados pacientes com diagnóstico de IAMCSST admitidos nos quatro hospitais com capacidade para realizar ICP no estado de Sergipe, sendo um público e três privados, no período de dezembro de 2014 a junho de 2018. Foi aplicada análise multivariada com modelo ajustado utilizando mortalidade como variável dependente. Em todas as análises, o nível de significância adotado foi de 5% (p<0,05). Resultados: Foram incluídos 878 voluntários com diagnóstico confirmado de IAMCSST, dos quais 33,4% eram mulheres. Apenas 53,3% dos pacientes foram submetidos à reperfusão miocárdica (134 mulheres versus 334 homens). A fibrinólise foi realizada somente em 2,3% de todos os pacientes (1,7% das mulheres versus 2,6% dos homens; p=0,422). Nas mulheres, a taxa de ICP primária foi menor (44% versus 54,5%; p=0,003) e a mortalidade hospitalar foi maior (16,1% versus 6,7%; p<0,001) que nos homens. Conclusão: As mulheres apresentam taxas significativamente menores de ICP primária e significativamente maiores de mortalidade hospitalar que os homens. A taxa de reperfusão em ambos os gêneros foi baixa e houve nítida subutilização de agentes trombolíticos.

Abstract Background: Myocardial reperfusion is a fundamental part of the treatment for ST elevation myocardial infarction (STEMI) and is responsible for reducing morbidity and mortality in affected patients. However, reperfusion rates are usually lower and mortality rates higher in women compared to men. Objectives: To evaluate the prevalence of the use of reperfusion therapies among women and men with STEMI in hospitals where percutaneous coronary intervention (PCI) is available in the state of Sergipe. Methods: This is a cross-sectional study that used data from the VICTIM Register. Patients diagnosed with STEMI admitted to the four hospitals (one public and three private) where PCI is available in the state of Sergipe were evaluated, from December 2014 to June 2018. A multivariate analysis with adjusted model using mortality as a dependent variable was made. In all analyses, the level of significance adopted was 5% (p < 0.05). Results: A total of 878 volunteers with a confirmed diagnosis of STEMI, of which 33.4% were women, were included in the study. Only 53.3% of the patients underwent myocardial reperfusion (134 women versus 334 men). Fibrinolysis was performed only in 2.3% of all patients (1.7% of women versus 2.6% of men; p = 0.422). The rate of primary PCI was lower (44% versus 54.5%; p = 0.003) and hospital mortality was higher (16.1% versus 6.7%; p < 0.001) in women than in men. Conclusion: Women have significantly lower rates of primary PCI and higher hospital mortality. Reperfusion rates were low in both sexes and there was a clear underutilization of thrombolytic agents.

Este trabalho relata a experiência de integração ensino-serviço-comunidade vivenciada por membros de equipes de Saúde da Família e graduandos de Medicina e Enfermagem participantes do PET-Saúde da Universidade Estadual de Santa Cruz. A referida integração contextualizou-se com o desenvolvimento do projeto "Unidos contra a dengue", mobilização conjunta da instituição de ensino com o serviço de saúde e a comunidade no combate à dengue no bairro Nossa Senhora da Vitória, Ilhéus-Bahia. O foco das ações desse projeto foram as atividades de educação em saúde, nas quais se empregou preferencialmente metodologias ativas de ensino e aprendizagem. A mobilização contou com a participação expressiva da população das mais diversas faixas etárias. O sucesso do projeto ressalta a importância da implantação de programas como o PET-Saúde, que fortalecem a interação entre ensino-serviço-comunidade - importante para construção de serviços de saúde mais qualificados, capazes de inter-relacionar promoção, prevenção e assistência à saúde, possibilitando ações nessa área mais próximas das reais necessidades do SUS. Com esta experiência, espera-se ter contribuído para a formação de graduandos em saúde e qualificação em serviço de profissionais dessa área mais dispostos a trabalhar de forma integrada e integradora com a comunidade.

This study reports on a practical experience with the integration between learning, services, and the community including members of family health teams and undergraduate medical and nursing students participating in the Educational Program for Health Work (PET-Saúde) at Santa Cruz State University. The integration took place within the context of the project "United Against Dengue", an integrated mobilization of the teaching institution, health services, and community in the fight against dengue fever in the district of Nossa Senhora da Vitória, Ilhéus, Bahia State. The projects focused on health education activities, especially with active teaching and learning methods. The mobilization included a significant range of age groups. The project's success emphasizes the importance of implementing programs such as PET-Saúde that strengthen the interaction between learning, services, and the community, important for developing more qualified health services with the capacity to interrelate health promotion, prevention, and healthcare, fostering health activities more in line with the needs of the Unified National Health System (SUS). This experience aims to contribute to the qualification and training of health professionals and students that are willing to work in an integrated and integrating approach with the community.
