Between councils and conferences: deliberative arenas on human rights in Minas Gerais conferences

RESUMO Introdução: A literatura sobre sistemas deliberativos recomenda investigar as conexões dos fóruns deliberativos com outras partes do sistema político e da sociedade. O artigo analisa a participação de atores governamentais e da sociedade civil em conselhos e conferências de políticas públicas sobre direitos humanos no estado de Minas Gerais, Brasil. Os objetivos são dois: propor um modelo teórico para estudar subsistemas deliberativos; e identificar os conectores de uma rede multinível composta por conselhos e conferências de Direitos Humanos. Materiais e métodos: Adotamos uma metodologia centrada na teoria de subsistemas, priorizando a dinâmica das arenas de deliberação e a sinergia entre conselhos em âmbitos local e estadual. Reinterpretamos o conceito de subsistema e desenvolvemos um modelo empírico que incorpora sua dimensão temática. Para examinar as interligações entre conselhos e conferências, recorremos a uma abordagem analítica conhecida como análise de redes de dois modos, também referida como análise de redes de afiliação, situada no domínio da Análise de Redes Sociais. Resultados: Os achados sugerem que o subsistema é altamente integrado. Contrastando com pesquisas anteriores, notamos que, paralelamente à relevância dos agentes governamentais na interligação das arenas, os participantes da sociedade civil desempenham um papel preponderante na articulação das diferentes áreas que constituem o subsistema. Este resultado destaca a especialização funcional dos agentes de ligação no contexto do sistema de deliberação. Discussão: Nossas descobertas contribuem em dois aspectos fundamentais para a literatura existente. Primeiramente, introduzimos um modelo teórico inovador para a análise de sistemas deliberativos. Em segundo lugar, recomendamos a utilização da análise de rede multinível como uma ferramenta eficaz para identificar as conexões entre múltiplas arenas. ABSTRACT Introduction: The academic literature on deliberative systems suggests exploring the connections between deliberative forums and other components of the political system and society. This article investigates the participation of governmental actors and civil society in councils and conferences addressing public policies on human rights in the state of Minas Gerais, Brazil. The objectives are twofold: to propose a theoretical framework for examining deliberative subsystems and to identify the connectors in a multilevel network consisting of human rights councils and conferences. Materials and methods: We adopted a methodology centered on subsystem theory, prioritizing the dynamics of deliberation arenas and the synergy between councils at local and state levels. We reinterpreted the concept of subsystem to develop an empirical model that incorporates its thematic dimension. To examine the interconnections between councils and conferences, we employed an analytical approach known as two mode network analysis, also referred to as affiliation network analysis, situated in the domain of Social Network Analysis. Results: Our findings suggest a high level of integration within the subsystem. In contrast to prior studies, we found that civil society actors, alongside governmental agents, significantly contribute to interconnecting various arenas within the subsystem. This finding underlines the functional specialization of liaison agents within the context of the deliberation system. Discussion: Our findings offer a twofold contribution to the existing literature. Firstly, we present an innovative theoretical model for analyzing deliberative systems. Resumo Na Mata Atlântica, as distribuições de espécies não são uniformes. O bioma foi dividido em 8 sub-regiões biogeográficas, como o Centro de Endemismo de Pernambuco (CEP), no nordeste do Brasil. A Floresta Nacional de Nísia Floresta (Flona) é uma área protegida situada no município de Nísia Floresta, no Estado do Rio Grande do Norte, abrigando vegetação nativa (Mata Atlântica e "Restingas ou Tabuleiros"), bem como áreas em regeneração, com parcelas experimentais de plantas exóticas para produção de madeira. Aqui, apresentamos, pela primeira vez, uma lista de espécies da herpetofauna levantada na Flona ao longo dos últimos 10 anos, utilizando uma amostragem padronizada e de longo prazo, juntamente encontros visuais e ocasionais. Realizamos levantamentos mensais em toda a área utilizando 84 armadilhas de queda distribuídas aleatoriamente em 21 conjuntos ao longo das três zonas fitogeográficas da floresta (Restinga, Mata Atlântica e Regeneração). Além disso, caracterizamos a diversidade da herpetofauna terrestre nessas três zonas ao longo de um ano inteiro. Coletamos um total de 39 espécies de répteis (entre lagartos, serpentes, quelônios, crocodilianos e anfisbênios) e 24 espécies de anfíbios. A família mais frequente encontrada para os répteis foi Dipsadidae, seguida por Colubridae. Para anfíbios, Leptodactylidae foi a mais comum, seguida por Hylidae. A diversidade da herpetofauna em áreas de regeneração é menor do que em florestas, que por sua vez são marginalmente menos diversas do que a Restinga. Descritores da comunidade, como equidade, presença de espécies exclusivas e diferenças em abundâncias e composição indicam que estratégias de manejo distintas para cada área são necessárias para esta área protegida. Abstract In the Atlantic Forest, species distributions are not uniform. The biome has been divided into 8 biogeographic sub-regions, such as the Pernambuco Center of Endemism (PCE), in northeastern Brazil. Nísia Floresta National Forest (Flona) is a protected area situated in the municipality of Nísia Floresta, Rio Grande do Norte state, harboring native vegetation (Atlantic Forest and "Restingas or Tabuleiros") as well regenerating areas, with experimental plots of exotic plants for timber production. Herein, we present, for the first time, a species list of the herpetofauna surveyed at Flona over the past 10 years, using a standardized, long-term sampling design along with visual encounter surveys. We conducted monthly surveys across the area using 84 pitfall traps randomly distributed in 21 arrays throughout the forest's three phytogeographic zones (Restinga, Atlantic Forest, and Regeneration). In addition, we characterized ground-dwelling herpetofauna community diversity across these three zones over the course of an entire year. We collected a total of 39 species of reptiles (among lizards, snakes, chelonians, alligators, and amphisbaenians) and 24 species of frogs. The most frequent family found for reptiles was Dipsadidae, followed by Colubridae. In frogs, Leptodactylidae was the most common, followed by Hylidae. Herpetofaunal diversity in regeneration areas is smaller than forests, which in turn are marginally less diverse than the Restinga. Community descriptors such as equity, presence of exclusive species, and differences in abundances and composition indicate that distinct management strategies for each zone are needed for this protected area. Abstract Cardiomyopathy is characterized by structural and functional abnormalities in the heart muscle that are not influenced by coronary disease, hypertension, valve disease, or congenital heart defects. Genetic testing, which helps diagnose the molecular bases of cardiomyopathy, can affect prognostication and clinical decisions. Recent guidelines advocate classifying cardiomyopathies based on phenotypic categories, which encompass a range of clinical presentations, underlying causes, and outcomes. Beyond deepening our grasp of the disease, genetic testing can distinguish between diseases with similar presentations, known as genocopies, which can profoundly influence treatment decisions. Genetic testing is also essential for reproductive counseling and risk evaluation among family members. Combined with advanced imaging techniques such as magnetic resonance imaging, genetic investigation offers a comprehensive understanding of the disease. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT Coronary artery fistula draining into the coronary sinus is a rare vascular malformation, and its diagnosis and clinical manifestations usually occur late. We describe the case of a 72-year-old female patient with dyspnea on exertion (New York Heart Association Class III) associated with palpitations. The transthoracic echocardiogram showed significant tricuspid insufficiency. Cardiac catheterization showed aneurysm of the circumflex coronary artery and fistula of this artery draining into the coronary sinus. The patient underwent fistula ligation and tricuspid valve repair, with excellent surgical results.

Resumo O artigo busca avançar na agenda de investigações sobre o poder judiciário, aproximando as pesquisas sobre resolução de conflitos nos tribunais constitucionais àquelas sobre a mobilização de leis por grupos de interesse e movimentos sociais. As Audiências Públicas (APs) realizadas pelo Supremo Tribunal Federal são analisadas como arenas estratégicas de processamento de temas contenciosos nas quais atores diversos têm oportunidades de contribuir com as práticas jurídicas. O objetivo central do artigo é identificar os mecanismos de conexão e de transmissão das deliberações presentes nas audiências públicas realizadas pelo Supremo Tribunal Federal (STF) no período de 2007 a 2019. O problema de pesquisa foi formulado com o propósito de se investigar e analisar como a sociedade civil atua no âmbito do Supremo Tribunal Federal. O resultado do artigo confirma as organizações da sociedade civil como principal conector das APs realizadas no STF, pois representam cerca de 40% das organizações presentes, simultaneamente, em mais de duas audiências.
We intend to advance the research agenda on the judiciary by bringing research on conflict resolution in constitutional courts closer to that on the mobilization of laws by interest groups and social movements. The Public Hearings (PAs) held by the Supreme Court are analyzed as strategic arenas for processing contentious issues in which diverse actors have opportunities to contribute to legal practices. The central objective of the article is to identify the mechanisms of connection and transmission of deliberations present in the public hearings held by the Supreme Court (STF) in the period from 2007 to 2019. The research problem was formulated with the purpose of investigating and analyzing how civil society acts within the Supreme Court. Finally, this article’s main result confirms civil society organizations as the main connector of the APs held at the STF, as they represent about 40% of the organizations present, simultaneously, in more than two hearings.
Resumen El artículo busca avanzar en la agenda de investigaciones sobre el Poder Judicial integrando las investigaciones sobre resolución de conflictos en los tribunales constitucionales con aquellas sobre la movilización de leyes por grupos de interés y movimientos sociales. Las Audiencias Públicas (AP) realizadas por la Suprema Corte Federal (STF) son analizadas como espacios estratégicos para la tramitación de asuntos contenciosos en los que diferentes actores tienen oportunidades de contribuir a las prácticas legales. El objetivo principal del artículo es identificar los mecanismos de conexión y transmisión de las deliberaciones presentes en las audiencias públicas celebradas por el STF en el período de 2007 a 2019. El problema de investigación se formuló con el propósito de investigar y analizar cómo la sociedad civil opera dentro del ámbito de la Corte Suprema Federal. El resultado del artículo confirma que las organizaciones de la sociedad civil son el principal conector de las Audiencias Públicas realizadas en el STF, ya que representan alrededor del 40% de las organizaciones presentes, simultáneamente, en más de dos APs.
Résumé L'article cherche à faire progresser l'agenda des enquêtes sur le pouvoir judiciaire en intégrant les recherches sur la résolution des conflits dans les cours constitutionnelles à celles sur la mobilisation des lois par les groupes d'intérêt et les mouvements sociaux. Les audiences publiques (AP) organisées par la Cour fédérale suprême sont analysées comme des arènes stratégiques pour le traitement des questions litigieuses dans lesquelles différents acteurs ont la possibilité de contribuer aux pratiques juridiques. L'objectif principal de l'article est d'identifier les mécanismes de mise en relation et de transmission des délibérations présentes lors des auditions publiques tenues par le Tribunal Fédéral (STF) entre 2007 et 2019. Le problème de recherche a été formulé dans le but d'enquêter et d'analyser comment la société opère dans le cadre de la Cour suprême fédérale. Le résultat du article confirme que les organisations de la société civile sont le principal connecteur des auditions publiques tenues au STF, puisqu'elles représentent environ 40% des organisations présentes, simultanément, dans plus de deux AP.

RESUMO - RACIONAL: O câncer colorretal é a terceira neoplasia mais frequente e metade dos pacientes desenvolvem metástase hepática. O melhor fator prognóstico na metástase hepática de câncer colorretal (MHCCR) é a possibilidade de ressecção com margens livres, porém a maioria permanece irressecável. O racional em realizar transplante hepático (TH) em pacientes portadores de MHCCR está na ampliação do número de pacientes ressecáveis através de uma hepatectomia total. OBJETIVO: Apresentar protocolo brasileiro para realização de transplante hepático em pacientes com MHCCR irressecável. MÉTODO: O protocolo foi realizado por duas instituições com grande volume de ressecções e transplantes hepáticos no Brasil, baseado no trabalho realizado pela Universidade de Oslo. A elaboração foi dividida em 4 etapas. RESULTADO: É apresentada proposta de protocolo para esta doença a ser validada na aplicação clínica. CONCLUSÃO: Foi possível elaborar protocolo de transplante hepático para MHCCR irressecável a fim de uniformizar o tratamento e melhor avaliar os resultados cirúrgicos.
ABSTRACT - BACKGROUND: Colorectal cancer (CRC) is the third most common neoplasm, and half of the patients with CRC develop liver metastasis. The best prognostic factor for colorectal liver metastasis (CRLM) is the possibility of performing a resection with free margins; however, most of them remain unresectable. The justification for performing liver transplantation (LT) in patients with CRLM regards an increase in the number of resectable patients by performing total hepatectomy. AIM: The aim of this study was to provide a Brazilian protocol for LT in patients with unresectable CRLM. METHOD: The protocol was carried out by two Brazilian institutions, which perform a large volume of resections and LTs, based on the study carried out at the University of Oslo. The elaboration of the protocol was conducted in four stages. RESULT: A protocol proposal for this disease is presented, which needs to be validated for clinical use. CONCLUSION: The development of an LT protocol for unresectable CRLM aims to standardize the treatment and to enable a better evaluation of surgical results.

RESUMO Racional: O colangiocarcinoma hilar representa mais da metade de todos os casos de colangiocarcinoma; tem prognóstico reservado e sobrevida global mediana de 12- 24 meses após o diagnóstico. A proposta de realizar transplante hepático surgiu para ampliar a possibilidade de margens livres através de hepatectomia total nos portadores de tumoresirressecáveis com melhor prognóstico. Objetivo: Apresentar protocolo brasileiro para realização de transplante hepático em pacientes com colangiocarcinoma hilar. Método: O protocolo foi realizado por duas instituições com grande volume de ressecções e transplantes hepáticos no Brasil, baseado no trabalho realizado pela MayoClinic. A elaboração foi dividida em quatro etapas. Resultado: É apresentada proposta de protocolo para esta doença a ser validada na aplicação clínica. Conclusão: Foi possível elaborar protocolo de transplante hepático para colangiocarcinoma a fim de uniformizar o tratamento e melhor avaliar os resultados cirúrgicos.
ABSTRACT Background: Hilar cholangiocarcinoma represents more than half of all cholangiocarcinoma cases, having poor prognosis and presenting a median overall survival after diagnosis of 12-24 months. In patients who have unresectable tumors with a better prognosis, the proposal to perform liver transplantation emerged for expanding the possibility of free margins by performing total hepatectomy. Aim: To provide a Brazilian protocol for liver transplantation in patients with hilar cholangiocarcinoma. Method: The protocol was carried out by two Brazilian institutions which perform a large volume of resections and liver transplantations, based on the study carried out at the Mayo Clinic. The elaboration of the protocol was conducted in four stages. Result: A protocol proposal for this disease is presented, which needs to be validated for clinical use. Conclusion: The development of a liver transplantation protocol for cholangiocarcinoma aims not only to standardize the treatment, but also enable a better assessment of the surgical results in the future.

Resumo: O objetivo deste trabalho foi comparar o acúmulo de forragem e as características estruturais de dosséis durante o período de diferimento das cultivares Marandu, Piatã, Xaraés e Paiaguás de Urochloa brizantha. O delineamento experimental foi inteiramente casualizado, com três repetições. O experimento foi repetido por dois anos (2017 e 2018), sob condição de corte mecânico, e o dossel foi diferido por 90 dias, de março a junho. No primeiro ano, a massa de forragem foi maior para as cultivares Xaraés e Paiaguás; porém, no segundo ano, Xaraés apresentou massa de forragem maior do que a das demais cultivares. O número de perfilhos vegetativos no final do período de diferimento foi maior para Paiaguás, contrariamente ao que foi observado para número de perfilhos reprodutivos. Já a taxa de acúmulo de forragem foi maior para as cultivares Paiaguás e Xaraés. O capim Paiaguás se destaca por sua alta taxa de acúmulo de forragem durante o período de diferimento e por seu maior número de perfilhos vegetativos e menor número de perfilhos reprodutivos do que os das outras cultivares.
Abstract: The objective of this work was to compare herbage accumulation and canopy structural characteristics during the stockpiling period of the Marandu, Piatã, Xaraés, and Paiaguás cultivars of Urochloa brizantha. The experimental design was completely randomized, with three replicates. The experiment was repeated for two years (2017 and 2018), under mechanical cutting conditions, and the canopies were stockpiled for 90 days, from March to June. In the first year, forage mass was higher for the Xaraés and Paiaguás cultivars; however, in the second year, Xaraés showed a forage mass higher than that of the other cultivars. The number of vegetative tillers at the end of the stockpiling period was higher for Paiaguás, contrary to what was observed for number of reproductive tillers. The herbage accumulation rate was higher for the Paiaguás and Xaraés cultivars. The Paiaguás grass stands out for its high herbage accumulation rate during the stockpiling period and for its higher number of vegetative tillers and lower number of reproductive tillers than that of the other cultivars.

Resumo: Introdução: A residência médica é reconhecida como o “padrão ouro” para a formação de especialistas. O estudo do perfil dos egressos da residência médica é importante para identificar potencialidades e fragilidades da especialização. Objetivo: Este estudo teve como objetivo conhecer o perfil e a satisfação profissional dos egressos dos programas de residência das áreas básicas de um hospital-escola do Nordeste. Método: Trata-se de estudo de corte transversal que utilizou a plataforma eletrônica Survey. Incluíram-se residentes das áreas básicas que concluíram o programa no período de 2013 a 2017. O Termo de Consentimento Livre e Esclarecido foi enviado com o formulário. Realizaram-se análises descritivas das variáveis, e os dados foram apresentados em frequências absoluta e relativa. O trabalho foi aprovado pelo Comitê de Ética do IMIP. Resultados: Dentre os 194 egressos, tivemos a adesão de 79 (40,72%). Com relação aos participantes do estudo: 73,4% eram do gênero feminino e 60,8% já estavam casados. Destacamos que 55,7% informaram que tinham uma renda mensal de dez a 20 salários mínimos. Dos egressos, 54 (68,4%) tinham cursado graduação em instituição de ensino superior privada. Sobre a pós-graduação stricto sensu, 19 egressos (21,7%) tinham mestrado. Sobre a atuação profissional, 93,7% exercem a especialidade e 54 (68,4%) trabalham no estado onde cursaram o programa. Em relação ao serviço público, 64,6% são vinculados ao Sistema Único de Saúde do estado de Pernambuco. Sobre a quantidade de horas de trabalho semanais, 43% trabalham entre 40 e 60 horas. Cerca de 75% dos egressos afirmaram que cursariam o programa novamente na instituição e declararam que a realização da residência facilitou a vida profissional deles. Conclusão: A monitoração periódica de egressos de programa de residência é um instrumento útil para avaliação do programa e permite monitoramento das intervenções implementadas, viabilizando inclusive a obtenção de informações que ajudem no planejamento de novos programas.
Abstract: Introduction: medical residency is known as the “gold standard” for the qualification of specialists. Research into the profile of medical residency program graduates is important to identify potentialities and fragilities of the specialization. Objective: The objective of this study was to understand the profile and professional satisfaction of the graduates from the residency programs of basic medical areas of a teaching hospital in the northeast of Brazil. Methods: a cross-sectional study was conducted through an online questionnaire that was sent to the participants via electronic mail. The study population consisted of residency program graduates of the basic medical areas who graduated between 2013 and 2017. The Informed Consent Form was sent together with the form. Descriptive analyses were performed and data were presented as relative and absolute frequencies. The study was approved by the Research Ethics Committee of the IMIP. Results: of the 194 graduates, 79 (40.72%) answered the questionnaire. 73.4% were female and 60.8% were married. Approximately 55.7% of the graduates earned salaries corresponding to 11 and 22 minimum monthly wages. 54 graduates (68.4%) had attended a private medical school. Regarding stricto sensu postgraduate studies, 19 (21.7%) had a master´s degree. 93.7% are practicing their medical specialization and 54 (68.4%) still live in Pernambuco. Regarding public service, 64.6% are linked to the Unified Health System of Pernambuco state. 43% of the graduates worked between 40 and 60 h per week. About 75% of the graduates stated that they would attend the program again at the hospital and declared that completing the residency at the institution facilitated their professional life. Conclusion: the periodic monitoring of graduates from a residency program is a useful tool for evaluating the program and allows for surveillance of implemented interventions

RESUMO Introdução: Os marcadores pró-inflamatórios desempenham papel importante na severidade de pacientes com COVID-19. Assim, terapêuticas anti-inflamatórias são agentes interessantes para potencialmente combater a cascata inflamatória descontrolada em tais pacientes. Delineamos um ensaio para testar tocilizumabe em comparação com o tratamento padrão, tendo como objetivo melhorar os desfechos por meio da inibição da interleucina 6, um importante mediador inflamatório na COVID-19. Métodos e análises: Este será um estudo aberto multicêntrico, randomizado e controlado, que comparará os desfechos de pacientes tratados com tocilizumabe mais tratamento padrão com o tratamento padrão isoladamente em pacientes com COVID-19 moderada a grave. Como critérios de inclusão, serão exigidos dois dos quatro critérios a seguir: dosagens de dímero D acima de 1.000ng/mL, proteína C-reativa acima de 5mg/dL, ferritina acima de 300mg/dL e desidrogenase lática acima do limite superior do normal. O objetivo primário será comparar a condição clínica no dia 15, conforme avaliação por meio de escala ordinal de 7 pontos aplicada nos estudos de COVID-19 em todo o mundo. O desfecho primário será avaliado por regressão logística ordinal assumindo razões de propensão proporcionais ajustadas pelas variáveis de estratificação (idade e sexo). Ética e disseminação: O TOCIBRAS foi aprovado pelos comitês de ética locais e central (nacional) do Brasil em conformidade com as atuais diretrizes e orientações nacionais e internacionais. Cada centro participante obteve aprovação do estudo por parte de seu comitê de ética em pesquisa, antes de iniciar as inscrições no protocolo. Os dados derivados deste ensaio serão publicados independentemente de seus resultados. Se tiver sua efetividade comprovada, esta estratégia terapêutica poderá aliviar as consequências da resposta inflamatória na COVID-19 e melhorar os resultados clínicos.
ABSTRACT Introduction: Pro-inflammatory markers play a significant role in the disease severity of patients with COVID-19. Thus, anti-inflammatory therapies are attractive agents for potentially combating the uncontrolled inflammatory cascade in these patients. We designed a trial testing tocilizumab versus standard of care intending to improve the outcomes by inhibiting interleukin-6, an important inflammatory mediator in COVID-19. Methods and analysis: This open-label multicentre randomized controlled trial will compare clinical outcomes of tocilizumab plus standard of care versus standard of care alone in patients with moderate to severe COVID-19. Two of the following four criteria are required for protocol enrolment: D-dimer > 1,000ng/mL; C reactive protein > 5mg/dL, ferritin > 300mg/dL, and lactate dehydrogenase > upper limit of normal. The primary objective will be to compare the clinical status on day 15, as measured by a 7-point ordinal scale applied in COVID-19 trials worldwide. The primary endpoint will be assessed by an ordinal logistic regression assuming proportional odds ratios adjusted for stratification variables (age and sex). Ethics and dissemination: The TOCIBRAS protocol was approved by local and central (national) ethical committees in Brazil following current national and international guidelines/directives. Each participating center had the study protocol approved by their institutional review boards before initiating protocol enrolment. The data derived from this trial will be published regardless of the results. If proven active, this strategy could alleviate the consequences of the inflammatory response in COVID-19 patients and improve their clinical outcomes.

ABSTRACT BACKGROUND: The accuracy of magnetic resonance imaging (MRI) for making the diagnosis of subscapularis tears presents wide variation in the literature and there are few prospective studies. OBJECTIVE: To compare the findings from MRI and arthroscopy for diagnosing subscapularis tears. DESIGN AND SETTING: Diagnostic test study performed in a tertiary care hospital. METHODS: We included patients who underwent arthroscopic rotator cuff repair and who had firstly undergone high magnetic field MRI without contrast. The images were independently evaluated by a shoulder surgeon and two musculoskeletal radiologists. Sensitivity, specificity, positive and negative predictive values, accuracy and inter and intra-observer agreement were calculated. RESULTS: MRIs on 200 shoulders were evaluated. The incidence of subscapularis tears was 69.5% (41.5% partial and 28.0% full-thickness). The inter and intra-observer agreement was moderate for detection of subscapularis tears. The shoulder surgeon presented sensitivity of 51.1% to 59.0% and specificity of 91.7% to 94.4%. The radiologists showed sensitivity of 83.5% to 87.1% and specificity of 41% to 45.9%. Accuracy ranged from 60.5% to 73.0%. CONCLUSION: The 1.5-T MRIs without contrast showed mean sensitivity of 70.2% and mean specificity of 61.9% for detection of subscapularis tears. Sensitivity was higher for the musculoskeletal radiologists, while specificity was higher for the shoulder surgeon. The mean accuracy was 67.6%, i.e. lower than that of rotator cuff tears overall.

Abstract INTRODUCTION: Patients with Chagas disease (CD), caused by Trypanosoma cruzi, present a higher risk of developing other chronic diseases, which may contribute to CD severity. Since CD is underreported in the southern state of Paraná, Brazil, we aimed to characterize clinical and epidemiological aspects of individuals chronically infected with T. cruzi in Southern Brazil. METHODS: A community hospital-based study was performed, recording clinical/demographic characteristics of 237 patients with CD from Southern Brazil. To estimate the association between different forms of CD and sociodemographic and clinical variables, multiple logistic regression models were built using the Akaike information criterion. RESULTS: Mean age was 57.5 years and 59% were females. Most patients’ (60%) place of origin/birth was within Paraná and they were admitted to the CD outpatient clinic after presenting with cardiac/digestive symptoms (64%). The predominant form of CD was cardiac (53%), followed by indeterminate (36%), and digestive (11%). The main electrocardiographic changes were in the right bundle branch block (39%) and left anterior fascicular block (32%). The average number of comorbidities per patient was 3.9±2.3; systemic arterial hypertension was most common (64%), followed by dyslipidemia (34%) and diabetes (19%); overlapping comorbidities were counted separately. Male sex was associated with symptomatic cardiac CD (OR=2.92; 95%CI: 1.05-8.12; p=0.040). CONCLUSIONS: This study provided greater understanding of the distribution and clinical profile of CD patients in Southern Brazil, indicating a high prevalence of comorbidities among these patients who are a vulnerable group due to advanced age and substantial risk of morbidity.

OBJECTIVES: We designed a cohort study to describe characteristics and outcomes of patients with coronavirus disease (COVID-19) admitted to the intensive care unit (ICU) in the largest public hospital in Sao Paulo, Brazil, as Latin America becomes the epicenter of the pandemic. METHODS: This is the protocol for a study being conducted at an academic hospital in Brazil with 300 adult ICU beds dedicated to COVID-19 patients. We will include adult patients admitted to the ICU with suspected or confirmed COVID-19 during the study period. The main outcome is ICU survival at 28 days. Data will be collected prospectively and retrospectively by trained investigators from the hospital’s electronic medical records, using an electronic data capture tool. We will collect data on demographics, comorbidities, severity of disease, and laboratorial test results at admission. Information on the need for advanced life support and ventilator parameters will be collected during ICU stay. Patients will be followed up for 28 days in the ICU and 60 days in the hospital. We will plot Kaplan-Meier curves to estimate ICU and hospital survival and perform survival analysis using the Cox proportional hazards model to identify the main risk factors for mortality. RESULTS: We expect to include a large sample of patients with COVID-19 admitted to the ICU and to be able to provide data on admission characteristics, use of advanced life support, ICU survival at 28 days, and hospital survival at 60 days. CONCLUSIONS: This study will provide epidemiological data about critically ill patients with COVID-19 in Brazil, which could inform health policy and resource allocation in low- and middle-income countries.

Abstract Aim: This cross-sectional study aimed to investigate the association between life satisfaction and purpose in the life of the elderly hydro gymnastic practitioners. Methods: A total of 92 older people of both sexes, who practice hydro gymnastics in the city of Maringá, State of Paraná, Brazil, participated in the study. A sociodemographic questionnaire, the Satisfaction with Life Scale and the purpose in life Questionnaire were used as instruments. Data analysis was carried out by using Kolmogorov-Smirnov test, Mann-Whitney U test, Kruskal-Wallis test, and Spearman’s correlation test, in addition to the Path Analysis (p <0.05). Results: The results showed a greater life satisfaction for the women comparing to the men (p = 0.023). Moreover, it was found that the elderly who perceived themselves with a better health condition obtained higher scores according to the purpose in life scale. Life satisfaction had a positive and moderate association (β = 0.24) with the purpose in the life of the elderly, which explains 6% of the variability. Conclusion: It was concluded that sex can be considered as an intervening factor when concerning life satisfaction, time of practice of hydro gymnastics and self-perceived health related to the purpose in life of the elderly. In addition, satisfaction with life was positively associated with the purpose in the life.
